Shady Grove is a factory. Good results, but I only saw Dr. Osborn 3 times the whole time I was there...Multiple locations, convenient, runs like a well oiled machine, but also feels like an assembly line...I actually signed up for shared risk program where you pay 2.5 times the cost of one cycle, with money returned if there was no live birth after 6 cycles.

Soon as she [Dr. Sandy Li] realized my husband had siblings with sickle cell she and her nurse sent repeatedly asked me to reconsider undergoing genetic testing. I declined initially due to the timing of the test. I am not a carrier and am not opposed to genetic testing. Even with a signed decline form, they asked 3-4 times for me to reconsider. Dr. Li herself called and asked me to reconsider taking the testing and that later on, I might not want to keep embryos that have the trait for sickle cell anemia. These delays brought us from October to December. Then they told me they won't start a cycle in December due to the lab undergoing quality assurance. I never heard from her or the nurse again. I reached out and requested a different team in February.

Advocate for yourself [at Shady Grove Fertility]...They had me repeat initial workup tests and then stalled my treatment when I declined genetic testing...Initial workup was 2000 for the testing and repeated tests. Mostly covered by insurance. They quoted me 10000 for IVF...A strength is that they have multiple locations in the DMV area. There are many doctors. Ideally, if one doctor is not a good fit you may switch to another doctor. Weaknesses are that they push hard for eSET, they treat you like a number, be prepared to ask them to explain their protocol.

How was your experience with Sandy Li at Shady Grove Fertility?

Strengths are that she makes herself available to her patients. Weaknesses is that she doesn't review charts appropriately and has patients repeat the same tests multiple times. She also allows her personal bias to cross ethical boundaries. She essentially delayed my treatment because I refused to approve genetic testing. I am African-American they screen for sickle cell anemia at birth. She heard my husband say he has a history of it on his side of the family and took it upon herself to tell me she wouldn't select an embryo with sickle cell trait or sickle cell anemia for transfer. This is beyond medical advice and crossed into telling a client what they should and shouldn't do with their own embryos. At this point imaginary embryos that have not possibility of developing sickle cell anemia. I essentially was discarded to the side because I didn't sign a consent form for genetic testing.

Advocate for yourself.

Dr. Widra is not what most people would call "warm" but he is very knowledgeable and straightforward, traits that are more important to me in a fertility doctor. He's very good at explaining options and addressed my concerns without me feeling like I had been dismissed.

Not a major error, but the care coordinator missed the window for a diagnostic test.... Shady Grove has some great aspects: they do a lot of volume, so I think their lab is very good, and they have a lot of locations which is very convenient, especially for me as I live close to one lab but work closer to another. Downside is that it can feel somewhat impersonal and like you're not getting a lot of individualized attention.

they seem to be a well oiled machine of processing patients. due to high volume of patients this probably translates to good treatment based on their experience. they behave in a very profit-driven manner, where time is of the essence for them to move a new patient forward quickly through the process for their monetary gains rather than an individual's window of fertility. A lot of enthusiastic interaction from clinic in moving me forward with consult and beyond but little to no outreach in response to my confusion about the financial parts and the details of that.

I came to Dr. Csokmay because of my age (over 35), a past miscarriage, and the length of time it was taking to try and get pregnant again after that miscarriage. From the beginning he was attuned to my needs and desires, and was very clear about next steps and how it could escalate if need be. Luckily we didn't need to escalate much further than the initial diagnosis, since we got pregnant after 2 IUI cycles!

The [Shady Grove Fertility] clinic is centrally located in DC - easy to get to. Early morning appointments are good because they stay on time. He took the time to get to know me, my spouse, and our concerns. He was always open to questions, extending our time...anything to make us feel more comfortable.

We did two cycles of medicated IUI. When the first IUI cycle didn't work, we talked about what would happen/change if the second IUI cycle also didn't work. We were going to do another test, to make sure nothing was missed in the pre-testing phase, to suggest we should go about a different course of action. Luckily, we never got to that point since I was pregnant with the second IUI cycle!

I didn’t feel the personalized experience that I had expected....Dr Garde put me on very high dosages of medication due to quote, my age. I took 450 iu Gonal F, 150 iu Menopur, Cetrotide, and 10000 HCG. I hyper stimulated my first retrieval and was in extreme pain for a month. The next cycle she change my trigger to Lipton and my ovulation was delayed so she switched me back to HCH for my third cycle.

I would go in to the office [Shady Grove] twice a week sometimes three and I never got a warm and fuzzy from the staff. I had to contact a supervisor on one of the nurses because she was being very insensitive. I also had a hard time dealing with the fact that they allowed children to attend appointments.... We decided on an IUI cycle because previous unmonitored clomid and letrozole cycles resulted in no pregnancy. The protocol started with letrozole 5mg for 5 days, followed by ultrasounds every 3 days. This disagree had no effect on follicle growth so she added Gonal-F 75IU injected once a day for 4 days followed by another ultrasound. There was still no follicle growth so the dose was increased to 150IU and continued for another 3 days. The next ultrasound also showed no change so Dr Garde then informed us that in order to increase the dose any more would need to switch to an IVF cycle in case of OHS and maturing too many follicles. We could not afford to switch to IVF so we cancelled the cycle without completing the IUI.

Ashley was so compassionate and understanding. It was like having a good friend walk you through everything. She celebrated with us and hugged me when I cried. Definitely the kind of nurse I wanted in that situation.

Dr Garde is my primary RE. She may not perform every IUI or Egg retrieval, but she gets the information and makes the decisions and either she or her immediate staff contact me...She is very thorough and takes the time to explain the options and processes involved...She answers my questions in a down-to-earth manner and makes sure we understand what's going on and why.

Strengths [at Shady Grove Fertility] Multiple locations with varying hours. Open every day. 24 hr nurse line. They don't allow children, so that makes it more comfortable. Weaknesses On weekends I go to a different location, so it's not as personal as my primary location...Multi-cycle IVF pkg with ICSI and Assisted Hatching = $21,500. Does not include meds or PGS testing.

She answers my questions in a down-to-earth manner and makes sure we understand what's going on and why.

Describe the protocols Rachana Garde used in your cycles at Shady Grove Fertility and their degree of success.

First 2 IUI's were with clomid. No pregnancies. Next 4 IUI's were with FSH and trigger shots. 1 pregnancy ended in miscarriage due to genetic problem. ER-rationale- age factor, need to move on from IUI. 2 egg retrievals. Both using FSH. 1st-2 embryos that were PGS tested as abnormal. 2nd-3 embryos have been sent for PGS testing. Awaiting results.

Nurse is very helpful and responsive. The rest of the office is as well.

She [Dr Garde] always took plenty of time when me. I never felt like I was rushed. She answered every question I had and I never felt like a question was ignored....We traveled from another state because having a singleton was very important to us. Dr. Garde understood that. We did two cycles with her and she always adjusted the protocols as needed along the way.

We traveled from out of state because we wanted to be at a clinic that helped people achieve singleton pregnancies. We felt that other clinics downplayed the risk of carrying twins (not just higher-order multiples). They [Shady Grove] were very used to working with out-of-state patients and we appreciated that.

Every time we called Mary, she picked up the phone herself. We never had to leave messages with voicemail or with a receptionist. We had ONE nurse and she was easily accessible. That meant a lot to us.

We traveled from out of state because we wanted to be at a clinic that helped people achieve singleton pregnancies. We felt that other clinics downplayed the risk of carrying twins (not just higher-order multiples). They were very used to working with out-of-state patients and we appreciated that.

Describe the costs associated with your care under Rachana Garde at Shady Grove Fertility.

Another reason we chose this clinic is because they had one of the most generous shared help plans in the country. We did misunderstand the plan (our fault) and did not use it the first cycle (although we had an income-based discount). If we had had a full understanding of the program, we would have ended up saving ourselves the cost of that one cycle.
