When I was doing the IUIs (2), I felt more like a number. But as we moved into our IVF cycles, I had increased interactions with Dr. Widra. After my first miscarriage, I felt Dr. Widra responded with genuine compassion and personalized care. From then on, I felt he really knew me and my history...I found Dr. Widra to be compassionate, but also frank. He didn't sugarcoat anything, but I certainly never found him cold....
I actually went through 2 nurses [at Shady Grove] who were not good communicators, not organized, and with whom I was not happy. After my negative experience with the second, I spoke with my doctor and he had me transferred to another nurse, who was excellent. She was clearly so much better than either of the first two....Strengths: multiple locations, very good hours for appointments, usually pretty efficient monitoring hours Weaknesses: takes some time before various personnel get to know you. your doctor is likely not going to do your procedures
How was your experience with Eric Widra at Shady Grove Fertility WMA?
My husband and I found Dr. Widra to be compassionate, but also frank. He didn't sugarcoat anything, but I certainly never found him cold. When trying for my first, we went through a number of retrievals and transfers and I appreciated feeling "known" despite being at a very large clinic that has a reputation for being a factory. I am detail oriented and Dr. Widra always took the time to answer my questions, whether in person or on the phone. While much of my communication was with my nurse, if there was difficult or sensitive information to deliver, he always called me himself and I never felt rushed off the phone. We ended up doing PGS testing and when I became pregnant, Dr. Widra told me to call him if I ever had any questions. We were indeed told at one point in my pregnancy that we should consider an amnio to rule out chromosomal abnormalities. At this point it had been a few months since I saw Dr. Widra, but I called him and he returned my call within hours. We went back to Dr. Widra when we were ready to try for our second, and unfortunately we did not have success. After two cycles, he recommended we move on to donor egg. While we weren't (aren't) ready to do that, I appreciated his honesty about how he views our situation.
What's one piece of advice would you give a prospective patient of Eric Widra at Shady Grove Fertility WMA?
That he may not be the warmest upon first impression, but you should not take his frankness for a lack of compassion.

In the early part of my treatment, when I was doing the IUIs (2), I felt more like a number. But as we moved into our IVF cycles, I had increased interactions with Dr. Widra. After my first miscarriage, I felt Dr. Widra responded with genuine compassion and personalized care. From then on, I felt he really knew me and my history.
Describe the protocols Eric Widra used in your cycles at Shady Grove Fertility WMA and their degree of success.
I began with fairly low doses of menopur and bravelle, 75 and 150, respectively. But that cycle did not produce many follicles. In my subsequent cycles, I was using higher doses -- around 150 menopur and 300 bravelle. I did two antagonist protocols: bcp followed by stims, with cetrotide added when the follicles got to a certain size. While these yielded a decent number of eggs, Dr. Widra thought we might be able to get more with estrogen priming, so we did that for my last cycle when trying for my first. When trying for my second, we started with estrogen priming since that had yielded the most eggs when I was trying for my first. However, that did not result in many eggs, so Dr. Widra proposed a micro-dose Lupron protocol.

I actually went through 2 nurses who were not good communicators, not organized, and with whom I was not happy. After my negative experience with the second, I spoke with my doctor and he had me transferred to another nurse, who was excellent. She was clearly so much better than either of the first two. I actually had a similar experience the second time around. I started out with a pretty subpar nurse, but spoke to the office manager and was transferred to a nurse who was incredible. So I think the bottom line is that there is a range of nurses at the clinic and I would recommend self-advocating if you feel unhappy with your nurse.

Shady Grove is huge and therefore has the expected strengths and weaknesses. Describe your experience with your monitoring appointments at Shady Grove Fertility WMA.
I found monitoring appointments pretty easy because there were multiple locations I could go. There were often multiple women waiting, but that didn't negatively impact my experience. I knew that the earlier I went, the less time I would have to wait. Very occasionally, I would be taken to a room and then had to wait for 10-15 minutes, but usually once I was taken to a room, I was seen within a few minutes.
Describe the costs associated with your care under Eric Widra at Shady Grove Fertility WMA.
I was part of shared risk, both times. That meant I paid an upfront fee (~25,000) that covered up to 6 cycles and I got my money back if it didn't work.
Describe Eric Widra's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Shady Grove Fertility WMA.
Dr. Widra had a strong preference for eSET given that we were exclusively transferring 5 day embryos. I appreciated this approach, given that it is safer for both women and babies.

SGF is big but Sibley is a small office so you don't feel like just a number the way you might in the Rockville office. I believe that all the doctors at SGF are required to follow strict guidelines so there is not a lot of experimentation that I've seen so far but I have a fairly straightforward case. Dr. Osborn has always been friendly, considerate and willing to answer any questions that I've had. She has been the one to share good and bad news with me and will always contact me if I've requested to hear from her. She is realistic and clear with what she expects from treatment.

I couldn't get a straight answer from her [Dr. Barbara Osborn] about nutrition/supplements to before/during egg freezing. Overall I sometimes felt like I was talking to a customer service or sales rep instead of a doctor - she just was unable to be personable...bedside manner isn't great, but it's not the worst I've ever seen...Doctor expected ~13 eggs per cycle, of which ~9 would be mature. First cycle was way less successful than expected, so they used the same protocol but increased the dosage significantly.
There were some issues with communication [at Shady Grove Fertility], and one very serious error where Rachel had a trainee nurse call me, and she gave me the wrong instructions for medication changes that night. Luckily I had to contact Rachel for something else that day and I was able to catch the error, but if I hadn't, I likely would have lost that entire cycle...$12,500 for an egg freezing cycle: 20 mature eggs or 4 cycles, whichever came first. For 3 cycle, I paid the pharmacy ~$15K. SGF also tried to bill me for blood tests and a consultation with the doctor in between cycles. I was nickel and dimed.

Dr. Osborne's bedside manner isn't great, but it's not the worst I've ever seen. Fertility clinics seem to be extremely nervous about getting a patient's hopes up, which means they are so neutral that they veer into actually withholding information, and being obtuse. I found Dr. Osborne to be this way. For example, I couldn't get a straight answer from her about nutrition/supplements to before/during egg freezing. Overall I sometimes felt like I was talking to a customer service or sales rep instead of a doctor - she just was unable to be personable.
What's one piece of advice would you give a prospective patient of Barbara Osborn at Shady Grove Fertility WMA?
Come prepared with all your questions, and don't be afraid to press further if you don't get a full answer to a question, or one that you understand.

Shady Grove as a whole is too large and the way they are set up means less personalized care. Communication should have been better, and errors on the part of the nursing staff would have been less likely with a smaller practice. The financial department has no idea what they are doing and I spend several thousand more than I needed to due to their lack of attention and individual care. SGF is a for-profit business and it shows.

Age: 38, AFC: 18, AMH: 1.58. Doctor expected ~13 eggs per cycle, of which ~9 would be mature. First cycle was way less successful than expected, so they used the same protocol but increased the dosage significantly. Then 3rd cycle they used a different protocol. First 2 cycles: birth control, then antagonist protocol (Gonal-F, Menopur, Cetrotide, Leuprolide trigger). 3rd cycle, no birth control, estrogen priming instead, then same antagonist protocol, except HCG instead of Leuprolide trigger. 1st cycle: 5 eggs retrieved, 3 mature 2nd cycle: 12 eggs retrieved, 8 mature 3rd cycle: 14 eggs retrieved, 9 mature

Overall my nurses were good. I was however disappointed when my primary nurse was promoted to another location and I was abruptly switched to a new nurse with no transition period or explanation beyond, "you're transitioning to a new nurse today." There were some issues with communication, and one very serious error where Rachel had a trainee nurse call me, and she gave me the wrong instructions for medication changes that night. Luckily I had to contact Rachel for something else that day and I was able to catch the error, but if I hadn't, I likely would have lost that entire cycle. I was shocked at how dismissive and non-apologetic Rachel was about the error.
Describe your experience with Shady Grove Fertility WMA.
The clinic is waaaaaay too big, and people are treated like a number. Read your contract VERY carefully and make sure you understand exactly what is and is not covered. If they ask you to do an additional test or appointment, make sure you check to see if it is included, otherwise you will be stuck with an extra bill. The financial department is also a complete mess. They couldn't have cared less about my questions and they were unresponsive, and sullen on the phone. They didn't do a good job of figuring out my insurance coverage, and I had to make a number of additional calls to my insurer to make sure everything was correct. They also advised me to not bother to check the prices of medications with other pharmacies, since they had a deal with a specific pharmacy. After I had already bought the drugs, I talked to other SGF women who had found drugs from lots of different pharmacies for cheaper. My nurse also did a terrible job of helping me to order the exact right amount of medications for my last cycle, which means I have unusable drugs in my fridge now. They let you donate them back to the clinic, but you can't return them, which is ridiculous. I felt really manipulated and take advantage of by the end of my care at SGF. The care I received from monitoring nurses and retrieval staff was excellent, however.

Vanessa [nurse at Shady Grove] is very responsive via both phone and email. I always get a response within a few hours (in business hours)...There was just one instance where one nurse called and gave me instructions and then an hour later another nurse called and said they talked to another doctor and changed the protocol. I am happy there was more than one doctor involved (my doc was on vacation) but it was confusing and made me question the process a little.

As I am a single woman, without any known fertility issues, she started me with two unmedicated IUI cycles. These were unsuccessful so then we talked and I said I wanted to be more aggressive so the next cycle I went did Clomid, Follistim, trigger, and Progesterone. This was also unsuccessful but will try another cycle with the same protocol.

It is a business and run like a business but I trust their success rates. There is not a lot of personalization but I have not pushed too much for that either. There are a lot of locations so my main office is near my home for the monitoring and blood work appointments but I go to a further office for the IUI's as that is where they store the donor sperm.

Dr. Osborn was recommended to me from a friend doing IVF. I was not in the same situation as a patient, but I found her care and compassion to be top notch. This is such an emotional process and so expensive-- it is imperative to have a doctor who you can trust, who is there to answer all your questions, and who can provide the kind and supportive bedside manner one absolutely requires in the fertility treatment experience.
What's one piece of advice would you give a prospective patient of Barbara Osborn at Shady Grove Fertility WMA?
I would recommend going to Sibley for your screenings so you have a chance to interact with Dr. Osborn more frequently. I chose to go to a closer Shady Grove center, which I would have done differently in hindsight. I would also make sure you ask to have Dr. Osborn do your retrieval procedure if possible.

I was pretty run of the mill in terms of treatment course. The medicines and dosages were pretty par for the course of friends of mine who have gone through retrieval for IVF. I was happy to see i responded well to the medicines and would up using a sub-cutaneous trigger shot. I wound up having my retrieval a few days earlier than expected, too.

Vanessa was newer at the time, which I did not know. I had a very bad experience with a different doctor at the Arlington office, and had called to say I never wanted to be screened by her again. I asked at the time to be assured said doctor would not be doing my retrieval, which the nurse said wouldn't happen. Of course, come morning of my retrieval, the Arlington doctor was my surgeon. I later found out that my concerns and my notes on this other doctor were not expressed to Dr. Osborn. I needed to be more assertive and insistent with my nurse. Lesson learned for any future situations. But beyond that one hiccup, the nurse was responsive to my myriad questions and always kind and supportive.

With Shady Grove's system, you can go anywhere for monitoring (which is awesome in terms of the flexibility it enables). That said, I'd try to see Dr. Osborn as much as you can which will help you build a relationship with her and get instant answers to questions and next steps.

Very friendly bedside manner. Took account of my baseline knowledge and provided the right level of detail into all explanations. She listened to me and took into account my perspective which informed how we proceeded. She set reasonable expectations at the outset and met or exceeded all of them.

If you're looking for someone who will be overly supportive and involved in every way (complete every procedure, call you directly with monitoring appointment information, hand holding) that's not how he [Dr. Eric Widra] or the clinic operates. But if you're comfortable with a more formal manner, direct, thoughtful and respectful manner - then he'll be a good fit... He was not initially responsive to the additional testing I wanted to have done (Leiden V) or putting me on additional medications. But after the second miscarriage, he was amenable and willing to accommodate my requests. Through these difficult times he has been thoughtful, caring and respectful.
Shady Grove is a factory. And you have to know that and be 'OK' with that going into the process. You will not get a lot of attention or hand-holding from your primary RE. You will be seen for your monitoring appointments and procedures by whomever is on duty that day. Do not expect your primary RE to perform any of your procedures. He or she is there to oversee your 'case'. Your nurses are your rock. You are assigned one primary nurse and they do everything. And you have to be OK with that - otherwise Shady Grove is not the place for you.
How was your experience with Eric Widra at Shady Grove Fertility WMA?
Eric is a smart doctor. And you'll see him for your until consultation to devise a treatment plan. But he's not a warm and fuzzy doctor. Neither is he's hands on, nor especially client interfaceing until things don't go as initially as planned. We barely saw him over the course of our three IUIs. He did not perform any of the monitoring appointments or procedures. Due to the clinic's large size, those tasks were performed by whichever doctor was available at the time. However, once we moved onto IVF cycles - we saw Dr. Widra more frequently. Still however, the procedures were completed by available daily doctors (not Widra). We finally got to experience Dr. Widra in a more hands-on and compassionate way after I began to experience unexplainable RPL of chromosomally normal embryos/fetuses. He took the time to meet with my husband and me as often as we felt we needed to come in and consult with him, and have him reassure us. He was not initially responsive to the additional testing I wanted to have done (Leiden V) or putting me on additional medications. But after the second miscarriage, he was amenable and willing to accommodate my requests. Through these difficult times he has been thoughtful, caring and respectful.

During the IUI process I felt like a number. We completed 3 unsuccessful cycles. We met with the doctor before we began the process but after that initial meeting we were passed off to nursing staff and whatever doctor was on call at the time to complete the IUI procedures.

Diagnosed with PCOS and unexplained fertility issues. Completed 3 rounds of IUI with Clomid and ovidrel. Resulted in good egg stimulation for all three cycles but only one chemical pregnancy. He was not open to changing protocol as I requested to include metformin and femerra. Moved onto IVF. Egg stimulation and retrieval: Cetrotide 250mcg Gonadotropin FSH ( Gonal F 450 iu pen x 3) and 75 IU FSH&LH ( Menopur 75 iu vials x 15) Lupron 2 week kit with 1ml luerlock syringe, 27 g ½ “ needle - “Trigger” subcutaneous. Zithromax 1 gram - taken evening before egg retrieval Resulted in 18 eggs retrieved. 8 made it to 5-day blocyst; 5 remained after PGD testing. I've had two primary nurse coordinators over the past year and a half and they have both been phenomenal. They address all of my questions, calls, and emails promptly and thoughtfully. They get answers back from my doctor quickly. They are always on the ball - calling in all of my medications before I even ask. They are compassionate when breaking diasapointing news and never complain about being nagged by me or my husband. They do the daily hand holding and support needed during this process.

I think that personality-wise, she might be a bit shy and is respectful of your privacy so she is not as nosy about your preferences as other REs might be. But you just need to be very well informed and go into your appointments with her with a lot of questions because she is patient and will answer every last one of them honestly and truthfully. She is forthcoming about why Shady Grove recommends some things (for business/cost reasons vs. strictly medical outcome reasons).
Shady Grove is like Starbucks. But the multiple locations, range of hours for monitoring, and fact that they provided dedicated financial advisors, and on site acupuncture made the process much better than most medical experiences.... Shady Grove has many locations and a lot of staff but I always felt it was key to have a very good nurse coordinator and to schedule as many appointments in the Sibley office as possible so that you build rapport with the staff in that office. The Sibley office is smaller but it is more boutique like than the other locations so the staff does get to know you.

Shady Grove is a busy practice and Dr. Osborn is not going to spend time with you providing endless amounts of information if she does not think you are interested. But if you ask her specific questions about the latest research, about your situation, etc., she will take the time to answer honestly and thoroughly.

4 fresh IVF cycles with own oocytes, ICSI and assisted hatching. After a failed cycle, Dr. Osborn adjusted the medications and protocol to encourage more eggs to be developed for retrieval. Adjustment in medications led to successful outcome.

My nurse coordinator was essential to the success of my treatments. She was organized and communicated with me via phone or email daily or multiple times a day during a cycle. Without her organization and attention to detail, I would have messed things up for myself.

We also considered going with GW, and the doctors are great there and always gave great insight into our issues, but the staffing and locations were just more limited than what is available thru Shady Grove.

With insurance, IVF with ICSI and assisted hatching was $3500 out of pocket. Medications were approx. $500. After employer dropped IVF coverage from health insurance, IVF with ICSI and assisted hatching was $10,500 and medications were approx. $4,000.

Dr. Osborn actually recommended multiple embryo transfer because of my age but I requested eSET twice because that was what I wanted. On one cycle, where the embryo transfer was not a 5-day blastocyst but 5-day morulas, I agreed with multiple embryo transfer recommendation.

Describe the specific things that went wrong at Shady Grove Fertility WMA.
During one embryo transfer, the RE doing the procedure informed me that one blastocyst would be available for transfer, and one would be frozen. I had asked for two embryos to be transferred if none could be frozen. But unfortunately, the RE had misinterpreted the grading on the embryo (a perfect blastocyst as she described it) that she said would be frozen and it was actually not good enough to withstand the rigors of freezing and discarded. But had I known that that blastocyst could not survive freezing, I would have decided to have that additional blastocyst implanted as well. This was tremendously upsetting. Dr. Osborn did investigate what happened and apologized for the RE's misunderstanding of the grading and said she spoke to the other RE about it and that they will improve procedures going forward.

I found her [Dr. Osborn] to be very warm and caring. We wanted a lot of detail and she was happy to provide it. She had an analysis of our results and expected outcome using different treatments. We were going to go forward with IVF with her...She's very warm in individual meetings....Very research / evidence based. Some people don't like that they won't deviate from protocols but from my perspective there's a reason those protocols exists

3 rounds of IUI with no success, spontaneous twin pregnancy ending in miscarriage. Decision to move on to IVF but had another spontaneous pregnancy prior to starting.

Myasia is very friendly and makes you very comfortable

Shady Grove is THE place to go! They know exactly what they are doing and have all the facilities with the most recent technology. SGF is a well oiled machine. They have several locations which makes treatment convenient. Sibley is nice, Shady Grove is super nice, K Street is probably the least nice of the three in terms of space/ambiance but that is to be expected because it is a city location. Providers were competent....Susan [a nurse] was incredible. She's very kind and positive, and knows her stuff. She responded to my questions with a very quick turnaround and was very accessible.

I felt Dr. Bromer was very warm and kind in his approach...very informative and willing to work with us despite challenges we had run into with previous clinics. I had been told I was simply a poor responder to medication, Dr. Bromer was convinced with the right treatment I could respond and he was right...I did one IVF cycle with a fresh transfer with Dr. Bromer. I had poor results in previous retrievals so he monitored my medications very closely and took a different protocol than previous doctors.
I had a very positive experience with Shady Grove. I found them to be highly skilled in their field and thorough. Each appointment was very quick. My egg retrieval was super smooth in the Rockville office as well...My nurse Kelly was amazing! So response and supportive during my entire time under her care. We never even met in person and I felt extremely taken care of and connected to her.
