Thus far, my impression of Dr. Patel is that she treats her clients with the utmost care and respect. She has listened, taken extra time and researched certain topics that I’ve inquired about...She agreed that with my age there were other tests that should be done to rule out reasons that could be impacting our chances to conceive naturally besides my balanced translocation before considering IVF as our next step
The only complaint I have [regarding CRM Orlando] is that I have not always received a call from the nurse to review results of various blood draws, however, this is something I can review for myself online and does not always require a phone call. I try to check my portal every week to stay up to date on any new results to my portal in case I do not receive a phone call...It is evident that Dr. Patel and her staff work hard to provide their patients with the utmost care and respect.

Dr. Patel was warm, caring and truly made me feel like I was getting individualized care. From the start of my diagnosis with balanced translocation my journey had led me to several professionals prior to making my way to a fertility specialist. Unfortunately, not all of these experiences were good ones. I came to Dr. Patel seeking answers and guidance. After the loss of 4 babies, I needed that. Thus far, my impression of Dr. Patel is that she treats her clients with the utmost care and respect. She has listened, taken extra time and researched certain topics that I’ve inquired about to make sure that I feel comfortable, informed and supported. My husband and I have not yet reached the point that we are considering IVF as our next course of action which Dr. Patel has been incredibly supportive of. She agreed that with my age there were other tests that should be done to rule out reasons that could be impacting our chances to conceive naturally besides my balanced translocation before considering IVF as our next step. I cannot say enough wonderful things about Dr. Patel and her staff. I wanted a doctor who has a warm bedside manner, is knowledgeable and who truly has my best interests at heart. I believe I have found that with Dr. Patel.

Dr. Patel has suggested a broad range of blood test and or procedures to explore as our next steps. I want to have all the information we can gather prior to heavily considering IVF. She understands that about me and supports my wishes while also offering guidance.

The nursing staff were friendly, compassionate and prompt with phone calls and messages through my portal.

CRM is a large well known clinic in the Central Florida area and have treated a variety of hopeful parents with various needs. It is evident that Dr. Patel and her staff work hard to provide their patients with the utmost care and respect.

This far I have lid for 2 consultations and various blood tests. I will be able to better answer this question further into our treatment process as we are really at the beginning.

We have not reached this point in our treatment with doctor Patel so I’m unsure of the answer to this question. From what I know of her practices I would assume she would suggest the option that poses the least risk to mother and baby.

In my first consultation Dr. Patel was extremely thorough in my work up and miscarriage history...Unfortunately my second cycle ended in a miscarriage at 10 weeks for unknown reasons. Dr.Patel acted quickly to preserve the pregnancy for a D&C to obtain genetic test...preference is single [transfer] to minimize risk to both mother and babies...has called me directly to discuss results. She remembers our past conversations and questions I’ve had. She has been empathetic throughout this process

In my first consultation Dr. Patel was extremely thorough in my work up and miscarriage history. At my “results” visit she went over every result and what the treatment plan for it was. Unfortunately my second cycle ended in a miscarriage at 10 weeks for unknown reasons. Dr.Patel acted quickly to preserve the pregnancy for a D&C to obtain genetic test. She has since been working with Dr. Kliman at Yale for his endometrial function test on the D&C products as well as an EFT within an IVF cycle. She has been warm, empathetic and thorough throughout my journey

Be very clear with her nursing staff. It is easier to get a response through their portal messages than calling

Describe the protocols Sejal Patel used in your cycles at Center for Reproductive Medicine Orlando (Prelude Network clinic) and their degree of success.
The first two cycles were times intercourse with trigger injections. We used clomid day 3-7 after ovary check, then checked day 14 for tigger injection with low dose steroids, followed by progesterone suppository until pregnancy testing. This was coupled with daily baby aspirin, prenantal vitamins, metformin 1500 a day, and vitamin D

They can be short and unclear at times but clear messages to them result in clear responses

I don’t recall exact costs. A biopsy was roughly 500$ medications ranged from 8-60$ and my hysteroscopy with anesthesia was 2000$

When speaking with Dr. Patel I felt like she was championing my journey. I wish I could see more of her throughout the process. She made my husband and I feel comfortable with her and she truly gained our trust, which is usually difficult for my husband. She made us feel like she was just as invested as we were to help us become parents....Our final round of IUI we came to the conclusion to do a more aggressive form of IUI with Letrozole (5 days) plus Gonal F shots (5-6 days) to help my follicles grow, trigger shot, and IUI...I felt like she was championing my journey. I wish I could see more of her throughout the process. She made my husband and I feel comfortable with her and she truly gained our trust, which is usually difficult for my husband. The Celebration office [of Center for Reproductive Medicine] which I would go to mostly was small and homey. The staff got to know my husband and I much better than in the Winter Park location. The Winter Park location was larger and you almost felt like just another number going through there. The staff is fantastic which is their biggest strength. But you loose that one on one quality time when you go to such a big location.

When in her presence I felt like a human, I felt like she had the same goal as me, she understood my frustrations. However, for follow ups and some monitoring I felt a bit like a number.

At first we did one round of Letrozole (5 days) with a trigger shot and relations. We then moved on to two rounds of IUI with Letrozole (5 days), trigger shot, and IUI. Our final round of IUI we came to the conclusion to do a more aggressive form of IUI with Letrozole (5 days) plus Gonal F shots (5-6 days) to help my follicles grow, trigger shot, and IUI.

The staff was very helpful in answering all of my crazy questions. They have an online portal which I would use to schedule all my appointments and ask all kinds of questions and they were so kind each and every time.

There are two clinics for CRM. The Celebration office which I would go to mostly was small and homey. The staff got to know my husband and I much better than in the Winter Park location. The Winter Park location was larger and you almost felt like just another number going through there. The staff is fantastic which is their biggest strength. But you loose that one on one quality time when you go to such a big location.

Luckily for us our IUI's were covered under my insurance. However, as we move onto IVF we've seen that our out of pocket cost will be about $14K.

Appointments are quick, and they usually run on time. Depending on your doctor’s schedule, expect to see one of the other doctors in the practice from time to time (although they are all good)...The first time I used Dr. Patel, her nurses were amazing and followed up with me to make sure i was on track and understanding next steps. The first time I saw Dr. Patel, she was amazing and I felt like she was determined to get us pregnant. She was warm and caring, yet informative and direct. I am “seeing” her again for a year now, and I have not seen her once, only her PA, Heather. Heather is sweet and knowledgeable, but I feel like it’s just business this time.

I used Letrizole for one cycle to see how i was ovulating. Then I used Letrizole + a trigger shot + IUI, which resulted in a healthy baby. She diagnosed me with PCOS and said that sometimes I ovulate and sometimes I don’t. She thought and trigger shot and IUI was the best fit for me.

Diagnostic testing was covered under insurance, so only small copays. Once diagnosed, everything else came to approximately $1300.

This time around (currently still going), I was told my doctor was calling in a particular medication, but according to my bloodwork, I did not understand why (indicator was in range). When I called the office, I spoke with the doctor’s PA. She did not know why I was prescribed the medication and it took some digging to find out why. We went back and forth about it, and they ended up sending me to an endocrinologist.

Dr. Patel answered all of questions whenever I had any. When my first cycle of timed intercourse with letrozole and ovidrel injection didn't work, she moved me on to IUI. Her warmth and personality in our meetings have made me feel comfortable. And her confidence that I would get pregnant has made me hopeful!... I went in to my first visit with a ton of questions. She patiently sat and answered all of my questions (one was even a little weird and she looked up the info right there!) She never made feel rushed or bad for having so many questions.
Describe the protocols Sejal Patel used in your cycles at Center for Reproductive Medicine Orlando (Prelude Network clinic) and their degree of success.
I had one Timed Intercourse cycle. I went in for monitoring on cd 3, took 2.5 mg Letrozole from cd 3-7 then went back for monitoring at cd11. My numbers were where they needed to be so I did a trigger shot of Ovidrel that night with intercourse that night and the next two nights. I also used Prometrium during my luteal phase. Sadly, I did not get pregnant. I had to wait to be on my husband's insurance so we are now in the middle of our first IUI cycle. Again, I went in for monitoring on cd 3 and took 2.5 mg of Letrozole from cd 3-7. I went in for monitoring on cd 11 but my numbers were not where they needed to be. I went in again on cd 13 and they were still not high enough and my lining was thin so she put me on estrace and I will go back for another ultrasound and bloodwork on cd 15.

I had done my research on my endometriosis and what our treatment options may be before meeting with Dr. Patel. When I had my first meeting, she was very warm and caring and explained the different types of infertility treatments available but she jumped straight to IVF being our only way of getting pregnant. Once I explained that we were not quite to that point she tailored a plan to my needs.

I arrived early for my appointment and they were able to see me 20 minutes before my scheduled time. My only real complaint would be with financing.

The 2 times I have actually SEEN Dr. Patel she has always been warm and caring and answered all of my questions. I only saw her for my initial infertility consultation and then my second consult after surgery to start IUI's.

I had done my research on my endometriosis and what our treatment options may be before meeting with Dr. Patel. When I had my first meeting, she was very warm and caring and explained the different types of infertility treatments available but she jumped straight to IVF being our only way of getting pregnant. Once I explained that we were not quite to that point she tailored a plan to my needs.

First we did labs and an ultrasound to get a good baseline. Then we moved on to IUI. Baseline scan CD1-3, letrozole 5 mg CD 3-7, follicle scan CD11 and trigger to follow. IUI 36 hours after trigger. Progesterone level draw CD 21 (mine was on the lower end so I started suppositories CD21 until 28). Take home pregnancy test 14 days after IUI.

Every nurse that I have come in contact with in the office and over the phone have always been WONDERFUL. I am a night shift nurse and sometimes I need my appointments first thing in the morning after work/between shifts and they are always super accommodating.

There were a lot of mistakes by the nurse that was working with us and I was driving to different office location because they were messing up medication orders and trying to make it up but in the end it seems she must of been let go because we got a new nurse after transfer and she seems to be doing well so far no complaints yet.

He treated me like a human. He was very compassionate with my husband and I both. He always gave us as mush as we wanted during Office visits and made sure to answer any questions we had. He ever allowed my husband to conf call in when he couldn’t make it to my appointments.

Only two made it to blast and only one came back Pgs normal. We also did lupron and progesterone prior to transfer and progesterone during the tww. Unfortunately it didn’t take and we didn’t get above a 0 on my beta. We are now prepping for the 2nd cycle with new supplants for about 3 months prior to retrieval. Hoping for better quality eggs this time.

Beautiful offices, front desk staff need work. As a patient if infertility issues I would expect the front desk staff to be over friendly and inviting and they really aren’t. There’s no welcome or greeting when you walk in. No eye contact just a “sign in and we’ll call you when we’re ready” it’s like that at both clinics. One clinic is definitely nicer then the other but the front desk staff just needs customer service training.

Just during stims meds were told to be ordered which needed to be over nighted we spend about $200 and came towards the end of the day when we were supposed to trigger we were told we didn’t need the trigger shot or the meds and that they made a mistake and I needed to drive to the Orlando Office instead of celebration to pick up a shot that I need to take the next day. I drive over an hour the Orlando and pick up the shot and got a call saying I didn’t need the shot and to bring back the unused injection during my pre op appointment. This nurse isn’t there anymore so I’m hoping my second round doesn’t have any issues. Also I was supposed to use suppositories and the nurse forgot to instruct me on that and the doctor was not aware I wasn’t on them.

We started with 2 cycles of medicated cycles with letrozle, orebyl trigger and progesterone. Then we moved to iui with the same protocol. Then the third iui we added estradiol. After the third failed iui we decided to take a break from treatment and also to use that opportunity to try to lose weight to improve fertility. We will resume treatment in May 2017, 30 pounds light. We will likely try two more iui before IVF.

She was very nice especially when delivering bad news. She didn't always have the answers to my questions but was always good about getting the Doctors answers and getting back to me.

Combo cycles with femara and injections with trigger. Ovulation occurred but no pregnancies. Last cycle, no response to meds. IVF with PGS/ICSI freeze all cycle with FET. Pregnant and had a singleton.

Several nurses were great but there was one in particular who most definitely was not. She didn’t belong in the infertility field. Very cold and gruff.

I had 3 failed rounds of Clomid and 3 failed rounds of Letrozol at the highest dose that could be prescribed by an OB/GYN. She did all of the prerequisite tests to make sure I could become pregnant and stay pregnant, and then she raised the dosage of Letrozol for me. We continued with ultrasounds to check follicle size and then did a forced ovulation trigger shot. It technically worked the 1st time but resulted in a chemical pregnancy. We tried again immediately (counting the chemical pregnancy as a period bc I don’t have them on my own) and we were successful

He [Dr. Silva] was very kind and explained our situation well. He checked me to see if I had a block from my previous surgeries...went up my cervix with a camera to check to see if there was any scar tissue blocking me from getting pregnant from my c-section. Next step was 3 IUIs, then to IVF. He thought there might be a blockage where the sperm cannot reach the egg...went through extensive details of why me and my husband might not be conceiving

He was very nice and went through extensive details of why me and my husband might not be conceiving- we had my former OB who I did my Clomid and Letrozole treatments with and husbands sperm analysis send over her paper work. He went through each page with us, reading carefully. He checked my tubes during a office visit and then went up my cervix with a camera to check to see if there was any scar tissue blocking me from getting pregnant from my c-section. Next step was 3 IUIs, then to IVF. He thought there might be a blockage where the sperm cannot reach the egg. We didn’t continue treatment because of the out of pocket costs and traveling out of town missing work.

The mounting costs of all these procedures weighted heavy on our shoulders ontop of the drive to the clinics (45 minutes away) and missing work. My insurance doesn’t cover anything with fertility treatments

I attended both locations [of Center for Reproductive Medicine] in Celebration, FL and Orlando, Fl. The clinic in Orlando is where they completed more procedures like HSG testing and IUI's, etc. There were long wait times at this clinic and it wasn't very comfortable temperature wise (it was fairly warm in the waiting room). The Celebration clinic was a lot smaller and was more so for consultations and blood work. These appointments rarely ran behind. The staff at each place was nice.

Their offices are nice and very comfortable. She rocks! She explains everything to me in normal people terminology meaning not doctors words. She is hands on and really cares about her patients! I travel over 2 1/2 hours to go to her center to see her even though I have an IVF center here where I live.

The costs are ridiculous for IVF however I don’t believe the doctor can change that. Our insurance doesn’t help either.

She is very sweet and friendly who try to help me with other treatments because I couldn’t afford ivf...She did all my tests and after HSG test they find out my tubes are blocked because of fiborids . So she suggested me egg retrieval before surgery because i was at low AMH . But now when i can’t afford ivf so i am going surgery before as her suggestion

Dr Patel did natural cycles for me, taking into account my preferences. She worked with me when it became apparent that ovulate late and didn’t cancel my transfer cycles because of that—both transfers were successful.

Never once have I felt like we were "just another couple" seeking fertility treatments...CRM has their main clinic facility in Winter Park with an attached surgery center for most procedures...they would return the call that same business day unless the call was made after 3:30pm and not time sensitive

Dr. Loy is very personable and takes the time to get to know you and your situation. He took the time to answer the many questions we had about the different options available to us. He carefully reviewed all lab and procedure results to carefully select a treatment plan that he thought would be most beneficial for our specific situation. When it came to deciding if we would do PGT-A testing on any embryos we had frozen, he had originally recommended against it. Ultimately we decided it was the best option for our family and he supported our decision. I respect the medical decisions and options he set before us because I know that ultimately, he wants us to achieve a successful pregnancy and will do whatever he can to get us there, even if it means adding in additional testing/procedures before transferring one of our precious frozen embryos.
What's one piece of advice would you give a prospective patient of Randall Loy at Center for Reproductive Medicine Orlando (Prelude Network clinic)?
Have your list of questions ready. Dr Loy is a well respected reproductive endocrinologist in our area and will take the time to hear you. Don't be afraid to ask the big, important questions. Be patient. It's very possible that he will add in testing or procedures to your protocol. Don't take that as a negative, see it as a positive. He is doing his best to ensure a viable environment for a healthy pregnancy. Do the PGT-A testing on your embryos, even if he says you'd be okay without it. It will give you peace of mind to know that your embryos are viable for transfer.

During any treatment cycle, any appointment, I felt seen. It didn't matter if it was with Dr. Loy, one of the nurses, or one of the other doctors...I could tell there was genuine compassion for meeting the needs of myself and my partner. Our care team has taken the time to get to know us personally from making appointments work with our work schedules to calling post-op to check in on how we were feeling. Never once have I felt like we were "just another couple" seeking fertility treatments.
Describe the protocols Randall Loy used in your cycles at Center for Reproductive Medicine Orlando (Prelude Network clinic) and their degree of success.
For our first retrieval we followed an antagonist protocol. We started off with 225iu of Menopur in the morning and 225iu of Follistim in the evening. After my first blood draw we dropped the Menopur to 150iu and added in Ganirelix. Follistim stayed the same the entire protocol. We used Lupron and Pregnyl for trigger shots. These were not prefilled syringes (the trigger shots) and were difficult for us newbies. The cycle led to 16 follicles, but only 8 eggs were retrieved. Of those 8, 5 matured and 3 fertilized. We ended up with one PGT-A tested normal embryo. We consulted with our care team afterwards and made some slight adjustments for our second retrieval. We lowered Menopur to 75iu and kept Follistim at 225iu. Ganirelix was added on day 4 of stims and we triggered with Lupron and Ovidrel (this time prefilled syringes). Our hope was that by starting with lower doses of Menopur, my follicles would be able to grow around the same rate thus providing us with more eggs to retrieve. On my retrieval day we got 9 eggs. Of those 9, 3 matured and 3 fertilized. We sent off two of the three for PGT-A testing are awaiting those results.

The entire nursing staff, both in the clinic and their surgery center, show genuine compassion and care during treatment. Even when not in an active treatment cycle, they are available to answer any questions and schedule appointments. They take the time to check in to see how you are feeling (after a procedure), they ask if injections are going well, and are quick to return calls if they are not assisting other patients at the time. The nurses that I have had the privilege to work with have validated my feelings as they have both gone through similar treatments in the past. It was helpful and made me feel better to know I'm not alone in this. It also gave me hope for the outcome of my treatment cycles.
Describe your experience with Center for Reproductive Medicine Orlando (Prelude Network clinic).
CRM has their main clinic facility in Winter Park with an attached surgery center for most procedures. I found this surgery center to be extremely beneficial because it is easier to get scheduled in for procedures than going to a hospital. They also have a second location in Celebration which was helpful for me when trying to get early morning monitoring appointments that would affect my work schedule the least amount. All of the staff between both locations are extremely friendly and helpful. They are like an additional support group that are constantly rooting for you and your treatment cycle. When trying to get in touch with the staff, most of the time you will have to leave a voicemail and wait for them to return the call later that day. Sometimes I found that to be a tad bit frustrating because often times I had time sensitive questions that I wanted an answer to rather quickly. But, they would return the call that same business day unless the call was made after 3:30pm and not time sensitive. They do have an answering service that kicks in at 4:00pm and will forward your calls to the on-call nurse. Depending on the nature of your call, that nurse will either call you back the same day or the next.
Describe the costs associated with your care under Randall Loy at Center for Reproductive Medicine Orlando (Prelude Network clinic).
The cost of one IVF cycle included embryo freezing, monitoring appointments, egg retrieval, embryology, PGT testing for up to 5 embryos, and freezing. This total cost would be around $15,000 and was expected to be paid upfront before a baseline appointment. If there were more than 5 embryos that were to be tested, the clinic would balance bill for each additional embryo. PGT-A cost was $350 per embryo + $100 flat shipping fee. The following were not included in our IVF cost breakdown, but were required: medications were predicted to cost between $4,000-$6,000. The facility fee for the retrieval is $1,000 and anesthesia for the retrieval started at $500 for the first 30 minutes and would go up to $750 ($125 for each additional 15 minutes). Storage fee for embryos is $60/quarter. Hysteroscopy was $1,650 plus an $800 facility fee. This is considered diagnostic and could potentially be billable to insurance. CRM also included a breakdown of FET fees that we will be responsible for when the time comes. Medications for the FET are expected to be anywhere from $1,000-$2,000. Transfer day is approximately $4,210 plus a $900 facility fee. This does not include any monitoring appointments or HCG tests. Of course with any clinic, these prices will vary per couple as each couple’s treatment protocol will be different. Because I am a teacher, we were also able to qualify for a small discount on our overall IVF cycle fee.

We have not reached out FET yet, but Dr. Loy will most likely recommend transferring one embryo at a time since we have three frozen embryos.

Some [nurses at Center for Reproductive Medicine] were nicer than others.. but overall it just felt like a lot of unneeded anxiety from never knowing the answers to the questions to giving us the options to do things if "we wanted to."...If your cycle happens to fall or trigger or IUI on the weekend- it is very stressful to try and get intouch with the Dr. or the staff bc you have to go through a call center...Very clean clinic, I feel safe and seems like a good place to be. Everything is on site which is great.

Sharon is very very smart and definitely knows what she is doing but when you come as a patient that is scared and has never imagined they would be doing fertility treatments or that you would like a deeper explanation bc you want to know what is going to be happening to your body and WHY? Or if you need that extra bedside manner to make you feel comfortable I don't think she is the right match for that personality. If you don't need any of that then she probably would be perfect. I wish we would've felt like we were having more attention paid to us. To us this was a very delicate emotional rollercoaster in our lives and we need that compassion and patience.

The first meeting with Dr. Jaffe I felt very good minus some speeding through discussion about treatment and having us answer questions to what we preferred when we didn't even understand what we were answering. I feel that some of my blood work was rushed through and could of been rechecked after failed treatment. We felt like we wanted to know more details and we would get vague answers from original paperwork given copied and pasted to our questions not from Dr. Jaffe but from her nursing staff. Nothing was changed after each fail and that was very frustrating or extra effort to help my husband achieve more sperm. We had to advocate for ourselves to freeze sperm. Just felt like a rush to get through 3 IUI and then next IVF. Very little contact with the Dr. if we wanted to talk to her again we had to get in line for another consultation.

There were several nurses that would respond or call us back. Some were nicer than others.. but overall it just felt like a lot of unneeded anxiety from never knowing the answers to the questions to giving us the options to do things if "we wanted to." One nurse in particular was pretty short and very easily annoyed with questions from us almost was condescending. While we are not Dr. we both were very good at educating ourselves and reading and researching so we had valid questions and concerns. I believe if we were talking to Dr. Jaffe she would have answered all our questions - quickly - but not impatiently.

Very clean clinic, I feel safe and seems like a good place to be. Everything is on site which is great. Weaknesses seems to be a lot of people there all the time which adds to the feeling like a number. Things have gotten disorganized or my appointments mixed up and sat in the waiting room for far too long over an hour sometimes. When we had to wait an hour on an IUI day it was very concerning bc we knew it was time sensitive and made us feel like a number. Another weakness is the way you have to contact the clinic on the weekend. If your cycle happens to fall or trigger or IUI on the weekend- it is very stressful to try and get intouch with the Dr. or the staff bc you have to go through a call center and say its URGENT and we would've missed a call 1 minute from the nurse and then call back and the call center people won't transfer you or be difficult taking your message and we had to be like THEY JUST CALLED US

Consultation and visits $50, monthly storage $60, Medications some covered by insurance some not $5-500 depending on what medication. Bloodwork $30-60? IUI $450. Each IUI overall about $800 (Our insurance covered the Ultra-sounds thankfully)
