[Dr. Lorna Timmreck] didn't seem to care about us as patients and was always very rushed. Her responses were passive at every meeting, which was only two or three times during my 3 attempts at Timed Intercourse...Dr. Timmerick was very passive and seemed more like she was going through a routine and reading off cue cards than actually listening, providing answers, and caring about me as a person.
My nurse [at Shady Grove Fertility] was a mess; she was my main contact at Shady Grove and never had any clue who I was, my history, or what treatment I was doing...they were to schedule weekend testing for me at Rockville but Rockville had no information on me having been scheduled when I arrived...The clinic staff in Columbia never had any clue who I was, my history, or what treatment I was doing. They actually sent me lab results from another patient with a similar name and were moving forward with treatment based on those results until I noticed it and had it corrected. I had a few experiences for testing at Rockville and Towson locations and they seemed better but would be too far a distance for me to go there regularly.

Again, they tried to schedule treatments for me based on lab results they uploaded for me, which I noticed were for another patient and were just erroneously uploaded to my chart. There were two instances where my medications weren't called in; they claimed it was the pharmacy's error but I work with the same pharmacy with a different clinic now and have no issues so am inclined to believe it was their problem. There was also an instance where they were to schedule weekend testing for me at Rockville but Rockville had no information on me having been scheduled when I arrived.

He [Dr. Jason Bromer] is very straightforward and clear, which I like. If you want a very gentle bedside manner, that's not really him, but he's not rough either. Somewhere in between. Very realistic, but he clearly cares too...Ask lots of questions - he is happy to answer. Expect realism but not coddling...I am in a same-sex marriage, and I'm a good candidate for IUI so the one cycle we've done so far has been natural with trigger shot. He gave me the option to go without trigger, but I wanted it. We may change protocol next time, but we'll see.
Counseling with a social worker [at Shady Grove Fertility] for using 3rd party genetic material was out of network and was $300. Natural Cycle IUI is $1200 per cycle, but we were approved for the Shared Help Discount at 15% (income based), so natural cycle IUI for us is $1020...They are a well-oiled machine, so they have the process down pat. However, you need to be organized and know your insurance coverage and all your financials. Don't trust them to get it right. They have a patient portal, which is great, but it's severely underutilized, in my opinion. Mainly for messages with appointments. I had to ask for my lab results to be posted, and my treatment estimate never was. The portal is great, but they should really use it more.

Shady Grove Fertility is a well-oiled machine, so sometimes you feel like a number while in the office, but Dr. Bromer and my LPN make me feel human.

I am in a same-sex marriage, and I'm a good candidate for IUI so the one cycle we've done so far has been natural with trigger shot. He gave me the option to go without trigger, but I wanted it. We may change protocol next time, but we'll see.

We have some coverage, so each office visit was a $40 copay. Counseling with a social worker for using 3rd party genetic material was out of network and was $300. Natural Cycle IUI is $1200 per cycle, but we were approved for the Shared Help Discount at 15% (income based), so natural cycle IUI for us is $1020. Medicated is $1800/cycle ($1530 for us). Sperm is separate. We do not have insurance coverage for IUI. HSG was covered, as was all lab work.

I've seen Dr Doyle on 3 occasions; 1 consult, 1 follow-up after testing, and during 1 monitoring appointment for my 3rd IUI. He was very thorough in explaining the testing, the results, the treatment plan, and the odds of success. He was also very compassionate and positive when discussing the treatment plan and the failure of the first 2 IUIs.
It is pretty easy to get an appointment [at Shady Grove Fertility] and the doctors/nurses/staff all seem very compassionate. It is a large clinic with many patients per day so my doctor did not perform any of my monitoring or IUI procedures except that he checked in during one monitoring appointment for my 3rd IUI attempt. The other sonographers/technicians/doctors were great so it didn't bother me that he wasn't at all of my appointments.

My treatment protocol is up to 3 medicated IUIs using Clomid (75mg for the first IUI, increased to 100mg for 2nd and 3rd) on days 3-7, Ovidrel trigger shot to induce ovulation, and Prometrium (progesterone) vaginal inserts after the IUI until the pregnancy test. The rationale is that we conceived our first child on our own in less than a year and the testing showed some mild male factors so IUI would bring the sperm closer to the egg. He prescribed Clomid because I am 39 so the hope was that 2 eggs would be released at ovulation increasing the odds that one of them would be of good quality and get fertilized. We are in the two week waiting period for the 3rd IUI. The first 2 did not work.

Shady Grove Fertility has high success rates. It is pretty easy to get an appointment and the doctors/nurses/staff all seem very compassionate. It is a large clinic with many patients per day so my doctor did not perform any of my monitoring or IUI procedures except that he checked in during one monitoring appointment for my 3rd IUI attempt. The other sonographers/technicians/doctors were great so it didn't bother me that he wasn't at all of my appointments.

Each IUI cost $1800 plus the cost of the medications which was ~$250-260. My treatment is not covered by insurance. Most of the testing was covered. I think we paid ~$200-300 out of pocket for all of the testing procedures for copays/coinsurance/deductibles.

I couldn't get a straight answer from her [Dr. Barbara Osborn] about nutrition/supplements to before/during egg freezing. Overall I sometimes felt like I was talking to a customer service or sales rep instead of a doctor - she just was unable to be personable...bedside manner isn't great, but it's not the worst I've ever seen...Doctor expected ~13 eggs per cycle, of which ~9 would be mature. First cycle was way less successful than expected, so they used the same protocol but increased the dosage significantly.
There were some issues with communication [at Shady Grove Fertility], and one very serious error where Rachel had a trainee nurse call me, and she gave me the wrong instructions for medication changes that night. Luckily I had to contact Rachel for something else that day and I was able to catch the error, but if I hadn't, I likely would have lost that entire cycle...$12,500 for an egg freezing cycle: 20 mature eggs or 4 cycles, whichever came first. For 3 cycle, I paid the pharmacy ~$15K. SGF also tried to bill me for blood tests and a consultation with the doctor in between cycles. I was nickel and dimed.

Dr. Osborne's bedside manner isn't great, but it's not the worst I've ever seen. Fertility clinics seem to be extremely nervous about getting a patient's hopes up, which means they are so neutral that they veer into actually withholding information, and being obtuse. I found Dr. Osborne to be this way. For example, I couldn't get a straight answer from her about nutrition/supplements to before/during egg freezing. Shady Grove as a whole is too large and the way they are set up means less personalized care. Communication should have been better, and errors on the part of the nursing staff would have been less likely with a smaller practice. The financial department has no idea what they are doing and I spend several thousand more than I needed to due to their lack of attention and individual care. SGF is a for-profit business and it shows.

Age: 38, AFC: 18, AMH: 1.58. Doctor expected ~13 eggs per cycle, of which ~9 would be mature. First cycle was way less successful than expected, so they used the same protocol but increased the dosage significantly. Then 3rd cycle they used a different protocol. First 2 cycles: birth control, then antagonist protocol (Gonal-F, Menopur, Cetrotide, Leuprolide trigger). 3rd cycle, no birth control, estrogen priming instead, then same antagonist protocol, except HCG instead of Leuprolide trigger. 1st cycle: 5 eggs retrieved, 3 mature 2nd cycle: 12 eggs retrieved, 8 mature 3rd cycle: 14 eggs retrieved, 9 mature

Overall my nurses were good. I was however disappointed when my primary nurse was promoted to another location and I was abruptly switched to a new nurse with no transition period or explanation beyond, "you're transitioning to a new nurse today." There were some issues with communication, and one very serious error where Rachel had a trainee nurse call me, and she gave me the wrong instructions for medication changes that night. Luckily I had to contact Rachel for something else that day and I was able to catch the error, but if I hadn't, I likely would have lost that entire cycle. I was shocked at how dismissive and non-apologetic Rachel was about the error.

The clinic is waaaaaay too big, and people are treated like a number. Read your contract VERY carefully and make sure you understand exactly what is and is not covered. If they ask you to do an additional test or appointment, make sure you check to see if it is included, otherwise you will be stuck with an extra bill. The financial department is also a complete mess. They couldn't have cared less about my questions and they were unresponsive, and sullen on the phone. They didn't do a good job of figuring out my insurance coverage, and I had to make a number of additional calls to my insurer to make sure everything was correct. They also advised me to not bother to check the prices of medications with other pharmacies, since they had a deal with a specific pharmacy. After I had already bought the drugs, I talked to other SGF women who had found drugs from lots of different pharmacies for cheaper. My nurse also did a terrible job of helping me to order the exact right amount of medications for my last cycle, which means I have unusable drugs in my fridge now. They let you donate them back to the clinic, but you can't return them, which is ridiculous. I felt really manipulated and take advantage of by the end of my care at SGF. The care I received from monitoring nurses and retrieval staff was excellent, however.

On the third cycle, we started doing monitoring through Shady Grove, meaning bloodwork and ultrasound to track ovulation and introduced 50mg of Clomid to the cycle. After this cycle was not successful, Dr. Roth suggested we do an HSG test to make sure there were no blockages in my fallopian tubes and that my uterus was normal. After getting normal results back from that test, we started our 4th IUI cycle.... ultimately decided to try IVF. I began the cycle taking birth control to suppress natural ovulation. We also used Menopur (75 IU), Follistim AQ (200IU), and Ganirelix (250MCG). We used Lupron (1mL) for the trigger before egg retrieval. After the Fresh Embryo Transfer, I began taking Estradiol (2mg tab) twice daily, and Endometrin (100mg tab) 3 times a day. This transfer was not successful. I have just started taking birth control again for the frozen embryo transfer and will begin Estrace/Estradiol 2mg tab 3x a day soon.
We've had 3 nurses over the course of our treatment at Shady Grove...I think, now that they have figured out the staffing changes, that both nurses we have dealt with are doing their best to make us feel like patients and not just numbers. Overall, our experience has been a positive one... It is a little bit of an inconvenience that the Frederick office is only able to do bloodwork and/or ultrasound and I have to travel for any other type of procedure. Initially, when we started this process over a year ago, they were rearranging their staff and we went through 3 nurses before we landed with Janine.

Lauren is supportive, empathetic and professional. We've had three treatment consultations with her. In each meeting, she clearly stated where we were in the treatment process, what the next steps or options were, and any risks associated with procedures. She would check in with us to make sure we understood everything and was able to do this without us feeling patronized. We asked many questions and Lauren always made sure we had as much information as we could before making any decisions. We felt/feel that she gave us her honest opinions and she supported us in each decision we have made thus far. Whether that be opting in or out of genetic testing, choosing to continue IUI after our miscarriage and switching to IVF, Dr. Roth has been supportive of each decision. She has also been quick to respond to any messages or emails we have left with questions. She never makes you feel ashamed or stupid for asking questions, even if they are as silly as "is it okay to dye my hair?" During procedures (we have done multiple IUI's and one fresh embryo transfer with her and will be doing a frozen transfer at the end of October), Lauren talks through the procedure as it's happening. This was especially helpful and comforting for my wife and me. I know my wife would have been asking a million questions if she had not been walking us through it. After our miscarriage, Dr. Roth was sympathetic and gentle about asking when we wanted to proceed to the next step. We have never felt rushed or pushed in a certain direction. We started this journey without medications. The first two cycles of IUI were natural. At the time we started the process I was 29 and had no health issues that would indicate the need to use anything else. We also used at home Ovulation Predictor Kits (OPK) to track my LH level and predict my ovulation window. We used the Ovidrel trigger shot starting in cycle 2. On the third cycle, we started doing monitoring through Shady Grove, meaning bloodwork and ultrasound to track ovulation and introduced 50mg of Clomid to the cycle. After this cycle was not successful, Dr. Roth suggested we do an HSG test to make sure there were no blockages in my fallopian tubes and that my uterus was normal. This cycle resulted in a chemical pregnancy. 5th IUI we also introduced an FSH injectable (Follistim AQ, can't remember the dosage). On the 6th IUI cycle, we continued with Clomid and Follistim, increasing the dosage slightly on the Follistim. We were successful in this attempt and found out we were pregnant on New Year's Eve. I made it 14 weeks and had a miscarriage in March of 2019. We used April as a recovery month and did a saline ultrasound in May to confirm that all the tissue was removed and my uterus was clear to try another IUI. We continued with the protocol that we used in the 6th cycle except I used Letrozole 2.5mg instead of Clomid because of a national shortage. This protocol was successful for us so Dr. Roth felt that we didn't change it. After this cycle was negative, we discussed our options with Dr. Roth and ultimately decided to try IVF. I began the cycle taking birth control to suppress natural ovulation. We also used Menopur (75 IU), Follistim AQ (200IU), and Ganirelix (250MCG). We've had 3 nurses over the course of our treatment at Shady Grove. Allison Catalani was our initial point of contact and she was organized and helpful. She was only our nurse for a few weeks (staffing changes). Throughout the IUI process, our nurse was Jody DeRaimo. Jody was friendly and knowledgeable, however, I did feel that she could be unorganized at times. She would ask me what medications I was taking and what kind of cycle I was doing or say she was going to send something to the pharmacy that day, but it didn't get sent. It felt like she had too much on her plate. In comparison, our IVF nurse, Toni Benjamin is "on the ball." She is very organized, and she has been able to answer all of my questions without having to ask or confirm with someone else. She reminded us to get our sperm shipped to the clinic, and then followed up with the Rockville office to make sure it got there. She sent medications to 3 pharmacies so we could get the lowest cost. She has been very nice to work with thus far.

We were lucky in that our insurance covered 6 IUI cycles. However, we had a $50 copay for any visit to the clinic regardless of the type of appointment, and the IUI's were only covered after we met our deductible. Medications were partially covered as well. The HSG test was not covered, and that was $350 up front. The billing for this was a bit confusing because Shady Grove's system was a few months behind, so even though we paid up-front for the HSG exam, they applied that money to other charges on our account. In the course of the IUI treatment, we spent $16,000 (not including sperm). As we moved to IVF treatment, nothing is covered by our insurance. We opted for Shady Grove's Shared Risk Program, which is $19,500.00 plus the cost of medications (so far $2,331.00).

When we asked about the number of embryos, Dr. Roth explained to us that because of my age (30) if we did multiple embryos we would only be increasing our chance of twin rather than increasing the chance of pregnancy. She felt that the odds were good to transfers only one since the rate of pregnancy for someone my age is around 50%.

I think if she had chosen to be more empathetic and compassionate with her words, we might have continued our work together.

Despite having a very busy practice and being its medical director, Dr. Levens frequently goes the extra mile to answer detailed and numerous questions, calling ahead of and after treatments, and being available to do procedures himself whenever possible. He listens carefully and attentively so I have never felt rushed during consults or even with middle-of-the-hallway questions during monitoring. Dr. Levens encourages patients to be engaged in their care and always invites follow-up inquiries as needed...he has a terrific bedside manner. Infertility treatments can make male partners feel marginalized yet Dr. Levens' attitude and soft-spoken nature are very approachable to men as well and he puts them at ease.
I liked the flexibility of being able to go to any of [Shady Grove Fertility's] numerous locations (even in several states) as medical records are linked. Overall, getting an appointment is easy especially for monitoring, which can be done in different centers across town rather than needing to be at one's primary clinic every time. Having said this, I found it necessary to book a post-treatment follow-up appointments with my RE weeks out (typically at the start of a cycle) if my goal was to not miss any time due to scheduling availability. While getting a phone consult to regroup was an option, in-person consults had to be scheduled further out. Even though one has to medically qualify, SG's programs such as Shared Risk allow for a certain sense of security to the stress of many unknowns associated with IVF cycles.

From the first time I met Dr. Levens, it was very clear that he makes a personal investment in the care and well-being of his patients. I tend to have a lot of intricate questions and he regularly engages in answering these in sufficient detail. While Dr. Levens has a game plan with treatments, he is open to patients' input and, if so desired, will discuss the pros/cons of what one might have found in researching the latest protocols or merits of various options. I have recommended Dr. Levens to numerous friends and acquaintances as he has a terrific bedside manner. Infertility treatments can make male partners feel marginalized yet Dr. Levens' attitude and soft-spoken nature are very approachable to men as well and he puts them at ease. Although this stage of the relationship deepened with IVF (and was less so during the TI/IUI phase of my treatment), Dr. Levens frequently calls ahead of procedures if he isn't the one on-call that day and even after to offer support/encouragement. As the complexity of my clinical situation expanded, Dr. Levens' personal attention to my case increased and he communicated updates to me much more often rather than relying on the nursing staff. When one egg retrieval went particularly awry, he updated me regularly on what was going on. It was clear that Dr. Levens was as interested in the outcome of my medical care as my husband and I--further, he even commented on how personally took this clinical setback so Dr. Levens is definitely in the trenches with his patients. Through my several years as his patient, I particularly appreciated how Dr. Levens sought to keep as many options open for his patients as possible (e.g., endometrial prep for a fresh transfer just in case we decide to go for this option on day 5 vs do a freeze-all, go/no go options on PGS testing, etc.). The language a person chooses to use can say a lot about one's true take on a situation and Dr. Levens speaks of 'we' vs 'you' as in 'we are in this together' and that shows up in every aspect of the care I received to date--infertility really is a team sport.
What's one piece of advice would you give a prospective patient of Eric Levens at Shady Grove Fertility?
Although he has a very caring nature, Dr. Levens tends to have a more pessimistic/realistic view of the situation so be mindful of this. For instance, Dr. Levens regularly subtracts 10% from assorted success statistics I've seen (e.g., chance of implantation, odds of pregnancy from procedure, etc.) so consider this when hearing both good news (it's likely better) or bad news (it might not be as bleak). Dr. Levens is also really approachable so do take him up on the offer of reaching out with questions in between consults and/or during monitoring as he will make time to speak with you. As with any infertility treatment, be your own advocate.

Despite having a very busy practice and being its medical director, Dr. Levens frequently goes the extra mile to answer detailed and numerous questions, calling ahead of and after treatments, and being available to do procedures himself whenever possible. He listens carefully and attentively so I have never felt rushed during consults or even with middle-of-the-hallway questions during monitoring. Dr. Levens encourages patients to be engaged in their care and always invites follow-up inquiries as needed.
Describe the protocols Eric Levens used in your cycles at Shady Grove Fertility and their degree of success.
I did 2 medicated timed intercourse cycles with Clomid, an IUI with Clomid and supplemented with Gonal-F, 2 non-consecutive IUIs with Gonal-F, an antagonist IVF (Gonal-F and Menopur with Cetrotide followed by HCG trigger), an agonist IVF (Lupron, Gonal-F, and Menopur with HCG trigger), an antagonist IVF with a dual trigger (Gonal-F and Menopur with Cetrotide followed by HCG and Lupron triggers), and 2 estrogen priming antagonist cycles (i.e., estrogen is used to prime follicles mid-luteal phase ahead of a stim cycle the following month). After 5 IVF stim cycles, I finally got pregnant from my 2nd estrogen priming cycle. During this time and ahead of the FET, I also used CoQ10/ubiquinol (~6 months), acai berry (~2 months), and acupuncture (~5 weeks ahead of transfer). For the FET, I was on PIO, vaginal Estrace, low-dose aspirin, and Lovenox. In terms of rationale for stim protocols, we were working on reaching greater levels of egg maturation by various mechanisms in hopes of making more embryos. My last cycle yielded 2 AA blasts (a personal record) of which 1 became my daughter.
Describe your experience with your nurse at Shady Grove Fertility. (Assigned nurse: Layluma and Chelsea)
I've been in treatment for so long that managed to have 2 nurses. Layluma was fantastic--very sweet and caring--both as my primary nurse (for TIs and IUIs) then we kept in touch upon her promotion to leading the surgical team. Chelsea took over as my primary nurse thereafter and has been great about answering my many questions promptly and patiently. During a particularly stressful time during my doctor's rare day off, she even went the extra mile of getting timely guidance on protocol modifications from an RE late on a Friday afternoon. As a testament to her approach to patient care, I decided to stay with Chelsea even after she switched to another office. Since most of your clinical relationship is with the nurses, I was fortunate to have had a positive experience with a responsive team.
Describe your experience with Shady Grove Fertility.
Shady Grove's (SG) strength is in its reputation and size as apparently the biggest fertility clinic in the nation. With experienced REs across multiple centers, the within network expertise is available to guide treatment plans in difficult cases. I liked the flexibility of being able to go to any of its numerous locations (even in several states) as medical records are linked. Overall, getting an appointment is easy especially for monitoring, which can be done in different centers across town rather than needing to be at one's primary clinic every time. Having said this, I found it necessary to book a post-treatment follow-up appointments with my RE weeks out (typically at the start of a cycle) if my goal was to not miss any time due to scheduling availability. While getting a phone consult to regroup was an option, in-person consults had to be scheduled further out. Even though one has to medically qualify, SG's programs such as Shared Risk allow for a certain sense of security to the stress of many unknowns associated with IVF cycles. While I haven't used SG's Shared Risk program, either party (clinic or patient) can choose to withdraw at any time in which case the patient receives a full refund. Although I learned of this too late to fully benefit when first seeking care, SG provides a free consultation for would-be patients who attend a free seminar. This is a terrific idea since some insurance companies make women jump through multiple hoops before being allowed to see a specialist yet timing of early diagnosis is so critical to improved outcomes. SG's major weakness is that patients do not see an RE at every appointment (far from it) and unless one really tries, it's possible--likely even--to have very little interaction with the clinician overseeing your medical care. I had to specifically work on getting attention from my RE between consults since SG is nurse-centric-in patient care, which is admittedly to a detriment relative to competing clinics. For instance, I had to actually ask to receive attention from my RE between consults via call-backs by request through my nurse or run-by Q&A during monitoring if we happened to intersect (his idea & willingness, by the way, for mid-hallway dialogue). Admittedly, my RE was quite open to grant the extra time and even came in purposefully early on numerous occasions to do my egg retrievals himself. Were it not for my RE taking an interest in seeing me succeed in having a baby, I would have left SG for one of the several competing clinics offering more ready access to its REs. I further think that much can be missed by having REs only analyze images rather than being there to see results firsthand and would urge SG to restructure its practice towards a greater fraction of direct patient care.
Describe the costs associated with your care under Eric Levens at Shady Grove Fertility.
I am fortunate enough to have had insurance coverage for infertility treatments. With BCBS for IVF, my costs were usually $5k-$7k for medications, and $2.5k+ for procedures. Via this insurance, patients could bank embryos up to a $25k ART procedure max and $15k medications max (medications coverage was charged at a much higher rate by the pharmacy such that this sum only lasted for 0.5 of an IVF cycle). With my new insurance through Aetna, coverage of procedures and medications varied by whether met the high deductible ($2.8k for family) but then medications and procedures were a reasonable copay. After hitting the OOP max ($4.3k for family including deductible), then everything became free for the remainder of the year. However, this insurance plan has a 4 IVF cycle lifetime max and the insurer directed whether patients were covered for each procedure (e.g., no embryo banking possible). Overall, we spent $25k+ in fertility treatments and medications to have our daughter.

As most REs, Dr. Levens would recommend transfer of a single blastocyst especially following PGS testing although is willing to consider multiple embryo transfers when clinically indicated (e.g., untested embryos, following past implantation failures, advanced maternal age, diminished ovarian reserve, etc.).

While overall treatment plans ran smoothly, there were a few instances with room for improvement. The front desk staff didn't always have accurate information on which RE was scheduled to do a particular procedure even a day or two out. As a planner, this kind of information was important to me especially since infertility is a process where patients have so little control over anything. When my RE ordered the use of a dual trigger protocol, I received conflicting instructions from the weekend nursing staff of when the two triggers needed to be done relative to each other and the dosing. Although this got resolved because the day of trigger ultimately fell on a school night, if I were not the type of patient to self-advocate and seek out answers then that cycle may have been jeopardized. For one of my embryo transfers, I did not get notification ahead of the procedure how many embryos survived or their status (e.g., morula, blastocyst, etc.) until we were already in the procedure room. The RE on-call even already prepared paperwork with her plan before indicating to my husband and me what were our options. The RE did end up giving us some time to figure out what to do without pressure but I would have appreciated being informed the morning of about the status and grades of my embryos so that could be in a better place to make a clear-headed decision. Further, calls to inform of bad news were on a few instances saved until the end of the day or even after hours. While one has to admire the fact that my RE wanted to give difficult news himself so as to be available to provide better guidance than the nursing team, the wait was excruciating and unnecessary. A better approach would have been for a nurse or even the front desk can do an initial call or even send an email to say that the RE will call to discuss the results. Lastly, the financial department is very much hit or miss so incorrect information was provided on a few instances. For instance, I was told that PGS testing was covered for at least the biopsy; however, this ended up being denied about 9 months later by insurance. In another instance, I was given the green light to start an IVF cycle despite the fact that the financial department could not confirm whether my insurance had enough to the max coverage sum to fund another IVF cycle. Instead, I had to piece together this mystery myself and then pull out of stimming upon having started the cycle because--to the best of my understanding after many, many calls to all responsible entities--insufficient insurance coverage funds remained. Although it has been over a year, this situation still unsettles me greatly as this could have cost me the opportunity to have another child. In speaking with others, discrepancies of various kinds with SG's financial department are not a rare occurrence.

When we initially met Dr. Steingold for a consultation, we had already been through 3 rounds of clomid with my OB. So, we first completed the HSG and initial ultrasound. I also told him I noticed less cervical mucus with clomid, so he also did a post-coital test (although he said they tend to do less of those because they are more outdated/not as accurate). He decided it couldn’t hurt if it was something I was concerned about. HSG went well; Fallopian tubes were clear. Post-coital went well (it was really cool to look in the microscope!). Ultrasound showed something near / on my right ovary. (He was out of town for this, so it was another doctor who performed it, and he was very vague about it, but just told me I might need to get surgery which worried me.) Dr. Steingold followed up with a second ultrasound with him to see what was going on before starting any treatment. Second ultrasound proved clear, which was confusing, but he was very happy with. He said we will continue to do ultrasounds to make sure whatever was there was gone — and to monitor egg size. Third ultrasound was CD 14 proved to be clear again, and egg size looked good with one Letrozole pill, but got a negative pregnancy test. Fourth ultrasound looked good as well. This round was two Letrozole pills, a trigger shot on CD 16 for ovulation, and IUI on CD 17. Just received negative pregnancy test today, so we are entering into another 2 letrozole pill / IUI round before we change to different dosage or shots with IUI for third IUI round.

The one thing that initially made me a little sad, was he stated in the beginning that he is a fan of single births and that he would not implant more than one embryo. She also went above and beyond to speak with the broker for my husbands insurance to determine exactly what, if anything, was covered. Overall, it’s been a great experience.

Dr. Beall adjusted my medicines throughout the process, as needed, based on my hormone levels...Given my age, my doctor recommended transferring two embryos. I was in agreement with her...bedside manner is very kind and gentle...She gives unbiased information, details, and facts, which I like, without pushing you to choose a specific path or process. My doctor decided to do a fresh transfer of my own embryos on Day 6 instead of Day 5. She told me the staff [at Shady Grove Fertility] (working the weekend, so not the normal staff) would call with a time to be at the clinic on Monday morning. I never received a call. So I had to talk to my doctor on Monday morning and she told me to just go in and they would be ready. However, I waited all day Sunday for a call to update me on the time to go in for my transfer - and no one ever called. That was frustrating, as I tried to reach someone Sunday afternoon and evening and was told that someone would call (they just never did).

Be sure and have a list of questions and have done a little research on your own. Talk to others who have gone through this process. I've learned a lot of helpful tips or things to ask from other women who have used fertility doctors.

Dr. Beall's bedside manner is very kind and gentle. But SGF overall feels like a factory. They are efficient with visits and check all the boxes, generally, but you do feel like just one of many patients most of the time.
Describe the protocols Stephanie Beall used in your cycles at Shady Grove Fertility and their degree of success.
I did two rounds of IVF with a sperm donor. Both treatments were very similar. I used birth control for three weeks before stopping and then starting daily injections. I injected menopur and follistum daily for approximately 10 days, and added in a daily injection of ganarillix for the last five days or so. I went into SGF for an ultrasound and bloodwork every other day until about day 7 or 8, and then went daily. My hormone levels responded well to the hormones, and the tests all went well. Dr. Beall adjusted my medicines throughout the process, as needed, based on my hormone levels. The final trigger shot, given 36 hours before egg retrieval, was pregnyl. Egg retrieval went smoothly, with no adverse reactions to the anesthetic. The first round of IVF, I did PGS testing on the two embryos that reached blastysis, and both were found to be abnormal and were not transfered. The second round of IVF, I chose not to do PGS testing and did a fresh transfer of the two embryos that reached blastysis. In preparation for that transfer, I took a daily pill and used suppositories three times a day for progesterone and estrogen (estrace and estridol I think were the names). I stopped all medicines two weeks after the transfer, when my blood test was negative for pregnancy. Currently, I am looking into using an embryo donor or egg donor.

Casey was very responsive to messages I left, and patiently answered any and all questions I had. She was quick to find out information from Dr. Beall, if needed, and also helped facilitate communication with the financial coordinator at SGF. She's been wonderful.

SGF is very well-known in this area and has a good success rate. I have multiple friends and acquaintances who have used them for fertility treatments. That is why I chose to go with them. However, you do feel like another number at times. The staff has all been very friendly and personable! But it's a factory and you do feel like women and couples are constantly in and out.

They were easy. I was in early and didn't have to wait long. No complaints.
Describe the costs associated with your care under Stephanie Beall at Shady Grove Fertility.
I paid for a multi-cycle IVF program, which gave a small discount for paying for two IVF cycles at once. That cost $23,500 including the PGS testing I elected to do ($18,500 without PGS). In addition, medications were approximately $4,500 per IVF round, or $9,000 total. And the sperm donor cost about $2,000 total (two vials). In addition, I had to pay for a therapist consultation ($300, insurance partially covered and I think my cost was $125) and a few other bloodwork and ultrasound tests that insurance generally covered. I also elected to do acupuncture during this whole process, and that was $45 per visit (found the practitioner on my own and not through SGF). Overall, the cost of two IVF rounds cost me about $37,000 out of pocket. I am not pursuing donor embryo or donor egg for my next step.

My doctor decided to do a fresh transfer of my own embryos on Day 6 instead of Day 5. She told me the staff (working the weekend, so not the normal staff) would call with a time to be at the clinic on Monday morning. I never received a call. So I had to talk to my doctor on Monday morning and she told me to just go in and they would be ready. However, I waited all day Sunday for a call to update me on the time to go in for my transfer - and no one ever called. That was frustrating, as I tried to reach someone Sunday afternoon and evening and was told that someone would call (they just never did).

My husband sperm came back a very high count so he was fine my test came back that my rate tube was completely blocked with endometriosis . At that point we had another sitdown meeting face-to-face with him which I liked he was with us every step of the way . He suggested that we do IVF and he explained the reasons why . We have been trying to get pregnant for over three years we had had one miscarriage and one ectopic pregnancy. Ivf would be the safest and best route for our family . I was given Esterase tabs and progesterone injections daily. We had to do several ultrasounds I was given a medication to have my follicles ready so that we can do an egg retrieval. Again Dr bromer was the one doing the procedure of the egg retrieval I was very emotional and nervous but he was so kind and right there with me the whole time explaining to me what he was doing . The lab fertilize my eggs and we had to wait to see if they took . Dr Bromer Called me himself to tell me that we had six that we were going to keep frozen. Then we did a mock transfer. And then we did the actual embryo transfer. They work very well with your schedule even the receptionist was very kind and understanding . The doctor was there for all of my monitoring it was very reassuring to see his face and for him to take a look at everything and let us know how the progress was going

The cost for my medications was around $1200 . My co-pay was $20 for every visit . I had to travel three hours for the procedure so I had a lot of money in gas and food . We had an antagonist protocol [with Dr. Silva]. I did birth control pills to prime. Once we started stims, I was on agonal F and Menopur. Cetrotide was added later. I did a Lupron trigger. I ended up with 14 Mature Eggs, 13 fertilized, 6 day 5 Blasts and then 4 PGS normal...He really cares about his patients. I’ve never had a problem getting an appointment and the staff has always been accommodating when they could be. By far the worst part of the process so far has been the financial aspect as the Annapolis office doesn’t have a financial coordinator on site.
How was your experience with Jeffrey McKeeby at Shady Grove Fertility?
Dr. McKeeby displays a great deal of experience and knowledge. He thoroughly answers all questions and provides examples (and drawings!) to make sure we understand and feel comfortable. Early on in our treatment, Dr. McKeeby built a more personal relationship with us and tried to relate to us about things we had in common, which was helpful in building an overall positive rapport. We’ve had ups and downs so far in our treatment, and Dr. McKeeby has had to communicate difficult or disappointing news to us on multiple occasions. Although sometimes he can be very blunt/direct, he recognizes when we need him to communicate information differently and he will re-state his point. After a difficult first cycle which didn’t end the way we had hoped, he was positive/reassuring and recommended a change in protocol to try and refine the results and increase our chances of success. While it’s too early to say how things will end, we trust his opinion and are very satisfied with our interactions with Dr. McKeeby to this point.

The first IVF cycle we did was a “standard” protocol - baseline bloodwork and birth control for 2-3 weeks, begin injections of stimulating hormones and attend routine monitoring appointments (bloodwork and ultrasound). Trigger shot of HcG and a scheduled egg retrieval 36 hours later. Although we achieved pretty good results with a decent number of eggs retrieved, we did not have any embryos to transfer this cycle. Dr. McKeeby explained that we’ll change up the protocol for the next cycle in the hopes that we have a more efficient stimulation synchronizing follicle growth and hopefully resulting in more (and better quality) eggs retrieved. Additionally, after the first IVF cycle Dr. McKeeby referred us to another Shady Grove physician to gather some additional information about the male component and to make sure we’re taking the best approach for our situation.

Beth is amazing - she is organized, responsive, compassionate, knowledgeable, and she truly goes above and beyond! She has made us feel cared for and supported throughout the process. We were initially assigned to a different nurse but requested to change to a new nurse very early on in the process due to a disconnect in communication styles and expectations.

Dr. Sasson took on our case after a very bad experience at a previous clinic. He walked us through our lab results and the plan he has created. Throughout our care there have been some complications. Dr. Sasson was there on the phone calling checking in, and taking care of every detail. We feel so lucky to be in his care and know one day he will be the reason we have a child. He is truly a blessing and a very kindhearted intelligent individual who we have full confidence in!

[Dr. Devine] definitely has a direct communication style but I have never felt like a number. During my appointments with her, she was always patient enough to answer my questions, and I tend to have a great deal of questions. :) She also provides advice that seems to come from a place of care and concern. I felt more “connected” with her after the first meeting. That also might have been because I was overwhelmed about the process.
I am on round one of IVF with donor sperm [at Shady Grove Fertility]. My protocol includes birth control pills and IVF “shots” to include the trigger shot. I have had to increase my Vitamin D. I also elected for PGA testing. Going straight to IVF instead of IUI made the most sense for my age (42)...[nurses are] very knowledgeable about the process and allowed for extra discussions about it. I’ve asked them both at different times to walk me through a notional timeline.
How was your experience with Kate Devine at Shady Grove Fertility?
Dr. Divine, from the beginning, has been honest and candid about my fertility options. Given that I am over 40, she shared the scary statistics without making them feel daunting. Instead, she highlighted all possible options including ones I had never considered (e.g., using donor eggs if mine are not viable). Although she has a direct communication style (think: straight, no chaser), I found her candor to be refreshing. You definitely know where you stand, both with her, and your fertility options. After every appointment, I was clear on what to do next. She has been thorough, rigorous and quick minded I implicitly trust that she will do everything possible to ensure that I have the best possible options during this journey. Given that I struggle financially and, like many, juggle other life stresses, it is a relief to have a doctor and medical team who don’t add unnecessary stress due to lack of bedside manner, incompetence, etc. Overall, I am pleased with Dr. Divine and I look forward to having her continue to guide me through this process!

Dr. Divine definitely has a direct communication style but I have never felt like a number. During my appointments with her, she was always patient enough to answer my questions, and I tend to have a great deal of questions. :) She also provides advice that seems to come from a place of care and concern. I felt more “connected” with her after the first meeting. That also might have been because I was overwhelmed about the process.

I am on round one of IVF with donor sperm. My protocol includes birth control pills and IVF “shots” to include the trigger shot. I have had to increase my Vitamin D. I also elected for PGA testing. Going straight to IVF instead of IUI made the most sense for my age (42)

I started with Sheila and ended up with Kelsey when Sheila left SGF. Both were very knowledgeable about the process and allowed for extra discussions about it. I’ve asked them both at different times to walk me through a notional timeline. They literally walked through each step very slowly and deliberately. I felt like I was not alone when we talk through the process. They were also very reassuring. I have appreciated working with them both

SGF is huge so it is ideal to have questions prepped in advance before making a phone call or laying a visit. Some appointments move very quickly. I would say that if you need to slow them down, advocate for yourself and slow the conversation down. It’s your money, your baby and your experience!

Overall, she’s been really nice, but at the beginning of our first cycle, they assigned us to a very inexperienced nurse who couldn’t even answer questions on the phone without checking with a colleague. When I called Dr. Timmreck to discuss, she tried to rush me off the phone saying, “I have patients waiting for me” as if I wasn’t a patient myself! First cycle: I don’t remember the dosages, but we did BCP for 2.5 weeks, then Gonal-f and Menopur injections for 14 days, at some point, they added in cetrotide injections, but I don’t remember exactly when. I did a Lupron trigger shot, then egg retrieval and 5 day transfer Second cycle: we had the Gonal-f and Menopur injections with the added cetrotide again, I don’t remember the dosages, but I do remember that this time it went on for 16 days. Lupron trigger and then egg retrieval. No eggs fertilized in this cycle, so no transfer occurred :( Third cycle in progress: we are on day 7 of Gonal-f and Menopur injections. They increased the Gonal dosage so we hopefully can trigger earlier this time.

This clinic is like a factory. The plus side: they have the system down pat, so you know exactly what to expect and can learn how to work the system to your advantage. The down side: it is EXTREMELY busy ALL THE TIME. They only have 2 blood draw chairs and 3 ultrasound rooms, so you could make an appointment for monitoring at 7:30am and not be seen until 9:00 or later. They do have several other locations in the area, so you can go to any of those locations to be seen if more convenient. But they are all just as busy!

I liked Dr. Sasson because all our our treatment was customized to us. I didn’t feel like he just threw the same medications/ treatment at everyone. We tried to be the one to preform all procedures as well and did both of our transfers. The absolute biggest strength of the Shady Grove Chesterbrook office is that they do everything there. Since this office has a lab, a surgery center, and weekend monitoring I never had to go anywhere else. I was in the surgery center at least 5 times and the staff always remembered us and was so comforting and nice. The same goes for monitoring, you see the same faces and they remember your case and don’t just treat you like a number.

I felt like a human because our treatment felt truly customized for us.
Describe the protocols Isaac Sasson used in your cycles at Shady Grove Fertility and their degree of success.
The first step was a hysteroscopy to correct a septum in my uterus. This surgery did not remove the full septum however. We were going to do IUI next but insurance wanted us to move right into IVF. We did IVF and frozen everything, then did another hysteroscopy for my septum. We did a frozen transfer of one embryo that failed followed immediately by a frozen transfer of two embryos that led to a successful singleton pregnancy. I was on Gonal F, Menopur, and cetrotide. I triggered with Lupron. For stims we started on low dose meds because I had a lot of follicles and did the lupron trigger for the same reason. This was successful in preventing OHSS. For frozen transfers I did del estrogen injections every 3 says and progesterone in oil injections daily until 10 weeks pregnant.

Efficient well oiled machine. We live in Raleigh so I would fly in and out same day and they always accommodated me. I was also able to do some monitoring at my GYNO locally and my doctors worked very nicely with them.

We paid 30,000 for a package that includes 2 retrievals, as many transfers as we want and covers some PGS testing. Some tests are partially covered by insurance or we had to pay our deductible. Our medicines are very expensive and not covered through insurance and one thing that does frustrate me is that they over prescribe.

Somewhere in between - the sheer numbers of patients going in and out of that clinic definitely made me feel like a number. However, Dr. Nair always made an effort to know me and my story.

3 unsuccessful IUIs. Then 1 IVF using antagonist protocol. Then microdose Lupron protocol with PGS for the 2nd. Then microdose Lupron without PGS - could only do a fresh transfer for the 3rd. Getting ready to start a Lupron stop protocol.

1. Always schedule the earliest appointment you can. They are efficient for sure, but if you have to get to work by a certain time, it can be very stressful on the days when they are not running on time. 2. Know your health insurance and DOUBLE CHECK everything that you are quoted from the financial counselors. And monitor your bills very closely. If I didn't understand what I do about health insurance, I think we would be out of a lot of money. There are 3 different people I've dealt with regarding payment and insurance on the exact same issue and no one seems to be communicating with one another, or really trying all that hard to fix any problems. I still have a credit on my account after I paid for a service that they assured me I owed - turns out I was right. But they won't let you start your treatment until you are financially "cleared" and you have paid in full. Just watch all of that very, very closely. And don't be afraid to second guess when someone else is asking for money - like the desk when you check out - the communication between parties is terrible there. I've paid I don't know how many co-pays that we unnecessary until I smartened-up and realized I could just speak up.

Before my IUI, My primary doctor found that my thyroid was WAY off (hypothyroid) and Dr Khan , knowing this, still followed through with the IUI that I paid for out of pocket. *I* then had to ask him after I learned that it failed, if my thyroid could have caused the IUI to fail, he said I was correct my thyroid wasn’t optimal for fertility and to go and seek an Endocrinologist...he was quick to wave off newer studies I’ve found. I let him because he’s the doctor, not me. My test came back with high FSH and possible clotting disorder. Otherwise “unexplained infertility”. I asked if I should take baby aspirin, “it’s not part of our protocol” I pushed back “will it hurt if I take baby aspirin.

I am all around disappointed. Honestly, from the start I was a little put off by feeling like a number. It’s the closest clinic to my house, so, I stayed when I shouldn’t have. Dr, Khan did take the time to hear me out most of the time when we had a meeting. But, he was quick to wave off newer studies I’ve found. I let him because he’s the doctor, not me. My test came back with high FSH and possible clotting disorder. Otherwise “unexplained infertility”. I asked if I should take baby aspirin, “it’s not part of our protocol” I pushed back “will it hurt if I take baby aspirin. Since it could help prevent another miscarriage?” “no it won’t hurt anything”. Alrighty fighting for clarity and treatment as an individual and it was only the beginning. One medicated timed intercourse failed. Then I set up for an IUI, our insurance doesn’t cover it, in Northern VA fertility treatment is very pricey. I then went to my primary doctor who ran regular tests and found my thyroid was off (hypothyroid). A few days before IUI I told my clinic about my thyroid levels they “‘made note of it”. Went ahead with the IUI. It failed. I then discovered that my levels were way off for fertility and could be a reason for infertility and early miscarriage. When I brought this up to doctor Khan he said “yes. Your thyroid is not optimal for fertility. If you’d like, go and see an Endocrinologist”. That was the last contact I had with this clinic. I won’t be returning. They took my money and instead of holding off on the IUI that was perhaps destined for failure from the start and waited for my thyroid levels to regulate, they did the IUI. And I had to figure out for myself that my thyroid being off needed to be taken seriously. So frustrating and disheartening. You must be your own advocate. Ask questions. Do your own research!

Do your own research. Be your own advocate. Ask all of the questions. Be persistent. Also, don’t be afraid to go somewhere for a second opinion.

From my understanding for timed intercourse and IUI - its all the same. I did not do any IVF rounds or egg retrieval’s. For T.I and IUI - take clomid or letrozole starting day 3 of your cycle. Then around day 12 you go into the office for blood work and ultrasound to count follicles to see if you’re ready to take the trigger shot (ovadril). Trigger shot and two days later go in for the IUI. Then follow up with progesterone suppositories until negative blood test. Continue them if positive. My lining was thin during my T.I. and I had to take estrogen suppositories until negative pregnancy test.

We struggled the first month when I had too many eggs with whether we should try timed intercourse and take our chances, and she was extremely professional and honest with her advice. Suggesting we think of life down the road, do we have room for triplets in the car? In our house? Can we feed them? She put things into perspective and was the voice of reason I needed at the time. I was grateful for her honesty. I appreciate a blunt approach when it comes to my medical care. She was also compassionate when I needed her to be when we could t do IUI.

They were always understanding and flexible. With traffic in the morning sometimes i would call if I would be late and they were always very understanding. Their receptionists knew you by name quickly. They offered very early appts which were helpful with our work schedules.

The very first statement Dr. Bromer made at our initial consultation was “Ok, I’ve reviewed all your previous records, but I want to hear it from you. What have you been through, how has this process been for you?” That was the first time I had a doctor ask me that. And not immediately look at my age and just shrug me and my husband off and say “oh you’ve got plenty of time!”
Describe the protocols Jason Bromer used in your cycles at Shady Grove Fertility and their degree of success.
I was started on Menopur 75 IU and Gonal F 150 IU as my stimulation drugs. I also took Cetrotide. Lupron was my trigger drug prior to egg retrieval. I was stimulating really quickly so my doses were lowered part way through the stimulation process. I ended up with 19 follicles, 9 eggs, and 5 embryos. Unfortunately only one of my embryos made it to Blastocyst stage. It was transferred on day 5. We were unsuccessful. We plan to initiate another cycle soon.

Jamie has been fantastic through this process. She has returned messages in a timely manner, has been very compassionate, and has tried to help us as much as possible in this journey. She even made sure to call at times that worked with our work schedules so my husband and I could get results together.

Our primary nurse coordinator has been responsive but very flat in her communication with us. On several occasions we looked to her for some critical additional information and she fell short. For example, I had to take progesterone shots for 10 weeks, and asked her for some tips on administering the shots. All she provided was a link to a video which we had already seen. Alternatively, during our transfer, we had access to another nurse who provided a wealth of useful information regarding the shots. Our nurse coordinator was prompt with her responses, but was not very good at going the extra mile to provide helpful information.

We paid $13k for donor eggs. Our insurance (Cigna) has been great thus far and has covered many costs. We paid $300 for the Clinic's counselor, and probably several hundred in various appointments and local monitoring (blood work, ultrasounds). We are still waiting for some claims to process through insurance.
