He [Dr. Crochet] took the time to get to know us and the concerns we have. Being able to meet remotely allowed me to better fit within my work schedule and allowed me to have more time to discuss the options and questions I had. This can be a frustrating process but I feel like he listens to our needs and is honest and open about the next steps...Have your questions ready, he is good about going through them...
They [nurses at Shady Grove Fertility Houston] are warm and welcoming and also help you understand the process. They are also rooting for you and wishing you success...They are usually pretty good at getting you in quickly and scheduling around your schedule. The patient portal isn't the most user friendly but it does have a lot of good information. They are responsive too and call you back quickly...

Dr Crochet was realistic but positive about outcomes for us. He was determined to help us and he did. He was compassionate when we didn’t get the results we wanted...After our first meeting, Dr Crochet remembered both my and my husbands occupation and the reason we were seeking his help...trust what he tells you. It’s not a quick process and it takes time and patience to get it right. But Dr crochet will do everything he can to ensure the best outcome.
[SGF Houston] Staff was always great and knowledgeable...your call will be returned and in a timely fashion (you won’t miss your opportunity for transfer, etc) because they didn’t call and/or leave instructions...They were always very responsive and understanding of the stressful nature of my calls. The phlebotomists were amazing. All were always kind to both me and my husband.

I had my tubes tied before I met my husband. I am older to be having babies but we did an egg retrieval on me and then implanted. Unfortunately that didn’t work so he advised an egg donor. The selection process was easy and I appreciated the genetic testing on the donor and my husband. We avoided potential complications because of that. Once the donor was successfully selected, we were kept up to date with the fertilization and freezing. When the first transfer was unsuccessful, Dr Crochet ordered more tests and tweaked the process for the next one— which ended with a perfect little girl. We are now using the last embryo and are currently 10 weeks along with our second little girl.

Dr. Crochet made me comfortable in a very emotional time...When he had to cancel my first cycle (wasn’t responding to meds) he told me right away and of course I was devastated. He looked me in the eye and said “I know this isn’t ideal, but we want the best chance of having a baby possible.” And I immediately felt at ease. I trust him whole heartedly. He celebrates our triumphs and he mourns the lows with us.
When [nurse at CORM] called me with my negative beta results she was so amazingly sweet when I was bawling on the other end. I wish she could be my nurse at every doctor I see!...I’ve had a few problems with the billing department. They have made mistakes that if I didn’t catch, they would have even more money from me...Just keep an eye on what is being charged and keep receipts... paid around $30,000 for the contract and $10,000 for medicines.

1st IVF cycle - canceled due to poor egg response *Dr. Crochet changed to Minimum stimulation IVF (this contract included 2 retrievals) 2nd IVF cycle - (min stim) retrieved 3 eggs, after PGS testing we had 5AA boy. 3rd IVF cycle - (min stim) retrieved 8 eggs, after PGS testing we had 5BB boy and 5BB girl. Embryo Transfer - transferred 5AA boy. 14 days past transfer - Negative pregnancy test. Signed documents to transfer 5BB boy after a laparoscopy and ERA. Then because of Covid 19 I was laid off and my husband (oil and gas) just got laid off too. Now we are just in limbo.

I do feel as though he’s [Dr. John Crochet] taking his time and putting my health at the top priority. I feel like he is a great communicator and doctor. I’ve only had one IUI so far and I do wish he would’ve done it instead of a nurse...Don’t be afraid to ask questions. He’s always been happy to answer any...they know me as soon as I come in for testing. They know my situation is unique and they know it has to be very precise.
I think they [CORM clinic] are very organized and they try to make you feel comfortable. They will answer any and all questions you have even if they have to check with the doctor and call you back. The only thing I didn’t like was when I did my IUI, I had to go to the Houston location (2 hours from us)...[the nursing staff] They are always aware of my situation, my monitoring and my testing. They recognize me and they make me feel very comfortable.

My first attempt at IUI, we did 2.5mg of Letrozole for 5 days. Then we did 75iu of Menopur over the course of 4-5 days while they monitored me. My follicles never really grew and I ended up ovulating early so we missed that cycle. My actual first IUI: We did 5 days of 7.5mg of Letrozole and on my first monitoring visit, I had a follicle at 19.8 Wednesday. I triggered at 9:30pm on Friday and we did the IUI procedure on Sunday at around 9:30-10am. This IUI failed. We are waiting for the clinic to open back up and either continue with another round of IUI or switch to IVF because of my unicornuate uterus.

Dr. Crochet makes me feel like we are a team and our goal is to help my husband and I conceive our first child. He’s caring and honest. He is also understanding to our financial situation...trust him and go in with a positive attitude...He’s a great doctor
The clinic [at CORM] is clean and inviting...have a lab on sight for blood work...The nurses know you by name...have a rule that no children are allowed in order to stay sensitive to the fact that many of the patients coming there are having problems conceiving their own kids.
How was your experience with John Crochet at Shady Grove Fertility Houston (formerly CORM)?
He’s a great doctor and I pray that once this covid 19 is over. My husband and I can move forward with our first cycle of ivf. When I first met Dr. Crochet our game plan was to run blood test and get a ultrasound of my reproductive organs. Unfortunately there were some cyst that were found and I was scheduled for a surgery to remove them and then start fertility treatment like a medicine or something that would help. Well during my surgery both my tubes had to be remove because of damage that could not be fix. While in surgery my husband and my mom said that Dr. crochet was very informative on why he needed to remove both tubes and he would make sure to take good care of me. When I can out of surgery and was told what happened I was devastated because I knew I would never be able to conceive naturally and my biggest fear was coming true. I would have to do ivf. But Dr. crochet reassured me at my follow up appointment that everything would be ok. He explained every to were I could fully understand why he had to remove my tubes and what would be our next step.

When I first started going to see Dr. crochet he got to know me and my husband medical background first. Once blood work came back as well as my husbands sperm analysis Dr. crochet proceeded to put me on some vitamins and thyroid medicine. Like I explained already I had to have surgery to remove cyst and a pelvic exam under anesthesia. That’s when Dr. Crochet made the decision along with my husband to have both tubes removed because of damage. After surgery we talked about ivf but haven’t been able to move forward with the process because of money and because of the covid 19 virus

Dr. Crochet was really helpful with helping me manage my husband. I let him know that my husband could be a bit non-compliant when it came to taking meds and supplements. He also answered all of our questions, and was very thorough with my work up. He did everything he could to make sure we could do minimal treatments, but we ultimately chose IVF.
The clinics policy is to only transfer 1 embryo, I think they will allow 2 with untested embryos after multiple failed transfers...Nursing staff were great! They were also friendly, quick, and efficient so I could get on with my day...I think CORM is great at getting people in and out for their appointments. There doesn’t seem to be much communication between the clinical staff and financial staff. I had several problems with the funicular side of things, so definitely stay on top of things, especially if you’re using insurance.

For egg retrieval, I was on birth control for 3 weeks, then started Cetrotide, Gonal-f, micro-dose ovidrel, and I triggered with Lupron. I had 20+ follicles in each ovary, and this protocol help grow and mature as many eggs as possible, without putting me at an increased risk of OHSS. For transfer, I was on letrozole, Gonal-f, prednisone, triggered with ovidrel, then 2 additional mini doses of ovidrel, estrace, and endometrin.

I’m still waiting for that to be finalized, but I used insurance. I believe without insurance ER is $3900 + meds and monitoring and transfer is $1100 + meds and monitoring.

Describe the specific things that went wrong at Shady Grove Fertility Houston (formerly CORM).
I have had appointments cancelled on me with no notice or even a phone call, I had several issues with the billing department (overcharging me, refusing to return my money, waiting months to submit claims to insurance), my financial counselor didn’t call for pre-approval for my ER until I asked about it 2 days before my ER, then we came to an agreement that I would only pay after my insurance has paid but the reception desk was still instructed to collect payment from me.

[Dr. John Crochet] does a wonderful job at explaining things for my understanding. He makes sure I don’t have any questions before leaving his office and anytime I need to call him, he always calls me back with instructions...He will make you fell at ease and like a friend instead of another person walking through the door...The doctor and staff make you feel like your somebody and not just another patient...
The front staff [at CORM] is welcoming. The girls even know my name when I go in...The hours are wonderful as well. I don’t have to be at work until 8:30 and they always accommodate my appointments even at 6:30 in the morning sometimes...The online portal allows me to also stay connected with test results and appointments confirmations...if things like lab work was to expensive they would help my writing my lab order for me to have done at a cheaper outside lab.

Dr. Crochet does a wonderful job at explaining things for my understanding. He makes sure I don’t have any questions before leaving his office and anytime I need to call him, he always calls me back with instructions. He has diagnosed me with unexplained infertility and has not given up ways to help my husband and I to hopefully have a baby someday.

Describe the protocols John Crochet used in your cycles at Shady Grove Fertility Houston (formerly CORM) and their degree of success.
The first 3-4 months we increased famara. No pregnancy resulted. So we had 2 IUIs and added in Menopur and Ovidrel. Still not pregnancy. The next step is IVF. Which we cannot afford at the moment.
Describe your experience with your nurse at Shady Grove Fertility Houston (formerly CORM). (Assigned nurse: Melissa, Anne)
Ann and Melissa are the best nurses I could ask for. They always return calls and emails and always make sure I’m okay. I feel like the staff is my family.
Describe your experience with Shady Grove Fertility Houston (formerly CORM).
The front staff is welcoming. The girls even know my name when I go in. And when I leave they make sure to say goodbye. The doctors make sure you understand everything that is going on. They always ask if you have any questions and make sure you feel at ease for anything even if it’s a consult visit. The doctor will tell you everything he is going to do so you don’t feel scared. The hours are wonderful as well. I don’t have to be at work until 8:30 and they always accommodate my appointments even at 6:30 in the morning sometimes. I can’t name of any negative of CORM. All I can think of are good and wonderful things. The online portal allows me to also stay connected with test results and appointments confirmations. I can access is on a desktop and/or my phone if needed. They have been nothing but awesome from my very first appointment.
Describe the costs associated with your care under John Crochet at Shady Grove Fertility Houston (formerly CORM).
I don’t have insurance so I have to pay out of pocket. But if things like lab work was to expensive they would help my writing my lab order for me to have done at a cheaper outside lab.

Dr Crochet is a compassionate and knowledgeable doctor. He practices solely from research based techniques combined with the well being of his patients. He has made my treatment seemless and has walked hand in hand with me along the way ... For IUI stimulation my protocol was 10mg of letrozole for 5 days, 150mg menopur for 3 days, then Ovidrel trigger shot topped off with progesterone suppositories. For IVF stimulation I did Gonal-F, Cetrotide, and Novarel trigger, then the incorporation of baby aspirin, estrace, and progesterone suppositories.
The Clear Lake/Webster clinic [CORM] has a great response time to voice messages and/or emails sent. ... The Finance group is open to answering any questions and periodically offers suggestions for financing and things of that nature. The contracts are straight forward and easy to understand. I can’t think of anything that I would consider a weakness. ...The wait time is better than any other doctors office I’ve ever visited in that your appointments are handled promptly. The front desk ladies are always in the best mood and very helpful.

He [Dr. Crochet] is very caring and takes his time with you. He has answered all my questions and explained everything I had a hard time understanding. He is very nice and makes you feel at ease. was referred to Dr. Crochet in January of 2018 because my gyno diagnosed my fibroid and she felt he was more advanced to perform the surgery via the Da Vinci robot. From the first consultation with him to surgery day I always felt comfortable, he explained exactly what he was going to do. After recovering from surgery for about 6 months my husband and I both went back to Dr. Crochet to continue our path to becoming parents. Soon after it was discovered that my husband had very low numbers and Dr. Crochet put him on Clomid for 3 months. In January of 2019 we went back to test his numbers and they had significantly improved and we got the go ahead for our first IUI. We did our first IUI on March 16, 2019 and on March 29, 2019 it was confirmed that the treatment had failed.
The front desk staff [at CORM-Webster] are very polite and always greet you when you walk in. Check is very smooth and when you've had a sensitive appointment they'll actually sit there and comfort you. I broke down a couple of times and none of them were awkward or dismissive. The only weakness that I've experienced is that one of the financial assistants is very rude and cold.

Never once did he [Dr. John Crochet] make me feel like a number. He is very personable and he makes me feel like he is my friend as well as my doctor. He is funny when needed and he is serious when needed. He makes me feel more at ease about this entire journey I'm going through... I did my first IUI right away, and did get pregnant. Unfortunately, it resulted in an ectopic pregnancy, and I had to have emergency surgery and get my right tube removed. Dr. Crochet performed my surgery and it went great. I just went to my post op appointment and even after being through something so traumatizing, Dr. Crochet STILL made me feel hopeful. I think that's pretty amazing and I'm lucky to have a doctor who cares so much.
All of the staff [at CORM] is really great. I suffer from anxiety, so I call up there a lot. They never make me feel silly for calling and having a lot of questions. You can tell they really enjoy what they're doing...My IUI was about $1,500 not including medications. The next round will be more expensive because we plan on adding Gonal F...This clinic is the best. I truly feel privileged I get to go there. Everyone is so nice and caring. I never feel like just a number. They remember my name and specific things from previous appointments. They are always steps ahead of me. I really can't say enough good things about this clinic.
How was your experience with John Crochet at Shady Grove Fertility Houston (formerly CORM)?
During my consultation, I told him everything. He felt for me and was passionate, while still remaining positive and confident. He has made me believe I WILL have a baby. Describe the protocols John Crochet used in your cycles at Shady Grove Fertility Houston (formerly CORM) and their degree of success.
During my first consultation, I told Dr. Crochet I was just finished taken Clomid prescribed by my OB. He did an ultrasound and discovered it was actually perfect timing for an IUI two days later, so we got to get started right away! Before I found out if I was pregnant or not, he found out from my bloodwork that I have very mild PCOS. He decided the next IUI cycle he would switch me to Letrozole and we would also go Gonal F injections to raise my chances. I did end up pregnant, but unfortunately it was ectopic. At first I was doing Methotrexate, and my bloodwork had indicated the treatment was working. Unfortunately, it ruptured and I had to have emergency surgery. Dr. Crochet is still confident we can conceive through IUI, and we plan to try again in July.

Describe your experience with Shady Grove Fertility Houston (formerly CORM).
This clinic is the best. I truly feel privileged I get to go there. Everyone is so nice and caring. I never feel like just a number. They remember my name and specific things from previous appointments. They are always steps ahead of me. I really can't say enough good things about this clinic.
Describe the costs associated with your care under John Crochet at Shady Grove Fertility Houston (formerly CORM).
My IUI was about $1,500 not including medications. The next round will be more expensive because we plan on adding Gonal F.

Dr Crochet is a very considerate and supportive doctor. He is not willing to give up and try different styles to ultimately give us a baby. We absolutely believe he is the best dr for us. We have had some issues with the clinic but decided to go through round 3 with them again just because we didn’t want to lose dr crochet... He spends a lot of time with us going over each detail and has been compassionate about our loses. He is constantly striving to get us pregnant through different techniques to advising about special diet plans and such. His office has had some hiccups which we were not happy with but because of him we have continued this journey into yet another cycle and couldn’t imagine going through this with any other dr.
[The staff at CORM] told us to begin regimen and order the prescriptions to begin the cycle and then billing stepped in and cancelled due to a change in our insurance and stated the claim was submitted when it was not...delayed us goin on 3 weeks now...Their strengths are their doctors, weaknesses are billing and communication. I don’t like that you have to call and never speak to anyone and hope you get a return call. The lab updates are on par, billing and insurance is lacking in Communication and explaining things clearly

Dr. Crochet took the time to get to know us as a couple and as patients. He has a great bedside manner and remembered conversations we had prior. He tells it like it is, and helped us get the results we were looking for!...Strengths - helped us get pregnant! incredibly smart, calming manner Weakness - you don't always see Dr. Crochet at each visit, it's definitely usually the nurses, he "tells it like it is" We liked the facetime with Dr. Crochet and moved our schedule to when he was available.
$10,000 for IVF egg retrieval [at CORM], $4,500 for embryo transfer, medications were separate. Cycle restart fee was $500 or $800...Single Embryo Transfer - CORM has a policy to only put one embryo back in for the health of the mother...The clinic doesn't always have the most updated paperwork - billing. I paid for a visit, but it wasn't reflected in their notes, so I had them look into it and even saved my receipts. Take advantage of any promos or discounts offered through CORM - ex. 25% off for their anniversary promo. The clinic was creative in getting ways to cover certain visits or procedures, when Infertility isn't covered under my plan, so that was a huge help!

I have PCOS and high AMH levels. We did 2 injectible IUI's that were not successful. My husband and I decided to move on to IVF after the 2nd failed IUI, as the results are much better. I'm glad we did cause we put that $ from what've would've been the 3rd IUI, towards IVF and it worked on the first try!
Describe your experience with the nursing staff at Shady Grove Fertility Houston (formerly CORM).
Nursing staff was great. Try to call before 2:30/3pm to get a call back the same day. They did the prior authorizations for my prescriptions ahead of time (before IVF) so I wasn't as stressed battling the insurance companies, when the time came to fill the prescription. They faxed it to local pharmacies, specialty pharmacies, and mail order. They answered my many questions and re-assured me when I needed it.

I have uterus didelphys (two uterus). [Dr. Crochet] knew exactly what we needed to do. We started with IUI and when that didn't work the first two times we tried, he just came out and said it's not the route we need to take. He tells you the hard news in a way that makes you feel like its going to be okay...explained that we are going to do eSET because of my anatomy he said it would be irresponsible of him to put in multiple embryos
[CORM] has many strengths! It is a very clean clinic and organized. Each patient has a login number and ID so when you get there you can type it into the computer, so you don't have to wait on someone in front of you...They have their own nurse call line that you can call at any time of the day. If you have to leave a message they will call you back within two hours. They care about you, your treatment, and success. They are also very good at telling you bad news and are compassionate
How was your experience with John Crochet at Shady Grove Fertility Houston (formerly CORM)?
Dr. Crochet is very knowledgeable in what he does. I have uterus didelphys (two uterus). He knew exactly what we needed to do. We started with IUI and when that didn't work the first two times we tried, he just came out and said it's not the route we need to take. He tells you the hard news in a way that makes you feel like its going to be okay. He knows how hard this is for you and wants you to become pregnant. He isn't just a doctor, he is invested in your treatment. The whole clinic is invested in your treatment. They remember who you are from appointment to appointment, they work with your schedule for appointments, they are always in a good mood, and they are always available to call. When you call the nurse line and leave a message you will always get a call back that day, usually within two hours. Dr. Crochet is a great doctor and we will continue to see him for our IVF journey. I highly recommend him in your fertility treatment if you are in the Webster-Houston area! I wouldn't want a different fertility doctor in this tough journey we are all in. We are in this together and he makes you feel that way!
What's one piece of advice would you give a prospective patient of John Crochet at Shady Grove Fertility Houston (formerly CORM)?
A piece of advice I would give a prospective patient is that they need to start now. We did our first consultation in 2018. We wanted to know what our chances were with my unique anatomy. He was very intelligible in my condition and told us that is was not going to be easy and that it might require different treatments. A year after our consultation we started out IUI treatments, when that didn't work he told us to stop doing them because it would not work for us and that we needed to then do IVF. While we are saving up for IVF, they are still in contact with us through the process. So my advice is to start now, even if you are not ready to start treatments yet. Dr. Crochet is where you want to start and they will work with you through all the treatments and the waiting!
During treatment, were you treated like a number or a human with John Crochet at Shady Grove Fertility Houston (formerly CORM)?
Dr. Crochet treats everyone like a human. He tells you the hard news in a way that makes you feel like its going to be okay. He tells you the hard news and then immediately goes into next steps, this helped me feel like he knew the hard news was hard but knew what I needed to hear next.
Describe the protocols John Crochet used in your cycles at Shady Grove Fertility Houston (formerly CORM) and their degree of success.
We started our treatment process with IUI. I used hormone pills, shots, and suppositories. While the shots were concerning for me because you do them on your own, the nurses were there to demonstrate how and where to do them. If you forgot how to do them, they had videos you can watch. If you still were nervous about them, they had an on call doctor you could call at anytime of the night. Yes, I called him at 10:30 at night multiple times and he was always there to help me through it! When we did IUI the first time, we could not do insemination because I developed a cyst on my ovary. So instead he explained to use we would then have timed intercourse over the next tree days and then would go in for a blood pregnancy test. When that came back negative, they were very kind when telling me. They knew the news was not easy to hear but then they said we would try again! The next IUI treatment we were not able to get past the initial ultrasound because I developed yet another cyst on my ovary. We then went into for an appointment for next steps. Dr. Crochet explained to us that the option that makes the most sense to him would be IVF because my anatomy is not on our side and this would be our best chances. So now we are waiting to save up money for IVF. Through the whole process Dr. Crochet was able to give examples, numbers, and success stories of what we were going through. He made sure we understood what he was saying and if we didn't we could always call and they would answer any questions we had.
Describe your experience with the nursing staff at Shady Grove Fertility Houston (formerly CORM).
The nursing staff is amazing. They remember who you are by name and they remember your job. They always greet you with a smile and ask you how you are doing. They are knowledgeable in what they do and how to do it. When getting blood draw, I have very small veins, the nurses at this clinic are the only nurses I have ever had that only need to stick me once. They also are always there for you when you have questions. They have their own nurse call line that you can call at any time of the day. If you have to leave a message they will call you back within two hours. They care about you, your treatment, and success. They are also very good at telling you bad news and are compassionate.

Describe the costs associated with your care under John Crochet at Shady Grove Fertility Houston (formerly CORM).
The costs of my IUI treatments that I did were normal for an IUI treatment, each one including medications were around $3,000-$4,000. My cost for IVF are going to be around $17,000-$20,000. We have not started IVF so we do not know the final cost yet.
Describe John Crochet's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Shady Grove Fertility Houston (formerly CORM).
For me, Dr. Crochet has explained that we are going to do eSET because of my anatomy he said it would be irresponsible of him to put in multiple embryos. I have uterus didelphys which means I have two uterus and neither of them our the size you want to have multiple pregnancies in. He explained that having twins in one of my uterus would not be ideal because there is just not enough room for two babies to grow.

He [Dr. Crochet] has an amazing bedside manner. Even though I didn't see him often because he went to other clinics, he always knew what was happening with me. So when I did see him again, he never missed a beat and knew everything. ... Weaknesses - I wish I saw my dr more often than I did. I saw him in the beginning and the end when I had a miscarriage. But he was there and talked with us before he performed the d&c.
Strength - they [CORM] give you hope that other clinics couldn't. They really do try to get you pregnant. They always have someone on call when something happens after hours which I did have to utilize a couple of times. Weaknesses - I wish I saw my dr more often than I did. ... It's just 1 older nurse who gave me the wrong phone number to a facility to do genetic testing on our embryo after miscarriage.

only 1 lady in the whole practice didn't make me feel comfortable as if she couldn't remember me and my situation. The other nurses all know about me and my situation. So I didn't have to keep reminding them of what happened and what i'm there for. They already know - except this older nurse. Taylor and Jordan have been amazing as well as the rest whom I can't remember their names.

Dr. Crochet tells us all the facts but also makes it light and tries to be funny about something if able to. It was nice to finally know our issue. He helped us understand that there’s nothing we could have done on our own to increase our chances. ... So far all we’ve been able to try is have my husband on Clomid and a cocktail of supplements. He also has done blood testing and several ultrasounds on me.
I haven’t had any problems with the staff [at CORM-Beaumont]. We’ve only been 4/5 times so I also don’t have much to go off of. I do really like the lady that draws my blood though, she’s sweet and does a great job.... I love that they have everything they need right there in Beaumont. They have great stats and it makes me hopeful. I’ve read tons of reviews and no one has anything bad to say. Like I said before, I always recommend asking questions.

He [Dr. Crochet] was warming and very compassionate from the start. He helped me with 1 surgery & told me what to expect & was our next steps will be. It doesn’t make you feel like just a number. I have enjoyed having him as my doctor. He helped me understand my fertility & what we need to do next...Hes very caring & explained what was needed to help me conceive, but our insurance doesn’t cover it & we can’t afford it right now
Strengths [CORM] is their bedside manner. Weakness is not calling back as soon as you need it. They will call you hours later, but they explain what you need to do when they do call you back. They’re very welcoming & make sure you understand every procedure & test....They’re very caring & will answer any questions you have.... I know they quoted me for 1800 for our iui. Which we’re saving up for. 1200 for just timed intercourse with meds.

Dr. Crochet is very knowledgeable, professional, and transparent. He desired as much success as my husband and I. He was just as saddened by out negative results as we were. Although our outcome was devastating, he was professional throughout the experience.... We used the agonist protocol. We retrieved 10 eggs with our first round that resulted in 5 embryos. We had PGS testing that resulted in one normal embryo. We went directly into an embryo bank cycle with agonist protocol
The nursing staff [CORM] was caring and supportive. They always answered the phone or quickly responded. Even during Hurricane Harvey, when Orange, Texas was isolated from flooding water and the interstate was shutdown, the team was on the phone helping to assist with medication delivery and attempting to help with a plan of action. Many of these nurses lost their own homes and went above and beyond. Dr. Crochet has an amazing and loving team.

Do you research and ask questions. Dr. Crochet is not a mind reader. Infertility treatment can be scary and questions lead to responses you sometimes do not what to hear. Ask the hard questions. He will answer any and all questions but research and ask the right questions. He will be honest and provide an answer.
During treatment, were you treated like a number or a human with John Crochet at Shady Grove Fertility Houston (formerly CORM)?
Dr. Crochet is very personable. He is encouraging and takes the time to know his patients. He is kind and understanding.
Describe the protocols John Crochet used in your cycles at Shady Grove Fertility Houston (formerly CORM) and their degree of success.
We used the agonist protocol. We retrieved 10 eggs with our first round that resulted in 5 embryos. We had PGS testing that resulted in one normal embryo. We went directly into an embryo bank cycle with agonist protocol, although in hindsight this was a bad choice because we had just lost our home due to Hurricane Harvey, and the additional stress resulted in 3 eggs retrieved with no viable embryos. We transplanted the one normal embryo that resulted in a negative pregnancy.
Describe your experience with the nursing staff at Shady Grove Fertility Houston (formerly CORM).
The nursing staff was caring and supportive.
Describe your experience with Shady Grove Fertility Houston (formerly CORM).
The nursing staff is always supportive and helps with assistance. The financial department was confusing and hard to contact , , ,however if you asked Dr. Crochet to assist you, he immediately directed them to address the issue.
Describe your experience with your monitoring appointments at Shady Grove Fertility Houston (formerly CORM).
Monitoring appointments were quick and adjusted to fit my busy work schedule.
Describe the costs associated with your care under John Crochet at Shady Grove Fertility Houston (formerly CORM).
Costs where listed directly after a free consultation. The financial secretary was hard to make an appointment with. She was very busy. I didn't take it as personal.
Describe John Crochet's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Shady Grove Fertility Houston (formerly CORM).
We had only one viable embryo to transfer so we are unable to give feedback to this question.

He [Dr. Crochet] was very presentable and conversational. He had great bedside manners and made sure to explain everything. He spoke directly to us and not through us if that makes sense...He was very sweet and detailed. Even though we did not conceive our first attempt he was quick to find a solution for the second round...at first gave me menopur 175 iu, I had to request for four more injections due to eggs not maturing fast enough.
[Costs at CORM were] 3,000 roughly. We had 1,200 for the blood work, and visits. 500 for the iui and the rest was medications...Very clean facility. Very nice staff. The nurses were very seeet. The lab was very sweet as well. The only downfall was the assistant during my procedure for iui presser a little too hard during the ultrasound. Other than that the staff were a delight...Any questions I had I contacted her and even if she didn’t answer immediately she would call me back that day.
How was your experience with John Crochet at Shady Grove Fertility Houston (formerly CORM)?
As stated before he was very personable and explained everything is great details. The first time we met with him he explained everything we needed to know moving forward. He was very sweet and detailed. Even though we did not conceive our first attempt he was quick to find a solution for the second round. We just didn’t procede since we are paying out of pocket. But we certainly would’ve had him as our doctor again.

Dr.Crochet used femara 5mg, trigger shot was ovidrel, and he at first gave me menopur 175 iu, I had to request for four more injections due to eggs not maturing fast enough. The menopur was the priciest medications. I was also tested for diabetes and while not diabetic he prescribed me 1,000 mg of metformin xr.

I wish I remembered he name, but she was as sweet as pie. Any questions I had I contacted her and even if she didn’t answer immediately she would call me back that day. Every visit we saw her and she was so sweet.

3,000 roughly. We had 1,200 for the blood work, and visits. 500 for the iui and the rest was medications. They did send us to a mail in medications, however, since I had to take last minute medications there was one that cost a little over 300 for menopur.

All I was told [by Dr. John Crochet] was that my tubes needed to be completely removed. I had to get an HSG done and then I was diagnosed with Hydrosalphinx in both tubes. In order for me to have children, I would then need to do the process of IVF...I just felt like I wasn’t being taken very seriously. That because of my age, that it shouldn’t be something to worry about at that moment...First it was my uterus shape that would be a problem then it changed to it wouldn’t be but my tubes were going to be a problem. I just wasn’t sure what I was going to be told next.
They [staff at CORM] told me the wrong time when to take the pain medicine for the HSG procedure. I was told one thing then another happened. I felt the entire process and was feeling the medicine later when I didn’t need it anymore. It honestly has traumatized me...Communication is a weakness. They were very nice, very friendly and helpful but after my HSG experience, I’d never go back. I’m glad that when I did have a question when I wasn’t there, I was able to get a hold of the doctor and I received a call back.

Describe your experience with the nursing staff at Shady Grove Fertility Houston (formerly CORM).
The most thing that I was upset with, was the timing on when to take the pain medicine for before the HSG. I was told by one nurse to take it when I got there and that after 10-15 minutes when it has kicked in, then they’d be doing the procedure. That is not at all that happened. I took it when I arrived and literally right after, they took me to the back and got me on the table. I felt everything. It was the worst pain I’d ever felt. It wasn’t till 15 minutes after the procedure was done, that the medicine kicked in. Somewhere along the way, their communication was messed up.

My husband has a low sperm count, so he placed him on clomid. After several month of trying naturally with the clomid, we moved on to IUI. The first IUI was unsuccessful, we decided to do a double insemination with the second IUI. That IUI took, but we miscarried a 5w4d. So we decided to try again. The 3rd IUI failed, but I did not respond as well to the medications. Dr. Crochet changed my meds, and we gave IUI one more chance. When it failed, we decided to move on to IVF. I am currently under stimulation medication and plan to have my retrieval early next week.
