Anything more complicated regarding questions about protocol, [the Extend Fertility nurses] would let me know they would relay to Dr. Klein and would call me back with an answer. How was your experience with Joshua Klein at Extend Fertility?
Dr. Klein is fantastic. I was nervous about this process and came into the clinic for a second opinion after a not so great experience at what is considered the "top" fertility clinic in New York. I was hesitant about the whole process, and also came prepared with lots of questions, and felt completely comfortable moving forward by the time I left. He is honest and straightforward so as not to get your hopes up, but communicates information in a kind and empathetic way. I had a disappointing result after my first cycle and was devastated (my body just didn't respond to the meds as well as we had anticipated). Dr. Klein called and gave me the news himself and I could tell he was just as disappointed. He made himself available every time I asked him to speak again to ask more questions about what went wrong and made suggestions on what changes he would make to the protocol for the next round (while also letting me know we may not get better results the next time). He never gave me false hope, but I felt confident in his plan and decided to do two more rounds a year later. When I came back to the office, he remembered everything from our last round, we moved forward with the new protocol. I was stressed throughout the process but he was always available to answer questions (and I always had a lot of questions) and I always felt like he truly cared about the outcome. All of the changes he made to the protocol worked and I had two more retrievals with great results :)
What's one piece of advice would you give a prospective patient of Joshua Klein at Extend Fertility?
If you are unsure of anything or have questions, just ask! I had questions about my diet, different protocols, and a million other things. He was always open to listening to me and providing thorough explanations that helped to make me feel more at ease.
During treatment, were you treated like a number or a human with Joshua Klein at Extend Fertility?
Everyone in the office is kind and helpful; they are efficient and understanding. I always felt like I was in good hands, whether with the nurse, sonogram technician or a doctor. They are friendly and make themselves available to answer questions as needed.
Describe the protocols Joshua Klein used in your cycles at Extend Fertility and their degree of success.
All 3 rounds we used follistim and menopur at varying doses, as well as cetrotide. The first round used a lupron-only trigger. For the second round, I primed with estrogen patches with the rationale that it would help the follicles grow in cohort. I also started at a higher dose of follistim and used a dual-trigger to help the eggs mature. Third round we did the same as the second.
Describe your experience with the nursing staff at Extend Fertility.
I really only spoke with the nurses over the phone when they gave me instructions, and when I came in for my surgery. However, they were always knowledgable and answered any questions I had as they came up. Anything more complicated regarding questions about protocol, they would let me know they would relay to Dr. Klein and would call me back with an answer. They were consistent and reliable - I don't think I ever had to call to follow up on anything.
Describe your experience with Extend Fertility.
I think Extend is great at what they do. The office is clean, the staff is efficient, and it is run like a well-oiled machine. They are so much more affordable than other clinics in the city, but the doctors and staff are top notch. As I mentioned, before deciding on Extend, I went to one of the "top" research clinics in the city, and while I was ready and willing to pay 14k for at least one round of egg freezing, I had a really bad experience with the doctor relaying the wrong information and doing so in a rude a callous way. I am so glad I did my research and found Extend. I was able to do 3 rounds for about the same amount of money as one round of egg freezing elsewhere, with an amazing team, much better experience, and great results. They're really a great team - even the embryologists emailed me after I had questions the nurse couldn't answer. Advice to a new egg freezing patient: do your research - on the doctors, the lab, and the clinic. Understand how many eggs you want to freeze going into it, and how many cycles that may take. Make sure you feel comfortable with your doctor and the team you're working with - it's a stressful process, so do your best to limit additional stress as much as you can by making sure you feel you're in good hands.
Describe the costs associated with your care under Joshua Klein at Extend Fertility.
Each cycle was between $5k-6500. I know their prices have changed since the first time I was there, but each time I paid I was fully aware of the payment, and that was the only payment I had to make to the clinic which I appreciated. I know other clinics charge additional fees for blood work, consults, anesthesiologist, etc. I had to pay for my medications separately, which is standard. I paid out of pocket, and appreciated knowing this fee covered everything as it was one less thing to worry about during treatment.

I am especially appreciative for a specific out-of-the-box decision he [Dr. Joshua Klein] made during my treatment that I believe contributed to the success of my first IVF cycle with him. I didn't feel like there were cookie-cutter protocols, but rather that each patient's success was given adequate thought...cares genuinely about each patient and it shows in his communication, responsiveness and throughout every step of the journey. I have never been disappointed.
I liked the extend fertility clinic. It's nicely designed! When you walk in for monitoring in the morning, you are greeted warmly (it's not a cold silent room like waiting rooms in fertility clinics are known for). Most of the staff knows you. Things run efficiently. I liked that there weren't set appointments for monitoring, you can just come in within a certain time range, at your convenience...Jordana, my IVF coordinator was extremely helpful and able to answer my questions throughout the process. She was super organized and helped with all the long-distance logistics too.
How was your experience with Joshua Klein at Extend Fertility?
Though there are plenty good doctors on the West Coast, there are none I found as caring and skilled as Dr. Klein. There is nothing like the peace-of-mind and reassurance that comes with fully trusting that your doctor has your best interest in mind. Personally, if I had to choose between a doctor who was extremely skilled, had great success rates, professional etc. vs a doctor who is kind caring and gentle - I would opt for the former, a doctor whose expertise I can rely on completely. But the greatest gift for me was that I didn't have to choose. He excels in both areas. I was comfortable to bring up any questions that I had and always received a clear and thorough response. He's fully present when he's with you and takes his time to explain things thoroughly.
During treatment, were you treated like a number or a human with Joshua Klein at Extend Fertility?
Not only does Dr. Klein stand "head & shoulders" above other Reproductive Endocrinologists that I've seen but he stands out among any doctor I've known. Dr. Klein cares genuinely about each patient and it shows in his communication, responsiveness and throughout every step of the journey. I have never been disappointed.

Although I do not remember the specific medications, I do remember feeling pretty decent throughout stimulation. Jordana, my IVF coordinator was extremely helpful and able to answer my questions throughout the process. She was super organized and helped with all the long-distance logistics too. Most of our correspondence was by email. The nurses were great - everyone knows who you are. Always got a call from them pretty early in the day with the medication instructions, plus a clear email. If I had a question for them, I'd call and get a call back pretty quickly usually. Being able to email them would've been nice since often when I called they were on the phone with other patients, in a meeting etc - but it was fine, since they would call me back quickly. And if I ever had a very pressing question or situation and a nurse wasn't available that minute, there was always someway somehow that someone would be able to help me. As a team, they all cared.

Extend Fertility had a very calm, soothing atmosphere, patient privacy seemed paramount, and it didn't feel like a clinical environment. Those are the positives. The negatives are that they don't--or didn't, in my case--seem to offer any flexibility in terms of caring for patients with co-morbidities that may affect egg retrieval cycles.
How was your experience with Michael Guarnaccia at Extend Fertility?
I explained at the outset of my first consultation that I'm not the average patient, given my conditions, but Dr. Guarnaccia launched into the general statistics for egg freezing/fertility by age, the relevance of AMH, potential side effects--the standard dog and pony show (Extend was not the first clinic I visited--I also went to NYU, but the doctor there acknowledged me and handed me a packet and told me to schedule an appointment). My experience with Dr. Guarnaccia wasn't negative--he was nice and friendly, but the treatment felt rote, and his medication recommendations were very standard combinations of medications for me that had also been recommended to other women at the follow-up experience where you're taught how to inject yourself. I had specifically asked if he'd be willing to consult my endocrinologist and rheumatologist and my pain management doctor who manages my MCAS diagnosis, and he demurred. It's such an enormous investment of time, money, physical pain, psychological pain (I'm terrified of blood draws) that I wasn't satisfied I'd get the tailored treatment a patient like me probably needs, which was disappointing.
What's one piece of advice would you give a prospective patient of Michael Guarnaccia at Extend Fertility?
If you're young and healthy, he'd likely be a good choice for you. I think I'd heard he's not known for his bedside manner, but I found him to be likeable enough. I actually cared less about those metrics and much more about my doctor's commitment to tailoring my care/medications to fit my needs, and I didn't have enough confidence that my concerns about that were being heard.
During treatment, were you treated like a number or a human with Michael Guarnaccia at Extend Fertility?
I was and am an extremely informed patient wanting to pursue egg freezing at the time, but concerned because of my medical history, which includes autoimmune issues. I had been led to expect that I would get tailored information during the consultation and afterwards, and during my appointments for bloodwork which would be tailored to my conditions, and could help decide whether egg freezing would be right for me, which medications might work, which ones might cause flare-ups, etc. What I got from the doctor was instead a very rote, paint-by-numbers list of statistics and charts of what to expect success-wise by age and AMH, with no accounting for other factors. I expected doctors to meet me where I am, as a person and a patient with co-morbidities, but instead this made me feel like I was a potential customer being offered standard packages.
Describe the protocols Michael Guarnaccia used in your cycles at Extend Fertility and their degree of success.
I have them written down somewhere, but I don't have access to them at the moment--basically, Extend has four medication packages they offer in conjunction with the pharmacies they partner with, with varying strengths/dosages of medications. On a scale of 1 (being the least intense) and 5 being the most intense, he recommended dosages of Gonal-F and Menopur (I believe) at the 4 level/amount/dosage. I wish I could be more specific.
Describe your experience with your nurse at Extend Fertility. (Assigned nurse: Kezia Onel)
Kezia was, I believe, my overall coordinator, but my questions were about my current autoimmune medications and conditions (thyroid levels, other medications I was/am on, etc), so I was referred to a nurse, Stephanie. She didn't really have any answers for me--which wasn't entirely unexpected, as she wasn't a rheumatologist or endocrinologist, which is why I was really hoping that my doctor would consult with my other doctors to provide the right course of treatment for me. But otherwise my nurse and care coordinator were very personable and friendly.
Describe your experience with Extend Fertility.
Extend Fertility had a very calm, soothing atmosphere, patient privacy seemed paramount, and it didn't feel like a clinical environment. Those are the positives. What they offer are packages (dosages of medications and treatment schedules)--and my sense was that they fit the patient to the package, not the package to the patient, which someone like me needs. If you have thyroid issues, rheumatology issues, MCAS, a condition like Ehlers Danlos, or really just any medical issue that requires you to take medication(s) that affect your endocrine system, or if you receive more specialist care than you do from your GP, Extend Fertility may not be the right place for you.

At the time, with a promotion, a single egg freezing cycle was $5500, not including medications. My medications (the ones/dosages recommended to me) were in the $4500 range. So, about $10,000. I believe the costs now are about $12,000.

I sent in all of my labwork from other clinics and my personal specialists, and there were major inconsistencies in my bloodwork done at Extend vs. not only my regular endocrinologist, but also at Cornell and NYU. My Day 1, 2, and 3 results were wildly inconsistent with other labs; for example, my AMH was 2/3 lower than each of the other labs (Sunrise as well as Cornell and NYU's labs). My E2, FSH, and LH were also wildly different at Extend's lab vs. the other labs, which were in the same general range, and the Extend Lab values made me seem less likely to succeed with one cycle only. I asked about this, but got a general answer that "lab results differ." It seemed an odd coincidence, though.

In 2017, I went through 2 egg freezing cycles [with Extend Fertility] which I had a total of 16 eggs frozen. In 2019, I started the IUI process. After 4 failed IUI attempts we decided to try for IVF cycle. My body didn’t take to the IVF medication so we fertilized my frozen eggs. 5 embryos were created using ICSI and then PGS tested. We transferred one which didn’t result in a pregnancy. I tried another round of IVF which this time my body took to the medications. Dr. Klein retrieved 10 eggs and 5 embryos were created using ICSI. I did not get PGS testing done. He transferred 2 and now I’m pregnant with twins.
How was your experience with Joshua Klein at Extend Fertility?
Dr. Klein is a great communicator. As I mentioned before, it’s necessary to provide the patient with statistics; however, he never left me without hope. I had my frozen eggs fertilized and it resulted in 5 embryos. I sent it out for PGS testing with 4 coming back abnormal and 1 inconclusive. He was willing to transfer the inconclusive as that is what I wanted- which did not lead to pregnancy. I then went through an IVF cycle and he was able to retrieve 10 eggs. Again, we had 5 embryos. This time I did not want to have PGS testing done- which Dr. Klein respected. He transferred two and now I’m pregnant with twins!

Have your questions ready because Dr. Klein is very thorough and ready to answer any questions.

Where do I start? It's been such a long journey for me. It started off in 2017 when I went through two rounds of egg freezing cycles at Extend. In 2019, I became an Expect Fertility client, as I was ready to begin my road to motherhood. Through it all, Dr. Klein has been extremely patient with my numerous questions. He has the best bedside manners and always thoughtful as he explains everything that he's doing. I'm a very detail oriented person and he was great with providing information without my having to ask. He's also very open minded to hear about my findings on the internet and discuss the subject matter. I never felt like he was condescending and he never scoffed at what I had to say. He truly listens to his patients. And he always had hope. I understand that in this industry, the doctor has to be data and statistics driven. As a patient, it's always nice when after given all of the ratios and numbers (which is a huge dream zapper), the doctor ends it with- it only takes one! It allowed me to keep the faith and stay optimistic.

Molly, Stephanie and Valerie. They had the most patience with me. I had SO many questions. They never seem exasperated with me. They made me feel like all of my questions and concerns were valid to them. When I had a situation happen- Valerie was calm while I was frantic. She said that she would call me in 30 minutes to check on my symptoms- and she called in exactly 30 min. She followed up a couple of more times afterwards. She truly cared.

The staff truly cares and treats you like a human. They call you by name and will cheer you on. If you get there first thing in the morning for monitoring, it’s easy to get in and out. It tends to get more crowded later in the morning.
Describe your experience with your monitoring appointments at Extend Fertility.
Early mornings when they first open would result in less patients in the waiting room. They would be more women as it got later in the morning. Though, even with a lot of women waiting- the staff were very efficient. They would get your blood work and shortly after you would have your ultrasound.

The clinic [at Extend Fertility] is a friendly, easygoing environment. They are very forgiving if you run late, though this also means you may end up waiting for a while..avoid monitoring visits on Sundays -- if necessary, go as early as possible in the day. The clinic also allows children in the office, so be prepared to see babies on occasion, especially on Sundays...The nursing staff is friendly, compassionate, and expert
How was your experience with Baruch Abittan at Extend Fertility?
I found Dr. Abittan very personable, comforting, and competent during a difficult time. At our first meeting (consultation), I was concerned that he was not taking the difficulty of my personal medical situation seriously enough -- he seemed a little flippant. However, I found him to be a dedicated and caring clinician. I think he just has a more upbeat attitude. Other doctors may be more cautionary when discussing expectations. He has kindly and clearly answered all of my panicked questions. When I ask technical questions, he provides a solid, clear, evidence-based answer. I transferred to Extend Fertility from another clinic and did have to push a little for Extend to speak with my prior provider (beyond reading my medical chart) -- but I think this was a logistics issue. Dr. Abittan will not lie or exaggerate, but he seems more likely to emphasize the positives -- actually usually very helpful during these stressful procedures.

Dr. Abittan is quick, comprehensive, relatable, and warm when responding to emailed questions. He also makes sure to check in and conduct office exams himself when in a difficult part of the cycle. (Otherwise, sonographers and other physicians may conduct monitoring exams.) He is very upbeat and optimistic -- at first, I thought this might be flippant, but it is just good, kind nature.

The clinic is a friendly, easygoing environment. They are very forgiving if you run late, though this also means you may end up waiting for a while in the waiting room for a monitoring visit or procedure. It can be anxiety-provoking to wait for a procedure (e.g. retrieval surgery), but the office is managing the time and your procedure will not fail because of a longer wait than expected. If possible, avoid monitoring visits on Sundays -- if necessary, go as early as possible in the day. The clinic also allows children in the office, so be prepared to see babies on occasion, especially on Sundays. This may be a little emotionally difficult if you are having a difficult time with your treatment.

I felt that Dr. Maslow was definitely educated and intelligent and when I did get to speak with her, she was very willing to explain everything. However, after the initial consultation, I did not see her once. She was not at any of my monitoring appointments and did not perform my egg retrieval. After the retrieval, she did call me with the results, even though they were worse than we hoped

I had two vials of menopur and gonal. At a certain point the menopur was increased the three vials and cetrotide was added. My cycle was not nearly as successful as we had hoped it would be.

The care coordinator was extremely helpful and always called me back quickly answering all of my questions. I felt that the nursing team in general did not take the time to make sure that the patient fully understood the instructions or results. I started with his standard protocol for my initial lab results. About half way through the dosage was adjusted. The results were very positive.

One of the things we loved about Dr. Klein was his strong communication. He would always answer emails with any questions we had or would be happy to jump on a call. Communication was very important to us when deciding on a doctor / clinic.
Describe the protocols Joshua Klein used in your cycles at Extend Fertility and their degree of success.
For our egg retrieval process we utilized the following drugs - menopur, follistim and ganirelix. I was on high dosages for all drugs due to my low AMH levels. We only had to do 1 egg retrieval process as this resulted in 6 genetically normal embryos. Given mine & my husbands age (late 20s early 30s) Dr Klein gave us a high confidence rating that 6 genetically normal embryos will yield in 2 children, potentially even 3. We were always on board for genetic testing so there was no persuasion needed by the clinic. They also did ICSI. For the frozen embryo transfer I did oral estrogen, progesterone suppositories and progesterone in oil shots. My progesterone levels were border line low hence the need for both suppositories and intramuscular shots. I was weaned off at about 10 weeks gestation. The first (and only) frozen embryo transfer worked and we now have a son.

The egg retrieval process not including medication or genetic testing was $11,000. Genetic testing was an additional cost of about $2000 (done by an outside lab) Medication costs were roughy $4000 for the egg retrieval. The frozen embryo transfer not including medication was $3000. Medication for the transfer was covered by insurance.

Dr Klein does not transfer more than one embryo at a time. He told us the goal is to have a healthy full term pregnancy, and transferring multiple embryos increases the chances of multiples which are usually higher risk pregnancies. We believe he does what’s best for his patients.

Additionally, a really big perk of Extend, is that it has an additional office/OR on the floor above its main clinic - This means that all procedures to be done happen in the same building, no hospitals to go to, just convenient and easy.

Fertility is unique, where everything is so tailored to the individual person - What works for some, may not work well for another, and therefore, having a great doctor, who tailors treatment to you, the individual, is so important. Additionally, fertility treatment has an immense emotional and psychological component to it, thats crucial for your doctor/care team to address and be conscientious of. As a patient of Dr. Klein, you get the best of both worlds - You receive the most incredible medical care available, tailored to you and your specific needs/bodily responses, in addition to having the most caring and compassionate doctor imaginable. Dr. Klein not only provided the utmost medical care, regarding all treatments performed, but he always made sure to address the psychological aspect of all that I was going through, and always suggested ways to help me cope and to not feel alone. There's truly not enough words/ways for me sing my praises to Dr. Klein. Dr. Klein, and all of the Extend Fertility team, made my experience so personal; I always felt cared about, I always felt like my questions and concerns were being validated, and I always felt like a conscious effort was made to make me feel as good and as happy as possible. Going through IVF can get really tough, both physically and emotionally, and I always felt like those tough aspects were addressed; whether is was by a nurse calling me with instructions, sonographers at monitoring appointments, for really by anyone I encountered or spoke with at Extend, my well-being was always addressed and I always felt truly cared about. Not once throughout the almost year and a half I spend there did I ever feel like a number - I always felt cared about, and like my care was personalized.
Describe the protocols Joshua Klein used in your cycles at Extend Fertility and their degree of success.
I originally started seeing Dr. Klein due to recurrent pregnancy loss. During my last pregnancy, before starting IVF w/ PGT-A, Dr. Klein thought out of the box, and prescribed me progesterone and lovenox, in the hopes that those medications would help sustain the pregnancy. When my pregnancy resulted in a miscarriage, we decided to pursue IVF with PGT-A testing, in hopes of transferring a genetically normal embryo and subsequently achieving a healthy, viable pregnancy. Since I am relatively young, with a normal ovarian reserve, I responded well to the stimulation medications, and had, what I believe to be a pretty normal stim's cycle protocol (the same protocol was used for both of my egg retrieval cycles) - Menopur, Follistim, Ganirelix, and a Luprolide Trigger. Both cycles yielded over 35 eggs, majority of which fertilized.

I have nothing but great things to say about the nursing staff at Extend Fertility. The nurses play a huge role in your overall treatment, as daily instructions and next steps are usually relayed by them. With that said, I'm so grateful for all of the nurses at Extend. They were all always so kind, so genuine, and so patient. They never made me feel rushed while on the phone with them, and they were always more than happy to address any questions or concerns that I raised. They showed tremendous compassion, and always tried to accommodate my schedule as best as possible. The nurses truly enhanced my overall care at Extend and always made me feel so cared about. When, after a long time, I had success, the nurses truly shared in my joy with me, and it was evident how much they cared and how genuinely happy they were for me.

Dr. Klein strongly recommended transferring only 1 embryo, especially since my embryo's had PGT-A testing and only genetically normal embryo's were being transferred. I completely agree with this approach, and was a big proponent of transferring 1 embryo as well. When, after 2 of my transfers/FET's failed/did not result in a pregnancy, before moving on to another transfer cycle, Dr. Klein recommended a short break, and he performed the Endometrio test/biopsy as well. When the biopsy results came back as receptive, I suggested/asked if transferring 2 embryo's could be done the next time around, in hopes of increasing the chances that at least 1 would implant - Dr. Klein still suggested that I transfer only 1, but said he was willing to transfer 2, since he understood the logic/reasoning behind my request. Needless to say, that transfer was, thank God, successful. At the end of the day however, had Dr. Klein been adamant about still transferring only 1 embryo, I would have happily went along with it - I completely trust him, and know he has his reasonings for doing things certain ways.

During treatment for egg retrieval it is a lot monitoring and blood work, but Extend Fertility was efficient, so that even when I had to go to work afterwards, it was a huge stress. I only wish that they had an Extend Fertility also in Brooklyn...I saw Dr. Guarnaccia and Dr. Maslow. Dr. Guarnaccia preformed my medical scan as well as my retrieval and Dr. Maslow met with me for my final follow up.

During my visits to Extend Fertility I saw Dr. Guarnaccia and Dr. Maslow. Dr. Guarnaccia preformed my medical scan as well as my retrieval and Dr. Maslow met with me for my final follow up. The process went very seamless and I had all of my questions answered. Dr.Guarnaccia is wonderful. I felt very comfortable under his care, especially for the actual egg retrieval. Following my procedure, I had no pain and the aftercare plan was very well communicated. I met with Dr. Maslow when I first was thinking about starting the process and she was very clear about what I should expect and answered all my questions, she also called in a few days after my retrieval to check in and answer any other questions, which really made me feel like I was in great, caring hands. The staff at Extend Fertility as a whole was amazing. Everyone was so caring and had no problem answering questions. The communication was great during my procedure which made me feel so much more comfortable. Tamar my fertility advisor is amazing. She made me feel much more at ease each step, because she answered all of my questions and was very caring. Molly the nurse I worked with is also amazing. She clearly and thoroughly showed me how to mix and administer my medication. She took her time in explaining and answering any questions. If it was not for her, I do not think I would have been as comfortable administering my medication.

Extend Fertility Strengths: Lovely and caring staff. Good communication. Clean and organized facility. The nurses did a great job with communicating next steps and answering follow up questions. During treatment for egg retrieval it is a lot monitoring and blood work, but Extend Fertility was efficient, so that even when I had to go to work afterwards, it was a huge stress. I only wish that they had an Extend Fertility also in Brooklyn.

I never experienced any anxiety or questioning with Dr. Guarnaccia because anytime something would happen, i.e., unexpected spotting I would email him and get a response back immediately.  I never had to sit there wondering if something was wrong or am I doing the right thing.  I am not an easy patient. Every Dr. visit I  would come in with a list of questions I found on forums or the internet. Before Extend Fertility I was at a very well known, highly ranked fertility clinic where I would call in when I had a question, wait for over 15min on the phone with the receptionist and then be told the Dr will call you back when they get a moment which meant the next day.  However, I felt like I won the lottery with Dr. Guarnaccia (Dr. G). When I shared with my friends how I can email him and get a reply back in less than 10mins they were shocked. Actually, the first time I emailed him and got a quick response I was shocked too. I have never ever experienced that type of stellar service with any other doctor.

I went with Expect Fertility because of Dr. Guarnaccia experience & knowledge, the rates, and the state of the art lab. I didn't go with RMA because they suggested a birth control protocol. I had a short agonist protocol which was closely monitored and had frequent dose adaptions [by Dr. Maslow] and an lupron trigger. The retrieval date was determined based on the monitoring. She did everything to ensure a good numebr and quality of eggs and the results with her were excellent and a lot better than in my previous treatments. She was able to retrieve a large number of eggs, almost all of them were mature.
Cara [nurse at Extend Fertility] was very helpful and well organized. She was always up to date with what was going on with my case and she took care of all issue...[Monitoring appointments are] Very well organized, quick and efficient. Staff is well trained and very kind and respectful... the set up is designed to improve the results. The monitoring is not just for safety but to optimize for the chances of a good outcome. They usually don't schedule the cycle but start based on the natural cycle which improves chances, also the retrieval date is based on the monitoring and therefore flexible, this increases the chance for an optimal result.

My work schedule forced me to come into the clinic very early when they opened at 7am and sometimes I would miss having him scan me. If its important to you to be always seen by the doctor I recommend coming in a little later (8am)

Dr G was always kind and honest during my scans and egg retrieval procedures. As a 40 year old woman with only 1 ovary I had to be realistic about my situation. I really appreciated that though it would be a longer road I was always told that there was still hope.
Describe the protocols Michael Guarnaccia used in your cycles at Extend Fertility and their degree of success.
A year prior I had to have my right ovary removed due to a dermoid cyst. Due to my age I had a diminished ovarian reserve and had to take the maximum amount of hormones to see good results. For my first cycle we did a Follistim and Menopur protocol. But my body didn't respond and the cycle had to be cancelled early on. I did an estrogen patch for the days leading up to day 1 of my cycle and then. Then the next two successful cycles I did a micro-flare protocol where I took Follistim (once a day), Menopur (twice a day) and Lupron. (twice a day) and a Pregnyl trigger. For my third and most successful cycle I took Follistim and Menopur once a day and then also Ganarelyx for about 5 days towards the end of my cycle and was triggered with Pregnyl. Please keep in mind that in order to even do the 3 full cycles and retrievals I had about 3 cancelled cycles.

The nursing staff at the clinic is wonderful. I've been going to Extend for 2 years. The only negative thing I have to say is that as they grow in popularity and become a lot busier small details fall through the cracks a bit in terms of being able to ask follow up questions and phone calls

The clinic is in a great location in midtown near a lot of train lines. The staff is very friendly and know what they are doing. I always felt that I was in good hands (which is important when you have to get blood drawn every other day!) The hours are great for people like myself who have to start work early (I would normally go there for 7am for my monitoring appointments). The only weakness I would say is that as they grow in popularity the waiting area can get a little cramped

The monitoring experience has changed over time. As the business is growing, there are more women in the waiting room, so you should plan to be there for an hour. A pharmacy didn't get the prescription and they never told me which type of trigger shot i needed until i asked. I did call to speak to a nurse after a treatment as i was experiencing vertigo and they were useless with that issue. However, this does not surprise me since it was not directly related to the procedure

Their practice is geared towards women who want to freeze their eggs for the future and not for any immediate treatment, which is what i was doing. It had a smaller feel then an RMI and was less expensive (my insurance did not provide coverage). Everyone is super friendly and nice and wait times are really managable. However, you have to stay organized the whole time as things can fall through the cracks. This will happen at any of the egg retrieval or ivf clinice.

They [Extend Fertility] failed to inform me that the doctor that I worked with for 6 months to freeze my eggs was not going to actually going to do the procedure! A per diem was going to take on my complex case. That was not what I signed up for. I paid this hard-earned money with no help from a loan or an insurance, that is the least I could have expected. ... That is unacceptable. I should have had the heads up to make a sound decision, instead of feeling pressed 10 mins before surgery in my hospital gown.

Dr. Guarnaccia came in as a tourist to perform surgery on me. It was the first time I saw him, ever, he was the per diem who came in to cover for my actual doctor that Saturday. Unlucky me. He asked me if I had any medical concerns. This was the first bad sign. When I said heart disease, history of fibroids, cysts and a clotting disorder, he started scrolling through his stack of papers. I was a little nervous at that point. He also asked me who "Jane Doe" was and I said she works here. I got alarmed then. He proceeded to go to the computer to get acquainted with my record as this was not necessarily going to be a walk in the park as he had expected. I took that moment to asked the nurse who that was. They explained my doc who I was confident in and choose to care for me, was away for Sabbath and this was the per diem who was going to perform the surgery. No one informed me of this ahead of time and I almost felt obliged to accept it. All I could think of as a patient was, I don't even know him, I just meet him and he doesn't seem competent; as well as all the money, injections, time off that it took to get to that day to freeze my eggs. I told myself it will all be fine, although I almost wanted to call my boyfriend who was sitting in the waiting room. However, the anesthesiologist came in, he knew exactly what he was doing and I thought it would be ok. A day after my surgery, I had intense pain at work ( i have a high pain tolerance) and my boss sent me home. I called my actual fertility doctor who said everyone reacts differently, if pain persists come for a visit. As far as the results, I am young with no ferlility issues whatsoever, but from the expected 12 eggs to be frozen, we only got 5. My actual doctor admitted it was very unusual. Unfortunately, the pain came back stronger a few weeks later. Basically, I was rushed to the ER from work - they said I had a cyst. Went home, still in pain for a week so I went to my actual doctor and he prescribed me medications. One more week of pain, I am rushed to the ER from my obgyn's office! She tells me this is bad and that I might lose my ovary. I got emergency surgery as the doctors at the hospital feared me losing my ovary due to an hemorraghic cyst. I was crushed. They said consent to have your ovary removed and I just felt numb at that point. When I woke up from surgery, my boyfriend tells me, you still have both your ovaries, instead I had been carrying a 6-week old infection that spread puss to my ovaries, uterus, abdomen, colon/rectum. You can imagine how I felt, emotionally and physically, all this pain. Infections are extremely rare and this was a very bad one. Because of all of this, my fallopian tubes are scared and I will probably need IVF to conceive as is will not happen spontaneously. I was just freezing my eggs as an option because of my heart and fibroid history. Basically, I have reduced my options when I thought I was being smart. And I wonder if this is what killed half of my eggs at retrieval. When I found this site and saw his reviews, I cried, I knew I had been screwed by this doctor. I had to be out recovering for about 3 months, not working. Ended up having to move upstate due to my precarious situation. My living condition is good but very stressful. I got the flu and to top it all, my second heart attack now. I will stop here but you get the point, my entire life has been hanged for the worst. I am thankful to this site and the other reviewers for sharing their story. I am not one to leave a review ever but I felt like a responsibility for anyone debating Dr. G’s care. With such a low review rate, I wonder why he’s even practicing and those hiring him to still be out there are to do their due diligence and be responsible.

Save yourself a bad ending. In my opinion, he should not be practicing and not hired as a per diem. You would think he is qualified and that you should trust him. I fell victim of an unprepared doctor with overwhelmingly bad reviews. Perhaps he has a calling in another field but not fertility.

Dr. Guarnaccia was the per diem performing my retrieval, the most crucial part of the process. He was not part of that process. I was unaware he would perform the surgery on me. 12 eggs were retrieve and only 5 were frozen. All I know is that this was an unexpected result and that I got infected under his care. Also, had extra anti-coagulant shots to take during the hormone treatment because of my blood disorder.

I do not have any grievances with the nurses, they were all very nice, so was my care coordinator. Actually, the time I came in a week after my first ER visit, 2 of the nurses asked me if I needed help getting dressed. That is how bad my infection was, they felt sorry for me. I am fair and would rate them fairly, according to my experience.

HE [Dr. Klein] took the time to explain everything very well in our intro meeting, while i did not see him a lot during the cycle, he was available if I needed to speak with him and was very nice...He was knowledgable, kind, and was willing to explain every detail and possible outcome. I loved the nursing staff [at Extend Fertility]. They are kind, compassionate, and thorough. Especially the doctors assistants who do the blood work and the vaginal ultra sounds. They gave me as much info as they were able to and were very sweet...Very few things went wrong, but just once or twice felt like we got down to the wire on getting results to adjust medication in time. But it only happened 1 time and otherwise were extremely on top of it.
How was your experience with Joshua Klein at Extend Fertility?
He was knowledgable, kind, and was willing to explain every detail and possible outcome. I appreciated his candor and felt very comfortable speaking with him. And that I was in good care. My procedure went as expected and I felt like he was honest about my odds of retrieving healthy eggs. Which turned out even better than i expected. I was very comfortable in his care.
What's one piece of advice would you give a prospective patient of Joshua Klein at Extend Fertility?
You won't see him too much, but i was not too concerned about that. Overall the clinic sets a style of working with patients that I appreciated.

HE took the time to explain everything very well in our intro meeting, while i did not see him a lot during the cycle, he was available if I needed to speak with him and was very nice.

I love that the clinic is specifically geared towards women freezing eggs and not IVF. Since none of the patience are necessarily ready to have kids, they truly understand the needs of working professional women who are freezing eggs for later. They really kept focused on the fact we are planning for something in the future and likely have very busy lives right now. They are great at scheduling everything in a way that gets you in and out as quickly as possible. There can be personal embarrassment about freezing your eggs and they do such an amazing job of making you feel confident in your decision.

It was well under the rates at other clinics.

Sarah was my fertility advisor and was amazing-I would email her with any questions or concerns and she always responded quickly. I saw Ana the sonographer for most of my ultrasound-she was the best! So caring and informative. All the medical assistants were great- never had someone take my blood with such ease and barely any pain. All the nurses who called with medication information were so helpful with any questions. So thankful for Tania and Angela who went out of their way to help me!

Describe your experience with your monitoring appointments at Extend Fertility.
They were very flexible with appointment times- able to go in really early (7am) when needed to go to work. But was also able to do later around 9 when didn’t have to rush in. Appointments were quick and usually less than 30 minutes. I would get my blood taken then have an ultrasound then would be done.
Describe the costs associated with your care under Joshua Klein at Extend Fertility.
They had plans based on your blood work and age. I paid $8050 for 3 cycles or 15 eggs whichever came first. I then had to pay $450 for storage and for medications. I got lucky and only had to pay for copays since my insurance covered most of the meds. My total medication cost for 2 cycles was $900
