I think Dr Levens is one of the best doctors I have seen at Shady Grove. I just wish I had seen him earlier because my situation could have ended differently. During treatment, were you treated like a number or a human with Eric Levens at Shady Grove Fertility?
Dr Levens actually listened to me as a patient, and he explained issues is detail to better help me understand the issues. He is very technical and had great bedside manner.

He used an antagonist protocol which helped me to produce more eggs than previously. It as not something my previous doctor tried.

Mary Hartman was great. She was very organized and helpful.

They were first thing in morning and they were quick to get you in since mostly everyone is going before work. I had to drive an hour each way before work so I'd be there right at or before 7

It was expensive but was able to do shared risk program so if our ivf didn't work we could get a refund

Dr. bromer recommended one embryo but said at most two and asked us to decide after giving us all information

Dr. McClamrock is very matter of fact and direct, which can be a bit unsettling. But he knows what he is doing. He took time to answer all of my questions, although he did seem a little put out with all of the questions that I had....He recommended single embryo transfer....Although he can seem uncaring, he knows what he is doing....Dr. McClamrock is very matter of fact and direct....Easy to schedule, and calm inviting atmosphere.

I am fortunate enough to be fairly healthy, and should not have problems conceiving, so most of our protocol has to deal with my husband's issues. Dr. Khan explained what the process would be at the beginning if I had no issues, and after all of our testing explained again what to expect. I was surprised that the results of my genetic testing was not communicated with him prior to my follow-up consult, and he had never heard of the syndrome that I am a carrier of, but had he been given the information prior, I think it would have eased my comfort level, because he would have been able to speak knowledgeably to my concerns. It would have been nice after our first failed IUI to have him explain why we were trying the same exact protocol as before, but it's understandable to have a nurse call us and talk to us as well.

Be upfront about an issues you are, or believe you are facing. Dr. Khan certainly isn't a mind reader, but answered every question that we asked.

Menupur (started at 75, ended at 225) Gonal F (started at 150, ended at 225) 6 mornings of Cetrotide .25 HCG trigger 10,000 after 13 days of stimulation Then endometrin and estrogen after transfer. 11 mature eggs (mostly from lef tovary because right was damaged following previous ectopic surgery). 8 eggs fertilized 2 morula embryos transferred on day 5 -- neither had made it to blastocyst 1 blastocyst to freeze on Day 6

I had two separate nurses (was transferred halfway through), and both were great, although they seemed very busy. It was wonderful to communicate via email and phone, and they were both very clear with what, if any, changes needed to be made each day. I asked a lot of questions, and sometimes I felt like I was annoying them, but I always got the info I needed in the end.

How was your experience with Eric Widra at Shady Grove Fertility?
I went through 9 cycles of various treatments with Dr. Widra (the last two of which resulted in ongoing pregnancies spaced two years apart). During my 7th cycle, I had an ectopic pregnancy that eventually ended up rupturing (not the fault of Dr. Widra or Shady Grove fertility). Dr. Widra called me from out-of-town to express his disappointment about the ectopic pregnancy and then called me again when he learned that I had emergency surgery to deal with the ruptured ectopic. That really impressed me. Additionally, Dr. Widra always treated me well and like a human during office appointments and monitoring appointments. I never felt like he didn't care. Also, Dr. Widra never cancelled one of my cycles. Even if it looked like I might be maturing too many follicles (during timed intercourse cycles), Dr. Widra would adjust the medications and I was able to trigger with every cycle. I can't say enough good things about my experience with Dr. Widra and Shady Grove.
What's one piece of advice would you give a prospective patient of Eric Widra at Shady Grove Fertility?
It can take some time to get an appointment with Dr. Widra, so if you want to start treatment in May, think about calling for a May appointment in Feb or March. In my experience, thought, this was only true for initial appointments; once I was undergoing treatment, I had appointments with Dr. Widra that we scheduled only a week or two out.

My treatment for my first child was primarily dictated by the requirements of my insurance company (which provided coverage for IVF, but only if I tried a certain number of timed intercourse and IUI cycles first). But within those requirements, Dr. Widra first had me go through 2 cycles of timed intercourse with clomid and progesterone suppositories, and then switched me to injectibles (Bravelle) and progesterone suppositories when I had vision issues with clomid (I wasn't taking a high dose). Then I went through 4 cycles of timed intercourse with injectibles to fulfill my insurance requirements, and an IUI cycle with injectibles (Bravelle) and progesterone suppositories again to fulfill insurance requirements. The IUI resulted in an ectopic pregnancy that Shady Grove quickly treated, but still ended up rupturing and requiring emergency surgery. After that point, I was finally allowed to proceed to IVF, which produced two 5-day blasts (using Bravelle, menopur, HGH, estradiol, and endometrin) . One was using during that first cycle (and resulted in my son) and the other was frozen for future use. I did end up with over-stimulated ovaries, but I don't think this was the result of Dr. Widra being too aggressive with treatment (as I didn't end up with dozens of retrieved eggs), but is just something that happened. Shady Grove treated me with a blood thinner and an outpatient procedure to drain fluid. A year after my son was born, I went through a frozen embryo transfer with Dr. Widra using the embryo that was produced during my previous IVF cycle (as well as a combination of endometrin, PIO injections, and estradiol injections). This cycle was also successful and (fingers crossed) my daughter will be born in May 2016.

I can not say enough good things about my nurses at Shady Grove (Carrie Andrews and before her Sarah Evans). So on top of things, organized, caring....they both had all of the qualities that you would want in a nurse and never, ever let me down. And in the case of Sarah, we worked together for 8 cycles over 1.5 years and I threw so many issues at her, and she always responded quickly and thoughtfully. I only worked with Carrie for 1 (successful) cycle, but she also displayed these same qualities, which I am sure would have continued had I needed additional treatments or cycles.

We were prepared to begin IVF with PGD but became pregnant (again) -- naturally -- just before that cycle would have started. We did one last round of testing based on some new information I had researched relating to the MTHFR genetic mutation. Turned out I had such a mutation and I was put on a prescription folate ASAP (worrisome, because I was already a tiny bit pregnant when I started the folate and didn't know if we had discovered the problem fast enough). I also used progesterone suppositories (as I had under my OBGYN for my prior two miscarriages) during the first trimester. I had weekly ultrasounds and blood work to confirm the advancing pregnancy.

Timing was unfortunate -- I was scheduled for my HSG during Christmas/New Years when the Office was closed. So I had to go to a different outside Radiological center and SGF failed to send over my paperwork/referral info; my husband had to literally run next door (both offices were on K Street) and hand deliver my paperwork before I could begin my procedure. That was nervewracking and unexpected.

Not sure this site will let me review the other two docs, who I was NOT happy with. But he was my last stop--I thought I would get a chance to even try with my own eggs, but I wasn't. Even with my DOR status, I had gotten pregnant 3 times on my own. I think he knew that despite being at the end of my emotional rope, that I really wanted to use my OEs despite my intense fear of another MC. I'm not sure there is anything I would tell you about him specifically, only that the fertility road is exhausting, emotional, expensive, and really makes you question everything as a person and what kind of information you can trust. I think Dr. Mottla is completely aware of all this. In the end, I had to trust him. Had to trust somebody.

Dr. Mottla was the 3rd doc I saw. 2nd at Shady Grove. I did not like Dr. McKeeby (my first doc) and transferred up to Johns Hopkins. I did feel like a number there and had no real personal interaction. Dr. Mottla was my last stop and although I didn't like what I heard (and still don't totally believe the actual source of my issue was DOR), I am almost due with my first baby, through the use of DE and couldn't be happier.

Because he would not let me use my own eggs (my AMH was less than 0.01, despite getting pregnant multiple times on my own), none of the treatments I had researched were used. My DE treatment was pretty basic with estrogen/progesterone shots after my cycle had lined up with the donor's. At Johns Hopkins, we were doing natural, medicated cycles using Femara and then the ovidril trigger shot. Only 2 of my 4 cycles cooperated. They had terrible communication up there and I was always confused by what they were telling me. They would have treated me with my OE, but by then the 2 hour round trip ride and the complete lack of communication had thoroughly exhausted me. Mottla felt that while I could have a miracle baby, that any egg my body produced would be far superior to any eggs they could get from me artificially.

We went into a fresh IVF cycle knowing we would do ICSI. We did the normal stems and then added in Menopur to help the quality of my eggs, instead of focusing on the quantity. I feel like this was the change we needed. We went through stems and then found what appeared to be a hydrosalphinx (fluid filled tube) so we went egg retrieval and froze all embryos. I had that tube removed and then we transferred two embryos, resulting in twins.

Dew was just like Dr. Chang in the way of compassion. She was always willing to help us in any way and answer all questions. She was available to us at all times, and if she wasn't available for any reason, she made sure we had someone to contact during her absence.

We got a discount on that and then we applied for pharmacy discount programs for our meds as well. We did have to travel and stay at a hotel while there, but it was worth it.
Describe Frank Chang's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Shady Grove Fertility.
He had a preference of transferring 1 embryo, but ultimately left it up to us. He was supportive of our decision to transfer 2 embryos after we had suffered 2 losses and the distance of travel for us between our home and the clinic.

We were on donor egg protocol with estrogen and progesterone. Usually, This is a straightforward protocol, but my body had difficulty in responding. Dr O'Brien changed dosage and frequency of estrogen injections to assist my body in stimulating the uterine lining. In addition she utilized other different medications (estradial, viagra and tomaxifin citrate) throughout my cycles to stimulate my uterine lining to develop enough to support conception. She also performed hysteroscopies, D &Cs and endometrial biopsies to optimize our chances at conception . During my 5 treatment cycles, Dr O'Brien consulted her colleagues and was open to suggestions of her peers to assist us in becoming successful. When we came to the decision to proceed with gestational carrier, our carrier was assigned normal protocol of estrogen and progesterone and she responded appropriately.

Our insurance did not cover fertility treatment. We were enrolled in shared risk/shared donor money back guarantee program, which I believe was $29,000. In addition, my insurance did pay some costs associated with procedures, testing and surgery costs.

Dr O'Brien always followed Shady Grove Fertility protocol and suggested transferring only one embryo per transfer cycle. At our last cycle, our gestational carrier requested to transfer two embryos because it was our last attempt before stopping fertility treatment. Dr O'Brien reviewed her health, age and prior pregnancy details and agreed she was healthy enough to carry a twin pregnancy. She agreed to allow our gestational carrier to transfer two embryos.

Dr. Esposito never took our infertility seriously. During my first time with her, she said, " You just need a little nudge." " It just takes longer in your 30's." I asked her about getting donor sperm b/c I knew there must be something wrong with my husband's sperm and she said, " It won't come to that." I told her about how the urologist told us that my husband's sperm has been attacked by his antibodies and Dr. Esposito said that the Dr. she trained with didn't believe in the antibodies attacking the sperm.
Some nurses [at Shady Grove-Frederick] were really nice and compassionate but I didn't feel that way about Allison Catalani. I felt like I was always telling her I wasn't pregnant and she didn't believe me. She never even said, " I am sorry," when I told her that I wasn't pregnant and I got my period. There was no compassion. She acted like she thought I would get pregnant soon. She was also disorganized. She put me down for timed intercourse once when the doctor and I knew that we were doing IUI. Once, she called my cell and left me a message with another woman's test results. She also forgot to tell me that my husband had to sign a paper before an IUI and it was a real hassle to have him get it done before the IUI could happen.

I wrote a lot about Dr. Esposito in another earlier question. She also didn't pay attention to if the ultrasound showed old brown blood at the top of my uterus before an IUI. I found that that's not something the doctor's typically look for but it should be something the doctor's look for. I had one IUI where the nurse was about to do the IUI and she said, " You have some old blood at the top of your uterus." That should have been discovered days before the IUI and cleaned out. That wasn't the time to be told about it. The nurse did the IUI anyway but sperm don't do well trying to swim through old blood. Dr. Bromer finally told me that.
What's one piece of advice would you give a prospective patient of Melissa Esposito at Shady Grove Fertility?
She may under estimate your infertility. Don't go to her if you want IUI or if your husband had a successful reversal and you still can't get pregnant.
During treatment, were you treated like a number or a human with Melissa Esposito at Shady Grove Fertility?
Dr. Esposito never took our infertility seriously. During my first time with her, she said, " You just need a little nudge." " It just takes longer in your 30's." I asked her about getting donor sperm b/c I knew there must be something wrong with my husband's sperm and she said, " It won't come to that." I told her about how the urologist told us that my husband's sperm has been attacked by his antibodies and Dr. Esposito said that the Dr. she trained with didn't believe in the antibodies attacking the sperm. So she was implying she didn't believe in it either. I asked her what my husband could take to help his sperm and she said, " Clomid." I thought that was really weird. After I broke away from Dr. Esposito and went to Dr. Bromer, I asked him the same question and he answered it quite differently. He said my husband should take a supplement with L-Carnitine in it. Dr. Esposito never told me about Letrozole. She only told me I could take Clomid to mature more eggs for the IUI. Dr. Bromer told me that Letrozole would be a good choice for me to mature more eggs for IUI. For my first IUI with Dr. Esposito, she didn't order a blood test to check my hormones. She only wanted to go by what the transvaginal ultrasound showed. So the ultrasound make it look like the right day was day 11, but my body was not ready then. I even had old brown chunks of blood come out that day. When I had IUI's with Dr. Bromer, he always ordered a blood test and those tests showed that the right day in my cycle was the 13th day. Dr. Esposito didn't tell me that my husband should lose weight to help the sperm. My husband's sperm back then ( 2014) had a motility of 1+/2 which is below normal. In 2015, my husband started exercising and lost about 20 lbs. After that, the motility gradually increased until it was 2+/3 at the best. Dr. Esposito should have made a big deal about the motility. It is important with IUI. Dr. Esposito never mentioned that there could be a problem with my husband's sperm binding with my eggs and penetrating them. When I saw Dr. Bromer, he told me there was likely a problem with my husband's sperm being able to bind with and penetrate my eggs.

She wanted to use the simplest treatment. She did my first IUI without having me get the blood test to check hormones. She just wanted to see what the ultrasound showed. She didn't have me take Letrozole or Clomid.

Some nurses were really nice and compassionate but I didn't feel that way about Allison Catalani. I felt like I was always telling her I wasn't pregnant and she didn't believe me. She never even said, " I am sorry," when I told her that I wasn't pregnant and I got my period. There was no compassion. She acted like she thought I would get pregnant soon. She was also disorganized. She put me down for timed intercourse once when the doctor and I knew that we were doing IUI. Once, she called my cell and left me a message with another woman's test results. She also forgot to tell me that my husband had to sign a paper before an IUI and it was a real hassle to have him get it done before the IUI could happen. I would sometimes email her a question and she would email me back with as little words as possible. It seemed like she was just emailing as quickly as possible without really caring how she came across. Sometimes, she never got back to me at all and I finally found out from another nurse that I could email my doctor directly. Allison Catalani didn't tell me that. So then after that, I just emailed my doctor with questions rather than Allison Catalani.

Some nurses here are more compassionate than others. My nurse seemed to be on vacation a lot. Some doctors are better than others. Most nurses were more optimistic than they should be. Don't go into the fertility center thinking you will be successful unless you are willing to pay thousands for IVF. IUI doesn't usually work and the doctors and nurses won't tell you that.

I think IUI was like $200. I think a transvaginal ultra sound was $250. I always had a bill of about $500 when I got IUI.

Standard antagonist protocol - BCP followed by FSH, menopur and then cetrotide I think. Attempting to keep growth slow but steady, and keep all follicles in range. Aiming for fewer high quality follicles (as someone who could produce a fair number, but of poor quality). Standard trigger criteria, but we held off a day or two based on history, so things were slightly larger. On my FET, it was medicated, so BCP and then something (can't recall).

I think the strength is in the solid lab, significant experience and range of doctors to choose from. They do seem to use standard protocols, but that was fine. I'm not crazy about the BCP use, but it's a must - there's not a lot of flexibility in that area. I was able to get some concessions on some policies by getting Dr. Moon on board, and it was frustrating to have to do so. The standard weekend hours and multiple locations was good - no worrying about timing there. They generally had a range of appointments available, though weekends could be hit or miss. Some things were very standardized which generally helped it run smoothly. But there were times things backed up, with waits of 45+ minutes. The financial people may have changed in the past 2 years, but that was the worst/most maddening part. It may be less so if you're not using insurance, but I had to do a significant amount of leg work on running down pre-authorizations and getting statuses and poking and prodding. It was incredibly frustrating, and I would say that's the part that is most off putting about the clinic.

They [Shady Grove] arw good at making appointments that work for you. The phlebotomist are super nice and they are the only ones who remember your name. The receptionist are very rude and unprofessional... I signed up for a certain type of plan where you pay for so many and if they arw unsuccessful then you get reimbursed. I should have had like 6 treatments. After the first one they started sending me bills for every little thing. When I inquired about it they claim that that was not what plan I was under and that I was a pay as you go pt. Many times when I would go in for my blood draws or scans there was only standing room because there were so many patients. It was very impersonal. Everyone there is nervous and scared and possibly desperate and they just chat about whats for lunch and what they did for the weekend. Very insensitive.

She also left surgery to come down for our first ultrasound after our positive pregnancy test.
My nurse forgot to provide the trigger shot which resulted in less eggs retrieved. I still had plenty but was quite stressful... Shady Grove has multiple clinics in the DC metro area. Testing is very process driven, which I appreciated. I knew exactly what and when things were going to happen and I simply followed that process. Some folks might not appreciate being another cog in the fertility factory but I found it comforting...

I had several primary nurses during my tenure at Shady Grove; some were better than others. I had one, who is no longer there, who did not like to communicate over email. Giving my busy schedule, email was much better than phone. My suggestion would be to establish which method you prefered communication when first meeting with your nurse coordinator.

I still had plenty but was quite stressful. 2 - There was an active shooter near a clinic on a day I was scheduled for blood work and ultrasound. This resulted in the clinic closing. No information was provided to patients; the voicemail at the clinic was not even updated. I was not informed of what I needed to do until noon that day, which is after the clinics stop monitoring. I spoke to someone at the head office about lack of emergency preparedness and was not provided a satisfactory answer. For example, all retrievals and transfers are done in the Rockville office and I asked for plans if that office is closed. The response was simply, " we would never close that office."

He [Dr. Frank Chang] was amazing. Very thorough and pleasant. Listens to me and asked for my opinion and thoughts. Treated me with respect and remembered me when I went back to him for my 3rd child. He worked with me to ensure that I would have the best success and respected my wishes even when he didn’t necessarily agree and never made me feel like I didn’t know what I was talking about. He also gave me a lot of professional courtesy bc I work in the healthcare field.
There was a lot of turnaround with nursing staff [at Shady Grove Fertility- Rockville] They were all very competent, but each time I went back for another IUI, it was a different nurse. I didn’t really care though bc I had a plan set with a Dr Chang and that’s all that mattered. The nurses just followed our plan and it worked out great...Very busy office...Sometimes I feel like it’s jist all business, but ultimately I got what I wanted from them and that’s all that mattered. The office staff, nursing staff and medical staff made me feel that way. But never Dr. Chang.

What's one piece of advice would you give a prospective patient of Frank Chang at Shady Grove Fertility?
Be direct, speak your mind. Listen to him as well. He knows what he’s doing and he would never intentionally steer you the wrong way.
During treatment, were you treated like a number or a human with Frank Chang at Shady Grove Fertility?
He listens to me and asked for my opinion and respected my wishes/thoughts on matters. He trusted that I knew what was best for me.
Describe the protocols Frank Chang used in your cycles at Shady Grove Fertility and their degree of success.
Straight forward IUI using Follistism to grow my follicles to approx. 20-22mm. I get the best results once my follicles are nice and big. My other doctor didn’t thing i should wait that long for my follicles to grow and therefore i had unsuccessful IUIs before i had my successful ones. Ovidrel was used to trigger my eggs. After finding which protocol worked for me, we never steered from it and it resulted in 2 more successful pregnancies that led to 2 more healthy babies for me and my husband.

Just office staff not relating important messages to my first doctor. Dr. Esposito in the Frederick, MD office was very difficult and they wouldn’t believe me when I told them my period had started. They insisted that it couldn’t have started already. I had to call the office manager to get them to schedule a day 3 appt. And if I didn’t do that I wouldn’t have gotten pregnant with my daughter. That’s why I switched doctors midway through my IUI treatment. Dr. Esposito didn’t listen to me, didn’t respect me and treated me like I was stupid. She talked down to me even though I told her I was a nurse and I was very familiar with medical jargon and understood procedural methods.
