Consultation, blood work and genetic testing were covered under my PPO insurance. My partner had kaiser and we got billed an insane bill for in house bloodwork that we could have taken the orders to Kaiser for at less cost.
Sperm collection and testing seemed reasonable, but all of it is expensive given that insurance does not cover most fertility related tests or procedures.

We utilized a known donor and there were many complications with having him access the clinic for donations. Also, we have had to prompt for test results, prompt for next steps, and prompt and inquire about our donors results, donation, appointments, bills, etc.
Dr Evans is super thorough and information based. We never left feeling confused about next steps or procedures or medication. If you have a question she will answer you in an email. When we were particularly stressed about a result, she took the time to call us and explain how it was not as bad as we were making it out to be. Also, we got results with Dr Evans, she is extensive in her testing and making sure your treatment accurately follows your personal diagnosis...

Two egg retrievals to make embryos using icsi. Drugs used. menapur, follistim and ganerelix. Lupron or HCG trigger. Embryos were PGTA tested
Transfer for wife estrogen pills, vaginal progesterone suppositories, Progesterone intramuscular injection.
Both me and my wife have adverse effects to birth control and she adjusted our egg retrieval protocol accordingly to not use birth control.
How competent was Michele Evans at LGBTQ+ care?
If I could give better than A++ I would. She respectfully asked for pronouns and gender identity and never ever mistook one of us seeking treatment as a straight person. This had happened multiple times at another clinic. The default can be to ask someone "your husband" if you present as a woman. Our charts were also linked from the moment we started treatment with her. She understood that this was an endeavor for us to make a child together. She remembered our unique circumstances and LGBTQ identities and respected them.

very thorough and explained all injections several times and provided calendars and always responsive to emails.

Strengths: Lgbt friendly, organized, responsive, science based but takes whole patient into account (mental health etc), easy scheduling, personalized care.
Weakness: only thing we can think of is if you cant travel, you have to go to westwood for retrievals and transfers. This was not an issue for us but something to note

When you have an appointment, [Dr. Evans] sees you 100% of the time. Every ultrasound is with her, not a nurse or PA. She knows you on a personal level and explains things in a very clear manner...She is optimistic in her tone but never gives false hope...it seems when it's not so great news the doctor will always call you to really talk through why something happened and a game plan.

Dr Evans explains things in a very clear manner. She also is very even keeled. She is optimistic in her tone but never gives false hope. I also appreciated (and i've noticed this because In the journey I have gotten both good and bad news) is it seems when it's not so great news the doctor will always call you to really talk through why something happened and a game plan. When it's good news, the nurse will call you. I like that the doctor always gives her time on the phone when it might not be great news.
I also liked that Dr Evans is technical and explains a lot of the process, but also does not bog you down with unneccessary details to avoid overwhelming. For instance, after PGS testing, I knew which embryos were euploids but she didn't tell me grading because I guess after genetic testing grading doesnt matter as much. She gives you just enough to stay hopeful and informed.
I had an egg retrieval that didn't go as great as we hoped. She retrieved a ton of mature eggs (23) and they matured to 15 blasts but my egg quality was terrible and I ended up with 1 euploid. She really steered me to the best possible outcome and I am now pregnant with my one euploid. She recommended an ERA to give it the best shot, and after discovering endo put me on a 2 month Lupron depot. My first and only embryo took and I am not 9 weeks. I pray this baby sticks but I credit the success so far to Dr evans steady steering of the ship and right recommendations along the way.

You might have to wait a few weeks for a consultation but she is worth the wait. She has a steady hand and you absolutely want her guiding you on this journey.

Dr Evans and her staff are all incredible. When you have an appointment, she sees you 100% of the time. Every ultrasound is with her, not a nurse or PA. She knows you on a personal level and explains things in a very clear manner. I had a different IVF doctor with an office ran like a factory (due to his treatment of a certain kardashian sister(s)), after 2 visits of being treated like a farm animal, I had a consult with Dr Evans and never looked back.
Not just her, but my nurse practitioner Ana was incredible and the insurance approval person Oyira was wonderful as well. The staff truly cares about the patients and are responsive and kind. They are also there for you and your questions. The office also never had more than a 5 minute wait. They don't overbook their patients. You may have to wait for a consultation but I am so glad I waited a few weeks to do it. There is a wait because she's not overextending herself like many IVF doctors in LA, she is dedicating appropriate time to each patient.

I was on Gonal, Menopur, and Omnitrope for stims. We got 23 eggs retrieved, 15 blasts, whittled down to 1 Euploid and 1 Low level mosaic.
We then did an ERA, 2 month Lupron depot prepping with Steroids, Crinone inserts, PIO shots, and estrogen pills.

Obsessed with Ana. She was responsive, kind, knew her stuff and gave clear and concise directions for the meds. I have emailed her about everything under the sun and she is always patient with me and very responsive in a timely matter. I credit her as much as Dr Evans for helping me make this baby as she made a stressful journey very low stress.

My PCOS diagnosis results in many polyps and Dr Evans has always scheduled hysteroscopies and has always made sure she sees me for both pre and post op. Her staff will call to check in and schedule next appointment for what comes next for treatment. Starting from zero we have now done one cycle of clomid which she monitored throughout. After it failed she immediately had a new plan in which she wants to see if my body will give itself a chance. She then prescribed Provera to start fresh with a round of Letrozole. She will then monitor again to see if my body responds if at all.

Dr. Evans has explained that once we are ready to transfer (3embryosRemaining) she will determine the amount of embryos to transfer once she has thawed and checked their quality. Since the embryos were retrieved by our previous RE this is the way she will decide quality and amount of embryos to be transferred to what can potentially be our last FET transfer
I did not do a retrieval with Dr. Evans because I brought my frozen embryos to her clinic, so I can only assess her treatment for a frozen embryo transfer cycle and an ERA cycle. ... Dr. Evans came up with a plan and we proceeded to do an ERA cycle and added a biopsy to test for endometriosis and endometritis. The biopsies came back normal and the ERA showed I needed just 4 more hours of progesterone.
PFCLA Glendale is the satellite office for PFCLA in Westwood so the only downside (if you dont live near Westwood) is you have to go to the Westwood office for transfers and retrievals. ... They promptly return phone calls and emails about any/all concerns and I never feel rushed with them at the clinic. ... However, these are not frequent visits so it is 100% worth enduring the traffic! The Glendale office is small, clean, quiet, and very lovely. It's very convenient location to folks in the northeast/downtown area of LA.

We switched clinics and chose Dr. Evans based on her stellar reviews from other patients. After our first consultation, my husband and I agreed that she was the most compassionate and professional RE we had seen. She was incredibly smart and up to date with the research in her field and she never treated us as just another number (our experience with our first clinic). She took the time to review our case, make adjustments to our protocol, and in addition, she was flexible when I asked to add certain drugs, etc to our protocol. She informed us of all risks, outcomes, percentages, etc and never sugar coated the information in a way that made false promises, however, she has *always* been hopeful and optimistic for us (after having two failed FETs with PGS embryos at our first clinic). Furthermore, she is incredibly attentive and responds to all questions via email in a very prompt manner, which is not my experience with our first RE. It is obvious she truly cares for her patients and their wellbeing, and we are so so happy to have made the switch to Dr. Evans and her clinic.

Don't be afraid to ask her any and all questions about your treatment. She breaks down all the information in a palatable way and will be upfront about your case while also being very supportive. If you need an RE who provides personalized care AND excellent professional medical treatment, she is the RE for you.

Dr. Evans and her staff and incredibly professional and provide personalized, attentive care with me and her other patients. She takes the time to answer all of my questions, concerns and does not sugar coat anything but rather provides information in a professional yet caring, supportive manner. I have seen 4 REs and she is by far the most incredible one. She has a perfect blend of professionalism and expertise in her field with personalized care and human quality.

I did not do a retrieval with Dr. Evans because I brought my frozen embryos to her clinic, so I can only assess her treatment for a frozen embryo transfer cycle and an ERA cycle.
After 2 failed FETs with PGS normal embryos at our first clinic, we felt frustrated and needed a better RE who would examine our case carefully. Dr. Evans came up with a plan and we proceeded to do an ERA cycle and added a biopsy to test for endometriosis and endometritis. The biopsies came back normal and the ERA showed I needed just 4 more hours of progesterone.
We then prepared for our first FET cycle with Dr. Evans (third one overall) and she decided to change our protocol a bit to address any potential immune or blood clotting issues. She added dexamethasone, lovenox, and claritin/pepcid on top of daily estrogen pills. She also timed my transfer *perfectly* to 124 hours, which is where I tested receptive based on the ERA results.
I am happy to report that I am pregnant from the first FET with Dr. Evans! I truly believe that she made all the difference in our ability to get pregnant. We know we still have to pass a few tests but this is a huge milestone for us and our very first positive pregnancy. I am convinced Dr. Evans was the reason why I am pregnant my first FET with her (and 3rd FET overall)!! :)

Dr. Evans cautioned against this and gave us all the statistics of high risk pregnancy that twins can pose. Thank goodness we followed her recommendations because we ended up pregnant with one pgs embryo! :)

There was just one minor incident with my progesterone instructions - Dr. Evans and the nurses mixed up the appropriate dosage for progesterone during my ERA cycle but it was not a major mix up and I was still able to continue my ERA cycle. It was only one minor incident and thankfully it did not affect our treatment! It was a human error!
Dr Evans connected with us like individuals with our own journey. We had recurrent miscarriages before becoming her patient and she expressed compassion and sympathy for all we’d been through. She suggested screenings and tests to figure out more about our history and gave us advice based on research. She is the perfect blend or heart and brains. Dr Evans did IVF with PGS with us. We have a history of recurrent pregnancy loss due to chromosomal abnormalities in the babies, so we did PGS to make sure we were transferring a normal embryo. For the egg stimulation we used menupur and follistim. During the transfer phase we used progesterone injections and estrogen pills.

I felt like I had to manage tests and plans to keep the process moving or else they would’ve forgotten to check and our cycles would get pushed back (it happened towards the beginning of working with the clinic.) We were also informed too late that tests needed to be repeated before a cycle could begin (because the staff “didn’t know” we were doing another cycle, even though that was the plan with Dr Evans), which also would’ve also pushed back our cycle another month (even though we were waiting the previous month for other test results, we were never informed that we needed to repeat tests to move forward with the next cycle). so many blood tests to start off with. Putting me on Levothyroxine .025 mg to help lower my thyroid level. Started me on prenatal vitamins, CoQ10, Vitamin D. Had an HSG test (found polyps- had them removed) Took 2.5 mg of Letrozole for all 3 IUI cycles. Monitored with ultrasounds and used trigger shot (ovidrel). On my 3rd IUI after a positive pregnancy test I did Estradiol 1mg (2x) and Crinone suppositories (2x). Our protocol will change in the future when I try again.

Going through IVF treatment has been one of the hardest things I've had to go through in my life. My chances of conceiving were unclear based on my low AMH, high FSH, and low AFC for my age (36 years old). Throughout my treatment, it transpired that my egg quality was poor.
Dr. Sahakian brought a wealth of experience and skills to the table. His communication style was direct yet empathetic, and he provided swift and precise answers to my questions. From the start, he was open to different options, including using my own eggs versus a donor’s eggs from abroad, which helped me stay as hopeful as possible.
The appointments were short, so I had to go in armed with questions as there wasn’t a lot of room to respond to what was often bad news in my case. Sometimes this felt rushed and I came out of them feeling like I had more questions to ask. On the upside, Dr. Sahakian was very responsive to follow up emails, and was happy to hop on zoom calls to help my husband and I process complex and tough information.
He was particularly supportive when I had a big bleed early on in my pregnancy and it was 50/50 as to whether I would miscarry. He was available by phone throughout the evening that this happened giving my husband and I advice, and booked us in the next day for a check-up. He saw us weekly after that until things stabilized (I was fortunate enough not to miscarry).
I’m not sure that Dr. Sahakian’s sense of humor is for everyone going through this treatment. Personally, I found most of his jokes to be amusing, well-timed and to lighten the moment making everything more human, but I could understand that this wouldn’t be the case for some. It all depends on what style works for you going through a process like IVF.

She also gave me her email in case I wanted to email her later. That was a very special moment and still brings tears to my eyes.

I found the clinic to be very knowledgeable, flexible, and dedicated to achieving success wherever possible for their patients. Every decision was made based on how the IVF cycle was going; egg maturation and retrieval timing was held as sacrosanct. If it was too close to call when a medication should be tweaked or introduced in the cycle, then the clinic brought you in for extra check-up appointments to be sure. If those fell on a weekend or holiday, no problem. The key was to get the timing right so that we had the best chance of retrieving the maximum number of mature eggs. In my case where my AFC was low and egg quality poor, this was everything.
They also accommodated a back-up option using donor eggs from abroad and worked closely with me to arrange those logistics alongside my own treatment. I was impressed at their commitment and flexibility around this arrangement.
I personally would have liked more in-depth onboarding to the upcoming process beyond an email from my care co-ordinator. IVF demands financial and physical commitment, and it’s time-sensitive. I’m the type of person who likes to understand the broad framework upfront. Thus, I would have preferred an introductory call and a sample treatment calendar with potential injection timings to frame our expectations and allow us to roughly plan around upcoming commitments.

After an unsuccessful first IVF cycle, the doctor confirmed that I would stay on the same medications and dosages for the next round as my body responded well (within what it is capable of doing), yet when I received the medication breakdown from the pharmacy, there was a new medication listed. Upon checking, I discovered that the doctor subsequently recommended adding this new medication but I hadn’t been informed by my care co-ordinator.

Hes checked out...Doctor didnt show to transfer without letting us know. He said he forgot to tell us he would be in hawaii. Further he told our surrogate shes crazy, my mouth fell to the floor. Further he refused to elaborate on his 42% success rate with me other than he doesnt guarantee anything. When I pressed for embryology reports I was told to take a hike in those words. However he added if I ever want to have children I need to do another cycle IMMEDIATELY. Yes i made 2 perfect embryos but the doctor sold the clinic mid cycle and checked out completely. Wait 2-3 more years until the clinic resolves the transition from the sale of the clinic and absolutely monitor clinic results. To be honest I would tell people to run for their lives!

Very weak communication, billing personnel are very disorganized and always reflect balances were owed after I had paid. I literally kept payment receipts on me every time I went to the clinic I knew someone was going to tell me I owed a balance. It's really really bad!
