I feel like I did educate myself enough because I had two failed transfer and should of did another retrieval for better embryos to have a better chance at success. You could talk to 5 different dr's, nurses and get 5 different opinions or recommendations on protocols, etc.

Dr Chang as a doctor is great. Pro's- Very knowledgable and has a lot of experience. Trust his recommendations and that he truly wants to help us have a family. He's quirky and funny and will make time for small talk if he can. The negatives - if you have a consult time set, expect to wait all day for a call. I dont mind if he cannot make the time, I just think they can do a better job communicating that he will be late and help provide patients a window that is realistic for him to call so we arent waiting around all day. Another neg is that I think he sees SO many women, I dont think he ever remembers me or our case and have to refresh him on everything every time I talk to him.

Expect to wait for your consult and dont get upset. You just have to go in knowing you may wait all day for him to call bc he is helping and seeing so many women. The consult will go well and he's a grast doctor.

The only negatives if it is one I would say are that he sees so many people anytime I talk to him or have a consult, he never remembers much about me and our case but I don't know that I can fault him as he is trying to help literally hundreds of women have a family. He is also VERY late to consults, but it is what it is. Overall pleased with dr chang and the buffalo practice!

Dr. Chang is changing my protocol from round one to two - bieth control for a month to help minimize cysts and allow more synchronus growth of follicles. Also doubling menopur dosage in an effort to get more eggs and pgt normals next cycle. Too early to say for results.

The nursing staff is all wonderful. Very warm & welcoming. Melissa in particular is wonderful, and I think her name was Katie day of retrieval was wondeful as well. Some nurses are better at ultrasounds than others.

The Buffalo clinic is wonderul. CNY overall as a company is a bit scattered and can be quite frustrating to work with, especially until you start your cycle with a local office. Its almost impossible to connect to a local facility and the general nursing clinical line is not great..some friendly, but most always sound annoyed when you are calling and can't get you off the phone soon enough. There is not great communication after the initial consult - you're left having to research and find out a lot of information for yourself. You definitely need to be your own advocate with CNY. For me its worth it though as they are trying to help women by the masses even though it feels like a puppy mill sometimes. The only other thing that i have found frustrating is that there is zero consistency across the practice. You could talk to 5 different dr's, nurses and get 5 different opinions or recommendations on protocols, etc. Everything from their facebook lives, instagram, kiltz, to the local practice. It can definitely be frustrating!

HCG wash and intralipids ordered before transfer, both were brought with me to the appointment. The nurse administered the intralipids but never mentioned the HCG so I thought it was for the day of transfer. Day of transfer I asked about the HCG and they said it should have been done days earlier with the intralipids so I was not able to have the HCG wash done.
Dr Chang is kind and likes to joke, which helps keep the mood light. He didn't have a lot of suggestions on what to change after our first transfer of 2 embryos failed. We still do not know why nothing is working for us...He is receptive to your own ideas regarding treatment...He suggested two the first time. We then had three left and I wanted to transfer all. He was receptive to this and allowed it.

Had surgery prior to remove endometriosis/endometrioma. Started to stim the month after only to find out the endometrioma came back. I was on Gonal f 300 IU once a day, Menopur 150 IU once a day, Letrozle 10mg once a day, LDN 4.5mg once a day, Prednisone 5mg twice a day, Cetrotide 0.25mg once a day, Prograf 0.5mg twice a day, Estrace 2mg twice a day, Endometrim 100mg twice a day, Progesterone in oil 50mg once a day. Only collected three eggs in which two became embryos. He decided to do a fresh transfer that failed. We needed up doing a frozen two months after with our last embryo and it failed as well

Sometimes I felt kind of lost [at CNY Fertility Center], like I was getting different information from each person I spoke with, like no one had reviewed my file before seeing me...I've only seen a doctor three times, and I really don't know who my doctor is...Everyone at the clinic is very kind. Some are friendlier and more personable than others, but all were very nice, and all seemed knowledgeable... They make a lot of people's dreams reality, so that's a strength.

I have only spoken to Dr. Kiltz once on the phone. I had a follow up call with him after several failed IUIs and before going to IVF. The call was early in the morning (7 or 7:15) and he was exactly on time, which was impressive. The call was good, and he agreed that IVF was a good next step. He was friendly and knowledgable. He was compassionate, but didn't really ask me too much about my history or seem to have reviewed my file. He spent a good portion of the call talking about the keto diet and that plants are killers. You can tell that he really cares about his patients and is passionate about what he does. I feel like there are some people who are lucky to see him often and get to know him, and then there are others who only interact with him once, if at all.

Expect to hear about keto, the bebbi diet, Kiltz's keto, and that plants are out to kill us. I'd also say that he seems to be super dedicated to his work and is very passionate. He regularly gives out his cell phone number, he does weekly fb/instagram live videos (daily during COVID19 pandemic), he starts his calls early in the morning. He must always be working! I wonder when he sleeps!

I selected that my care has been under two doctors, but I've only seen a doctor three times, and I really don't know who my doctor is. I had my initial consult with a PA who I have spoken to once since, I had a follow up call before going to IVP with Dr. Kiltz and he signs most of my prescriptions, and my HSG and egg retrieval were with Dr. Corley. Every other phone call and appointment was with a nurse...practically a different nurse each time. Sometimes I felt kind of lost, like I was getting different information from each person I spoke with, like no one had reviewed my file before seeing me. I really had to do a lot of my own homework and followup. I feel like my first egg retrieval was a real learning experience, so I'm eager to go into the second retrieval now that I know what to expect and what to do.
Describe the protocols Robert Kiltz used in your cycles at CNY Fertility Center and their degree of success.
I first planned to do three IUIs then move to IVF. My IUI cycles included a z-pak, prednisone, letrozol, ovidrel, and endometrin. I did not have any adverse reactions to the medication. I think I produced 1-3 follicles. The third IUI changed to a timed intercourse cycle, as my bloodwork indicated that I had either just ovulated or was about to. They said I could have still done the IUI, but there was an increased chance that the timing would be off.
For IVF, I started with a z-pak and prednisone, and stimmed with compound HCG and Follistim. I then used cetrotide and triggered with leuprolide acetate. I was supposed to trigger with HCG as well, but there was an error with the script and I never got it. I didn't realize I was supposed to have it. Apparently it wasn't signed and CVS Specialty sent it back to the clinic for signature, but never got it. Only four eggs were retrieved, 2 matured, and 1 fertilized and made it to blast. Genetic testing indicated it was abnormal.
I'm waiting for my second retrieval and the meds I have this time are follistim and menopur to stim, then cetrotide, followed by HCG and leuprolide acetate to trigger. During a follow up call with a nurse, she said that I responded as though I have a low ovarian reserve, even though my blood work does not indicate that. This was the protocol that she designed...I believe she called it the 450/150 protocol.

Everyone at the clinic is very kind. Some are friendlier and more personable than others, but all were very nice, and all seemed knowledgeable. I have seen a lot of nurses at the clinic and have spoken to many different nurses on the phone.

CNY Fertility sees a lot of people, and is willing to try unconventional things to help people become parents. There can be a significant wait time to get your consultation, but once you're in, it's pretty quick to schedule all appointments and procedures to get moving. They make a lot of people's dreams reality, so that's a strength. I like that they have and encourage yoga, meditation, and acupuncture. Unfortunately many people feel like just a number there...probably a bi-product of seeing so many people. I'm considered a travel patient, even though I only live an hour away. I don't really know who my assigned doctor is, and I speak to/see someone different almost every time I'm there, call, or message through the portal. I imagine that local, non-travel patients may have a different experience.
It's VERY important to stay on top of everything, so that no balls are dropped. Do your research, follow up, ask questions. There was an error filling one of my scripts for my egg retrieval and I didn't know that I was missing one of my triggers. There were so many back and forths with the clinic and my pharmacy that I didn't realize that something was missing. Then, leading up to my retrieval every appointment started with "so, we're doing an IUI?", to which I responded "no, IVF." Not sure how that wasn't in my file and why no one knew that. Made me extra nervous.
They're really the only clinic around, and I can't afford to travel elsewhere. They seem to get results and overall I've had a very pleasant experience there. Everyone is very nice, the clinic is clean, modern, and they take care of you well and make sure you're comfortable. I'd recommend it as long as you know to stay on top of things and advocate for yourself. Eager to start my next round and still hoping for my rainbow baby.

I'm still waiting for some billing to come through. My insurance covered my IUIs, so I only had to pay two copays at each visit, one for the appointment and one for bloodwork. My medications for my three IUIs cost several hundred dollars.
Since I live in NY and work for a company with more than 100 staff, my IVF cycles are now partially covered. Unfortunately they do not cover the techniques used to fertilize the eggs (assisted hatching and something else that escapes me at the moment) and I believe that will cost $1000. It was $700 for the embryologist to sample the blastocyst, and I believe another $700 for the genetic testing (would be higher if I had more than one embryo). I have not gotten a bill from CNY for the fertilization and the embryologist. My medications have probably cost around $1,000.
I need to follow up with the financial team to make sure that my next retrieval is covered. Dr. Kiltz mentioned last night in his fb live video that some insurances are tricky with what they mean by three rounds. If it is not covered, then I will have to go to out of pocket pay with a financed payment plan, and I believe that is around $5,000.
I know that I am fortunate to have insurance cover a lot of this, but it is still more than what my husband and I can really afford to pay at this time.

One of my prescriptions for one of my triggers for my egg retrieval was called in, but apparently was not signed. I didn't realize I was missing medication until my last appointment before the retrieval when the nurse came in to give me my instructions. CVS Specialty said they received an unsigned script and sent it back with a request for signature but never got it. I was never notified. Thankfully they adjusted on the fly and I was able to trigger with just the leuprolide acetate. Not sure if that had anything to do with only getting four eggs or not.
After my retrieval I had a call with an NP out of their Buffalo office to discuss next steps. It was a wonderful call and I felt confident moving forward. Plan was to prime for 60 days with omnitrope, then go into another retrieval. The NP told me to message her the month before my next retrieval month to schedule an ultrasound and bloodwork on day 20/21. She said that she might want to have me start some additional meds to prep. This was last month, as COVID19 was shutting down clinics. I read on the clinic's page and heard in Dr. Kiltz's live videos that they were not starting new cycles. So I messaged the NP and asked her what to do about the CD 20/21 u/s and bloodwork...asked if COVID was impacting the clinic. She said they were not stopping and to call to schedule my appointment. I called Syracuse the next day. The nurse I spoke to was shocked and didn't know why I was given that information. They were not scheduling those appointments or starting new cycles, like I had read on their site. She said that it's okay to keep priming for another 30 days. I felt deflated and disappointed to have to press pause on this journey. It lowered my confidence in the NP and made me feel like I don't know where to turn to get accurate information that is specific to my case.

Talking to multiple people led to a lot of confusion and needing to have the same conversation with several different people before getting the correct answer.
Dr Cheng took a lot of time to answer each and every question I had. I was very vocal about my feelings surrounding the upcoming FET with the embryos we had. His explanations and humor helped me feel better and more confident...I also had a new plan that I fully understood because he took the time and effort to explain each adjustment in the new protocol...Dr Chang is clearly knowledgable and cares a lot about his patients. Once he's invested in your treatment, you can rest assured that you're in good hands. He is arguably one of the more aggressive doctors at CNY in terms of what he is willing to do and the things he is willing to try.
CNY is huge and handles an extremely high volume of patients. Nurses and doctors are often juggling hundreds upon hundreds of inquiries, portal messages, phone calls, office appointments, etc. Balls inevitably get dropped, information inevitably gets lost in translation, and that can be frustrating. You really have to stay on top of your own treatment plan. I think having a good working knowledge of at least the basics of IVF would be extremely beneficial.
How was your experience with Eric Chang at CNY Fertility Center?
I first spoke to Dr Chang after our first IVF cycle and subsequent 3 transfers of 4 embryos failed. Needless to say, I was not in a great place emotionally or mentally. Dr Chang reviewed my first cycle's retrieval protocol and FET protocol and identified some areas he thought may have caused our problems. He was extremely kind and seemed extremely confident that he could help me. I began that phone call in a complete state of devastation and panic and ended it feeling reassured and slightly vindicated... I also had a new plan that I fully understood because he took the time and effort to explain each adjustment in the new protocol. He also instructed me to make sure that during my retrieval cycle, all monitoring updates would be handled by him personally to make sure everyone stuck to his specific protocol. The reassurance that he was personally handling things, even though I was not monitoring in Buffalo or retrieving there was a godsend during that very scary time. We ultimately got 16 eggs, 13 of which were mature... all 13 fertalized and 10 became 5, 6, or 7 day embryos. (The previous cycle I had 11 eggs, only 5 mature, 4 fertilized and were frozen early at day 3.) Grading of the embryos became a concern of mine as ours were almost all "fair". I was able to get a tele appointment with Dr Chang quickly and he, once again, was able to patiently and kindly explain why he still felt confident in those embryos and went through the FET protocol and broke down all of my options. His confidence and reassurance was extremely valued and appreciated.
Dr Chang is clearly knowledgable and cares a lot about his patients. Once he's invested in your treatment, you can rest assured that you're in good hands. He is arguably one of the more aggressive doctors at CNY in terms of what he is willing to do and the things he is willing to try. I really liked that he was able to explain everything in a relatively simple manner so that I fully understood his reasoning for all med or protocol changes. He does have a very funny sense of humor which I personally felt helped in such an awkward and relatively anxiety filled time. I also feel that he is very much to the point and will bluntly state his thoughts on things... something I also appreciated.
What's one piece of advice would you give a prospective patient of Eric Chang at CNY Fertility Center?
Don't be afraid to say exactly what you're thinking or feeling about your treatment. He was the only doctor I spoke to that didn't just repeat the same go-to information to placate me. He not only acknowledged that I was audibly upset or nervous, he took steps to get to the root of what was causing those negative feelings so that he could give me the information and confidence I needed to move forward.

Dr Cheng took a lot of time to answer each and every question I had. I was very vocal about my feelings surrounding the upcoming FET with the embryos we had. His explanations and humor helped me feel better and more confident.
Describe the protocols Eric Chang used in your cycles at CNY Fertility Center and their degree of success.
Dr Chang believed that the biggest issues with my first cycle were a dominant follicle and timing the retrieval. He basically came right out and said that he felt they sent me for retrieval too early and that's why my maturity rate was so poor. To address these issues, he recommended a full cycle on hormonal birth control (even though CNY doesn't seem to like to do this) and personally handled my monitoring. I ended up doing 12 days of injections vs 9 the first time. The birth control helped my eggs grow more evenly and the prolonged stim time helped me get more mature eggs than I had in my previous cycle. He also switched me from Lh-HCG to Menopur (this was orignally a suggestion of Dr. Ditkoff) solely because they both believed some patients respond better to one drug over the other.

I actually worked primarily with the travel team nurses, the nurses at my monitoring center (CNY Atlanta), and the ones at CNY Albany. Buffalo was not available for retrievals at the time I did my cycle unfortunately. CNY's nursing team is overwhelmingly great.

Dr Chang recommended a single embroy transfer as I had never done an FET with a day 5 embryo. However he did say that he would be willing to consider letting me transfer two if the first single transfer failed.

He was kind, and very well spoken. He treated me like I had been his patient for years and years. They [CNY Fertility Center] were very on schedule, I didn’t have to sit and wait a long time. They were very sweet and talked to you like you were family. Our initial consultation was with Sue Koller and she was so compassionate, informative and patient. She explained the whole process and listened in order to provide thoughtful answers to our questions and concerns. Sue even followed up with me personally to share especially good news and would often respond directly to messages we sent into the portal. For our daily check ins during the IVF cycle we worked with different nurses each time, but they all were among the same quality as Sue.

[Dr. Maribelle Verdiales is] Very polite and informative, she is kind. Makes you feel in control of your situation and listens to your concerns. Then provides the best plan with recommendations. You know exactly what’s coming next and you aren’t lost...She was very kind and soft spoken with real information that empowered me.
The clinic [CNY Fertility Center - Monroe] is set like a spa so it’s very calm and relaxing. It’s only weakness is that it is not open on Saturday nor is the ivf done at that facility. You will have to travel to New York for that once cleared to do so. So be prepared if you’re in Atlanta wanting to do ivf.

There will be blood draws and ultrasounds during each visit up until the procedure. Letrozole was used in my cycle with about 4 monitoring appointments with the final day as 5 or 6 around the 14th day of your cycle. Although the process was quite much the outcome wasn’t positive as we hoped sadly.

If your self pay make sure to have extra for medications.

Clinic did not call me to inform me of hour change and I wasn’t able to check levels after iui until a week later than I was supposed to. Not sure if that had anything to do with the failed iui but they were nice about it and still carried on with the next steps. I’m confident they will make our goal happen and be more proactive.
He [Dr. Edward Ditkoff] was thoughtful, thorough and helpful. Generally I saw the nurses more though and love them. We met Dr Kiltz once during the entire cycle and that was a random encounter while waiting for an ultrasound. The conversation was all about what the keto diet is and he certainly has super strong opinions on that. After the cycle and no seemingly viable embryo, I was expecting Dr Kiltz or someone from the office to call me to discuss the PGS results and potential options but nobody called us. I called the office and asked for Dr Kiltz's phone number
The clinic [CNY Fertility] is super busy and that means that I met a different nurse each time I went in and a different nurse responded to emails I wrote. A couple of times, that meant that the entire email thread wasn't read and that caused some miscommunication.... Super busy place and that's probably what makes it more affordable than other clinics. The cost of several things here, except IVF, might be close to what they might be elsewhere, especially if they are not in-house items

We met Dr Kiltz once during the entire cycle and that was a random encounter while waiting for an ultrasound. The conversation was all about what the keto diet is and he certainly has super strong opinions on that. After the cycle and no seemingly viable embryo, I was expecting Dr Kiltz or someone from the office to call me to discuss the PGS results and potential options but nobody called us. I called the office and asked for Dr Kiltz's phone number (something I'd learned on FertilityIQ) and texted him asking him to call me back. He was pretty responsive and we talked on the phone a couple of days after the initial text message. During the conversation, I was hoping he would help me figure out what might be the next best thing to do but he mostly cited stats and said making the decision about what to do next was my call. While I appreciate that, I would've liked a little bit of guidance in the decision making process given that he's the experienced expert. I've already read plenty research papers and looked up stats on FertilityIQ and elsewhere! The final decision, of course, would've been mine. Those are the only two times I've talked to him in person and the rest of my interactions have been over text. He is very responsive over text though sometimes his one word responses might be a little tricky to decipher. I still appreciate him making himself available outside of the portal and clinic and he has been super supportive and with me through my most recent pregnancy and miscarriage.

Do your research. Read through all the relevant courses on FertilityIQ and go in with specific questions you want answered that you couldn't get answered elsewhere. There is a lot of information on the CNY website as well, especially if you are already going through the IVF process there and want to consider options like donor eggs/sperm, surrogacy etc.

The clinic is super busy and that means that I met a different nurse each time I went in and a different nurse responded to emails I wrote. A couple of times, that meant that the entire email thread wasn't read and that caused some miscommunication. The online system (portal) doesn't let you 'reply' to an email - you need to write a new message each time and that is a bit painful because you need to reference the content of the previous email to make sure the context is clear to whoever responds next. Overall, though, I thought the nurses took good care of me and continue to do so and treated me like a person. From what I read on FertilityIQ after my treatment was that I could've asked for the same nurse each time! I once asked that a particular nurse not do my ultrasound and that request was respected. Since my most recent miscarriage, I've been in communication with the nurses to keep up with dropping HCG labs and ultrasounds especially since I've moved to a new city and don't yet have an local OB. The nurses and Dr. Kiltz have been super responsive. I found Dr Kiltz's phone number on the CNY website and I've been talking to him throughout the process from embryo transfer to miscarriage to D&C to post D&C complications to future directions and he has been amazingly supportive.

My first consultation was with a nurse and she set the treatment protocol. From my point of view, there was no input from Dr Kiltz regarding anything though I have no idea what goes on in the background. I asked to see the nurse who designed my protocol during one of my monitoring sessions but was told she is unavailable and I didn't ask again. I think my first consultation would've been with a doctor had I waited 8+ months for a phone consultation appointment. I went with the onsite nurse consultation as I got an appointment within 4 months which then got moved up by a couple of months as a slot opened up.

Overall, I believe the nurses run the place and they are doing a pretty good job. Most of them seem to have a good bit of experience and are understanding and accommodating. I'm a bit less enthusiastic about the embryology team as some of them seem to have less than a year's worth of experience with no background in anything bio related. This is another thing I learned on FertilityIQ - to look for the nurses and other people on LinkedIn to get a sense of their education and experience. The nurses could've been more pro-active with changing my estrogen dosage based on monitoring results (blood and ultrasound). They didn't do it until I asked for it and one of the doctors suggested they increase my estrace dosage, which happened after many expensive monitoring visits. If you don't see your lining start to get thicker in 10-12 days, take charge and ask the nurses to review your protocol with a doctor and change it accordingly. I spent a frustrating month going for blood work and ultrasounds and not seeing much change. The whole process could've been faster and cheaper had I asked to get a doc involved early enough.

Super busy place and that's probably what makes it more affordable than other clinics. The cost of several things here, except IVF, might be close to what they might be elsewhere, especially if they are not in-house items, for example, medicines (third party pharmacy cost), PGS (third party diagnostic cost) etc. That being said, the difference in IVF cost here and other places is HUGE making it an easy win over other places. Low cost does not translate into poor care. You just have to be well informed (FertilityIQ can help) and take charge of the process (communicate with the nurses). If you want a dedicated care team, want to see a doctor on each visit, and want the clinic to be initiating a lot of conversations with you instead of you having to call/email them for information constantly, this may not be the place for you.

For transfer: Lining prep medicines (don't remember exact number but likely under 250 as estrace and progesterone aren't as pricy as the retrieval meds) + monitoring (out of town so about 2000 because I was asked to go many times) + embryo transfer cost (varies if its part of IVF cycle or not) + travel and lodging costs for the embryo transfer + acupuncture cost ($70 for 2 sessions or 1, I don't remember) if you choose to do it before and after transfer.

I had to repeatedly ask for clarification on amounts, dosages and what the medication was.

I am doing platelet rich plasma (PRP) uterine washes this cycle. Based on the research I've read, this sounds very promising. Negatives: Dr. Kiltz has very strong beliefs about infertility being related to diet, which I do not agree with. After a failed cycle he spent the entire telephone consult (which took 2 1/2 weeks to schedule) telling me that my tube is blocked because I eat lean meats, fresh vegetables, and fruits. He believes in the Keto diet (high fat, moderate protein, low carb). He flat out told me he was an expert with tons of experience and that my tube was blocked directly as a result of inflammation in my body, due to my diet. He then prescribed a ton of additional anti-inflammatory medications. I have chosen not to take most of the extras (prednisone, prograf [used to suppress the immune system in transplant patients], intralipids, and hydroxychloroquine [used to treat Lupus and autoimmune diseases]. I had an ANA test (for autoimmune disease) after my failed cycle and it was negative. I did a lot of research on my own to decide which medications I wanted to use (aside from the standard meds like estrace, progesterone, etc). Thankfully I am a nurse and feel comfortable researching these things myself. It would still be nice to be able to have a partnership with my doctor and not feel like it is all on my shoulders, as I am certainly not a fertility expert.

Be your own advocate, do your own research, and do what you are comfortable with. Realize you will not get a lot of time with any doctor at CNYF, but you can reach out to them by email and you should if it is something time sensitive that is important to you. An "auto-immune" protocol was prescribed: prednisone, prograf, hydroxychloroquine, intralipids, low dose naltrexone, baby aspirin, lovenox. Most of these are immunosuppressants. The aspirin and lovenox thin the blood, which is supposed to increase blood flow to the placenta and prevent clots for those with clotting disorders. Low dose naltrexone is actually a very interesting drug used to treat autoimmune diseases as well as clinically low endorphins. It has very few side effects. It also boosts your immune system, which has helped me avoid getting sick. It also helps me sleep better. The only extra I took my first cycle was the baby aspirin. That cycle failed. I transfer again in five days. This cycle I have been taking LDN, baby aspirin, and start lovenox tonight. I did not find sufficient evidence that lovenox will increase my odds, but it has relatively few side effects, so I am trying it. I also took estrace vaginally, progesterone in oil and endometrium vaginal suppositories, and Cleocin (clindamycin - a vaginal suppository antibiotic) both cycles. Also, azithromycin orally at the very beginning of each cycle.

The nursing staff is generally nice. However, they are very busy. They do not go above and beyond by any means. There was medication education along the way. But I did not have a good idea of the process. My first cycle I was never sure what was coming next. One thing that upset me was embryo testing was mentioned briefly at my consultation (July), then never brought up again. On my day of transfer (end of October), I mentioned I was considering PGS testing. I was told I needed to decide this before the day of.... yet it was never brought up at ANY of my monitoring appointments along the way leading up to egg retrieval. I am a nurse and feel it would have been appropriate to mention at the baseline monitoring appointment. This whole process is so overwhelming and it is difficult to keep up with everything. However, I always felt it was my responsibility to ask questions and stay on top of it all myself. And when I did ask questions, I would get an answer, but I felt like I was being a pain. I would also get different answers from different nurses, so I never felt like I could trust the information I was given.

In general, this is a nice, warm, inviting clinic. There are great resources on the website. Dr. Kiltz goes above and beyond for patients. He has a live chat on Facebook weekly. He believes in healing adjunctive therapies such as acupuncture and massage and it is very easy to schedule these extras the day of transfer. The clinic is very busy, so just beware. You get what you pay for. The prices are cheaper, so you rarely see a doctor. It's not a perfect place, but I assume other clinics have similar drawbacks. Although I would hope pricier clinics offer more time with the doctor and more patient centered treatment plans with patient input. Also, Mondays and Fridays are crazy busy at CNYF. It is best to avoid appointments on these days as much as possible. I always make early morning appointments so I am not late to work. It is nice that they have appointments as early at 06:15 am. I've found that if I come in any later than 06:30, I end up waiting awhile to be seen (at least 30 minutes past my appointment time). It gets worse as the day progresses. First, when I had medications called in as a new patient, I asked multiple times which medications I needed to order right away. There were a ton and I was planning on doing a freeze all cycle. I was told to order them all. I ended up ordering all of them and not using several (I researched the meds more myself and did not want to take immunosuppressants since I don't have an autoimmune disorders). I was very disappointed no one took the time to go over the meds with me. If they had, I wouldn't have wasted my money on meds I will never use. As I mentioned previously, the nurses tend to give conflicting information when I ask different staff the same question. I now just email Dr. Kiltz when I need specific information because I don't trust what I'm told by the nurses. There was one occasion when a med was not called in for me. I just called and requested it again and it was taken care of. I was scheduled for my upcoming transfer without being asked if any days would not work, then the nurse had to change the day. When I checked my portal later, I was scheduled for transfer both days. If I had not checked, they could have potentially thawed my embryos a day early.

This is one of the things I knew going in - CNY handles many cases, and you need to advocate for yourself/your health if you go here.... It's comfortable. It's relaxing. Some people have told me that the wait would be long, but we never had to wait more than 10 minutes.

Take the Keto info with a grain of salt. Do not let it derail you. Yes, it can be done, and yes, there will be benefits, but it is *not* for everyone. I need it to manage my inflammation due to endometriosis. But it should not be how you are judged. You can cut carbs and get similar results. :) He knows his R.E. area, and he is very supportive once in there.

He was explaining the surgery to me, and I started to get upset, and he came over, sat next to me, held my hands, and whispered that it would be okay, we would come through this, we would have a baby. But first, the surgery needed to take place so I could be healthy enough for that baby. He said he would do everything possible to keep my ovaries, and in the end, he did.

This is one of the things I knew going in - CNY handles many cases, and you need to advocate for yourself/your health if you go here.
Dr. Kiltz, as busy as he is, has spent time with us during the consultation to address our concerns, and has picked up his personal cell phone to take my call. He is compassionate and seems to genuinely care.... While some of his methods and philosophies are unconventional, I find that I can take away some nuggets of truth from his advice and recommendations, even though not all of it (i.e. his extreme Keto diet) is for me. I have seen Dr. Kiltz at every visit to CNY (three to date), and he spent significant time with us on two of the three visits.

Dr. Klitz and CNY Fertility is well-known for offering affordable fertility treatments, as well as cutting-edge approaches. I feel fortunate to have found CNY fertility, as some of these new and innovative approaches are not available in my home state. Rationale: DOR, Fragile X carrier, low AMH/high FSH, poor responder to first round, so the meds/approach was adjusted for second retrieval. First retrieval resulted in 0 eggs/second retrieval resulted in 3 eggs, 1 frozen day 6 4AA embryo.

For the most part, my experience with CNY's nursing staff has been overwhelmingly positive and they have been so helpful and informative. The exception being one nurse who called me right before my second retrieval to tell me that the odds were against me, all of my follicles were likely cysts, and discouraged me from going through with the retrieval. I'm very thankful I called for a second opinion and checked in with Dr. Kiltz, because the retrieval resulted in 3 eggs (from 3 follicles, none were cysts) and 1 grade 4 AA embryo that we were able to freeze on day 6.

Just minor - I had to ask multiple times for a detailed meds list from CNY after my consult. There were some discrepancies in my meds list and what had been sent to the pharmacy. I was able to clear it up.
Dr kiltz is knowledgeable and he cares about the patient.... He gave us his cell number to call him or message him. It does take a while sometimes to hear back as he has a lot of patients. It can be confusing as the nurses run most of the show... He came in to speak with me multiple times retrieval day and after he knew I was nervous to get no mature eggs and did everything to assure me we would figure it out and had embryology call me 3 times day of retrieval to check to see if any matured in the lab post retrieval unfortunately none did but he was so kind

Dr kiltz is knowledgeable and he cares about the patient. He offered discounts and another round of ivf only having to pay anesthesia fee for retrieval due to getting not mature eggs. However knowing that we got over 25 eggs between 2 retrieval sand different protocols donor eggs would be the best option. He gave us his cell number to call him or message him. It does take a while sometimes to hear back as he has a lot of patients. It can be confusing as the nurses run most of the show but nurse kris in buffalo is amazing lines can get crossed especially if your a travel patient but they are pretty good at accommodating all questions and needs. I did feel like I had to be an avocation for myself as my situation was rare. Overall dr kiltz cares I wish he had less patients so he had more time with patients, but then again I'm glad he is helping so many people.

During my first embryo transfer in January, I was treated like a queen. My experience at the clinic was like none other. When I came back in March for the second embryo transfer, I definitely felt like a number. So many people are learning about CNY because of the cost, so a lot of people are going there. Dr. Kiltz is an amazing doctor. However, I feel as if the process was rushed the second time, and when the cycle failed my husband and I were left with a lot of unanswered questions. We are interested in trying again, but we are also afraid because we do not know what the problem is. We do not know if there is an issue with my uterus, embryo quality, sperm quality, etc.
Describe the protocols Robert Kiltz used in your cycles at CNY Fertility Center and their degree of success.
o Desogen 1 Pack, Take 1 tablet by mouth daily until instructed to stop.
o Estrace (Estradiol) 2 mg, One tablet by mouth twice daily
o Z Pak - -, Take 2 tablets by mouth the first day, then 1 tablet by mouth daily until all the medication is gone.
o Prednisone 5 mg, 1 tablet by mouth twice daily
o LDN (Low Dose Naltrexone) 4.5 mg, Take 1 tablet by mouth daily at bed time
o Aspirin, low dose 81 mg, One tablet by mouth daily
o Prenatal Vitamin one qd -, Take one by mouth daily
o Prograf (Tacrolimus) 0.5 mg, Take 1 tablet by mouth twice daily
o Cleocin Vaginal Ovules 100 mg, Insert 1 suppository vaginally at bedtime
o Endometrin 100 mg, 1 tablet vaginally twice daily
o Progesterone In Oil 50 mg, Inject 1 ml (50 mg) daily intramuscularly once daily
Before starting second cycle:
1) When you start your period, have an HSG (hysterosalpingogram) sometime between CD 5-12. An order for this has been uploaded fo your patient portal.
2) Complete an endometrial scratch between cycle days CD# 20-22. An order for this has been uploaded to your patient portal.
3) Call the office when you start your period AFTER the HSG and scratch are done. At this time, we will give you an order for baselines, call you once the results are received, and provide instructions on when to start the FET preparation medications (estrace, etc.)
4) Complete an Intralipid Infusion when you start your estrace, then repeat the day prior to FET with an HCG wash, then weekly. I have ordered the Intralipids and the HCG wash to Integrity for you. We will provide you orders for the Intralipid Infusion once we have dates (which we'll know once you start the period after the HSG and scratch.)
5) Start Neupogen injections when you start your estrace--this medication and the appropriate syringes have been ordered to Integrity for you and we'll provide you further instructions after your baseline.
6) Start Prograf 2 days prior to your FET. This has been ordered to Integrity for you.
7) Start Lovenox and Baby Aspirin when you start your estrace. The Lovenox has been ordered to Integrity and the Baby Aspirin (81mg) can be purchased over the counter.
6) Start plaquenil 200mg twice daily now. Then, stop it once you start estrace so that you can switch to Prograf.
7) Continue LDN 4.5 mg each night at bedtime.
o Neupogen 300 mcg, Inject 60 mcg (0.2ml) subcutaneously once daily as directed
o Lovenox 30 mg, Inject (1) prefilled 30 mg syringe subcutaneously once daily
o hCG- Uterine Wash 500 IU, Intrauterine wash- in office as directed AT 10AM
o Intralipids 20 %, 100 ml bag- Infuse 100ml as directed- 10AM
