First Urology

My wife and I are (at this time) TTC through IUI treatments. We had a positive test that led to a miscarriage in the Spring of 2023. At the time, my post-wash motile sperm count was at 6 million, and the clinic mentioned the potential of seeing Dr. Schrepferman if my counts continued to come in low. A previous sperm analysis had shown me on the lower end of the normal range. Once my wife and I started trying again, my number came in even lower at 4 million. I made the decision to reach out to Dr. Schrepferman, as after TTC for over 2 years, I didn't want to wait if there was an issue. During my visit, I was pleased to find out that Dr. Schrepferman and Dr. Hunter (of Kentucky Fertility Institute) have a close working relationship, even though they are at different clinics. Their frequent conferences discuss patients going through IVF so everyone is on the same page about treatment plans. My appointment with Dr. Schrepferman is one of the best doctor experiences I have ever had. I say this as someone who suffers from high anxiety about any sort of medical visit. I was impressed that every part of the patient/doctor interaction felt thought through, from the tone of voice to the language used to make sure I felt comfortable. It was obvious that he wants to minimize the negative stigmas men feel about infertility as a part of his assessment and treatment. My visit included a physical, which he dealt with with care. We quickly put together a plan for the near future, including adding supplements to my already existent routine, a hormone panel to check if any issues were present, and a follow-up after our next IUI to discuss new numbers. When my numbers came back much higher than I had had previously, we quickly decided that this was something to monitor to look for trends as I may just be someone who swings in my count month to month.

Like so many parts of infertility treatment: don't wait. There will likely be a waiting period to be seen as he is good at what he does. I had to wait around 3-4 weeks for my initial appointment. Don't let the fear of what you may have to face hold you back. Make the appointment so you can be seen ASAP, as you will be in good hands.

Dr. Schrepferman started with basic things we could do right away, including the recommendation to take FertilAid supplements (I was already taking Vitamin C & COQ10, but this rounded out what I needed) and a hormone panel to ensure my numbers were in normal ranges (I am considered obese). When the hormone panel came back normal, and after getting the latest post-wash analysis with the IUI that took place not too long after my appointment, he recommended taking a wait-and-see approach moving forward. He explained that men can have big swings in their counts (my post-wash count went from 4 million in September to 67 million in October), and if a pattern of counts lower than 10 million happened over several consecutive months, we could discuss a new strategy to our approach.

Unfortunately, my insurance did not cover this visit. The cost was $150.00. At this time, I have not received a bill for the telehealth follow-up.

Dr. Shep speaks to you like a human, makes connections through common interests. He also provides other alternatives to the expensive treatment we are seeking. But once we made it clear we are going to continue on the set path, his was behind us. He was candid about statistics and what chances he thinks we have. He also provides office hours in another city closer to us, will do follow ups via Skype, which is nice.

He is very busy so take whatever appointment they have.

My husband was put on clomid 50 mg a day to up his testosterone. He will be undergoing a Micro TSEE procedure in July to hopefully correct the male infertility factor.

We had to pay a deposit up front of $100 and have not received a bill for any other services from the initial meeting. We have to pay out of pocket for all the surgery costs which will be upwards of $7200, but worth it!

As a patient, he comes in the room with a smile and a firm hand shake. He went over EVERY minute detail of our fertility history and immediately had ideas and courses of action for treatment and we have gone from there. He is very thorough and makes sure that my husband and I know exactly what’s he’s talking about. He lays everything out for us in a strategic manner. He keeps ups updated and in the loop on bloodwork results and different courses of treatment and holistic pathways to go as well. You can tell he very patient oriented compared to some physician who treat you like a number. Could not recommend anyone better in this area.

Go into your appointment with him with an open mind and open heart. He’s a very warm person, not cold or standoffish. Have a list of questions and he will most certainly answer every one of them even if you feel like it’s a dumb question. Do not be nervous at all.

Our first line of treatment was a supplement to get my husbands sperm count up naturally. Our next line/course of treatment was blood work and sending me to another physician who focused on female infertility (he focuses on male infertility) then he sent us to genetics as well to test his chromosomes while my fertility specialist did the same for me. I have a history of 9 miscarriages, 4 with my husband. And Dr.Shrepferman wanted cover every basis possible to try to uncover and explain these losses. We have now seen genetics and my fertility specialist and my husband is on clomid and I am regulating my cycles due to pcos and we’re are discussing ivf in the near future.

The blood work and testing phase was pretty straight forward. Some things our insurance covers and some didn’t and we’ve made arrangements to cover them.

Very compassionate! Explained the up coming treatment plan and process very well! He gave us the facts but also included hope/honesty! We felt at ease and the process so far has been very clear and easy.

Do it! Make the appointment! Bring your questions because he will answer them! He will value your time!!!

My husband and I are being treated for male factory infertility. My husband is currently being treated with an oral medication to improve testosterone levels. We will have repeat labs in a few weeks in hopes of moving forward with a sperm retrieval procedure (TESE) with good results we can move on to IVF!!!

Our insurance was billed for our co-pay. They do require if not listed on your insurance card to pay up front 100.00 fee for consult. Labs etc billed to insurance first. Our upcoming procedure for my husband will be around 5900.00
