Dr. Glass we said we could try up to 6 IUIs but we should keep 4 straws for IVF. I was on 100mg clomid cycle days 4-8. Then had IUI. Did this for 3 cycles. For the 4th cycle we did 150mg clomid and IUI. Each cycle we had 4-6 follicles but no success.

Being a nurse myself it is something I appreciated...I’m glad he was upfront and honest about our problems and the reality of what it would take for us to have a child...After my first and second cycle we went from just clomid to clomid and letrazole and that is when I got pregnant with my daughter. I responded well to the combination of both so that is what we stuck with for all future IUI’s.

The office staff is mostly knowledgeable, there have been a few times I’ve been offered a cup for a donation to bring in for an IUI when I would think it’s clearly marked that we’re using donor gametes.

I had one last minute appointment scheduled that I was completely forgotten about. It was a busy day in the office. I did not end up going back to that location. I have had the wrong lab work sent for blood tests. And I had one instance of an rx not being called in on a weekend that I had to resolve with the on call doctor.
[Allison Bloom] was extremely defensive about being asked to meet and insisted we hadn’t scheduled properly and that she had “other patients who needed her attention.”...asked for her medical perspective given the outcome of cycle 1, she accused us of having unrealistic expectations...
[Main Line Fertility] is like a factory--too many patients, doctors, nurses, coordinators for anyone to know or care who you are or specialize anything to your needs...rarely see your actual doctor...different nurses every time...always calling into an automated thing instead talking to an actual person
How was your experience with Allison Bloom at Main Line Fertility (Prelude Network clinic)?
At age 38, I was referred to Dr Allison Bloom for deeper dive into my fertility. From day 1, it was a total nightmare. I have fully normal menstrual history, average egg supply, hormone levels, and physiology. In our first consult, I told Dr. Bloom I had started trying with my partner and wanted to have two kids, so I wanted to make sure there were no issues with my fertility. She immediately told me I should start IVF ASAP and started rapid-firing terrifying statistics about the low odds of getting pregnant during any given cycle, the percentage of decline in my fertility with each year, and I was shell-shocked and burst into tears. She condescendingly told me “hey, don’t cry! Come on! Don’t cry!” as if that would help.
The scare tactic worked and I started IVF with her right away. In every meeting she was rushed, distracted, and verbally misquoted my age, details about my health, or details about our fertility plan. To prepare for our first egg retrieval, we worked with a rotating cast of at least ten nurses who would share info back to Dr. Bloom for directions. We had decided to do frozen cycles to stockpiles embryos and in our first cycle we got one normal embryo. This was communicated to us through a lab technician with no context or understanding of our fertility plan or any sensitivity. We didn’t know if one embryo was good news or bad, and when we finally met with Dr. Bloom a week later, she was extremely defensive about being asked to meet and insisted we hadn’t scheduled properly and that she had “other patients who needed her attention.” When we asked for her medical perspective given the outcome of cycle 1, she accused us of having unrealistic expectations, said that she “had told us this wouldn’t be easy,” and made reference to my “advanced age and low ovarian reserve,” which were not an actual response to what we were asking.
She said that 1 normal embryo was good news, so despite how rude she was being, we decided maybe she was a good doctor and we should just stick it out with her. On our next cycle, we got zero embryos. This time, the follow up meeting was more of the same, but much worse. She insisted she did not have time for our scheduled meeting and had other patients waiting. Her only medical response to the outcome was to repeatedly name and blame my age, egg supply, egg quality, and general fertility, and once again accuse of having unrealistic expectations, offering no actual insights or encouragement. She also mentioned that we should consider adoption, donor eggs, or other means of building our family.
My partner and I were appalled. We got two second opinions—one from another doctor at Main Line, and one from another fertility specialist—and the meetings were like night and day. Both new doctors were thoughtful, considerate, and optimistic about our options. The both said “as I’m sure your doctor told you, we always say that one cycle does not predict another.” We had in fact been told the opposite. Both doctors questioned the protocol Dr. Bloom had used and were quite optimistic about the possibility of other protocols, or simply other batches of eggs, being more successful.
We changed doctors and practices to one where a doctor actually paid attention to our specific cycle history, and our new doctor found critical things Dr. Bloom had overlooked: my eggs were not low quality, but they needed an extra day to mature before retrieval. Egg immaturity was something I had specifically asked Dr. Bloom about after our failed cycle, and she had dismissed it defensively. My new doctor paid close attention to this for every following cycle, creating a custom protocol to keep my eggs inside one extra day. My new doctor also learned that my partner had DNA fragmentation, which could be the root cause of our challenge in previous cycle—NOT my eggs or age, as Dr. Bloom had constantly told us. DNA fragmentation was pretty easily addressed, and with our new doctor, we had 2 normal embryos in our first cycle and one normal in our second cycle.
Working with our new doctor and actually feeling cared for and paid attention, and getting better results, to has made me realize how truly inappropriate and inept Dr. Bloom was in her treatment of us. I’m sharing this review so that if anyone else experiences the same treatment, they know that there is something much better out there. I would never, ever recommend anyone see Dr. Bloom.

They outsource medication management so there is even another step and endless calls coordinating getting your Rxs, making it harder instead of easier.

She was extremely defensive in our meetings, dismissive of our questions, and insisted we hadn’t scheduled properly and that she had “other patients who needed her attention.” When we asked for her medical perspective given the outcome following each cycle, she accused us of having unrealistic expectations, said that she “had told us this wouldn’t be easy,” and made reference to my “advanced age and low ovarian reserve,” which were not an actual response to what we were asking.
Dr. Bloom overlooked key aspects of our fertility needs. She labeled my eggs "low quality" based on lab results of 2 cycles, but they actually needed an extra day to mature before retrieval, and the clomid protocol she selected was not effective. You may not get to see him as often as you might a physician at a smaller practice, but he has the experience and expertise for more difficult cases. It is worth your while to use Dr. Glassner.

Some days these appointments went quickly and smoothly while other days they took a while and you were ushered from one waiting area to the next. It really depended on how busy the clinic was.

I believe I was charged 5000$ for retrieval #4, 6000$ for retrieval #3, 7000$ for retrieval #2. Retrieval #1 was cancelled. My insurance paid for some of the monitoring. If you do FET With PGD the first transfer is only 2000$. I believe the ERA was 1,300$. PGD was somewhere around 7000$ with all associated fees.

Dr. Gocial is extremely kind and sensitive. He has an excellent bed-side manner. We have done one trigger with timed intercourse. We have also done 4 iuis. The first iui I took 2.5mg of letrozole and ovidrel for my trigger. The second iui I took 5 mg of letrozole and ovidrel for my trigger. The third iui I did 5mg of letrozole and 2.5mg of clomind and ovidrel for my trigger. The fourth iui I did 5mg of letrozole and 75ius of follistim and ovidrel for my trigger. None of these protocols resulted in an positive pregnancy test. We are currently in the stimulation phase of ivf. We are doing 75ius of menopur and 225 ius of follistim.

He kept using generic language like, "we'll get you pregnant," completely disregarding that our problem was repeated miscarriages and that in fact we conceived easily several times on our own.

Strengths: He represented himself as wanting to take an "aggressive" approach (he used that word a lot), and that sounded appealing to us. I wasn't getting any younger and we just wanted to hurry up and have the kid already.
Weaknesses/things that were not to my taste: As I said before, he seemed to take somewhat of a generic, impersonal approach to our case. He talked about getting me pregnant rather than getting me a live birth (my problem was/is multiple miscarriages and I felt he was not paying attention to that). He did not ask any particulars about the past miscarriages and did not seem to think that any details were relevant. He adopted a very masculine/"bro" attitude toward my husband; maybe other men like that or feel it addresses their insecurity around infertility treatments but we thought it was weird. He informed me what we were going to do (medicated cycles, combo of clomid/letrozole), rather than advising and explaining a course of treatment. The first cycle (clomid + timed intercourse) did not work. The second cycle when I came in shortly before ovulation, he informed me (rather than suggesting to me) that we were going to do an IUI. When I expressed some hesitation, saying that I'd heard success rates were not much higher than timed intercourse for people with my medical history, he retorted in a snappy tone of voice, "Fine, then don't do it." And turned away and started ignoring me. I finally agreed to the IUI but I felt like I was bullied into it. It didn't work anyway. For the first time in my life I was having trouble getting pregnant--when I had come to him not for help conceiving but with figuring out what was happening with my miscarriages.
The following year, I went back to MLF for monitoring on a new pregnancy (that we conceived on our own, not with their help) and ended up seeing him a couple times during that period. First, he gave me false hope when Dr Glassner had been telling me to prepare for another loss; when it finally ended in miscarriage again he came across as cold and unsympathetic. He scheduled me for a D&C--did not ask my opinion or preference--and again started talking about "getting aggressive". I let him do the procedure but I will never go back to him after this.

It's nice that they have multiple locations, however, you should know that most of the doctors rotate between locations. I chose the clinic mostly for its location in Bryn Mawr, but then I found out that if I wanted to keep seeing my same doctor I had to travel occasionally to the West Chester and Paoli locations as well. The nurses also change locations so it's hard to get to know them.

I think they are all a bit disorganized--front desk, doctors, everyone. My initial doctor didn't order a full reproductive loss panel though I had had repeated miscarriages; I've had nurses say they are going to call with test results and then they don't; nurses say they are going to call in prescriptions and then they don't; and one time I called to schedule a new cycle with my particular doctor and the receptionist scheduled me with him for a date and time at an office location where he wasn't even going to be on that particular day! I showed up for the appointment and they looked at me like I was crazy--"Dr Orris is never here on Wednesdays" etc. The doctor who was there was able to squeeze me in, but none of this was reassuring.
Although we have still not been successful, I have been very happy with my treatment to date. Dr. Gocial always listens to my concerns with compassion and presents options to consider. Although I do not see Dr. Glassner each time, when I do he is very open and honest about the route of treatment and what needs to be done. While he is clear on the information he gives, he does so in a way that makes the patient feel comfortable. He seems to understand this process is emotional and caters to that when discussing treatment, issues, etc, with his patients...[treatment included] the clomid challenge in order to see if I was able to grow more follicles (it worked, I grew more follicles) 2.) I had to pay for my copay at appointments, and I had to pay for the IUI’s Out of pocket... the parking is only street parking or a parking garage at the nearest hospital which is about 2 blocks away...

Although we haven’t had the results that we wanted, Dr. Gocial is amazing. He is always so open and thorough with explaining everything that is happening in our process. The Philadelphia office of Main Line Fertility is very clean, private, and easy to find. The waiting area is comfortable. The entire office is ran very efficiently. The only thing I don’t like is that since it is on a street in center city, the parking is only street parking or a parking garage at the nearest hospital which is about 2 blocks away. That is honestly the only downside. But I’ve been getting lucky and finding street parking near the office.

He is incredibly knowledgeable and ALWAYS followed up with my blood work results and even changed my trigger shot halfway through because my estrogen levels were getting high

Dr. Minassian is incredibly knowledgeable on all aspects of the fertility journey. From day one he told me he suspected it was MFI but we would do all of the necessary steps to make sure I wasn’t an issue as well. All of my previous doctors told me to not bother getting tested, so I had a growing anxiety that something may have been wrong with me. Dr Minassian truly cared about getting to the bottom of the issue. He asked me what were my goals and then got right to work. He wasted no time in getting my tests done and moving on to my egg retrieval. He made sure to review my blood work every single day, and to call me and let me know that my reserve looked great and I was on a good path. He called me and explained the entire IVF and PGT testing in depth and really made me feel respected and appreciated. I am so thankful to him and know I am in the right hands once my transfer finally comes around.

Due to my age and my fertility health, we did standard protocol. My baseline follicle count showed 21(age 32) and I had no endometriosis or any blood work issues, my tubes were open. We concluded it was MFI and so I did 150 units of menopur and follostim every night, I started ganirelix on day 5 of my cycle. He originally suggest that I use pregnyl as my trigger shot but upon seeing my blood work results closer to trigger shot time, he suggested changing it to Lupron because my estrogen levels were around 4300. He made sure to have my nurse explain to me why we were switching to lupron and that it would leave my system quicker. Upon egg retrieval day, we got 18 eggs and 15 of them were mature, so I would say the protocol was very successful and right in line with what he wanted. 9 of those eggs were fertilized(again, mfi so im grateful that any of them did) and 4 of them became day 5 blastocysts with 1 day 7 blast(5 total) they are currently out for PGT testing.

As women who are so anxious to start a family, no matter what your situation is in life, you want to feel like you are being heard and that you matter, and this office has set the bar high.

Scheduling monitoring appointments was really easy. This office runs so smooth and knows exactly what they are doing, I never had to wait very long at all. I would schedule them usually first thing in the morning and be in and out within a half hour with blood work and ultrasound done each time. They would call me each afternoon to go over results after Dr Minassian reviewed.

I had to pay for everything out of pocket. Unfortunately my insurance did not cover anything, but Dr Minassian did specifically ask me about my insurance when I went in for my consult. As a previous medical office employee, I’ve never had a doctor take my insurance into consideration at all so this was a nice surprise. I did receive a main line fertility discount for my prescription meds because I used their pharmacy. This is not a doctors office problem but an insurance problem. Insurance needs to take fertility a lot more seriously so that women of all kinds can have the chance to have a family of their own.

Ask all of your questions, and take notes. She doesn’t know what you understand and what you don’t. It’s a LOT of information and having it come at you all at once, kind of quickly, with IVF language and all that’s involved, it’s easy to walk out of a consultation overwhelmed and with even more questions. She never rushed me out of an appointment..she answered any and all of my questions no matter how long I took.

Dr. Brasile mentioned that they often start with Clomid, but in my case, dealing with male factor infertility, we’d skip that as it did not seem necessary. She had my hormones and thyroid checked, and I did in fact need thyroid medication to give it a boost. Dr. Brasile did 2 different hysteroscopies to check for polyps or other uterine issues to ensure my uterus was healthy and free of anything that could cause problems with my eggs/retrieval/transfer. We did one round of hormone injections and egg retrieval, only to come back with one little embryo. She suggested another round of hormones and egg retrieval, which we did, and were able to add two more viable embryos to our bank. We chose to freeze and do genetic testing with our 3, and came back with the news of one healthy embryo to transfer. My regret was not getting more information on what exactly was off about my other two embryos. Fortunately Dr. Brasile’s protocols are what gave me one very healthy, happy baby boy.

I cannot express enough how caring, compassionate, and loving the care team at main line fertility was. It has been 3 years and I wish I remembered specific names, but the women that cared for me seemed to really care. It made every-other-day appointments so much less daunting. Pleasant even! Imagine going in for ultrasounds and bloodwork multiple times a week and finding it pleasant?! I’m so grateful for all of these women. I only hope that I can touch someone else’s life that same way. What they do every day is amazing to me.

One major weakness, at least in 2018….the billing department. I had more stress and tears throughout my experience, due to incorrect billing and very unpleasant interactions with those involved, than the actual hormones/injections/or procedures. My husband spent hours upon hours dealing with the constant billing and insurance mess, as though it were his second job. It was quite traumatic. A constant black cloud and stress over an already emotional and stressful experience by nature. Again, this wax 2018. Hopefully they have a new billing person by now.

I did ask at one of my appointments with the care team, if I was able to transfer more than one embryo. They said that they do not normally do or recommend that, which I was disappointed about. But it was also something that I could discuss with Dr. Brasile. At the end of the day, I only had one embryo to transfer, so it didn’t affect me. But if I had two, I would have pushed for both to be transferred.

1. First cycle I was on clomid. Ovulated 3 eggs. Was told when I was ovulating via ultrasound and bloodwoork. Took Crinone until negative pregnancy test.
2. Second cycle took clomid and letrozole. Ovulated 2 eggs. Negative postcoital test. Did IUI based on ovulation monitored by bloodwork and ultrasound. Took Crinone. Negative pregnancy test.
3. This cycle took clomid and letrozole. Ovulated 1 egg based on blood work and ultrasound monitoring. Took Crinone until negative pregnancy test.
4. Fourth cycle took injections. Clomid, Gonal f, cetrotide, and trigger shot. Ovulated approximately 5 eggs. Did IUI based on ultrasound and blood work. Took Crinone until negative pregnancy test.
5. Took 1 month off
6. This month just took Letrozole in anticipation of possibility of IVF the following month. Monitoring of ultrasound and blood work. Ovulated 1egg. Took Crinone until negative pregnancy test.
7. September 2017 we did IVF. Was on Gonal F, cetrotide, and low dose Hcg. We had 40 eggs retrieved, 19 fertilized, and 11 made it to blastocyst stage on day 5 for all freeze. Dr Brasile recommended all freeze so that my body could recover. I did not have OHS.
8. October 2017 we did FET. Was started on Estrace 2 mg/three times per day and baby aspirin. Monitored via ultrasound & bloodwork. When lining was greater than 8cm - FET was scheduled. 5 days before transfer I was placed on medrol dose pack, PIO shots, Crinone, estrace, and baby asprin. Transferred 1 blastocyst and went on 24 hour bed rest. After transfer continued on medications until negative pregnancy test.
9. November 2017 same protocol. Only change was 1 10 mg Valium prior to FET. December got BFP after 2 week wait. Continued on PIO and Crinone. Ultrasound and bloodwork weekly. At week 7 no heartbeat. Diagnosed blighted ovum.
10. Did rush PGS testing in March prior to our 3rd transfer. Transferred on PGS normal embryo with same med protocol as our prior FET. BFN after 2 week wait. No remaining embryos from 1st retrieval.
11. In June did a second IVF retrieval. Was on Gonal F, cetrotide, and low dose Hcg. Retrieved 22 eggs, 9 fertilized, 4 ended up making it to blast stage and being frozen after PGS biopsy done. PGS testing revealed 2 normal, 2 abnormal.
12. In July we did FET. Was started on Estrace 2 mg/three times per day and baby aspirin. Monitored via ultrasound & bloodwork. When lining was greater than 8cm - FET was scheduled. 5 days before transfer I was placed on medrol dose pack, PIO shots, Crinone, estrace, and baby asprin. Transferred 1 blastocyst and went on 24 hour bed rest. Added Lovenox to medication regimen every night. Continued on medications until negative pregnancy test.
13. Did an ERA cycle based on Dr. Brasile's recommendations that revealed that I was 24 hours pre-receptive.
14. In September we did the same preparation for a FET. The only change was I took 24 hours more medication prior to transfer to correct for my ERA. Took Lovenox after transfer every evening until negative pregnancy test.

I started with letrozole with the plan to shorten my cycle for timed intercourse because of a travel plan. It actually lengthened my cycle and didn’t work. Then I did a cycle of clomid with timed intercourse and possible IUI. I was seen by a PA for my post cloital and she said we didn’t need the IUI. Everyone in the office is very thoughtful and compassionate. They sometimes have IVF in the morning so the doctor isn’t there first thing and they only draw blood in the AM so you can’t come in the afternoon. That can be difficult with work.

I am very impressed with Dr. Orris...On the third and final cycle [of IUI], I took a booster shot to ensure the timing was correct and luckily got pregnant on the third try...After each ultrasound and bloodwork, I was taken to the doctor's office to go over results/next steps...Due to my PCOS and anovulation, I was put on rounds of Clomid and Letrozole to induce ovulation. I would be seen at least once if not twice a week to measure my progress and to see if ovulation was occurring. Once they were comfortable with the ovulation taking place, I had two cycles with IUI that didn't time up properly.
The staff at MLF were so wonderful and helpful; all the doctors clearly explained everything to me at every visit. After each ultrasound and bloodwork, I was taken to the doctor's office to go over results/next steps. Every one's bedside manner was extremely warm...I cannot say enough wonderful things about Brie [nurse]. She looked after me almost every visit; took my blood (painlessly I might add), and was always on hand for my questions. MLF set the bar high for my expectations for OB/GYN care. In fact, I quite wish they were my OB/GYN! I am so thankful and feel so lucky to have been entrusted into their care. They did everything they could to explain everything I needed to do and follow up with me when it should have been done. They take great care of their patients and I cannot say enough about our experience there.

We just went straight to IUI, the 1st try failed and I am in the TWW for my 2nd IUI now.... She’s not the right doctor for a timid patient. She will not ask you if you have questions. I suppose she just assumes you already know what she’s talking about.
The staff [Main Line Fertility- West Chester] is very kind and warm but they almost never know why I am in that day. I came in with sperm for my first IUI and the nurse thought I was there for more monitoring.... They have a lot of locations which can make things a bit more convenient but the hours are not the best. They don’t open until 8am even for bloodwork. I am always late for work after an appointment. It feels like a revolving door at times.

100 mg Clomid for 5 days for my 1st IUI cycle with monitoring and timed IUI. I have no idea why she chose this course. 2nd IUI I did 150 mg Clomid for 2 or 3 days then 100 mg for the remaining days (5 days of Clomid total) with a trigger shot (7500 Novarel) for IUI because my fiancé went out of town. Booster of 2500 Novarel a week after initial shot to potential improve success. I am in the TWW now.

I was trying to schedule my 2 nd IUI not knowing which day it would be so I made two appointments at two of their clinics based on timing (two different days) or so I THOUGHT. Both appointments were made on the same day at the same location. It ended up working out but what if it hadn’t?
Dr. Orris is very open, clear, and honest.... Strategy was to try and get husbands count up and increase motility. He had him on different vitamins to help increase this. We did do clomid at least once but the main course of action was IUI. ... He was very honest with us, but let us make the decisions. He told us from the beginning that with the numbers ivf may be the best but we didn’t want to jump right to that and with us being young he was excited to try to explore different options. Easy to ask questions and you knew you were going to get a real response from him.
Strength [of Main Line Fertility] would be the professionalism and kindness. Weakness would be how slow things would take. Waiting rooms would be packed and you would wait for awhile.
