He is the only reason i'll more than likely try again with Shady Grove. one our first try, i barely responded to the meds, eventually i showed about 3 -5 eggs and while most doctors wouldve cancelled, he believed that my age and my physical condition would result in two or three great embryos and Alas! we ended up getting 3 eggs and two made it to day 5.
Strengths [Shady Grove] - The Best in the area! top notch , great facility Weakness - You do eventually start to feel like a number. If you care about being cuddles somewhat it'll irk you. if like me you just want the best results, you'll remind yourself its only for a little bit of time... - Insurance paperwork was lost a few times and i missed a condultation because they wouldnt let me see Doyle with the paperwork that shouldve already had!

Unfortunately i had to have an emergency C section to save my little boy at 25 weeks and he stayed with us a day before getting called to heaven. Dr Doyle was as affected by it as we were, two years later and he still calls to check in on us. He really is the best!
What's one piece of advice would you give a prospective patient of Joseph Doyle at Shady Grove Fertility?
Be Open, discuss as much as you can. Ask questions when you dont know. He goes out of his way to explain everything and will whip out a pen and draw on a pad if need be! utilize his time and expertise.
During treatment, were you treated like a number or a human with Joseph Doyle at Shady Grove Fertility?
My first time using the practice the staff was great , however i changed nurses twice and that made things somewat sensitive. i had to do alot of follow ups with the new Nurse , she might have been a newbie in the business. Dr Doyle however, is a true gem, straightforward, optimistic and empathetic. My second go around from start to end i felt like a number, my details were getting mixed up, call ins, scheduling, dr visits , everything was very clinical and this is on the staff side NOT Dr Doyle. towards the end of my second go Doyle called not just to apologize for the frustrations i'd gone through with miscommunication from the front desk on insurance coverage, he occasionally called also just to check in.
Describe the protocols Joseph Doyle used in your cycles at Shady Grove Fertility and their degree of success.
Memory fails me but if i remember correctly it was - Birth control Pills - Gonal F 150 then increased to 300 because i was a low responder - Menopur 25 or 50 and HCG. Both times were same protocol. However he would prefer to try something different when i go back because my LH levels are very high and he doesnt think starting with birth controls will be ideal.

My Last primary Nurse Effie was great, responsive and detailed. I got the feeling they were on top of things. The Nurse before her - i forget her name wasnt soo great and it was a relief when she eventually left the practice- she was terribly forgetful, couldnt remember my name to save her life! - not that it matters to me - i like results not pleasantries, but IVF is such a sensitive time i think every bit of being personable helps

Well oiled machine for the most part I preferred to go very early because of work , but i believe they are flexible

It was fairly reasonable with insurance coverage. There was an initial deposit of about 3500 and majority of the procedure was covered at the time- Out of pocket i want to say in total was about 5000 -6000 for IVF, Medications, HSG tests, ICSI(about $1200 or so ), Transfer. Embryo storage was 1600 initial deposit and 800 Yearly!
Describe Joseph Doyle's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Shady Grove Fertility.
Doyle's preference is single embryo transfer, he will not only lecture you oon the perils of multiples, but will have you sign a disclosure that you were educated on the potential issues. Since eqq quality for me is not an issue, ive not yet needed to ask for more than one transfer.

We used a fresh cycle-frozen embryo transfer [with Dr. Levy] because my lining responded better to IVF drugs, but we transferred a genetically tested 5 day embryo... saw us after we’d failed several courses of treatment and we thought we were ready to move on to donor eggs. He encouraged us to try with him again. We had our best cycle on record for a freeze all, and he studied MY DATA and saw that my lining didn’t respond on using traditional FET drugs so we used injectibles instead- boom- two Singleton babies later, we cannot be more satisfied with our doc.
We did two IVFs and 2 transfers to get our “take home baby” [at Shady Grove Fertility-Rockville]...runs like a well oiled machine. I love that the surgical bays are so clean and organized- trust me, I’ve been to some truely shady fertility clinics (pun intended). The teams for retrievals and transfers are military like in their processes- it makes me feel safe and secure as a patient, that my proceedure is in good hands and that they’re transferring the right embryos. They’re always stopping to match names and procedures.

As an out of state patient, be prepared for a long stay near the clinic before the IVF for monitoring, this means days off of work and hotel costs.

We didn’t get to talk to or email with Dr levy apart from the initial consultation and strategy changes after a failed cycle. But we had a dedicated nurse who was very much available by phone and e-mail for all of our questions and concerns. To us, it didn’t matter because we got two babies (singletons) and that’s all that mattered after not getting the same results for years of treatment before.

Oye. I think we paid 25k for two cycles of IVF plus including all FETs for the embryos created until we took home a baby. We did two IVFs and 2 transfers to get our “take home baby”. We are so thankful to have him be a part of this whole journey, and we are lucky that he is dedicated enough to care about our treatment even when he is not directly involved.

I was on Menopur 75 and Gonal F 225 for about 9 days which was a surprisingly short protocol. Triggered on day 10 and retrieval on day 11. I used the HCG trigger instead of the Lupron and there was a specific reason why Dr Chang decided on that but I can’t recall. They retrieved 19 eggs. 10 ended up fertilizing normally but only 3 tested noramally (PGS) without chromosomal abnormalities. All 3 FETa failed. One of the three was a chemical pregnancy.

The year we decided to start IVF, the US Federal government pulled ALL of the insurance carriers that offer IVF treatment off their benefits offerings.

We started with progesterone vaginal inserts and progesterone injections. Then, we moved to progesterone injections and was able to have implantation. As for estrogen, we started we just injections, second cycle we added estrogen vaginal inserts twice a day and now, inserts are up to three times a day. The change to estrogen have increase the uterine lining before implantation and increase of progesterone injections dosage have decrease bleeding after transfer.

Shady Grove takes great effort to create an environment with compassionate professional care from the front desk folks that greet you at check-in to the medical staff passed in the halls to even how the waiting space is broken up so you don't walk into a massive open waiting room akin to visiting the DMV. When you're having a tough day as many of us do in this process, those things are comforting. We've been with Shady Grove through three IVF cycles and the one area of weakness is the Financial Services team. The Finance team has high turnover, doesn't always lay out costs clearly and at times may take days to respond to your call or message. That said, they are still pleasant to work with. I would suggest doing most communication on finances via email so you have records.

I only did donor egg cycles with Dr. Levy. Due to a history of Asherman's and thin lining, I used stim meds (gonal F) to build a lining prior to transfer. He quickly moved to this after a poor response to estrogen alone and also at my request because I knew this worked better for me. After two failed transfers, we added lovenox pre and post transfer and that transfer worked.

My main nurse was not a great communicator. I would email questions and concerns and ask for them to be addressed to my doctor. I finally realized she was not telling him any of these questions or concerns and was just giving me her own responses. Half the time she would only answer one out of two questions and just ignore the other. It was extremely frustrating. When she was out of the office and I had other nurses cover my experience was MUCH better.

This is a very busy clinic. With that being said, most all staff members were compassionate and helpful. SGF has a free therapy group that patients can join and meet others in the same situation. That was very helpful. They also have a wellness center with acupuncture and massage. This was a saving grace for me! I was able to do acupuncture often, and on the day of my transfer.

The final nurse I worked with, Effie, Was really amazing. She was very compassionate and organized. The reason for the low readings in this area, though, or that I worked with about eight different nurses in the course of my treatment with Dr. Doyle. Some were very good at their jobs, others were not, but there was always a lot of turnover and often they were issues with the handoffs.

They have a lot of resources, great office hours, and a large staff so that you don’t run into issues with appointment availability or service. However, they can make you feel like a number given the size of the operation. They also often forgot about my tubal blockage diagnosis, and told me to go ahead with cycles that should’ve been canceled because I was ovulating on the wrong side. I then moved on to a medicated protocol [with Dr. Joseph Doyle] with a low dose of clomid, ovidrel trigger shot and progesterone support after IUI. I really liked this approach as it was aggressive by using the progesterone support, but used a low dose of clomid to reduce the chance of multiples. I only ovulated 2 eggs during that cycle and one was fertilized! I was able to have a healthy pregnancy. I also really liked that there were two ultrasounds early on at 5.5 and 7.5 weeks where I saw a heartbeat and could relax as the risk of miscarriage went down.
Each unmedicated IUI cycle [at Shady Grove Fertility] was about 300-800$ depending on ovulation monitoring. The medicated cycle was a flat fee of $1800 plus medication... Just be prepared to not feel like the one and only client at the clinic...I think the clinic's strength is that they have everything on site. It is a one stop shop for all screening and procedures. Because it is a busy place it can kind of feel like a factory a bit since there are so many people moving through the same procedures as you. However I found all of the nurses and doctors to be extremely friendly when it came time for a test or procedure.

I felt Dr. Doyle really listened to what I wanted and tailored my care to what I was comfortable with. I wanted a semi-conservative approach seeing as I was over 35 but not diagnosed with any fertility problems. We worked together to come up with a plan that we were both comfortable with and were confident would get me pregnant.

Be honest about what you want and your timeline. Make sure you explain your worries so he can give you the best information to set your mind at ease.

Dr. Doyle remembered me every time I went in, listened to my concerns carefully and addressed each one. We even exchanged hiking trail information because we had a common interest.

I did not want to be too aggressive with my treatment as I was seeking to get pregnant as a single woman over 35 with no known fertility problems. We had screening of hormones an antral follicle count and HSG to confirm everything was looking good. With that confirmation we decided on 2-3 natural IUI cycles and if that didn't work we would move on to a medicated cycle. I did three natural IUI cycles (the last two where my ovulation was confirmed at the lab with blood tests) but they were unsuccessful. The biggest difference for me is that they were so set on using progesterone in oil which I did not use in the past and my previous doctor wouldn't even consider and wouldn't give me an explanation. I used a donor so I didn't know her exact protocol as far as stimulation meds since it was specific to her.

Shady Grove Fertility makes you feel like you're in good hands. We were initially planning to go with a much smaller fertility center but have been so pleased with Shady Grove. Despite the size of the practice, the staff is well-trained, there's always someone available on-site or the phone to answer questions and they consistently treat you like a person. I've interacted with a handful of doctors beyond my primary one for monitoring visits and all have been great. They do everything they can to help achieve a successful pregnancy. The nursing and sonographer team is extremely well-trained too. The one area of weakness is the Financial Department. There's lots of turnover there (we've had 4-5 different financial counselors in a year on our account), information on costs and payment options requires lots of clarifying questions and the team seems rushed and less pleasant than those on the medical side.

Advocate for yourself. This process is so hard. When medications made me feel ill, when procedures felt like unnecessary torture, I let him know and he took the time to rework my protocol and make changes to make the process less miserable. When changes couldn't be made, he took the time to explain their necessity. He has many patients and is in high demand as a doctor so you really need to advocate for yourself, even if through your nurse.

I went to 3 sites. I primarily went to Rockville office where the wait times are terrible. I have waited hours for monitoring and have let the staff know how unacceptable it is. I will say, in recent visits, it has shown improvement. I try to go to the K street D.C. Office when possible because it is incredibly efficient. I have also been to sibley but their carrier leaves pretty early so blood work is more challenging there.

We were fortunate to have insurance coverage (we are carriers for a genetic disease so this was for disease prevention). Still, I'm sure we have spent at least 15K between medications, copays and embryo preservation/freezing and thawing.

I am pretty sure that they advise against it, and in the case of the shady grove guaranteed program, it does not make sense to transfer more than one at a time, since a single embryo has a better chance of survival, and if it does not survive, we can try again with another.

There is a bit of a disconnect between her [Dr. O'Brien] and her nurses...Dr. O'Brien told us up front that given all of our conditions it would be like finding a needle in a haystacks, but she was very persistent and after 4 years we finally got pregnant. She went out of her way to ensure she was available to do our retrievals and transfers...Our main objective was to produce as many eggs as possible since we were dealing with male infertility issues. She ensured the top embryologist was available during the retrieval for the ICSI afterwards. Additionally she was very receptive to our ideas re: protocol.
they [Shady Grove] accidentally thawed my husband's sperm without telling us but continued to charge us the storage fee. This was huge and they didn't see it as a big deal. When male infertility is the diagnosis and we felt the sperm at the time was good enough to freeze-its a big deal!...My biggest criticism with the clinic is their financial department. We're actually in the middle of a transition. I loved our nurse Debbie Rice and even though she no longer supported Dr. O'Brien she was still assigned to us so we could work with the same nurse throughout so she would work with Dr. O'Brien even though she worked for another doctor. For this upcoming cycle we have to work with a new nurse for the first time as Debbie is out on medical leave. We have already been disappointed. Debbie was so accommodating to our schedules and requests whereas our new nurse (Jessica Buggs) hasn't made any effort at all. It is so disappointing I hope it doesn't negatively affect our final cycle

For this, they didn't treat it as a huge issue even though I am pretty confident there are OSHA violations related to this. When my nurse was out of office, the nurse assigned to call with my dosing for that evening failed to call.

Dr. O'Brien was always compassionate and took time to talk to me about next steps in my treatment plan. She made adjustments to my treatment plan based on how I was feeling and always shared enough information so that decisions were well-informed. I have always felt that Dr. O'Brien saw me as more than just a patient/case. She saw me as a woman who desperately wanted to realize a dream of being a mother and she helped me accomplish that dream.
Describe the protocols Jeanne O'Brien used in your cycles at Shady Grove Fertility and their degree of success.
I do not remember much about my treatment protocols. For my egg retrieval, I used Menopur (increased dosing following monitoring appts), Follistim (300), and Cetrotide. I think the trigger was HCG. For transfer, I began with daily IM shots of progesterone in oil and 2x daily estrodial tabs (vaginal). Then, I introduced 3x daily progesterone suppositories and switched to IM progesterone in oil shots to once every 3 days (in addition to 2x daily estrodial tabs).

Jessica was compassionate, organized, communicative, and responsive. She was always willing to answer my questions (and ease my worries), responding to my emails and phone calls in a timely manner. She and Dr. O'Brien are the dynamic duo! I would not have made it through this journey without the support and encouragement of Jessica. She is a wonderful asset to the team at SGF and will always have a special place in my heart.

Among SGF's strengths are the office climate (everyone was friendly, warm, and welcoming at SGF), organization, and supportive environment. They set the bar high for patient care and you should expect to compare your experience at other offices against your experience at SGF. No matter who you engage with, you can be sure that you will be treated with respect, your questions will be answered, and that promises will be kept (if they say they will look into something for you or find out the answer to a question...they will follow through). Everything about my experience at SGF was positive.

For my cycle, I was on birth control to synch my cycles with the other recipients( in the 1:3 shared risk program) then was on Lupron for a week to prevent ovulating. Then on endometrium every 3 days, and progesterone in oil for the 5 days leading up to transfer.

My first egg retrieval was done on a weekend that Dr Chang wasn't on call, so his colleague had to perform the procedure. I had a lot of pain after that procedure. My second egg retrieval was performed by Dr. Chang. I had no pain whatsoever afterward. He's so so nice. He's been in the business for many years. If I ever needed to speak with him, all I had to do was contact my nurse. She would notify him and he would call me back.
Dew was the care coordinator [at Shady Grove]. She's fantastic and almost always responded to my emails or calls so so quickly! Kaitlin is my nurse. She is very caring and helpful. Always responds to my emails or calls almost always same day. Goes above and beyond to help!...It's a new office. Its very large. The decor is nice - very modern. There is almost always someone at the front desk so, finding your way around isn't difficult. Strength is that they are very prompt. I've never had to wait very long. He [Dr. Arthur Sagoskin] is very friendly and patient with my wife and I when we asked a lot of questions. He was able to make time for a last minute appointment when my cycle started and we realized my first IUI didn't work. He has always been on time for our appointments and never once rushed us.
We have enjoyed Shady Grove Rockville. The offices are beautiful. The desk staff are always so friendly and kind. The nurses who take blood have also been very gentle and kind. The Dr who did both of my IUIs was amazing and so friendly....Kate and Tina [nurses] are very friendly. When we are in person they are very warm and kind. They both answered all the questions we had. My cycle and questions always ended up happening when Tina was out of the office. It was hard to get in touch with her a few times. She also didn't place the order for my medication on time and I almost didn't receive it on time. I called the Pharmacy and it was noted that I was not in the system. I called and emailed Tina on Thursday but she was out of the office until Tuesday. She called me Tuesday afternoon and said that she talked to the Pharmacy and they would rush it to me. We decided to do all the pretesting to make sure I had a good follicle count and my tubes were clear. We did an medicated IUI first and if it didn't take we would be doing medicated. That is what we are currently doing.

Kate and Tina are very friendly. When we are in person they are very warm and kind. They both answered all the questions we had. My cycle and questions always ended up happening when Tina was out of the office. It was hard to get in touch with her a few times. There were so many treatment plans, each one was unique with the end goal being a baby. We immediately decided upon IFV treatment first after removal of cysts from my ovaries. Due to advance age and stage IV endometriosis, we felt it was the best route.

Sometimes you had to wait too long in the waiting room, there were times it seemed like they forget about me. This is for blood work and ultra sound work there. Dr. O'Brien is a busy person, to be sure, but I never had a day where I had a question or concern that I didn't at least get a return phone call by the end of the day. Her positivity is contagious and she was always ready with a plan b or c when plan A didn't work. As someone who despises going to the doctor it truly suprised me the day I realized that I saw here as more of a friend than a doctor. If you need someone in your corner, someone to keep faith that this will work even when you've lost all yours, Dr. OBrien is a great choice...
Shady Grove is a big clinic, so yes you sometimes feel a bit like a number, but if you find the right Doctor and Nurse, the long road of infertility suddenly doesnt seem so dark and lonely...Ask questions. You'll feel better knowing all the answers. And trust her when she says she'll make it work. She doesn't give up :)...While my nurse, Effie often called with day to day results, Dr OBrien always followed up with a phone call if the results were something important. She was very respectful of my feelings (and since I was on lots of hormones there were LOTS of feelings) and she always stayed hopeful...

I went to Dr O'Brien after having a rather negative experience with a different doctor and it was like night and day. Our case was a little unconventional. Because of how far away we lived from Rockville, we didn't meet Dr O'Brien in person until the day of egg retrieval but we both felt like we already knew each other from various phone interactions. Dr. O'Brien is a busy person, to be sure, but I never had a day where I had a question or concern that I didn't at least get a return phone call by the end of the day. Dr. [Joseph] Doyle is great and explains things well but in the end you speak to his nurse more than him. I would still return to him because he had a great sense of humor and when he felt that we were losing hope he did personally do one of the IUI procedures. I would and will be more insistent in communicating with him directly....Sheila and Jessica were Dr. Doyle's nurse and assistant. Both were on top of my treatment and would always be compassionate when they had to deliver bad news. I appreciated that.
Strength was the amount of success stories and their reputation. I never had to wait too long for an appointment and they were very responsive to emails and calls. A weakness was that it did, at times, feel like a factory.We are now taking a break from treatment just to clear our heads. My only real issue with the facility were some of the nurses. Dr. Doyle's nurse Sheila was great but some of the others were cold and not as friendly.

My husband and I attended a seminar held by Dr. Doyle. He was so nice and explained IUI and IVF clearly. We had 2 appointments with him and he did one of the IUI procedures. I felt completely comfortable with him but unfortunately there has been no success. He did warn us that nothing was guaranteed. We are now taking a break from treatment just to clear our heads. All of our tests were negative. We had unexplained infertility. Since we had nothing diagnosed as being "wrong" Dr. Doyle suggested we try IUI first since my insurance covered six cycles. Even though they were not successful I'm glad we tried it so we can be prepared for IVF.

Strengths [of Dr. Sagoskin]: experienced; knowledgable; offered suggestions for further testing (I wish he would have suggested a few tests must sooner); followed up after negative results and a lose; advocated for me during a miscommunication between my nurse/him/embryology. Strengths [of Shady Grove]: friendly staff, quick response times, great monitoring appointment times, usually minimal wait. Financial Counselors responsive. Weaknesses: Sometimes it feels like a factory, nurses and doctors lack empathy and need to think before they speak. Sensitivity training/refresher would be great for everyone. Recently added many more doctors and nurses and feels less personal, chaotic at times. What a new patient should know: Be realistic and take off the rose colored glasses. Prepare for failure. IVF is not always the answer, sometimes it doesn't work even after multiple attempts with seemingly great outcomes. Advocate for yourself, do your research, question everything, demand respect. Ask for detailed description of an egg retrieval, from talking to anesthesiologist in triage, to walking into operating room and what and who to expect to see. Take all of your pain meds after retrieval, even if you don't think you need them (you will). PGS test and only transfer normals (don't set yourself up for failure and heartbreak if you don't have to).

Very personal attention when I first started seeing Dr. Sagoskin. Loved him and his no nonsense manner. 2+ years later and no success, I feel like I'm being treated more like a number and an inconvenience at times.
Describe the protocols Arthur Sagoskin used in your cycles at Shady Grove Fertility and their degree of success.
I was 38 when I started treatment. Protocol 1: BCPs, Antagonist protocol, lower doses of Gonal F and Menopur, slow & long stim, Ganarelix, Lupron/HGC dual trigger. 20+ follicles, only 2 retrieved, both mature, both fertilized naturally, and 3-day transfer of visually great embryos, BFN. Protocol 2: BCPs, Antagonist, higher doses of Gonal F and Menopur, slow & Ling stim,, Ganarelix, HCG trigger, (I demanded aftee the suggestion of two other REs) double-lumen needle during retrieval, 25 eggs retrieved, at least half mature, all fertilized naturally, 6 high grade blastocysts, eSET, positive beta (but low), doubled two times, then deceased and miscarried at 6 weeks (they say chemical since no heartbeat was seen, I consider a miscarriage). Frozen five 5-day blastocysts. Protocol 3: BCPs, Antagonist, highest doses (yet) of Gonal F, Menopur, Ganarelix, slow stim, HCG trigger, double-lumen retrieval, 25 eggs retrieved, 13 mature, 13 fertilized with ICSI, freeze all with PGS testing, 2 6-day hatching blastocysts biopsied and frozen. Thawed 5 frozen blasts from Protocol 2, lost one in thaw, biopsied 4 for PGS testing and refroze. PGS tested 6 blasts, 2 (1 from protocol 2 and one from protocol 3) tested as PGS normal. FET #1: BCPs, Non-injectable E2 and P4 administered vaginally. eSET PGS normal 6-day blast from Protocol 3, BFN. Before next FET elected to do Endometrial Receptivity Array (ERA) biopsy/testing with BCPs then Injectable E2 and P4 + 5 days. Biopsy returned "non-receptive". Retest ERA with BCP then injectable E2 and P4 + 7 days. Biopsy returned "receptive". Waiting to do transfer of remaining PGS normal 5-day blast from Protocol 2. Will do BCPs, then back to back Endometrial "scratches" (one tests for endometriosis, one may aid with implantation) and then transfer. Waiting for CD1 to start new FET protocol with Injectable E2 and P4 + 7.

Very detailed and thorough. Extremely responsive with email and voicemail. Knowledgeable. Respected. We had a miscommunication that damaged our relationship, but we were able to put that aside. I trust her and would strongly recommend her to others patients.

I have some insurance coverage so meds for 2 of 3 cycles was fully covers and paid for one cycle of meds out-of-pocket. Specialist company's for each visit including monitoring. Insurance covered all procedures except for PGS testing (covered biopsy) and RhoGam shot after miscarriage (due to blood type).

Miscommunication between RE/Nurse/Embryology and was told frozen embryos were thawed, all survived, Biopsied for PFS testing, and sent to genetics lab for testing. 2 weeks later, no results. They were never thawed or biopsied. Once they were we lost one. Embryology said it was REs fault. REcsaid it was Embryologies fault. Nurse communicated erroneously and damaged our relationship for a long time.
