During our initial consultation, she took my personal history with care, answered every question I had -no matter how small, and laid out a clear plan of care.
During the hormone cycle, the nursing/ medication staff becomes your main contact, but Dr. Wiltshire’s attention to my case and quality of care did not waiver. When I had questions about my treatment, symptoms etc. via email or voice message, Dr. Wiltshire swiftly responded, with an attention to detail I’ve never experienced under any doctor’s care. Dr. Wiltshire and the supporting staff acted so synchronously that I was never concerned about falling through the cracks.

We initially requested a consultation via email/ the CU website and the initial consultation was promptly scheduled by her team. During our initial consultation via video chat, Dr. Wiltshire took a thorough personal and family medical history. We also discussed what the motivations and desired outcomes were for our fertility treatment. Lastly, Dr. Wiltshire walked us through the next steps and answered any additional questions we had.
Our next steps included blood panels to tests, currently fertility, health status, and any pre-existing genetic conditions. After the needed tests were complete & explained, we began the fertility medications. The medication treatment plan and administration was explained in great detail and had videos to provide step by step instructions. The staff checked in with me regularly and often and Dr. Wiltshire swiftly returned insight to any questions I had.
As retrieval day got closer, check-ins and blood work became near daily and the team was amazing the whole time. In the days leading up to extraction, pre & post-care operation was clearly walked through.
Lastly, Dr. Wiltshire provided day by day updates and the final results of our egg retrieval cycle.

From the costs that were paid out of pocket, since we do have a robust healthcare plan, we spent an estimate of $4-8k.

I was impressed how Dr. Wiltshire treated me with the utmost respect throughout my treatment. From beginning to end she answered all my questions and never rushed me. Dr. Wiltshire explained the complexities of my situation and was willing to do research to thoroughly answer my questions and formulate a plan that would give me a fighting chance. She check-in via email, phone, and zoom, before and during treatment to see how I was doing and talk through how to proceed. I can't recommend Dr. Wiltshire enough as a physician who is both intelligent and has a great caring bedside manner.

The first cycle was ten days of injections. I started with six follicles, three of which grew, but by the end of the cycle only one for possible retrieval so the cycle was ended with retrieval. The second cycle consisted of nine days of injections, this time I was able to proceed to retrieval and had two eggs retrieved and frozen.

Kate was great. She provided me with an email with an overview of how treatment schedule could go. Links to videos to prepare for injections and an FAQ from the center. She also did a practice injection session over zoom so I could be comfortable with giving myself injections. Kate responded promptly to my questions. She gave me clear instructions throughout and after egg retrieval for medication and restrictions.

The only unpleasant experience I had was one of the visits, I was late due to waiting for the elevator and they asked me to come back after lunch (I live in Brooklyn), luckily my doctor was there and I was able to meet with her that morning.

Dr. Ashley is a phenomenal doctor! I could not have had a better experience. I was very nervous coming into the treatments and also had a tight deadline due to switching jobs. She developed a treatment plan that worked with my schedule, optimized to leverage my insurance and have 2 cycles. She answered all of the questions I had, listened to my requests with wanting abdominal ultrasounds if possible, and allowed an alternative for drawing blood that eased the process a bit for me personally. She made sure to be there for both of my retrievals which made me feel as though I was truly in great hands.

The 1st cycle I went through about 10 days of injections, 3 follicles grew quickly (faster than the rest), so she modified the treatment. We were able to retrieve 8 and freeze 3.
I did a 2nd cycle immediately after the 1st. This time, Dr. Ashley adjusted my treatment plan starting with estrogen tablets & clomid at the outset, then I went through about 11/12 days of injections. The adjustments worked and we were able to retrieve 14 and freeze 12!

My primary nurse, Kate, was great! I had lots of questions around mixing the medications myself and she hopped on a few different video calls with me just to ensure I was doing it correctly.

She never over promises on results (which helps me manage my expectations) and is always calm during our sessions so that made me feel at ease. I have lupus so that was one of my primary concerns when I started on the IVF cycle given the many drugs needed for the cycle and she has helped me to achieve the results with minimal side effects in spite of my condition.
In general, all the care coordinators and nurses that I have encountered in Columbia are very nice and helpful. In particular, my primary IVF coordinator Kathryn Klinger is super-efficient and detail-oriented...The clinic itself is very modern and doesn't feel overcrowded on my appointment days as the scans and bloodwork are done in an efficient and timely manner so there are rarely backlogs in the queue. Before each process stage, I always get a detailed overview of what is upcoming which is very helpful for me as I will have an idea of what to expect. For the egg retrieval, I was on 225 units of Gonal f, 1 powder of Menopur, and Cetrotide. There was close monitoring of the blood work almost every other day to ensure that the follicles were growing in line with the medications taken. I had an adjustment to 2 powders of Menopur as a result of these monitoring sessions. I have minimal reactions to the medication dosage which I am very happy with given my lupus condition.

In general, all the care coordinators and nurses that I have encountered in Columbia are very nice and helpful. In particular, my primary IVF coordinator Kathryn Klinger is super-efficient and detail-oriented. She helps me to understand complex procedures in an easy and simple explanation and always gives me an overview of the processes that are upcoming and what to expect. She replies to emails promptly and I know that I am in good hands because she will always be there to address my queries relating to medications or side effects. Kathryn always sends back the test results as early as possible, so I never have issues which I need to chase her for updates or information. Marsha Darbouze (my care coordinator) arranges my scans and bloodwork appointments, and she is also very prompt in getting me the best appointment time that I need.

I like the clinic location as it is very accessible for me by bus but there is also a subway station nearby. The clinic itself is very modern and doesn't feel overcrowded on my appointment days as the scans and bloodwork are done in an efficient and timely manner so there are rarely backlogs in the queue. Dr. Wiltshire was always very patient with me and answered every question (even the very simple ones) with kindness and specificity. There were several times when she came into the room to do my ultrasounds to make sure she answered questions I had midway through the egg freezing process. She was thorough and was culturally competent with my fears and worries. I truly felt listened to and respected and as a Black woman, that often does not happen with physicians.
The [Columbia University] clinic was very efficient. I didn't stay longer than 15 mins per visit, which included bloodwork and the ultrasound. Every staff member introduced themselves and were very kind during my visits...The ultrasounds were also very fast and the team very clearly had an efficient process in place. As an employed person going through this, it was very important for me to have an efficient process so that I could get back to my office and that's exactly what happened. I was always impressed.
How was your experience with Ashley Wiltshire at Columbia University?
During my first round, Dr. Wiltshire found a cyst on my ovary and immediately shared her concern with me. I was very nervous about what that meant for my fertility treatment but she calmed my nerves and carefully walked me through her plans to watch the cyst over time and come up with another course of action (pausing the egg freezing process and wait for the cyst to shrink, which it did!).
Subsequently after the cyst concern, Dr. Wiltshire explained every step of the egg freezing process I was about to undertake, including the complicated injections I had to give myself every night. On the day of my egg retrieval, Dr. Wiltshire was professional and extremely prepared. I was nervous about the anesthesia but she made the operating room a very calm space. Once I came to, Dr. Wiltshire let me know my egg count and made sure I had everything I needed to recover. My mother picked me up after the retrieval and hugged and thanked Dr. Wiltshire for taking great care of her daughter :).
After the first retrieval, Dr. Wiltshire and I talked about plans for another cycle to increase the # of eggs we froze. She talked me through her plans to increase my dosage and push me a little further in order to retrieve more mature eggs. Her plan worked and we were able to retrieve and free more eggs the second time around!
It's been about a week and a half since my second retrieval and a few days ago, I had a follow up call with Dr. Wiltshire who wanted to check in and again, answer any questions I had. We talked in detail about options to use my frozen eggs in the future and this is where she really understood my concerns, specifically about fertilizing my eggs and transferring them...in other words, becoming a parent! She made me feel very comfortable sharing my fears and my nervousness about that process and assured me that the decision was mine and there was no wrong way to feel about this. I cannot thank Dr. Wiltshire enough for being so patient and kind throughout this process.
I've told several friends to consider seeing Dr. Wiltshire to freeze their eggs and start their fertility treatments. She is incredibly impressive and I am so grateful to have found her!

GO TO HER IMMEDIATELY! And ask all the questions - she literally has all of the answers and can walk you through everything with incredible precision.
During treatment, were you treated like a number or a human with Ashley Wiltshire at Columbia University?
Dr. Wiltshire was always very patient with me and answered every question (even the very simple ones) with kindness and specificity. There were several times when she came into the room to do my ultrasounds to make sure she answered questions I had midway through the egg freezing process. She was thorough and was culturally competent with my fears and worries. I truly felt listened to and respected and as a Black woman, that often does not happen with physicians.

Retrieval #1:
As mentioned, Dr. Wiltshire found a cyst on my ovary, which delayed my treatment but once it started, I started my injections and pills (Clomid pills, Gonal-F (300 units), and Menopur (2 power vials)) for about 4 days. During that time, I went to the Columbia Fertility Center to do bloodwork and an ultrasound every other day. Once my medication changed as we got closer to the trigger shot, I went in for bloodwork and the ultrasound everyday. For the trigger shot, I had to take HCG 10,000 IU and Leuprolide Acetate (40 units). Then it was time for the retrieval! Dr. Wiltshire retrieved 7 eggs but not many of them were mature (3/7). As a result, Dr. Wiltshire and I talked about a strategy for another cycle and pushing the days for how long I took my medication to try and retrieve more mature eggs.
Retrieval #2:
After a few months, I began my second cycle with slightly different dosages, as we discussed. I was on Clomid (100 mg), Gonal F (250 IU), and Menopur (150 IU). A few days later, I added Cetrotide 0.25 mg. I did all three injections for a few days until my ovaries grew to be much larger than they were during the first retrieval! Dr. Wiltshire was pleased with how I responded to the medication and she made an educated recommendation to add one more day to my process before the retrieval in an attempt to retrieve more mature eggs, which would hopefully be different than my last retrieval. The results were excellent! I still had a "small" number of eggs retrieved (6) but all except one were mature and were able to be frozen! I was so incredibly happy. Dr. Wiltshire's educated recommendation was spot on and after both retrievals, I ended up with a total number of 8 eggs, which may not seem like a lot but I was quite happy with the results.
Fertility treatments are not easy and honestly, I was praying for one or two eggs. So coming out with 8 is cause for celebration! At this point, I don' t think I will do another cycle but my employer does provide these cycles as part of our benefits so I may consider it next year. And if I do, I will immediately call Dr. Wiltshire!

Kathryn (Kate) Klinger was excellent. She stayed in constant communication with me and gave detailed descriptions about my medications and injections. She also helped me navigate ordering more medication from Alto when I ran low. Kate very extremely helpful!
Describe your experience with Columbia University.
The clinic was very efficient. I didn't stay longer than 15 mins per visit, which included bloodwork and the ultrasound. Every staff member introduced themselves and were very kind during my visits. Thankfully I have "good" veins and drawing my blood was incredibly quick. The ultrasounds were also very fast and the team very clearly had an efficient process in place. As an employed person going through this, it was very important for me to have an efficient process so that I could get back to my office and that's exactly what happened. I was always impressed.

My employer paid for 95% of the costs. I paid roughly $400 for the medication, which came from my HSA.

Describe the protocols Ashley Wiltshire used in your cycles at Columbia University and their degree of success.
I went through the egg freezing process. Dr. Wiltshire asked me about my preference in timing and developed a strategy around that. I wanted to make this happen asap balancing that I was also expecting to travel and my cycles not falling ideally. I was placed on birth control temporarily and once I was ready, Dr. Wiltshire gave me my dosages for my ovary stimulation/follicle growth. That involved monitoring my follicle growth very carefully. Dr. Wiltshire adjusted my dosages according to progress. Growth was slower than expected so we steadily increased dosages. Growth started picking up but Dr. Wiltshire wanted to make sure the eggs were the ideal size for retrieval so we kept on with the injections until the eggs were in the high-teens mm in size. On the day of the retrieval they retrieved 25 eggs of which 24 were frozen.

Nurse Kathryn Klinger was fantastic. She was an excellent communicator and very compassionate. She walked me through the medicines and helped calm my nerves of the preparation. She was available constantly for any questions and was super responsive. I felt at ease knowing I had her support during the whole process.
I had Sophie Clark as my care coordinator and she was very helpful in ensuring I had all the information necessary at the start of my egg freezing process.
Having the support team as a resource truly made all the difference.

They often ran on time, the most I waited was 10 minutes. They were very accommodating - I always request to get blood drawn laying down and that can often times disrupt their flow however they never let that phase them and always found a way to make it work for me.

Dr. Wiltshire has a very thorough consultation during the 1st visit. She explains the various possibilities that could explain the infertility. However, it wouldn't be confirmed until the extensive workup is completed, which also occurs quickly. She and her team are quite knowledgeable and provided frequent updates (mostly by email, at times phone) for any laboratory data obtained that morning you visited the clinic. Therefore, you are pretty much not in the dark of your own medical care.

Initially it was suggested that we opted for IUI after the retrieval. However, once it was realized the sperm quality (mobility) was a bit limited, it was suggested to opt in for IVF. The option for IVF was never forced from the beginning. It was thought we could use a less benign treatment if possible.

Ms. Klinger is a great addition to Dr. Wiltshire's team. She is very responsive to patients, providing thorough information for us to understand what our progress is going (for the retrieval and implantation) process. Even if there were concerns of different, unusual symptoms, she was always patient and kind to explain what they were, which helped reassure us that we were going to be fine.

I was TERRIFIED going into this process, but with Dr. Wiltshire felt supported overstep of the way. She answered all my questions and was patient when I was emotional. She encouraged me. I had a fairly uneventful egg retrieval and process. I will note that I expressed to Dr. Wiltshire, frequently, how expensive the process was and that this was my ONE shot doing it. When I was getting close to the end I was sore and tired and ready to give up, but she pushed me a little further along (an additional day or two of regular treatments vs. triggering too early). She reminded me about MY own goals of having a successful retrieval and was honest about weighing our options. She gave her expert opinion that I needed a little more time to be as successful as possible, and she was right! I appreciated her expertise and encouragement.

Basic lab work and consultation which was normal. An abnormal HSG, in-office hysteroscopy to confirm adhesions and operative hysteroscopy, then a follow up ultrasound to confirm that surgery was successful.

The first care coordinator, Daniella, was very on top of everything and clear with next steps. The second coordinator, Martha, needs to work on her attention to detail. For example, she scheduled my follow up saline ultrasound in a clinic that was very far away from my home and not accessible by public transportation. It was a huge inconvenience.

Area for improvement: the coordination of sperm analysis was very confusing and stressful for my partner. He was told to find a sterile container himself which no pharmacy carried. Once a container was finally obtained, the process of bringing it to the clinic himself was also nerve wracking. Why not just have the option to give the sample at the clinic as a first option if people want that?
Dr. Wiltshire displays and embodies what anyone ideally seeks in their healthcare provider. Simply, she was attentive, communicative, thorough, knowledge, kind and understanding. When I decide to proceed in moving with IVF in the future, without hesitation, I plan on working with Dr. Wiltshire. I recommended her care with my closest friends and family, that's speaks volumes of what I thought of her as my doctor.

In terms of weakness, I truly can't think of any for the procedure I elected. Dr. Wiltshire was exceptional in her level of empathy and understanding. I truly felt like I was consulting her for my options and expectations. I did not feel pressured or rushed, and despite the number of questions I had, she asked me if I had any other questions. During the egg freezing hormone treatment, Dr. Wiltshire and her team (shout out to nurse Kate), were exceptionally clear, timely and understanding. Prior to working with Dr. Wiltshire, I had one egg freezing round that was not as successful. Dr. Wiltshire leveraged the previous clinical work and results to determine the best course of action for medication, which yielded a much higher, successful second round. I recognize this may not be the case for everyone, but I appreciated her thoroughness and explanation of the upcoming medications.

Clinic strengths are: appointment availability (especially monitoring appointments), coordination and communication. In terms of weakness, I truly can't think of any for the procedure I elected. I recommend taking advantage of their email communication / patient portal of results.

Dr. Wiltshire was absolutely amazing! She is an exceptional communicator, taking time to clearly explain what was happening in terms that made sense to me. Striking the right tone can be so hard for doctors but she nailed it without dumbing things down or relying only on medical jargon. She reached out throughout the process to make sure she answered all my questions and always felt accessible. I never felt rushed and she listened carefully to my concerns to make sure she offered options that met my needs.
Dr. Wiltshire was extremely knowledgeable. Because she explained the process so well, I always felt fully informed about my treatment and I trusted her. When there were decisions I needed to make about how to proceed, she equipped me with the necessary information to weigh my options and make the best choice.
Dr. Wiltshire demonstrated she cared about me as a patient. She came in on a holiday to perform my procedure, which I really appreciated. Also, I mentioned having difficulty reaching someone for an administrative issue and she made sure they reached out to me so things moved forward, which was super helpful.

We started outside of my cycle because it was delayed so she adjusted my medicines. My cycle also ran long (I did injections for 14 days) because she wanted to be sure as many of my eggs were mature as possible.

I received information about next steps from various people and the person with billing information was initially unresponsive to my questions. This was straightened out on the front end but was confusing because I wasn't clear on the steps I needed to complete in order to begin the process.
Dr. Wiltshire was open and honest with me about the process and her point of view on certain aspects of the procedure. She was relatable, funny, not self-serious, and gave her honest opinions when asked. She was responsive when I struggled to understand which shots were which on my first night of treatment; even getting back to me after 10pm at night. We had a successful retrieval and we even celebrated together in the recovery room.
[Columbia Univserity is] Clean, organized, good location with competent staff and a flexible scheduling system. Those are the strengths. There are no weaknesses, except maybe how busy they are all the time? but I never had to wait for my appointment, I was always called in very soon after arriving. The nurses were lovely and it ran like a well-oiled machine...There were a few times the nurses struggled to find a vein but they were always compassionate and apologetic when they had to stick me multiple times.

She was kind, honest, and a clear communicator. She took my treatment seriously and made sure she was available to support me along the way.
She was very responsive on email and text, and even got back to me with answers to my questions about my medication dosage after hours. There was a question about whether she would be available for the extraction surgery or would be replaced by another doctor, but she was there. I was her only surgery that day and she came in to make sure she was performing, which was very important to me. We had a very successful retrieval (57 eggs) and we celebrated together as I recovered. She was funny, caring and relatable.
