We tried three medicated cycles with Femara. They were unsuccessful. We then began IUI with Femara, Gonal-F, and trigger shot. My body did not cooperate always, as it would spontaneosly ovulate. Dr. Grainger listened to my concerns and we paused treatment to run some tests. In the mean while, we found out that I was pregnant.
We are in a new cycle of treatment as we're trying to grow our family. We skipped straight to IUI and Dr. Grainger recommended being very aggressive (heavy medication) as I have diminished ovarian reserve.

Dr Grainger suggested skipping IUI based on our male factor diagnosis.
For retrieval, my husbands sperm was hit and miss so we froze a good sample prior to retrieval as backup. For stims, we used menopur and gonal. We started out on a conservative amount and then increased the Gonal since I was sort of a slow responder. We ended up getting more eggs than we had anticipated. ISCI was used and we had 9 out of 11 eggs fertilize.
For FET, it’s a priming of ESTRACE and then PIO started five days prior to FET. I did ask about other options like suppositories but Dr Grainger advised me that PIO is the best form due to several reasons. I also asked about testing like ERA/receptiva, Alice/Emma, etc. For my situation, there isn’t much data that supports those tests and an increase in implementation so we have not done them.
Due to a few other factors, we did do a hysteroscopy and laparoscopy. They ended up being very helpful and cleared up some issues (thickened tissue, endometria but not on ovaries).

My nurse has been great. She has the personality that I need but it might not fit everyone’s style. She’s direct and to the point. She’s open to questions and responds appropriately or refers to doctor if need be. The other nursing staff wasn’t always helpful. If my primary nurse wasn’t available I tried to just wait til she would be.

The Urologist told us it was same as previous tests (low count motility mobility) but then when sent to clinic, the nurse said my husband didn’t have any sperm which was a shock to us. We redid sample and confirmed we had sperm to work with.
Dr. Grainger is a warm, kind doctor and each time we met with him, he never rushed us through an appointment and answered all of our questions...He completed all the procedures except the IUIs. For the IUI cycles, I was prescribed letrizole. I don’t recall the medication for the trigger shot. Each time I had between 1 and 2 mature follicles. I have a diagnosis of endometriosis, and husband has morphology issues. Ivf egg retrieval included 225-300 IU Gonal F, and 75-150 IU Menopur. Cetrotide was added later in cycle.
Trigger shot, then retrieved 28 eggs.
7 made it to day 5, then we’re frozen. Went on birth control for a month and then on to estradiol, once closer to transfer, started letrozole injections. After transfer, continued both protocols. I just completed transfer, so not sure if the transfer was successful.

It was a different nurse every time. The first round of treatment the nurse told me how to take all the medications. It was a lot as this was my first time doing anything like this. She assured me that the directions would be on the prescriptions they were not. I called back after picking them up and talked to her about the directions on how to take the meds. I wrote down what she said. Well my cycle was severely delayed with a negative PT(and it never is). When I called to ask what was going on the nurse that time asked how I was taking meds and told me I was taking them all wrong. The next round of treatment the nurse told me
to take 3 doses of Fermara a day. I asked her why my treatment had changed and asked her to double check. She came back and told me that it was the same and I should take 2 a day. When I came in for my mid-cycle sono the sono tech could not find my ovaries (that has never happened) I asked her why she could not give me an answer. Told her about my experiences of getting wrong directions and two different ones last cycle and she told me my treatment plan has changed and the Dr was supposed to call me to tell that. No one had ever called and I was supposed to have taken 3 doses of Fermara.

Proceeded with retrieval that resulted 7 eggs, but only one embryo made it to transfer. First cycle ended with an initial negative pregnancy test. Second cycle was started with Lupron micro dose protocol followed by higher initial doses of injections. Produced even less follicles. Went to retrieval with 5 follicles, 4 eggs retrieved. Due to COVID-19, we were far enough along in treatment to continue to retrieval, but all embryos would be frozen. Currently have 2 frozen day 5 embryos awaiting transfer.

We were able to get almost twice as many eggs with this change in protocol.

The nursing staff is very friendly. A few of them actually remember me each time I come in. The ivf nurse, Megan, is great at answering all my questions. During my ivf cycle, embryo transfers and my pregnancy I had lots of questions and she always did her best to answer them for me.

I really like this clinic. The staff is friendly, the location is easy to get to. I really have no complaints. One thing that is good to note that could be seem as a con is that they are now partnered with KU Med in Kansas City, the clinic is in Wichita. Ultrasounds and appointments with the doctors are in Wichita but procedures like egg retrievals and embryo transfers are done in Kansas City by the staff there, not by Dr. Grainger.

The first month before doing IUI, I had a few ultrasounds to see how many follicles I had. He put me on Femara (1 pill 2 times per day) for 5 days. The result was 1 healthy follicle. We did an ovulation trigger shot (Ovidrel) and then went into the office 36 hours later. The IUI took place then.
The second month, I was in 3 pills of Femara 2 times per day and produced 3 healthy follicles.
My spouse wasn’t put on any hormones to help with sperm production and his numbers decreased making our chances extremely low.

The clinic was up front with pricing. The IUI treatments were $650 and the medication was done through an outside pharmacy. It was about $300 in medications and trigger shots.
If we had competed IVF with Dr. Grainger, it would have started at $11,000. We would have added genetic testing for about $1,500. The cost for egg transplant AFTER the first one would be $2,500.

He started me on a few rounds of just Clomid and then femira. Then added ovidrel. Those didn't work. I had a dye test done before i started doing iui. Did that 3 times and it was a bust. I had exploratory surgery to make sure everything looked ok and it did. I moved in to injectable meds after that for a few rounds which also resulted in neg tests. I have only 1 tube. In the end he said ivf was the only way i would get pregnant, but we can't afford that.

I had a nurse ask me if I had an internal ultrasound during the first couple days of my period as this is standard every other cycle - it was my 3rd cycle and I hadn’t had one any cycle. Also, had another nurse asked me if my femara had been increased to 3 pills instead of two this round as this would be standard after 2 failed cycles but again this was not done.
[Dr. Tjaden] was kind but did not sugar coat anything, which I appreciated. He got straight to the point and we began treatment/diagnosis right away. He explained everything and answered any questions we have... He is blunt and gets to the point...With each cycle I’ve done blood work and vaginal ultrasounds to check for follicle growth...He took the time to explain everything with me and my husband. While determining which medication to start on he said “if you were my daughter, I’d prescribe this.”

Started with a vaginal ultrasound to look at my uterus and ovaries. This is where he diagnose me with PCOS. He started me on Metformin and told to continue with ovulation prediction tests. He offered to perform an HSG procedure which we chose to wait and he was okay with it. After months of negative tests we decided to schedule the HSG procedure and a sperm Antalya is test which was both normal, good news! At the 2nd office visit we started on Letrozole 2.5mg and Ovidrel trigger shot. First round was negative, was increased to Letrozole 7.5mg and trigger shot. Second round also negative but since I produced 2 larger follicles this 3rd round we continued with Letrozole 7.5mg. Now on the 2 week wait before testing. Hoping this is works otherwise was told we’d try an extended cycle of Letrozole 10 days vs the 5 days I’m currently on. With each cycle I’ve done blood work and vaginal ultrasounds to check for follicle growth.

1 office visit was $350 which is what was left of my insurance deductible. 2nd office visit was $200. This clinic is not under my insurance 1tier. Ovidrel shots have been $84 thanks to the compassionate program. Look into this for help with medication costs. Letrozole is $10. The vaginal ultra sounds for follicle check have been $116 after insurance. HSG was $350, Sperm analysis done at lab located in same building was $125, insurance didn’t cover this unfortunately. With lab, procedure and medications, I’d say I’ve payed more or less than $1,500 so far

He is warm and genuine, but he can find humor through the hard. He is matter of fact and likes to keep things as simple and non-invasive as possible. The two nurses that I primarily see are Chelsea and Megan. However, all of them are great. Chelsea specifically has been an ongoing support for me and has become a friend. At the time of my miscarriage, she came into my room and just hugged me while I bawled. It was extremely comforting and I really felt her heart hurting for me (us). Megan is very informative and detailed, which I love and appreciate. She is organized and good about reassuring me. Again, I cried at the talk of potentially cancelling our IVF cycle due to poor response and she was good about letting me know that however it turned out, we'd make the right decision. As with Dr. Grainger, I love his staff just as much. They see more me than my own family at times.

We were male factor so he told me he didn’t want me to get tons of eggs because I’m a healthy 24 year old who is dealing with a husband with no sperm. He had me on Gonal-F, Menopur, Cetrotide, HCG trigger and progesterone post egg retrieval. At my retrieval, I had 13 eggs retrieved, 11 mature, 11 fertilized and 6 day 5 healthy embryos. One 5AA embryo was transferred and I am currently 8 weeks pregnant. The other 5 embryos have been frozen.

He told me ‘just because we transfer 2 it doesn’t mean you’ll be pregnant with 2 and it honestly doesn’t increase your chances of pregnancy.’ So we did one and are so happy we did!

When I experienced a chemical pregnancy in May 2018, Dr Grainger was so kind, gentle and reassuring that this didn’t mean we would never get pregnant again. He was, in fact, very positive, all things considered, because I was able to get pregnant. My experience with him, his nurses and the office has been great.

The strengths of this nursing staff are their communication skills. They are all extremely thorough, will answer all your questions. The nurses have always called me back less than 24 hours of when I’ve called. Typically, less than 5 hours of when I’ve called.
One weakness, the nursing staff is not always running on time. I’ve had to wait up to 30 min for an appt multiple times.

In the beginning he simply put me on a dose of Femara and planned on doing no follow up. No sonograms, no labs, nothing. I had already been started on Femara (CD 3-5) and dexamethasone (Start CD 3 for 10 days) from a previous Dr when we went in to see Dr. Grainger. I was responding well to what had been prescribed so we went forward with the IUI. We did do 250mcg Ovidrel to trigger ovulation and followed with an IUI 36hrs later. I had three large follicles and thick lining when I went in for the pre-IUI ultrasound. This IUI did not result in pregnancy. We are getting ready to do our first IVF cycle and so far he started me on Sprintec birth control to prevent ovulation prior to starting stimulation.

Every time we’ve had an appointment we have nevertheless had to wait and there was only one time there was anyone else in the waiting room. This was really nice for us as we are fairly private people. This clinic has designated collection rooms which was a first for us and made it easier for my husband to give his sample. Their lab facilities are fairly quick. We’ve never had to wait more than 15 mins to have blood drawn or be taken to a collection room. Everyone in this clinic is super helpful and knowledgeable. They are also open every day of the year except for Christmas Day and New Year’s Day which takes a lot of stress out of the unpredictability of ovulation and makes IUI timing easier.

We have met our insurance deductible for the year so our expenses have been minimal. However, they did charge $300 for an office visit and over $200 for a vaginal ultrasound. The cost of our IUI has not been submitted to insurance yet. The lab costs seem similar to other labs that I have had bloodwork done at.

What I really liked about Dr Grainger was his compassion and his curiosity and willingness to help me understand the cause of my infertility so we could then try to treat it. Before meeting with him, I was thinking that I would like to have laparoscopic surgery to check for endometriosis. During our conversation, he concluded that that was his recommendation without me bringing it up. He also did my initial ultrasound himself, and tried to locate one of my ovaries which had eluded many ultrasound techs. He was pretty sure he found it, but a month later I had surgery and there was no 2nd ovary there. Dr Grainger was also very curious whether I have a genetic condition that causes my fertility problems. He recommended genetic karyotype testing, which I was willing to do, I was just waiting for the clinic to tell me what to do. A month later I had laparoscopic surgery, then took a few weeks to recover, and then was ready to start a medicated timed cycle. When I called the nurses, I was told that I could not be treated until I had the genetic test, because pregnancy could be life-threatening if I did have the genetic condition. Needless to say, I was very angry that I had to wait another month to start any treatment while I waited for results when the test could have been done 2 months ago. I don't know if this was a communication problem between the Dr and the nurses, or if someone in the office dropped the ball somewhere. I also don't feel like Dr Grainger explained that if I had that condition it could be life threatening to get pregnant - I think I would have remembered that and wanted to know right away. Luckily, I do not have the condition. After my surgery, I had one short follow-up appointment with the Dr to discuss the surgery results and the plan going forward - 2 rounds of medicated (Letrizole) timed intercourse, and if that didn't work, he recommended going on to IVF (which their clinic doesn't do, I would have to go to KU Med in Kansas City). After the follow up appointment, I only had contact with nurses and the office staff. We did two months of medicated timed intercourse and then decided to move on.

They close early on Fridays, so make sure to not have your "day 1" on a Friday afternoon. I know you can't control that - it just seems like everything happened to me on Fridays and it was always so stressful trying to rush to get a bloodtest and get the results send to their office by 11am. I live 2 hours away from their office, so I couldn't just go there for tests and ultrasounds.

The first timed medicated cycle was with 2.5mg Femara. It should have been taken on days 3-7, but because of timing hiccups, I didn't get them until day 4, so I took 2 pills/day on days 4-7. Their protocol would have me do a day 12 ultrasound, and then give myself an Ovidrel trigger shot. Well I was out of town on day 12 and they would not authorize me to have an ultrasound out of town on a weekend, so I don't know how the medication worked on me, and I did not give myself the trigger shot. That cycle was a negative.
The second timed medicated cycle was with 2 pills/day of 2.5 mg Femara on days 3-7. I had an ultrasound on Day 12 in their office. I usually don't ovulate until day 16, so I did not bring the trigger shot with me. I had one 22mm follicle and two smaller follicles. I should have triggered that night, but we had hotel reservations that night (being that my clinic is 2 hours away from home). The nurse said don't worry, you're going to ovulate anyway, so just have intercourse. That cycle was a negative.
Dr Grainger never followed up with me for either of the cycles. I didn't know if this was normal, or what. But since we didn't do an ultrasound for the first cycle, I feel like we didn't even know if/how my body responded to Femara, so we wouldn't have known how to adjust the protocol.

In person, I felt like the nurses I met were very sweet and compassionate. However, I did feel like I met a lot of people in the office in the beginning and I was confused as to who was a nurse, who was my nurse, who was the insurance person, and who was the financial person. Once I started doing the medicated timed intercourse cycles, I think I was working with the IVF nurse at first. They seemed to know their stuff, but maybe weren't very compassionate or patient on the phone when I was confused about how/where to get a blood HCG test, and how I could get ultrasounds for monitoring when I wasn't coming 2 hrs each way for them. Maybe we should have discussed all this in the office when I was there in person? I guess I just didn't know what questions to ask and logistics to work out - and I would have liked the nursing staff to have brought that up while I was in office.

He monitored my IVF cycle closely and make the decision that a full freeze and frozen transfer would be the safest and result in a better outcome, which it did.

He also found a small area of Endometriosis that I didn't know about. His bed side manner is exactly what I need which is direct but with compassion. I have recommended him to many of my friends. There are not a lot of African American women who dicuss the stigma of infertility and he has made me comfortable with my journey.

We first did 2.5 mg Letrozole, but no IUI due to office holiday closures. Cycle 2: Letrozole 2.5 mg with IUI OF 32 million washed sperm. Cycle 3: increased dosage to 5 mg Letrozole. IUI cancelled, due to low sperm count. Referred back to urologist and can't continue treatment until cleared by urologist.

In our initial referral to the clinic for an SA (from my GYN), we were never informed of the initial results (neither us directly or my GYN's office). After a few weeks of no news, I made some phone calls and eventually got the results sent to my GYN, who relayed them to me.
Dr. Tatpati has many strengths. First she is very knowledgeable and takes the time to explain treatments. She also shows empathy for what people with fertility issues go through. She also consults with staff and other dr.s in the practice. The only weakness I can think of is that when we first were consulted at the clinic she seemed very clinical. But, in hindsight that makes sense because she did not have much data on my husband and I and she did not know us well.
The strengths are the dr.s and staff [at Center for Reproductive Medicine in KS]. As noted before they listen to patients, are always willing to help, and are compassionate. We pay out of pocket and they've always tried to work with us to make our dream of coming parents true. The only weakness I can think of is their office hours. I wish they were open earlier and later so appointments could work with work schedules.

During treatment, were you treated like a number or a human with Laura Tatpati at Center for Reproductive Medicine in Kansas?
At first I felt more like a number, but after a year of treatments I got use to the clinic and how it operates. I found they didn't mean to be cold or clinical. Also, Dr. Tatpati took the time to meet with me and explain how she really wanted to figure out my particular case and expressed empathy.
Describe the protocols Laura Tatpati used in your cycles at Center for Reproductive Medicine in Kansas and their degree of success.
She knew I had PCOS from blood tests and the ultrasound she performed. Also, she ordered a hsg and my tubes were clear. All of my husband's tests came back normal. She notified us that this was good news and I just needed to ovulate. We started with femara, then higher doses of femara, then clomid. With pills alone I was not ovulating, so she added injections and suggested iui if we wanted. We wanted our best chance so we opted for iui with injections. After a few cycles I stopped responding to meds and even overstimulated. So, my cycles were all over the place. She ordered more lab work because she had some suspicions about my thyroid and adrenal gland. Her suspicions were correct, my thyroid was too low and I had adrenal fatigue. She added a steroid to my protocol and a higher dose of levothyroxine and I started producing follicles for iui. I became pregnant on iui #2 but it was ectopic. We had to take a break as she suggested I take methotrexate rather than having a laparoscopy (that could further damage my tube). Months later we tried a few more times and on our last attempt, iui #6, I became pregnant again but miscarried early in the pregnant. She suspected something may be going on with my Fallopian tubes and/or uterus so she suggested a hysteroscopy and laparoscopy before we move on to more treatments. She let us decide, though she suggested we do to rule out other fertility issues. I opted for the procedures for piece of mind. The results were a small uterine polyp, a small amount of endometriosis, a damaged left tube from the ectopic pregnancy, and a thin right tube that may have caused my last miscarriage. She removed the polyp and the mild endometriosis. After the surgery she let us decide if we wanted to continue with more iuis since I got pregnant twice or to move on to IVF. We feared another ectopic or more failed attempts so we opted to go to IVF. She agreed, and said I'm a good candidate. I started stims this past week (on day 4 now) for our first IVF cycle. I am currently on 75u of menopur and 150u of gonal-f. So far my estrogen levels are right on track!

All nurses are friendly and open to answering any question. If they can't answer they always refer to the dr. and follow up. They are compassionate as well.

Mid cycle sonograms are $150
Iui is $375
IVF is roughly $20k
Meds are ordered through integrity pharmacy and range in price for menopur, gonal-f, ovidrel, cetrotide, and crinone. I do not know the exact prices for the meds because the clinic helped me sign up for a discount program that we were approved for. They also have given us meds patients have donated.
Overall we have spent about $16k so far because we are out of pocket.

Dr. Tatpati prefers one healthy baby due to the fact that multiples have higher risks. She will however transfer 2 max depending on the case.

His initial bedside manner at our initial visit had me second guessing if he was the right divorce for us but we had heard good thigns so we kept going (glad we did). He was brutally honest and blunt which I appreciated. Yet compassionate and caring, he even offered a prayer after our first embryo transfer. He always gave us hope.

When I was trying to become a patient they made me feel “dumb” and always blamed their lack of organization on “the fax machine” (I’ve heard that same story from many of my friends who have gone there.). They break HIPPA in the waiting room, luckily no one in there that I cared if they knew my business. After being a patient their for several months (9ish) they became more friendly and by the second time we came back they were great. Soooo to a new patient, yes they come off cranky and unprofessional but the outcome of a baby is worth it!

I felt that for the most part they were kind and compassionate. I loved Megan, Mary and Chelsie. I felt Deb was incompetent and always dreaded getting her. Thankfully I only got her a few times. The second time we went back in 2017 I thought Dr. Tjaden’s MA was as sweet as she could be. Can’t remember her name.
