How was your experience with Lisa Farah-Eways at Kaiser Permanente NorCal?
Dr. Farah-Eways is compassionate and really wants to make your dream of having a baby come true. You should note she is quite the multitasker, so when you have an in person consultation or phone appointment with her you may notice that she’s typing/performing computer task while talking to you. Please don’t mistake this for her not caring or not being focused on you and your treatment. At the end of your consultation/appointment you will have a summary in the form of emails with all things discussed, recommendations, plan of care and a detailed summary of your visit. Her typing/inputting things on the computer as she is discussing processes with you, I believe, is her way to ensure that no information during your conversation is missed in the hard copy of your plan of care and that all test you need to have done are ordered as soon as she decides you need them. Because she documents things while she’s talking, you end up having a complete reference in terms of next steps. This is important because there can be so much information, many dates, and timed studies to complete. I’m the type of person that comprehends and processes information quickly, so I prefer information to be delivered straight forward in the form of what the plan for me is, what my next steps are, and when they steps need to be completed. Having a hard copy plan of care with dates/times to complete and contact numbers has helped me feel more control in a situation that can be so out of your control. If you like information delivered this way I think you will be happy to work with Dr. Farah-Eways, also note that if ever there is a time that you feel you didn’t understand or you need to verify what the plan is, she responds best by email to address your concerns.
What's one piece of advice would you give a prospective patient of Lisa Farah-Eways at Kaiser Permanente NorCal?
Understand that she works within the Kaiser Permanente health system, so at times because the Kaiser process calls for systematic approach since they see a greater volume of patients than private REI clinics, you won’t always see her at appointments for procedures such HSG test, IUI or ultrasounds to verify meds such as clomid, femara, etc produced sufficient follicles for IUI. While you have a care plan by Dr. Farah-Eways, these procedures will be performed by her Nurse Practitioners (NP) and not necessarily the same NP each time. It comes down to who is available at the time you need the appointment. As for diagnostic procedures such as Hysteroscopy, endometrial biopsy, trial transfers, and saline sonograms you will see her or one of her colleagues that is an REI doctor. If you want her specifically for those procedures , this goes for IVF retrieval and transfers as well, you would have to wait for an opening on her schedule. If you don’t want to wait there are other REI doctors that can perform the procedures at the earliest date and report the findings to Dr. Farah-Eways. The process at Kaiser works where a REI doctor like Dr. Farah-Eways may be doing diagnostic test 1 day, clinic appointments the next day and IVF retrieval and transfer the all day the entire next week. So if you’re looking to see the same REI doctor/nurse at each appointment and it wouldn’t be okay with you if you didn’t, she may not be the REI doctor for you.
During treatment, were you treated like a number or a human with Lisa Farah-Eways at Kaiser Permanente NorCal?
Although Kaiser’s REI department is extremely busy, Dr. Farah-Eways has always been extremely kind and explained the plan of care and next steps for my fertility treatment in great detail regardless of the amount of time it took. Also, I was scheduled to have a saline sonogram with trial transfer, and endometrial biopsy 2 days after shelter in place orders were mandated due to COVID-19. This order encouraged healthcare facilities to cancel procedures that are considered to be “elective”. Although my scheduled procedures were considered to be elective, Dr Farah-Eways had her staff immediately call me and reassure me that she was still willing to do my procedure if I still wanted to keep my appointment and that she understood how important it was for me to complete these procedures as they are scheduled only on specific days within a given cycle. The fact that she and her staff allowed me to keep my appointment made me feel like they respected both my time and the emotional toll it takes to have to wait a whole cycle before a procedure can be scheduled again. This made me feel like human and not just a number to them.
Describe the protocols Lisa Farah-Eways used in your cycles at Kaiser Permanente NorCal and their degree of success.
Lab work was completed to rule out any clotting disorders or genetic abnormalities prior to IUI Pre-IUI supplements: -Prenatal -Co-Enzyme Q10 600mg/day -Methylfolate 1mg (folic acid in prenatals may not be absorbed in its form) 2000U of vitamin D (prenatals amount not enough) -Probiotics ( to help with gut health) For each IUI the medication protocol was used to make me have a stronger ovulation and produce more than one egg in attempt to increase the chances of successful IUI. Dr. Farah-Eways confirmed with tests and from multiple previous pregnancies that unfortunately resulted in losses, I ovulate regularly on my own. The progesterone was used to proactively to help aid in preservation of pregnancy if the IUI was successful. IUI was chosen as a first line of treatment because we had not been able to conceive naturally for close to 1 year after our 3rd miscarriage: IUI #1: femara 2.5mg/ twice per day x 5 days Ultrasound day 7 (verify mature follicles) Ovidrel for timed ovulation IUI 24-36hr following Ovidrel injection IUI #2: femara 5mg/ twice per day x 5 days Ultrasound day 7 (verify mature follicles) Ovidrel for timed ovulation IUI 24-36hr following Ovidrel injection Progesterone vaginal suppository 1x/day IUI #3: femara 7.5mg/ twice per day x 5 days Ultrasound day 7 (verify mature follicles) Ovidrel for timed ovulation IUI 24-36hr following Ovidrel injection Progesterone vaginal suppository 1x/day IUI #4: femara 7.5mg/ twice per day x 5 days Menopur 150units 1x/day x 5 days Ultrasound day 7 (verify mature follicles) Ovidrel for timed ovulation IUI 24-36hr following Ovidrel injection Progesterone vaginal suppository 1x/day Neither of the 4 IUIs were successful We had a natural conception 1 month after the 4th IUI attempt. That natural conception resulted in our baby not growing after week 6. I had a D&C in February. Followed by a saline sonogram, endometrial biopsy and trial transfer done in March by Dr Farah-Eways to clear me for the IVF process. The IVF protocol will include a Retrieval with Freeze All and Frozen Embryo transfer so that the embryos can have PGT testing to rule out genetic abnormalities. The medication protocol for IVF retrieval will include Follistim, Menopur, Omnitrope For FET, it will include Estrace tabs, Climara Patch, and progesterone vaginal suppositories
Describe your experience with the nursing staff at Kaiser Permanente NorCal.
I was seen by 3 Nurse Practitioners on different occasions including, Carla Schouten, Marion Colombo, and Lorraine Gyor. They were all compassionate, professional, and responded quickly to all emails and were also open to implement my input in regards to what I felt I needed to be added to my care plan.

Describe Lisa Farah-Eways's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Kaiser Permanente NorCal.
Dr Farah-Eways only performs single embryo transfer and cautions against multiple embryo transfer. She informed me that many studies have shown that single embryo transfers have less risk And higher success rate of making it to term live birth

Weaknesses: [Dr. Farah-Eways is] unwilling to put in more than 1 embryo at a time (at the beginning) until the 3rd round when she put in 2, which worked & producer a singleton. Strengths: she’s had good results and did change my treatment to remove a polyp which helped result in a singleton, but took till the 3rd round. ... Have your questions ready. She’s a busy doctor with not a lot of time to spend answering your questions if you aren’t prepared.
Very clean. Typical Kaiser office (seen at the Sunnyvale clinic, but transfer done in Fremont). Parking good at both. Chose because of location and proximity to my office. Front desk staff very helpful and remembered my name. ... All [nursing staff] very communicative, but busy office so sometimes wouldn’t hear back until the end of the day.

Overall like a human. It was hard having unexplained infertility to not have answers, but she did her best to answer all of my questions.
Describe the protocols Lisa Farah-Eways used in your cycles at Kaiser Permanente NorCal and their degree of success.
Unexplained infertility. Typical protocol re injections. 1st round put in a fresh 6-day embryo, resulted in miscarriage. 2nd round didn’t work. 3rd round did a polyp removal and had planned to do an endometrial scratch but didn’t do it because she said was unnecessary. Also agreed to put in 2 embryos instead of 1 since this protocol had worked for another patient. Resulted in a singleton whom I carried full term and is now 13.5 mo old.

All very communicative, but busy office so sometimes wouldn’t hear back until the end of the day.

All out of pocket. Nothing covered by insurance. Average prices.
Describe Lisa Farah-Eways's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Kaiser Permanente NorCal.
Very strong preference to only insert 1 embryo at a time. Said had resulted in too many multiples in the past and Board of OBs didn’t allow insertion of more than 1 unless there was a medical reason.
