she didn't push me to IVF but patiently answered my questions...Dr. Tazuke did the hysteroscopy and frozen embryo transfer. She was gentle and careful during the procedure. Before the transfer, she had the mock transfer to make sure that everything will go well during the transfer. She is usually busy and has tight schedules. If there was any urgency, she was able to squeeze her time to see me....
[at CCRM San Francisco] I have advocated for myself and ask questions through the portal or the nurse. Dr. Tazuke replied me even in the after hours if the nurse could not answer my questions. I wish that I could have more chances to directly talk to the doctor though...When I reviewed the transfer plan with my second nurse, she realized that it's a wrong protocol after I asked some questions. She immediately changed to the correct one...
How was your experience with Salli Tazuke at CCRM San Francisco?
During the first meeting with Dr. Tazuke, she didn't push me to IVF but patiently answered my questions. She didn't exaggerate the IVF success rate considering my case. Later on I decided to do IVF with PGT/CCS to minimize any future miscarriage as much as possible, she prescribed the comprehensive tests in order to find out any potential cause of my RPL. I went through 3 egg retrievals. Luckily, the first transfer succeeded and I am now pregnant. Dr. Tazuke didn't do these egg retrievals for me (the other two doctors in the same clinic did them due to their schedules). After the bad outcomes of my first egg retrievals and potential bad results of the second one, she called me and encouraged me not to lose hope, that I could also keep trying naturally if I didn't want to continue the IVF treatment. She also provided options for me to try next. Luckily, we eventually have normal embryos to transfer. Dr. Tazuke did the hysteroscopy and frozen embryo transfer. She was gentle and careful during the procedure. Before the transfer, she had the mock transfer to make sure that everything will go well during the transfer. She is usually busy and has tight schedules. If there was any urgency, she was able to squeeze her time to see me. When we discussed about the transfer plan, she insisted that Lovenox wouldn't bring much benefit to me and it's painful to inject. I am glad that she made this decision rather than letting me inject Lovenox just in case. If there is anything that could make my IVF journey less emotional, I wish that Dr. Tazuke could have explicitly warned me that I might go through multiple retrievals due to egg quality issues. Again, there are many factors that may impact the outcome and doctors may not know for sure before the treatments. As the treatments went on, I also learned more things about RPL and watched some videos from other IVF doctors online who explained RPL in detail. It's a surprise to know that there are two extreme solutions: one is to keep trying naturally and the other is to do IVF with PGT testing. Dr. Tazuke did mention that persistent is important for IVF. I would encourage people to advocate for yourself and prepare yourself with questions.

I would encourage people to advocate for yourself and prepare yourself with questions. If you are like me who tends to understand the whole process and why the doctor chooses this protocol not others, please don't be shy and ask questions.

I usually met Dr. Tazuke before and after one cycle of retrieval or transfer. During the cycle, the sonographer did the ultrasound scan for me and my dedicated nurse provided the new instructions from Dr. Tazuke. When there were bad results, Dr. Tazuke called me and provided her opinions about what to do next. Before I started IVF treatments, I was very worried that the clinic would treat me like a number. It turned out that it's better than what I expected. I have advocated for myself and ask questions through the portal or the nurse. Dr. Tazuke replied me even in the after hours if the nurse could not answer my questions. I wish that I could have more chances to directly talk to the doctor though.

I changed my primary nurse during the treatment because the previous one left the clinic. As patients, we need to closely follow up with them and ask questions. My first nurse is detailed and professional. My second nurse is very friendly and patient. It was a bit harder to hold on to a real person through the phone last year. It has been greatly improved since 2022. Some mistakes or miscommunications I experienced are as follows. My previous nurse forgot to prescribe some supplement for me after Dr. Tazuke mentioned it to me. I didn't follow up on that and only found out before the second retrieval. I am not sure if that supplement would change the outcome of the first retrieval. But it feels bad that I didn't try everything then. When I reviewed the transfer plan with my second nurse, she realized that it's a wrong protocol after I asked some questions. She immediately changed to the correct one.

According to others, CCRM has good labs and comprehensive protocols. I like them being more careful and thorough in terms of treatment plans. If it's possible, it would be nice to keep improving the client portal and communication channels between doctors and patients. It has been greatly improved since this year though. Please advocate yourself and prepare yourself with questions.

I have always felt comfortable with Dr. Tazuke in our visits and feel like I can ask my questions. Sometimes our telehealth visits have felt a bit rushed (when we are discussing test results) but I know that she is very busy...currently in stimulation for IVF, prepping for egg retrieval. I believe I am on an antagonist protocol. Priming was several estrogen patches and 3 Ganirelix shots about a week before my period. Current dosage is 75 IUs of Menopur and 225 IUs of Follistim per day, and I will be adding Ganirelix in a few days...
CCRM SF is an extremely efficient clinic. Every time I go there, I am amazed at the speed with which I am in and out (in a good way)...The portal is a little clunky but it's relatively easy to send messages and responses are always quick...she [nurse] is very responsive and always gets back to me very quickly whenever I have questions. All of the nurses have been very helpful and I never have to wait long for a response. My visits to the clinic and ultrasounds have always been pleasant...

I am always greeted warmly and during my in-person visits/procedures, Dr. Tazuke has always made me feel very comfortable.

I'm currently in stimulation for IVF, prepping for egg retrieval. I believe I am on an antagonist protocol. Priming was several estrogen patches and 3 Ganirelix shots about a week before my period. Current dosage is 75 IUs of Menopur and 225 IUs of Follistim per day, and I will be adding Ganirelix in a few days. Trigger is set to be a mix of HCG and Lupron. We have briefly discussed approaches to prep for the frozen transfer. Since I have endometriosis, Dr. Tazuke recommended either laparoscopic surgery or Lupron shots prior to transfer; I have not decided yet on which treatment strategy to pursue.

CCRM SF is an extremely efficient clinic. Every time I go there, I am amazed at the speed with which I am in and out (in a good way). Everyone, from the receptionist to the phlebotomist to the nurses to the doctors, are efficient and professional. I always feel like I am in good hands. Communication has been very good overall, although sometimes I've had to wait a little bit to get some answers from the financial coordinator. The portal is a little clunky but it's relatively easy to send messages and responses are always quick.

IVF cycle ($18,992) - includes CCS testing of 3-4 embryos (will be more if testing more embryos) and 1 year of frozen storage, $5000-$6000 for medication, $5392 for frozen embryo transfer, will be additional costs for hysteroscopy

I had one test done - the ReceptivaDX test - which was sent to a special lab. I was told that I would have my results in 2-3 days, and I waited for over a week with no word. I got an email from the lab itself about a bill, but still no results. So I contacted the clinic for an update and the next day they sent me my results. I wished I had received them sooner, when they were clearly ready sooner, as I was really anxious about receiving that information.

I had my first cycle at other clinic but failed. My amh is 2.0-2.3 and AFC is around 18. The first cycle I did at other clinic ended up with zero embryo. Then I went to see Dr. Tazuke at CCRM. The protocol she planned for me is very perfect and eventually I retrieved 19 eggs, 14 matured, 10 fertilized, 7 fertilized eggs turned to be blast and 4 embryos with high quality passed the genetic test. The four embryos are 5AA,5AA,5BA,5BB. I very appreciate Dr. Tazuke's help! She is very professional and knowledgable. Also, she takes care of her patients very much and always thinking of patient's needs first. She is the best doctor I have ever seen! I have a hyman tissue outside of vagina since I was born. I went to see many doctors and they all think it is fine. But I was very worried about this tissue could give my delivery a hard time. After vagina exam, Dr. Tazuke suggested me to do a small surgery to correct the position of this hyman tissue otherwise I would have a blood bleeding issue after delivery. She does not want me to suffer a painful during the vagina delivery and this hyman tissue makes my sexual life for a long hard time. Finally, she asked Dr. Salem who was on duty of my egg retrieval did the surgery on my hyman tissue. I was very grateful of Dr. Tazuke and Dr. Salem. Both of them have a very advanced surgical skills. After egg retrieval and uterine polyps removal surgeries, I even did not have any spotting on my underwear! The doctors at CCRM are amazing!

Describe the protocols Salli Tazuke used in your cycles at CCRM San Francisco and their degree of success.
The protocol I used is called microdose lupron protocol. I had four days vivelle patch and changed each one every other day. Then on day 4, I had the cetrotide injection. The first four days are priming treatment. The purpose of it is to suppress the eggs to the same baseline so they could grow up at the same pace. Then every morning I had 20 iu micro dose lupron injection and took one tablet of Dexamethasone. Every night I had 225 gonal F and 0.3 ml Omnitrope and 20iu micro dose lupron. I had the blood work and ultrasound exam every other day at CCRM. I was triggered at day 10 of stimulation. Finally I retrieved 19 eggs, 14 matured, 10 fertilzied, 7 embryos and 4 passed genetic test.

It’s been a long year of four retrievals and then two cancelled transfer dates (one was due to my mistake in missing a progesterone dose which put the transfer at risk, next time I ovulated). I was tired after the year but she was encouraging, suggesting that I hang in there, and here we are on the 3rd transfer 5 weeks pregnant. It’s early but the first ultrasound was promising... She recommended unblocking a tube and I found a surgeon to do that...I am 42, had diminished ovarian reserves, a blocked tube. It took 4 retrieval’s. First three had no viable embryos. The fourth resulted in two 5AA. I felt I hit the lottery. I then went and got the surgery to unblock the tube. I came back and we had two cancelled FETs - one due to a mistakenly late progesterone dose on my part, 2nd due to ovulation. She made the calls to cancel - they were good calls as disappointed as I was. We transferred this month and I am 5 weeks pregnant and hoping we are on the path.

How was your experience with Salli Tazuke at Palo Alto Medical Foundation?
Although I was with Dr. Salli Tazuke from an initial consult until before the first IUI (at which point we moved to another country), I felt that Dr. Salli Tazuke was very considerate and intelligent, and gave me a very accurate picture of the road ahead. She fully informed me of my diminished ovarian reserve, and said that we should do only 3-4 IUIs instead of the recommended 6. She also recommended starting IUIs directly, instead of timed intercourse with Clomid/injectables. She gave me plenty of reading material and was always available to meet me, and to answer my questions directly or via email. She was up to date with my test results and sent me notes on what the results meant, through the hospital's web interface.

Book an appointment well in advance. All the doctors at PAMF's fertility practice were booked solid for 4-8 weeks at any given time.

Dr. Tazuke was almost a non entity during the course of my IVF cycle. She did not perform any of my prep and never met me face to face during the course of my care. When my IVF cycle resulted in results that skewed negatively to range of results (Eggs retrieved to Eggs fertilized to Embryos), Dr. Tazuke's only guidance was to begin Acai supplements (through an affiliate link to purchase). She was either unwilling or unable to provide any guidance on what or how to adjust treatment in a subsequent cycle...
The nursing staff [at CCRM San Francisco] were very disorganized with managing prescriptions in the cycle which resulted in last minute orders to the pharmacy and stress around getting medications on time...The clinic is clean and efficient for appointments. There is a definite disconnect between the doctors, the nurses that perform your ultrasounds, and the nurse that handles your protocol. My doctor was completely absent from the process, and the nurses were giving very different indications of how well the cycle was progressing...

Dr. Tazuke was extremely detached from the process of my IVF cycle. I saw her twice during the process, in the initial consultation (sales pitch) and then after the cycle to discuss the results.

They used an Antagonist protocol.

The clinic is clean and efficient for appointments. There is a definite disconnect between the doctors, the nurses that perform your ultrasounds, and the nurse that handles your protocol. Dr. Tazuki was very nice and caring, but I do not think she is on the leading edge of fertility treatments. After my third IUI she realized she had not tested my husband's morphology which was less than 1%. Had we known this we would've gone straight to IVF. Next, she explained the differences between natural IVF and ICSI but did not steer us down the right path given my husband's morphology. I wish she would've been more authoritative in suggesting we do ICSI since she is the expert...
[PAMF - San Jose] Strengths: small and personalable. Weakness: not on the leading edge of fertility, not a well oiled fertility machine...No complaints [about the nurses], it really boils down to the lab and the doctor...Please look up her statistics on sart.org before moving forward. Yes, she is a very nice doctor, but given that you were are spending tens of thousands of dollars, I would rather have someone with much better statistics...

If you're nervous about the process of egg freezing, just do it - and just do it with Dr. Tazuke. If I had known what my experience would actually be like, I would have done it sooner!

Dr. Tazuke was incredibly approachable throughout my experience. She explained everything in a way I could understand and made me feel really comfortable. When I had questions about medication I was already taking affecting the process, her response was "YOU are our priority, not your eggs." She made me feel like a human and prioritized my individual needs in a manner I really appreciated. Throughout the entire process, I would remember what Dr. Tazuke said and knew that I was in a place with people who cared about my well-being. It never felt like the success of my procedure was the focus - it was MY well being and MY safety.

I worked closely with Rachel, my nurse and Angie, my care coordinator. Rachel went above and beyond to make sure I was ready, felt informed, and knew exactly what needed to be done for my treatment. She even gave me her cell phone # to text her when I had questions about medication and needed to share photos of something. Every conversation I had with her was reassuring and always made me feel better. Before I began the injections, I had an hour long appt with her where we went over EVERYTHING together. She was so patient as I asked a million questions and wrote everything down. Angie was incredibly kind and made me feel comfortable during what could be nerve wracking, stressful experience. Angie made sure I was comfortable with my medications/injections and answered my many questions. Her sweet, kind demeanor and the way she communicates makes you feel like you've known her for years and she truly cares about you as an individual. Overall, Rachel and Angie were wonderful to work with - I hope CCRM holds on to them!

From my first appointment, I loved CCRM. From their clean, welcoming environment and facility, to their wonderful staff, everyone I worked with was great. I did my entire process during COVID-19. I actually started the process in February and had to stop because of COVID (I hadn't started any medication yet, only preliminary appointments and testing had been done). Once CCRM opened back up for new patients to start their egg freezing process, I went back in and began again. Every necessary (and several that were just extra precautions) were set up. I never felt unsafe going into CCRM because I knew they were doing everything they could to make their patients feel safe. Feedback: The ONLY feedback I have for CCRM SF is in regards to their phone system. When you need to talk to a person and NOT just leave a message for someone, it can be really hard/time consuming/annoying trying to get to a human on the other end. Most of the time, I was forced to leave a message because the hold system would stop ringing. When you have time sensitive questions about medication, etc, it's much more reassuring to get your question answered live over the phone, so you can ask follow up questions, confirm your understanding, etc. That said, whenever I emailed the nurse team or left a message, they did get back to me very quickly.

I am extremely lucky and had insurance cover all but my annual deductible. I know this isn't the norm for most!

Dr. Tazuke took the time to review my history of prior cycles at another fertility clinic and worked with us to do things differently. She never rushed us and always took time to listen and answer questions.
Describe the protocols Salli Tazuke used in your cycles at CCRM San Francisco and their degree of success.
Dr. Tazuke suggested the use of a priming cycle and a growth hormone as part of the egg retrieval workup, which past IVF centers did not offer up. I believe this is what helped me get 3x more viable eggs than previous cylcles. She also. recommended taking an endometriosis test that helped diagnose the possibility of endometriosis even though I had no history or symptoms. The test came back positive which led us to treatment with Lupron to calm the ovaries to prepare for a transfer, which I also believe helped with a successful cycle.

Dr. Tazuke recommended transferring one embryo at a time due to risks, and we happily agreed...[she] knew the ins and outs of our infertility without ever having to read off the charts. She was constantly checking in on how we were doing throughout the process, and even checked in after graduating from the clinic.
Retrieval yielded 26 eggs, 19 mature, 17 fertilized with ICSI, and 7 day 5 embryos made...[approx cost] around 25,000 for retrieval and 5,000 for transfer...Beyond impressed with CCRM’s lab capabilities. Our case was severe MFI and they made us 5 high graded PGS embryos from one retrieval. Front staff was always friendly and helpful. Billing department while I was there was straightforward as they were not accepting insurance at that time.

Dr. Tazuke and staff worked their magic once again. We went back for our second transfer early last fall (first transfer resulted in our 2year old son) and I just gave birth to a beautiful baby girl. We would never have been able to make such high quality embryos outside the expertise of Dr. tazuke and CCRM labs. We owe both our children to the hard work and skill of CCRM San Francisco. Would highly recommend to anyone battling infertility.

Dr. Tazuke knew the ins and outs of our infertility without ever having to read off the charts. She was constantly checking in on how we were doing throughout the process, and even checked in after graduating from the clinic.
Describe the protocols Salli Tazuke used in your cycles at CCRM San Francisco and their degree of success.
I was on menopue and gonal f for retrieval. Retrieval yielded 26 eggs, 19 mature, 17 fertilized with ICSI, and 7 day 5 embryos made. PGS testing came back with 5 normals, 1 inconclusive, and 1abnormal. Transfer protocol was modified stim as my uterine lining responds best. Two transfers resulted in my two children.

As with any doctor, it is good to do some reading and research beforehand to prepare specific questions and make sure you make the most out of the precious appointment time. Going through the patients' Facebook group is also helpful.

She answered every single question I had thoroughly and most of all she was very supportive during this different process...when she performed a HSG on her, she was gentle, checked in with me at every step and also explained what she was going. The waiting room [at CCRM San Francisco] was always clean, stocked with snacks, magazines and relaxing music. The biller used to sit in an office with thin walls and people in the waiting room could hear. Once I gave the feedback, they moved that person right away. Doctors are working around the clock for our fertility needs!...most of my visits were on time. all the nurses were great and friendly. appointments were fast and efficient. there were some unorganized people at the front desk, but most of them were new and they were able to ramp up on training quick.

Dr. Tazuke treated me for infertility last year and I had a wonderful experience with her. She alwasy took her time with me during my visits, I never felt rushed and always was heard. She answered every single question I had thoroughly and most of all she was very supportive during this different process. I once had a terrible experience with a HSG at another client in the past, Dr Tazuke remembered this and when she performed a HSG on her, she was gentle, checked in with me at every step and also explained what she was going. We had a successful IVF experience and was able to conceive on the first transfer.
