My wife and I had a known donor who was willing to be tested and go through the works at the clinic, to help us save on costs and [Dr. Mark Ransom] highly discouraged that. So much so he talked us into using frozen donor sperm from a sperm bank which added $1300 to each cycle...is very kind and understanding. He seemed to genuinely care about me as a patient. He listened to my concerns and worries and answered all my questions without judgement.
The nursing staff [at ETSU] has been the biggest negative with this office. There are two nurses who work with Dr. Ransom. One of them has been very kind, compassionate and open minded. The other has zero personality and has not been kind...This nurse does not approve of LGBTQ and so we have felt the brunt of that...The clinics strengths are quick appointments. You will never sit in the waiting room longer than 10 minutes.
How was your experience with Mark Ransom at Quillen Fertility & Women's Services (ETSU)?
Dr. Ransom is very smart and compassionate. He takes the time to listen to your concerns and answers all questions. The only issue I saw with him was the initial consultation. My wife and I had a known donor who was willing to be tested and go through the works at the clinic, to help us save on costs and Dr. Ransom highly discouraged that. So much so he talked us into using frozen donor sperm from a sperm bank which added $1300 to each cycle. We were wanting to pay for all the known donor sperm testing out of pocket and the donor was 100% willing to comply and instead of trying to work with us he pushed against it. Otherwise, he is very knowledgeable and has been very nice.

Dr. Ransom is very kind and understanding. He seemed to genuinely care about me as a patient. He listened to my concerns and worries and answered all my questions without judgement.
Describe the protocols Mark Ransom used in your cycles at Quillen Fertility & Women's Services (ETSU) and their degree of success.
Dr Ransom did the initial testing; labs, ultrasounds, fem vue. And determined all my labs were normal and my Fallopian tubes were not blocked. He then proceeded to offer iui since we are a LGBT couple. This resulted in 3 cycles of using 100mg of clomid and one trigger shot. Each cycle he said I responded beautifully. Making 3,3 and 4 eggs each cycle. He didn’t really have an explanation. Prior to this pandemic during the last cycle the ultrasound revealed I had a small ovarian cyst that was less than 2mm. He said it was not surgical and would likely be reabsorbed within the next month. He offered birth control to speed up the process, so we did that. I finished the month gave them a call and they are no longer doing treatments due to the Covid 19. His plan was to try letrozole and add fsh shots. We are now just waiting until we can see him again.

The nursing staff has been the biggest negative with this office. There are two nurses who work with Dr. Ransom. One of them has been very kind, compassionate and open minded. I have had friends who have gone through fertility treatments at this office and they loved this particular nurse. So much so they became friends outside work. This nurse does not approve of LGBTQ and so we have felt the brunt of that. She isn’t kind, doesn’t explain things, has an attitude and it’s just not how I like to be treated. Every time we go we pray we have the other nurse.

The clinics strengths are quick appointments. You will never sit in the waiting room longer than 10 minutes. Once you’re back in a room Dr. Ransom comes within 5 minutes. The weakness I have seen is communication. I work as a full time nurse and can’t control my schedule so sometimes the date of lab draws has fallen on a day I can’t get off. They agreed to send the lab order to my Hospital so I could just get it drawn there. It went smoothly until I never heard back from Dr. Ransom about my levels. I called the office and they said they never received the fax and had me call the hospital and have them fax it. As a nurse, they should have been looking for this lab. Had I not called to ask about it they would have never noticed it didn’t arrive. It is not the patients responsibility to call the hospital and request my labs be faxed, that Is the nurses. Also, during this pandemic we never heard or got a memo, email or phone call notifying us that cycles were being cancelled. I had to call and ask for my next appointment for them to tell me it had stopped. Definite lack of communication.

The initial consult just to be seen was $250. My insurance only covered 50% of the diagnostic tests and zero for treatment. We ended up paying $600 for diagnostics. Each cycle for IUI was $1200. On top of that we had to order frozen sperm from an out of state facility which cost another $1300 each cycle. So in total per month we paid $2500 for the iui.

Lab results were not called to me. When I called the nurse to ask if they had received them 2 days later she responded with, they never got a fax so unless they see it on their desk they don’t know. They did not take the time to call the hospital to have it faxed to the office. Had I not called and asked they would have never looked for it or realized it wasn’t there. The office failed to communicate cycle canceling during the pandemic. I called on my cycle day 3 as we had been every month and that was when I was notified that they had stopped all cycles. Major lack in communication to their patients who pay out of pocket to use their services.

I am currently having to take a month of Lo Loestrin FE birth control pills to assist with an ovarian cyst on my left ovary from the second round of fertility meds. Dr. Ransom plans to begin treatment for my low progesterone at the beginning of my third round...The doctor was out of the country on travel and his nursing team did not call me regarding my progesterone results. I called 3 days after and they quickly told me to double my progesterone medication. This cycle resulted in a negative pregnancy test.

My treatment is currently still in progress. My first round was standard Clomid (150 per day on days 2-7). Confirmed negative. My second round, we tried Clomid (150 per day on days 2-7), Follistim 150 on days 8, 10, 11, & 12, and trigger (HCG shot) on day 14. The second round resulted in low progesterone indicators on my blood draws and a negative pregnancy blood draw. I am currently having to take a month of Lo Loestrin FE birth control pills to assist with an ovarian cyst on my left ovary from the second round of fertility meds. Dr. Ransom plans to begin treatment for my low progesterone at the beginning of my third round (which will hopefully begin next week).

Even though we have not gotten pregnant yet I have alot of faith in Dr Random he is very knowledgeable and explains everything he is doing in great detail including the risks and what the chances are that it will take. With every blood draw or anything going on I have received a phone call directly from him explaining what the results were the same day....Dr Ransom recommended we transfer 1 but this first time we wanted to transfer 2 which be did but he explained the risk of multiples.

Everyone in the office including Dr Ransom has treated me very well. From the personal phone calls from Dr Ransom and explaining everything in great detail and answering any and all questions me or my husband has had.

During IUI we did clomid and progesterone suppositories all resulted in negative pregnancy tests. For egg retrieval we did follistim, lupron, steroids, antibiotics (to prevent infection) then progesterone injections when we did our fresh 3 day transfer.

I really like the nursing staff there were only 2 nurses Lacy and Michelle are both great but at the moment they are down to just one so Lacy is having to do the work of two right now which makes things a little more complicated.

I've been very satisfied as a patient, even though I've not yet had the results I'm looking for.

Dr. Ransom is very knowledgeable. He takes time to explain things. My one issue is that he sometimes makes a recommendation but does not thoroughly provide rationale to the patient. I try to be very educated during appointments, and sometimes I lost a little trust because he would not give me all the information or background. I also wish he would give me more details related to treatment plans.... He also left a lot of the "driving" of things to happen to me as the patient, so just be aware that it will be up to you to keep things moving forward, especially in the preliminary stages.
This clinic [Quillen Fertility and Women's Services] is centrally located near the hospital where testing and procedures are done. I've had no problems getting in to see my provider and scheduling appointments to be seen within a reasonable amount of time. One downside is having to go off-site for lab work and blood draws.... If you go to see Dr. Ransom, take lots of notes and come with questions because the office is not going to provide you with patient pamphlets or info sheets. He also left a lot of the "driving" of things to happen to me as the patient, so just be aware that it will be up to you to keep things moving forward, especially in the preliminary stages.

Dr. Ransom and his clinic is the primary infertility practice in the Tri-Cities area. At times, he was very compassionate and took time to explain things. During other visits, he was short and very to-the-point, without providing a lot of explanation. This was frustrating. I also struggled when dealing with his staff. Certain nurses handle his surgery scheduling only, but don't know about in-office practices. I always felt like I was being passed from one person to another and didn't always feel confident in the level of knowledge of the office staff or in their level of compassion.
Describe your experience with your nurse at Quillen Fertility & Women's Services (ETSU). (Assigned nurse: Michelle)
The staff I worked with was a little inconsistent in terms of the care they provided. I worked with Jeanine on coverage issues; she was not always very patient with my questions (despite this being the first time I had to seek care outside of my benefits coverage). I also worked with Michelle, the nurse who schedules all of Dr. Ransom's surgeries with the local hospital (FWCH). Michelle did not always appear to have a clear knowledge of what was going on, which sometimes gave me pause. When I call to speak with her, she consistently needs to check with other office staff members before being able to answer my questions and commonly seems to fully understand what's going on with my treatment.
Describe your experience with Quillen Fertility & Women's Services (ETSU).
This clinic is centrally located near the hospital where testing and procedures are done. I've had no problems getting in to see my provider and scheduling appointments to be seen within a reasonable amount of time. One downside is having to go off-site for lab work and blood draws. I'm not sure if this is true for all infertility clinics, but there has been some instances in confusion about cost and coverage - I commonly have to call multiple places to get quotes to figure out just how expensive a test or procedure will be. While I have not gone through IVF, there was a clear breakdown of costs that was provided to me for that service. My office visits and most labs and diagnostic procedures were covered by insurance, so I was only responsible for paying my co-pay. IVF is not covered by my plan and I was told to plan on having at least $13,000 up front before proceeding with scheduling anything related to IVF. I did have some bad experiences with out-of-pocket costs at the hospital where Dr. Ransom does some of his outpatient testing. I was scheduled for a hysteroscopic septum resection and despite my insurance having pre-approved the procedure as medically necessary, I was told that the claim would likely be rejected. When I inquired to what my total costs would be for this outpatient procedure without insurance, I was quoted at over $25,000. As a result, I had to cancel the procedure. I'm not sure what was driving this cost up, but it did seem incredibly high.

He put me on Lupron to keep me from ovulating for 3 weeks and then added follistim for follicle stimulation while continuing Lupron. I skimmed for 11 days and then used Pregnyl to trigger ovulation before egg retrieval.

Lacy was nice and competent but could have been more compassionate. I felt like I was bothering her when I asked questions. She assisted with most of the ultrsounds. Michelle, the other nurse was wonderful and seemed always hapultrasounds. She assisted with medications and was there for egg retrieval and embryo transfer.

I started Lupron 20 units daily twenty-one days after my period. This was to suppress ovulation. I was on that for 3 weeks. Then I started Follistim to stimulate the ovaries. Then the Lupron was decreased to 10 units. I was monitored about every other day with an ultrasound and labs (estradiol). Depending on those results, he would adjust the dose of Follistim. I stimmed for 13 days. I administered the trigger shot the 14th day. The Egg retrieval was two days later. Post egg retrieval, I started progesterone in oil shots. I also took an oral steroid and antibiotic for prophylaxis. Three days after egg retrieval, I had a 3 day fresh transfer. I continued the progesterone in oil shots until I had my beta test at day 9. Beta was negative, it did not work.

I love Dr. Ransom, his bedside manner is great. He sees a lot of patients and there were a couple of times I had to remind him of our particular situation and procedures I had already had done. That was frustrating for me.... Initially he did the femvue after checking my labs, which were good. The femvue showed both tubes appeared to be blocked. I went in for laparoscopic surgery to try to clear the blockages l, but only one (left) was cleared. After healing, he started me on Clomid and HCG trigger.

I did 3 cycles of that but it’s over an hour to his office and between the cost of the medication and the time it got to be too much. Especially considering my chances were slim with the blockage and my age. He recommended IVF, but it simply isn’t financially feasible for us.

I have PCOS that causes me to not ovulate on my own. When I went to him [Dr. Mark Ransom] I had already given letrozole several tries and was not successful. I was ready to move on to injection medications but he wanted to give clomid a try. He said with his PCOS patients he likes to start pretty minimal because of the risk of over-stimulating. I understood and went with his recommendations. Three rounds of clomid + trigger shot and we still were not pregnant. Ovulating but not pregnant. I tried one IUI while on clomid + trigger. But my husband has no sperm issues so technically we should not need IUI. When that cycle failed, we moved on to a combo cycle of clomid + follistim + trigger shot.
The clinic [Quillen Fertility and Women's Services ETSU] is wonderful. I have not had any issues with scheduling appointments and they are always willing to work around my job schedule. Michelle [nurse at Quillen Fertility and Women's Services] is wonderful. She is informed, knowedgeable, compassionate, and takes time to answer all your questions. There is another nurse who I don't know her name, but she doesn't have much personality and wasn't very warm...[Monitoring appointments were] Very quick and efficient - in and out...Would love if they had extended hours. Also they need a lab in the clinic so you don't have to go somewhere else for blood work. Katie who works at the front desk is wonderful. She is efficient and goes above and beyond to answer questions. They get you in on Saturday if needed (while in a treatment cycle).

We went through several cycles with Letrozole (Femara). Then moved on to IUI, did three (two of which were injectable cycles). At this point I asked about IVF, which we did and it worked on the first try. We were "Unexplained".

As opposed to our previous doctor, Dr. Ransom was VERY friendly and incredibly honest and informative. He was ready to answer any and all questions we had and definitely wanted us to be successful!
Describe the protocols Mark Ransom used in your cycles at Quillen Fertility & Women's Services (ETSU) and their degree of success.
For our fresh cycle, we used started out with birth control and Lupron 20 units a day for two weeks. At this point, we had our baseline and then began the Follistim at 300 units a day for about 4-5 days. At our monitoring appointment, it showed that I was creating way more follicles than we had hoped, so he had my coast for a few days to allow my body to absorb some of the follicles back. We had our egg retrieval on day 10 and a three day transfer of two embryos that resulted in a negative pregnancy test. About 3-4 months later, we did a frozen transfer beginning with Lupron 20 units again for two weeks, then Progesterone in oil intramuscular injections and estrogen taken orally to prepare the uterine lining for transfer. This process was about three weeks long when we transferred our last two embryos...this cycle resulted in our eight month old beautiful rainbow baby boy!

On the consultation visit I felt like he [Dr. Mark Ransom] didn't really want to talk to me but just get my information. The first couple of visits I did feel like he was arrogant and didn't want to help me. However, after going to him for a while and getting to know him he is actually really nice and wants to help. He is very good about returning phone calls. When he calls you with different results it is him who talks to you and not a nurse. I appreciate that.
I used letrozole with the hgc shot [at Quillen Fertility and Women's Services]. Before using them I had to get my thyroid under control because it was off, I also had to have a hysteroscopy and a d&c to remove cysts. I was also on birth control for a couple of months in order to regulate my hormones...They have been good to work with and even know me by name when I come in. They are easy to contact and they get messages sent to the nurses and doctors quickly. They will also call you for a reminder for an upcoming appointment or if they need to change your appointment.

He was proactive and flexible with my treatment when we needed to make changes due to my body’s response. When seeking Dr. Assad it was to determine what was going on with my body. Yes, we wanted a child at the time but that was not at the top of our list. Dr. Assad knew this and I was passed along as quick as possible. My questions did not matter to him. He wrote me a prescription and was done. He didnt care that it wasnt helping.... Dr. Assad had terrible bedside. He was short with answers and my questions never seemed to matter to him. My visits were short for the distance I had to travel. I seemed like a number and all he wanted was my money.

Dont just go to a doctor for location. Go for someone you like and are comfortable with.

Wonderful, great experience, would do procedure again [with Dr. Ransom] . Very nice man, very smart & compassionate. Always felt very comfortable with him and his staff. Very thorough, goes over all the information every visit. Never feel left out...experienced and very caring...Letrozole with HCG injection, IUI procedures failed attempt once.

I was told [by Dr. Mark Ransom] I had a 5% chance or less of ever being pregnant without IVF because I have stage 4 endometriosis and my husband has low motility. I questioned a procedure that would help my husbands motility (varicocele repair) and he told us it likely wouldn’t work from what he’s seen in the past with his many years of experience, so we decided to do IVF instead. $18k later for IVF with ICSI we yielded low embryo numbers, which is not his fault, but when we went for transfer of our 2/3 embryos, he couldn’t even positively identify the catheter in the fundus of my uterus before he concluded it was okay and transferred them anyway. I was swollen still from egg retrieval, borderline OH
The nurse [at Quillen Fertility and Women's Services] was never very friendly...She is very distant and seems like she has no compassion for people, especially going through infertility treatments. I always felt like I was wasting her time and getting on her nerves...I loved the front office staff and they were very attentive and sweet. I never had an issue with scheduling or billing. They made it very easy. They were more compassionate than the medical staff. I did have issues getting previous medical records and call back in regards to that but otherwise they were great.

He’s all about money made from IVF and doesn’t try to support natural pregnancy. He shot down a varicocele repair for my husband and that’s the very surgery that got me pregnant naturally after failed ivf. He advised against it.

I cannot remember. I know we used Lupron 2 weeks and then FSH for almost 2 weeks. Egg retrieval and then steroids and progesterone.
