Dr. Katz was friendly and talked to me like a real person...explained things well. He did seem kind of rushed at times. He did seem to want what was best for me, and was not worried about if I would stay a full patient with his clinic...We ended up keeping our embryos in KC and performing an ERA test and doing outside monitoring with Dr. Katz. During the ERA test, he had me on oral estrogen 3x/day and then progesterone in oil for 6 days prior to the biopsy. The test results were that my lining was normal receptive 6 days after beginning the progesterone.
Jenna [nurse at Shady Grove Fertility-Bel Air] was very friendly and compassionate. She would call and explain things to me and answer any questions I had. We did have some times when we were playing phone tag and I had a little difficulty getting in touch with her...It took a while for the initial consult though. The nurses and staff were friendly and seemed knowledgeable regarding their jobs. There was a little miscommunication when I became an outside monitoring patient and they had trouble figuring out where my results were and where they were going.

Dr. Katz was helping me determine if we should move our embryos to his clinic in Maryland or keep them in KC and just do outside monitoring with Shady Grove. He explained the pros and cons well, and genuinely seemed to want what was best for me. We ended up keeping our embryos in KC and performing an ERA test and doing outside monitoring with Dr. Katz. Consultation was covered under my insurance. The ERA test/biopsy was $595. My ultrasound appointment as an outside patient were $295 (which was quite a bit more than the $187 my clinic in KC said they charge).

Allow him to explain things to you as you go but also do your own research and don't be afraid to advocate for yourself.

Describe the protocols Ricardo Yazigi (retired) used in your cycles at Shady Grove Fertility and their degree of success.
When I first saw him in 2016, I was treated for anovulation and was put on 100 mg of clomid with a trigger shot. I was a slow responder but after continual observation I did respond and had a positive outcome that resulted in our son. I came back in 2018 to grow our family and started with a similar protocol but no longer responded to clomid. We upped the dose without change. We then switched to femara in which I responded and got pregnant on the second cycle but miscarried. We started femara again a month post miscarriage but did not have success leading to a hysteroscopy to check for scar tissue. No scar tissue was found and Dr. Yazigi said we may be partially unexplained. He recommended moving onto IUI but since we were out of pocket we decided to pursue shared risk IVF instead.

Since I have been with SGF I have had four nurses. Rebecca was my nurse for my first round in 2016 and I loved her. When I came back I was originally assigned to Kristin who soon left for an alternative job. I then was transferred to Kelli and was with her for six months before she took another position. I am now assigned to Tina. It has been hard to have stability and effects some communication by being shuffled from nurse to nurse.

The clinic has multiple locations that make it easy to schedule appointments for all services. The offer in house procedures and work to get approval from your insurance company. You are also assigned a financial person who makes sure you know what is and is not covered ahead of time. As far as weaknesses, there have been times that information has not been properly relayed which is why it is important that you advocate for yourself in your treatment.

For timed intercourse, monitoring was covered by my insurance after copays were collected. I did pay for medications out of pocket. Shared risk IVF is costing 24,500 including PGS testing.

I have not yet gotten to this point but I think eSET will be what is recommended due to my age. I am comfortable with this decision.

I once had something marked in my chart as a follicle that was a cyst. This led to talks about selective reduction if a multiple pregnancy was the outcome. I also was told to trigger before I should have but could it prior to administering the medication.

Dr Yazigi always has a pleasant demeanor and explains the processes in detail. He is never frustrated by all of my questions.... He made the whole process very comfortable, even for my timid and very private husband. He never treated us like we couldn't understand something he needed to explain... Dr Yazigi had me take Clomid. I had already tried Clomid 3 times but now we would monitor and do IUIs. We tried this twice and he told us that the odds of us getting pregnant were only getting lower, so we decided to try IUI once more with Clomid, and then move on. IVF was our next step because my hormone levels indicated a high response to injectibles, and trying for IUIs would likely result in cancelled cycles due to too many follicles. First round of IVF had poor fertilization but still resulted in a single day 5 embryo which was a live birth. Now that we are trying for our second child Dr Yazigi has decided to add ICSI to the regiment.
Excellent clinic [Shady Grove]. They have enough resources that anyone can start any type of cycle at any time. There are a lot of faces sometimes but they know exactly what they're doing.... Tina was excellent. She always kept me promptly up to date on what I was supposed to be doing, dosages and appointments I needed. She also made sure I had all of the donated medications she could get for me as a cash paying patient. Without her we would not have a son.

IUIs were a flat rate per cycle ($1600), plus the cost of medications. When it came time for IVF we qualified for a shared risk program, which was 6 fresh cycles and unlimited frozen transfers for $20,000, plus the cost of medications. For our second child we decided to pay for a multi-cycle discount, $18,500 for two fresh and unlimited FETs. Diagnostic testing was covered at 80% by our insurance.

[Dr.] Yazigi was very welcoming, open and honest. He gave us the facts and statistics, he laid out our treatment options and literally said "here is your menu, I am your waiter". Shady Grove is great. It is a very large network of doctors which in the beginning can seem like a lot but usually you will be at your smaller local office were things aren't overwhelming. The nice thing about being so large is you can have your monitoring at any office with in the practice. If you know one day you have an early work day and another office is closer to your office that's fine schedule your monitoring with them...

During treatment, were you treated like a number or a human with Ricardo Yazigi (retired) at Shady Grove Fertility?
I felt like a human because everyone knew my name when I got to the office in the morning. They didn't have to look up my chart before they new what I was there for. The weekend monitoring in the larger office could sometimes be a little more impersonal but usually there was someone there working on the weekend that was from my local clinic so I would always have a familiar face.

I had 3 IUI cycles, two with clomid and one with another medication the name of which escapes me. I also took medication for lining thickening and to maintain the pregnancy after IUI. Then I did 1 IVF cycle. My IVF medications were pretty normal; Gonal-f, Menopur, Ganirelix, and HSG shot. We did increase the medications because I didn't respond as fast as Yazigi was expecting. I retrieved 17 eggs, 15 were mature, 12 fertilized normally. We transferred one and froze 6. Right now I am 26 weeks pregnant.

Tina was my nurse for the majority of my treatments. Unfortunately she left the practice in between my egg retrieval and positive pregnancy test. While she was there is was amazing and I have always wanted to run into her and let her know It Worked! I'm pregnant.

Monitoring appointments were really easy and fast for me. I usually wanted the earliest appointment, there was always someone there and I would get in and out fairly quickly.

My insurance covered most of our costs. We generally had $30 co pay but that was it. Insurance also covered most of the medication costs.

For my first iui [with Dr. Katz], I used Chlomid and it took a long time and a second dose to get my follicles to grow. After it was a failed iui, for the next one he suggested that I use letrozole. I absolutely love my nurse [at Shady Grove Fertility]! She is very compassionate and always available for when I have questions. One strength I think they have is their success rates. Some negatives would be some of their weekend staff at Towson seem to not like their jobs and another negative about the company is their billing department. Sometimes it seems like they are only out to get your money.

She [Dr. Esposito] was extremely compassionate. Very willing to take the time to talk to me about all of my concerns. Allowed me to ask questions and didn’t make me feel stupid for any of the questions I asked...For my egg retrieval I was on menopeur, and gonal F. It yielded 15 eggs, 11 fertilized and 4 made it to day 5/6 with 2 testing genetically normal through PGT-A testing. For my first transfer I was on estrogen and 5 days of PIO prior to the transfer. For my second transfer, after an ERA cycle adjustments were made to my protocol.
My nurse [at Shady Grove Fertility] was always willing to answer my questions. She was available by both phone and the portal. She made sure I understood everything completely and would review multiple times to make sure I understood...At times the office was difficult to get ahold of by phone. The front desk staff at times was not very welcoming. As a whole the clinic is very organized and extremely responsive. I would want a new patient to know that it’s ok to be persistent. That you must advocate for yourself not only with your doctor and clinical care team but at times with the front desk staff in regards to scheduling appointments.
How was your experience with Melissa Esposito at Shady Grove Fertility?
There was never a time where I didn’t feel heard or seen as a patient. Dr. Esposito was always understanding of my concerns and hesitations. She took the time to explain to me anything I had Questions about. After learning my son was born 14 weeks prematurely she made sure to check in to see how he was doing and has continued to check in on him. She truly cares about her patients and wants them to be successful in their treatment.

Go in with an open mind. Be open to hearing your options and don’t be afraid to ask questions.

My nurse [at Shady Grove Fertility] was a mess; she was my main contact at Shady Grove and never had any clue who I was, my history, or what treatment I was doing...they were to schedule weekend testing for me at Rockville but Rockville had no information on me having been scheduled when I arrived...The clinic staff in Columbia never had any clue who I was, my history, or what treatment I was doing. They actually sent me lab results from another patient with a similar name and were moving forward with treatment based on those results until I noticed it and had it corrected. I had a few experiences for testing at Rockville and Towson locations and they seemed better but would be too far a distance for me to go there regularly.

Again, they tried to schedule treatments for me based on lab results they uploaded for me, which I noticed were for another patient and were just erroneously uploaded to my chart. There were two instances where my medications weren't called in; they claimed it was the pharmacy's error but I work with the same pharmacy with a different clinic now and have no issues so am inclined to believe it was their problem. There was also an instance where they were to schedule weekend testing for me at Rockville but Rockville had no information on me having been scheduled when I arrived.

The only other thing she [Dr. Timmereck] has done repeatedly is bring up my age. I’m 43, and it’s mentioned at EVERY SINGLE appointment I have with her. I know my age! That’s one of the reasons we are there, I don’t need constant reminders!! But overall, since that one phone call, everything has been great. Just don’t expect any unscheduled discussions with her!...don’t expect to feel like you’re her only patients. She is very kind and understanding and was supportive after our miscarriage, but Dr. Timmreck does not make it a secret that she’s extremely busy.
SGF in general is like a factory. Overall, she’s been really nice, but at the beginning of our first cycle, they assigned us to a very inexperienced nurse who couldn’t even answer questions on the phone without checking with a colleague. When I called Dr. Timmreck to discuss, she tried to rush me off the phone saying, “I have patients waiting for me” as if I wasn’t a patient myself! But she did reassign us to another experienced nurse. The only other thing she has done repeatedly is bring up my age. I’m 43, and it’s mentioned at EVERY SINGLE appointment I have with her. I know my age! That’s one of the reasons we are there, I don’t need constant reminders!! But overall, since that one phone call, everything has been great. Just don’t expect any unscheduled discussions with her!

Just don’t expect to feel like you’re her only patients. She is very kind and understanding and was supportive after our miscarriage, but Dr. Timmreck does not make it a secret that she’s extremely busy.

First cycle: I don’t remember the dosages, but we did BCP for 2.5 weeks, then Gonal-f and Menopur injections for 14 days, at some point, they added in cetrotide injections, but I don’t remember exactly when. I did a Lupron trigger shot, then egg retrieval and 5 day transfer Second cycle: we had the Gonal-f and Menopur injections with the added cetrotide again, I don’t remember the dosages, but I do remember that this time it went on for 16 days. Lupron trigger and then egg retrieval. No eggs fertilized in this cycle, so no transfer occurred :( Third cycle in progress: we are on day 7 of Gonal-f and Menopur injections. They increased the Gonal dosage so we hopefully can trigger earlier this time.

This clinic is like a factory. The plus side: they have the system down pat, so you know exactly what to expect and can learn how to work the system to your advantage. The down side: it is EXTREMELY busy ALL THE TIME. They only have 2 blood draw chairs and 3 ultrasound rooms, so you could make an appointment for monitoring at 7:30am and not be seen until 9:00 or later. They do have several other locations in the area, so you can go to any of those locations to be seen if more convenient. But they are all just as busy!

Dr. Beall's bedside manner is very kind and gentle. But SGF overall feels like a factory. They are efficient with visits and check all the boxes, generally, but you do feel like just one of many patients most of the time.
Describe the protocols Stephanie Beall used in your cycles at Shady Grove Fertility and their degree of success.
I did two rounds of IVF with a sperm donor. Both treatments were very similar. I used birth control for three weeks before stopping and then starting daily injections. I injected menopur and follistum daily for approximately 10 days, and added in a daily injection of ganarillix for the last five days or so. I went into SGF for an ultrasound and bloodwork every other day until about day 7 or 8, and then went daily. My hormone levels responded well to the hormones, and the tests all went well. Dr. Beall adjusted my medicines throughout the process, as needed, based on my hormone levels. The final trigger shot, given 36 hours before egg retrieval, was pregnyl. Egg retrieval went smoothly, with no adverse reactions to the anesthetic. The first round of IVF, I did PGS testing on the two embryos that reached blastysis, and both were found to be abnormal and were not transfered. The second round of IVF, I chose not to do PGS testing and did a fresh transfer of the two embryos that reached blastysis. In preparation for that transfer, I took a daily pill and used suppositories three times a day for progesterone and estrogen (estrace and estridol I think were the names). I stopped all medicines two weeks after the transfer, when my blood test was negative for pregnancy. Currently, I am looking into using an embryo donor or egg donor.

Casey was very responsive to messages I left, and patiently answered any and all questions I had. She was quick to find out information from Dr. Beall, if needed, and also helped facilitate communication with the financial coordinator at SGF. She's been wonderful.

SGF is very well-known in this area and has a good success rate. I have multiple friends and acquaintances who have used them for fertility treatments. That is why I chose to go with them. However, you do feel like another number at times. The staff has all been very friendly and personable! But it's a factory and you do feel like women and couples are constantly in and out.

They were easy. I was in early and didn't have to wait long. No complaints.
Describe the costs associated with your care under Stephanie Beall at Shady Grove Fertility.
I paid for a multi-cycle IVF program, which gave a small discount for paying for two IVF cycles at once. That cost $23,500 including the PGS testing I elected to do ($18,500 without PGS). In addition, medications were approximately $4,500 per IVF round, or $9,000 total. And the sperm donor cost about $2,000 total (two vials). In addition, I had to pay for a therapist consultation ($300, insurance partially covered and I think my cost was $125) and a few other bloodwork and ultrasound tests that insurance generally covered. I also elected to do acupuncture during this whole process, and that was $45 per visit (found the practitioner on my own and not through SGF). Overall, the cost of two IVF rounds cost me about $37,000 out of pocket. I am not pursuing donor embryo or donor egg for my next step.

I have been prescribed clomid because I am an insulin resistant PCOS, I do not ovulate on my own. First approach was 4 months of clomid with intercourse, all unsuccessful. The IUIs were thought to work because my husband has only a 3% morphology, after the wash he has about 7-10 million usable sperm. We tried 3 months clomid with IUI, all unsuccessful. Dr. said we could continue doing a few more IUIs, but doesn't suggest it.

I did 3 rounds of clomid by themselves. Then 2 rounds of clomid and IUI. All clomid rounds I took pills from days 3 to 7. All were negative. IUI #3 was injectibles, also negative. IVF #1 was the antagonist protocol. My progesterone was too high the day before transfer so that cycle became freeze all. 11 retrieved, 2 made it to freeze. IVF #2 was long lupron protocol. 9 eggs retrieved, 6 mature, 6 fertilized, 1 transfered and 4 frozen. This resulted in 1 live birth. Then FET #1 was estradiol and pio, 1 embryo transfered from IVF 1 batch, that resulted in live birth.

It is not just caring for multiples, but added risk to mom and babies. We transfered one and discussed that of it didn't work, we would look into multiple.

We started with IUI [with Dr. Stephanie Beall] and with the first try had a chemical pregnancy. We tried 2 more IUI cycles before we met with Dr Beall again. When we met with Dr Beall, she suggested we move forward with treatment and try IVF as that had a more impressive success rate. We did our first round of IVF and my hormone levels were too high to do a fresh transfer. It was recommended that I do a frozen transfer so that is what we did.
All the staff at SGF Columbia has been amazing. They are compassionate and organized. Even the front desk staff is excellent... The monitoring appointments are always easy to find a time slot. The people who do the ultrasounds and blood work are always friendly and timely.... My clinic is always clean and usually easy to get appointments.

Dr Beall is as sweet as pie. She was recommended to me by my cousin. She is very knowledgeable and does not give false hope. She meets with me to discuss any issues I am having or concerns. She will personally call me if I ask to speak with her. She is compassionate and caring.

We transferred 2 frozen embryos after being educated about the risks of having twins. Unfortunately we had another chemical pregnancy with this round of IVF. [Shady Grove - Colombia] Strengths: Good infrastructure, competent and caring doctors and staff, a "no child" in the waiting room policy, and a high success rate. Weaknesses: Some patients feel like the clinic is a "factory." I personally have not felt that way, but my case was fairly straightforward...Kim Sorrentino [Nurse at Shady Grove] was extremely warm and friendly. She answered questions within the same day, and when she was not available (time off, etc.), my questions were answered by other staff members. I always felt like I had all the information from Dr. Timmreck. She gave us our odds, but did so in a way that was compassionate. She wasn't as warm as our nurse, but she was not cold by any means. I felt that she understood the significant emotional side of infertility and was patient and kind. When we were ready for treatment (before she recommended it), she supported our decision. When for financial and emotional reasons, we were ready to to move from one form of treatment to another (before she recommended it), she supported our decision. I felt that she was rational, calm, and patient. I stress "patient" because it was very important to us that ALL of our questions were answered fully. I felt like she was fighting WITH us, and I never had a question about the next step that was not answered.

General Day 3 testing (AMH, prolactin, FSH, E2, etc.), HSG, semen analysis, repeat pregnancy loss panel (RPL). No red flags--"unexplained infertility." No apparent reason for previous losses. Timed intercourse for 1-2 cycles. Clomid (100 mg for 5 days) + Ovidrel trigger+ IUI for 2 cycles. Progesterone suppositories (200 mg twice a day vaginally) until beta test. (Clomid--hoping to get more than one egg during the IUI process to increase the chances of getting a "quality" egg.)

Dr. O [Joseph Osheroff] made sure my treatments would not cause more damage to my body or fertility after i experienced a molar pregnancy. He always gave the good news with the bad which helped my husband and i to make the hard decisions. Dr. O is very positive, but made sure we always understood the risks.... With my two iui cycles with clomid i had trouble with uterine lining being thick enough, the doctor gave me additional hormones to take to help. Now on my third iui cycle, we are switching up medicines so this problem does not happen this cycle.
My only issue [with Shady Grove-Columbia] is the weekend nurses called and left a message you were unable to return the call and ask questions. You had to wait until monday to ask questions and that would interfere with work or other life scheduling.... Natalie was great. Always called with my plan, answered all questions i had. She was very helpful and calming.

After 4 unsuccessful natural cycles, she switched me to a medicated cycle with clomid. I got pregnant on the second medicated cycle...She's perfectly nice, I just didn't develop a relationship with her. I saw her outside the office and said hi. She didn't recognize me after being her patient for 7 months.....She's a good doctor to see. Another doctor did most of my IUI procedures, so choosing a clinic is more important than choosing a specific doctor....

The bad: patients are just numbers - no individual treatment plans. Patients are mostly seen by nurses and rushed staff members.

Started with a natural iui, then progressed to three standard iui's, then to two ivf's with ganeralix, gonal f and menopur. Second ivf resulted in a pregnancy, but Timmreck had me stop the endometrin due to low Betas, so I lost the baby.

She was kind, but nervous. The staff was ok and there was one phlembotomist who was great. The other one I tried to avoid.

Dr. Osheroff was detailed and supportive of us. He is not overly emotional but was very informational. The worst part of the process at Shady Grove is that your contact with your doctor is limited by having to see other physicians when you are required to do most of the IVF procedures at the main clinic.

The nursing staff is the best part of the Columbia office, and frankly they are the ones you have the most contact with due to your daily monitoring.

In my opinion Shady Grove is a numbers oriented practice, but you should not let that discourage you from seeking treatment as the standard of care is high. Every step of our treatment was ultimately our decision and we were respected.
