In my first two appointments, I felt very rushed and that my lab results were not fully discussed. I was just told everything looked normal. I would have liked more of an explanation and analysis. However, as we prepare to move into the next steps after several IUI and two miscarriages, I booked a consultation with Dr. Fuji and he took about 40 minutes to really talk me through the process of IVF, which I greatly appreciated.

With my unexplained infertility diagnosis, Dr. Fuji recommended moving to IUI. We did clomid, mid-cycle ultrasound, trigger shot, and then the IUI through an outside clinic. However, none of these appointments were with Dr. Fuji.

While the nursing staff is expected to manage a great deal of patients, I have had tremendous difficulty getting in touch with them. Incorrect information has been communicated regarding the wait lists, which led to a wide range of emotions.

We were told initially that our IVF cycle would be in the January-February or March timeframe. Then, when I called to see if there was anything I needed to do, I was told it would not be until August-October of the following year. Then I received a call that I would be doing IVF in 2 months, which made it impossible to apply for most grants and significantly changed the way we were planning financially.
Dr. Levy really makes time to listen to you and explain things to you. He takes into account what your goals are as far as family size goals. He is even easy to get a hold via email if you are having trouble reaching the nurses...Dr. Levy will talk to you at the level of experience you have with fertility treatments and education. So, if you want more say in your treatment, make sure that you have done some homework in advance because he will candidly answer your questions and have discussion with you.
You should know that Tripler in general does not do fresh transfers. They conduct retrievals at a local clinic (Fertility Institute of Hawaii - also great). Dr. Levy really makes time to listen to you and explain things to you. He takes into account what your goals are as far as family size goals. I know that if we had wanted to speak to him personally, that we could have easily arranged that. We would have continued with him through to pregnancy had we not been required to move for my job.

Do your research about what you want before you show up. Dr. Levy will talk to you at the level of experience you have with fertility treatments and education. So, if you want more say in your treatment, make sure that you have done some homework in advance because he will candidly answer your questions and have discussion with you.

Dr. Levy was fantastic to work with at Tripler. We felt like he really reviewed and cared about our case. We came to him after an egg freezing cycle and six months of trying to conceive naturally. We felt like he not only took our individual case into account, but really worked around the extensive demands of my job for scheduling - which was really important to his.

We did three antagonist protocols. The first was cancelled due to poor response. The second, we made it to retrieval and got 13 eggs - combined with 4 eggs that survived the thaw from a freezing cycle - 1 genetically normal embryo came out of it. We decided to go for embryo banking and do another cycle, adding HGH, dexamethasone, and Clomid. That cycle resulted in 10 eggs, 1 genetically normal embryo and 1 mosaic. Following that cycle, we had to transfer to another city.

The whole nursing team at Tripler was great when we were there. They are compassionate and kind, definitely willing to take time to explain everything to you. There is a long waiting list for cycles and you don't get to choose your doctor really. The nursing team will get back to you via email within a day or two usually. We had a specific difficulty managing both a cell and work telephone number, which made it really hard for me to get results sometimes because they would always seem to use the wrong number meaning we would have to wait for days to find out the status of our embryos or genetic testing.

The strength of this clinic is that you receive medications without charge, usually a single doctor per cycle, and the finances are managed by a very responsive crew at the civilian clinic they partner with. The prices are really reasonable and the staff is trustworthy and good. The issue will always be the limited capacity they have to cycle patients meaning wait times for a cycle are often six months. This is more a function of the military medical system's resource allocation and not them in particular.

Billing is actually handled through the civilian partnership clinic and they are fantastic! I didn't even realize I was supposed to get a refund form the genetic testing charges and they sent it without me having to ask. They always got back to me immediately and could be easily reached by phone. They actually asked to meet patients when they came in for retrieval so they could get to know them as well. I really liked the financial department I worked with there.

The only thing I can say, is that if you go with Tripler, you have to deal with extended waits and a nursing staff that doesn't always have the capacity to be immediately responsive as I've experienced at the civilian clinic that we now go to. The other issue was that the one time that I texted the "emergency pager," I never received a reply. Thankfully, the issue was just fine, but I never found out why no one responded.

The nursing staff (3) seems overwhelmed and works out of a very small office. Out of the three staff members one staff member is the most knowledgeable which the other two staff members often ask for assistance on locating paperwork or understanding a certain process such as needle injections. My recommendation would be to provide the nurses with a larger office and hire another nurse to assist with the hectic workload.

He explained the risks of multiple embryo transfers and why he will advice only one embryo per transfer with PGT testing. As a medical professional I do not need as much explanation as other patients may need and would appreciate a more medical explanation of the bodies process during this treatment. I do think, though, that he is compassionate, professional, knowledgeable, sensitive to the situation, and encouraging even when I am not given the best circumstances. I do not know his views on certain non pharmacological interventions but have not been offered any suggestions from him.

Transvaginal ultrasounds to measure uterine lining and follicle size. Subq gonal f (225) and menopur( 150 and later moved up to 225). Daily u/s and lab work of progesterone and estradiol before retrieval for 4 days straight. Am antagonist when some follicles were developing more quickly than others and he wanted to give other follicles a chance to catch up ( progesterone labs showed it was safe to do so).

Slightly disorganized feel because 2 doctors left the practice and two more were coming in. They are also the only ones in island that service military so the clinic is constantly bombarded with patients

We can also have egg retrieval’s at ideal times even if it falls on the weekend because of the use of this facility which is not the case for every military clinic.

There was an instance where the doctor did not submit the order for my medications for the pharmacy and got caught up in his next appointment. Once he got out of his appointment he submitted the order. This resulted in an hour wait for me at the pharmacy. The nurses accidentally scheduled me for an appointment for the day after my egg retrieval. When I went to check in at the front desk they asked if I was there for an ultrasound and I said no. The nurse who scheduled me for the appointment came out to apologize and said she meant to schedule someone else instead.
Doctor Middleton never rushes with you, she takes the time to bring you to her office to answer and questions or concerns and seems like she genuinely cares about what you want...She is willing to be flexible with you schedule wise, she is honest without being rude, and always makes time to answer your questions...I was put on birth control, then took anti biotics before my retrieval at my retrieveal, the procedure was quick and painless. I then went home took more aintbiotics, waited on my period so i could start birth control again for a frozen transfer. They are pretty good about calling you with results but may take a while as they are consistantly busy. It can be very hard to reach them by phone, i have found emailing works better

I have been seeing doctor middleton for about 6 months now, and honestly she is one of my favorite doctors at triplers rei. She is willing to be flexible with you schedule wise, she is honest without being rude, and always makes time to answer your questions. You may sometimes have to wair a little longer at your appointment as she does this with all of her patients, so its worth it because she will do the same for you.

I was told by one of the scheduling ladies that I wasn't supposed to start Gonal F Shots until Day 5. She was very clear that I had for 2 round taken the shots wrong. The Doctor told me to start them on Day 2. Another incident with the same lady, She failed to tell me that I needed to arrive one hour before my appointment for the IUI so they will begin the Sperm Thaw. Luckily they were able to accommodate the timing. I was told by the other scheduling lady that "Everyone wants a baby" in response to how booked up the doctors are, also stated "we have more patients than doctors" Again causing undue frustration and stress.
Dr. Middleton is very honest and to the point when it comes to treatment plans and what's realistic for you personally. She's kind in her delivery and really made me feel that our desires were important and she was going to do everything she could to make it happen.... Because of my age we started with injectables and iui. After 3 failed iui's, Dr. Middleton felt that it would be best for me to move on to ivf, but let me try one more iui while I was on the wait list.

Because of my age we started with injectables and iui. After 3 failed iui's, Dr. Middleton felt that it would be best for me to move on to ivf, but let me try one more iui while I was on the wait list. The 4th oil failed as well, so we moved on to ivf. The egg retrieval has been completed and now were waiting to start the frozen embryo transfer cycle.

I believe there were two nurses that I have been working with in regards to scheduling and instructions. They are very helpful and to the point and always return my calls in good time.

The REI clinic can feel disorganized at times, and the nurses and staff over worked. It can be hard to get ahold of nurses when starting cycles, but once situated with a doctor the process seems easier . i do wish they had an embrologist on staff so that i didnt have to be sent out for the egg retrival but i like that dr.levy was also registered with the outside clinic... Tripler REI clinic has been wonderful but i dont feel that the head doctor enjoys her job or cares for her patients. I was very lucky to get paired with dr.levy over the head doctor of the clinic. Nurses from the gyno clinic some times help out the nurses from the REI clinic and the gyno nurses some times give out bad or wrong info.
I appreciated her insight and she was very helpful for the times that I saw her. She is knowledgeable and kind and truly wants success for her patients. She does not think of her patients as numbers only....She’s wonderful, but make sure you ask any questions you might have because she’s very busy!... She prescribed Femara and IUI. This was the 3rd time using Femara and the 4th overall IUI. She only prescribed Femara because that’s what I had taken before and there wasn’t time to do the cycle with injections. This was due to my cycle starting over a holiday weekend and the clinic being closed. (The joys of a military clinic...)
They [Tripler Army Medical Center] are super busy and can’t always fit you in when needed. I don’t feel like the care is very personalized. I’ve done 4 IUI cycles and have seen different doctors for each one and none of the cycles were successful. I haven’t seen my “assigned” doctor since my initial appointments. It’s frustrating. However, I am thankful that I am able to go to them and that a lot of the treatments are covered or partially covered under insurance.

We were trying to get my husband in to the urologist and it turned into quite the process trying to get the referral sent. Once we had the referral it took forever to make an appointment for semen analysis. Some of this was the lab’s fault, not the clinic’s fault.
The only negative thing about her [Dr. Middleton] is that she is often running late, but this is because she takes so much time to fully address the concerns of her patients...Dr. Middleton also managed to help me avoid OHSS, even with 27 eggs retrieved, by switching me to a lupron/hcg trigger instead of a the standard hcg trigger given by the clinic. After one IVF attempt we had a successful fresh transfer of one embryo and we have nine frozen, so we couldn't be happier!..Even in a very busy military clinic, Dr. Middleton took the time to address any question or concerns that we had.
you have to try each treatment [at Tripler Army Medical Center] a certain number of times before getting on an ivf wait list which can be long...strengths: Medication is free (including injectables)! Bloodwork, ultrasounds, appointments, and other diagnostic procedures (I had an hsg, saline ultrasound, and laparoscopy) are also free. The REs are great once you are finally able to see them. They work with a civilian fertility institute for IUI and IVF procedures so you are receiving the highest quality of care for a fraction of the price...Because the cost of medication, blood work, ultrasounds, and diagnostic procedures/tests were provided by the military, we only had to pay for the actual procedures done at the civilian fertility institute. Each IUI with sperm wash was $233 and a full IVF procedure with retrieval, ICSI, and transfer came out to about $7500. Embryo storage is about $600 each year.

weaknesses: Scheduling an appointment is a challenge and sometimes they cannot fit you in when needed due to military hours. The main REs are great, but when you first start you may not see them. You may see a nurse practitioner or intern instead. Also, you have to try each treatment a certain number of times before getting on an ivf wait list which can be long.
