This is the most recent data from the CDC. The CDC cautions that it should not be used to compare doctors and clinics. This data represents the outcomes of IVF cycles when using a person's own eggs.
For more information please see our FAQs.
Where you see a “?” on the chart, a clinic has done fewer than 20 cycles in this category and age range, so the CDC says showing percentages from this small data set would be misleading and unreliable.
I never met [Dr. Cindy Duke] throughout the entire course of treatment...I was told Dr. Duke was overseeing my ultrasounds, even though she’d never met me in person...never even saw her in passing in the office any time I went there for treatment...IUI included birth control Norethinidrone 5mg for a few days at the start of my period, then clomid 200 mg for 5 days. Then HCG trigger shot. Insemination...huge lack in communication and transparency
There are a lot of weaknesses with [Nevada Fertility Institute]...you never even meet the doctor so there is no doctor overseeing your care. Communication and follow up is disorganized. The staff is under qualified for the jobs they are doing...cannot answer your questions at all...only met the NP once in person...staff is unfriendly...treated like a number...never introduce themselves when they walk in a room...You do not get a calendar
How was your experience with Cindy Duke at Nevada Fertility Institute?
I never met her throughout the entire course of treatment. Instead, her NP was the only person I ever saw. Yet, I was told Dr. Duke was overseeing my ultrasounds, even though she’d never met me in person. I never even saw her in passing in the office any time I went there for treatment.
What's one piece of advice would you give a prospective patient of Cindy Duke at Nevada Fertility Institute?
Don’t go here. Fertility treatments are hard enough. But there is a huge lack in communication and transparency that makes it very difficult to work with Dr. Duke. I never even met the woman during my care so how can I put faith in her being a capable doctor with a plan to meet my specific needs.
During treatment, were you treated like a number or a human with Cindy Duke at Nevada Fertility Institute?
When you go in for ultrasounds and say, “take off your clothes” and leave. Then the ultrasound tech and a medical assistant will come in, they won’t even introduce themselves. For the longest time, I thought the ultrasound tech was my NP because I only saw her on zoom until the insemination. No one tells you next steps or what you’re there for. I only spoke to a nurse once, and she had such a thick accent that I could not understand her answers to my questions, and this was after a lot of miscommunication. They do not upload your results until after the procedure is finished. No one showed me how to do the injection. I ended up looking it up on YouTube. Everyone is scowling and unfriendly in the office except the front desk. Someone hung up on me while I was asking about results of my ultrasound. I was told conflicting information 3 times about how many eggs I had which made me wonder about whether they were actually looking at my chart.
Describe the protocols Cindy Duke used in your cycles at Nevada Fertility Institute and their degree of success.
IUI included birth control Norethinidrone 5mg for a few days at the start of my period, then clomid 200 mg for 5 days. Then HCG trigger shot. Insemination. Then Endometrin and E2 afterward to support the pregnancy.
Describe your experience with the nursing staff at Nevada Fertility Institute.
I only spoke to a nurse once. The people you see in the office are medical assistants, and they are the ones doing the bulk of the care. They are very unfriendly. They never greet you. They never introduce themselves when they walk in a room. When you ask questions, you get a lot of push back quickly because they aren’t nurses and don’t know how to answer the questions. This results in patients feeling like they are going through this blindly. You do not get a calendar. There is one in the portal but it is not updated until days later so it is easy to make a mistake with your medicine since you mostly have to rely on the verbal instructions. When I told them my calendar wasn’t updated, they were quick to argue that it was. And I had to tell them I had just checked that morning, and it still wasn’t, after multiple contacts to get that fixed. This was the day of the procedure. They do not give you instructions on injections. I was hung up on once by a member of the staff when they didn’t want to let me speak to the nurse who had just called me and I’d missed the call. When I called back and told other staff that this woman had hung up on me (which I hadn’t been rude, I was just trying to explain the situation) they told me a patient experience coordinator would reach out. No one ever did. The one nurse who I spoke to had such a thick accent it was very hard to understand what she was saying when I asked questions. The day I got my medication instructions, I was told I had two eggs that were big enough for insemination. Then the day of the procedure, the NP said I had 5 eggs. When my ultrasound was updated online, several days after the procedure, I actually had 8 eggs, but only 3 of them were large enough to be fertilized. My lining was 5.3 though, which no one had ever addressed with me before hand but that is not good for implantation which I later learned.
Describe your experience with Nevada Fertility Institute.
Their strengths are they will want to do every test under the sun to figure out your fertility issue. This can be good because they don’t just push you into procedures right away to get pregnant but it can also be unnecessary. I had uterine fibroids and a polyp. They did not want to remove them because of the risk of scar tissue. I had an ultrasound, an MRI, and an HSG. HSG showed tubes were open. Ultrasound and MRI showed fibroids. The NP wanted me to get an SIS. I’d already spent thousands on the HSG, MRI, and US. So, I asked why the SIS because we know I have fibroids. We didn’t plan to do anything about them. We know their location and size based on the MRI (they were outside the uterus). She said she wanted to make sure the uterus would expand, but what she would do if it didn’t was unclear to me. I decided not to do the SIS. No other doctor I’ve seen after has recommended it. There are a lot of weaknesses with this clinic the first of which being you never even meet the doctor so there is no doctor overseeing your care. Communication and follow up is disorganized. The staff is under qualified for the jobs they are doing. They cannot answer your questions at all. I only met the NP once in person. The staff is unfriendly. You are treated like a number. The NP also ordered another CBC (unbeknownst to me) when I’d just had one done two months prior. Nothing had changed on the CBC and I was charged 250 dollars.
Describe the costs associated with your care under Cindy Duke at Nevada Fertility Institute.
It was 2000 for IUI not including medication. It was probably about another 350 for the meds. My insurance covered a lot of the pills. The HCG trigger was the most expensive. I think it was around 250. The rest were like 20 to 30 bucks each. The Clomid might have been a little more.
Describe Cindy Duke's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Nevada Fertility Institute.
I don’t know because we never got that far. I moved clinics after the first IUI.

Describe the specific things that went wrong at Nevada Fertility Institute.
Everything went wrong with the IUI because the communication was so poor. From how many eggs I had, to how to take the medication, there were always people telling me different things. I ended up taking the aspirin days before I should have. I was also never informed about my uterine lining being 5.3. You cannot get a qualified health care professional on the phone to ask any questions. They are now trying to bill me for labs that were covered as part of the IUI and I’ve spent months going back and forth with billing about this. They say it will be taken care of but then I’m still receiving a bill for labs I already paid for as part of my IUI. Also, getting my medical records, they will make you wait until the last day of the 30 days. By law, they have to provide your medical records in 30 days and I had to remind them at the 30 day mark, and then, all of the sudden, they sent it right over, leading me to believe they could have done it much sooner.
All in all, Dr. Duke is a caring, patient, and friendly doctor who treats me like an individual and human. I just wish our expectations weren't set quite so high and that I could meet with her more often...My last two cycles I did not have OHSS because Dr. Duke adjusted my medication, which I appreciated...I also felt like what Dr. Duke had explained would happen was not happening.
The [Nevada Fertility Institute] office is very nice (it looks like a fancy spa)...The receptionist knows my name now and always warmly greets me when I come in. The phlebotomist is phenomenal and by far the most friendly person in the office that I interact with regularly...The nurses are still not terribly friendly during my ultrasounds, but they are more cordial and usually greet me.
How was your experience with Cindy Duke at Nevada Fertility Institute?
Dr. Duke is an incredibly compassionate and skilled doctor. We've had two tele-medicine calls with her and she has performed 3 IUI's for us so far. Whenever I've met with her she has been patient and understanding as I ask question after question. The one thing that is a little hard about Dr. Duke is she is incredibly optimistic. After our first meeting and before we started IUI, I left our meeting feeling like we would 100% get pregnant from IUI. I know she didn't say it was that certain, but she was so confident with our numbers and age. There has also been some confusion about statistics and IUI. She told us our first IUI had a 20% chance of success and each consecutive IUI chances would increase by 20%. So, our second IUI would have a 40% chance and our 3rd 60% chance. I later understood that isn't exactly what she meant by those stats and learned in reality the chance is only 20% tops each time and doing multiple IUI's helps in those stats, but doesn't effect my body. I learned this clarification from a nurse in our first IUI cycle and it was deeply upsetting to me. Now that I know the stats, I was able to go in to our next IUI cycle with eyes wide open.
All in all, Dr. Duke is a caring, patient, and friendly doctor who treats me like an individual and human. I just wish our expectations weren't set quite so high and that I could meet with her more often.
What's one piece of advice would you give a prospective patient of Cindy Duke at Nevada Fertility Institute?
Write down your questions and don't be afraid to ask them even if you think they are silly. Also, don't rely on Dr. Duke to know how much your treatment will cost. Definitely speak with the office manager about that. We were told our IUI was covered and didn't realize till midway through the cycle that coverage did not include medication, which cost us about $1,500 per cycle.
During treatment, were you treated like a number or a human with Cindy Duke at Nevada Fertility Institute?
I felt like a number our first IUI cycle. When I went in for my first appointment, I thought someone would explain in greater detail what would happen with the IUI and would be available to ask questions. But, I discovered those appointments are just for bloodwork and an ultrasound, not necessarily for asking questions. I was also frustrated because when they showed me to the room, no one instructed me about getting undressed (I later found out this was a mistake and they apologized) Basically, it took me a while to understand that they would call me after each appointment. That was incredibly frustrating at first and I felt like I was moving blindly through the process. I also felt like what Dr. Duke had explained would happen was not happening. For instance, she said my baseline ultrasound would be around day 3 and I wasn't scheduled until day 6 or so. I now understand that isn't a big deal, but no one explained that to me at the time. That all being said, things have vastly improved since then and I have completed 2 more IUI cycles with Dr. Duke's office. I've developed a relationship with the staff and feel a lot more brave when asking questions, which has helped. It has been a night and day experience compared to my first IUI cycle.
Describe the protocols Cindy Duke used in your cycles at Nevada Fertility Institute and their degree of success.
Dr. Duke has had me take supplements since before we started IUI: COQ10, myo-Inositol, and a prenatal vitamin. For the IUI's I took Clomid, follistim, and sometimes estrace. I also took progesterone after each IUI cycle. Lastly, I did an ovidril trigger shot. Each cycle I have had quite a lot of eggs and a risk of multiples as a result of these medication and my possible PCOS. My first cycle I had OHSS. My last two cycles I did not have OHSS because Dr. Duke adjusted my medication, which I appreciated.
Describe your experience with the nursing staff at Nevada Fertility Institute.
I have had a variety of experiences with nurses. During my first IUI cycle, I felt like the nurses were cold, impatient with my questions, and I struggled to understand their instructions and reasoning for my instructions. They also sometimes didn't greet or acknowledge me during my ultrasound appointments.
However, things have greatly improved since my first cycle. The nurses are still not terribly friendly during my ultrasounds, but they are more cordial and usually greet me. Over the phone they are usually very friendly and I loved that a recent nurse called me and excitedly told me that it was time for my trigger shot. I loved having someone share that excitement for me.
Describe your experience with Nevada Fertility Institute.
The office is very nice (it looks like a fancy spa) and conveniently located right off the freeway. The receptionist knows my name now and always warmly greets me when I come in. The phlebotomist is phenomenal and by far the most friendly person in the office that I interact with regularly. She knows I need to use "baby" needles in my arm and is always so excited and optimistic about me having a baby. I recently told her I just celebrated my 4th anniversary and she told me we'll celebrate our 5th with our baby. She is seriously the best. And Dr. Duke really is a great doctor who cares about her patients and knows exactly what she is doing with my treatment. The nurses are also generally friendly and helpful.
The office does have weaknesses. They usually take a while to upload my ultrasounds (sometimes days or weeks) which I don't appreciate as I am so eagerly following along in this process. Ultrasounds are usually totally silent and cold, I wish they would be more friendly and warm to me. There is one room that doesn't have a big screen set up so I can't see my ultrasound while they are doing it. I also wish they would just tell me my ultrasound results right away at the end of the appointment. Instead I have to look at the screen (when there is one to see) and try to guess the sizes of the follicles. Communication was also a struggle, especially at the beginning of the process.
All that being said, if this third IUI doesn't work, I do plan on continuing with this office and doing IVF.
Describe the costs associated with your care under Cindy Duke at Nevada Fertility Institute.
My IUI was covered by insurance. But, I had to pay for medications and co-pays out of pocket. This ended up being about $1,500 per cycle
Describe Cindy Duke's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Nevada Fertility Institute.
I did IUI, not a transfer. She warned me thoroughly about the dangers of multiples and we had to specifically agree that we understood the risks before proceeding. We have spoken to her about IVF and she prefers transferring a single embryo, but is open to no more than two embryos.
What specific things went wrong at Nevada Fertility Institute?

Describe the specific things that went wrong at Nevada Fertility Institute.
At the beginning of the process there were a few documents (agreements to share info from another doctors office) they needed me to send to them. I ended up sending the documents multiple times as they kept saying they didn't receive them. I also have signed the some of the same forms over and over again.
Dr duke is super nice and she does explain things to you only if you ask. The telemedicine appts always seem rushed. I have only met with her in person at the egg retrieval, and frozen embryo transfer...She lets you choose which I think is great. She gives you the option 1 or 2 but tells you the risks and complications...I respond well to medicine so egg retrieval was pretty basic. However I did get OHSS from it
The [Nevada Fertility Institute] nursing staff and phlebotomist are great! The ultrasound tech is a bit rough though. The nurses will always answer my questions and know name when you walk through the door...The staff is super nice and the place is really clean. It’s not usually busy but when it is everyone gets in really quickly. They use a texting service so they can answer you faster. However, there is no one answering on the weekends unless it is really important.

Make sure you do things for you. If you want something done ask because they might not give you that option

I feel like Nevada Fertility Insitute treats you individually but to a point. It seems that when you have a failed cycle the next try your protocol doesn’t really change
Describe the protocols Cindy Duke used in your cycles at Nevada Fertility Institute and their degree of success.
I respond well to medicine so egg retrieval was pretty basic. However I did get OHSS from it. Frozen embryo transfers start out with letrozole then gonal, cetrotide, and trigger shot. endometrin suppositories are used instead of the shot

$15000 for IVF without meds- does include 1 frozen transfer

$600 for yearly embryo storage options
Describe Cindy Duke's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Nevada Fertility Institute.

Describe the specific things that went wrong at Nevada Fertility Institute.
It might just be me but it seems though they make assumptions that things will work instead. I am young and I feel like everyone was really surprised when my FET did not work. They also told me that since I was young there wasn’t a need to test my embryos even with MFI.
Dr. Duke is very kind and remembers personal details about you that wouldn’t be in your medical file. While she’s straight forward with the facts she also remembers that you’re human and infertility is a sensitive subject so she treats you like you matter...[she] typically advocates for a single embryo transfer which I’m all for..
The [NFI] clinic itself is clean and typically runs nearly on time, no drs office is ever on time. Their communication did suffer a bit but now seems to be getting better especially with the text option. They almost never answer the phone which is a bit difficult but you can typically get a response on the text line. I like that the clinic is a complete clinic so no outside centers for testing or transfers. The office is very comfortable and clean and they have refreshments and phone chargers which is nice.
How was your experience with Cindy Duke at Nevada Fertility Institute?
I’ve seen Dr duke over the years for various rounds of treatment. I switched to her after negative experiences with other clinics. As she’s gotten busier you do see her personally less but when you do see her she is still caring and thorough

I’ve had many cycles with various drs and am still ttc so it gets fuzzy, but the only dr who actually kept searching to find more and to look into my unique case has been Dr. Duke. She’s the one who found majority of my issues and then treated for those to help me conceive. We did IVF with immunotherapy such as intralipids and adding blood thinners due to conditions I have

[Dr. Cindy Duke] puts a lot of the decisions in your hands which sounds great but was not helpful. Her major downside is communication. Out of all the times that I had questions about what treatment was going to entail, how we needed to adjust my meds, or what decision I should make regarding cancelling a cycle, I only spoke with her twice. Almost all communication was through a nurse. And when we were hoping to speak with her shortly after a failed cycle to reassess plans, it was over a month long wait for a telehealth appointment.
nursing staff [at Nevada Fertility Institute] seemed like they had too many patients to give their undivided attention to. And that while questions were understandable, it was an inconvenience to answer them in a timely manner...Dr. Duke is a great doctor but does not have a great staff running things...grateful to finally be done with her clinic once we moved forward with another clinic.

She's a great doctor, but her office staff and communication leave a lot to be desired.
During treatment, were you treated like a number or a human with Cindy Duke at Nevada Fertility Institute?
Dr. Duke is a great doctor but does not have a great staff running things. I rarely spoke to Dr. Duke regarding the outcome of any of my ultrasounds and bloodwork. Trying to get a follow-up with her regarding a cancelled cycle was booked over a month out.
When we were asked if we wanted to cancel our egg retrieval cycle due to possible poor response, I never received a call from her and was asked to make a decision within an hour. It was also never made clear that if the cycle was cancelled, I would be penalized over $8500 for 2 appointments and 2 blood draws.
Describe the protocols Cindy Duke used in your cycles at Nevada Fertility Institute and their degree of success.
Cycle 1 was IUI with clomid. I did not respond well to the lowest dose of clomid and was on it for a second week at triple the dose. It resulted in two 18-20mm follicles. The cycle did not end in pregnancy.
Cycle 2 was another IUI but with letrozole. I did not respond to the letrozole at all. We added in 75iu of follistim and 37.5units of menopur. I ended up with 2 15mm follicles that I ovulated prior to being able to complete an IUI. The cycle did not end in pregnancy.
Egg Retrieval Cycle: 2 weeks of estrogen priming, doxycycline, Gonal-f 400IU, Menopur 150IU. At my second ultrasound I only had 5 small follicles and was given the choice to convert to an IUI, cancel the cycle, or continue. I was called at 7pm and asked to make a decision within the hour without speaking to Dr. Duke.
I was concerned about using birth control to prime/time a cycle and so estrogen was suggested instead.

My nurse and the rest of the nursing staff seemed like they had too many patients to give their undivided attention to. And that while questions were understandable, it was an inconvenience to answer them in a timely manner.
Describe your experience with Nevada Fertility Institute.
Dr. Duke is a great doctor but she is not board certified in the subspecialty of reproductive endocrinology and infertility and her office staff treats patients as a number and not a human. The VP of NFI was more concerned about charging us over $8500 for a cancelled cycle that only involved 2 appointments and 2 blood draws than she was about putting a couple already dealing with a difficult situation through a more painful and financially difficult situation.
While I am sure that plenty of other couples had good results with Dr. Duke, we did not and I was incredibly grateful to finally be done with her clinic once we moved forward with another clinic.
Describe your experience with your monitoring appointments at Nevada Fertility Institute.
Monitoring for the most part was quick and simple. My first appointment I waited for over an hour just for blood work. I was assured that was not normal.
Describe the costs associated with your care under Cindy Duke at Nevada Fertility Institute.
Traditional IVF cycle was 15k
Mini-IVF was $7500
Describe Cindy Duke's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Nevada Fertility Institute.
Dr. Duke preferred single embryo transfers. I lost twins at 24 weeks so this was fine with me.

Describe the specific things that went wrong at Nevada Fertility Institute.
Our egg retrieval cycle was cancelled due to poor response. What was not conveyed to us was that we would be charged the quoted refund amount for a cycle that was cancelled due to circumstances out of our control. As mentioned previously, I was called at 7pm with the results of my second ultrasound and asked if we wanted to continue the cycle, cancel it, or convert it to an IUI. When I asked for clarification on what we would be charged if we cancelled the cycle, my nurse stated she was "pretty sure" we would not be charged the amount quoted in our contract because it was cancelled due to poor response, not because it didn't work with our schedule. When I was called by the clinic manager to explain how much money we had to put towards another egg retrieval, I was told that we had a little over 8k out of 15k. Which meant that our ONE cancelled cycle that involved two ultrasounds and two blood draws would cost us over 7k. It took over 2 months and several calls to the VP of NFI to get this cleared and in the end we were refunded $10,500 out of 15k. Which means we paid over $4500 for a cancelled cycle (not including meds). Throughout this entire process, we were never contacted once by Dr. Duke or the nursing staff.
Dr. Duke is incredibly knowledgeable and took all of our history into account without relying solely on it to decide her course of action. She did the testing she needed to make the best decision she could for us, which resulted in finally getting a positive pregnancy test...You won’t see Dr. every time you go in the office, but you don’t need to...Due to my age and the number of IVFs and failed embryo transfers we suffered before coming to Dr. Duke, we did three mini IVFs

They had my questions answered almost immediately...They’re 100% in-house, which was nice because when I woke up from anesthesia, it was helpful to see my partner there rather than be in a separate hospital recovery room...Briana and all of the nurses that I interacted with were attentive, patient, and kind. They were available and very responsive to me.

Due to my age and the number of IVFs and failed embryo transfers we suffered before coming to Dr. Duke, we did three mini IVFs with Dr. Duke. We didn’t get a lot of eggs, but they were good enough to get three embryos to pass genetic testing. After 4 previous IVFs with another doctor and 4 failed embryo transfers that lost 7 embryos, Dr. Duke finally got one to take and we are forever grateful.

The clinic added a texting capability that made communicating with them about questions and concerns SO much easier. They had my questions answered almost immediately. The phlebotomist, Marika, was always amazing. I have a hard time with the needles due to medical trauma and she was always kind, patient, and comforting. If someone can’t help you with your progesterone shots, the nurses there will help during business hours. They’re 100% in-house, which was nice because when I woke up from anesthesia, it was helpful to see my partner there rather than be in a separate hospital recovery room.
Describe your experience with your monitoring appointments at Nevada Fertility Institute.
Hope was amazing. Very kind and thorough and answered all of my questions.
She [Dr. Duke] is a smarty, honest, and educated woman. ... Her caring attitude and honesty was apparent from the first appointment and we love her! ... She diagnosed me with PCOS. So we started with IUI treatment. She put me on clomid and provera. I had regular ultrasounds to monitor follicle growth. Then I started I injections of follostim to help grow follicles to a good size.
They [Nevada Fertility Institute] are very committed to providing you with excellent care and communication. Even the from desk lady is so nice and helpful! Our only complaint was with the financial coordinator. She made a comment that we should get a job with Starbucks or amazon to help pay for fertility benefits. We allready have careers, so we were a little Put off by that comment.
How was your experience with Cindy Duke at Nevada Fertility Institute?
I really don’t see any weaknesses. She is a smarty, honest, and educated woman. You can tell she cares about you more than other docotors. We are looking forward to success with her! Our last doctor didn’t recommend IUI before IVF, and they also didn’t tell
Is IUI was covered by our insurance...our first appointment with Dr.Duke she was open and honest and hopeful, her positive attitude and sincere care for us was so refreshing!
What's one piece of advice would you give a prospective patient of Cindy Duke at Nevada Fertility Institute?
Enjoy her positive attitude!

Describe your experience with Nevada Fertility Institute.
They are very committed to providing you with excellent care and communication. Even the from desk lady is so nice and helpful! Our only complaint was with the financial coordinator. She made a comment that we should get a job with Starbucks or amazon to help pay for fertility benefits. We allready have careers, so we were a little
Put off by that comment. We called Dr.Duke to let her know and it turned out to be a misunderstanding. She did not mean to come across the way that she did and quickly apologized.

Other than medications and co-pays the IUI was covered by insurance. 40$ co pay every visit and about 700-800$ in medication.

Describe the specific things that went wrong at Nevada Fertility Institute.
Just one time the nurse forgot to call in my prescription. I called and it was resolved right away.
Dr Duke is very knowledgeable and helps me understand things that I may be unsure of. She explains treatments and procedures thoroughly and is always there if I have questions which I often do. She makes eye contact with me and on numerous occasions has held my hand and hugged me to put me at ease after a procedure. ... Dr Duke had prescribed estradiol medication along with clomid and hcg trigger shots to help my ovulation process
Weaknesses- nursing staff [Nevada Fertility Institute] need to be on the same page as Doctor Duke. Not as responsive and they lack communication skills. Hard to get ahold of nursing team and answers are usually left unanswered till I reach out to the doctor personally ... Had to reach out to nurses few different times to rush my medication to the specialty pharmacy to have it ready for me to take on my overseas vacation. Insurance did not have enough time to approve and I had to pay out of pocket

She makes eye contact with me and on numerous occasions has held my hand and hugged me to put me at ease after a procedure. Dr Duke also went out of her way to cover the cost of my polyp removal when my insurance would not cover it. Not many doctor would do that, this is why I have been going to Dr Duke now for 3+ yrs. I believe in her and I know I will one day have a positive pregnancy result and ultimately give birth to a healthy baby.
What's one piece of advice would you give a prospective patient of Cindy Duke at Nevada Fertility Institute?
Don’t give up on yourself, because Dr Duke will not give up on you.

Dr Duke is a compassionate doctor, very easy to talk to and makes me feel at ease it a very stressful time in my life. I felt that she would be the doctor that would ultimately help me get pregnant. It’s a work in progress as we came close at one point during my 4th cycle but ultimately led to a chemical pregnancy
Describe the protocols Cindy Duke used in your cycles at Nevada Fertility Institute and their degree of success.
Dr Duke had prescribed estradiol medication along with clomid and hcg trigger shots to help my ovulation process. I only have 3-4 eggs to work with and these medications helped my eggs to mature . I also took crinone twice daily to help prep my uterus for pregnancy.

Unfortunately Dr Dukes are not on the same professional level as she is and are a bit unorganized and noncommunitive. It’s hard at times to get answers from her staff but fortunately but Dr Duke is accessible to me when I need to speak to her directly which is very helpful

Dr. Duke is extremely nice. The treatment process wasn’t explained in as much detail as we would have liked and felt like it wasn’t tailored specifically to our needs initially. It get like we were pushed toward IVF based on numbers and less on our personal needs. ... She recommended transferring two embryos before we even started IVF and encouraged it even when we expressed concern about transferring more than one. We have not decided how many we will transfer if we get this far.
We went in for a follow up appointment with a change in insurance and they [Nevada Fertility Inst] thought we were new patients and had us fill out unnecessary paperwork. They didn’t schedule a necessary procedure before starting IVF and called us to add it to another appointment we had. They forget to order prescriptions a few times and told us prescriptions would be covered when they weren’t.
How was your experience with Cindy Duke at Nevada Fertility Institute?
Dr. Duke is very compassionate and candid. She presented our options thoroughly and left the decision to us. We would have liked more detail on the process and what to expect, as well as our treatment plan. We didn’t receive a full explanation as to what our IVF protocol would be and the reasoning.
What's one piece of advice would you give a prospective patient of Cindy Duke at Nevada Fertility Institute?
Ask lots of questions.

Describe your experience with the nursing staff at Nevada Fertility Institute.
Nursing staff would call multiple times to relay the same message. Had trouble reaching them to ask questions on occasion. They were very compassionate but we would have preferred more communication.
Describe your experience with Nevada Fertility Institute.
Weakness is communication. It is hard to get ahold of anyone as they don’t answer the phone. You need to follow up as they sometimes forget to call in prescriptions. Also be careful with finance and billing. We were told our insurance covered the treatment and medications which was not accurate and we did not find out otherwise until the Pharmacy called asking for a LARGE payment.
Describe Cindy Duke's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Nevada Fertility Institute.

I felt that I didn't know everything about the prospects for pregnancy for my wife and feel like we got vague and unrealistic responses to our questions about success factors... I later learned that in our case, it would likely take 3-5 IVF cycles due to age ... had we been given this honest evaluation by the Dr. [Cindy Duke], it would have saved my wife and I some very difficult conversations
They [Nevada Fertility Institute] were honest to the point of telling us that IVF would likely take 2 cycles and that there is a measured approach to the procedure so that we didn't spend if we weren't going to have a chance at success, I later learned that in our case, it would likely take 3-5 IVF cycles due to age
How was your experience with Cindy Duke at Nevada Fertility Institute?
I felt that I didn't know everything about the prospects for pregnancy for my wife and feel like we got vague and unrealistic responses to our questions about success factors. I didn't know what questions to ask at the time and though I expressed a need for details, only the questions I asked were answered, but the likelihood of success in our case seems glossed over given what I now believe I know about it.
What's one piece of advice would you give a prospective patient of Cindy Duke at Nevada Fertility Institute?
Education! Have a web site and materials that teach. Verify with patients that they have reviewed the materials and understand the costs and potential for success.

I was given clomed as was my wife. My sperm was tested and though apparently low in count they were of good quality. My wife was also tested for various conditions prior to treatment. All of these were very expensive.
Describe your experience with the nursing staff at Nevada Fertility Institute.
Nursing staff was fine. However again, when we tried to assess our chances for a successful pregnancy, that was very difficult to get out of anyone. They were honest to the point of telling us that IVF would likely take 2 cycles and that there is a measured approach to the procedure so that we didn't spend if we weren't going to have a chance at success, I later learned that in our case, it would likely take 3-5 IVF cycles due to age. I discovered this by watching the FertilityIQ videos, and had we been given this honest evaluation by the Dr., it would have saved my wife and I some very difficult conversations.
Describe your experience with Nevada Fertility Institute.
Very clean and professional environment. Staff pleasant to work with, private room specifically for sperm donation (not a bathroom as I've heard about at other clinics).
nothing else, you should remove the 200 minimum character requirement!
Describe the costs associated with your care under Cindy Duke at Nevada Fertility Institute.
Quoted about $20k per cycle for IVF, about $2k for IUI
She [Dr. Cindy Duke] told me the next steps but as a person who did not do all the research I had to figure out what the terms meant that she had used...She remembered my spouse and my names...She was very kind and compassionate... The next cycle Dr Duke changed some things up...trigger injection site, days till IUI etc and no pregnancy...We are going to do our 3rd cycle soon
It is very difficult to get thru on the phone [to Nevada Fertility Institute] if there is a question or concern. It's hard when you are starting a new cycle and trying to tell them or to find out about the trigger meds and instructions...I feel it would be better to have I primary nurse for each cycle...I have spoken to a few different ones...I do have a email for one which is great and she is so very helpful

The next cycle we skipped because of Christmas
The following cycle we did IUI and were so surprised to get a positive test, the numbers were not good and the pregnancy was over before it started.

In the beginning we were told my husband needed to bring his sperm sample to their office but when he got there they told him no he had to go to the lab and drop off his sample...he was very upset and very frustrated
Also there was a mistake telling us we had an appointment but it was actually just blood work for myself
From day one Dr. Duke was warm and inviting. She spoke to me kindly and showed compassion when discussing my diagnosis and the side effects that come along with PCOS. She assured me that the issues I've experienced weren't my fault and that was a first for me since other doctors had been very cold about my weight and the other issues that come with it. Dr. Duke spent time with me answering questions and never rushed any consults or appointments I've had with her.
The strengths [Nevada Fertility Institute] are compassion and ease of scheduling. Everyone is so kind and they are all very friendly. They remember your name and treat you like friends when interacting with you. It's also very easy to get in for an appointment and move things along smoothly. One opportunity I've seen is wait time. Sometimes we had to wait a while after our appointment time but it was never anything awful.... They were easy to communicate with but from time to time difficult to get a hold of. When my medication changed the nurse drove 30 minutes to my home to get me the correct medication which amazed me!
How was your experience with Cindy Duke at Nevada Fertility Institute?
I did have questions from time to time that weren't answered before leaving and that required me to call and ask for clarification. My medication was switched with no explanation from the doctor and the communication for this came from my nurse, this was the only hiccup I ran into and it was easily fixed and discussed just after the fact.
Dr. Duke was always optimistic and kind when working with my husband and I. She takes time to speak to you about concerns and risks and she speaks to you like you are knowledgeable and allows you to ask questions without being frustrated in you asking.
What's one piece of advice would you give a prospective patient of Cindy Duke at Nevada Fertility Institute?
Just ask questions. If you are unsure about what is happening or why, ask. You can discuss multiple options with her and she will give you her honest opinion. Don't be afraid to discuss treatment options she is open to many and very knowledgeable.

I started with letrozole 7.5 mg CD 3-7 and started estrace 2mg twice a day starting on CD 9. We were initially prescribed a different trigger but it changed the night of the trigger to preglyn 10,000. Endometrin 100mg twice a day starts two days after IUI

At the start of my cycle my medications were not called in like they should have been so I had to call and request them and right before my IUI I had to request my trigger shot, again which made it all rushed but they got on it quickly after I addressed it.
Dr. Cindy Duke was amazing! She answered all our questions, no matter how small or large the questions were. She explained everything to me and she truly wanted the best result for us, and found every possible way for us! She didn't give up on us.... She is a strong believer of single embryo transfer because she wants the best chance and opportunity for both the embryo and the mother to be.
I am not sure if communication failed because it [Nevada Fertility Institute] was a new establishment or something else, but email communication with our care coordinator was a hit or miss with me. My husband seemed to have better luck with her than me, but even at times, he would get frustrate by the delay of time she would respond. There were multiple times where we had to cc'd Dr. Duke to get a response from her or directly from Dr. Duke

The clinic is still new but we were there from the start. I can't say anything really negative about the clinic as Dr. Duke has made us feel like we matter and we are not just a number or a paycheck. As the clinic gets more establish, we notice the wait to see her at our set appointment gets delay a bit, but nothing to where I would say it's a bad thing because we always had her taking care of us instead of some nurse from the first place we went to.

She [Dr. Cindy Duke] is VERY personable. Clear directions....We are dealing with male factor infertility. My husband started taking clomid and his counts went up a little. My husband then started taking HCG shots with clomid and blood work went well. We have been on it for two months (hcg) and hope to be pregnant in a couple months. Still no positive pregnancy tests, but my husbands numbers have improved...Bedside manner is great, she is so nice and left me feeling hopeful! I wish she would call between visits. When we would go into the office it was always a great visit, but when we leave and post-visits, I always felt lost in steps to take next.
Nurses [at Nevada Fertility Institute] told us the wrong information. It was VERY frustrating...$100 every time we went in. $25 clomid a month and $365 HCG a month...Very clean, cute decorations, welcoming and fresh. We only went through a IVF consult so far [with Dr. Cindy Duke]. I am needing to do the SIS testing and the go from there...I asked how many she would implant and she said 2 would be the maximum for my age...The minute I stepped foot into Dr Dukes office, I was a human! Dr Duke spent the time to go over everything and answer our questions...does care about her patients and makes sure she lets you know what to expect. My last Dr was all about the payment but Dr Duke understood the journey of infertility. She is hands on and very detailed with the treatments.
They [Nevada Fertility Institute] do offer military discounts for the IVF procedure and the medications...They are educated and empathetic to the journey you are on. They will be there for you every step of the way. You are not alone. I felt very comfortable and ready to proceed with our IVF treatment. I felt like I was finally on the right path...They [nurses] were GREAT! I’ve already emailed Kelly twice the next day after our consult and she has been very responsive.

At times, she would often give information that I did not know I wanted to know...felt like she had all the time in the world for us...During times of setbacks, she was empathetic and compassionate about the experience, and optimistic moving forward.
Spending multiple days a week at the clinic, you want to be somewhere you feel safe and comfortable; this [Nevada Fertility Institute] is the place. Despite having a lot of clients, they always remembered my name, who I was, and random facts that made me feel like I was visiting friends...I was given the time I needed to feel comfortable, ask questions, and calm any nerves I might have had. At times, the staff went above and beyond my physical medical needs, and ensured my emotional health needs were meant as well.

I was always addressed by my first name, and details of my medical history were remembered, never asked multiple times.
Describe the protocols Cindy Duke used in your cycles at Nevada Fertility Institute and their degree of success.
Egg Retrieval: We used Gonal, Menopur, and eventually Centrotide during stimulation. I was frequently monitored so that the timing would be accurate and optimal. I triggered with Lupron and Pregnyl. Due to my blood work and high number of follicles, adding Lupron to the trigger was to help avoid OHSS I believe.
FET: Our first attempt at an FET was a fully medicated cycle. As I followed OHSS protocol after the retrieval, this was to avoid it again. I was frequently monitored. Unfortunately, my lining did not grow, and that cycle was cancelled. I used Lupron for supression, and then Estraidol tablets, patches, and injectable estradoil to stimulate lining growth.
Our second attempt at an FET was a medicated natural cycle. I took letrozole for 5 days, then used Gonal to stimulate, grow follicles, and grow my lining. My lining did not reach 8 mm before my progesterone naturally spiked. Dr. Duke decided to cancel that cycle as well due to not being able to verify the timing. While this was not the desired outcome, we potentially and likely would have not had a successful cycle.
Our third attempt was cancelled before we got started as my estrogen was too high and needed more time to drop below an optimal threshold.
Our fourth FET was ultimately successful. We followed the same protocol, except we added in Centrotide earlier than previous cycles. My lining reached 8mm, we triggered, and transferred one embryo. Our BETA was positive after this FET.
Describe your experience with your nurse at Nevada Fertility Institute. (Assigned nurse: Kelly, Stephanie)
The staff is amazing! They all take their jobs and role in helping you grow your family very seriously. I was assigned a nurse for each step of my procedures, and was able to grow a relationship during each step of the process. This allowed me to gain comfortability, and openness with the staff.
Describe your experience with Nevada Fertility Institute.
Fertility is one of the most intimate and emotional things I have gone through. I could not imagine going through it with any other practice than Nevada Fertility Institute.
From the moment I booked an appointment, so our final appointment, we felt like we were in the company of family. Appointments were easy to make and schedule, and the knowledge of Dr. Duke and her staff is unmatched.
Ultimately, we chose to pursue IVF to grow our family. Spending multiple days a week at the clinic, you want to be somewhere you feel safe and comfortable; this is the place. Despite having a lot of clients, they always remembered my name, who I was, and random facts that made me feel like I was visiting friends.
Each time I would speak to my nurse at night, I was given the time I needed to feel comfortable, ask questions, and calm any nerves I might have had. At times, the staff went above and beyond my physical medical needs, and ensured my emotional health needs were meant as well.
Going through all this during the COVID pandemic as well created its own unique set of challenges. Their office was always extremely diligent about following research based protocols, and holding all patients and staff to them. I never felt exposed, or anything when going to the clinic multiple times a week.
Describe the costs associated with your care under Cindy Duke at Nevada Fertility Institute.
The clinic did a great job coordinating with our insurance as much as possible.
Describe Cindy Duke's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Nevada Fertility Institute.
We chose to transfer one tested embryo. We were consulted on the statistics of transferring two embryos, the impact that twins can have on the body, and the potential risk to mother and baby.
met directly with Dr. Duke to discuss our options available through her clinic...Personally, we knew that using an egg donor would be best for us because of our failed attempts with my eggs in the past...My husband and I left the meeting with somewhat of a renewed hope in our hearts and minds, again thinking that we actually had a chance at being parents...The egg donor process moved quickly and efficiently; we were able to discuss a small set of parameters and come to a decision within about a week
Personally, I had no issues with [the Nevada Fertility Institute] clinic. I made my appointments for first thing in the morning and prepared for a small waiting period. The office runs very efficiently, has a small staff performing their specific duties, and responded to all phone calls within a day...From start to finish, with the egg donor included, we've spent approximately $28,000 while under the care of Dr. Duke / NFI.
How was your experience with Cindy Duke at Nevada Fertility Institute?
My husband and I first met with fertility specialists in 2015 in Colorado; during that time, every step we took was in the backwards direction. We continued to receive bad news and nothing was working for us so we had stopped trying. When we first met with Dr. Cindy Duke at Nevada Fertility Institute (NFI), we had very little confidence in my ability to get pregnant and stay pregnant. As a matter of fact, I had recently spoken with my GYN about an early hysterectomy so I could finally put the fertility matters behind me, but she referred us to NFI instead for further evaluation before she would consider the hysterectomy for me.
At our first meeting with NFI we filled out paperwork, health history, wrote down our goals of being parents, had some blood drawn and then met directly with Dr. Duke to discuss our options available through her clinic. Personally, we knew that using an egg donor would be best for us because of our failed attempts with my eggs in the past. Once we made that decision, it was a brisk talk through the necessary testing which would identify for her any lingering issues within my body. My husband and I left the meeting with somewhat of a renewed hope in our hearts and minds, again thinking that we actually had a chance at being parents.
After just a few weeks of private discussion in our house and within our family, we all decided to move forward as soon as possible (I'm mid-30s and we all know there are extra risks as an "older" mom-to-be.) The egg donor process moved quickly and efficiently; we were able to discuss a small set of parameters and come to a decision within about a week. Once the egg was received in the office, my husband provided his sample, and it was just a matter of a few days when we received the call with our embryo count. I couldn't believe it! We successfully had embryos to work with and a healthy body cleared for transfer.
One of the best things to know about Dr. Duke and NFI is that they don't really leave anything up to chance. The science is down to an art form and once we were on the medication schedule and lined up for the trial round, it was up to us to simply stick to the routine. Follow the directions, take the injections, show up for the blood work & ultrasounds. Honestly, compared to our last experience in Colorado, this felt seamless. We knew we had the entire team helping us through every single aspect of the IVF process; we felt supported and encouraged.
It is now December 2020 and we are 18 weeks pregnant. Our first attempt at frozen embryo transfer resulted in one successful pregnancy, with no complications. We remained in the care of Dr. Duke and NFI until Week 12, graduated and then sent on our way to grow this baby under normal OB supervision and circumstances. As a woman who's been receiving Abnormal test results for nearly two decades, I feel like our time at NFI was like being under a canopy of Magic. I feel like hearing the words Normal and Pregnancy in the same sentence are all I need to continue to be happy. It is only with the care and attention that we received from Dr. Duke and the entire team at NFI that our dreams to be parents were made into reality, and we look forward to seeing the whole team again when it's time for us to transfer our second embryo.
