Dr. Cohen is very caring, straightforward, but also optimistic about treatment options. I never felt like he gave me false hope. Has has always been honest about potential outcomes and realistic success rates. To be honest, I wish I was able to work with him more frequently than other members of office staff. My only qualm is about a diagnosis. I was initally referred for bilaterally blocked tubes. He completed an hsn and stated all was clear and that my tubes were fine.

There is no weekend monitoring [at IHR] which can cause some difficulties. The office staff had been very helpful when I have called with questions. I was just thrown into treatment and would have liked a better understanding of exactly what to expect each month. It makes planning for monitoring a little easier as I live in another state. The billing department does not seem to be on top of things at all. It was several months into treatment before I even received an EOB for my initial visit.

What's one piece of advice would you give a prospective patient of David Cohen at Institute for Human Reproduction?

If possible try to complete your initial consult visit at his main office. He came to my OB/GYN's office for my initial visit. He had no staff with him and was doing everything by himself. While this isn't necessarily a problem, I feel like we would have had more material like handouts to take home and review if the visit had been at his main office. He doesn't leave a lot of time in the first appointment for questions. So make sure you are upfront with those early in the appointment to get your answers.

During treatment, were you treated like a number or a human with David Cohen at Institute for Human Reproduction?

Dr. Cohen gave us pros and cons for each type of treatment option. He always asked if I was ok, and genuinely seemed to care. He would often say, "if you were my sister I wouldn't have you do.." for x treatment.

Describe your experience with the nursing staff at Institute for Human Reproduction.

Nursing staff has been prompt. If you call you are unlikely to get a response immediately. I believe they are not in untill late morning/afternoon. I have found the best mode of communication with my nurse is email. They also have a voicemail patient portal to leave patient's messages about results. My coordinator has always make sure to talk to me directly if she is calling about protocol instructions. That way I can ask questions and ensure I am on the same page.

He [Dr. Cohen] was compassionate and easy to talk to, very understanding, reassuring and experienced. I felt very comfortable and positive with him...Dr. Cohen was very informative and understanding.... I went through 3 retrievals - 4, 2 and then 7 5 day embryos resulted and were frozen to go through PGS, with 9 recommended for transfer. In retrospect I appreciate that he was conservative and I have enough embryos to potentially have additional children, but hind sight being 20 20 I could've stopped after 2 retrieval cycles and likely had plenty.

I was referred to Dr. Cohen by a friend who had just delivered a perfectly healthy baby and shared her experiences with me along the way. He was compassionate and easy to talk to, very understanding, reassuring and experienced. I felt very comfortable and positive with him.

What's one piece of advice would you give a prospective patient of David Cohen at Institute for Human Reproduction?

Be patient with the frequent appointments, multiple times a week. They do have a lot of patients, but I appreciated that it was "first come first served" - on days that I needed to get to the airport for a 9am flight, I was able to come in at 6:30 and get my ultrasound and blood work done efficiently.

Everyone I talked to at IHR, from Nona the financial consultant to the nurses, were great, and except for one or two instances they were great with communicating the daily information that was needed. Dr. Cohen was very informative and understanding.

Describe the protocols David Cohen used in your cycles at Institute for Human Reproduction and their degree of success.

I started with Dr. Cohen in January after switching insurance so it would cover the procedures- I had been 6 months with another doctor on drugs and timed intercourse with no results. He gave me the option of doing IUI or going straight to IVF, which is what we chose due to my age (39, now 40) and everything was straightforward. I went through 3 retrievals - 4, 2 and then 7 5 day embryos resulted and were frozen to go through PGS, with 9 recommended for transfer. Sue was fantastic - compassionate and understanding, and worked with our schedules. I know she worked late hours responding to voicemails and emails at all hours. unfortunately she left the clinic towards the end of my treatment and there was a bit of confusion during the transition.

The surgery center where the procedures took place was great! Clean and organized, and a convenient location for me. I also liked the loft-style office and that they played the Today Show in the mornings while I was waiting for my appointments :) They were usually quick to get back to me.

Describe your experience with your monitoring appointments at Institute for Human Reproduction.

They had monitoring hours in the morning, so as I stated previously, it was very convenient for me to be able to go in early and still get to work on time (or occasionally to the airport for a flight!).

Describe the costs associated with your care under David Cohen at Institute for Human Reproduction.

Ugh! I had to go on an Exchange Blue Cross Blue Shield plan because neither mine nor my husband's employers companies covered the treatments because they were "self insured". That out of pocket max was around $6,000 which I hit with my first order of drugs, plus a $400/month premium. However, everything else was covered except for the genetic testing, and each retrieval cycle was a few thousand dollars, and then because I had more than the average (7) embryos go through PGS, there was an additional charge for that as well (but I suppose that's a good problem to have).

Describe David Cohen's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Institute for Human Reproduction.

Because we went through PGS, Dr. Cohen only transferred a single embryo, which made sense to me.

Dr Tur Kaspa knows my name, my situation, my story - he has empathized with my situation, hugged me, given me kleenex to cry. His partner, Dr Cohen performed my second egg retrieval, and I was so nervous when he popped in to introduce himself. His response, "I know all about you, your story, your history. Dr Tur Kaspa spent a long time speaking to me about your case, making sure I knew who you are. You are special. You are in good hands." I cried. I felt in good hands.

Describe the protocols Ilan Tur-Kaspa used in your cycles at Institute for Human Reproduction and their degree of success.

EPP on estradiol, 400-450 Gonal F, 150 menopur, 1/2 vial omnitrope (hgh), ganirelex, lupron trigger. EPP, MD Lupron 10iu/2x daily, omnitrope 1/2 vial daily (2 days), add in, menopur 150 am, gonal f 450 pm (continue with MD Lupron and omnitrope) tried lupron cycle when my follicles didn't respond one cycle

The nursing staff is fine. Some are friendly and helpful, while others seem exhausted and annoyed. And once, I went to my coordinator's condo complex to pick up a box of meds after hours. Now that's care that doesn't happen at larger clinics.

Monitoring takes place from 6:45-9:30am. Women begin showing up at 6am. The US tech is quick and efficient and begins taking patients early. From start to finish, I am usually there 1 hr.

Describe the costs associated with your care under Ilan Tur-Kaspa at Institute for Human Reproduction.

My OOP cycle was $10,500 plus meds. Embryo biopsy and freeze cost approximately $3000. I was charged a batching fee of $500 per cycle, which I did not like - why does it matter how many cycles I am doing? I tried to argue this charge and lost. Genetic testing is $4750 which is the highest of any clinic I have been to - which was disappointing to learn. Most of my cycles have been covered by insurance thankfully. For insurance covered cycles, I paid deductibles for meds, I paid cost of meds not covered by insurance, the batching cost, embryo biopsies and freezing and genetic testing (one test for all batched embryos).

He is willing to try new things, but will always refer to the real statistics and scientifically proven data. He is down to earth, kind, trustworthy, good humored, and very available. I found his level to of care to be exceptional. Another thing I appreciated about him was that he didn't take advantage of us in terms of requiring procedures that were not necessary. I was desperate to have a child, and would have paid for every test under the sun, but he was confident that he had the information we needed, given our circumstances, and that with time we would have success...

As a new patient, I would advise that you be assertive, but patient and set realistic expectations in terms of turn around time for yourself and your RN. Be direct and honest with Dr. Cohen. Ask him for his thoughts and reasoning behind doing things a certain way. He is a great blend of warm, but honest.

Dr. Cohen is AMAZING. He is caring and compassionate while setting realistic expectations. He is available to his patients and you are in constant interaction with him. He is not the kind of doctor who you do not see. He is down to earth, level headed, and treats you like an individual. He will move as quickly or as slowly as you like, but always keep your ending goal in mind. I feel so fortunate to have found him and know he is the best dr. around.

Describe the protocols David Cohen used in your cycles at Institute for Human Reproduction and their degree of success.

Given I have PCOS - we used Menopur and Follistm. I believe the dosage was between 75 - 150, so very minimal. He also recommended ICSI as my husband has sperm issues. His protocols resulted in tons of day 5 embryos. We initially did transfers without testing the embryos, but after no success, he recommended testing the embryos, which we did. My next transfer worked. We just had to find the right one, which he was saying all along.

She was always on top of my care, results, and status of where we were at. She was kind and available when needed. I appreciated her very much.

Our NC responded quickly to my emails with questions and was patient in explaining things. The only thing that was sometimes a drag was the early morning monitoring appointments. They are first come first serve so you never knew whather you'd be out in an hour or in 5 minutes. But never too bad and always quick once you were called. We're really pleased with the thoughtful care from Dr. Cohen and the Nurses we worked with during the IUI's, as well as our NC...He took time to meet with us, answered all questions and appropriately used humor to put us at ease. Very kind...

I was on Clomid 50mg (increased after first failed cycle I think), Ovidrel, Estrace (3x/day vaginal) and Crinone 8% 1x/day, combined with IUI. The second cycle was when the estrace was added (due to a thin lining, found during the first cycle), which is when we became pregnant!

Describe your experience with your nurse at Institute for Human Reproduction. (Assigned nurse: Jocelyn)

She was very kind and very patient worth my multiple emails. When she was out of the office, another NC would get back to me and they were great too. The nurses who did the IUI were also very warm and patient and spent a good amount of time with my husband and I answering questions.

[IHR is...] Small, intimate. Front desk staff - not friendly or warm. The phlebotomist is the nicest one! I felt like I had to be my own secretary- they often forgot paperwork and left me doing a lot of leg work to make sure each appointment happened. We waited over 45 minutes for an appt and Dr Turkaspa (Cohens partner) was not even made aware we were there waiting. It's walk in each day from 6:30-9am for blood work. I liked this aspect. I knew if I got there early I would be in and out fast!...

Be choosy about the coordinator they assign you - they are not all equal. Ask for email or how to contact after hours. I often had to scramble when things came up on weekends or evenings

Vernita had no "bedside" manner. She left confusing voicemails, was difficult to contact and once sent me someone else's test results ! We requested a new coordinator after that.

Hippa violation- sent me results with another patient name at top Consistently not being clear about what tests I had done or needed to do in the beginning of treatment. Mixed messages on doses. No way to contact after hours. The billing office took days /over a week to respond to questions. The Surgery Center at 900 Michigan had RNs with little bedside manner. My husband said the room where he had to sit to produce the sperm sample was dirty! He had to sit on his coat because the chair was not clean.

Dr Cohen himself was nice, appeared competent and experienced. He is personable...he didn't even take my history into consideration. It felt that he made his recommendations based on routine practice instead of an individual assessment... The actual clinic is a mess and then the surgical center he uses for all of his procedures is also not the best (difficult parking, EXPENSIVE even with insurance and flighty staff).

The financial lady [at Institute for Human Reproduction] was terrible. Actually a large part of why we left th clinic. You correspond with your nurse more than the MD so if the nurse/ancillary staff are not strong it will be difficult to get through. Even with he best MD recommendations and prescriptions you need a good nurse to guide you.

As someone who has a decent medical history he didn't even take my history into consideration. It felt that he made his recommendations based on routine practice instead of an individual assessment.

Also very hard to reach staff by phone.

Since we didn't have any other issues other than unexplained infertility, we followed fairly regular protocols with the addition of estrace to increase lining. Dr. Cohen didn't require birth control between each cycle which I preferred because of the added delay and birth control sometimes hurt more than helped my cycle. When we did have to use birth control in preparation for a test or getting to the next cycle, I often didn't have to go through the whole cycle before we could move ahead. Since the waiting can feel like forever, these small adjustments made a big difference in feeling like we were moving forward.

It is a smaller clinic [IHR] so you get more personalized care. The doctors are great, the nurses and staff, particularly the coordinators, can be hit or miss. The morning baseline and test routine is taxing and difficult, especially if you work and don't know how long things will take each time you go in. You don't always get results by the time you are supposed to which is also stressful. However, when there are scheduled procedures or appointments things go very smoothly and people are very nice and helpful.

Dr. Cohen was very proactive with our care. When things didn't work he would suggest what next steps and tests we could do to find out answers. He wasn't pushy with these options (PGS, ERA, etc) since some cost extra money, but giving us the options right away was helpful rather than going through more failed cycles before trying. From the beginning he was wonderful and informative to meet with, however I don't think he set up our expectations on the time and emotional and physical toll everything can take, as well as about potential delays that can happen. Once we started to understand all of these items over time, on our own we were able to deal with them better. I just wish I had understood more about how long and taxing the whole journey could be earlier on.

Don't be afraid to reach out to him with questions or concerns. He is caring and willing to explain things, but also busy so you also need to be your own advocate if you have questions or concerns.

They have a lot of patience, and can be slow to respond or not very compassionate. Sometimes they don't explain things well and several times there were mistakes, mix-ups or medicines not ordered. I had to definitely be my own advocate and push to get the right information or things corrected. Usually they assumed things were my mistake when in fact they almost never were. Also, the protocol for morning baselines and test is long and trying. You have to be there early and never know how long it make take. Once you are meeting with someone, however, the staff are in general very nice, especially with it being so early in the morning.

Monitoring was only in the morning between 6:30 am - 9 am, first come, first serve, where you would have to go in and wait. At a minimum it would take 30 min if it was empty, other times I was there for close to 2 hours. You could not schedule time and you never knew how long it would take, so adjusting for work was difficult.

One or two times, I didn't get my results and would have to call in to make sure I knew what I was supposed to do next.

On Dr. Tur-Kaspa himself, he was very kind and professional. He told my husband and me all about our odds and what our procedures might look like. At our initial consultation he mentioned that I was overweight and that I would have a better chance of getting pregnant if I lost weight (this was not a problem but in retrospect I felt he did not stress this enough and that I was given no tips based on any of my test results). We decided to wait a cycle before getting into medications due to a uterine polyp removal. After the cycle wait, we put together a plan and I started on the medications for IVF and egg retrieval. Almost immediately he left the county for a couple weeks which meant he himself would not do my retrieval or my transfer and that I was handed off to the other office doctor. Make sure that you ask him about his potential travel plans so that you will be aware if he plans to leave during your treatment.

Funny the first time, but after months of exhaustive fertility treatments, by retrieval #3 and #4, I wanted to scream "WE GET IT." A little pushy with the 'sales front', but I do feel like he was extremely attentive, and always willing to adjust protocol to get desired results. Overall I'm torn over the entire experience as a patient with him, we were happy with results, but felt like he could be insensitive.

Results were left on the call-in voicemail, through e-mail, and phone calls, and I never really knew which method to check, or WHEN to expect them [at IHR]. Made for very stressful days waiting on results. Positive pregnancy results were usually phoned in, but one time was left on the voice-mail call-in, so after waiting an entire day past the 5pm close time of the office, I was justifiably angry to find out they had been on the call-in all along. Also, pregnancy results should ALWAYS be communicated via a phone call.

Ie: the first call shouldn't be about when you will schedule your next transfer.

To clarify... I saw Dr. Cohen when he was the head of the University of Chicago's office. He is now at a different practice. This review is of the UofC's clinic. Their small size is their strength. They can treat patients with respect and kindness.

Monitoring appointments were first-come, first-serve, first thing in the morning, but that worked fine with my work schedule. There was one ultrasound technician, Deb!, who was kind and funny. There were two changing rooms to get changed in and the procedures were quick and easy.

Dr. Tur-Kaspa [at IHR] did the ERA test on me in November and took too much of a sample, which appears to be unheard of. I had to redo the test, which was stressful. There is only one ultrasound tech and phelobotomist that works in the mornings at the Chicago location, which can cause a backup when trying to get to work in the morning. They are organized and are accurate with insurance checks and scheduling.

I felt comfortable with him from the moment we met!...Our first cycle was natural. Dr. Tur-Kaspa is a terrific and knowledgeable Dr. The first live birth took me 7 transfers (3 miscarriages followed by 3 non pregnancies but he stayed positive and tweeted my meds until we got our baby. Then with 1 embryo left 2 years later, he told us about a test that made sure they were transferring at my most receptive window. The ERA test told us that I needed 12 more hours of progesterone so we went for it with just the 1 embryo and I am now 14 weeks pregnant!...

The charge was over $500 for a 45 minute conversation...the entire office ran very smoothly. It was a very bright and welcoming office. Everyone was friendly and very responsive. And when visiting the oak brook location, the office was very dingy and when I was preparing for my ultrasound, there was a dead cockroach on the floor.

Dr Cohen got to know me and my husband over the last years and the staff at IHR is pretty good about remembering me. There have been mix-ups, but I have not felt like a number.

IHR runs well during monitoring hours. I have only had to wait as long as an hour once. I'm usually done with bloodwork and ultrasound in about 45 minutes or less.

A few missteps along the way. My clinic coordinator communicates via email. When she is out of the office, she does not put on an out-of-office auto-reply, which in the middle of an IVF cycle, does not seem responsible. I have found myself waiting for a response and feeling unsure about what to do or how to reach someone who will know what my situation is. We also had a lot of trouble getting the information we needed about donors when we switched to that process. And the clinic mixed up which donor we wanted to work with. Most of the time things go right, but I've been frustrated on occasion.

I was said I could not get pregnant with my own egg by another doctor and Dr Tur Kaspa agreed to work with me and the treatment was successful if getting a viable embryo from my own egg. He also found cyst on my ovary that the other doctor had missed. He didn’t over promise but delivered the result...You are in good hands. Don’t hesitate to discuss your own expectation...Growth hormone for reproductive use

Go early for blood works [at Institute for Human Reproduction] so you don’t have to wait long. They have different times you have to visit so you may have to change your work schedule. I was said I could not get pregnant with my own egg by another doctor and Dr Tur Kaspa agreed to work with me and the treatment was successful if getting a viable embryo from my own egg. He also found cyst on my ovary that the other doctor had missed. He didn’t over promise but delivered the result.

[The nurses at Institute for Human Reproduction] Forgot to call in my meds. Was Olgas [nurse] fault...Olga is difficult to work with. Olga is difficult to work with. The coordinator was new, so I understood there were some difficulties there, but Olga was truly the unpleasant, difficult person in the office.
