She [Dr. Schnell] made false promises about my transfer dates (by whole months). My benefits reset on January 1. I had met my out of pocket maximum by August. She promised me to be done by the new year. They [Shady Grove Fertility] didn't update my chart with just about anything so that I had to remind them about everything...The only strength in this clinic is a nice phlebotomist. My surgery was a bad experience. My thyroid treatment went bad. How was your experience with Vicki Schnell at Shady Grove Fertility?
She gives me misinformation regularly. Then blames others. She refused to believe the thyroid meds she gave me made me sick. Three doctors and a pharmacist disagreed. It took her 3 weeks to be convinced to see me about it. She made false promises about my transfer dates (by whole months). My benefits reset on January 1. I had met my out of pocket maximum by August. She promised me to be done by the new year. It took me trying to move my embryos to another facility to get her to reach out to me about doing the transfer during the timeline that was about to run out. I made my transfer with 5 days to spare only because I requested to leave that clinic for good.
What's one piece of advice would you give a prospective patient of Vicki Schnell at Shady Grove Fertility?
Don't go there. You'll be put at risk.
During treatment, were you treated like a number or a human with Vicki Schnell at Shady Grove Fertility?
They didn't remember anything going on with me. I started my period even on progesterone, no one realized I was a week early. They forgot to call a rx in for me and then had no idea how long I'd been on the new dose. They forgot which trigger shot they told me to take.
Describe the protocols Vicki Schnell used in your cycles at Shady Grove Fertility and their degree of success.
She put me on estrogen priming before my stims. My period started a week early anyway. They said it was OK but not why. Then they started me on a very high dose of gonal f. They refused to bring me in for labs early even though I felt so dizzy I couldn't work. I finally convinced them and they lowered my dose because my estrogen was rising too fast. I was told I'd be triggering with lupron to avoid ohss. Then the nurse called me to tell me to trigger with ovidrel. She was clueless about the lupron and said it wasn't in my chart. Told me to trigger with the lupron and that she'd take my word for it. Then, after a different doctor did my egg retrieval after all of doctor Schnell's staff said I wasn't at risk of ohss, the retrieval doctor put me on cabergoline and told me they, "pushed me very hard." One follicle was larger 1.5x the size of my actual ovary. I got 10 eggs. 7 mature. 4 made it to day 5. 3/4 were pgt normal. 1/4 was low level mosaic.

Messed up 10 prescriptions. Argued with me that cvs carried a drug. (Like I'd make up leaving CVS after being told, "we don't carry that medicine here.") Medical assistant out stepping her limitations and trying to deny me being monitored when I was ill. Saying, "the doctor isn't in office today so we wouldn't adjust your meds unless it's an emergency anyway." After they lost that battle, I had to have my meds adjusted because it was actually an emergency. They almost never knew what they were doing.

The only strength in this clinic is a nice phlebotomist. My surgery was a bad experience. My thyroid treatment went bad. My ivf made me sick and scared for my life with my doctor and her team not bringing me in for enough monitoring. They didn't update my chart with just about anything so that I had to remind them about everything. I think a person who isn't micromanaging their own care at this clinic is in actual danger.

I'm lucky in that I had fertility benefits. Once my out of pocket was met, everything was covered completely. No idea how much it would have been without it.

10 prescription errors. Failed to call in 3 times. Called in to random pharmacies twice. Called in but canceled for no reason. Gave the wrong instructions on a suppository. Made sick by reaction to levothyroxine and refused to acknowledge or test for over 3 weeks. Made ill during injections and was refused early monitoring. Was told by doctor to trigger with Lupron. Then was told by nurse to trigger with hcg. Nurse couldn't find lupron in my chart but told me to use it. Was told by doctor my transfer would happen in December. Was told by medical assistant that my transfer was impossible to occur before February. Appointments getting canceled and unable to be rescheduled. Was told I'd be off my suppositories by week 10 of pregnancy. Won't even start weaning until week 11. $500 extra dollars in medicine and the whole reason is because the clinic got their appointments backed up. I have no medical reason to stay on them and they have told me they aren't even checking my blood to ensure I'm safe to come off them. I tried switching to a different clinic in November and they never transferred my files. I also am going to my OB now and have requested my files over 4 weeks ago with no follow up. I've asked for work documents, been charged for them, never refunded but they never got filled out. They severely mistreated my thyroid and my endocrinologist told me that I was, "textbook failing" the medication. I specifically asked Dr. Schnell if we could put me on something other than levothyroxine. She said it wouldn't make a difference. 3 doctors and a pharmacist disagreed. I lost 19 pounds and missed almost a week of work.

I would describe Dr. Csokmay as an angel of a provider as I could not have dreamed or created a better doctor myself...I had 100% faith and trust in him to keep me safe, healthy and cared for during my fertility treatments...Not once was I not considered in my protocols, procedures, or decision making. Dr. Csokmay ensures your comfortability and treatment with the utmost care. I have never felt more respected or cared about than under the care of Dr. Csokmay.
Shady Grove Washington DC- K street location: The receptionist are incredibly kind and there to literally cheer you on throughout your entire time at SGT. The phlebotomists are by far my favorite as they always got my draw right on the first stick and create a comfortable and fun environment with music throughout your time there. The ultrasound techs are always light hearted and make sure you are comfortable and at ease during every visit. The only weaknesses of K-street is the PARKING and BILLING OFFICE.
How was your experience with John Csokmay at Shady Grove Fertility?
To be completely transparent I only attended Shady Grove Fertility to be under Dr. Csokmay's care. I knew he was the doctor I needed to go through such difficult surgeries and procedures. I had 100% faith and trust in him to keep me safe, healthy and cared for during my fertility treatments. I honestly would have followed his care to any clinic as that is gravity of care Dr. Csokmay provides. If I had the opportunity to have Dr. Csokmay be a provider in my overall health care I absolutely would. I could speak praises until I ran out of breath and would still have an infinite number of positive praises for him.
What's one piece of advice would you give a prospective patient of John Csokmay at Shady Grove Fertility?
Talk to him, know your personality and comfortability as that always comes first especially when it comes to fertility treatments. I would describe Dr. Csokmay as an angel of a provider as I could not have dreamed or created a better doctor myself. I would say run, if your going to Shady Grove, he is your doctor. If your looking through clinics, look up Dr. John Csokmay, there is absolutely no better!
During treatment, were you treated like a number or a human with John Csokmay at Shady Grove Fertility?
I followed Dr. Csokmay out of military care to Shady Grove specifically because he treated me as a human. Not once was I not considered in my protocols, procedures, or decision making. Dr. Csokmay ensures your comfortability and treatment with the utmost care. I have never felt more respected or cared about than under the care of Dr. Csokmay.
Describe the protocols John Csokmay used in your cycles at Shady Grove Fertility and their degree of success.
For my egg retrieval we used the antagonist protocol, which included Menopur and Gonal- F with a lupron trigger. Our retrieval resulted in 23 eggs retrieved after 11 days of stims and 6 embryos created. My FET was catered to my body's response to birth control, I had 14 days of Estrace prior to transfer and about a week of Progesterone prior to transfer.
Describe your experience with your nurse at Shady Grove Fertility. (Assigned nurse: Jamilla, Sadie, Madison)
I had 3 different nurses as I moved through the process. Jamilla was my nurse throughout testing and gearing up for egg retrieval. Sadie was my IVF nurse and was my point of contact through my FET. Madison was my final nurse after completing my transfer and through my graduation from the clinic.

The ultrasound techs are always light hearted and make sure you are comfortable and at ease during every visit. The only weaknesses of K-street is the PARKING and BILLING OFFICE. Parking is especially difficult and confusing in D.C. no control of the clinic but be prepared. BILLING however, I received multiple bills of other patients that were not mine and did not really get fixed due to similar emails. In addition my husband a retired navy vet and I qualified for a Bob Woodruff foundation grant for a percentage off of our total clinic cost. I reached out numerous times for the paper required to apply for the grant, spoke with the foundation and due to the billing office not providing the SART certification, we went into large credit card debt to afford IVF. All which could have be alleviated with the assistance of the billing office. Once I paid SGF there was no care to assist or communicare from the billing office at all.

The only issues I had with SGF arose from the Billing office. There is lack of communication and ball dropping when it comes to the financials. We were to have received a Bob Woodruff grant and could not due to the financial office not providing the clinics SART certification. This gave us a large increase in credit card debt which could have been alleviated with paperwork. In addition, they contacted me when a bill was due but would not respond to inquiries. Lastly, as a tricare patient there is a waiver I am to sign prior to paying or receiving treatment that acknowledges lack of coverage. This form was not provided until after care had been provided and payment submitted and was a cover their butt moment as it stated without my knowledge of said form payment could not be collected.

Dr. Bromer was always great to talk to. He spoke matter of factly, but was always warm and compassionate. He always took the time to answer my questions in depth and with thought. Anytime I needed more information or wanted an additional test, he would take the time to explain and/or do the testing for my reassurance...If I had a question, he would thoughtfully answer it, and let me pursue additional testing to appease me.
The [Shady Grove Fertility] billing department left something to be desired. Sometimes they took a few days to get back to you. We had insurance but I wish they would have provided a better idea of the costs prior to treatment. Dr. Bromer was always great to talk to. He spoke matter of factly, but was always warm and compassionate. He always took the time to answer my questions in depth and with thought. Anytime I needed more information or wanted an additional test, he would take the time to explain and/or do the testing for my reassurance.
Describe the protocols Jason Bromer used in your cycles at Shady Grove Fertility and their degree of success.
I was started on a normal dose of a standard antagonist protocol due to my unexplained infertility. I typically had an AFC of 15 but this cycle they could only confirm 8. 150IU Gonal-F and 50IU of Menopur. I had monitoring appointments often. I did have lots of follicular growth, but it was slow. Dr. Bromer increased my dosage gradually over the next few days until I was up to 400IU Gonal-F and 150IU Menopur. After 14 days of stims, I was able to trigger with HcG, 10,000IU. I ended up with 23 follicles, 20 eggs retrieved, 17 mature, 12 made it past 5 day, ended up with 8 blastocysts with the remaining 4 that stopped developing. We ended up with 4 PGT-A tested euploid embryos, 1 low-level mosaic (which they will allow me to transfer), 2 high-level mosaic, and 1 abnormal

The nursing staff was good. They always called on time and provided the results in a compassionate and informative way. They were good about replying back in the portal. The clinical team in the office were separate from the nursing staff. I loved the clinical team, they were always so nice and happy to see you. You could tell they truly wanted the best for you. They would always describe what they were seeing so you knew what was going on before you left.

The billing department left something to be desired. Sometimes they took a few days to get back to you. We had insurance but I wish they would have provided a better idea of the costs prior to treatment. Going through fertility treatments is hard and I was hoping we would have our first baby on the first try. We did get pregnant from our IUI but miscarried. Dr. Malik advised me on doing IVF FETs as I was already 38 when I first met her. We did 2 rounds of embryo preservation and I am so grateful for that advise. We have a 2.5 year old toddler and are currently pregnant with twins from our FET procedures. I am grateful for having all my options explained to me so we could make the best decision for our growing family.
What's one piece of advice would you give a prospective patient of Shruti Malik at Shady Grove Fertility?
Just have patience, it takes a while to understand the long process of trying to get to the point of being pregnant. Also, don't give up hope. It's a numbers game and you have to keep trying until your successful with a baby.

We did two cycles of medicated IUI. When the first IUI cycle didn't work, we talked about what would happen/change if the second IUI cycle also didn't work. We were going to do another test, to make sure nothing was missed in the pre-testing phase, to suggest we should go about a different course of action. Luckily, we never got to that point since I was pregnant with the second IUI cycle!

During treatment, were you treated like a number or a human with Eric Levens at Shady Grove Fertility?
Dr. Levens was always quick to reach out when something went awry or to be available when I needed to talk through my many questions. His nursing team is exceptionally responsive and knowledgable, but he would also give us a call anytime something unexpected happened, including once just to check in on a Saturday after a disappointing retrieval. I'm sure that he is like this with all of his patients, but we felt like he really wanted us to be successful and made sure we knew he was there for us.
Describe the protocols Eric Levens used in your cycles at Shady Grove Fertility and their degree of success.
We began with an estrogen priming protocol which produced four embryos, the first of which became our son. About a year after he was born, we proceeded with three frozen embryo transfers, none of which were successful. We later discovered a hydrosalpinx, which may or may not have been impacting those transfers. Two subsequent retrievals -- one estrogen priming and one microdose lupron only yielded one viable embryo. After surgically addressing the blocked tube, I got pregnant via a frozen embryo transfer of that final embryo.

The nursing team is incredible! You honestly get so spoiled with the level of care and prompt communication here. Believe it or not, I still can remember the details. After that I was treated under her close monitor. She always personally reviewed every one of my exam/test results. Actually in many fertility centers different monitor doctors may review your results and provide the doctor's order, which may make the patient a little worried. But Dr. O'Brien always rechecks everything. This made me feel that she was always with me. She also provided me with a lot of valuable advice. What she cares about is what you need, but not how much money the center will earn. Although I am super cautious about a lot of things, she never recommended the test which was not necessary. She used her knowledge to convince me. And she was right. She has the top skills in operations. I really trust her that I'd rather wait weeks to fit in her schedule, and let her do the transfer for me. I am so lucky that I met her at the beginning of my fertility journey. Here I also want to thank her team. Dr. O'Brien has a very strong team. All team members are kind and full of patience. For my initial consultation, Dr. Amrane (and nurses) really stressed me out and it definitely felt like I was more of a number. I had a blood pressure spike and no one really reassured me about it - and it was only a consultation! I don't think there was enough explanation/caring about just how complex, time-sensitive and demanding the process really is... For my virtual meetings with Dr. Amrane, she was ok/relatively nice, but her demeanor was still sort of "stress-inducing."

I did four IUIs; the first was a natural cycle, because I was hoping to avoid chemicals, etc. She was very willing to try different things (i.e., a natural cycle) and said it's worth a shot. When the first 2 IUIs were unsuccessful, she said I should consider moving on to IVF. She did note my lower ovarian reserve, and I think that was a big reason for her suggestion. I used clomid for one cycle (which didn't end up going all the way to an IUI, because I got sick), and letrozole for another cycle. I was switched to letrozole because I may have had visual side effects from clomid, so they wanted to be on the safe side. For the IVF, I had menopur and gonal-f, and ultimately cetrotide; also, Dr. Amrane had me do a final boost of those meds before the trigger shot. I wasn't told the rationale for that... but currently I'm waiting to see how many of my fertilized eggs turn to blastocysts. I had 3 eggs fertilize.

Beth Goode was awesome! One of the best health care providers I've ever had. She is always very organized, knowledgeable, calm, professional, understanding, takes LOTS of time to answer any questions I have... She was good at reassuring me when I worried about things and good at giving a very thorough perspective on everything. Most of the nursing staff was also very good.

So I haven't been happy with the financial situation for my IVF, so far... Overall, the clinic is very organized, staff is very nice and professional. Yes, you will probably "feel like a number," but it's Shady Grove and it's a busy clinic... If you make peace with feeling like a number, it's not that bad. For a new patient, however, it is honestly an extremely stressful experience, because they don't seem to take into account the level of complexity and anxiety that the whole fertility situation creates. I was told during my consultation that my blood pressure was high enough that I could go to the ER - friggin' ridiculous! So, I understand that the nursing staff are primarily gynecologists, not cardiologists, etc, but hopefully new patients can take a lot of their comments and suggestions with a grain of salt, because outside of the fertility stuff, they don't necessarily know what they're talking about. Shady Grove is clearly *extremely* focused on avoiding any type of liability, so they warn you about every possible little thing, presumably so they don't get sued. So I just wouldn't listen to all the risks and everything like that that they tell you.

I think IUI was approximately $1,500 a cycle. (I paid out of pocket at the time.) Ovidrel was $68. My IVF charges are still being processed, but... I was so far billed about $6,500 by Shady Grove (which includes about $3,600 for PGT testing). This does NOT include an embryo transfer - but allegedly does include the cost covered by insurance. I have also been charged, at this point, roughly $6,000 just for medications. But everything is still going through insurance, so I'm not sure how this will all shake out.

Like I said earlier, I really wasn't given a comprehensive overview of the financial picture for IVF treatment. I wasn't told that if I did PGT testing, I couldn't do a fresh embryo transfer, which would add like at least $6,000 to the treatment (I thought embryo transfers cost closer to $3,400). No one also told me how much IVF meds cost, like even a ballpark estimate. I realize I could have done more of my own research ahead of time, but the way the clinic downplayed the cost of the meds, etc, I just wasn't that focused on it. So that's the main thing I wasn't happy with.

I have a good egg reserve for my age and wanted to try with my own eggs and not be pushed into donor when I had other options. So I felt that my care was not as aggressive as could have been because they just didn’t think it would work...Demand more, do your research and ask tons of questions. They [Shady Grove Fertility] have a age cut off, a BMI cut off and the financial team is very bad at getting back to you in a timely fashion. They staff you worked with most were the ones that took your blood, and the ones that did the ultrasounds. They were very kind, compassionate and friendly...the weekend nursing staff were a bit more compassionate than the weekday staff. But overall not a bad experience

While I am sure she is a great Dr, she was very clear about the expectations and my doing donor eggs. I have a good egg reserve for my age and wanted to try with my own eggs and not be pushed into donor when I had other options. So I felt that my care was not as aggressive as could have been because they just didn’t think it would work. After the 3 failed IUI I reached out about a change to my regimen because clearly what I was on was not working. They said no it would stay the same and I was responding well. Well after 2 more failed attempts Dr. chasen now wants to talk about changing the regimen which I felt was a little late. I aged out of IVF at that particular clinic but never got a change to do a successful retrieval so I moved on to a clinic that didn’t have a age cut off of 45 for your own eggs. I didn’t even bother with the last IUI….more money wasted with no measurable change to the regimen

Demand more, do your research and ask tons of questions. Don’t assume because they are the Dr they know more. You have to advocate for yourself and what you would like done

I barely spoke to Dr. Chasen more so her nurse. Overall while they were nice I didn’t feel that they cared as much as I would have liked during this emotionally stressful period. They kept pushing me to do donor egg although my egg reserve was good. I do feel that my failed egg retrieval could have also been avoided if certain things would have been noted before hand.

For the IVF…I was on BC pills initially, then Lupron, added in 150 menopour and 275 gonal f. They started high because of my age and I reacted better than they thought so they had to lower the gonal f to 150 so I would not go into ohss. I went into retrieval with them seeing at least 16 follicles mature and ready. I woke up saying they were unable to retrieve any because they could not access my ovaries vaginally and abdominally they saw a blood vessel in front of the ovary and decided to not proceed. It was devastating They then told me that they would not move on with IVF for that reason….no trying to figure out how we can go around that because I clearly responded well. They then moved to IUI which was 100mg clomid for 5 days and on day 3 150 gonal f and day 5 150 gonal f. When I went in each time they saw 2-3 follicles that were mature and ready for insemenation. The sperm was great according to them, but Nothing took.

They have an efficient process which is nice, but it is impersonal and the wait time can be really bad (over 45 mins on several occasions). Ask questions- he’s great at explaining things and giving answers, but he doesn’t necessarily talk a lot about things unless you ask for specifics. The nurse gives more of the details.

Although the clinic over-all makes me feel like a number, whenever I’ve met or spoken directly with Dr. Doyle, he is very kind, affable, and never seemed rushed or dismissive. He’s matter of fact but still willing to explain things to me and answer my questions in a detailed way when I want/need him to. One time he even called me back after hours to talk to me about my misgivings from recommendations by the weekend doctor and nurse.

Ovidrel + IUI + monitoring of bloodwork & ultrasound x1 cycle resulted in 1 mature follicle, no pregnancy 5 pills of 2.5 letrozole, ovidrel + monitoring of bloodwork and IUI with donor sperm x1 cycle for PCOS - 2 mature follicles, no pregnancy 5 pills of 5 letrozole, ovidrel + monitoring of bloodwork & IUI with donor sperm x2 cycles for PCOS- resulted in 2 mature follicles and one mature follicle before IUI was scheduled Told me that with our next cycle we can do 5 pills of 7 mg letrozole for PCOS -

Describe the protocols Rebecca Chason used in your cycles at Shady Grove Fertility and their degree of success.
1st cycle was an antagonist protocol using birth control start, Menopur, Gonal F, Ganirelix and Lupron trigger (I do not recall the dosage). I had a high AF count and we wanted to prevent OHSS, and she felt I would be a good responder. I stimmed for 8 days, triggered on the 9th. We retrieved 21 eggs, 13 mature, 4 fertilized, 2 made to it to blast and 1 was euploid. That transfer resulted in a chemical pregnancy. For the 2nd round we made a change to improve our fertilization rate, we switched to a Lupron stop protocol. The goal was to produce better quality eggs to increase fertilization. I used Lupron for 9 days once I had a positive ovulation test, then I stimmed for 8 days triggering on the 9th using Menopur, Gonal F and a HCG trigger. We retrieved 19 eggs, 13 mature, 12 fertilized, 10 made it to blast, 7 euploid, 1 transferred resulted in a miscarriage. This last round is still being monitored for my HCG levels to drop and we will discuss next steps. I between round 1 & 2 retrieval I had a mock transfer which discovered a polyp, and between round 2 retrieval and transfer I had one hysteroscopy to remove the polyp and endometritis was discovered as well as low visibility of my right tube. I had a course of antibiotics for 2 weeks to rid the inflammation, followed by an endometrial biopsy which showed improvement but endometritis still present. At that time I opted for an additional hysteroscopy for a repeat biopsy and to look at the right tube. The biopsy showed the endometritis cleared, and the right tube entrance had scar tissue that was cleared.

My husband and I have always felt comfortable and heard when we meet Dr. Chason. She's eternally optimistic yet realistic. We feel hopeful, supported and confident whenever we leave our appointments. The [SGF Annapolis] receptionists are lovely and encouraging. Scheduling is easy. However, there have been a couple times, over the years, that I've waited almost an hour to be seen. I adore the techs and phlebotomists. Unfortunately, one of my absolute favorite phlebotomists left during the pandemic. I had a concern with one of the NPs, which I addressed with my primary nurse. I haven't had any issues since.

Dr. Chason personally follows up through out our cycles. She went above and beyond to provide me with an excellent surgeon to manage our ectopic pregnancy. She also followed up with the surgeon post op to continue consulting on my case.

(2017-2020) We had 11 IUIs using donor sperm; 3 rounds were medicated (Clomid x2, Femara x1). I am sensitive to medication and prefer to limit it's usage. I had an allergic reaction to Clomid during our second medicated cycle. We had our first positive test with our 6th round. However, I miscarried a few days before our first ultrasound. (2021) We opted to try a round of IVF. We used estrogen patches, Menopur, Ganirelix, Gonal-F, Crinone and Estradiol. We had a fresh transfer on day 5; we opted to have our remaining embryos tested (PGT-A). Our cycle resulted in a positive pregnancy test, but the pregnancy was ectopic. We have not yet started preparing for our first FET.

Ann took over my care when my previous nurse took a new position. She's calm and reassuring. She's very responsive when I reach out by phone or email. She's always so sweet when I see her around the office.

I don't remember the specifics. I had 2 IVF cycles which yielded a total of 3 PGT tested normal embryos.

Generally I had positive experiences with my nurses but found the nursing team approach to be frustrating and ineffective. Information would get lost between the nurses, I would get asked the same questions by multiple nurses on the team. I wished I had one point of contact instead of multiple. The staff who draw your blood and perform your ultrasounds were kind and skilled.

She is so lovely and nice. I always enjoyed by short interactions with her. The front staff is nice enough. Sometimes the mornings are very busy (most of my appointments) but there is that 1-2 times where I was the only person in the wait room.
How was your experience with Brad Swelstad at Shady Grove Fertility?
I wish that I could have had more face-to-face interactions with Dr. Swelstad but I understand that at a large clinic such as SGF, that's not always possible. In the two times that Dr. Swelstad completed my ultrasound (during two IUI rounds) he always made me feel at ease and made me feel like he knew who I was based on our virtual meetings. Dr. Swelstad is direct and considerate. He added dashes of humor to our interactions and I liked that about our conversations. Although the IUIs were unsuccessful, Dr. Swelstad's guidance and medication adjustments were welcomed. My husband and I have been very happy with our relationship with Dr. Swelstad. We are at the beginning of an active pregnancy journey and we hope to be able to share happy news with Dr. Swelstad by the end of this year!

Don't be afraid to ask Dr. Swelstad questions. He might not give you warm, fuzzy responses, but he is honest and clear.

She [Dr. Esposito] was extremely compassionate. Very willing to take the time to talk to me about all of my concerns. Allowed me to ask questions and didn’t make me feel stupid for any of the questions I asked...For my egg retrieval I was on menopeur, and gonal F. It yielded 15 eggs, 11 fertilized and 4 made it to day 5/6 with 2 testing genetically normal through PGT-A testing. For my first transfer I was on estrogen and 5 days of PIO prior to the transfer. For my second transfer, after an ERA cycle adjustments were made to my protocol.
My nurse [at Shady Grove Fertility] was always willing to answer my questions. She was available by both phone and the portal. She made sure I understood everything completely and would review multiple times to make sure I understood...At times the office was difficult to get ahold of by phone. The front desk staff at times was not very welcoming. As a whole the clinic is very organized and extremely responsive. I would want a new patient to know that it’s ok to be persistent. There was never a time where I didn’t feel heard or seen as a patient. Dr. Esposito was always understanding of my concerns and hesitations. She took the time to explain to me anything I had Questions about. After learning my son was born 14 weeks prematurely she made sure to check in to see how he was doing and has continued to check in on him. She truly cares about her patients and wants them to be successful in their treatment.

For my egg retrieval I was on menopeur, and gonal F. It yielded 15 eggs, 11 fertilized and 4 made it to day 5/6 with 2 testing genetically normal through PGT-A testing. For my first transfer I was on estrogen and 5 days of PIO prior to the transfer. For my second transfer, after an ERA cycle adjustments were made to my protocol.

My nurse was always willing to answer my questions. She was available by both phone and the portal. She made sure I understood everything completely and would review multiple times to make sure I understood.

At times the office was difficult to get ahold of by phone. The front desk staff at times was not very welcoming. As a whole the clinic is very organized and extremely responsive. I would want a new patient to know that it’s ok to be persistent. Dr. Devine is the director of research and I trusted that she had up to date, accurate, and thorough information to guide us. She presented options and answered our questions. All the while, she was reassuring about our prospects. She is direct, so we felt that she would have told us if she weren’t optimistic. Dr. Devine seems busy - we had 3 zoom appointments with her and then met her in person when she came to do our second transfer (which was a pleasant surprise)! We would go back to her for baby #2.

Dr Devine was kind and reassuring to us right off the bat. She used numbers and statistics to explain her reasoning for decisions and for her overall optimism for us. Although we had previously only met her on zoom, Dr Devine showed up to do our second transfer even though she wasn’t on call for transfers that day, which was a lovely surprise. The nurses helped me feel like a human because every time we talked on the phone, they were willing to answer all of my questions and I knew they were in contact with Dr. Devine.
Describe the protocols Kate Devine used in your cycles at Shady Grove Fertility and their degree of success.
We were not quite at a year so Dr Devine recommended starting with unmedicated IUIs. We did 3 without success. At that point, we were at a year and she gave us the option of medicated IUI or straight to IVF. She did not push us either way, but told us why a couple might opt for one or the other. We went straight to IVF because of its higher likelihood of success. My stims cycle was pretty straightforward, with a Lupron trigger prescribed over hcg due to worries about OHSS (which I didn’t ultimately get). I opted for a freeze all cycle and we did PGT-A testing for our 9 embryos, 5 of which came back normal. We did two medicated transfers, the first of which failed and the second of which has resulted in a current pregnancy. The first cycle, we did estrace 3x daily plus PIO every third day and endometrin inserts twice daily. For the second cycle, I think dropped the inserts and did PIO daily (my preference). She offered us a modified natural cycle which we would have tried except that we had a lot of travel that precluded it. That would have been our next choice of transfer 2 had not worked.

The nurses were lovely. In particular, Stephanie was always so kind, reassuring, and willing to answer my MANY questions along the way through messages and on the phone.
Describe your experience with Shady Grove Fertility.
Strengths: multiple facilities in the area, relatively easy to make appointments once you’re a patient, top notch doctors and care team. The Rockville office is calm and the facilities are modern. The nurses and nurse anesthetist for egg retrieval were stellar. Dr Humm did my retrieval and was superb. Despite having many patients, they seem to get everyone scheduled when they need to be! Cons: there are a lot of patients and they do seem busy at times, sometimes appointments can start late esp for bloodwork and ultrasound. Sometimes it feels like until you’re an IVF patient, you’re low priority. Sometimes I felt like the coordination between the andrology lab and the other areas was lacking.

IVF was about $7000 after insurance. FET cycles were about $1000 plus a $645 “thaw fee” not covered by insurance. Embryo storage is $60/month per retrieval cycle.

Dr Devine almost always recommends eSET vs multiples. Multiples was not an option for us, which we didn’t mind.

Dr Malik went through our entire history...She was willing to do all of the potential testing that I wanted to do and provided unique guidelines based on our situation. She always approached things very methodically instead of emotionally - laying out the facts and the possibilities that we could consider (but ultimately we were the ones who made the decision of how to move forward)...Overall, I felt like she was very invested in our journey and I trusted her.
SGF strengths - compassionate staff, lots of appointment availability so you can get the appointments you want, relatively short wait time, depth of knowledge by practitioners SGF weaknesses - the only negative is that the clinic you sign up with is the one you have to always go to. I signed up for a further clinic because they had the soonest appointment time and then I got locked into that as my location for 1.5 years.
How was your experience with Shruti Malik at Shady Grove Fertility?
Dr Malik is a very methodical, practical doctor. She approaches things with a level head vs. being emotional and lays out multiple paths that you can consider (including the risks and benefits of each). I had a few other fertility specialists that I knew in other states that I got second opinions from, and she always welcomed that other feedback/insight and never felt overly protective of doing things "her way" if I felt strongly about doing something else. When our IVF egg retrieval went poorly (we retrieved eggs but discovered poor egg quality and did not get to the blastocyst stage), she came prepared with a new treatment plan for us to adjust with this new data. Ultimately IVF is a long process and each new test / result is a new data point to customize in the future. I felt like Dr. Malik was in it for the long haul with us. We did get spontaneously pregnant after our failed IVF cycle and Dr. Malik made sure to be the one to do our ultrasound at 7 weeks. She also made sure to be our doctor on days where I needed certain procedures done since she knew my chart the best. Overall, I felt like she was very invested in our journey and I trusted her. Even though we got spontaneously pregnant, I would have felt comfortable going forward with another IVF cycle with Dr. Malik despite the first one not going well because I felt like she had a customized plan as a next step. Would definitely recommend her to others.

Do your own research and ask questions - Dr. Malik will lay out all the facts but for us she relied on us to make the final decision on how to move forward. This allows the patients to have control over the situation but I think it's good to understand a bit on your own before going into appointments.

Rachel was my nurse at SGF and she was amazing - she was my primary POC during the process even though I knew Dr. Malik was the one dictating my treatment plan in the background. Rachel was always available to talk and I felt like she really was my cheerleader during the entire process. The one thing for patients to remember is that I think sometimes the nurses can be overly positive - and its always good to remember that you never know the results until they officially come in. Regardless, I always appreciated talking to Rachel and I loved working with her.

For IUI cycles, Dr. Reh prescribed Letrozole with a trigger shot. All 4 IUI cycles resulted in at least 1-3 appropriately sized follicles. None were successful. For egg retrieval, I followed a "standard" protocol and had a successful retrieval with 19 eggs, 13 mature, 11 fertilized, 6 day-5 embryos, 4 PGT-A normal.

I was an out of pocket patient. An IUI cost about $2,000 per cycle with medications. I paid for the Shared Risk Program with PGTA testing - this cost about $26,000. Medications for egg retrieval were about $4,000. Overall, I paid about $40,000 throughout treatments.
