How was your experience with Emily Goulet at Fertility Institute of Hawaii?
Dr. Goulet has been the most wonderful RE I have ever seen. I underwent IVF from 2016-2018 in another city, and again from 2022-2024 at Fertility Institute of Hawaii. I have seen multiple RE physicians over the years and can confidently say that Dr. Goulet is the best physician I have worked with. She is extremely knowledgeable, understanding and kind, willing to spend time with her patients to help them understand each situation, and uses her knowledge and creative thinking to devise the best plan for each individual and their needs. She was gracious when I had concerns and willing to re-think plans or protocols if something wasn't working well. And most of all, she was supportive and optimistic through both good and bad times. I have a long and complicated fertility journey and credit Dr. Goulet with my successful pregnancy after years of unsuccessful IVF attempts.

Dr Goulet spends as much time with patients as they need. She takes the time to explain everything in detail, she is warm and caring, and always happy to answer questions or change plans/ procedures when the patient feels things aren’t working.
Describe the protocols Emily Goulet used in your cycles at Fertility Institute of Hawaii and their degree of success.
We tried a standard natural FET protocol which did not work well enough to get my uterine lining thick enough. Previous doctors simply repeated the same protocol multiple tries but Dr Goulet was willing to completely rethink the protocol and medications, and the result was very successful.
Describe your experience with your nurse at Fertility Institute of Hawaii. (Assigned nurse: Anissa Leehang)
I have been with Anissa, my IVF coordinator, for the past 2.5 years and with 3 different doctors at Fertility Institute of Hawaii. Anissa is the most supportive, optimistic, and knowable provider I have ever had. She is excellent and timely with communication, she is always open to discussion and very knowable with the most detailed questions. Anissa is an asset to FIH and I’d recommend her to my closest friends. If it weren’t for Anissa I probably would have melted down from the anxiety and fears over all the unknowns and all the failures along my journey. But she gave me the support to keep trying until we were finally pregnant!

FIH is very friendly to all people: couples, singles, same sex couples, even international people. It is a welcoming atmosphere. The clinic was completely renovated in 2024 and has excellent informational posters, videos, and patient resources. They even have 2 phlebotomists who come to patient rooms to make it easy on patients. They are fairly straightforward about costs and payments, which is helpful.
Describe the costs associated with your care under Emily Goulet at Fertility Institute of Hawaii.
FET costs were approximately $4000 / cycle and included all ultrasounds and many of the medications. If repeat cycles were needed because of failed transfers, a discount was given for subsequent FET.

Dr. Goulet was compassionate and clear with me from the very first conversation, where she spent more than an hour with us educating us about donor eggs and financial options. I think because she’s been in my shoes, she is extra compassionate towards patients. When we “graduated” she wanted us to keep in touch and bring in our little one to meet her.

Describe the protocols Emily Goulet used in your cycles at Fertility Institute of Hawaii and their degree of success.
I did only donor cycles with Dr. Goulet, so no egg retrievals. She recommended a fresh donor, but we had concerns about ID disclosure and so we chose a frozen donor. She still supported us in this decision. Our first cycle resulted in a negative, and so we chose to get EMMA ERA testing. The second cycle resulted in a chemical, and so she suggested sterioids and lovenox. We got pregnant with our third cycle.
Describe your experience with your nurse at Fertility Institute of Hawaii. (Assigned nurse: Myra)
Myra is so so great. I was a little nervous that they only allow communication over Klara, a messaging portal, but honestly Myra always responded in less than an hour. She was always clear about med schedules and several times when I was fully panicking for various reasons, she calmed me down, helped me get what I need, moved appointments, etc. I could tell she was actually rooting for me the whole time.

Donor egg cycles are about 45k and that includes PGS testing, zymot, and one transfer. Each subsequent transfer was 4k. They gave me a discount on my third transfer.

Dr. Goulet has a warm demeanor and immediately makes you feel comfortable. She is a wealth of information and is invested in each of her patients. I specifically appreciated that Dr. Goulet knows the literature and research and when the classic approach to a frozen embryo transfer wasn't working, she created a protocol unique to my needs that is backed by science. I also underwent procedures outside the typical egg retrieval and embryo transfer that were performed by Dr. Goulet and those other procedures went smoothly and as well as they could have. In fact, those other procedures helped my fertility and I'm so thankful Dr. Goulet informed me about them. There is nothing I would change about the treatment protocol I received from Dr. Goulet. It is also invaluable to me that Dr. Goulet is patient and compassionate and allowed me the space to grieve when setbacks occurred. Knowing that Dr. Goulet remained hopeful for me and wasn’t giving up on me gave me the courage to continue, ultimately leading to the best possible outcome. I can't thank Dr. Goulet enough. She is a blessing to the IVF community and she is truly gifted as a fertility doctor and human.
What's one piece of advice would you give a prospective patient of Emily Goulet at Fertility Institute of Hawaii?
Make an appointment to see Dr. Goulet - you will not regret it. Dr. Goulet was the second IVF doctor I saw. The first was someone from a different clinic and I did two IUIs with this other doctor who has a very good reputation in our IVF community but I felt this other doctor was condescending and was always walking out the door before I could ask any questions. I was nervous to switch doctors after having invested a lot of time and money with the first doctor, but I'm so so thankful I did. From my first appointment with Dr. Goulet it was obvious how different my experience would be with her. It was like night and day. I have never looked back after my first appointment with Dr. Goulet! I hope she never retires, because I plan on going back to Dr. Goulet to grow our family even more. A small note is that my first appointment was a consultation by video, then each subsequent appointment was in person. Even by video, Dr. Goulet greatly exceeded my expectations. I would be open with her about your concerns and she is more than willing to have a dialogue with you about your treatment.

Dr. Goulet is passionate about the work she does and genuinely cares about her patients. She listens and always allows you time to gather your thoughts and ask questions. She is always pleasant to be around and makes sure the patient understands what is happening. Dr. Goulet is always respectful in her communciation and interactions.
Describe the protocols Emily Goulet used in your cycles at Fertility Institute of Hawaii and their degree of success.
In order to qualify for IVF coverage, my insurance requires three IUIs. I did two IUIs with a previous doctor and clinic. I then saw Dr. Goulet who discovered a uterine polyp and removed it. I then did the third IUI with Dr. Goulet (Clomid and Ovidrel trigger shot). Then we moved to IVF and due to my age and my preferences, I chose to do PGT testing (paid extra). We paid extra for Zymot (our choice) and ICSI (strongly recommended). For my age, our total oocytes retrieved and the attrition rate were normal, and in the end we only had one PGT-normal embryo. Knowing that one embryo does not always equal one baby, I opted to do a second egg retrieval with same protocol and got two PGT-normal embryos. We then did one unsuccessful frozen embryo transfer - my body responded well to the standard FET meds, my lining was beautiful, it just didn't take. It took several attempts at a second FET, but my lining wasn't getting there and I decided to take a break for a few months - which Dr. Goulet fully supported. When I came back she informed me of what looked like a uterine fibroid and explained my options. I chose to go forward with the transfer. The transfer took but later ended in a miscarriage. Dr. Goulet then discussed again my option of having surgery which I opted for this time. The surgery was successful and she also discovered adenomyosis which I had never heard of. It was a scary diagnosis but in the end it was helpful to know and I believe the surgery helped tremendously. I healed well and the surgery helped my menstrual cycle. After healing, we attempted a third FET which took a few attempts to get there due to lining issues. I'm glad Dr. Goulet was patient with me. We added Prednisone and Lovenox and did a semi-natural cycle and it worked! I appreciate that Dr. Goulet reviewed my chart and asked me about my experiences and she created a protocol that worked for me. It's been a journey, but looking back, there's nothing I could have changed or done differently, because I didn't know how my body would react to each protocol. Also, each cycle is different and what worked for me for the first FET did not work for the second.

Tiare is very upbeat and a positive person. She is always rooting for us and answers all of my questions and responds to my messages on their messaging platform. Every single staff I came across at FIH (from the front desk, to the phlebotomists, to the other nurses and doctors) are all so friendly and respectful.

The clinic is run very well, especially for how busy it is. Most times the waiting area is full, yet the staff is always friendly and pleasant. The clinic is pretty efficient at how they run things. Of course because it is a very busy clinic with many patients, you cannot expect to be in and out in a flash. I think the clinic does a very good job of staying on schedule and communicating. I don't have any suggestions on how they can improve anything. I'm happy with how things are run.

My insurance covered part of the first IVF cycle and because we opted for some extras (PGT, ZyMot, ICSI) the first IVF cycle was under $9k (it includes one frozen embryo transfer). The meds for the first IVF cycle and each subsequent was under $3k. The second through fourth IVF cycles were out of pocket and with extras (PGT, ZyMot, ICSI) they were each around $20k (this is just by memory, not exact) (each IVF cycle includes one FET). I received a small discount for the third and fourth IVF cycles which I was grateful for.

I do wish she or her team would have given me a little more information throughout the process. I sensed she was seeing a very large amount of patients each day and so even though I saw her at each visit it was always for less than 10 minutes. Because of how busy this clinic is I got the feeling that they take for granted that this is something they do every day and so it can feel like they are very casual about it all, when for the patient it is usually a very big deal and so I wish there was a little more emphasis on holding the weight of these visits in a more serious manner. On one hand the casualness of it all made me feel like I had nothing to worry about, but I also caught myself putting on a brave so that I didn’t take up more of the doctors time.

Ask as many questions as you need to and don’t feel pressed for time. You are paying for the time to gather all the information you need to.

Overall the doctor was kind and answered my questions when I had them but the visits were quick, there were always quite a lot of people in the office coming and going, and I would have liked more details regarding my progress at each visit and also what to expect next. When you’ve never been through an egg retrieval process before you don’t know all the questions to ask or what it even is that may have been helpful to know, but now that I’ve been through it I can think of several things that would have been nice for my doctor and care team to explain to me in more detail to have achieved a higher level of care.

The care team was disappointing, continually got my information wrong leading up to the procedure (ie I was traveling from another island to do it and she tried several times to make appointments for me having forgotten that I don’t even live there). She also wasn’t able to answer some basic health/science questions such as, why exactly it was important to avoid taking aspirin during the process. She also told me something completely wrong about how the egg retrieval process works.

Busy clinic with lots of patients but clean and efficient once you get your appointment started. Parking is easy. Staff are all very kind. Being a patient of Anatte Karmon has made my hopes go higher. I felt more encouraged as her patient because of the sincere love kindness and patience she shows towards me and my partner. To continue to trudt the plan, follow all instructions but also do extra on what your told to keep up with. To never lose hope because some doctors dont care and some like Anatte Karmon does. I get more anxious to see her then stress about anything i feel might not happen.

Anatte Karmon has been the sweetest doctor ever. She opens all options for you but makes sure you take the route thats best for you based on your bodies needs. She makes me feel so comfortable and has never discouraged me through any results i did not want to hear after trying. Its expensive just to purchase sperm vials just for it to fail many times. Ive had to put things on hold all becausz i didnt have enough money to continue. Stress and worries for in between allowing me to go e.r since it caused problems within my stomach due to stress. I packed on the extra weight due to drinking becausz i gave up, i really wanted a baby and to start my family as were the aunties that supports everyone who has babies around us. And because we picture all we would buy for our baby one day, every baby we blessed had good gifts from us. Its to a point i hope being blessed can happen from all weve blessed other children and babies growing up.
Describe the protocols Anatte Karmon used in your cycles at Fertility Institute of Hawaii and their degree of success.
Anatte Karmon has first consulted with me on what steps to take first. Upon consulting she then moved on to step 1 that was an option for me to take, she encouraged that every women sometimes will go through the entire process just to possibly find their tubes was blocked. So its an extra procedure to do just to be safe that it wont be the cause of any egg being stopped from traveling. Its called an HSG OR SIS. Basically a procedure involving an xray to show dye that they shoot into your uterus to see how far it travels. As long as it goes all the way through, it shows that your tubes and lining are perfectly clear to go. It relieves the stress from future news you dont want to hear. After that step and getting all information about your cycle how ofteb it is blood work to show levels of where your at for your menstrual. You can now move foward to your next step which is waiting till your menstrual shows. If you dont get your menstrual at the time your supposed to from when Dr karmon had asked of you for your last dated cycle, she then had helped me take pills to start my menstrual so i can now start my process to growing my follicles. Day 1 i called Dr Karmon to notify her that i started my menstrual. She then brought me in on day 3 which is when the ultrasound can now detect growth size of follicle (like a egg shell) . I was giving stims which is a liquid you refrigerate and put into a pen that you can shoot in the left or right side of your stomach which is a more effective way to boost chances of egg growth and pregnancy. Because im diagnosed with PCOS my eggs grow amot slower which then took me almost 2 weeks to get to a normal 16-88 mm in size for full growth or mature eggs. After reaching the size she needed me to get to , she now can give me another shot to help increase ovulation (follicles releasing eggs to be fertilized by sperm) 2 days counting after that Dr Karmon could do insemination day to finally see if the sperm will fertilize my egg doing iui treatment! I also ordered my sperm vials from Seattle spermbank who is by far the best since you pay a flat rate of $50 to view unlimited of every info possible for each donor. They also extended their genetic testing on top of everything the sperm donors has to test for to be a safe donor. So the test extends to 100+ more screening they can detect for anything donor carries. It makes the search so much more easier, they also work with fertility institute of hawaii as well so shipment is a piece of cake! All in aml even if it failed, it happens and we have to keep our hopes up that there is help out there and praying that even help financially will make the stress easier and may help the body to decrease stress opening a window for a positive pregnancy test.

When i walk in i dont feel stood out for a problem i have, it makes mz feel okay to know everyone in the office is aware but its normal to feel the way we feel. They never make you feel like it doesnt matter and that they go through this job daily. They show that they truly care about their job and to adress people concerns in theur fertility struggles each day.

Ive honestly have never seen or felt or know any weaknesses in this clinic. They have been nothing but over exceeding the care they are expected to give already. The strengths of this clinic is being able to handle so many patients at once becauqe timing is so important in multiple different fertility issues that each individual patient faces. They are welcoming they continue to show excitement for every patient well-being and fertility struggle.

The cost wasnt to bad at all for all the services i was given. I was allowed to pay small portiond at a time. They never rushed me or called me constantly to harrass me for payment. They never did call for payment at all. They were very understanding and if i needed any info on balance i could just give them a call.

But i do hope that this cycle works so i can give other people a chance and getti g any financial help out there they need. I do need the help but this last cycle i pray it goes through. If not i will gladly need the help as i also just relocated since owners had to sell our home we were renting. My hours got cut at my job as well.

Dr. Frattarelli and his staff are always so welcoming and warm at every visit. He does his best to ensure that I understand what is happening and the options we have. He takes his time talk consulting with us about our treatment plan and the results of the different tests.
Describe the protocols John Frattarelli used in your cycles at Fertility Institute of Hawaii and their degree of success.
I don't recall exactly what protocol I was on. What I do recall is taking that max dose recommended during the stimulation process. I was also give Growth Hormone sooner in my second IVF cycle to hopefully help boost my egg quantity and quality. We also did Zymot the second cycle and added in Femara I believe. All medications recommended were to support ideal stimulation results. During our first cycle 9 eggs were retrieved. 8 were mature and fertilized. 4 made it to the blastocyst stage. 2 were genetically normal. Both embryos were unsuccessful transfers. Our second IVF round resulted in 4 eggs. 1 embryo. We decided not to genetically test. Our transfer protocol had me on extended does of birth control to combat possible endometriosis and he added a vaginal probiotic. We had a successful transfer but unfortunately lost the pregnancy at 8 weeks. We are gearing up for our third cycle. We are hoping to get additional testing done to determine any endometrial infections.

I absolutely love our care coordinator. She is always answering my questions and reassuring me when I'm feeling overwhelmed or stressed out. She very thorough when providing information and directions. Due to the clinic being on another island I am constantly calling and emailing with questions and clarifications to ensure I am doing what I should be. Taylor is warm and welcoming on the phone, in emails and in person. She is great at relaying information to Dr. Frattarelli and getting answers back to us in a timely manner.

My coordinator Aki was sweetest and considerate person I’ve ever met. Since day 1 she stands by my side, give me all the explanation I need, be there for me. Since I’m single I have a friend who help me to being sperm donor, I did all the treatment by myself. But I never felt uncomfortable or worried or nervous. It’s because of Aki, she talks to me, email me all the time, she responds right away. Even on the surgery day, I couldn’t get Uber somehow my account wasn’t working, I was so upset almost crying and called Aki. She was really professional she got me a cab in 10mins, told me it’s ok to be late for my appointment. She is always so kind, being just like my sister or mom(I mean she is much younger than that!) . Without her I wouldn’t go through this experience. Definitely second time, if she wasn’t there I wouldn’t have done that. I’m appreciate how she is, and be there for me. I actually miss her so much. Thank you so much for being excellent coordinator for me. Because of you, I had an awesome experience that I’d never forget.

I did consultation with him at his office. First we tried timed intercourse using an app for fertility. Afterwards I was tested for Tubal blockage and that result was that my right tube is blocked. With that information I scheduled and underwent a surgery to look at my tubes and only one was blocked that was a repairable tubal blockage and that result was that my right tube is blocked. With that information I scheduled and underwent a surgery to look at my tubes and only one was blocked that was not repairable, so it was severed and now I only have one presumably working tube. Subsequently after the surgery, I stopped getting my periods which was concerning, but Dr. Frattarellis said that was most likely my PCOS that was affecting my cycles. That resulted in my need to take ovulation prescriptions for each IUI. So far, I have not conceived.
