Dr. Wilshire has never seemed like he was rushing and instead took his time in our consultations. He has listened to my concerns and engaged with me at the level of my knowledge, instead of talking down to me or dismissing my own research and questions...He also did a good job explaining things in a plain-spoken way for my husband. He is optimistic without giving false hope. I am confident that he stays current in all the trends and research and responds to that when appropriate.
Sometimes I felt a little rushed to try and get questions in before they [nurses at Missouri Fertility] rushed out of rooms, but overall I appreciate how they respect all patients' time by staying on schedule and keeping check-in appointments brief...So far, I've had six attempted treatment cycles...They charge $15 to give you the trigger shot at the office. For IVF, we've been quoted $15,000 for IVF with one transfer included and $6,000 for the genetic embryo testing. Additional frozen transfers are $300, if I remember correctly.

How was your experience with Gil Wilshire at Missouri Fertility?
Dr. Wilshire can be a bit wild with his jokes and responses. We were always comfortable with the joking, but it might not be to everyone's taste. He is a straight shooter about my situation and I feel like he takes my symptoms and gynecological history more seriously than any other past doctor. Instead of discouraging me from reading about options, he respects the knowledge I bring to the table and responds on that level. He also did a good job explaining things in a plain-spoken way for my husband. He is optimistic without giving false hope. I am confident that he stays current in all the trends and research and responds to that when appropriate.
What's one piece of advice would you give a prospective patient of Gil Wilshire at Missouri Fertility?
Go in to your visit with Wilshire knowing that he is very jokey. He is very open to questions or challenges about why he recommends something, so feel free to ask him anything. Infertility can be really depressing and sad, and Dr. Wilshire absolutely understands that, but it can also be helpful to laugh through the process and he leans into that. But he is dedicated to his patients. He is aggressive in treatment because he wants you to have a successful pregnancy sooner, so do some thought before you go in about what level of treatment you are ready to take on and what pace you want to take.

The entire clinic is very good at interacting with patients, from check-in to procedures. Dr. Wilshire has never seemed like he was rushing and instead took his time in our consultations. He has listened to my concerns and engaged with me at the level of my knowledge, instead of talking down to me or dismissing my own research and questions.
Describe the protocols Gil Wilshire used in your cycles at Missouri Fertility and their degree of success.
So far, I've had six attempted treatment cycles. Cycle One - Clomid + IUI: 50mg clomid on days 3-7 with HCG trigger (pregnyl). I had one follicle at time of trigger. IUI 36 hours after trigger. Ultrasound two days after IUI showed the follicle never ruptured, no pregnancy possible. Cycle Two - Clomid + IUI: They increased to 100mg clomid on days 3-7 since I did not ovulate in cycle one. Two mature follicles at time of trigger (pregnyl). IUI 36 hours after trigger and ultrasound confirmed ovulation the next day. Progesterone suppository 2x/day for 14 days was used to encourage implantation. Negative pregnancy test. Cycle Three: I took 100 mg clomid days 3-7 and was planning to do trigger after ultrasound showed one mature follicle, but I had an allergic reaction to an antibiotic I took for an unrelated MRSA infection, so I cancelled the IUI. Did not ovulate on my own (despite mature follicle) until over a week later. Cycle Four: I took 100 mg clomid on days 4-8, but ultrasound on day 11 showed ovulation had already happened. Period started six days later. Cycle Five - Letrezole + IUI: I asked to switch to Letrezole since I'd only had only one good response to Clomid. I took 5mg Letrezole days 3-7 and had one mature follicle at time of trigger shot (pregnyl). IUI ~24 hours after trigger. Progesterone suppository 2x/day for 14 days was used to encourage implantation. Negative pregnancy test. Cycle Six - Letrezole + IUI: Same protocol as cycle 5. Triggered with ovidrel for this cycle. One mature follicle at time of trigger. Progesterone suppository 2x/day for 14 days was used to encourage implantation. Negative pregnancy test.
Describe your experience with the nursing staff.
The nurses were not overly sentimental, but have always been very open to questions and compassionate about our situation. Sometimes I felt a little rushed to try and get questions in before they rushed out of rooms, but overall I appreciate how they respect all patients' time by staying on schedule and keeping check-in appointments brief. I've also called in with questions a few times and gotten prompt responses. In terms of care, I feel very comfortable with all the procedures nurses have done. I also like that they are personable and share their own personalities. It makes it feel much more like a community than a typical doctor's office.
Describe your experience with Missouri Fertility.
The clinic is easy to get to, has good parking, and is very responsive on the phone. I LOVE Victoria who is frequently the front receptionist. She is kind, compassionate, and organized. I've really appreciated how clear they've been about costs. Scheduling has never been a problem and they had a lot of options for me before 8am so that I did not have to take time off work. I would want new patients to know that they will call you with your partner's sperm count results. Just be prepared that you will be the one receiving and relaying that information. The history of their work in our community is long and strong.
Describe the costs associated with your care under Gil Wilshire at Missouri Fertility.
My IUI treatment cycles were about $850 each. Each ultrasound (included in the 850) was about $130. I don't believe the IVF consult had a cost, but initial consult was under $100, I believe. The trigger shots were $100 each, progesterone was about $60 each cycle. Clomid and letrezole were very cheap. They charge $15 to give you the trigger shot at the office. For IVF, we've been quoted $15,000 for IVF with one transfer included and $6,000 for the genetic embryo testing. Additional frozen transfers are $300, if I remember correctly.

I am only just starting on an IVF cycle with Wilshire, but I came into our IVF consult with a strong preference for minimizing the chance of multiples. He seemed very affirming of my preference to only transfer one embryo at a time (assuming we have embryos!). Because we decided to do pre-implantation genetic testing and my age (29), he encouraged the single embryo transfer.

He told me I should tolerate clomid well, but if I don’t, I shouldn’t hesitate to call him. I ovulated and followed all instructions as laid out, and got our first positive pregnancy test.

Starting from the nurse Amanda, to all firstline staff to Dr Gil, I was address with my first name, they know my name once they hear my voice, I was treated like a human, addressed , my concerns were addressed accordingly. They were more happy than I when I had my first positive pregnancy test. So optimistic right from the beginning, I hung on their faith, and their faith boosted mine. They’re literally the best.
Describe the protocols Gil Wilshire used in your cycles at Missouri Fertility and their degree of success.
He ran series of tests to know if my prolactin level is within normal range, check if I’m immune against German disease and others I can’t remember. He started me on clomid with instructions on when to start, when to test for ovulation, when to have sex, and to just be on the lookout. He told me I should tolerate clomid well, but if I don’t, I shouldn’t hesitate to call him. I ovulated and followed all instructions as laid out, and got our first positive pregnancy test.

OMG, Amanda is an angel, very compassionate and has a listening ear. After seeing Dr Wilshire in order to start our first clomid treatment, I called to ask if I should call for just anything, she told me the only she’s expecting from me is to tell her I’m pregnant. And that’s how she won my heart. She’s very optimistic.

I feel like our situation was treated just like everyone else with similar situations. I just felt sometimes he [Dr. Gil Wilshire] was insensitive to our unique situation and comparing us to others. I don’t believe all of our issues were resolved but we are on our way...has always treated us with the utmost respect. He has been open-minded and very positive. PCOS- changes in diet and exercise regimen, laparoscopic surgery to remove cysts and to do a bumper to bumper examination, clomid for my husband’s low count and referral to a urologist who specializes in male infertility. IVF beginning in May 2019.

This clinic located in Mid-Missouri is a asset to our area. We are fortunate to have it instead of traveling to KC it St. Louis for such an emotional, needy and stressful journey. However, I do not feel that he is open minded to adjusting treatment and retesting based on changes in patient condition or concerns. He is very skilled and knowledgeable, but seems rigid in care.... for all of my IUI's we used a 5 day clomid regimen to produce 2-3 follicles, and a specifically timed insemination in conjunction with hcg injection. I cannot remember the exact medications and order of receiving them for my IVF cycle. But when I originally researched it, it was seemed to be based on the same protocols of many skilled fertility centers.
Over all I do think this clinic [Mid-Missouri Reproductive Medicine and Surgery] does good work, but need to be better at reviewing patient charts more regularly.... When I first started the IVF process, they did not tell me that the protocol and prices had changed from the year before.... They are very caring and compassionate when they understand every aspect of what is happening. They are very skilled and knowledgeable. However, I also feel that they have an approach that is very rigid when it comes to alternative viewpoints.

I was also frustrated because i had to order excess medication because they were unprepared for how my ovaries would react to the IVF meds.

When I first started there as a patient I felt very much like a human, care felt very personal. After my initiation of the IVF process, I felt much more like a number due to the fact that they often forgot to give me necessary information, rushed me through appointments, brushed off concerns, and care was more protocol than personalized.

for all of my IUI's we used a 5 day clomid regimen to produce 2-3 follicles, and a specifically timed insemination in conjunction with hcg injection. I cannot remember the exact medications and order of receiving them for my IVF cycle. But when I originally researched it, it was seemed to be based on the same protocols of many skilled fertility centers. I do know that I was taking IM progesterone, which I became allergic to around my 5th week of pregnancy, right after switching medications is when I MC. It should be mentioned that I asked for an alternate protocol with higher dose steroids for longer period of time because of immune concerns, and was dismissed.

They should have done some research before going to the clinic. They should realize that every problem may not be fixable. Over all I do think this clinic does good work, but need to be better at reviewing patient charts more regularly.

I'm not really sure because they didn't review my insurance right before we started, so I want to say the total amount before the review was 14,000 with meds and icsi and freezing. However after they took my new insurance into account the cost was around 6,600. Then there was a little less than 6,000 for PGD testing.

They did a very good job of explaining why this is the preferred method for transfer.

Describe the specific things that went wrong at Missouri Fertility.
When I first started the IVF process, they did not tell me that the protocol and prices had changed from the year before. They did not go through the financial responsibilities with us prior to starting the treatment. They did not re review my insurance for changes. They did repeat any prior labs for previous conditions that I may or may not have had; or at least they did not report that they did so. And on at least 3 occasions, I was told a prescription had been called in, only to be told by the pharmacy that they had not received it. Each time I had to call the clinic and have them resend it, and then the pharmacy would get it, but at the risk of myself not getting the medication in time. They also failed to inform me that I didn't have to use the pharmacy that they routinely ordered meds from.

He [Dr. Wilshire] sat down with us and went through in detail about our file and after surgery sat with me and explained everything in depth and answered any questions I had...He can be blunt and he is not afraid to give you any bad news. I truly think he wants each and every patient to have a child... Next transfer using last embryo nothing was changed protocol wise. Same results. Asked him about possible egg quality issues or other tests that could be done. Essentially he said no. And we should just get a hold of him again when we want to do another retrieval and ivf cycle.
All of the nurses [at Mid-Missouri Reproductive Medicine and Surgery] are very nice and compassionate to the situations we are in. They are very positive and it is a pleasant experience when we are there. Kim is always nice and asks about our lives...A time or two after having blood work they did not call to tell us the new amount of injections to do. We had to call to get answer...Their billing and insurance processing is slow. Some visits we pay the usual amount and then sometimes don't pay anything. After that I can expect a big bill at the next appointment. They usually say insurance paid more or didn't cover it.

He is a very busy doctor. Doing surgery and ivf takes him out of the office a lot. When we talked to him though he does not speak to fast and allows us to stop him and ask questions whenever we are confused.

He sat down with us and went through in detail about our file and after surgery sat with me and explained everything in depth and answered any questions I had. His nurses are very nice and always remember us and ask us questions about us that we discussed last time we were there.
Describe the protocols Gil Wilshire used in your cycles at Missouri Fertility and their degree of success.
Since we already had several tests performed by prior doctor he went over file with us. Started a course of action to do a few iui and if not pregnant go in for surgery to see if endometriosis exists. Once through surgery he wanted to do a few more iui. We have since decided to move on to ivf. Started iui with clomid and last 2 cycles have been with injections of folistim. We moved on to IVF and completed one egg retrieval. Egg numbers were higher than average and at the stage of freezing we were left with 6 I believe to send to be genetically tested. The first transfer we did two embryos, 1 implanted but miscarried. No explanation as to possibly why. Next transfer using last embryo nothing was changed protocol wise. Same results. Asked him about possible egg quality issues or other tests that could be done. Essentially he said no. And we should just get a hold of him again when we want to do another retrieval and ivf cycle.

Their billing and insurance processing is slow. Some visits we pay the usual amount and then sometimes don't pay anything. After that I can expect a big bill at the next appointment. They usually say insurance paid more or didn't cover it.

Some of the services were covered under insurance. Not sure of their original prices. 63 for trigger shot. 125 for semen collection and washing. 600 unit of folistim is $420.

He prefers to do single transfers. Our first transfer we did two and at multiple steps he always wanted to make sure we knew two could mean twins and make sure we wanted two.

It was hard to know the cost of everything. That is more to do with insurance and not necessarily their office. They do their best to give you a worst case scenario so that's what we planned for.

We have been with Dr. Wilshire's team off and on for 5 years. We have 2 children and currently pregnant again. We have always had very personalized care by both Wilshire and his team.
Describe the protocols Gil Wilshire used in your cycles at Missouri Fertility and their degree of success.
1st child: We had already done clomid before. So we started off with Wilshire doing injections with folistim and IUI's. After about 4 we did and exploratory surgery and them 2 IUI's to get preganant with our first child. 2nd child: We did several IUI's and then had 3 miscarriages. Took a short break. Did another clean out surgery and then a few more IUI's. Then we had out 2nd child. Last set: We did three IUI's woth injection s and got pregnant with twins.
Describe your experience with the nursing staff.
The nursing staff is amazing. We had great experience with all of them and they really felt more like family after all our experiences with them. They cried with us and rejoiced with us in the good news.

I felt like Dr. Wilshire really cared about me and the nurses as well. Dr. Wilshire can come across as a little abrasive at first but he is absolutely hilarious. He never made me feel uncomfortable. He and his wife struggled to conceive himself so I know he has empathy for the situation his patients are in. He was willing to work with what we wanted, but did suggest things. The chances of me getting pregnant were really small because of my low egg count. He didn't give up even with the 4 IUIs and 1 chemical pregnancy resulting from our first trial of IVF. After that happened he had us meet with him in his office (no charge for visit) to make sure we were doing ok, what his plan was for us moving forward and even talked about what could have gone better.
What's one piece of advice would you give a prospective patient of Gil Wilshire at Missouri Fertility?
Trust Dr. Wilshire. He has a reputation of getting people pregnant for a reason. Every woman I know who has needed help conceiving in the greater Columbia area has gone to him. Dr. W says that after you leave his office after getting treated you will be a better person, and it is true.

I felt happy every time I had to go in because I felt like the people I was dealing with there were like my friends and really concerned with my outcome.
Describe the protocols Gil Wilshire used in your cycles at Missouri Fertility and their degree of success.
With IUI- 3 trials of Clomid with trigger shot to ensure ovulation and 1 trial of injectables (follistim) with trigger shot. With IVF-Follisim to increase my number of eggs, menopure, ganarilix before retrieval. Retrieval occurred under sedation, fresh transfer of 2 embryos resulted in chemical pregnancy, frozen transfer of last 2 embryos resulted in singleton pregnancy which I am currently in my 11th week of carrying! No real adverse reactions to meds. After frozen transfer I was put on estrogen and progesterone suppositories as well as progesterone injections daily which were not bad.

All the nurses were great, they didn't always fully educated, however I would ask lots of questions which they were able to answer either in person or over the phone.

I think total costs for IUI were around 400-500 per cycle. IVF in total was around 15000. Some was covered by insurance

Dr Wilshire was very relatable due to the fact that he struggled with infertility as well. He took time to explain everything and all of our options. Dr wilshire explained how important weight and a healthy lifestyle was when dealing infertility so I decided to take a break from treatment and get healthy. I was able to loose 40lbs and concieved naturally in 2016 but I miscarried at 10wks under Wilshire's care. He preformed a D&C and then shortly after he completed another Laproscopic surgery in the beginning of 2017 to remove the endometriosis that had grown back. We completed a 6th IUI with injections but unfortunately it failed.

In 2014 We completely a Laproscopic surgery to diagnose and remove my stage 3 endometriosis. We coukd not afford IVF so Wilshire explained IUI to us as well as the cost. We were given several options as far as medications we could use and chose the cheaper option to begin with which was Clomid. When clomid failed for the 4th time, we moved onto Gonadotropin injections, with another IUI but unfortunately it failed also. Dr wilshire explained how important weight and a healthy lifestyle was when dealing infertility so I decided to take a break from treatment and get healthy. I was able to loose 40lbs and concieved naturally in 2016 but I miscarried at 10wks under Wilshire's care. Dr. Wilshire went through both mine and my husbands medical histories. He suggested that I may have PCOS. Upon doing the first US, a nurse did it, but Dr. Wilshire stood behind her and explained everything to us and I did in fact have PCOS. He laid out our treatment plan and told us we could start right away! He was very nice and compassionate during the whole process. Unfortunately we didn't see him at all during our first cycle as he was out for surgery on his shoulder. But we saw him at the very next cycle.
They [Mid-Missouri Reproductive Medicine and Surgery] have a lab in house for semen testing. They have multiple treatment rooms. They are able to perform hysteroscope in the office. Pharmacy next door for easy access to meds... $124 per each US, $10 for clomid, $85 for HCG trigger shot. $240 for semen prep and IUI. Around $600-700 total per cycle

US on cycle day 3, start clomid 100mg for 5 days immediately. Return for US 8-9 days later to see lining and follicles. Then a HCG trigger shot. Timed intercourse night of shot and after IUI. IUI 36 hrs after shot.

They all are very friendly upon calling you back. They engage you in friendly talk about the weather or what plans you have for the weekend. They all are very competent in doing the US. Most of them are good at explaining stuff to you.

For our first appointment, we watched a video that explained the fertility process and then Dr. Wilshire came and brought us to his office. He asked us all of the life style and medical history questions- in most offices the nurses do this, we liked that he did. He explained what the next few months would look like for us and then did an ultrasound. On my next cycle, I took Clomid followed by a trigger shot and IUI. We were successful on our first round!

1. Clomid 50 mg-was not able to do iui this cycle. Neg hpt. 2. Clomid 100 mg, progesterone supp, hcg inj. Successful but miscarriage at 6 weeks. 3. Clomid 150 mg, progesterone supp, hcg inj. Negative hpt. 4. Clomid 150 mg, progesterone supp, fsh injections, hcg inj. Successful and currently 10 weeks w one beautiful baby!

They are SO organized and to the smallest detail. I think they knew my cycles better than me! It took so much stress out of the process for me knowing that they were on top of everything. From bloodwork, to diet, exercise, meds, schedule...I just had to do what they said. And that made me worry waaaaaay less having someone just tell me what to do. Literally the only thing I didn't like was their after hours answering service... they weren't great at delivering messages.

US were $120 and then eventually insurance covered, Clomid depends on in but mine was around $30, Progesterone was $65 per fill, hcg inj was $90, iui wax around $300. We thought all charges were reasonable. Working in healthcare, I feel like I have a pretty good handle on cost of procedures.

We did fresh IVF cycles with ICSI. Our first pregnancy also required a TESA procedure. Our first cycle was partially covered by insurance. We had to pay about $14,000 up front and were reimbursed by the clinic after insurance paid, how total cost for that cycle was about $6,000. For our second cycle insurance covered most of it. We still had specimens froze so we didn't have to do another TESA procedure. Our insurance was checked this time and we only had to pay what insurance wasn't going to pay. At the time of our consultation we had different insurance so our consultation was $157.75. Our insurance changed at our cost were $320 for Hysterscope with practice transfer 2 office visits for ultrasound and Estradiol Levels $53.06 each $1500 for IVF/ICSI 3 office visits for ultrasounds after transfer $25 each For a total of in office cost of $2001.12 Medication Costs were as follows Birth control pills prior to starting cycle no cost Valium $0.68 for hysterscope Doxycycline $10 Follistim 4 vials $50 Menopur 8 vials $100 Ganirelix 3 vials $50 Endometrin $50 Augmentin $3.90 Aspirin $0 HCG $20 Total Medication cost $284.58

The staff knew my name, always took great care with my numerous calls with questions, and returned my calls very quickly. Treatment is highly personalized.
Describe the protocols Gil Wilshire used in your cycles at Missouri Fertility and their degree of success.
Initially, I had an exam which showed some ovarian cysts. Dr. Wilshire said I may have PCOS, so he did a procedure to possibly diagnose that and to remove any cysts that may be there. That went well. I didn't have PCOS, but several cysts were removed. This increases fertility. I took one round of Clomid, and did an Hcg injection, then went to have an IUI with donated sperm. A few weeks later we found out we were pregnant! Dr. Wilshire told us there was about a 60% chance of getting pregnant after three cycles of trying IUI. So we didn't expect it to happen so quickly! We are 10 weeks pregnant now.

Amanda was always available to answer questions, or called me back very quickly. All nursing staff was professional and courteous.

The clinic was easy to get to, with great parking. Appointments were easy to schedule, staff was friendly, and we enjoyed our entire experience there.

I have a high deductible, so I paid film price for everything except the procedure I had. All office visits and ultrasounds were under $150.

Dr. Wilshire totally made me feel like a human. He sat down and talked with me for over an hour about every detail of my life that could be affecting my fertility while not making me feel bad for any of it!
Describe the protocols Gil Wilshire used in your cycles at Missouri Fertility and their degree of success.
I had a total of 2 rounds of IUI treatment. For each round, I went in on the first day of my period for an ultrasound and then was given a prescription for clomid (25mg the first round, 50mg the second) in which I took for 5 days towards the beginning of my cycle. Then in the middle of my cycle, I would go in for another ultrasound to make sure my ovaries had grown appropriately and that there was an egg ready. The gave me an hcg trigger IM injection to do at home that night and then the following day, my husband would go in and deposit a sample in which the lab in Dr. Wilshire's office would spin out the 'bad ones' of the bunch. 2 hours later I would go in and they would inject his sample directly into my uterus. It was completely painless and then I would rest there with my knees up for 15min. Later that night my husband and I had intercourse to set the 'environment.' After the IUI treatment I had to take progesterone suppositories until I got a negative pregnancy test or until 8 weeks gestation. 2 weeks later I took a pregnancy test. The first round it came back negative but on the second round it came back positive. I had to do IUI treatment because I have endometriosis and PCOS along with a blocked tube. The only thing that was not so pleasant about the whole expericence (other than it just being an uncomfterable situation anyway) were the meds. They made me feel pretty moody but it was all so so worth it because I now have a 3 month old baby boy!

My husband and I saw fertility doctor after fertility doctor due to recurrent loss. I feel like we got nowhere and got different answers every single time. I have several family members and friends who have full blown infertility and they got pregnant successfully within 1-3 visits with Dr. Wilshire after years of trying. When I got an appointment with him, we walked out very confident in Dr. Wilshire's plan for us. We we lucky enough to get pregnant the first round of IUI. This pregnancy is the first pregnancy out of 4 total, that is healthy. And I believe it was because of his approach. He kept a close eye on me through about ten weeks pregnant with my history of miscarriages. When we had to depart it was very bittersweet. I have been referring everyone who I know that has infertility to him. My husband and I will definitely be back if need be for our other children.
What's one piece of advice would you give a prospective patient of Gil Wilshire at Missouri Fertility?
If you want to go through treatment, you have to be willing to spend a little money and be able to take off work last minute. This wasn't even an issue for us though because IUI was very affordable and they were able to get me in at 7 am before my work even started. Each of the visits had no wait in the waiting room and I was in and out within less than 15 minutes and that even included the quick IUI procedure. I was even able to go straight back to work after that with no restrictions.

I did a series of tests with several different doctors and even a hysteroscopy and laparoscopy to try to figure out recurrent loss. When I got to Dr. Wilshire, he did one more round of tests to see if I needed to do IUI or IVF. Lab work came back good so we did IUI. I did 50 mg of clomid with ultrasound monitoring, trigger shot and IUI. Then he put me on progesterone. I had only one dominant follicle. First round worked and I got pregnant successfully with a singleton. I am currently 11 weeks pregnant.

The nurses showed a lot of compassion through the tough journey. When I had phone calls every time I had a worry, they saw me right away or gave me the answer I needed. I grew attached to the staff because it was such a personal experience.

Since I was diagnosed as having PCOS one of the biggest parts of my treatment plan was to be on a low carb diet. This helped to regulate some of my levels in order to make my IUIs successful. Our medication was modified as needed based on what my ultrasounds would show.

The nurses were always available when I called with questions. They were knowledgable of our treatments and time was not wasted having to explain why we were there. They were always very friendly, kind and helpful.

There was never a time where we went home disappointed in the care we received [from Dr Wilshire]. We may have been disappointed in outcomes along the way, but those were outcomes that were not in anyway in his control. Dr Wilshire from the get go was very forward. He didn't sugar coat things, but he also was not too harsh or gruff in explaining the hard facts. From the first visit we were very confident. He moved forward with us immediately with a plan, from day 1. If we experienced any set backs or curveballs, he wasted no time in coming up with a new plan and putting it into place immediately. There was never a time where we went home disappointed in the care we received. We may have been disappointed in outcomes along the way, but those were outcomes that were not in anyway in his control. They were outcomes left to nature and my body that did not cooperate with our plan. Dr Wilshire was always available for questions. He never turned us away or referred us to ask his nurses questions. He prepared for the truth. Nothing will be sugar coated. He is honest in what baseline factors need to be present to conceive. Whether it's losing weight, or gaining weight, or picking up exercise and getting healthier, or cutting back on exercise and letting your body heal. He knows what he is doing. Trust it!

The nurses or Dr Wilshire himself were available to us 24/7. During treatments, if we left appointments and thought of questions later, we could call at any time and have any question answered. When we had breakdowns and cried, Dr Wilshire himself lingered in the room for comfort. When it came to finding a high risk doctor for our successful IVF twin pregnancy, they did all the work in getting an appointment set for us. Even after hours, even once we were past their care, they still answered pages in the middle of the night and put to rest any concerns we had.
Describe the protocols Gil Wilshire used in your cycles at Missouri Fertility and their degree of success.
We first tried clomid, but thanks to my pituitary issues, my body did not respond to this at all. So we immediately moved go injectables. It was a fast change of plans, and scary. We didn't expect that so quick. But Dr Wilshire knew clomid was a waste of our time, so we moved on! IUI + follistim was next for us. Since I had plenty of eggs, and maturation/ ovulation was my issue, we started with half the typical dose. Being conservative worried me, but he was right, because even at a half dose I over-responded and the cycle was cancelled. The next cycle, they monitored me every other day instead of every few days, to titrate my dose as needed. With this, I even still was over stimulated and cancelled yet again. We gave it one more shot, and this time went so low and low that after two full weeks of injections, we had no lead egg(s) jumping out and they were in fact fizzling. So we cancelled and moved to IVF. Our protocol with this was going to still be conservative, and have both an agonist and antagonist approach. We chose this since I do get overstimulated easily. We did not want premature release of any follicles. So about half way into stims we added antagonists as well. We went: follistim, follistim+ menopur, menopur+ ganirelix, and trigger. That worked beautifully. Resulting in an abundance of eggs.
Describe your experience with your nurse at Missouri Fertility. (Assigned nurse: Marnee)
We worked primarily with Amanda at first. She was very quiet but kind and compassionate. She answered any questions we had and always made us feel comfortable. Kim did our IUI, and was a very fun, but professional nurse to work with. Marnee was our IVF nurse, who is very to the point and honest, but cares deeply for each patient. Marnee can come off as stern, especially when explaining risks, but we quickly realized this is just because she genuinely cares and worries about each patient. The relief on her face when she saw me after we suffered from OHSS (ovarian hyper stimulation syndrome) said it all. We had a rough patch, but one office visit we came in and I was feeling much better. She let out a huge sigh of relief and said she'd been worried sick every night for us. This made us feel even more comfortable in their care.
Describe your experience with Missouri Fertility.
The clinic is set in an older building, but don't judge a book by its cover. Their office is in fact very nice, clean, and comfortable. They recently received new ultrasound machines which are much improved. Their surgery prep room and surgery room and situated right next to the lab where embryos are kept. During transfer you can literally see your eggs being drawn up into the catheter by the embryologist. It is comforting to be able to see all the "behind the scenes" action, because these are our babies we are talking about!

Ultrasounds were roughly average, ~$100. IUI was average as well, aside from injectables, the procedure cost just a couple hundred. Ivf cost was reasonable and average. ~15K, including ICSI, not including genetic screening.

Dr. Wilshire agreed with our rationale and put me on birth control to start. Before the cycle started we did a mock transfer which showed I had a pseudo cyst which he removed right there. I like how proactive he is. We did two weeks of injections but I only produced 4 follicles. Since we were expecting to get 20 this was disappointing. Dr. Wilshire discussed cancelling the cycle so we didn't spend the big bucks for 4 eggs. We decided that was best and the cycle was converted to IUI. The IUI worked and we now have a beautiful 4 month old!

He [Dr Gil Wilshire] easily could try and sway our decisions to improve his numbers but he cared about us as people and our mental health through the process... All of the first 3 cycles were slightly different, working to find something that would work for us. After trying a double transfer that he had not done before and having success, the same protocol was used when we returned to conceive our second baby.
[Mid-Missouri Reproductive Medicine and Surgery was] professional, organized, clean, welcoming. You always feel so important and as if you are their only patient... [Nurse] Marnee is one of the greatest people I've met. If I ever had any question, she was always available. I had a surgery first to remove a Cyst from my left ovary. Then we started IUI rounds [with Dr. Wilshire] a few months later...is very through and will go over the procedures and expectations step by step. He is direct, but I appreciate that. He went over all my needs and always answered every question. He wants to help you achieve your goals and is very open to LGBT...I cannot say enough good things about him...
It is an older building, but it is very clean and very nice in the office [of Missouri Fertility]. I know they are moving soon--but to me it doesn't matter. The inside office and the great team is what matters. The baby pictures they have posted are so cute in there!!!...The care is full of love and kindness, you can tell they are as dedicated to your goals as you are...They [nurses] are very through and always follow up--even when our third party ultrasound didn't do their follow up, his nurses always did. Dr Wilshire explained everything in detail throughout the process. Even after our failed cycle, he took time out of his busy schedule to personally meet us and answer any questions we had about the procedure and moving forward.... We had a great experience with Dr Wilshire from the initial consultation through the egg retrieval and after. We felt he took the time to thoroughly explain the procedures. We felt Dr Wilshire was very honest with us about our chances.
Although at times there were delays in hearing back from the nursing staff [Mid-Missouri Reproductive Med] my overall experience was very good with that clinic. Both the doctor and the nurses took time to explain the procedures in the medications throughout the entire process.... The total costs related to our first cycle of IVF was a little over $22,000. This included the ultrasounds, the egg retrieval, ICSI, PGD testing, and the medications.

We started with Clomid [with Dr. Gil Wilshire] and it took me 3 weeks to respond to it. So the next cycle we started with Clomid and then switched to Follistim because of the reaction time of my body. The last cycle we did all Follistim and I still had a 3 week reaction time. I did develop follicles every time it just took weeks. All 3 IUIs were successful but ended in chemical pregnancies...We did 3 cycles of IUI. I have PCOS and no other known issues so this seemed like the most practical approach.
They [staff at Mid-Missouri Reproductive Medicine and Surgery] always knew exactly who I was...I think they should update the design and make it more comforting and inviting. He completed another laparoscopic surgery after a d/c to remove endo that had grown back and then completed a 7th IUI with that failed. Currently trying naturally due to finances.
