All in all, Dr. Duke is a caring, patient, and friendly doctor who treats me like an individual and human. I just wish our expectations weren't set quite so high and that I could meet with her more often...My last two cycles I did not have OHSS because Dr. Duke adjusted my medication, which I appreciated...I also felt like what Dr. Duke had explained would happen was not happening.
The [Nevada Fertility Institute] office is very nice (it looks like a fancy spa)...The receptionist knows my name now and always warmly greets me when I come in. The phlebotomist is phenomenal and by far the most friendly person in the office that I interact with regularly...The nurses are still not terribly friendly during my ultrasounds, but they are more cordial and usually greet me.
How was your experience with Cindy Duke at Nevada Fertility Institute?
Dr. Duke is an incredibly compassionate and skilled doctor. We've had two tele-medicine calls with her and she has performed 3 IUI's for us so far. Whenever I've met with her she has been patient and understanding as I ask question after question. The one thing that is a little hard about Dr. Duke is she is incredibly optimistic. After our first meeting and before we started IUI, I left our meeting feeling like we would 100% get pregnant from IUI. I know she didn't say it was that certain, but she was so confident with our numbers and age. There has also been some confusion about statistics and IUI. She told us our first IUI had a 20% chance of success and each consecutive IUI chances would increase by 20%. So, our second IUI would have a 40% chance and our 3rd 60% chance. I later understood that isn't exactly what she meant by those stats and learned in reality the chance is only 20% tops each time and doing multiple IUI's helps in those stats, but doesn't effect my body. I learned this clarification from a nurse in our first IUI cycle and it was deeply upsetting to me. Now that I know the stats, I was able to go in to our next IUI cycle with eyes wide open. All in all, Dr. Duke is a caring, patient, and friendly doctor who treats me like an individual and human. I just wish our expectations weren't set quite so high and that I could meet with her more often.
What's one piece of advice would you give a prospective patient of Cindy Duke at Nevada Fertility Institute?
Write down your questions and don't be afraid to ask them even if you think they are silly. Also, don't rely on Dr. Duke to know how much your treatment will cost. Definitely speak with the office manager about that. We were told our IUI was covered and didn't realize till midway through the cycle that coverage did not include medication, which cost us about $1,500 per cycle.
During treatment, were you treated like a number or a human with Cindy Duke at Nevada Fertility Institute?
I felt like a number our first IUI cycle. When I went in for my first appointment, I thought someone would explain in greater detail what would happen with the IUI and would be available to ask questions. But, I discovered those appointments are just for bloodwork and an ultrasound, not necessarily for asking questions. I was also frustrated because when they showed me to the room, no one instructed me about getting undressed (I later found out this was a mistake and they apologized) Basically, it took me a while to understand that they would call me after each appointment. That was incredibly frustrating at first and I felt like I was moving blindly through the process. I also felt like what Dr. Duke had explained would happen was not happening. For instance, she said my baseline ultrasound would be around day 3 and I wasn't scheduled until day 6 or so. I now understand that isn't a big deal, but no one explained that to me at the time. That all being said, things have vastly improved since then and I have completed 2 more IUI cycles with Dr. Duke's office. I've developed a relationship with the staff and feel a lot more brave when asking questions, which has helped. It has been a night and day experience compared to my first IUI cycle.
Describe the protocols Cindy Duke used in your cycles at Nevada Fertility Institute and their degree of success.
Dr. Duke has had me take supplements since before we started IUI: COQ10, myo-Inositol, and a prenatal vitamin. For the IUI's I took Clomid, follistim, and sometimes estrace. I also took progesterone after each IUI cycle. Lastly, I did an ovidril trigger shot. Each cycle I have had quite a lot of eggs and a risk of multiples as a result of these medication and my possible PCOS. My first cycle I had OHSS. My last two cycles I did not have OHSS because Dr. Duke adjusted my medication, which I appreciated.
Describe your experience with the nursing staff at Nevada Fertility Institute.
I have had a variety of experiences with nurses. During my first IUI cycle, I felt like the nurses were cold, impatient with my questions, and I struggled to understand their instructions and reasoning for my instructions. They also sometimes didn't greet or acknowledge me during my ultrasound appointments. However, things have greatly improved since my first cycle. The nurses are still not terribly friendly during my ultrasounds, but they are more cordial and usually greet me. Over the phone they are usually very friendly and I loved that a recent nurse called me and excitedly told me that it was time for my trigger shot. I loved having someone share that excitement for me.
Describe your experience with Nevada Fertility Institute.
The office is very nice (it looks like a fancy spa) and conveniently located right off the freeway. The receptionist knows my name now and always warmly greets me when I come in. The phlebotomist is phenomenal and by far the most friendly person in the office that I interact with regularly. She knows I need to use "baby" needles in my arm and is always so excited and optimistic about me having a baby. I recently told her I just celebrated my 4th anniversary and she told me we'll celebrate our 5th with our baby. She is seriously the best. And Dr. Duke really is a great doctor who cares about her patients and knows exactly what she is doing with my treatment. The nurses are also generally friendly and helpful. The office does have weaknesses. They usually take a while to upload my ultrasounds (sometimes days or weeks) which I don't appreciate as I am so eagerly following along in this process. Ultrasounds are usually totally silent and cold, I wish they would be more friendly and warm to me. There is one room that doesn't have a big screen set up so I can't see my ultrasound while they are doing it. I also wish they would just tell me my ultrasound results right away at the end of the appointment. Instead I have to look at the screen (when there is one to see) and try to guess the sizes of the follicles. Communication was also a struggle, especially at the beginning of the process. All that being said, if this third IUI doesn't work, I do plan on continuing with this office and doing IVF.

My IUI was covered by insurance. But, I had to pay for medications and co-pays out of pocket. This ended up being about $1,500 per cycle

I did IUI, not a transfer. She warned me thoroughly about the dangers of multiples and we had to specifically agree that we understood the risks before proceeding. We have spoken to her about IVF and she prefers transferring a single embryo, but is open to no more than two embryos.

Dr duke is super nice and she does explain things to you only if you ask. The telemedicine appts always seem rushed. I respond well to medicine so egg retrieval was pretty basic. However I did get OHSS from it. Frozen embryo transfers start out with letrozole then gonal, cetrotide, and trigger shot. endometrin suppositories are used instead of the shot

It might just be me but it seems though they make assumptions that things will work instead. I am young and I feel like everyone was really surprised when my FET did not work. They also told me that since I was young there wasn’t a need to test my embryos even with MFI.

The [NFI] clinic itself is clean and typically runs nearly on time, no drs office is ever on time. Their communication did suffer a bit but now seems to be getting better especially with the text option. They almost never answer the phone which is a bit difficult but you can typically get a response on the text line. I like that the clinic is a complete clinic so no outside centers for testing or transfers. The office is very comfortable and clean and they have refreshments and phone chargers which is nice.

I’ve seen Dr duke over the years for various rounds of treatment. I switched to her after negative experiences with other clinics. As she’s gotten busier you do see her personally less but when you do see her she is still caring and thorough

Almost all communication was through a nurse. And when we were hoping to speak with her shortly after a failed cycle to reassess plans, it was over a month long wait for a telehealth appointment.

Dr. Duke is a great doctor but does not have a great staff running things. I rarely spoke to Dr. Duke regarding the outcome of any of my ultrasounds and bloodwork. Trying to get a follow-up with her regarding a cancelled cycle was booked over a month out. When we were asked if we wanted to cancel our egg retrieval cycle due to possible poor response, I never received a call from her and was asked to make a decision within an hour. It was also never made clear that if the cycle was cancelled, I would be penalized over $8500 for 2 appointments and 2 blood draws.
Describe the protocols Cindy Duke used in your cycles at Nevada Fertility Institute and their degree of success.
Cycle 1 was IUI with clomid. I did not respond well to the lowest dose of clomid and was on it for a second week at triple the dose. It resulted in two 18-20mm follicles. The cycle did not end in pregnancy. Cycle 2 was another IUI but with letrozole. I did not respond to the letrozole at all. We added in 75iu of follistim and 37.5units of menopur. I ended up with 2 15mm follicles that I ovulated prior to being able to complete an IUI. The cycle did not end in pregnancy. Egg Retrieval Cycle: 2 weeks of estrogen priming, doxycycline, Gonal-f 400IU, Menopur 150IU. At my second ultrasound I only had 5 small follicles and was given the choice to convert to an IUI, cancel the cycle, or continue. I was called at 7pm and asked to make a decision within the hour without speaking to Dr. Duke. I was concerned about using birth control to prime/time a cycle and so estrogen was suggested instead.

My nurse and the rest of the nursing staff seemed like they had too many patients to give their undivided attention to. And that while questions were understandable, it was an inconvenience to answer them in a timely manner.
Describe your experience with Nevada Fertility Institute.
Dr. Duke is a great doctor but she is not board certified in the subspecialty of reproductive endocrinology and infertility and her office staff treats patients as a number and not a human. The VP of NFI was more concerned about charging us over $8500 for a cancelled cycle that only involved 2 appointments and 2 blood draws than she was about putting a couple already dealing with a difficult situation through a more painful and financially difficult situation. While I am sure that plenty of other couples had good results with Dr. Duke, we did not and I was incredibly grateful to finally be done with her clinic once we moved forward with another clinic.

Monitoring for the most part was quick and simple. My first appointment I waited for over an hour just for blood work. I was assured that was not normal.

Dr. Duke preferred single embryo transfers. I lost twins at 24 weeks so this was fine with me.

Our egg retrieval cycle was cancelled due to poor response. What was not conveyed to us was that we would be charged the quoted refund amount for a cycle that was cancelled due to circumstances out of our control. As mentioned previously, I was called at 7pm with the results of my second ultrasound and asked if we wanted to continue the cycle, cancel it, or convert it to an IUI. When I asked for clarification on what we would be charged if we cancelled the cycle, my nurse stated she was "pretty sure" we would not be charged the amount quoted in our contract because it was cancelled due to poor response, not because it didn't work with our schedule. When I was called by the clinic manager to explain how much money we had to put towards another egg retrieval, I was told that we had a little over 8k out of 15k. Which meant that our ONE cancelled cycle that involved two ultrasounds and two blood draws would cost us over 7k. It took over 2 months and several calls to the VP of NFI to get this cleared and in the end we were refunded $10,500 out of 15k. Which means we paid over $4500 for a cancelled cycle (not including meds). Throughout this entire process, we were never contacted once by Dr. Duke or the nursing staff.

Dr. Duke is incredibly knowledgeable and took all of our history into account without relying solely on it to decide her course of action. She did the testing she needed to make the best decision she could for us, which resulted in finally getting a positive pregnancy test...You won’t see Dr. every time you go in the office, but you don’t need to...Due to my age and the number of IVFs and failed embryo transfers we suffered before coming to Dr. Duke, we did three mini IVFs

They had my questions answered almost immediately...They’re 100% in-house, which was nice because when I woke up from anesthesia, it was helpful to see my partner there rather than be in a separate hospital recovery room...Briana and all of the nurses that I interacted with were attentive, patient, and kind. They were available and very responsive to me.

Fertility treatments are hard, but Dr. Duke is amazing at what she does. You won’t see Dr. every time you go in the office, but you don’t need to. You’ll be well taken care of.

Everyone in the office was kind and considerate. Whenever we were scheduled to see Dr. Duke, she took her time with us and made sure that all of our questions were answered before she left.

Due to my age and the number of IVFs and failed embryo transfers we suffered before coming to Dr. Duke, we did three mini IVFs with Dr. Duke. We didn’t get a lot of eggs, but they were good enough to get three embryos to pass genetic testing. After 4 previous IVFs with another doctor and 4 failed embryo transfers that lost 7 embryos, Dr. Duke finally got one to take and we are forever grateful.

The clinic added a texting capability that made communicating with them about questions and concerns SO much easier. They had my questions answered almost immediately. The phlebotomist, Marika, was always amazing. I have a hard time with the needles due to medical trauma and she was always kind, patient, and comforting. If someone can’t help you with your progesterone shots, the nurses there will help during business hours. They’re 100% in-house, which was nice because when I woke up from anesthesia, it was helpful to see my partner there rather than be in a separate hospital recovery room.

Dr Duke is a compassionate doctor, very easy to talk to and makes me feel at ease it a very stressful time in my life. I felt that she would be the doctor that would ultimately help me get pregnant. It’s a work in progress as we came close at one point during my 4th cycle but ultimately led to a chemical pregnancy

Dr Duke had prescribed estradiol medication along with clomid and hcg trigger shots to help my ovulation process. I only have 3-4 eggs to work with and these medications helped my eggs to mature . I also took crinone twice daily to help prep my uterus for pregnancy.

Unfortunately Dr Dukes are not on the same professional level as she is and are a bit unorganized and noncommunitive. It’s hard at times to get answers from her staff but fortunately but Dr Duke is accessible to me when I need to speak to her directly which is very helpful

I was given clomed as was my wife. My sperm was tested and though apparently low in count they were of good quality. My wife was also tested for various conditions prior to treatment. All of these were very expensive.

Nursing staff was fine. However again, when we tried to assess our chances for a successful pregnancy, that was very difficult to get out of anyone. They were honest to the point of telling us that IVF would likely take 2 cycles and that there is a measured approach to the procedure so that we didn't spend if we weren't going to have a chance at success, I later learned that in our case, it would likely take 3-5 IVF cycles due to age. I discovered this by watching the FertilityIQ videos, and had we been given this honest evaluation by the Dr., it would have saved my wife and I some very difficult conversations.

The next cycle Dr Duke changed some things up...trigger injection site, days till IUI etc and no pregnancy We are going to do our 3rd cycle soon

I did have questions from time to time that weren't answered before leaving and that required me to call and ask for clarification. My medication was switched with no explanation from the doctor and the communication for this came from my nurse, this was the only hiccup I ran into and it was easily fixed and discussed just after the fact. Dr. Duke was always optimistic and kind when working with my husband and I. She takes time to speak to you about concerns and risks and she speaks to you like you are knowledgeable and allows you to ask questions without being frustrated in you asking.

Just ask questions. If you are unsure about what is happening or why, ask. You can discuss multiple options with her and she will give you her honest opinion. Don't be afraid to discuss treatment options she is open to many and very knowledgeable.

I started with letrozole 7.5 mg CD 3-7 and started estrace 2mg twice a day starting on CD 9. We were initially prescribed a different trigger but it changed the night of the trigger to preglyn 10,000. Endometrin 100mg twice a day starts two days after IUI

I interacted with a few different nurses. They were extremely kind and compassionate when speaking with me. They were easy to communicate with but from time to time difficult to get a hold of. My husband then started taking HCG shots with clomid and blood work went well. We have been on it for two months (hcg) and hope to be pregnant in a couple months. Still no positive pregnancy tests, but my husbands numbers have improved...Bedside manner is great, she is so nice and left me feeling hopeful! I wish she would call between visits. When we would go into the office it was always a great visit, but when we leave and post-visits, I always felt lost in steps to take next.

I am needing to do the SIS testing and the go from there...I asked how many she would implant and she said 2 would be the maximum for my age...The minute I stepped foot into Dr Dukes office, I was a human! My first experience wasn’t pleasant. I was at the very beginning stage of our infertility journey. So I picked the clinic closest to our house. Every time I went I felt like it was just a business and I was a paying customer. The minute I stepped foot into Dr Dukes office, I was a human! Dr Duke spent the time to go over everything and answer our questions. Her staff were all great as well!

I was always addressed by my first name, and details of my medical history were remembered, never asked multiple times.
Describe the protocols Cindy Duke used in your cycles at Nevada Fertility Institute and their degree of success.
Egg Retrieval: We used Gonal, Menopur, and eventually Centrotide during stimulation. I was frequently monitored so that the timing would be accurate and optimal. I triggered with Lupron and Pregnyl. Due to my blood work and high number of follicles, adding Lupron to the trigger was to help avoid OHSS I believe. FET: Our first attempt at an FET was a fully medicated cycle. As I followed OHSS protocol after the retrieval, this was to avoid it again. I was frequently monitored. Unfortunately, my lining did not grow, and that cycle was cancelled. I used Lupron for supression, and then Estraidol tablets, patches, and injectable estradoil to stimulate lining growth. Our second attempt at an FET was a medicated natural cycle. I took letrozole for 5 days, then used Gonal to stimulate, grow follicles, and grow my lining. My lining did not reach 8 mm before my progesterone naturally spiked. Dr. Duke decided to cancel that cycle as well due to not being able to verify the timing. While this was not the desired outcome, we potentially and likely would have not had a successful cycle. Our third attempt was cancelled before we got started as my estrogen was too high and needed more time to drop below an optimal threshold. Our fourth FET was ultimately successful. We followed the same protocol, except we added in Centrotide earlier than previous cycles. My lining reached 8mm, we triggered, and transferred one embryo. Our BETA was positive after this FET.

The staff is amazing! They all take their jobs and role in helping you grow your family very seriously. I was assigned a nurse for each step of my procedures, and was able to grow a relationship during each step of the process. This allowed me to gain comfortability, and openness with the staff.
Describe your experience with Nevada Fertility Institute.
Fertility is one of the most intimate and emotional things I have gone through. I could not imagine going through it with any other practice than Nevada Fertility Institute. From the moment I booked an appointment, so our final appointment, we felt like we were in the company of family. Appointments were easy to make and schedule, and the knowledge of Dr. Duke and her staff is unmatched. Ultimately, we chose to pursue IVF to grow our family. Spending multiple days a week at the clinic, you want to be somewhere you feel safe and comfortable; this is the place. Despite having a lot of clients, they always remembered my name, who I was, and random facts that made me feel like I was visiting friends. Each time I would speak to my nurse at night, I was given the time I needed to feel comfortable, ask questions, and calm any nerves I might have had. At times, the staff went above and beyond my physical medical needs, and ensured my emotional health needs were meant as well. Going through all this during the COVID pandemic as well created its own unique set of challenges. Their office was always extremely diligent about following research based protocols, and holding all patients and staff to them. I never felt exposed, or anything when going to the clinic multiple times a week.

My husband and I first met with fertility specialists in 2015 in Colorado; during that time, every step we took was in the backwards direction. We continued to receive bad news and nothing was working for us so we had stopped trying. When we first met with Dr. Cindy Duke at Nevada Fertility Institute (NFI), we had very little confidence in my ability to get pregnant and stay pregnant. As a matter of fact, I had recently spoken with my GYN about an early hysterectomy so I could finally put the fertility matters behind me, but she referred us to NFI instead for further evaluation before she would consider the hysterectomy for me. At our first meeting with NFI we filled out paperwork, health history, wrote down our goals of being parents, had some blood drawn and then met directly with Dr. Duke to discuss our options available through her clinic. Personally, we knew that using an egg donor would be best for us because of our failed attempts with my eggs in the past. Once we made that decision, it was a brisk talk through the necessary testing which would identify for her any lingering issues within my body. My husband and I left the meeting with somewhat of a renewed hope in our hearts and minds, again thinking that we actually had a chance at being parents. After just a few weeks of private discussion in our house and within our family, we all decided to move forward as soon as possible (I'm mid-30s and we all know there are extra risks as an "older" mom-to-be.) The egg donor process moved quickly and efficiently; we were able to discuss a small set of parameters and come to a decision within about a week. Once the egg was received in the office, my husband provided his sample, and it was just a matter of a few days when we received the call with our embryo count. I couldn't believe it! We successfully had embryos to work with and a healthy body cleared for transfer. One of the best things to know about Dr. Duke and NFI is that they don't really leave anything up to chance. The science is down to an art form and once we were on the medication schedule and lined up for the trial round, it was up to us to simply stick to the routine.
