Dr. Pasternak was extremely relatable and shared her own experiences of going through the process of egg freezing which I really appreciated. She made me feel taken cared of and safe, and never dismissed any concern or emotion I felt every step of the way. She had a perfect balance of being extremely knowledgable and professional with someone who could be a friend...you might not see Dr. Pasternak every visit and that the nurses might be the ones doing your ultra sound. How was your experience with Monica Pasternak at Spring Fertility San Francisco?
Dr. Pasternak was whom I had my initial consult with. She was there for a few of the mid-treatment check ups and performed my egg retrieval. I was grateful to have her as my doctor for the egg retrieval because I didn't expect to get emotional right before the procedure. Her bedside manner was not only comforting, but felt maternal in that any nerves I had prior to undergoing the procedure were subsided by her ability to understand my fear and provide reassurance.
What's one piece of advice would you give a prospective patient of Monica Pasternak at Spring Fertility San Francisco?
Know that you might not see Dr. Pasternak every visit and that the nurses might be the ones doing your ultra sound. She has a great team who is very qualified and communicative and depending on what day your egg retrieval is, you may have another doctor perform your retrieval (per chance Dr. Pasternak is off that day). These are just things to keep in mind and ask about. Additionally, a piece of advice I would give is ask a lot of questions -- the team is so understanding and will answer all of them. This helped me feel informed, reassured and eased my tensions!

Describe the protocols Monica Pasternak used in your cycles at Spring Fertility San Francisco and their degree of success.
I asked if I should be taking any vitamins, doing acupuncture, etc. Some vitamins were suggested and I began taking vitamin C every day and occasionally melatonin to help me get a restful sleep. Ultimately I'm not sure if this strongly impacted my results, but I presume it didn't hurt.

Super relatable, understanding and demonstrated kindness and patience. She was always empathetic to how I was feeling and gave her own examples of friends and family who had gone through the egg freezing process.

Weakness: Initially, it was a little difficult to schedule a first appointment. I had filled out the form online but hadn't heard back so it required a phone call and scheduling myself. Additionally, I had to follow up a few times with the finance department to send through invoices so that I could submit to my employer for reimbursements. The doctors office and finance team are a bit disjointed and I expected it to be a bit of an easier process to receive the paperwork.

I lived close to the clinic which was very helpful. There were times where the window to come in was small, however I didn't find it problematic because I was prioritizing this and was happy to accommodate.

My only negative is that once you have your initial appointment with Dr. Tran, you don't see him again until your first monitoring appointment, so if you have questions or concerns that come up after the 1st appointment, you pretty much have to rely on talking to the nurses to answer those questions...While he wasn't what many would call overly sweet, he was respectful, concise, and thorough in his answers...On the day of retrieval, Dr. Tran answered my questions and was kind to me, holding my hand right before the procedure, as I started to cry from being scared/anxious.
They [nurses at Spring Fertility] also sent several follow up emails to check in on my pain and recovery. They also called to check on me. I felt like my concerns were addressed always in a timely manner, even if it wasn't my primary RN responding...she spent a lot of time explaining things in depth and I felt so much better after talking to her...
How was your experience with Nam Tran at Spring Fertility San Francisco?
Dr. Tran is incredibly smart and advanced in his field. In fact, my friend's boyfriend who is an embryologist (not at spring) actually mentioned his name specifically as a good doctor to consider using. To me, this meant a lot and after researching and reading reviews I decided to use Dr. Tran and I'm glad I did. As a travel nurse for 13 years, I'm familiar with the wide array of physicians personalities and bedside manners. While he wasn't what many would call overly sweet, he was respectful, concise, and thorough in his answers. To me, knowing his experience level, and that he is the medical director, I am perfectly fine having a doctor be blunt and to the point about my medical care. I came to my appointment was a decent understanding of egg preservation after research and talking to other women who have undergone the procedure. I think because I was prepared, he didn't spend a lot of time explaining the process from beginning to end - but I think this is because he knew I had done my research. I think if I had not done this research, I would have needed a bit more of an in-depth explanation about the process than I received. I had read others tips to be prepared with questions for him, which I was and he answered candidly. My 1st 3 appointments for monitoring were with Audry, the NP who was also fantastic. Because I needed midday appointments I saw her, as Dr. Tran does AM monitoring. Later in my process, I had morning appointments with Dr. Tran. I believe he was training another Doctor who did a couple of my ultrasounds (I cannot remember this doctor's name) while Dr. Tran was in the room and answering my questions. This doctor that was training was not as gentle and communicative as I would have liked during my ultrasound. Perhaps she was distracted, but I think some work there can be done on bedside compassion and communication. On the day of retrieval, Dr. Tran answered my questions and was kind to me, holding my hand right before the procedure, as I started to cry from being scared/anxious. My only negative is that once you have your initial appointment with Dr. Tran, you don't see him again until your first monitoring appointment, so if you have questions or concerns that come up after the 1st appointment, you pretty much have to rely on talking to the nurses to answer those questions - I was told his phone appointments were booked well in advance when I asked to speak to him before my monitoring appt. Luckily, I was able to talk to one of the nurses who answered my questions before my 1st monitoring appointment. Dr. Tran managed my meds and care flawlessly. I was at high risk for hyperstimulation and we developed a plan to prevent this from happening. I'm not sure other doctors would have been so cautious with my care (as I've met other women who were also potentially high risk but their doctors at other clinics were not as cautious with their medications.) Because Dr. Tran managed my meds and procedure so well, I'm ever grateful I chose him, as he is confident and extremely experienced and did a fantastic job with my care. The only thing I think could have been done better, was a better explanation of the types of discomfort I would feel. I had expected "bloat and discomfort" but as I stimulated, I was having notable cramping - the left side was more pelvic and the right side radiated into my lower back and was quite intense. I had not expected this pain and wish someone had warned me. Also, after my retrieval, I had expected more pelvic cramping but the pain I experienced was high up in my abdomen under my ribs all the way down to my lower abdomen and my entire abdominal cavity was distended and hard. This pain was more gastric in nature - like cramps/gas discomfort you might experience with a bad gastrointestinal bug - only had pelvic/vaginal pain a few hours after the procedure.
What's one piece of advice would you give a prospective patient of Nam Tran at Spring Fertility San Francisco?
Research on your own before you come, so you are prepared with questions. Take your time asking what you want to know and speak up if you don't understand. Personally, as an ER nurse I knew I wouldn't be able to work during the stimulation/right after retrieval, as risk of ovarian torsion with high follicle count must be considered and work in the ER is very active. Also, even if that wasn't a risk, I was so uncomfortable mid stimulation- 1 week post retrieval that I know I couldn't have gone and worked 12 hours. So definitely ask Dr. Tran the true expectations of how you will feel and how it will affect you/and your work schedule. Everyone is different, but I'm really glad I was able to be off, as my experience was more uncomfortable than what I had been prepared for.

Describe the protocols Nam Tran used in your cycles at Spring Fertility San Francisco and their degree of success.
Knowing my follicle count was initially 39, I started with Menopur 200 iu/Gonal F 150iu for 3 days. After my first monitoring appointment, It changed to Menopur 150iu/Gonal F 150iu x 4 days. On stimulation day 7, I started the agonist, Cetrotide daily in the AM(this one burns/itches fyi - normal). On stimulation day 8, menopur 75iu/gonal f 150iu. On stimulation day 9 meopur 75iu/gonal f 75iu x 2 days. On day 11 of stimulation (saturday - luckily Dr Tran was on call so I saw him in office) I just did my agonist in the AM and only did ONE TRIGGER shot- LUPRON only at 8:30pm- this was the plan, as I was high risk for OHSS due to high number of follicles and the HCG trigger shot is notorious for making OHSS worse for high risk women. The next day I didn't do any more shots - just went to the Oakland clinic for a blood draw to make sure the Lupron trigger alone was enough to finalize maturation of the eggs (we had planned that if it wasnt, I would also trigger with HCG - but only if necessary). The Lupron worked, so I didn't have to do any more shots. My retrieval was at 0830 the following Monday. 42 eggs collected and 37 were frozen later. Dr. Tran also called in Norco for pain, which I did end up using for about 3 days as my abdominal discomfort post retrieval was pretty intense. He also called in 7 days of a birth control pill to help offset any symptoms of OHSS. About 3-4 days after my procedure the abdominal pain and swelling/bloating started to significantly decrease and I was able to control my discomfort with tylenol and motrin.

Sharon was my primary nurse, but during my stim she was on vacation some, so I did interact with other nurses. After my initial appt, but before my 1st monitoring appt/start of stimulation, I had some questions and was not able to speak to Dr. Tran. I was confused about the medication packages and options as well as had concerns about OHSS. I spoke with Nurse Jill and she spent a lot of time explaining things in depth and I felt so much better after talking to her that day. Also, I saw Audry the NP for several monitoring appts and she was great - very compassionate and gentle, as the ultrasounds can be uncomfortable as your ovaries grow. She was also very present with me, as I cried one day in the office (stupid hormones) and gave me a better understanding of the pain/discomfort I would feel as my ovaries grow, as I was surprised at how much back pain I was having. I also spoke to Ursula the NP on the phone one night after stimulation as my abdominal gas/cramping/distension had gotten pretty intense. She helped alleviate my concerns and called in more pain meds for me. They also sent several follow up emails to check in on my pain and recovery. They also called to check on me. I felt like my concerns were addressed always in a timely manner, even if it wasn't my primary RN responding.

I really liked Spring and have already recommended it to friends. There's a lot of prep work - documents, financials, etc to complete and while my coordinator was helpful, It felt like a lot to complete while also working a lot. I would have preferred to speak to financial department sooner than I did. Perhaps somewhat my fault as I was working night shift a lot and was limited on daytime hours to talk to them.

I was able to have a successful first retrieval [with Dr. Pasternak]. I did another one straight away as we had a small number of embryos and doing the second retrieval allowed me to bypass the month of hormones. In the end we got 5, day 5 embryos...always took time to help answer all my questions in an easy to understand way. I never felt silly or rushed for my questions and I felt like she really understood both my husband and I...She was very down to earth, though clearly competent. She was always very straight forward with us while interjecting helpful bits of information along the way. She took the time to learn our stories and about us and I always felt like she is a friend, which made the whole process so much easier.
I love that it [Spring Fertility] is a one-stop shop place. Blood draws, scans, transfers, retrivals, etc are all done there. Another strength is that all of the doctors and team are excellent. I had met a few for various scans and they were all warm, knowledgeable and highly competent...I mainly worked with Markie [nurse] and she was excellent. Always very quick to respond or tell me what she was waiting for...I appreciated Spring's single embryo approach as I felt confident with their testing and rational. I am happy this is what we ended up doing and now have one healthy girl!

I was on a month of injectable hormones before I did the first egg retrieval round as I was not cycling, depsit having a healthy uterus and a good number of eggs. It worked and I was able to have a successful first retrieval. I did another one straight away as we had a small number of embryos and doing the second retrieval allowed me to bypass the month of hormones. In the end we got 5, day 5 embryos.

Dr. Tran really understands his work. He personalized your care to your specific situation. He does not use a cookie cutter protocol. He is quick but thorough. He sometimes explains things too quickly. And I would ask him to clarify and he would...We used a mini stimulation protocol. The goal was to collect as many eggs to inseminate over a period of 3 months of back to back treatments. Dr Tran wanted a more natural approach so we did not use any steroids such as Omnitrope

Even though I know Dr. Tran is very busy, he always took the time to describe what was going on with my treatment. Spring went the extra mile with customer service. They sent me flowers after my embryo transfer and once my son was born they sent a onesie and a personalized card.

We used a mini stimulation protocol. The goal was to collect as many eggs to inseminate over a period of 3 months of back to back treatments. Dr Tran wanted a more natural approach so we did not use any steroids such as Omnitrope. I started off on birth control then with Clomid. Then we would layer on Gonal-F and Menopur with a Cetrotide Antagonist. Gonal-F at 150IU and Menopur at 75IU. We used ICSI to inseminate the eggs. We then froze the embryos on Day 3 instead of Day 5. The reason for this was that sometimes embryos are more successful inside the womb than in a lab. We then transferred 5 embryos which resulted in a pregnancy and a live birth of one healthy male child.

I had one son and then 4 miscarriages before the reproductive endocrinologist at Kaiser suggested IVF, and listed Spring as one of his three suggested clinics. I was not disappointed. Dr. Tran walked us through he different approaches we could take and we agreed that IVF seemed like the best route (i had no trouble getting pregnant, it just seemed like the eggs weren't of good quality). He told us based on my age and experience I'd likely get 0-1 "good" Day 5 embryos for a cycle. So we went in prepared to need a few cycles to get the embryos we'd want for transfer.We were thrilled that we ended up with 6 (!!) good Day 5 embryos. We would've much rather he. underpromise and overdeliver than the other way around. With the exception of the initial, hour-long appointment, our appointments were never long with Dr Tran. Nevertheless, I always felt like he recognized me immediately and was familiar with my history. He clearly either has a phenomenal memory or reads his notes/my file before entering the room (or both). Even though most of our interactions were short I always got my questions answered.

All medical assistants, nurses, and doctor tried to use my preferred name and seemed genuinely excited during our procedures. We had an unexpected complication and the doctor took the time with us to weigh our options. They put my health above my desire to continue treatment.

We used mini-stim for three cycles. The first cycle we used clomid along with injectable medication. After the first cycle, I had a health complication that we originally thought was from the medications. It turned out to be a chronic issue, but we changed the Clomid to Letrozole for the last two cycles and retrieved more viable embryos. We genetically tested the embryos retrieved which allowed us to know the actual amount of normal embryos we have.

Slow to respond if they respond at all and they struggle to effectively work with insurance companies on claims. I’ve learned a ton about medical billing during the process because of the errors and lack of proper communication with our insurance company. Be sure to understand the entire cost of treatment and check your bills and claims frequently.

Out of pocket costs were a bit more expensive than larger Bay Area clinics but they take insurance (big positive!) and you get more time with the doctor than in the larger clinics. They offer good deals for medication packages and genetic testing. Insurance costs have not been clearly described and it is extremely challenging to get clear answers (or any answers) from the spring fertility finance team.

The care coordinator and nursing staff is only available 9-5pm Mon-Fri - which sometimes made it difficult to get answers questions quickly if needed.

It was terrifying to find out my Amh was .46 at onset of this process. I was 36 and foolishly thought I had more time. When we went to Dr Tran he gave us the realistic and sobering news of our fertility. He drew us an inverted triangle showing us how many eggs he expected 3-4- to possible 0-1 embryo for pregnancy with Ivf. We wanted to try iui but soon found one of my tubes was blocked and scraped the idea. We went all in on the Ivf and I have a handsome 10 month old. We got two genetically perfect embryos from our single Ivf attempt. One is still frozen.

We have had only a positive experience partnering with Dr. Pasternak during our IVF process. My partner and I pursued IVF after discovering we were both genetic carriers for a hereditary disease and we always felt well-informed and supported and by Dr. Pasternak. Dr. Pasternak offers a balance of warm beside manner and straight forward and honesty delivery of information. She is incredibly easy to talk to and we felt like she was 100% on our team. Dr Tran is very thorough and keeps his methods in line with the latest medical science has to offer. I had 2 retrievals with similar eggs but only 1 embryo in first vs 6 in the second cycle. All because Dr Tran recommended to add another step in the process to boost fertilization. After my ecotopic pregnancy emergency he was on call right after to show support. Didn’t give me any false hopes of capability of medical science to avoid/handle better any future scenarios of ectopic pregnancy for us. Knowing the risk involved upfront prepared us much better mentally to enter into future transfer cycles.

Don’t worry about less in person meetings with Dr Tran, know that he keeps a close track of your case and is on top of every results that come while you go through the cycle. Trust the team to carry on the cycles with utmost sincerity.

Dr Tran is very thorough with his approach and keeps things very scientific. We did not have too many in person meetings with him but honestly his team made all the difference. It was a delight talking to my nurse every step of the way and gave me the belief Dr Tran is reviewing everything to the T and recommending what is the next best step for us. I ended up having an unfortunate ecotpic pregnancy which had to be handled with a emergency surgical procedure. Even after that I had the confidence to go ahead with future transfers given the confidence I got from the team there was nothing scientifically possible to avoid what happened.

Dr. Zore is the reason we switched fertility clinics to Spring. After our first meeting with her I felt seen and like I was going to be treated as a person not just a number. She listened to our concerns and desires and laid out a plan that she felt was reasonable and would give us either the best shot at getting to our goal or at least provide us with more information. We were very fortunate to have such a great retrieval cycle that yielded above expected eggs of good quality. Although our first transfer cycle wasn't successful, Dr. Zore made herself available to answer all of my questions as well as provide all options that we could explore, providing me both her opinion but also the freedom to accept or decline things. Where she felt it was imperative I do something, she would tell me. I feel a strong partnership with Dr. Zore.

Ask a lot of questions, come prepared with a list, but leverage the time you have with her. She's incredibly knowledgable, make sure you take advantage of that. Most doctors won't go down different paths unless you ask for it... ASK!

Dr. Zore explains things in a matter of fact way, however does so with compassion and takes time to let us process what she's said and then asks follow up questions or allows us to ask follow up questions. I felt like she really understood what we were trying to achieve and give us the best shot towards that

Had a very positive response to the protocol that Dr. Zore used. Some combinations of Clomid primer, Gonal, Menopur, Cetrotide, and Pregnyl trigger. We had a baseline AFC of 8 and ended up collecting 19 eggs, 14 which were mature to fertilize. Ultimately 10 embryos made it to freeze and biopsy for PGT testing

Overall I think Nina has done a good job with my care. We had a fairly poor embryo transfer outcome in that it ended in an ectopic. This was not my first ectopic and felt really off guard when she shared the unexpected rise in HCG after expecting that our initial betas didn't rise appropriately. I didn't feel like she was taking into account how traumatizing this would be for me to hear and I didn't feel emotionally prepared or set up to receive this information. In hindsight, it would have been better had Dr.Zore delivered this to me as it was incredibly sensitive and alarming for me.

They have recently moved to a "Dr of the week" model. This hasn't impacted me as we are no longer in a retrieval cycle. We chose Spring because of the model they used to have of a consistent doctor/patient relationship. Overall, having a team dedicated to me, a doctor that I would always have a touch point with was really important to me. Their app is a useful tool for communication and mostly works well.

I was so disappointed. I called and sure enough spring apologized as they did see my note indicating to hide all the genders. Now it can be unseen.

Dr Pasternak was very straightforward and to the point, and also very understanding and caring - she could relate to how the treatment felt, especially at the end of the cycle when everything is enlarged and uncomfortable. Opposite to Dr Pasternak, when I came in for an ultrasound and on-call physician saw me, I had to ask her to move slower because the physician was causing me pain. This physician looked surprised at my request, and I would have preferred to be seen for all visits by Dr Pasternak.

Describe the protocols Nam Tran used in your cycles at Spring Fertility San Francisco and their degree of success.
I started on 150 IU of Menopur and 225 Units of Gonal-F in the evenings for 6 days; then began 250 mcg of Cetrotide (antagonist) in addition to the 150 IU Menopur and 225 Gonal-F for 1 day, then reduced to 150 units of Gonal-F, then reduced the Menopur to 75 IU, then reduced to 100 Gonal-F and on the last day only injected Cetrotide in the AM and the Leuprolide trigger shot in the PM. Every couple days after the first ~5 I adjusted my dosage every couple days. My total days completing injections was 12 days.

Overall, loved the staff at spring. They were so kind, made everything super easy, really helped with the stress/anxiety that I felt along the process (because I am an anxious person). I felt very supported by them every step of the way, and they were always quick to respond to any questions I had.

I had a very straightforward situation-I was freezing eggs for fertility preservation while I had insurance coverage, and am lucky to have a high egg count and that I responded very well to the treatment. For my situation, Dr. Tran and spring in general was perfect. It was efficient, clear, and easy to work around my busy work schedule. I felt totally clear about what I needed to do when and knew that if I had questions, answers would be easy to get from Dr. Tran's team. For anyone in my scenario I would enthusiastically recommend Dr Tran and Spring. I'm sure he is also great for more complex cases but I can't speak to that because I haven't experienced it.

I was pretty much consistently on 150 of Gonal-f and 75 of Menopur (and certitude towards the end) for most of my treatment, with a few days of higher and lower doses at the beginning and end of the cycle. I'm not sure the rational for this. I think I had a high follicle count and they responded well to a moderate amount of medication, so my case was relatively straightforward.
Describe your experience with Spring Fertility San Francisco.
Spring runs a tight ship. In my first round of egg freezing, the time sensitivity of the whole process and the urgency of getting questions answered made me a bit nervous. Spring ensure you always are able to get the support you need. My nurse and patient coordinators were incredibly communicative, and if they were out or off, there was always someone else who could help me. I felt clear about what I needed to do every step of the way. My second round of egg freezing I was less concerned with seeing my actual Dr. because I was confident in the process and Spring, which enabled a lot of flexibility of being able to work around my schedule. It was also great to be able to split up some of my appointments between San Francisco and Danville since some days one location was more convenient for me. I'll note that I had a straightforward case of egg freezing without any known fertility issues, so lots of time with my Dr. wasn't super important to me. My first round I saw my Dr. for every appointment. The second time, I didn't actually see him once-mostly because I he works in the mornings and I tend to have lots of meetings then. I saw the NP and another doctor the second time-they were great, and my outcomes were just as good the second round. I just note that if seeing your Dr. throughout the whole process is important and you have a difficult schedule, that might be something to consider, but for me it didn't matter.
