I know this is unknown and unwanted territory so it is hard to have trust, especially when treatments fail. However, rest assured that Dr. Smikle and his team know exactly what they are doing to help you become a parent. Dr. Smikle is in the business of good outcomes - he wants this for you too, so just trust in him.

I had two cycles that didn't go to transfer due to the lining not reaching an ideal measurement. While this was hard, I appreciated that Dr. Smikle didn't allow it me to go to transfer because it would have failed and I would've subsequently lost 2 embryos (children). During the third cycle he performed endometriosis surgery and 2 PRP injections. The lining increased, but still wasn't perfect. The third cycle went to transfer and failed. The fourth cycle we did another round of PRP injections and then we did trigger shots - that were used during the egg retrieval process. The lining measured the same as the 3rd cycle, BUT it worked and I am now pregnant!

Leslie was my biggest fan throughout my IVF journey and she displayed so much tenderness along the way. I can’t even count the number of scans she performed for me throughout this past year, and until recently, the scans were always less than ideal. But, she had a gentle way of breaking the news to me that made me feel cared for in such times of need.
Describe your experience with Laurel Fertility Care.
The clinic and staff was always rooting for me. They are like a cheer squad and it is lovely. They all work very well together and are a solid team. I attended the Modesto clinic AND I live an hour away. The one thing that was challenging for me was that they only did appointments in the morning (8:30 -12). Thank goodness my work schedule was flexible, but this is something you need to keep in mind. Lastly, make sure that you USE your "patient portal." They don't always answer their phones which can be a little frustrating but I learned to use my portal and found it easier to convey what I needed better in writing anyways. Additionally, they're super responsive with the portal messages too - more so than the phone. That's the only thing I think you should know. And again - remember that they want this for you too. Just trust them and don't be afraid to ask as many questions as you need as it can be overwhelming at times. They won't make you feel stupid.

As of right now, they have only one Doctor in the office because Dr. Ekpo is no longer at Laurel Fertility. I was saddened because she left and was not able to do my embryo transfer. Dr. Smikle did my transfer and it was a success with my only embryo!

After being frustrated with OBGYNs that were the absolute worst, it was a breath of fresh air finding someone who understood what we were going through, kind, compassionate, and answered all of our questions. They were available 24/7, they were prompt, efficient, and didn't waste a single second with us.
They actually understood when I said we wanted at least 6 kids and wanted to start our big family right away, that we had tried unsuccessfully for a year and wanted something done. The other doctors we tried said come back in a year and sent us away without bothering to run tests.
Dr. Smikle immediately started us on an IVF cycle, ran tests to see what was going on, and got us successfully pregnant on the first try within a couple months.

I was disappointed at first that they went with single transfer rather than multiple embryo transfer, but at the same time, I appreciate that they put the health of me and my baby first. Their reasoning for this was that I am still considered young and we do not need to do multiple transfers at this time.
Dr Colin Smikle barely talked to me during my consultation. I did appreciate that he gave room for a new very nice and caring Nurse Practitioner to practice nextside him to lead, but I didn't feel like Dr Smikle was very alert, nor did he speak or provide follicle count in the room which makes a patient nervous. I believe he's experiences, but his care does not feel personal or warm.

Dr Collin Smikle is excperiences, but ultimately I didn't particularly like his communication style or bedside manner. He is the quiet type. He also falsely told me that some of my homone levels were elevated, which made me worry. Ultimately, the lab was not done on the right day, and the E2 number confirmed the lab result wasn't accurate. I only found this out after going to another clinic with a doctor to reassure me, explain, and take repeat labs (which were normal). I felt like Dr Smikle wanted to sell me on a big promise on number of eggs and create urgency based on the off labs.

Be prepared to lean on nurses and other staff for warmth if that's what you need. Ask Dr. Smikle detailed questions as well as follow-up questions if you feel you aren't getting enough information.

Dr. Smikle consulted me on his treatment plan with lab work that was not accurate and that other doctors debunked quickly through logic and repeat labs.
Dr. Ekpo recalled conversations we had months prior. I'm assuming her now-taking is top-notch, but it felt natural and very significant...She is not a saccharine-sweet practitioner, but she is respectful, candid, and kind. And incredible good at her job. I wouldn't want to get treatment from anyone else...I underwent a FET with donor embryos add a 'first stop' on me conception journey. There was a mock transfer / saline sonogram. The transfer cycle protocol was BC pills, followed by estradiol + PIO. Very traditional FET protocol, and it worked the first time...
The patient portal / messaging system is clunky. The billing system is not great, however when I was billed erroneously, the was immediately fixed after calling the office. The staff is definitely competent when it comes to LGBTQ fertility, though the vibe was more accommodating than affirming, if that makes sense. MOST IMPORTANT: Dr. Ekpo is no longer with this clinic. While I did have a good experience overall, the most important part of my care was Dr. Ekpo, and I'm not sure if I will come back to this clinic for future care knowing I wouldn't be able to see her...

I underwent a FET with donor embryos add a 'first stop' on me conception journey. There was a mock transfer / saline sonogram. The transfer cycle protocol was BC pills, followed by estradiol + PIO. Very traditional FET protocol, and it worked the first time.

The nursing staff was great! It seemed like I saw a different nurse every visit, but they all actually read the chart, so NBD.

The patient portal / messaging system is clunky. The billing system is not great, however when I was billed erroneously, the was immediately fixed after calling the office. The staff is definitely competent when it comes to LGBTQ fertility, though the vibe was more accommodating than affirming, if that makes sense.
MOST IMPORTANT: Dr. Ekpo is no longer with this clinic. While I did have a good experience overall, the most important part of my care was Dr. Ekpo, and I'm not sure if I will come back to this clinic for future care knowing I wouldn't be able to see her.

I had 1 procedure with progesterone suppositories and estrace and FET transfer which resulted in a chemical pregnancy due to a variety of unforeseen problems ( a degenerative fibroid and fluid in the uterus). Then after we stablized the fibroid and I had surgery to remove my fallopian tubes, I had a stimulated transfer with Gonal-F, PIO and Estrace. The FET failed and we are trying to figureout the reason.

After moving to San Francisco from Europe I was so new to the US health system and I was scared to trust any fertility doctor. I didn't know where to go but having read good reviews about Doc Ekpo online, we decided to give it a try. We ended up spending 20 months under her care.
During the first visit, I explained my situation, already assessed in Europe (Premature Ovarian Failure) and that our first IVF didn't produce a single embryo to transfer. I told her that many doctors in UK recommended me to go for donor eggs. She carefully listened and set up 2 treatments with my own eggs we could discuss. Unfortunately after a couple of ultrasounds it turned I had a polyp so before doing any ivf we had to remove it. I was operated by Doc Ekpo herself at CPMC some months after. She was excellent in her job (removed 2 polyps), no post operational issues, pains or bleeding, and very sweet looking after me and my worries. Our journey was just started though. After some months, Doc Ekpo was back to work and we tried again the treatment. This time 3 eggs and 2 embryos. We were so happy but during the transfer she realized that there was a "false passage" in my uterus that was sending the catheter the wrong way. So she honestly told me she would make a disservice as a doctor if she used that false passage for the embryos. She asked me to be still more patient as we needed to do another diagnostic test, something similar to a hysterosalpinogram (don't remember) and with that Doc Ekpo managed to assess carefully my situation for the next transfer. We transferred 2 frozen embryos in late January 2019, but again it didn't work.
I started to feel very sad and hopeless at that time (4 failed ivf in total since 2017). We had a very honest chat where she told me that my case was a bit of a lottery, and that we knew we were doing the right treatment but my number will always be quite low doing to my amh at 0.4 (I'm 36). Being the numbers low, we were never able to reach the blasto stage at the lab because it was safer to transfer the embryos after 1-2 days. So we were never able to test the quality of the embryos. She started mentioning donor eggs, we also have discussed it when we first met, and went a lot into detail, explaining us how it works, the chances, the procedures. I took 5 months break from IVF to enjoy life and think about donor egg. We then made up our mind and tried it. We bought 8 eggs of which, 6 thawed well and 4 fertilized. 2 were transferred and one is still inside me, after 21 weeks :). Still I had to face the scare of a subchorionic haematoma during week 6 and 7, while we were still under Doc Ekpo's care. I had two very scary episodes of bleeding. We were supposed to travel overseas around that time and she strongly recommended not doing it. We listened to her advice and continued being visited every week (!) by Doc Ekpo until the haematoma luckily resolved and she happily told us: "my job is now done, I wish you all the best"!
I can't be more thankful to have met her, not only because we managed to get pregnant but because she was always a professional and kind human being, really caring about our case through all the 18 months we spent under her care. After some months of being with her, mostly after the polyps operation, I really started to trust her advice and recommendations, they were genuine and professional.
What's one piece of advice would you give a prospective patient of Geraldine Ekpo at Laurel Fertility Care?
As every doctor, she may seem quite cold at the beginning, but she actually is a professional and wants to listen carefully to assess your case. She's been very good managing my situation and after the first 2/3 appointments she started showing her genuine care and empathy.

We spent 20 months under her care. She gain our trust pretty quickly showing genuine attention to our case and compassion to our feelings. I guess it can happen, he was probably very busy without Doc Ekpo around but as an ivf patient, that's the worst wait ever...!
Dr. Ekpo was a straight shooter, but kind and warm. She was very knowledgeable and always took time to talk to me at every appointment. I never felt like I was not heard.... We did 1 IUI just to see. We went more aggressive afterwards. Secondary infertility is quite challenging. I had 1 egg retrieval and got 9 eggs. 6 became 5 day blastocysts. I had 3 failed embryo transfers. Our last talk, Dr. Ekpo said she would change the medications and protocol. All 3 were antagonist protocols.Dr. Ekpo highly recommend PGS. I was not told the correct billing information on that, so we decided against it. The embryologists said it was okay. Who knows.... I say to also ask for embryo grades. I never go them, wish I had.
The only problem I had with Laurel, was the billing. I would get bills 3 months later, which was frustrating. I was told it was because of my insurance company. That was not the case. I would call United Healthcare, and they would have already received the bills and paid Laurel. I just don't think Laurel Billing Department was organized and on top of their work. Nothing- she is a great Doctor. Although I did not get pregnant with my IUI or my 3 rounds of transfer I would still recommend her. I would not have left Laurel, but United Healthcare ended the contract with Laurel. I think best thing to do for choosing any clinic, is get referrals AND check SART reports. Ask the Doctors at the clinics to go over theirs with you.

Yes we were fully informed when we needed to make a decision on the number of embryos to transfer, we all were in agreement to transfer one embryo. We did PGS on our embryos before any transfer was done.
Dr. Ekpo was very thoughtful, considerate and supportive through my IVF procedures. She was very hands on with me. I would encourage others to work with her even though my two egg retrievals were unsuccessful.... Despite the unfavorable results of my egg retrievals, she really tried to be accessible for any questions or concerns I had. She really does care about her patients despite the outcome.... In my situation, I was given much more positive information and feedback in the beginning because I had gotten pregnant naturally without medical intervention and based off of my initial blood work, I can understand why Dr. Ekpo felt more positively about my potential to have a successful IVF procedure. I just wish that someone told me that if might not work the first or second time around, that IVF sometimes take more than once or twice until it works.

Dr. Ekpo was comfortable with transferring up to 2 to 3 embryos because of my age.
At first I really liked Dr. Ekpo. She was blunt but kind, and seemed knowledgeable about the the entire fertility field.... I do feel that she mismanaged my expectations... My ultimate dislike and distrust came after my 2nd and 3rd IUI with Laurel Fertility.... My first cycle they measured my lining at around 3.5 mm, but still went forward with the IUI.... I feel it was morally irresponsible for them to perform 3 IUI's on me when my lining was less than 5 each time
The office [Laurel Fertility Care] is small and outdated, the exam rooms could barely fit me and my husband in it together making us feel cramped and uncomfortable.... $490 PER ULTRASOUND! And during all my cycles with Dr. Ekpo, I needed 4-6 ultrasounds before the IUI.... Nurses were lovely and kind people. I had a couple issues with them not updating my calendar in a way that worked for me, but otherwise they returned calls, answered questions, and were very pleasant to be around.

At first I really liked Dr. Ekpo. She was blunt but kind, and seemed knowledgeable about the the entire fertility field. My ultimate dislike and distrust came after my 2nd and 3rd IUI with Laurel Fertility. One thing off the bat, the online calendar and app is just terrible. Very difficult to access and with all the medications and timing needs, this was frustrating.
My first cycle they measured my lining at around 3.5 mm, but still went forward with the IUI. At the time, I just put my blind trust in the doctors and figured they would do what they thought would be most successful-NOT TRUE. Any other Dr. will tell you that you want your lining to get to 8mm to have a decent shot at pregnancy, and I wasn't even halfway there. I feel it was morally irresponsible for them to perform 3 IUI's on me when my lining was less than 5 each time. Also, my cycles required LOTS of monitoring ultrasound appts, and at $490 per appt, this became ridiculously expensive. These utlrasound appts were 5-10 minutes, for $490!!! insane. They didn't make any attempts to get my insurance to cover it as a diagnostic tool, so I had to pay out of pocket for everything. Dr. Ekpo also straight up lied to me about a certain procedure which is what ultimately makes me feel I would NEVER recommend anyone go there. She told me there was no way to do a HSG dye test in the same cycle as an IUI, and it just isn't true. Many other doctors do this and I believe the only reason she told me I couldn't was to get more money and more visits out of me.

Do your research and don't go with a private clinic because you think you'll receive better care, it just isn't true.

Dr Ekpo was kind and I didn't have an issue with her personality or her demeanor during our visits. I do feel that she mismanaged my expectations and given what I know now, there is no way she was ever going to get me pregnant so it really feels like it was more of a cash grab, preying on my desire to have a child, while not addressing the reality that my lining was not even close to thick enough to move forward with the IUI's she performed on me. It gave me a lot of false hope without addressing diagnostic issues that they completely skipped over.

Each IUI cycle I was on Letrazole, with lots of monitoring ultrasounds to see how my follicles and uterine lining were developing. The first two were about the same-letrazole, trigger shot, IUI. The third one they added estrogen injections mid cycle to try and boost my lining since I never got past 4mm with previous two cycles, then another trigger shot and another failed IUI. I wasn't given any other alternatives except for IVF which they began pushing on me after the first failed IUI.

Nurses were lovely and kind people. I had a couple issues with them not updating my calendar in a way that worked for me, but otherwise they returned calls, answered questions, and were very pleasant to be around.

The office is small and outdated, the exam rooms could barely fit me and my husband in it together making us feel cramped and uncomfortable. And while this isn't the clinics fault, the building was under massive construction which made parking impossible and getting into the building a real pain.

$490 PER ULTRASOUND! And during all my cycles with Dr. Ekpo, I needed 4-6 ultrasounds before the IUI. It was absurdly expensive.

My nurse is the person who help and walk me through make sure all your blood test and ultrasound appointments were done. She work around my schedule to make thing convenient for me. A few time I came in before the clinic open. She was my first port of call if anything when wrong while I was undergoing treatment as far as like breaking a vial of medicine, or having unusual pain when I shouldn’t. She offer solace and advice when unusual things happen during the treatment process.

We had a hiccup with respect to insurance coverage of some of our medications as well as PGS. Luckily, it all got sorted out as quickly as possible by the clinic. However, we were blindsided by the cost of PGS because we had been told "everything" would be covered.
Dr. Smikle is amazing. While he is clearly very intelligent, he also has a great bedside manner and explains things in a way that is easy for the layman to understand. When my treatment was done and I had to switch back to my regular OBGYN i was actually sad.... When the first round of IVF didn't work, I felt like everyone involved cared just as much as we did...and the thing I loved most was, after they comforted me, we all went straight back into planning mode. Dr. Smikle wanted to try something a little different, just so we didn't keep doing the same thing with the same result,

I was referred to Dr. Smikle by my OBGYN after an ultrasound revealed that I had endometriosis that was potentially affecting my fertility. My husband and I had been trying to conceive naturally for about a year with no luck and, while not quite desperate, we were definitely looking for a different and hopefully more successful approach. I actually remember how comforted I felt the first time Dr. Smikle shook my hand...I just knew I was in the right place. At my initial consultation, after the exam, he worked with me to outline a plan on how to proceed. I wasn't quite ready to try IVF, so we started with Chlomid and IUI, but with the knowledge that I would likely have to try IVF if we wanted to have more than one child. After 3 unsuccessful attempts with IUI, we decided to switch to IVF, which is an intimate, intricate and sometimes very difficult process. The whole time, Dr. Smikle, Donna my nurse, and the rest of the staff were always available to answer any questions, fill my prescriptions with the lowest cost possible options, and even give me a shot or 2 when I was initially scared to do it myself. In short, I felt like they treated my like they would a good friend...not just a patient. When the first round of IVF didn't work, I felt like everyone involved cared just as much as we did...and the thing I loved most was, after they comforted me, we all went straight back into planning mode. Dr. Smikle wanted to try something a little different, just so we didn't keep doing the same thing with the same result, and, when my second round of worked, I felt like the whole office was cheering for us. They even asked to see the ultrasound pictures...which I'm sure they have seen a million tiny gray "kidney beans" on a piece of paper...but when it's your "gray kidney bean" it's pretty special.
In short, my experience at Laurel Fertility was exceptional..The service was incredibly personalized and professional and Dr. Smikle and the entire office staff were always available (on the first ring of the phone) to answer any questions I had...I look froward to going back for baby number 2!

3 IUI - after 2 negative results he recommended we switch to IVF because he didn't think another IUI would be successful but I asked for one more since I already had enough medicine to move forward.
2 IVF... one with progesterone and estrogen injections and one without. After the first round didn't work, he suggested trying something different. We essentially "tricked" my body into thinking it got pregnant naturally. and it worked

Dr. Ekpo always went over my options with me and we made decisions together, she always made me feel comfortable/reassured. I never had to wait long to see her, she wouldn't rush me out and always spent an appropriate amount of time with me. She would see me on the weekend when necessary and give me a heads up on who would be her coverage if she was going to be away.

I have PCOS - irregular/no periods - I went through several unsuccessful cycles of IUI using Letrozole and Ovidrel. Then we added Gonal-F in with an increased dosage of Letrozole but it made too many follicles so we switched mid-cycle to IVF and I had to add more Gonal F and Ganirelix and HCG. Then I had my egg retrieval. Then I was on Estrace and Endometrin before my transfer through 8 weeks into my pregnancy then kept taking the Endometrin up until 10 weeks pregnant.

I went in for monitoring according to my schedule and the process was quick and efficient. Dr. Ekpo handled all of my monitoring and ultrasounds, and we debriefed after each appointment. My meetings with Dr. Ekpo were followed with check-ins with the nurses about the protocol/medicines/etc. and then someone would call me with the blood test results. Everything was very well-organized.

Based on egg yield success at my previous IVF clinic, Dr. Smikle tweaked and added to the estrogen priming protocol. Dr. Smikle added human growth hormone in an effort to enhance egg quality in addition to the potential higher yield. Both egg retrieval rounds did not yield as many eggs as the past, however, more embryos made it further than before.

My primary nurse and coordinator were always available by phone or through the patient portal. They were always very patient and careful to explain the process step by step. Blood draw, vitals and weight were always performed in a private room rather than in a common area and all attention was focused on the patient.

Laurel SF has all the strengths of a larger clinic while getting much more intimate care. Egg retrievals and transfers are done in the clinic and the laboratory is on-site. They are much more progressive with pre-cycle preparation than other clinics. They do recommend supplements and lifestyle changes in preparation for cycles which is different than other conventional IVF clinics. They are also open to working with acupuncturists especially before and after transfer. I think that all clinics can use improvement in the area of working with integrative medicine practitioners to improve the general health of the patients and with success rates, but it was a comfort to know that Laurel was open to this course of side-by-side treatment.
This clinic works with health insurance which is a plus for decreasing stress on the finance side of things.
Although, we were not successful with Laurel, our overall experience was very good. There is no regret that we might have had better success at a different facility.

Laurel is a small clinic that gives you personal attention. You feel like a human going through a difficult time and not just a number designed to give the doctors more money. Laurel's staff was the only one I've had who provided a detailed printout of what my insurance would cover. As a result, I had no problems with prior authorization. Given that my subsequent clinics have all had hiccups at the prior authorization phases for IVF, this says something about how thorough Laurel's staff is.

IUI with Letrozole, with an Ovidrel trigger. Negative pregnancy test, but this was to be expected with our diagnosis. IUI was basically a required step for our insurance to cover IVF, so we did not expect a pregnancy.

Laurel is a small clinic that gives you personal attention. You feel like a human going through a difficult time and not just a number designed to give the doctors more money.
Laurel's staff was the only one I've had who provided a detailed printout of what my insurance would cover. As a result, I had no problems with prior authorization. Given that my subsequent clinics have all had hiccups at the prior authorization phases for IVF, this says something about how thorough Laurel's staff is. Since I had to leave for relocation reasons, I hold Laurel's staff in high esteem and have no problem coming back to them with questions or concerns.

Simply the office visit co-pays and prescription co-pays. For an IUI, that was about $80.

She [Geraldine Ekpo] was very warm from the first visit she was optimistic. She explained everything she always made herself available via phone or email. I cannot express enough how amazing she is. She felt more like a friend of the family then my doctor. Amazing bedside manner. If we have to go this route for the next baby I hope to use her again...It [Laurel Fertility Care] is very small which allows them to treat each patient as such rather than a number in a huge hospital. The staff remember you they are warm and compassionate which make the entire experience less stressful.

she took the time to speak with us at every appointment. I was not passed off to a nurse. She remembered my partners name even though he wasn't at many of the early appointments. She was very kind and sincere about everything.

The initial script for one med was not called in however they did have some at the office to tie me over for a few days. At one point I was told to "keep taking my meds" after the results of a blood test. I was taking 8mg and at this point should have cut down to 4mg but wasn't informed until I tried to get a refill and was told by the pharmacy that I wasn't due for a refill.
When I began to have major side affects to the growth hormone he [Dr. Collin Smikle] didn't seem concerned. When I hyperstimulated he also wasn't too concerned even after I ended up in the emergency room. He was unapologetic when we miscarried. My biggest complaint is the lack of medical information protection we experienced at the clinic. At each appointment the ultrasound screen would show the last patient full name, date of birth and last image taken. They didn't sign out of the machine after each appointment. The front desk area had no privacy, everyone waiting for an appointment can here us verifying our names, date of birth, payment details, at some point I wrote down my social security number so that no one could here it. The positive was I got pregnant, the negative was the OHSS and miscarriage. The nurses were friendly but left the clinic shortly after our cycle ended. Overall it was an awkward cycle and experience.

The ultrasound machine has been one of the best (clear imaging) from all clinics. The egg retrieval room doesn't feel like an operation room which is a positive but also concerned me if it was sterile. We did not see the embryology lab therefore cannot comment. The clinic is small, clean and in a nice SF building. The bathroom was equipped with feminine hygiene products (tampons, pads, wipes). I found this really nice after appointments for clean up.

Don't expect warm and fuzzy bed side manner. Keep records of all your payments. Demand medical data protection.

Felt like a human because of one nurse who was always happy to see us, responded to my inquires in a timely manner.

Estrogen primed Antagonist with growth hormone. I do terribly on birth control, Dr. Smikle agreed to work with my natural cycle which other clinics at the time wouldn't. Due to previous miscarriage he had had me on baby aspirin and a steroid as an anti-inflammatory. The growth hormone wasn't explained. The overall result was Pregnancy with hyperstimulation followed by a miscarriage. I was on medical leave for 3 months and it took 1 year to gain my health back. For a year we couldn't do IVF treatment which was super upsetting.

If they were unsure they'd quickly get an answer. They always greeted us with a smile, cared about our cycle and showed compassion with my OHSS symptoms and miscarriage.
