Attention to Detail

Attention to detail is on a 1 - 10 scale, and is scored based on the rate, and severity, of errors committed by the clinic according to the patients.

For example, a severe error might be failing to inform a patient of critical information, while a small error might be failing to call a patient with results.

CDC Success Rates

This is the most recent data from the CDC. The CDC cautions that it should not be used to compare doctors and clinics. This data represents the outcomes of IVF cycles when using a person's own eggs.

For more information please see our FAQs.

Where you see a “?” on the chart, a clinic has done fewer than 20 cycles in this category and age range, so the CDC says showing percentages from this small data set would be misleading and unreliable.

Dr. G has a good bedside manner--straightforward but warm. He is decisive about the treatments he pursues but makes sure you feel on board with each stage of treatment. He definitely seems like he knows his stuff and answers questions thoroughly and explains the science behind his decisions without being dismissive.
Extend is a clean, modern, well-run clinic with a fantastic nursing staff. Affordability is a priority for them, and I appreciate their transparent pricing and care with managing the financial aspects of the process. I felt very well supported and treated with care and dignity...I can't say enough good things about the nurses and support staff at Extend. After a horrifying few visits to a different clinic, walking into Extend felt like a breath of fresh air. They're incredibly organized, competent, and supportive. I never felt like I was falling through the cracks, and everyone greeted me by name despite it being a very busy practice. Kezia, specifically, as my patient coordinator, was truly a gem.

What's one piece of advice would you give a prospective patient of Michael Guarnaccia at Extend Fertility?
The monitoring sessions can be brief, so if you have questions make sure to have a list ahead of time. Contact him by email if needed.
During treatment, were you treated like a number or a human with Michael Guarnaccia at Extend Fertility?
Dr. G is friendly and upbeat and present for the majority of my monitoring sessions (or as much as needed on request). He takes my questions seriously and is encouraging but realistic.

Describe the costs associated with your care under Michael Guarnaccia at Extend Fertility.
$6000 per retrieval, $3000 per thaw/fertilization, PGT $300 per embryo.
Describe Michael Guarnaccia's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Extend Fertility.
They prefer eSET and PGT. This was what I planned to do anyway...not sure if they require it.

Dr. Maslow was candid and took the time to ask me my understanding and knowledge. She was very direct and sometimes a bit more understanding/soft and caring approach would have been helpful . For example, while discussing my progress mid-cycle and that I was slower than expected, the communication was very direct and matter-of-fact. I think I would’ve preferred some more validation of the challenges associated with the process.
The [Extend Fertility] clinic staff warmly welcomed me each morning I came. They (mostly) patiently answered questions I had in my afternoon phone calls...The care coordinator was organized, efficient, and very responsive and responsible whenever I contacted her...The clinic is efficient and mostly warm/caring staff. I wish they would’ve had primary nurse(s), so that I would know that a concern I discussed with someone one day, I could follow up on with the same nurse another day, rather than sharing all over again. If there’s a staff person who you are more comfortable with, request them!

What's one piece of advice would you give a prospective patient of Bat-Sheva Maslow at Extend Fertility?
Don’t hesitate to speak up and share questions or concerns and advocate for yourself. If you ask, she will take the time to answer. But you have to initiate/ask first.
During treatment, were you treated like a number or a human with Bat-Sheva Maslow at Extend Fertility?
The clinic staff warmly welcomed me each morning I came. They (mostly) patiently answered questions I had in my afternoon phone calls. When I requested to speak with Dr. Maslow, my request was received and arrangements made for us to speak, rather than brush aside my concerns.
Describe your experience with your nurse at Extend Fertility. (Assigned nurse: Avigayil)
The care coordinator was organized, efficient, and very responsive and responsible whenever I contacted her. I needed paperwork completed and different prescriptions and she took care of it all. I did not have a primary nurse(s), which would’ve been helpful. On the day I received my trigger call, a nurse I had never spoken with called with my instructions which were different than what a different nurse (who I had spoken with numerous times) told me. I was disappointed to hear from someone new (to me) on my last day. When I requested to speak with a nurse in the clinic and by phone, they were mostly very responsive, compassionate, and patient with my questions.

Describe the specific things that went wrong at Extend Fertility.
I got my phone call with trigger instructions and they contradicted other information I had been told by a different nurse in the clinic earlier that same day. When I questioned the nurse on the phone, she tried to brush aside my comments/concerns. Later in the afternoon, the original nurse I had spoken with called me and I straightened everything out with her.

Dr. Maslow was very nice and communicated everything well to me. I've only had one consultation with her so far but I have no complaints, everything went well. I look forward to having more appointments with her once I start the process...She took the time to explain the entire egg freezing process to me and also answered al the questions I had. I didn't feel rushed and felt like I had her undivided attention and felt very comfortable speaking to her....
The clinic [Extend Fertility] was quiet and spacious, taking into consideration social distancing. Everything was organized and I was in and out fairly quickly...The first appointment I had with the nurses was very quick and easy. just one quick blood draw...if the patient is looking for a female physician at this clinic. As the only female physician, I did have to wait a few extra weeks for an appointment....

Describe your experience with Extend Fertility.
The clinic was quiet and spacious, taking into consideration social distancing. Everything was organized and I was in and out fairly quickly. There was some construction as the clinic is expanding but the only signs of that were signs in the clinic space. Otherwise, I would have had no idea, there was no noise or disturbance that impeded the appointment

He [Dr. Joshua Klein] explained what the procedure was about, and answered my questions...good bedside manners Cons: was late for appointment, like 1 h late, and didn't offered other alternatives. I think I should have tried ivf, but i didn't understand the benefit...He is very knowledgeable, but you have to ask many questions
[Extend Fertility] Strengths: very clean and welcoming ambiance. They have nice candies :) Weaknesses: far from my apartment and from public transportation. They were not open on Saturday. Waiting time was some times long...There were 2 nurses that were excellent, and were so skilled...It just happened one time. The nurse didn't explain about the medication.

I am seeing Dr. Maslow for egg freezing.... Dr. Maslow was extremely helpful in explaining the complicated elements of a pleasing to me. She was easy to talk to you even when my results were not so great in the beginning stages. She was encouraging but honest throughout the process, which I really appreciated.... She also went through other hypothetical AMH blood levels with me and how that might impact my anticipated results.
Extend Fertility provides egg freezing exclusively, so I found the prices are lower than other clinics for this very specific service. They used to offer free consultations and diagnostic testing, but now they charge $250 and if you end up using them to freeze your eggs you get to apply that amount to the cost of your egg freezing cycle.... There was one time when I showed up for a blood test that the nurse told me my test wasn’t on the calendar.

I would come with a list of questions for her. There’s a lot of basic information that needs to be told every single patient but beyond that it really is up to you to do your research and make sure you ask the right questions so that you get the answers you need to feel comfortable with your personal care.
During treatment, were you treated like a number or a human with Bat-Sheva Maslow at Extend Fertility?

He [Dr. Klein] was very knowledgeable and intelligent, and inspired confidence in his decisions. While this approach may not be comforting to everyone, I appreciated his analytical perspective and focus on the data and statistics. I like information and expertise, and I never felt as if I was lacking in either of those during the course of my treatment.... I only have one embryo to transfer, but he said he did not like to risk losing multiple embryos in one shot, along with the risk of multiples.
Because much of this new practice is still entwined with the egg freezing practice (Extend), there is a high volume of patients, but Dr. Klein and his team ensured that there was a personal quality to visits and treatment.... It's busy, and I did feel like I had to follow up with certain aspects of treatment (e.g., making sure the pharmacy had my medication orders, etc.). I think this may have been because I began my cycle on a holiday, however.

They are about half as much as other clinics, and they are also very clear in advance. I previously did IUI cycles at another clinic and they ended up costing about three times what I was told to expect. There were no surprises with Expect.

I started the day after New Year's Day, so the pharmacy did not have my order, and they also kept thinking that labs that I had previously submitted were missing. It is early stages for this branch of the clinic.

Dr Klein was reassuring and there for me as a patient. He encouraged me to email him with any questions. During the consultation Dr Klein explained the science, statistics, and procedures. He was also sensitive to my religious needs and worked around them...The doctor was always available and responded to all questions...
The [Extend Fertility] clinicians cared about how I would react to the meds and they opened early and didn't effect my work day. The office staff always offered a choice of water -cold or room temperature A weakness would be that street parking is not easy there and garage parking is expensive...There were a few nurses who all seemed nice. I didn't have much interaction with them.

Dr. Maslow cares about her patients and women's education around fertility. She is passionate about what she does and wants her patients to have positive outcomes. She communicates over the phone and over email.... We agreed to do 2 cycles of egg retrieval (I am 40) and we would determine if a third is necessary. After the 2nd, we agreed to a 3rd but she was confident that completing a 4th cycle was unnecessary.
Communication [Extend Fertility] was problematic and obtaining records was an issue. The steps of the process were not made transparent. Ex. How long would the eggs be stored in the clinic and then shipped to the storage facility was not readily available. Had to request that details be made in writing. For example, they called you over the phone to tell you the number of eggs that were going to be frozen but did not offer a written confirmation of the record unless I requested.

Unsuccessful
Dr Klein is very nice...But the methods they use are so aggressive and ineffective. They are very rigid with statics, and treat every patient like how a textbook would instruct. They try to pump as much medication into you as possible, hoping to get the best results. But it turned out to be very counterproductive and I am afraid its harmful to my body in the long run. And the results are so poor. I only got 50% of mature eggs the first time, and the second time half of the eggs magically disappeared ( I think they ovulated earlier because the doctor was late!)
I wish someone had shared their experiences with Extend, so I could have skipped my 2 cycles with them, and saved my ovaries Dont take me wrong, Dr Klein is very nice. The clinic is pretty well organized. (Dr G is a different story. Very rude and impatient ) But the methods they use are so aggressive and ineffective. I only got 50% of mature eggs the first time, and the second time half of the eggs magically disappeared ( I think they ovulated earlier because the doctor was late!) They use manupor unnecessarily. They ask you to inject full dose of ganerilx every shot. They don't time well the ovulation Window. They just rigidly go by the 36 hours rule. (I strongly suspect in the second cycle, the missed the ovulation window, and lost half counts of my eggs. Because the count of eggs was only half of what the unltrasound indicated. But the doctor was not willing to tell me why. They tried to hide the reasons. I even got yelled by doctor G for asking about the reason ) I only found out how these things are wrong after I went to New Hope for my third cycle this month. I was pretty skeptical about them at the beginning, and even wanted to go back to the extend. The extend sells them well for all the charts and statistic based result predication But at New Hope, they customize the medication for you. I didn't have to use manupor. They minimize both the ganerilx and trigger shots. I felt less influenced by the drug during the whole cycle. And the result was better. New hope is not the best. For example, they could have given me the wrong trigger shot medication if I didn't triple check with the nurse. just for a full disclosure But if I could have done it again, I wouldn't have gone to the extend at all. I feel really regretted, not for the money, but worried about how all the cycles might have impacted my health. But it's my fault that I didn't do a thorough research first. So I just want to share the experience here. I hope it will help someone who is going through the same situation-- egg freezing

Successful
One thing I feel kind of unsure about is that when I told him [Dr. Joshua Klein] that I knew that the success rate of freezing and thawing eggs was not as high as freezing and thawing embryos, he disagreed with me and said otherwise. I am pretty sure that it is a known fact, but since his entire clinics business model is freezing eggs, I'm sure he feels that he has to believe this.
Extend is mainly for people who want to freeze eggs, so you will not have as many interactions with him [Dr. Klein] as you might if you were going through the full process of IVF, etc. I thought he was fine for the purposes of egg freezing, patient and answered questions throughout the process, but it was really the staff and the educational materials on Extend's website that were the standout for me.
How was your experience with Joshua Klein at Extend Fertility?
I did not see Dr. Klein very much through the process, once at consultation and then again at the retrieval, but I did not really feel like I needed to see him that often, given that I was only doing an egg retrieval. He was very pleasant and kind when I did see him though. During treatment, were you treated like a number or a human with Joshua Klein at Extend Fertility?
I liked the Extend Fertility, the nurses were very sweet and kind and the clinic was never full. I never had to wait very long for my appointments, and the staff was very kind and understanding.
Describe the protocols Joshua Klein used in your cycles at Extend Fertility and their degree of success.
I have PCOS so I wasn't on the shots for very long - maybe a week and a half before they triggered with Lupron.
Describe your experience with the nursing staff at Extend Fertility.
As I said, the staff was really the standout here. They were really compassionate, always available, and very pleasant.
Describe your experience with Extend Fertility.
The clinic is really nice, and there was never a long wait. I felt like I was going to a spa every time I visited. Very new, sterile, and modern. The educational materials they have available on their website are awesome. Everyone going through the IVF process should read those materials b/c they do such a good job of explaining it in a way that you can understand.
Describe your experience with your monitoring appointments at Extend Fertility.
It was really easy to make appointments, always available before work, and never had to wait long to get seen.
Describe the costs associated with your care under Joshua Klein at Extend Fertility.
W extend, you pay around $5K. When I shopped around, it would have cost me at least $2K more to do it at a "normal" clinic.
What specific things went wrong at Extend Fertility?
  • Failed to call with results
Describe the specific things that went wrong at Extend Fertility.
The only thing I wish that they did was send me paperwork to confirm the number of eggs retrieved. But when I called and asked for it, they sent it right away.

Success w/ Doc Too early to know
It is very early to review Dr. Klein, but I will say that in our initial consult, although he was 30 minutes late to our zoom appointment, he took plenty of time to answer all my questions, and I felt strongly that he and I were on the same page. He was very thorough with me, and while he was direct and straightforward about my situation, not avoiding the facts (IVF is going to be difficult b/c of my DOR and my age), he was also reassuring and optimistic, which I appreciated. He seemed smart and dedicated and he was kind.
So far, I've been very happy with this clinic [Extend Fertility], except that my consult with Dr. Klein was 30 minutes late. But they have been great in reaching out regarding financial/insurance info, and with booking appointments...the finance team was great and let me know what to expect regarding upcoming costs and my insurance coverage.
How was your experience with Joshua Klein at Extend Fertility?
So far I have only had an initial zoom consult with Dr. Klein but he was excellent. Explained everything so that I could understand, gave good analogies, and took all questions in stride. He also got back to my email within 24 hours (which has not been my experience with my previous doctor). He was easy to connect with, very smart, very calm.

Describe the protocols Joshua Klein used in your cycles at Extend Fertility and their degree of success.
Since I have tried IVF twice now, at RMA, with an aggressive "microflare" protocol, he said he might try a different protocol, a slower, less aggressive approach, priming with an estrogen patch, then using a mix of Clomid and the other injectables. I think only one amp of menopur. He also said that he would recommend that I didn't do a fresh transfer (as I did previously), but that he would freeze the embryo, then transfer the next cycle.

Describe Joshua Klein's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Extend Fertility.
This is not really an issue for me since I produce so few eggs each cycle, it's unlikely I'll get more than one viable embryo. I will add more if it's an issue when I try IVF next.
