How was your experience with Halina Wiczyk at Baystate Reproductive Medicine?
Dr. Wiczyk is a tell-it-like-it-is kind of doctor, but in a process that is emotionally and physically draining, I found that refreshing. Thanks to her I went into the process with hope, but realistic expectations. From day 1 she recommended that we consider donor eggs but didn't push us in that direction. She answered all of our questions thoroughly and when she felt she had exhausted the options for treatment using my own eggs, she referred us to a top specialist. In the end, we decided to continue our treatment with her using donor eggs.
What's one piece of advice would you give a prospective patient of Halina Wiczyk at Baystate Reproductive Medicine?
Be prepared not to have things sugar coated. It's a good thing, trust me!

Dr. Wiczyk was upfront with me at all times, never gave me false hope and yet was sensitive to my concerns and fears.

My first protocol was a microdose lupron protocol, which is what they use for woman expected to be poor responders. This wasn't wildly successful, we got two eggs and one resulted in a good blast and a transfer, which didn't take. She decided to mix it up for round two and I did the patch protocol with the addition of HGH. This cycle was a disappointment resulted in two more eggs, but none survived for transfer. My third protocol, which was the most successful, was back to the microdose lupron with the addition of HGH and we got 7 eggs, three of which made it to blast. We transfered 2 fresh and 1 frozen. None of these resulted in pregnancy. I did not have any genetic testing done on any of the embryos.

I love all of the nurses at Baystate. Most of them have been doing this a long time and have great advice and reassure you when you need it. They are patient and answer all of your questions, even when you might think they seem silly. You can tell that they feel the emotions that go with this grueling process right along with you - they pray and hope for your success right along with you. I worked most closely with Ellen and Tina and they were fantastic!

I have been very happy with Baystate Reproductive. As with all offices, there can be delays, but they are mostly on time with appointments and I found it very easy to get in and out quickly when I was doing the daily monitoring of bloodwork and ultrasounds, which was important because I live 1.5 hours away from the facility.

It was very easy to get in and out. I usually showed up right when the opened so I was first on the list. There was one ultrasound technician who was more gentle than the other, but everyone was kind and efficient.

I am lucky that my IVF cycles were covered by insurance. The only out of pocket costs I incurred were co-payments, storage fees and HGH was not covered because it was considered experimental by the insurance company. This was about $1,000 per round. I am currently in the middle of a donor cycle, which is not covered by insurance because of my age, but the meds are. I have paid $15,000 for a frozen egg cohort from Fairfax Egg Bank, which is their partner organization and I anticipate about $10-12K in expenses for the procedures.

We had so few embryos available to us that we didn't have much discussion of this, but we did decide to transfer two fresh embryos, which was recommended for someone my age.

My treatment started with a preoperative visit on March 16, 2021 at 9:40 in the morning. Then on March 30, 2021 at 7:30 I received a Tubal Reanastomosis mini laparotomy. The surgery took about 3 hours. I also had an HSG on December 20 2021, which reveal that the surgery was successful on the left side the right tube was not intact. I began Clomid in February 2022, two pills a day for 5 days. In March 2022, I tried Clomid again this time three pills a day for five days. Both times I was instructed to test for ovulation and have sex every other day and report my day one if I got my period and repeat process if I had a day one.

My surgery was the most costly part of my procedures. The cost was $10,587.00. Now for my Clomid I pay $38 per round.
She [Dr. Cynthia Sites] was understanding and gave me a few moments to compose myself after I had been told IVF was the best option (I had gone in thinking IUI). My only complaint is that I felt like she wasn’t the best communicator, specifically towards the end of appointments. It seemed like my husband and I were always left hanging a bit only being given a “call the nurses this day.” But we never really knew what was coming next. It felt a bit awkward because it seemed like that was her way of saying goodbye. Other than that, my experience with her was fine. I felt very lucky that she was the one to do my egg retrieval and embryo transfer, although it really seems to be up to chance there.

Ex: Call the nurses this day, tell them it’s this cycle day and they will set up your calendar and send you an information packet. (Because I had no clue when I’d be getting medication information and instructions and I like to be prepared.)

My husband and I met with Dr Sites 3 times between late January and early February. After the first appointment she greeted us both by name and remembered small details about us. Due to Covid, our IVF cycle was postponed from March until late June. Although I had not seen her since February, she remembered me and chatted with me on my way into the office one June morning.
Describe the protocols Cynthia Sites used in your cycles at Baystate Reproductive Medicine and their degree of success.
I was put on the microdose lupron flare protocol, as I was expected to be a poor responder due to DOR. I started with 10 units of microdose lupron twice per day for two days. Then added 225 units of gonal-f twice per day while continuing the lupron. I was on gonal-f for 10 days (lupron for 12) before triggering with ovidrel. 11 eggs were retrieved, 6 fertilized, 1 perfect embryo was transferred on day 5 and 1 embryo was frozen on day 6. I was put on vaginal progesterone 3x daily and estrogen patches from the day after retrieval to 6w3d (first scan-saw the heartbeat).

Well I’m not sure if we were assigned a specific nurse, I did speak primarily with one in the beginning but upon returning to the clinic after the Covid shut down I spoke to a few. I’m not sure what is normal for the clinic. That being said, every nurse I spoke to was wonderful! Each one was super helpful. I never felt like I was calling too much or asking too many questions. They seemed to communicate well with each other and I never had to re-explain something to the next person I spoke with. I normally called in the morning and they would get back to be my early afternoon. A couple of times I called in the afternoon and was actually transferred straight to a nurse which was a pleasant surprise. There was always a nurse in with me for the ultrasound explaining everything and letting me know that it was normal or on target for where I was in the cycle. They were great!

My insurance only covered single embryo transfers, so that’s what we went for. That being said, Dr. Sites said she also recommended a single transfer for us given my age (27) and history (no previous pregnancies, no losses).

At our first ultrasound after a positive pregnancy test we were told that the embryo was not viable (heartbeat was not good). It can be a confusing time because your hopes are suddenly dashed and while you're still in the office you're coming to terms that this means you're going to miscarry. I was given this devastating news prior to a planned vacation and I was told by the nurses that no one could predict when I was going to miscarry. Dr. Sites helped me by prescribing Misoprostol which gave me some kind of control over the very negative experience of miscarrying. When I was originally given the bad news, another doctor at the clinic (Dr. Wiczyk) stated that she could not give me the Misoprostol because they were a fertility clinic but that I could go to Planned Parenthood; I considered this to be very unhelpful given that they had just told me the pregnancy was not viable. It seemed like torture to have to wait for the inevitable or go to a different facility to get the medication that was necessary to end the pregnancy. I was able to talk to my doctor (Dr. Sites) at the same visit who understood my decision and prescribed the Misoprostol and told me what to expect with the drug.

Dr. Sites told us at our consult that she expected IVF with ICSI to work the first time and if not the first, then the second. We had a successful singleton pregnancy after the third frozen transfer. Our diagnosis was male factor infertility; I was prescribed the luteal lupron protocol for the retrieval. After the retrieval, I was scheduled for a fresh transfer but I experienced moderate OHSS symptoms and was encouraged by the nurses to do a frozen transfer instead after they saw me in the office. The first frozen transfer resulted in a miscarriage at seven weeks. The embryo was tested after the miscarriage and it was genetically normal. After I completed recovery from the miscarriage and before the second frozen transfer, my uterus was biopsied and I was told I had an infection; it was treated with oral antibiotics. I asked why my uterus was not biopsied prior to the transfer and Dr. Sites said that is not done when they're expecting everything to go right. For the second frozen transfer, I switched to intramuscular shots of progesterone rather than the vaginal suppositories. Dr. Sites explained that the intramuscular shots are considered by some REs to be the "gold standard" but that she considered the suppositories to have similar results to the shots. After the second frozen transfer resulted in a chemical pregnancy, Dr. Sites recommended implanting two embryos; I am told this is standard operating procedure after 2 failed transfers. After this second failure, we reached out to another RE (Dr. Howe) for a second opinion. Dr. Howe encouraged us to stay at our clinic because we had frozen embryos already and each lab has their own procedures for how they're frozen/unfrozen and it's best to stick with that lab. My husband and I decided to another transfer at Baystate and only transfer 1 embryo; that third time was successful.

The nursing staff is very helpful, warm and knowledgeable. At times, I was frustrated that lab orders were not in the system when I went for bloodwork; it wasn't hugely inconvenient because the lab where I went for bloodwork was in the same building but it involved going up 4 flights of stairs and when you're trying to get all this done before you head to work, it was annoying. This happened to me 3 or 4 times. When I finally complained, they were very apologetic and understood my frustration and after that, I did not have any problems. When I did finally become pregnant, one nurse in particular, Ellen really helped calm my anxieties by explaining very well all the aches and pains I was feeling. During my miscarriage, I was treated with a ton of compassion from Alisa and it was really nice that she could tell me what I could expect in terms of pain and how long it would last. Tracy was another nurse that was very kind and helpful.

Overall, the care I received at this clinic was excellent. Sometimes, I felt there was a disconnect between the lab and the doctors but I can see why that happens since they're in different locations. Because of the round the clock nature of infertility care, you don't often see "your" doctor however, I was confident that everyone was united in the ultimate goal of a successful pregnancy.

This didn’t result in a pregnancy...is very concerned about her patients and also thoughtful. One time I felt rushed through an appointment, but at times she also called me on the weekend...she thoroughly explains what to expect...You can explain Dr Lynch to treat you with respect and care about your treatment.
Most of them [nurses at Baystate Reproductive Medicine] were really great, sharing their experiences with IVF, and being compassionate...I had to wait an extra cycle because no one told us that because I was over 40, I had to do a Clomid challenge test. I feel like they should have told me this before I turned 40, not the month afterward...I didn’t like that I had to go for bloodwork every time I had an ultrasound. It seemed unnecessary as sometimes a decision was made prior to them receiving the lab result. The clinic setting is very nice, the ultrasound techs were great. One of the phlebotomists bruised me twice and I had to specifically request someone else.

For several cycles I used Clomid then switched to Letrazole while using ovulation testing. We did this because my husband and I weren’t ready for anything else yet. Next we did 3 IUIs with Letrozole. The last one resulted in a chemical pregnancy. We then moved onto IVF with ICSI. The first month I used an ovulation prediction kit. 10 days after the surge, I applied a Vivelle patch (3 patches and changed them every other day). 11 days after the surge, I started Ganirelix (for 3 days). For month 2, I came in on day 2 of my cycle for baseline blood test and US. I started Menopur (4 vials). That day we both started antibiotics also (for 7 days). Gonal f was started that night. On day 6, another US, then started Ganirelix. I had to go in one week on Monday, Wednesday, Friday, Saturday, and Sunday as they monitored the growth of my follicles (which were a bit slow to grow), along with bloodwork each time. Then Ovidrel and egg retrieval 2 days later. We had planned to transfer 2 embryos, due to my age, but were down to only one healthy one, so it was transferred on day 5. This didn’t result in a pregnancy.

Each IUI was ~$600 plus medication costs. Our IVF wasn’t terribly expensive because I had already reached my annual deductible. $60 copay, ~$600 for meds. Now that a new year has started, it’ll be $1,200 for deductible, plus meds, plus copays.

When I go into my fertility clinic Dr Lynch and the nurses take the time out to sit and talk with me when I come in they know me by name and make me feel comfortable and not just another patient.

The treatments, medications and protocols we decided to use was based on my pcos diagnosis. My main problem I had was ovulating on my own so my first round of medications we started with was 5 mg of letrozole and timed intercourse. With no success of getting pregnant after a few cycles of that we upped my dose and started 7.5 mg of letrozole, hcg trigger shot and our first round of IUI. I had gotten pregnant after our first round of IUI but later miscarried. Although I was in fact ovulating with the medications getting pregnant after the first time seemed to be a lot harder. I did a few more rounds of the 7.5 mg letrozole, hcg trigger shot and IUI and added progesterone suppository with no success. I did take breaks in between medicated cycles and did ovulate on my own and did timed intercourse but no pregnancies.

I love the nursing staff. They really take the time to know what's going on with you and what's going to best for you to start your family. They know me by name without looking at my chart and they make me feel comfortable even in the most stressful situations. They show such compassion when medications don't work or losing a pregnancy that you wanted for a really long time. They call to check on you after dealing with difficult news but also get back to you right away when you just have questions. They actually listen to you and even share their own experiences. Although I'm there to get pregnant every nurse I've dealt with makes conversations easy and puts you at ease.

Being a big facility they do get very busy but they always make sure your not too inconvenienced by it and try to accommodate you as much as possible. The front desk ladies know me by name which is very nice because that makes you feel like your not just another patient to them. Getting a hold of the nurses is easy but there are days where you have to call back or hold for a moment because of how busy they get but still make you number one when talking with them and they get what ever messages you to need to get to the nurses in a timely fashion. The clinic operates very smoothly and if there a mishaps they don't show it because they take care of things very professionally.

Since I'm self pay with everything it is very expensive. Ultrasounds are $300, IUI $500 and just visits were $150 if I can remember correctly. Hcg shot was $118 but most of medications(letrozole, progesterone suppositories etc.) were covered or only a few dollars. We start IVF soon and haven't gone over those cost yet but last time I talked with one of the nurses they said it had actually went up the medications range from $600-$700 and the retrieval and other procedures for IVF are starting at $9,000

Down regulation, control cycle with bc pills for 3 weeks
Started 10IU of Lupron on day 18
After day 3 US, reduced Lupron to 5IU and mixed in 300IU Follistim and 175IU Menopur (this was not for a set period of time; time on the meds differs by response). I took these for about 10 days
9 follicles, 6 were eggs. All 6 fertilized. 4 were ready on day 5 and 2 were ready on day 6. Transfered 1 fresh on day 5. It stuck but only in week 5 so far.

They fixed the order in their system but I have yet to be charged for anything (not even copays) so we'll see how that works out.

She [Dr. Lynch] and the nurses were amazing and truly cared about me as a person and our want to be parents. Explained everything before anything was done and the reasoning behind it. Sent my insurance a letter explaining why IUI wouldn’t work with my condition and they approved us for IVF without hassle...She explained everything so we completely understood what we were doing and why. I felt like the sonographer, Amy, knew me my entire life. I mean, when she’s svanning you every other day, she knew me better than my husband! She saw me at my worst and going back after our son was born to introduce them was amazing. She is vital to the practice. The nurses were amazing as well, I just happened to see Amy 90% of the time. I especially loved the fact that a majority of the people working there also have their own IVF miracles. They truly know what you’re going through. Tracy was the nurse I had the day they told us IVF was the only way we’d be parents. I was convinced I wasn’t strong enough to go through that. She sat down, eye level, and talked to me. So, I put my big girl panties and scheduled our IVF class And that started it all. I hope they know what they are doing for people. They are literally making dreams come true for so many people.

We first met with a resident, which I had not been expecting. The resident was kind and asked us additional questions about some of our health history before all of us met with Dr. Sites. Dr. Sites was warm, friendly and matter of fact. She presented a course of treatment, explained the process and the steps that would need to happen prior to starting the process. She was patient, and did not seem to be in any rush. She did not present any aspect of her course of treatment in terms of options, in that we could do any aspect one way, or another way--she only presented a standard plan. When we asked about another option, she was slightly dismissive and stated that it would likely not be necessary in our case. She was thorough in her explanation of why we would not need to pursue this (we had asked about PGS). This was the only time she was slightly curt with us. She laid out next steps clearly and offered us resources to pursue as we waited to be able to take those next steps. She also laid out a standard pricing structure for a cycle, not including medications.

We are very early on in the process, but I did have the sense that you might need to advocate for yourselves. I am not sure how willing she would be to deviate from standard protocols. Again, in our case there does not seem to be a reason, but when I asked about fresh vs frozen transfer and PGS/PGD (the clinic almost always does fresh, and rarely does screening without strong evidence to support the need), she did not give the impression that she was open to either.

She [Dr. Sites] explains everything clearly, answers any and all questions, and has a very kind and calm demeanor which is appreciated during what can be a stressful process. I’ve never felt rushed during any of my appointments with her, and she shows real concern for the comfort of myself and I’m sure other patients.... I am using clomid and a trigger to provide some control over timing as my cycles aren’t totally regular. I am using clomid and a trigger to provide some control over timing as my cycles aren’t totally regular. Dr. Lynch was very compassionate. She went over everything from diet to vitamins and medication. She gave us the time we deserved and so desperately wanted. Both my husband and I went to every appointment together and she spoke to both of us equally. His questions and comments were as important as my own. ... She gave us the option of how aggressive we wanted to be. We started off slow and let me dictate what we wanted to move on to next when something didn't work.
Weakness would be that they [Baystate Repro Med] are really busy but they are busy for a good reason. They are good. I loved the lab technicians that handled the specimen. They always had a great sense of humor. From the lab techs to the nurse that did the IUI's. They would always make you feel comfortable. ... I got a call from UCONN a few days prior to my appointment with them saying that Baystate had not sent my file to them.

Even though the was always running late to appointments it was because she gives each patient the time they need to ask all the questions they needed/wanted answered.

My insurance dictated the order in which we needed to do things but we had some wiggle room. She gave us the option of how aggressive we wanted to be. We started off slow and let me dictate what we wanted to move on to next when something didn't work. We did timed intercourse the first month. Then moved up to medicated timed intercourse. Then moved up to medicated/monitored timed intercourse the next month finally moving up to Medicated IUI the third month. Before going into treatment I didn't have any strong opinions on how aggressive I wanted to be. Once we actually started appointments I didn't want to wait any longer. When something didn't work we moved on to the next step(that was allowed by insurance) Dr. Lynch was very accommodating to our approach.

I loved the nursing staff. They were very companionate. If you had any questions they answered them. My only complaint with them is that they are a very busy office and would tell you of the next step in your procedure and assume that you knew what to do. For my first injectable they just told me to do it.. Luckily I was able to find a video from the pharmacy how/where to administer the trigger shot. From that point on I knew I had to ask questions immediately.

When we switched to a clinic closer to home when we decided that we were going to move up to IVF I requested that my paperwork be sent down to UCONN well in advance about 2 months. I got a call from UCONN a few days prior to my appointment with them saying that Baystate had not sent my file to them. So I had to leave work to go up to Springfield to get my medical file the day before my appointment with the new dr. Who had required to have the paperwork prior to the appointment to have a full appointment.
I rarely saw Dr sites. Besides my first consult with her. ... I had to suggest/ask to use progesterone bc I felt my 11 day Luteal phase was too short. ... After that we met with Dr. Sites and she gave us the Diagnosis of LOR. she suggested a test to check my tubes (one of which turned out to be blocked). She then said our first course of action would be IUI (due to our insurance requirements). She explained everything well and we were satisfied. But that was i believe the only time we saw her. After that it was only nurses and US techs.
Although the rest of her [Dr, Sutes] nursing staff was very nice and helpful, the clinic [Baystate Repro Med] is large and you situation is not well known by most people you see ... Clomid for the first round. trigger shot and IUI after 36 hours. This one failed.
Switched to Letrozol, did not trigger since this was on our blocked side.
Letrozol again, triggered and IUI after 36 hours. Failed.
After this we requested her office send a letter asking for us to start IVF.

The nurses were very nice and helpful. After my first IUI i asked the nurse about progesterone for my short lateral phase. I said my periods/ spotting started very early. She said just ask me for it and I will RX it for you. not a problem.

It's one of the only clinics in the area. They do a good job and are well organized. But it is a hospital clinic so you are mostly a number. Nurses are great.

When she could not be part of a procedure, she assured us that we were in good hands. She was on hand to celebrate our success as well, which was amazing...She is more than willing to answer anything that you may ask, and quell any fears you have. The more educated you are, the less scary this process seems.
All of the doctors there [Baystate Reproductive Medicine] work really well together. When Dr. Lynch wasn’t available, the other doctors stepped in. They definitely work well as a team. It was nice to know someone was always available when needed...We did a 3 day transfer, as all 4 of my embryos were unsure to make it to day 5. We implanted one high quality embryo, but nothing else made it to freeze...They [nurses] always made me feel so supported. My insurance required 3 IUI’s before IVF. Those were all unsuccessful for me. Because of my PCOS, she used a low dosage of fertility meds for our first round of IVF. That only produced 8 eggs, but 2 of my embryos were very high quality. We did a 3 day transfer, as all 4 of my embryos were unsure to make it to day 5. We implanted one high quality embryo, but nothing else made it to freeze.
