During treatment, were you treated like a number or a human with Peter Klatsky at Spring Fertility?
The first time I came into the clinic in 2018 I felt like I was treated like a human and not just a number. My Dr. and all of the nurses knew me by name and by the end of my egg retrieval the started to feel like family.
When I came back in Feb 2020 to start the embryo transfer process, my experience was a bit different. It seemed like the waiting room was more crowded, procedure dates and times were changed 2-3 times due to scheduling issues (not due to my cycle) and there was some miscommunication happening. Thankfully, the missteps that were made were quickly corrected along the way and did not affect my treatment protocol.
Describe the protocols Peter Klatsky used in your cycles at Spring Fertility and their degree of success.
My husband and I had been trying for over a year to get pregnant before we came into the clinic. Dr. Klatsky preformed a saline sonogram, and HSG, an ultrasound and ordered blood work for both my husband and I. He then diagnosed me with Unexplained Infertility saying that all of the tests came out clear. He also took into account the results of my husbands sperm sample that we had previously had done. He recommended IVF, stating that IUI has less chances of working to get us pregnant. We went with his recommendation and started IVF asap. We did a retrieval under his care and did pgs testing at his recommendation to prevent miscarriage or failed FETs.
We then did a medicated FET that was not successful. Dr. Klatsky recommended a medicated FET (with intramuscular progesterone shots and estrogen patches), because during the retrieval he stated that my lining was pretty thin and that the medicated protocol would rule out the lining being a factor. Dr. Klatsky, as well as my husband and I were all pretty surprised when the first round didn't work, since everything went well, lining looked good and the embryo was a "perfect" 5AA. After following up after the transfer, Dr. Klatsky has changed his approach for our next FET to a natural transfer, as I reminded him of my history of pelvic tension issues, interstitial cystitis and spotting. He wants to make sure that endometriosis isn't a factor, although I don't have many of the symptoms and he has done the majority of non- invasive tests to rule it out. Just in case I do have it he does not want to give me estrogen for the next cycle, which causes endo to flare. It will be a risk because there is a chance my lining may not be thick enough on transfer day, but he thinks it is best to try something different than last time to rule out endometriosis or any other factors that may be preventing us from getting pregnant.

Laurel Maley is our nurse coordinator. She is a great communicator, responded promptly, is compassionate and always in the know of what medications I am taking and was stage of my cycle I was in. She was great and one of the best parts of my experience at Spring.

Overall Spring is a good clinic. From the waiting room to the Dr.'s offices it is very clean and welcoming. The nurses and staff are all very nice and greet you with a warm smile every time you are in the office. It seems like their client load has increased since we first came for our IVF retrieval in 2018, which I think was the cause to procedures times being altered multiple times and long waits for appointments. The billing department is very responsive, but it took many emails and hours over the phone with them to get the proper paperwork to try and submit to my out of network insurance. Dr. Klatsky recommended against putting in multiple embryos at my age (33) and with my diagnosis (unexplained infertility) as it could cause a high risk pregnancy (multiples). I don't even think the clinic allows people to ask for multiple transfers at my age or with my diagnosis, but I am sure there are certain exceptions that they may do so. We allowed the clinic to pick the best out of our 3 pgs tested embryos and asked for the gender to be confidential and not shared with us.

My tansfer procedure was rescheduled to 3 different times the day leading up to it, one of the times being only an hour and a half before the procedure was originally scheduled for.
Dr. Tran did my transfer the day of and I was not informed that it would not be my primary doctor Dr. Klatsky until Dr. Tran walked in to do the procedure and introduced himself. The changing of appointment multiple times and not knowing that the Dr. for my transfer had changed until the moment of my transfer added a lot of stress to an already stressful process. It was apologized for by the nurse and Dr., but was definitely very upsetting and frustrating.
Dr.K was easy to get a hold of and his team was always readily available. He gave us a very detailed consultation BEFORE we even joined Spring, and was very clear and positive when it came down to our decision to go through another round of IVF. I had multiple questions about the treatment strategy and we also had a very important conversation about a potential surgery that we decided not to go with prior to preparing for our embryo transfer. All in all, he felt like someone who actually cared about us!!

I was blown away the amazing treatment I received under Dr. Klatsky's care, and am so thrilled with the results of my egg freezing cycle with him.
Dr. Klatsky understood that I have some irregular factors (low ovarian reserve, irregular cycle.) He also knew that I'd gone through a cycle 1 year prior at a different NYC clinic that did not go well.
The biggest differentiating factor is that Dr. Klatsky customized my treatment from beginning to end, adjusting my meds throughout the cycle in response to every scan and blood test. Dr Klatsky and the team did not rush me into starting a cycle, were patient with my irregular cycles, made a point of understanding what was going on with my body before going ahead with forming a plan, and generally treated my case with the level of attention and thought it deserved... And the result was a doubly successful cycle under his care vs my cycle at the other fertility clinic.

I began with 8-9 antral follicles, 9 were retreived, and 6 were mature/frozen.

First retrieval - July 2019: did high stimulation protocol. I had 6 follicles so we debated doing high stim (a more typical cycle) vs low stim. He decided to go for high stim. Unfortunately, we ended up with nothing. 11 eggs retrieved, 5 were mature, 4 fertilized, 3 made it to day 3, 1 made it to embryo, did genetic testing and it was not genetically viable.
2nd retrieval - March 2020: Did low stim protocol. Primed with estrogen and then used Gonal-F and Menopur for about 2 weeks at a lower dose than before. Got 6 eggs. 5 were mature. 4 fertilized, all 4 made it to embryo stage. Did genetic testing and 3 viable embryos as a result. We were really happy with this outcome, given my number of follicles.

He is warm and compassionate, and has a quirky sense of humor...he will be candid and forthright...Mid-luteal cycle start for my second cycle as I responded poorly at the first retrieval...is kind, patient, and thorough...He followed up with me when the number of eggs retrieved at my first retrieval was not what we were expecting, with a commitment to figure out what was going on.
Spring is a nice clinic. I appreciate that it felt almost spa-like. For people with grueling work schedules, they are able to see you early morning (e.g., 7 am), and it runs like clockwork at that time. The receptionists are nice and remember your name/face. The early morning flexibility with appointments and the concierge-like service was the most important factor for me in choosing Spring vs. UCSF...Catherine is my patient coordinator, and she is absolutely wonderful in every way. Dr. Klatsky was there to sell us IUI/IVF. We had to push him to get off that track and discuss our bodies, where we are physically, and where we are on this journey (at the time, we weren’t ready for IUI/IVF). When I redirected the conversation, I accidentally referred to embryos as babies, prompting a long, condescending lecture on how embryos are a distinct protected class. As a patient, it is not my job to get all of the terms right. I do not have an MD. I should not be spoken to that way when I slip up on fertility vocab. It is my job to get what I need out of an appointment and with Dr. Klatsky, that was a struggle. He didn’t connect to what we wanted, he was focused from the get go on selling us IUI/IVF, and he became defensive and condescending when we tried to direct the conversation to where we needed it.

He called to inform me it was negative and genuinely sounded disappointed with me. During my visits he listened to all of my concerns and helped me discern what was "my fault" and what absolutely wasn't my fault.

1 IUI with Clomid because I needed 3 IUIs in order to move onto IVF for my insurance. (I did 2 other IUIs through Kaiser.) No babies produced. Then I did 1 round of IVF resulting in 4 perfect embryos - 1 failed fresh transfer and 1 positive FET - currently 8 weeks. (I had NEVER had a positive pregnancy test, so it's possible!)

I had no idea I needed to stay on top of my inbox for an email from them, even though I had already discussed everything in detail with the nurse at the clinic after my appointment. Halfway through my IVF meds, they simplified and only had 1 nurse contact me with next steps, which was more organized and thoughtful. I knew what to look for. I would suggest for the patient to set expectations on communication from day 1.
Dr. Klatsky is very friendly and his calm personality always put me at ease. He explained the process simply and made sure I understood everything before I left his clinic. Since we have setbacks, he knew it wasn’t what either of us wanted so he was apologetic and ensured he’s trying everything he could so we can get started and get going. I’m not sure which specific protocol Dr. Klatsky put me on, but I had the saline sonogram and he saw that my uterus was healthy and smooth so I didn’t undergo a lot of testing. As I’m young, healthy and technically not infertile, I will be on menopur/Gonal-f/cetrotide/lupron. I have insulin injections as well. Dr. Klatsky said my injection amounts are a little more aggressive so they’re hoping I will produce more good quality eggs.

Helen is really friendly and very responsive to her emails. IVF is covered through my work, but I realized their retrieval and transfer facility is out of my network so I had to pay for their facility fee which is not bad. Dr. Klatsky knows PGS/PGD testing is pricey so he works with a company that if they diagnosis you then they offer $99 PGS/PGD testing (not including shipping/storage fees). With all those fees included, the testing would cost $600 which is way better than $5k+.

He never pressured us to choose a course of action or even undergo subsequent fertility treatments, which he could have because it meant more revenue for him. For example, he even told us that we were likely to get pregnant on our own at some point and it was up to us whether we wanted to wait for that outcome or pursue treatments if unwilling to wait. We really appreciated that.

Helen was really supportive, responsive and showed great compassion. The only weakness we experienced was around proactive communication. There were multiple instances in which we had to email or call her to ask questions about the timing or process of the next stage of the treatment cycle instead of her reaching out to us. Furthermore, sometimes when she told us she would contact us with some update on X date, that date would pass with no contact requiring us to follow up. I think that much of this delay was due to the clinic being unexpectedly understaffed for the number of patients it treated so I think that Helen was just trying to stay afloat.

Just once in an email a nurse said the name of another medication I was not on, I disregarded it and she owned the error. It was not a big deal to me.
He [Dr. Klatsky] was great and a pro. Not super soft bedside manner but very smart and funny. He presented facts in a clear and accessible way and was very direct. I trusted him completely and though he could be a little flippant, I felt like he had my absolute best interests at heart... Went on birth control to regulate predict my period. Then, I started on Menopur and Follistem for about 12 days. I was going in basically every other day while Dr. K monitored with ultrasound and blood draws. He adjusted the med levels accordingly and was very in tune. He explained why he made the choices he did and I was really impressed with how he calibrated.
The nurses [at Spring Fertility] are so incredibly kind and responsive and the doctors are just sharp as tacks. I trust them to get the absolute optimal outcome and are really invested in their patients. Also, lovely touches - no hospital atmosphere, nice little gifts (chocolates after my egg retrieval!) and amazing snacks in the waiting room. I also love that I had one doc and one nurse for the entire process so they KNEW me and my case.

The doctor projected 10 - 12 eggs would be retrieved; however, to my surprise, 21 eggs were retrieved and all matured.

Okay The medical assistants vary in their abilities and some struggle with blood draws which can be frustrating. Weaknesses The finance team is a nightmare to work with if you are using insurance. Slow to respond if they respond at all and they struggle to effectively work with insurance companies on claims. I’ve learned a ton about medical billing during the process because of the errors and lack of proper communication with our insurance company. Be sure to understand the entire cost of treatment and check your bills and claims frequently.

Out of pocket costs were a bit more expensive than larger Bay Area clinics but they take insurance (big positive!) and you get more time with the doctor than in the larger clinics. They offer good deals for medication packages and genetic testing. Insurance costs have not been clearly described and it is extremely challenging to get clear answers (or any answers) from the spring fertility finance team.

The care coordinator and nursing staff is only available 9-5pm Mon-Fri - which sometimes made it difficult to get answers questions quickly if needed.

Spring fertility team are just amazing. They are all supportive, organized and friendly. They treat us as human and patient and not numbers. Whenever we showed up for an appointment, they knew our names from the minute we entered the clinic. They were all very passionate and well trained.
