Attention to Detail

Attention to detail is on a 1 - 10 scale, and is scored based on the rate, and severity, of errors committed by the clinic according to the patients.

For example, a severe error might be failing to inform a patient of critical information, while a small error might be failing to call a patient with results.

CDC Success Rates

This is the most recent data from the CDC. The CDC cautions that it should not be used to compare doctors and clinics. This data represents the outcomes of IVF cycles when using a person's own eggs.

For more information please see our FAQs.

Where you see a “?” on the chart, a clinic has done fewer than 20 cycles in this category and age range, so the CDC says showing percentages from this small data set would be misleading and unreliable.

No other options except IVF were discussed...My medical notes contained information that stated that all of this was discussed with me when it hadn't been...I blame her [Dr. Natalie Crawford] for not doing follow-up bloodwork, or an HSG, or any tests for that matter before I paid $30,000 out of pocket for IVF... As soon as any questions that challenged her suggestions were asked, all bets were off.
The clinic [Aspire Fertility (formerly RMA of Texas)] itself is run like a well-oiled machine as far as I was concerned. Other than refusing to allow me to transfer to another doctor when I did not get along well with Dr. Crawford, I had a very pleasant experience there & I was comfortable at each appointment...The staff was very nice, accommodating, & responsive. But Dr. Crawford didn't do any tests of her own & simply suggested (highly) that I START with IVF because I want a second child down the road.
How was your experience with Natalie Crawford at Aspire Fertility - Austin (Prelude Network clinic)?
If I could give 0 stars I would, but to be fair, the staff at Aspire were always very nice & accommodating. I don't really know where to begin with why you should avoid this place like the plague (specifically Dr. Natalie Crawford). I was referred here by my OBGYN after some slightly low test results, & during the very first visit (a consultation) I was immediately told by Dr. Crawford that it would be in my best interest to move forward with IVF. Mind you, I was 27 years old, with low AMH & slightly high FSH. My husband & I had been trying to conceive for less than a year. However, due to the fear those test results caused me, knowing nothing about infertility & the other options I had, as well as trusting my doctor's opinion, I truly believed that IVF would be the only thing that would get me what I wanted. So I went ahead & paid out of pocket for 1 round of IVF. Also, no more tests were run before I paid full price for this. We went solely based on the bloodwork I had received from my OBGYN. By no fault of the doctors, none of my embryos made it to blastocysts. Of course this was devastating, & my husband was very upset after doing more research, as he felt that Dr. Crawford talked me into something & used my vulnerability & desire for a child to persuade me into this huge (& expensive) step before trying anything else. He voiced this to her during our follow-up appointment, to which Dr. Crawford responded that he was "verbally aggressive" & interrupted him with "no, it's my turn to talk, stop talking". As soon as any questions that challenged her suggestions were asked, all bets were off. She was rude to my husband as well as myself, answering my questions with 1 or 2 words when I was already in a very fragile & confused state. She told me I likely had an egg quality issue, & would probably never become pregnant on my own without more IVF. I ended up making an appointment with another RE for a second opinion, & she used to work at Aspire. She told me that she would ask Dr. Skillern if I could transfer to her so that I could maintain some sort of normalcy & familiarity through my second round without having to see the same doctor. I was later told by this RE that I could not transfer to Dr. Skillern because "Dr. Crawford had told Dr. Skillern that my husband asked to leave the practice"; which never happened in any discussion that we had, a blatant lie. I was floored. So there went another option for me (or so it felt like). To make things even worse, I requested my records from Aspire, & while going through them, noticed so many false statements I couldn't believe it, despite everything that had happened. Dr. Crawford reported in these notes that during our first consultation, she went over all options of fertility treatment with me, & that "it may take several IVF rounds to have a successful outcome". NONE OF THAT WAS EVER EVEN BROUGHT UP TO ME IN THE SLIGHTEST! To the contrary, I asked one of the nurses after my appointment with Dr. Crawford was over if Dr. Crawford felt that I may need more than 1 round & she responded with "no, Dr. Crawford will tell us if she believes there's a high chance of that, & she didn't say that so that means she feels that this will truly get you what you want". She also wrote that risks of my egg retrieval were reviewed with me, such as needing a blood transfusion in severe situations. Again, never even mentioned in the slightest. Let me just clarify, I do not blame Dr. Crawford or Aspire for my failed cycle. I know that nature does it's own thing, & no doctor can be sure what the outcome will be when it comes to fertility. I blame her for being rude, for taking advantage of my vulnerability & fragile mental state at the time (that I'm still very much affected by), for straight-out lying in my medical notes (which I believe is very illegal). I blame her for not doing follow-up bloodwork, or an HSG, or any tests for that matter before I paid $30,000 out of pocket for IVF. I want to say more, but I'm out of room! P.s. I am now 15 wks pregnant (naturally!!!) with a little girl :)

When first researching her [Dr. Natalie Crawford], I didn't like her strong social media presence and the fact that she talked about patients (without mentioning their names) on social media...My first consultation was OK. During one point in the consultation I got upset (Infertility is emotional people, but I'm preaching to the choir, you all know this.). Dr. Crawford just kept talking and didn't stop to address my emotions. This should have been my first red flag.
During my previous visit, I mentioned to Dr. Crawford, my fertility coach, and other staff [at Aspire] how uncomfortable my husband would be giving the SA. Even leading up to our appointment, I spoke back and forth with office staff on the possibility of him doing it at home and bringing his sample in...they placed my husband in a room right by the nurses desk and the waiting room. He could hear every word the nurses said, every person who checked in at the desk, and even when they called me back to our consultation room. Why would they have not taken notes about how uncomfortable I told him he would be and placed him in the room furthest away from everyone? Instead, they placed him in the least private of all the SA rooms
How was your experience with Natalie Crawford at Aspire Fertility - Austin (Prelude Network clinic)?
Summary: Dr. Crawford has poor bedside manner when dealing with emotional patients. My OGBYN referred me to Dr. Crawford. When first researching her, I didn't like her strong social media presence and the fact that she talked about patients (without mentioning their names) on social media. I also lived over an hour away from her office, so it would be a pain to make the trip often; however, my OGBYN loved her so I decided to give her a try. My first consultation was OK. During one point in the consultation I got upset (Infertility is emotional people, but I'm preaching to the choir, you all know this.). Dr. Crawford just kept talking and didn't stop to address my emotions. This should have been my first red flag. However, I let it go with the thought that maybe she thought if she just kept talking I would be able to pull myself together and we could move forward. I came back for my second appointment for my husband to do a semen analysis (SA). Once his SA was complete, Dr. Crawford would be able to go over next steps with us. During my previous visit, I mentioned to Dr. Crawford, my fertility coach, and other staff how uncomfortable my husband would be giving the SA. Even leading up to our appointment, I spoke back and forth with office staff on the possibility of him doing it at home and bringing his sample in. Ultimately, he decided to do the analysis in the office because we were concerned with the 1 hour time frame that we had to get the sample to the lab (remember, we live 1 hour away) . The day of our appointment, they placed my husband in a room right by the nurses desk and the waiting room. He could hear every word the nurses said, every person who checked in at the desk, and even when they called me back to our consultation room. Why would they have not taken notes about how uncomfortable I told him he would be and placed him in the room furthest away from everyone? Instead, they placed him in the least private of all the SA rooms. Unfortunately, he was not able to provide a SA. We then waited on Dr. Crawford for our consultation. We were greeted by a nurse who said Dr. Crawford would not be able to see us today because my husband had not been able to provide a SA. I immediately start crying. First, We had driven an hour, how can she not just come into the room and talk to us and explain possible options (option 1: if his SA came back good. option 2: if it came back bad). Second, I'm her patient too, not just him. Come talk to me and my husband about what's going on with me and the blood work results from the last appointment. Because I got upset, the nurse went to get Dr. Crawford. As soon as she entered the room Dr. Crawford was on the defense. Instead of giving me a chance to talk and explain my concerns, she immediately started explain why it was unethical for her to treat us without his SA. Unfortunately, I tried to talk over her in order for me to express my concerns. Her response: "I feel like you are personally attacking me." UMMM.... WHAT?!? She then proceeded to say "It's unethical for me to f***ing take your money without his SA." So now she has refused to see us, said I was attacking her, and she cursed. I should have trusted my gut from my first consultation when she ignored me when I got upset and kept talking. Based on her reaction to my emotions at my second appointment, she clearly needs more work in how to properly deal with patient emotions. I needed a hug, a "it's going to be OK, here are next steps," not cursing and refusal to see me.

Describe the specific things that went wrong at Aspire Fertility - Austin (Prelude Network clinic).
After my first consultation, I was supposed to take an antibiotic. When I got to my pharmacy, over an hour after my appointment, they still did not have my prescription. I emailed my nurse and she immediately called it in, apologized, and asked my pharmacy to do what they could to fill it quickly because I was waiting.

Dr. P [Prokai] was always patient with all my questions...took Progesterone shots pre transfer and until week 10 of pregnancy. Estrace 6mg (2mg 3 times a day) as well as estrogene patches (2 patches every other day...i had to ask questions rather than information always being presented. And since I had previous experience I knew what to ask...I had a much clearer picture of what to expect along the process...
They [Aspire Fertility Austin] are clear on costs up front, they provided me a detailed calendar of medications, appointments, etc. I had a much clearer picture of what to expect along the process. The clinic was never overly crowed like the other clinic I had gone to, so I didn't feel like a number in a cattle call...Great at getting back to you quickly. Compasionate and caring. The one thing is i had to ask questions for some things because she didn't always proactively tell me things...
How was your experience with David Prokai at Aspire Fertility - Austin (Prelude Network clinic)?
Of the two fertility clinics I've tried, Aspire has been a breath of fresh air. They are clear on costs up front, they provided me a detailed calendar of medications, appointments, etc. I had a much clearer picture of what to expect along the process. The clinic was never overly crowed like the other clinic I had gone to, so I didn't feel like a number in a cattle call. I felt like they actually had the same goal in mind as I did, to get pregnant. Where my other clinic I felt like they just really wanted my money. I'd recommend Aspire and Dr. P for sure. There's always a few things they could do better, i had to ask questions rather than information always being presented. And since I had previous experience I knew what to ask. However overall I still give them a 9 out of 10 compared to my other experiences. I felt like they all cared about me and our outcome and gave us the time and patients we needed during the process.
What's one piece of advice would you give a prospective patient of David Prokai at Aspire Fertility - Austin (Prelude Network clinic)?
Make sure to ask questions. If you don't know how to give a shot, make sure to ask the nurse. It might be because I had already gone through the experience before. They are much more clear on the process than others, but still didn't cover everything. Do your research no matter who you see and ask questions.
During treatment, were you treated like a number or a human with David Prokai at Aspire Fertility - Austin (Prelude Network clinic)?
Great customer service. They knew my name when I called, were responsive and Dr. P was always patient with all my questions.
Describe the protocols David Prokai used in your cycles at Aspire Fertility - Austin (Prelude Network clinic) and their degree of success.
We only did an embroyo transfer with Aspire. They use a pretty agreesive approach compared to my other fertility clinic, which I much appreciated. They were great at making sure the uturus lining was the right size - over 7mm. I took Progesterone shots pre transfer and until week 10 of pregnancy. Estrace 6mg (2mg 3 times a day) as well as estrogene patches (2 patches every other day.
Describe your experience with Aspire Fertility - Austin (Prelude Network clinic).
Great at getting back to you quickly. Compasionate and caring. The one thing is i had to ask questions for some things because she didn't always proactively tell me things (i.e. how to do Progesterone shots). But overall really felt like she cared, she knew my name and my case well.
Describe the costs associated with your care under David Prokai at Aspire Fertility - Austin (Prelude Network clinic).
My care was a different cost because we used a donor service. We bought a package that had the ability to refund if we don't get to a live a birth through 6 donor cycles. That package was $39k plus we did pay for embroyo testing $4100 and additional embroyo biopsy $1500. I believe we also paid a little extra for a mock trial required by the donor service, maybe $500.
What specific things went wrong at Aspire Fertility - Austin (Prelude Network clinic)?
  • Failed to call with results
Describe the specific things that went wrong at Aspire Fertility - Austin (Prelude Network clinic).
Oh i wouldn't say "go wrong" - i just didn't get a call one time for extra blood work I asked for. We had to monitor my TSH and Vit D during my process due to my health issues. The question asked if anyone ever didn't call me with results, it happened one time and wasn't a big deal. I just called them.

We transferred one embryo under Dr. Prokai's recommendation. He fully explained the trade offs between implanting one vs. two...During our conversation, I felt more heard and seen than I had ever felt during my year of fertility treatment...[he] gives you first class care, but also understands how to be there emotionally for you is incredibly important
The entire [Aspire Fertility] nursing staff rallied around us and helped us save our cycle after we had to transfer mid-cycle from [another clinic]...I was so devastated to leave him and his incredible staff who rallied around us and gave us the care and support we needed to make this little boy. I will never be able to thank the team at Aspire enough for the wonderful gift they have given us. We love them like family. They want constant updates from me and my sister
How was your experience with David Prokai at Aspire Fertility - Austin (Prelude Network clinic)?
I can't say enough good things about Dr David Prokai. He is the most emotionally supportive, talented, and caring doctor that I have ever met. Fertility treatment is so incredibly traumatic. Having a doctor who not only gives you first class care, but also understands how to be there emotionally for you is incredibly important. Dr. Prokai genuinely cares about his patients. In 2020 I was working with another fertility clinic in Austin and my body wasn’t responding to stimulation treatment. I was told that I needed to start exploring donor eggs. I went to Dr. Prokai for a second opinion and I felt an immediate connection to him. During our conversation, I felt more heard and seen than I had ever felt during my year of fertility treatment with my other clinic. At my previous clinic I felt like a number in their system, and had no emotional connection to my doctor. During my appointment with Dr. Prokai I remember him saying "I understand that every step in this journey is a loss, I can only tell you what I would tell you if you were my sister, and if you were my sister, I would agree that a donor egg is your best chance". I was blown away, my other doctor had never acknowledged the emotional weight that fertility treatment carries. We stayed with my old clinic and went forward with stimulating my sister Sarah, who agreed to be my egg donor. Unfortunately, after a horrible experience with that clinic, we were forced to abandon treatment mid-cycle. I called Aspire Fertility in tears on Friday night and they called Dr. Prokai who not only talked to me that Friday night, but agreed to rush us in for an appointment. He rescued our cycle and was able to continue Sarah’s stimulation. During Sarah’s egg extraction, she grabbed Dr Prokai’s hand and said “you’re such a blessing for our family. Thank you so much for everything you’ve done. I believe god gave you to us.” Dr. Prokai got misty-eyed, but is a superstitious man so didn’t congratulate us just yet. Even though my sister also has a low ovarian reserve, the cycle resulted in three grade A healthy and viable embryos! Not only did Dr. Prokai make me feel taken care of emotionally and clinically, he also made my sister Sarah feel cared for as one of his own patients- not just “the donor.” At one point during our treatment, it looked like we were going to have to stop her cycle and try again with another medication protocol. Thankfully, Sarah started responding to treatment and we were able to move forward. So many ups and downs. Dr. Prokai confessed that he was so worried that he would have to call off the cycle that he had spent the weekend practicing how to tell us with his wife, so he could make sure that he told us in as gentle of a way as possible, so we wouldn’t lose hope. This is who he is. He cares so much. We are now three months pregnant with our baby boy! Due on my sister’s bday- another blessing. Dr. Prokai wanted so badly to FaceTime Sarah & her husband during our first heartbeat appt, and it was such a beautiful moment, every team member on board, listening to our miracle’s strong heart beat. During our last appointment with Dr. Prokai I was a sobbing mess. I was so devastated to leave him and his incredible staff who rallied around us and gave us the care and support we needed to make this little boy. I will never be able to thank the team at Aspire enough for the wonderful gift they have given us. We love them like family. They want constant updates from me and my sister. It has been such a long and hard journey, but I could not be more grateful that it was Dr. Prokai who helped us create this little life. It was all meant to be, but with the right doctor.

Describe the protocols David Prokai used in your cycles at Aspire Fertility - Austin (Prelude Network clinic) and their degree of success.
Dr. Prokai stimulated my sister to secure donor eggs, he used birth control, injectable hormones and estrodial (among other medications which I can't remember). For my transfer he used birth control, estrodial, progesterone injections and other medications I can't remember.
Describe your experience with your nurse at Aspire Fertility - Austin (Prelude Network clinic). (Assigned nurse: Inthira)
The entire nursing staff rallied around us and helped us save our cycle after we had to transfer mid-cycle from Texas Fertility Center. They took our call on a Friday night and got us in Monday for an appointment with Dr. Prokai. They cried with us, celebrated with us, and were incredible caretakers.
Describe your experience with Aspire Fertility - Austin (Prelude Network clinic).
He [John Wu] seemed genuine - called me himself or explained things when I didn’t get an update. Was apologizing for a mistake the lab made etc...He explained things in an understandable manner. I didn’t leave with questions of his explanations. I did however feel post failed transfer that there was things that should have been explained better which could have told us more information - having now done more research of my own...Do your own in-depth research and ask questions

I felt like I could be myself and be heard and supported by Dr Prokai and his team. Infertility is a horrible thing to go through and it made such a difference feeling like I could be participatory in my care by asking questions and not just be told what the cycle was...He spent almost an hour talking with us and was willing to explain anything I asked
Great covid protocol. They [Aspire Fertility clinic] would only have one person in the waiting room. Really nice facilities and you can always get someone on the phone which was an big issue with my last clinic. They even got me needles when I couldn’t get the right kind from my pharmacy...FET was about $4000...They [nurses] are super responsive and always made sure someone got back to me
How was your experience with David Prokai at Aspire Fertility - Austin (Prelude Network clinic)?
Changing doctors to Dr Prokai was the best decision for me. Yes, I did have a successful transfer and that of course makes things extra great...but I felt like I could be myself and be heard and supported by Dr Prokai and his team. Infertility is a horrible thing to go through and it made such a difference feeling like I could be participatory in my care by asking questions and not just be told what the cycle was.
What's one piece of advice would you give a prospective patient of David Prokai at Aspire Fertility - Austin (Prelude Network clinic)?
If you want to feel truly like a person and not a number, go to Dr Prokai!
During treatment, were you treated like a number or a human with David Prokai at Aspire Fertility - Austin (Prelude Network clinic)?
I first met Dr. Prokai after a failed FET with another clinic. I had many questions about my situation so I wanted a second opinion. I knew I would only switch to him if I really really felt it was the right fit. My husband and I met with him (remotely due to covid). He spent almost an hour talking with us and was willing to explain anything I asked. Many times Dr Prokai would call me personally if I had a question about anything. He is a great doctor and a down to earth person. Cassy (his nurse) and Marci (his patient point person) are wonderful and attentive.
Describe the protocols David Prokai used in your cycles at Aspire Fertility - Austin (Prelude Network clinic) and their degree of success.
After a failed FET with a pgt tested normal Embryo I wanted to rule everything out. Dr Prokai suggested a cycle that started with about 2 weeks of birth control. Then we did a mock transfer (I had a extremely painful first transfer and this was a huge selling point for me that he said he always does a mock to make sure he knows how to place the catheter before it’s actually the time for the transfer). He also did a test for inflammation and that was normal. I did estrogen and PIO before the transfer. Had the transfer (it was soooo easy) and then continued the medication until 12 weeks pregnant.
Describe your experience with your nurse at Aspire Fertility - Austin (Prelude Network clinic). (Assigned nurse: Cassy)
Cassy (nurse) and Marci (coordinator) are great! They are super responsive and always made sure someone got back to me. When I first started seeing Dr Prokai Cassy was out of the office for a family emergency and so I had a couple emails to her that took a while to get back. But that is soooo out of character for her I learned. Also dr P and Marci would call me and answer questions for Cassy when she was out so I was taken care of!

Dr. Kelley is very warm and explains every detail thoroughly. She definitely was honest with my protocol and setbacks and made me feel comfortable along the way. My only "weakness" for her is that you don't get to see her for every monitoring appointment along the way and other doctors in the practice are not nearly as thorough and understanding as she is...I have low AMH (1.5) and my husband has low sperm count, motility, and shape. Therefore, IVF was our best option. For my first retrieval, I did a Lupron lead in + Microgestin birth control..
All female staff [at Aspire Fertility] including doctors and nurses. I love Dr. Kelley & Dr. Crawford - they are both very compassionate and knowledgeable. Dr. Crawford is wildly popular in the instagram community...Monitoring was between 7-9 am. That worked well with my schedule because I could go in before work...

What's one piece of advice would you give a prospective patient of Angela Kelley at Aspire Fertility - Austin (Prelude Network clinic)?
My advice with any doctor would be do some research before-hand, so that you have a list of questions to ask. Dr. Kelley will answer all of them and put your mind at ease.
During treatment, were you treated like a number or a human with Angela Kelley at Aspire Fertility - Austin (Prelude Network clinic)?
Dr. Kelley is very kind and patient with explaining every detail and answering every question. I never felt like she was rushing to get to the next patient. She remembered me and my situation and was very warm every time I saw her. This is different from another doctor in the practice.
Describe the protocols Angela Kelley used in your cycles at Aspire Fertility - Austin (Prelude Network clinic) and their degree of success.
I have low AMH (1.5) and my husband has low sperm count, motility, and shape. Therefore, IVF was our best option. For my first retrieval, I did a Lupron lead in + Microgestin birth control. There was a mix up on how long I was supposed to take BC, so I had to push my cycle out a month. Then when we started back, I had two lead follicles at my baseline, so we had to continue Lupron for a few days. Then I started a Gonal-F (300 daily) and Menopur (150 daily) protocol for about 10-12 days. Dr. Kelley retrieved 7 eggs but only 2 were mature and produced 1 emryo that made it to Day 6 blast and tested PGS Normal. For the second cycle, Dr. Kelley suggested we add a couple of days on stims to give the eggs more time to mature and we also added Omnitrope (25 units daily) to help with egg quality. This time we got 7 follicles, 5 were mature and 3 made it to blast. We got 1 PGS normal embryo, 1 low mosaic, and 1 abornormal.
Describe your experience with your nurse at Aspire Fertility - Austin (Prelude Network clinic). (Assigned nurse: Morgan Weaver)
I loved working with Morgan. She was always available and prompt to get back to me with any questions I had. My only issue with Morgan was that we had a mix up on how long I was supposed to take Birth Control, which caused me a month delay in my cycle. Better communication and extra explanation on the timing/making sure I understood the ramifications if we messed up would have been helpful here.
Describe your experience with Aspire Fertility - Austin (Prelude Network clinic).
Pros: All female staff including doctors and nurses. I love Dr. Kelley & Dr. Crawford - they are both very compassionate and knowledgeable. Dr. Crawford is wildly popular in the instagram community. Cons: Dr. Skillern is the most tenured doctor there, but she has a terrible bedside manner. I always felt like a number at the meat counter when she was doing my monitoring appointments. Another con is that they have a schedule between the three doctors for monitoring, so I only saw Dr. Kelley (my doctor) a couple of times before my retrieval. By far my BIGGEST issue with Aspire Fertility is the way they have handled COVID-19. MY assumption is that because they are part of a large network (Prelude), my clinic doesn't have a tone of control over their response. While they squeezed my second retrieval in, we had to bump it up a couple of days. This was concerning as my first retrieval was too quick and we only had 2/7 eggs mature. Since then, they have been closed to all treatments, while the other big clinic in Austin (Texas Fertility Center) remains open for business. Like I said, I blame this response on being part of a large conglomerate and the cowardness of Aspire's legal team. If I had known this would have been their response to the pandemic, I would not have sought treatment here in the first place, as I have now had my embryo transfer put on hold for a month and am still waiting.
Describe your experience with your monitoring appointments at Aspire Fertility - Austin (Prelude Network clinic).
Monitoring was between 7-9 am. That worked well with my schedule because I could go in before work. I generally had to wait, which was annoying. The other negative I had was that I rarely saw my doctor, as each of the three doctors rotated on a schedule. I think this is a miss. One of the three doctors, Dr. Skillern, definitely made you feel like a number at the deli counter and I had to stop her from leaving before I could ask her questions.
Describe the costs associated with your care under Angela Kelley at Aspire Fertility - Austin (Prelude Network clinic).
I use Progyny insurance, so I'm unsure of the out of pocket costs. I did have Aspire quote me on an additional cycle if we needed to do a third egg retrieval. I don't have itemized costs yet, but the total for an egg retrieval cycle with all monitoring, genetic testing, assisted hatching, anesthesia, ICSI, and cryopreservation for a year was $17,999. Meds are an additional $5-6k.
Describe Angela Kelley's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Aspire Fertility - Austin (Prelude Network clinic).
Dr. Kelley and the other two physicians at Aspire lead with eSET. I agree with this approach.

Describe the specific things that went wrong at Aspire Fertility - Austin (Prelude Network clinic).
I already mentioned them - we had the miscommunication on the number of days that I was supposed to take birth control. This cost me 2 weeks of Lupron and an additional month.

Dr Skillern really promotes single embryo transfer. ... She was available on nearly every visit. ... She does not sugar coat things, but she shows compassion. She is relatable, trustworthy, honest. Many tests to determine reasons for infertility. 4 egg retrieval’s. Genetic testing on embryos. Biopsy of my uterus to determine correct day of implant. 2 embryos implanted. 3 frozen embryos.
They [Aspire Fertility] respect your time with visits. Their facility is immaculate. Their team is spot on. They are a bit more expensive than other, but I’d pay it all again after my experience. Post baby they invite you back in for a foot print ceremony. They personalize the entire experience. Having the acupuncture in the office is a plus as well! Misty is wonderful.,

Despite being diagnosed with fibroid (which are a KNOWN cause of infertility), she [Dr. Amanda Skillern] wanted us to move forward with IUI or IVF. I asked about surgery to remove them to which she replied, "No, we'll leave them in. That would be a bloody mess." She only wanted money from us and didn't care about the outcome.... She was candid about me having fibroids but completely focused on getting us to move forward with IUI or IVF. She never provided any other alternatives and was just matter of fact in saying that I had "unexplained infertility" -- which was most definitely not the case after a second opinion
They [Aspire/RMA of TX] don't provide you much information, even if you ask for it regarding IUI or IVF pricing and procedures. That's a huge life decision that you don't just jump into blindly. After two weeks of calling every other day, the info they provided was only high level pricing and zero details. They need to get their stuff together!... When seeking a second opinion, a records request was sent 3 weeks in advance. My new doctor never received any records. While sitting in the doctors office, we called them about this and they blatantly lied to us telling us it had been sent, but they'd send it over now. Still to this day, it was never received by the new doctor.

Initially just copays. But ended up having to pay out of pocket because they fraudulently coded my consultation as "infertility". All I went to them for was diagnostics!!! She never did ANY procedures or treatments on me.

We thought everything was being check with our blood draws and come to find out she [Dr. Skillern] never tested our husband's testosterone levels. We found this out after 2 retrievals and couldn't believe this wasn't done before hand. We wasted two potential cycles and hard earned money as insurance didn't cover IVF. Extremely disappointed.
A year after we were called by RMA that we owe money for the storage of our abnormal embryos, never once contacted about this until they asked for money. It was resolved but couldn't believe it when we got the invoice. They offered us a discount for a third cycle and never discussed time restraints when when asked. We had some unfortunate medical issues arise and we unable to commit until a year after. Now the discount is no longer available! A few months ago it would of cost $3,000 with insurance covering upto $10,000 and we're being told $17,000+ out of pocket.

She was always so busy with patients, you only get to talk with her if you schedule an appointment after that you only get to email with your nurse and she's busy too. It's such a stressful experience would have hoped they'd be there for you.
Describe your experience with Aspire Fertility - Austin (Prelude Network clinic).
We had two unsuccessful retrievals when we were trusting in Dr. Skillern to have our best interest at heart. We went to RMA with Male infertility issues and couldn't believe after the two cycles that she never tested my husband's testosterone levels which is a key factor in three success of IVF. We spend over $30,000 without help from insurance and are still trying to work off that debt. My husband and I both felt very comfortable with Dr Arredondo from the initial consultation throughout our years of treatment. His passion and knowledge was very evident!! We loved how easy it was to just sit and ask questions... Because his main office is in San Antonio, we did not see him at every appointment but we had equally great interactions with other doctors in his practice!
The staff at RMA felt like a true team! There were many moving parts to our particular situation with using adopted embryos and we felt well informed and guided through many unknowns!!... I like the location of the office and that the appointments have always been on time. The RMA team is professional and personal! To a new patient, I would say to have a consultation with more than one doctor and find who you feel connected to because that dynamic is very personal.

My husband and I both felt very comfortable with Dr Arredondo from the initial consultation throughout our years of treatment. His passion and knowledge was very evident!! We loved how easy it was to just sit and ask questions...and felt like his questions to us were not just clinical but really covered emotional, financial, relational topics as well! Because his main office is in San Antonio, we did not see him at every appointment but we had equally great interactions with other doctors in his practice!
What's one piece of advice would you give a prospective patient of Francisco Arredondo at Aspire Fertility - Austin (Prelude Network clinic)?
If it is important to you to see the same doctor at every appointment, you may want to travel to San Antonio to see Dr. Arredondo or choose another doctor at RMA who is always in Austin! We also fell in love with Dr. Skillern, who did our embryo transfer and who will do our next transfer as well!

With our embryo transfer, we wanted to try a natural cycle because I had taken many medications in previous fertility attempts. He and his staff were supportive in our wishes but also advised and encouraged when to deviate from our original plan, such as the need for progesterone after the transfer!
Describe your experience with your nurse at Aspire Fertility - Austin (Prelude Network clinic). (Assigned nurse: Inthira-coordinator)
I never had a negative experience with any care at RMA! When facing challenges getting our embryos transported from Houston to Austin, as well as getting my thyroid medication dosage just right, our transfer was delayed many times. It was frustrating and emotional, but I never felt like I was tackling it on my own! My coordinator, Inthira, was so positive and was in constant communication! She felt like a true friend, helping me figure it all out!

She [Dr Amanda Skillern] was very honest and transparent. No sugar coating. She knew me and my husband and made me feel very comfortable. I never felt like their office was extremely busy and they were getting me in and out. I always had great conversation and she answered all my questions. She even gave me her email address if I needed anything.
I was fortunate enough to have insurance that covered IVF [at RMA of Taxas in Austin]. I only had to pay 20% after my deductible was met. I did pay for CCS services and that was $2,500 I think cash payment of IVF ranged from 10k-25k...All the staff were fantastic! I had a fertility coach who helped guide me with our plan and medications. She was great at helping and always was available to answer questions.

Got pregnant on our 1st round of IVF with RMA under her care. She was fantastic and I highly recommend. She always set expectations and was in every appointment besides blood draws. Loved her!
What's one piece of advice would you give a prospective patient of Amanda Skillern at Aspire Fertility - Austin (Prelude Network clinic)?
She will be very frank with you and not sugar coat. She does not talk to much about the future and talks about today and the next step. I'm a huge planner, so it took me awhile to adjust to this, but with infertility it was the best for me. You can't plan to far ahead with this. You need to be in the moment and think about today and the next appointment and what needs to be done between then.

Dr. Skillern is not the most touchy-feely, but she is very honest and pretty easy to talk to. She was completely up-front with us from the get-go, and seemed to communicate with myself and my husband as equals. Also, did every appointment except for blood draws, and was always willing to answer a call or email if I had questions...I always felt like a human, the practice seemed relatively small, never a long wait or crowded waiting room, and the doctors, nurses, and staff were always warm and friendly and answered any questions I had quickly and thoroughly...
The [RMA - Austin] clinic is small and cozy. It is well decorated and relaxing and there was never more than one other person in the waiting room in all my visits. Also I never waited more than 5 minutes for an appointment and the doctor was never double-booked for an appointment time (unlike other clinics in town we tried). The clinic staff is also well organized and easy to deal with in case of insurance issues or other admin questions...

Describe the protocols Amanda Skillern used in your cycles at Aspire Fertility - Austin (Prelude Network clinic) and their degree of success.
I used the antagonist protocol, with a couple weeks of birth control pills before starting stims. Because of my large follicle count, and risk of OHSS, we planned a freeze-all cycle from the beginning. I started off with relatively conservative dosage amounts of Menopur and Follistim and did reduce as we moved forward. Also, since we did a freeze-all, I triggered with Luperon instead of HCG. I started with 24 eggs retrieved, 20 were mature, and 17 fertilized with ICSI. After six days, I had six remaining embryos for transfer. I was able to transfer my next cycle, after a hysteroscopy to double-check my uterus. Because of the grades of my embryos, we transferred one and it was successful.
Describe your experience with your nurse at Aspire Fertility - Austin (Prelude Network clinic).
I had one primary nurse who took care of my case and scheduling, she was very organized and good at explaining all the steps. She was also very thorough at our IVF class where we discussed my calendar and medications. Also, the rest of the staff including the other nurses and those who took blood and helped out with my retrieval and transfer were great. They make you feel welcome and I remember during my transfer having nearly all the nurses who worked with me while I was being treated in the room keeping my company and wishing me well.

Describe the costs associated with your care under Amanda Skillern at Aspire Fertility - Austin (Prelude Network clinic).
Unfortunately I did not have insurance coverage for IVF. The clinic offers a service that provided a "bundle price" for the procedure plus medications that was lower than the "list price." For all meds, plus IVF, plus a frozen transfer I paid roughly $16,000. The clinic does a good job billing what they can to insurance and they helped me out writing care summaries and letters to insurance to get me a couple claims covered that were originally denied.

He [Dr. David Prokai] was empathetic for our situation with our failed round (a different clinic). It seems like he LOVES what he does and definitely has the right bedside manner for such an emotional medical area. He quickly put a plan in place for our next round based on how I responded to my initial round. I am so glad I met Dr. Prokai and just wish I did earlier.
There's a lot of people on the care team [at Aspire Fertility Austin] so you feel like you're in good hands. The only weakness that I've run into so far is that they don't have an online payment system. It's not a big deal though since they will call to collect credit card details...[nurse] Jenna has been amazing so far! She is so sweet and detail oriented. I am looking forward to working with her this round.

Dr. Prokai reviewed my previous protocol (with a different clinic) and adjusted the plan according to my response. I will be on estorgen priming instead of birth control and then off to the races with a mixture of follistim, menopur, and omnitrope. I am so hopeful this time around.

She [Dr. Amanda Skillern] is very blunt. I do appreciate this but prefer more compassion than I got from her. She did not do both of my egg retrieval’s which I didn’t appreciate. She set realistic expectations on how many eggs I should expect and viable embryos after testing. I haven’t done the transfer yet so can’t review that process.... She prescribed - Lupron 10 ml while taking birth control pill then I moved on to Follistim 300 IU, menopur , omnitrope 50 ml. Then Pregnyl shot.
[Aspire Fertility/RMA of TX is a] Very nice clinic. All facilities onsite including where you do the egg retrieval. Clean and parking is covered. My husband said the male quarters could be enhanced. Very small room with little frills.... Full IVF $17,000 after slight discount Egg retrieval $11,000. Testing covered under insurance since we are CF carriers.... Since I did two retrieval’s I have worked with two different nurses who were both great!

I would listen to her [Dr. Natalie Crawford's] podcast As A Woman she discusses great topics and when you go in you know some of the lingo and what to expect...She went over every detail with me and made me feel more like a cousin than a patients. She asked me if I had any questions or concerns after every statement she would make...She has excellent bedside manner, for instance I had my hsg done and found out my left tube had hydrosalpinx. Dr. Crawford explained what that meant and gave me percentages on me getting pregnant naturally, went over the risk of not removing that tube and gave excellent analogies to help my husband and I understand. She is just amazing all around
I had to do genetic carrier screening [at Aspire Fertility] which was 8 tubes of blood, I waited for my results then I asked about them at my next follow up 2 weeks later and they couldn’t find them so i will have to redo the screening...Strengths would be it’s very open and huge, they have an acupuncture room and several consult, and sonogram rooms. Everyone makes you feel comfortable and you are checked in immediately and can have coffee while waiting

Dr. Crawford explained what that meant and gave me percentages on me getting pregnant naturally, went over the risk of not removing that tube and gave excellent analogies to help my husband and I understand. She is just amazing all around

Dr. Skillern was great, but at times I felt she was rushed or didn't take time to reassure me in the midst of concerns. She did everything right though, and I'm pregnant now. Compared to experiences I've heard at other clinics, she is a lot more on top of potential complications and preventing them. I didn't mind her blunt personality... My protocols seemed to be fairly standard. They do use birth control to start your cycle at the best time for them to manage office visits, but that actually helped my cycles to go by the book.
[RMA-Austin is] Small, stay on schedule, quick in and out, all services done in house, but they do keep everyone on a similar schedule so it can feel like an assembly line if you let it. I didn't mind it and appreciated that they started on time.... My protocols seemed to be fairly standard. They do use birth control to start your cycle at the best time for them to manage office visits, but that actually helped my cycles to go by the book.

She [Dr. Amanda Skillern] is pretty frank in the chances of conception. She does seem to go through a planned script but I imagine that is intentional to be sure all details are in order...Dr. Skillern is not chatty and no nonsense. There is no time for banter. Come prepared...The baseline examinations are very quick. If you have questions during a procedure, be sure to let them know you have questions and they need to be answered.
[The nurses at RMA of Texas] Very thorough in answering of questions and timely in getting back to me during stimulation cycle...the self pay price with genetic testing is about $20,000 + medication...They seem very organized and centrally located...16 eggs produced, 9 fertilized, 2 made to embryo stage, 1 was normal Lupron = 20 units for 10 days / 10 units during stimulation phase Follistim = 150 IU for 11 days Menopur = 150 IU for 11 days Omnitrope = 25 IU for 11 days I had a failed cycle before where the embryo was not normal. Dr. Crawford is the exact right combination of sweet and compassionate and direct and knowledgeable. We have been with her for a long time after a bad first experience with another doctor. She is very accepting of us as a same Sex couple and me as a person. She advocates for her patients and I trust her...We did both clomid pills and injectable and femara. It takes a lot of medicine to make my body ovulate so tried both pills and injectables.
The clinic [Aspire Fertility/RMA of Texas] is really well run, I genuinely have no complaints. Everyone is so nice and efficient. Make sure you speak up if you have concerns so the staff can make you feel better. And if it’s getting late in the day and you are waiting for a phone call about a pregnancy test just call them. I love this clinic. It’s clean, well run and nice people...We did both clomid pills and injectable and femara. It takes a lot of medicine to make my body ovulate so tried both pills and injectables.

Dr. Kelley is kind and compassionate, but will be truthful with you when needed. We felt listened to and respected while in her care. My wife went through 2 egg retrievals which resulted in no viable embryos , but we knew it was a long shot due to advanced maternal age. Dr. Kelley tried everything she could to help us achieve our goals.
Strengths are great communication and bedside manner. A weakened would be the patient portal - it doesn’t always load right and not super easy to see results. Other than that all the employees we came into contact with were friendly and helpful.

Dr. Prokai is a good person and I really think that he wants the best for his patients. We chose to use him as our doctor because we really felt good with his bedside manner...We decided to do a monitoring cycle with Letrozole to "gather information." That cycle really didn't tell us much. From there, we were told to continue on Letrozole, which we did for many months with no positive test
[The Aspire Fertility] app interface is awful and I regularly would send questions and would never get a response. This total lack of communication was extremely frustrating and we "lost" several cycles as a result, despite starting planning conversations a week+ out from my expected period date...The front desk staff was always so lovely and helpful. They clearly work hard to accommodate patients and often schedule over the weekends and holidays. The clinic also offers parking validation, which is helpful. The clinic supposedly offers support services like financial counselors and mental health support. However, despite paying for those services during the timed cycle, we never had access to them.
How was your experience with David Prokai at Aspire Fertility - Austin (Prelude Network clinic)?
Dr. Prokai is a good person and I really think that he wants the best for his patients. We chose to use him as our doctor because we really felt good with his bedside manner. When his staff was causing problems, he offered to communicate with us directly, which I appreciated. In retrospect, I wish he had been more proactive with setting a path for treatment. We weren't financially ready to spent $1,400/month on Aspire IUI treatments and we were vocal about that. I wish he had been more creative with ways to help us given our financial limitations at the time. For example, adding progesterone, trigger shot, etc.
What's one piece of advice would you give a prospective patient of David Prokai at Aspire Fertility - Austin (Prelude Network clinic)?
When communication breaks down, don't just accept it as is. You are your own strongest advocate and you HAVE to play hard ball at Aspire to get what you need.
During treatment, were you treated like a number or a human with David Prokai at Aspire Fertility - Austin (Prelude Network clinic)?
Dr. Prokai is thoughtful and really did try to help us, but his staff was a complete disaster. The left hand was never talking to the right. They also never helped us create a plan. Just kept telling us to take Letrozole over and over again with the same negative results. By the end of our time with them, we were actually the ones suggesting treatment ideas. Their app interface is awful and I regularly would send questions and would never get a response. This total lack of communication was extremely frustrating and we "lost" several cycles as a result, despite starting planning conversations a week+ out from my expected period date. Every time I allowed myself to believe that we had worked out the communication issues or that maybe we had a solid plan, Aspire disappointed me again, and again, and again. In addition to the horrible communication, they also sent me IUI contracts on multiple occasions without the procedure even being initially discussed. It felt like they were trying to sell us an IUI (at the outrageous price of $1,400) and if we weren't ready for that financial commitment, then we weren't worth the time. We desperately tried to find less expensive options to try before moving to IUI, but they were too busy to notice or care apparently.
Describe the protocols David Prokai used in your cycles at Aspire Fertility - Austin (Prelude Network clinic) and their degree of success.
After we completed the final diagnostic tests, we were told that I had some kind of ovulatory issue, but it wasn't super clear. I do ovulate on my own, but it is often delayed. We decided to do a monitoring cycle with Letrozole to "gather information." That cycle really didn't tell us much. From there, we were told to continue on Letrozole, which we did for many months with no positive test. Finally, we asked about incorporating a trigger shot or even progesterone just to try something different. We added progesterone, though my labs didn't suggest that I needed any additional, in this last month with no success. We were supposed to do a timed cycle this month again but their office never responded to my messages at the first of the month with concerns about timing of the cycle and upcoming travel. So, that was the final straw and I'm going back to my OBGYN for an IUI next month. WHICH BY THE WAY only cost $292!!! At Aspire, they charge over $400 for a monitored cycle and $1,400 for an IUI. Absolute highway robbery.
Describe your experience with your nurse at Aspire Fertility - Austin (Prelude Network clinic). (Assigned nurse: Cassy and Kathy)
Dr. Prokai's staff was a disaster in terms of communication. I would sometimes have to call and email 3+ times to get a response. Kathy was particularly short and rude to us, which made an already painful situation even more so. There was a specific occasion when I had finally had enough of the miscommunications about treatment, appointments, etc. My husband called and spoke directly to Dr. Prokai and they agreed to have a meeting for the three of us to discuss next steps and clear up confusion. I got a call about an hour later from Kathy who was insistent that we were not supposed to have an appointment at all and that my husband was clearly wrong. It was, in fact, Kathy who was wrong and we did schedule the conversation with Dr. Prokai directly. This is just one example of the many, many instances of miscommunication and poor coordination from his team.
Describe your experience with Aspire Fertility - Austin (Prelude Network clinic).
Aspire is a beautiful, clean, one-stop shop clinic. The front desk staff was always so lovely and helpful. They clearly work hard to accommodate patients and often schedule over the weekends and holidays. The clinic also offers parking validation, which is helpful. The clinic supposedly offers support services like financial counselors and mental health support. However, despite paying for those services during the timed cycle, we never had access to them.
Describe the costs associated with your care under David Prokai at Aspire Fertility - Austin (Prelude Network clinic).
A timed cycle cost $650+ medications An IUI cycle cost $1,400 I later found out that an IUI with my OBGYN cost $292. Needless to say, we are not doing IUI with Aspire.
What specific things went wrong at Aspire Fertility - Austin (Prelude Network clinic)?
  • Failed to call in prescriptions to pharmacy
  • Lost appointments
  • Provided conflicting information
  • Failed to convey critical information
Describe the specific things that went wrong at Aspire Fertility - Austin (Prelude Network clinic).
Communication was the #1 issue. Currently, their online portal is set up so that you can message the doctor and his/her care team all at the same time. They can respond to your message, but you cannot write back in the same chain. This month, I asked about delaying my period 2-3 days so that we would be able to schedule an IUI around a weekend that I was going to be out of town. I got one response saying that I could take a "short course" of birth control but then I would lose an entire month of opportunity. I wrote back asking if I could just continue with progesterone instead to bump it back a few extra days. Never got a response. So, my cycle came as anticipated and I'll now be out of town for ovulation-- thus losing another opportunity. Another cycle, I called three times and emailed several times trying to get a re-fill of Letrozole. Despite reaching out over a week before my cycle began, they didn't get back to me until CD5, so that cycle was a bust too.
