This is the most recent data from the CDC. The CDC cautions that it should not be used to compare doctors and clinics. This data represents the outcomes of IVF cycles when using a person's own eggs.
For more information please see our FAQs.
Where you see a “?” on the chart, a clinic has done fewer than 20 cycles in this category and age range, so the CDC says showing percentages from this small data set would be misleading and unreliable.

What specific things went wrong at Utah Fertility Center?
We went to see Dr Gurtcheff for a consultation. She expressed compassion for what we are dealing with and took the time to listen to all our concerns and make sure she understood our history. She then gave us the information we needed for next steps and let me know I would need to get some blood work done. She explained what they were looking for and what the results would mean and how they would communicate those results to me.
There is a long waitlist [at Utah Fertility Center], so be prepared to schedule a ways out. They also are not always great about answering their phones, but they do get back to you fairly quickly if you message them online in the patient portal. Everyone I spoke to was kind and the front desk receptionist was warm and welcoming. They have a nice office with nice amenities like coffee or hot chocolate in the waiting area. It felt clean and inviting.

She explained what they were looking for and what the results would mean and how they would communicate those results to me. She then did an ultrasound and explained to me what PCOS is, which I was pretty sure I had but my OB did not officially diagnose. She confirmed to me I did have it and explained what it is and symptoms I might experience as a result. She made our next steps very clear and provided us with a folder of information so we wouldn't forget anything. We didn't feel rushed by her at any point.
What's one piece of advice would you give a prospective patient of Shawn Gurtcheff at Utah Fertility Center?
Make sure you know your own history. She will want to know testing, procedures, diagnoses, all of that so make sure to have it written down if you are forgetful of all the medical terms like me.
During treatment, were you treated like a number or a human with Shawn Gurtcheff at Utah Fertility Center?
She was thorough and actually listened to my symptoms and what I had to say. She made sure to answer all my questions and I never at any point felt like she was rushing through the appointment which I have felt with other doctors.
Describe the protocols Shawn Gurtcheff used in your cycles at Utah Fertility Center and their degree of success.
We have not been treated by Dr Gurtcheff yet, we only had our initial appointment. She did explain what medications I would take and what the process would look like if we decide to move forward with IUI.
Describe your experience with the nursing staff at Utah Fertility Center.
I had a nurse check my vitals and let me know the doctor would be in shortly. She was polite and kind. I had another nurse do my blood draw and she knew I was nervous as I always am for that type of thing, and she talked me through it and distracted me which I appreciated.

Describe the costs associated with your care under Shawn Gurtcheff at Utah Fertility Center.
Our consultation was $325. My blood test was around $100
Dr. Gurtcheff is a godsend...gave us hope in our personal circumstances (MFI and PCOS)...asked purposeful questions, took notes, and made every effort to understand our situation from her professional lens...she explained that she is based in Pleasant Grove and Murray and that she would be present at all major appointments (IUI, IVF if it came to it)...asked if we would be allowed to transfer 2 embryos at one time, Dr. Gurtcheff talked to us in depth about all the possible outcomes. Because of my recurrent pregnancy loss, it was ill-advised because there is a higher likelihood of miscarriage with multiples.
Utah Fertility Center is very organized and there is a clear process to follow...there is a friendly face ready to help. I haven't had one negative interaction with all the nurses/staff I've seen at the Murray/Midvale, Ogden, or Pleasant Grove clinics...When I experienced my miscarriages, I got hugs from several of the ladies...never failed to respond or call me to better answer my question
How was your experience with Shawn Gurtcheff at Utah Fertility Center?
Dr. Gurtcheff is a godsend. My husband and I transferred to UFC, coming from a different doctor (in a different clinic/company) who was abrasive and as impersonal as one can be. During our initial consultation, Dr. Gurtcheff gave us hope in our personal circumstances (MFI and PCOS) as well as during the COVID-19 Pandemic. We met with her via Skype and she listened to our full story. She asked purposeful questions, took notes, and made every effort to understand our situation from her professional lens. From day one, she explained that she is based in Pleasant Grove and Murray and that she would be present at all major appointments (IUI, IVF if it came to it). She explained that my nurse would be in touch and that any data, results, and other information would be passed by her even if I didn't see her at routine appointments.
We started with IUI in 2021 and along the way (we did 7 IUI cycles with Gurtcheff), she was always just a phone call or email away.
When we decided to move forward with IVF, she was our biggest cheerleader. My body is an overachiever (not a shocker if you know me) and she was able to retrieve 50 follicles. Out of those, 21 eggs were fertilized and 14 made it to blast and were frozen. I experienced severe OHSS and once I recovered, the next month (August 2022), we had our first Frozen Embryo Transfer. It went beautifully! I got a positive pregnancy test, heard our little baby's heartbeat at two ultrasounds, and then, unfortunately, miscarried at 7w5d due to a sub-chorionic hemorrhage. Once my HCG was in the negative, we prepped for another transfer. In November, baseline scans revealed I had a Uterine polyp and I had that removed (by Dr. Gurtcheff) in December. In February 2023, we had another seamless embryo transfer and in March, we found out it was successful! We heard our baby's heartbeat, which was a little slow for 7w1d, but within an acceptable range. A few days later, 7w6d, I miscarried again.
During a phone call with Dr. Gurtcheff to talk about the next steps, she was very sympathetic and kind as we discussed what our options were. We decided that we needed to get our remaining embryos tested for any genetic abnormalities before attempting another embryo transfer.
What's one piece of advice would you give a prospective patient of Shawn Gurtcheff at Utah Fertility Center?
If you are closer to the Ogden clinic, know that you won't see Dr. Gurtcheff at every appointment. Be prepared to travel to Pleasant Grove for any major procedures or surgeries.
During treatment, were you treated like a number or a human with Shawn Gurtcheff at Utah Fertility Center?
Everyone in the offices (PG, Murray/Midvale, Ogden) remembered my name and a few even remembered that we have two dogs and would ask about them. They always double-checked that I was comfortable and they were ready to answer my questions. When they gave me bad news (e.g. miscarriage, uterine polyps, etc.), it was always with compassion, kindness, and sensitivity. No matter which branch/clinic I was in, everyone cheered for us at each stage of IUI, IVF, FET, and still.
Describe your experience with your nurse at Utah Fertility Center. (Assigned nurse: Amy Mitton)
Amy Mitton probably received dozens of emails from me every week when we were in active treatment. She never failed to respond or call me to better answer my question. I would come in for a blood draw and see her in her office. I'm sure I was interrupting something, but she always had a smile and would ask how I was doing. She called me regardless if the news was good (pregnant) or bad (not pregnant) and was always sensitive to my feelings/reactions. I know I could call her today letting her know we're ready to test our embryos and she would be ready to get us all settled and scheduled for any testing/baseline appointments.
Describe your experience with Utah Fertility Center.
Utah Fertility Center is very organized and there is a clear process to follow. No matter where you are (exam room, waiting for a blood draw, or by one of the reception desks), there is a friendly face ready to help. I haven't had one negative interaction with all the nurses/staff I've seen at the Murray/Midvale, Ogden, or Pleasant Grove clinics. If one doesn't know the answer to my question, there is another person they can turn to. I've never felt belittled for questions or asking for them to go over something again.
When I experienced my miscarriages, I got hugs from several of the ladies. I had no words, was still in shock, but their hugs or hand on my arm meant the world to me.
Describe the costs associated with your care under Shawn Gurtcheff at Utah Fertility Center.
For IUI, our insurance covered everything but the cost of injections, which was anywhere from $270-$450.
Our insurance covered $10,000 of our IVF cycle. This covered everything *except* injections/medications and genetic testing.
We paid approximately $2,400 toward genetic testing for myself and my spouse.
We took a loan through Lending Club for just over $3,300 for our injections (Follistim, menopur, and another I can't remember the name of). The payment is $163/monthly.
We paid approximately $1,000 more toward freezing our embryos, other non-injection medications and testing/appointments not covered by the $10k number.
We also paid $550 for anesthesia three times for my two hysteroscopies and my egg retrieval.
UFC partnered with CryoVault to manage the payments for our frozen embryos. We pay $62.50 monthly.
We need to pay just over $5,860 to cover the PGS/PGT for our remaining 12 embryos.
Describe Shawn Gurtcheff's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Utah Fertility Center.
When asked if we would be allowed to transfer 2 embryos at one time, Dr. Gurtcheff talked to us in depth about all the possible outcomes. Because of my recurrent pregnancy loss, it was ill-advised because there is a higher likelihood of miscarriage with multiples.
[Dr. Foulk] used the very latest medicine to help us conceive our child...plan of care was innovative...manner is reassuring...willing to communicate with me about setbacks or about my concerns...only drawback about such a great doctor is that he is super busy and I did not see him very often...Our protocol was rather long and also intense...PGS testing was my choice because my husband and I did not want multiple failed transfers. We had Dr. Foulk’s full support.
[Utah Fertility Center is] incredibly clean and everyone is so nice...loved the phlebotomist, the anesthetist, and the embryologist...front desk often struggles to connect you with someone who will return your calls...Billing is easy to work with and helpful. They were great at helping with payment plans...[nurse] was on top of my care, gave me her cell phone number to text anytime, called me to review all results.
How was your experience with Russell Foulk at Utah Fertility Center?
Dr. Foulk is an incredible doctor who used the very latest medicine to help us conceive our child. His plan of care was innovative. His manner is reassuring. And he always was willing to communicate with me about setbacks or about my concerns.
The only drawback about such a great doctor is that he is super busy and I did not see him very often. I did feel that I could always communicate with him at any point, but, I do wish I had seen him at some other visits.
What's one piece of advice would you give a prospective patient of Russell Foulk at Utah Fertility Center?
Be ready for aggressive treatment, he is willing to move at your pace, but, if you want a baby he will move heaven and earth to get one in your arms. He’s optimistic but also realistic. You won’t see him or speak to him often, but I do know that he oversaw all my details and was always ready to answer questions.
During treatment, were you treated like a number or a human with Russell Foulk at Utah Fertility Center?
Dr. Foulk was kind, calm, compassionate, and knowledgeable. He knew the details of my case and was always willing to answer questions.
Describe the protocols Russell Foulk used in your cycles at Utah Fertility Center and their degree of success.
We used IVF with ICSI and PGS testing. Our protocol was rather long and also intense. I told him that I wanted everything that could increase my chances of conception. It included HGH, and an antagonist protocol. We knew my eggs were compromised with an endometrioma on my one ovary (the other had been removed many years prior) and tried to get as many eggs as possible.
The PGS testing was my choice because my husband and I did not want multiple failed transfers. We had Dr. Foulk’s full support. He was a good balance of “here’s the facts here are my suggestions and you tell me what you want to do”.
Prior to transfer we also did an endometrial scratch and embryo velcro.
Describe your experience with your nurse at Utah Fertility Center. (Assigned nurse: Chelsee)
Chelsee was AMAZING. She was on top of my care, gave me her cell phone number to text anytime, called me to review all results. I loved working with her.
Describe your experience with Utah Fertility Center.
They are incredibly clean and everyone is so nice. I loved the phlebotomist, the anesthetist, and the embryologist. Every person I interacted with was kind and helpful.
The front desk often struggles to connect you with someone who will return your calls. This is a source of frustration and I found talking to my nurse coordinator about it helped.
Billing is easy to work with and helpful. They were great at helping with payment plans.
Describe the costs associated with your care under Russell Foulk at Utah Fertility Center.
I believe our costs were around $30,000 including PGS testing, ICSI, and a procedure for polyp removal I needed.
Describe Russell Foulk's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Utah Fertility Center.
He encourages a single embryo transfer for us because we only had two normal embryos and I had a history of difficult pregnancy.
What specific things went wrong at Utah Fertility Center?
Failed to call with results
Describe the specific things that went wrong at Utah Fertility Center.
There was a mistake where my original nurse didn’t accurately call back with tests. I requested a new nurse coordinator when we did IVF and had a better experience.
He [Dr. Parker] is obviously doing what he does because he cares. He gets moving quickly on procedures- I appreciated that as he was my 6th doctor due to moves & I had been worried I would have to “start over” in some ways delaying my process- but that wasn’t the case. During my egg retrieval injection process- he contacted me twice to discuss adjusting my protocol. ... I have endometriosis with an AMH of 4. I am known to have high E2 levels as well. ...
The lab [at Utah Fertility Center] has two really great phlebotomists, but there is a newer one there that seems to struggle a bit (I was stuck multiple times for one blood draw due to missing my vein). But I’m positive with experience she will get better! They are quick to catch any mistakes- and only one minor one was made during my process (sent me the wrong lab results once) but caught it in a matter of seconds.
How was your experience with Jason Parker at Utah Fertility Center?
He is obviously doing what he does because he cares. That made me feel like he was on top of ensuring my IVF cycle went as good as it could! He also made an effort to again call me when my PGS results didn’t turn out as expected and apologized and then spent 15 minutes on the phone going over our options from there and why we saw those results. He performed my egg retrieval’s which I see as a strength for him because I am aware some doctors have other doctors do it- so I appreciate that he was able to be present and be the one to perform them. He also was aware of our financial concerns & I so appreciate that he did his best to help us make the best financial choices.
What's one piece of advice would you give a prospective patient of Jason Parker at Utah Fertility Center?
Ask as many questions as you need. I felt like he assumed we knew a lot- which I again appreciated as we had been to many clinics prior- but I had to voice my questions often if I wanted them addressed- & once I did that- I got the answers I wanted.
During treatment, were you treated like a number or a human with Jason Parker at Utah Fertility Center?
I stayed pretty neutral as we’ve only ever interacted at consult & right before going under anesthesia for egg retrieval. Because of my limited time seeing him, I’ve felt like a number, but when my first retrieval cycle yielded only 1 PGS normal embryo- he made it a point to call me & expressed his congratulations but frustration as well which made me feel more human.
Describe the protocols Jason Parker used in your cycles at Utah Fertility Center and their degree of success.
I have endometriosis with an AMH of 4. I am known to have high E2 levels as well.
First retrieval cycle: Lupron 20 units 2x daily for 2 days, then added Gonal-F 150 2x daily and Menopur 1 powder 2x daily. I also took a baby aspirin and dexamethasone every day & continue to take dexamethasone. (To control my endo?) I eventually dropped my Gonal-F dosage twice during the process because my numbers from my E2 lab work was too high. Triggered with HCG shot. Post retrieval I took letrozole for 5 days to help combat OHSS which I thought was interesting.
2nd retrieval: exact same protocol but we lowered my gonal F to 75 for one of my two daily injections from the beginning. Triggered with pregnyl. Same recovery protocol as well.
Describe your experience with your nurse at Utah Fertility Center. (Assigned nurse: Shailee Taylor )
Strengths- they were always willing to answer whatever questions I had. They helped me mix my Lupron medication , answered my questions, had a detailed calendar, and during ultrasounds- they were always ready to explain to me what I was seeing.
The only difficult part was sometimes it took awhile for them to call back because they were out of the office on certain days, busy, etc.
Describe your experience with Utah Fertility Center.
The clinic is on top of things. The overall staff works hard to show effort that they remember who you are & that they care! The front office does a great job answering phone calls and transferring you to who you need (I have never been on hold with the front office) They try to keep nurses consistent for you- but again, scheduling makes that tricky sometimes. The lab has two really great phlebotomists, but there is a newer one there that seems to struggle a bit (I was stuck multiple times for one blood draw due to missing my vein). But I’m positive with experience she will get better! They are quick to catch any mistakes- and only one minor one was made during my process (sent me the wrong lab results once) but caught it in a matter of seconds. The overall layout of the clinic is great- the only thing that bothered me was how open the front reception area is as it carries conversations. I visibly watched & heard a woman crying as she signed away her embryos to be donated- something that was personal but due to the layout was heard by everyone.
Describe the costs associated with your care under Jason Parker at Utah Fertility Center.
They quoted us at around 14,000 for our IVF cycle (not including medications or PGS testing)
We have a small amount of insurance coverage (lab work) so we are waiting to get our final bill from the first retrieval. And we never made it to transfer so I don’t know the overall cost.
Describe Jason Parker's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Utah Fertility Center.
Because I did PGS testing, they recommend only transferring 1.
Dr. Conway has been amazing. She was straight forward with us. She let us know there was hope and set up appointments for my husband with the correct doctor to see what was causing his MFI, but she was very realistic and clear about what we were currently looking at for treatment, costs, etc. She explained everything well and at my level. She’s been so kind.
The doctors are great. The nursing staff is pretty good and nice. However, there’s some issues because they [Utah Fertility Center] don’t call back or call when they said they would. I don’t know if this is part of why Dr. Conway calls me with things sometimes. She’s amazing and makes up for their lack there of. ... They’re plenty nice but for some reason I have to try too hard to get results when they said they’d call and I have to make many calls to get a call back.
How was your experience with Deirdre Conway at Utah Fertility Center?
She’s kind, straight forward, thorough...she truly cares. She covers everything with you. She made me feel very comfortable and cared for. She’s been very involved, calling me to check in after I have other appointments, etc. on her personal time and talking to me for 20+ minutes at times. She’s involved in my plan and I love that. I wouldn’t move anywhere else because of her in particular. I feel so confident in her and trust her opinion and plan of action. I believe she can get us pregnant. She didn’t guarantee it of course but she’s been amazing and I feel like that will be the outcome, or at least that she is the best doctor to give us the very best shot at it.
What's one piece of advice would you give a prospective patient of Deirdre Conway at Utah Fertility Center?
Trust her. She cares and won’t lead you astray. Ask questions because she doesn’t mind answering them!
During treatment, were you treated like a number or a human with Deirdre Conway at Utah Fertility Center?
Dr. Conway has been amazing. She was straight forward with us. She’s spent 20+ minutes on the phone not scheduled or charging me and by calling herself. She’s told me I could call if something big came up. She’s been so involved in every step and part of this even when she could’ve passed it to someone else.
When we first went in the male factor looked bad and he had a history of a surgery that could affect fertility. She was kind but straight with us and I appreciate both. She said essentially, I want to send you to this other doctor and if there is something to find he’ll find it, but right now and judging by your history you’re looking at IVF and ICSI. I appreciated knowing there was a small bit of hope but I also appreciated the realistic information. It helped us to accept it and deal with it. She also made me feel confident that no matter what treatment we ended up needing with her that she could give us our best shot at pregnancies and babies. She didn’t guarantee anything but was optimistic and made me feel like she truly cared to give us our family. I believe she can do it or give us our best shot! I thought about going another place that offered different things and in the end the other clinic didn’t have the reviews to persuade me to leave my amazing doctor.

So far I’ve spent very little because of insurance. They’re good at making sure you’re in network and they do let me know what something will be out of pocket before we do it just in case. I got an accurate quote for IVF, bloodwork, etc. You have to pay either through insurance or not for bloodwork, meetings...my original ultrasound was free and they let me know ahead of time. They’ll charge for sperm freezings but will give you one free. Finances have been clear overall.

Describe the specific things that went wrong at Utah Fertility Center.
The only time I got a call when I was told I would get a call about results was when they needed me to start a thyroid medication. Besides that the staff tells you they’ll call then doesn’t. When we did our two semen analysis’ here they said they’d call, never did. I had to call and call and call to talk to someone and then I got a staff member who didn’t actually know about the results and tried to relay them. Also, they send you to the lab people but that line always goes to voicemail and they almost never call back. They haven’t released all results they’re suppose to either.
Dr. Gurtcheff is very sensitive to my situation and needs as her patient. She is always caring and makes sure that I'm informed about what's going on and takes time to answer any questions I may have and to reassure me that even when I don't always see her (sometimes the nurses or NPs perform ultrasounds) she is always overseeing everything for my treatments and she is always informed and aware of what's going on. She is very compassionate, kind and respectful.
The clinic [Utah Fertility Center] overall is really great and cohesive. They do a great job at communicating between the nurses, Drs and other departments. Whenever I have a question I usually call the nurses and if it's something they don't know they ask Dr. Gurtcheff and get back to me right away. I moved out of state while working with UFC but have still done treatments with them and the nurses are very organized and great at following up with the out of state Drs that I've worked with. If I was supposed to go in and get an ultrasound or bloodwork and it wasn't faxed to them they call me that day and find out where I did it and they call the clinic to to get the results. Very organized and great at communicating within their clinic and while working with other clinics.
How was your experience with Shawn Gurtcheff at Utah Fertility Center?
Dr. Gurtcheff is very understanding, compassionate and caring. As her patient I truly feel that she has my best interest at heart and she is doing her absolute best to find the treatments and procedures that will work for us. She is always willing to take time to answer my questions and to make sure that I'm well-informed. She shows compassion when the results didn't turn out like we expected and thoroughly talks me through our next options and her recommendations. I've never felt that she's pushed me into anything, she always gives me the info and the facts and I usually ask for her recommendation, which she gives, and then she leaves all decisions about treatment up to myself and my husband and she is willing to go whichever direction we want to go with treatment. She is very knowledgeable and she does a great job at explaining things to us. I moved out of state while still working with her and didn't want to change clinics because I've been very happy with Dr. Gurtcheff and we have frozen embryos at the clinic and she has been incredibly helpful with our move, making sure that we know our options and working with us over the phone to set up more treatments. She normally doesn't schedule transfers as far in advance as I needed one but I was trying to buy plane tickets to come back and I needed a date so she went ahead and scheduled the transfer a few months out, which was incredibly helpful to me! She is a sweet person that genuinely cares about her patients. I have felt very well taken care of with her.
What's one piece of advice would you give a prospective patient of Shawn Gurtcheff at Utah Fertility Center?
Get ready for an emotional roller coaster. There are a lot of ups and downs with fertility treatment but Dr. Gurtcheff does her best to keep you well-informed and to help you expand your family.

I used Follistim, Ganirelix and Menopur in preparation for my egg retrieval and ended up stopping a day early because the ultrasound showed that I had a lot of follicles. We did the egg retrieval a day early and got 40 eggs, which was great. I was really bloated and my ovaries were huge from releasing so many eggs and I was at risk for OHSS so Dr. Gurtcheff told me to take it easy and eat a high protein, low am carb, low sugar diet and drink electrolytes and I never got OHSS. The bloating went down within a few days and I felt better. We did Progesterone in Oil injections and suppositories leading up to both Frozen Embryo Transfers and got a positive pregnancy test with both (one ending in a chemical pregnancy and one in a miscarriage at 9 weeks).
Describe your experience with your nurse at Utah Fertility Center. (Assigned nurse: Morgan Baker)
The nursing staff are very kind and really treat you with a lot of respect and dignity. Vaginal ultrasounds can be invasive and hard and I've always felt that the nursing staff are very considerate about the fact that it's an uncomfortable process and very respectful and allow you as much privacy as possible. Morgan Baker specifically is a great IVF coordinator and she is great at communicating via phone and email about test results, schedules, medications, etc. I always understood my IVF schedule very well and it was all laid out for me and easy to read, which I really appreciated! Morgan always takes the time to talk and go over the schedule as well, which is helpful!

Very organized and great at communicating within their clinic and while working with other clinics.
Describe the costs associated with your care under Shawn Gurtcheff at Utah Fertility Center.
$1,000 per cycle of IUI. $12,000 for an Egg Retrieval/Fresh Embryo Transfer cycle. $3,000 per Frozen Embryo Transfer. $750 initial cost to freeze embryos and $250 per year after that.
He [Dr. Russell Foulk] is understanding, he wants to try new things, he is determined to help me start my family, and that's exactly what I have needed in a doctor. I do know how in demand he is and sometimes that shows through with how quickly he is in and out of appointments, but I wouldn't trade that for the type of care I receive from him. He gave me more answers in one appointment from looking at my history than 2 other doctors gave me by performing tests themselves. I appreciate him and trust that he is doing everything he can to help me in my situation
UFC is amazing...I know they are busy with so many patients so their biggest weakness can be response time with some things, however, overall I have had such a good experience with them. They're friendly, they keep me informed with my plan of care, and they always follow up with me after major procedures...Dr. Foulk is very busy and sees a lot of patients because he is so good at what he does. I appreciate so much how he remembers my situation, he remembers me, and is constantly following up on what's going on.
How was your experience with Russell Foulk at Utah Fertility Center?
Dr. Foulk is so in tune and so educated and it shows. I have switched doctors so many times because of my doctors not listening, not knowing what to do next, and being so out of touch with my situation, Dr. Foulk is different. He is understanding, he wants to try new things, he is determined to help me start my family, and that's exactly what I have needed in a doctor. I do know how in demand he is and sometimes that shows through with how quickly he is in and out of appointments, but I wouldn't trade that for the type of care I receive from him. I appreciate him and trust that he is doing everything he can to help me in my situation
What's one piece of advice would you give a prospective patient of Russell Foulk at Utah Fertility Center?
Just be honest about how you're feeling with everything. Tell him exactly what you need and ask him lots of questions. Sometimes doctors forget that you don't know the details of what you're talking about, but if you ask him questions, he's so good about explaining things exactly how you can understand them.
During treatment, were you treated like a number or a human with Russell Foulk at Utah Fertility Center?
Dr. Foulk is very busy and sees a lot of patients because he is so good at what he does. I appreciate so much how he remembers my situation, he remembers me, and is constantly following up on what's going on. Sometimes it feels a bit rushed and I wish I could spend more time getting his thoughts and opinions, but overall, he does an amazing job keeping me informed with what's going on.
Describe your experience with your nurse at Utah Fertility Center. (Assigned nurse: Morgan Chappuis)
Morgan has been my coordinator through all of this and she has been amazing. I have bugged her with so many questions and concerns and she responds so quickly and with such grace. I appreciate having her along the ride with me so that I always have someone I can reach out to! I always get different nurses when I go into the office but they have all been amazing too!
Describe your experience with Utah Fertility Center.
UFC is amazing! I originally was there, then switched to a different clinic, did not have a good experience, and then transferred back to UFC. I know they are busy with so many patients so their biggest weakness can be response time with some things, however, overall I have had such a good experience with them. They're friendly, they keep me informed with my plan of care, and they always follow up with me after major procedures.
Describe the costs associated with your care under Russell Foulk at Utah Fertility Center.
It seems like every time I have an appointment that I actually see Dr. Foulk, there is some kind of cost. I do have really good insurance which is helpful, however, I at least have a co-pay when I see him. IVF costs are different as those are excessive no matter what, but since he's a specialist, the cost is a bit higher when I see him directly.
[Dr. Jessie Dorais] has prioritized and taken care of me to the best of her medical and personal capability...had to pursue egg retrievals and freezing as a single woman due to cancer...because of my diminished ovarian reserve, we have experimented with very high-dose stimming, and also lower-dose stimming, and saw similar success.
The nurse [at Utah Fertility Center] seemed overworked and unorganized...staff performing ultrasounds were usually fairly compassionate...phlebotomists were pretty casual comfort-wise and good at their job...There was confusion with the nurse and myself over who would be calling in the prescription
How was your experience with Jessie Dorais at Utah Fertility Center?
I met Dr Dorais a few years ago now and I think she is extremely intelligent in what she specializes in. She is also extremely compassionate towards me and has helped me navigate my fertility treatments after cancer in the most empathetic way.
What's one piece of advice would you give a prospective patient of Jessie Dorais at Utah Fertility Center?
She is involved in everything, but you won’t be hearing directly from her on a daily basis during the egg retrieval process, you’ll more likely be hearing from your nurse or clinical staff.
During treatment, were you treated like a number or a human with Jessie Dorais at Utah Fertility Center?
I’ve seen her for years now, and I truly feel she has prioritized and taken care of me to the best of her medical and personal capability. I’ve had to pursue egg retrievals and freezing as a single woman due to cancer, and she’s been a great comforter and caretaker for me for years now. She makes sure I understand all of my options, and she writes out and even makes drawings so I understand the purposes of all my treatments. We’ve seen success and we’ve experienced losses together, and I’m grateful I’ve had her on my team.
Describe the protocols Jessie Dorais used in your cycles at Utah Fertility Center and their degree of success.
It’s been almost two years since my last egg retrieval, but because of my diminished ovarian reserve, we have experimented with very high-dose stimming, and also lower-dose stimming, and saw similar success. We also experimented with Human Growth Hormone to see if it would increase egg numbers and in my case it did not. Dr Dorais always consulted with me and made sure I was aware that higher meds weren’t a guarantee for higher egg count.
Describe your experience with your nurse at Utah Fertility Center.
I’m not sure if she currently works at UFC anymore but I remember she seemed overworked and unorganized. Hopefully she got some additional staff or help somehow.
Describe your experience with Utah Fertility Center.
The nurse seemed overworked and unorganized. The staff performing ultrasounds were usually fairly compassionate considering it’s a compromising and uncomfortable clinic visit for patients. The phlebotomists were pretty casual comfort-wise and good at their job.
Describe the costs associated with your care under Jessie Dorais at Utah Fertility Center.
I think my discount for the clinic was 20% because of my cancer diagnosis. I also got compassionate care discounts for my prescriptions. I think without insurance, my cost was around $8,500 each time I had an egg retrieval.
What specific things went wrong at Utah Fertility Center?
Failed to call in prescriptions to pharmacy
Describe the specific things that went wrong at Utah Fertility Center.
There was confusion with the nurse and myself over who would be calling in the prescription. It was pretty stressful for me.
Dr Conway was incredibly helpful in helping me understand my body, and the different treatment options available. She was able and willing to step in when needed for complicated procedures. I appreciated the time she took to explore possible factors and offer advice and support...kind and supportive. She was able and willing to help in exploratory procedures
I've experienced working with the [Utah Fertility Center- Pleasant Grove] clinic pre-Covid and during the height of COVID. They have always been kind and caring, and have always presented with a smile on their face. The only weakness I would consider is that their waiting area literally is cold...I have worked with several different nurses at this clinic, and most all of them were phenomenal. Those that weren't, were still kind and compassionate, but didn't go above and beyond like the others did

Describe the protocols Deirdre Conway used in your cycles at Utah Fertility Center and their degree of success.
Its hard to remember all of the details. I remember taking birth co tell, progesterone, Femara, progesterone suppositories. I completed HSG twice, as well as water ultrasound in order to rule out any additional concerns.

Describe the costs associated with your care under Deirdre Conway at Utah Fertility Center.
Again, it is hard to remember all of the costs. I think in total we have spent about 10,000 directly with this clinic, with additional costs for required medications. I have tried to avoid IVF due to financial concerns, but have reached a place where I understand I need to move forward and can't rely on IUI.
Describe Deirdre Conway's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Utah Fertility Center.
NA, I have not yet utilized eSET. Plan to this summer.
Dr Conway is kind and compassionate. She was willing to answer questions I had and always willing to schedule another appointment to talk over more questions if needed. I did not see her every appointment, but she acted as if she knew me well and was patient and did not make the situation feel to serious...Dr. Conway prefers to transfer only one embryo at a time, but is open to discussion options...
Once assigned an IVF nurse coordinator [at Utah Fertility Center], it was easy to reach her and communicate and get questions answered. The people who did the vaginal ultrasounds made it seem so normal and not awkward at all....My 3 IUIs including meds abs everything we’re about $4000 total. My IVF cycle including meds and everything else cost about $16,500...

Dr. Conway is very kind and caring. She makes you feel comfortable and doesn’t make the situation feel too serious.
Describe the protocols Deirdre Conway used in your cycles at Utah Fertility Center and their degree of success.
Dr Conway had me start with 3 IUIs, which none ended in a pregnancy. We met again after the 3rd failed IUI to talk about the next steps. Because my diagnosis is unexplained infertility, she could not really know for sure if trying another IUI even had a chance to work. She recommended IVF at that point, as research shows the chances of IUIs working after the 3rd try go down. With my IVF cycle she recommended freezing the embryos and then waiting and doing a frozen transfer a month later, after my body had time to he. This was really good for me because I got really sick and miserable after the egg retrieval and my body needed that time. She also had me do progesterone shots starting about 5 days before my transfer and for 10 weeks to help prep my uterus and keep it the best environment to keep the baby in there. I was happy with how she helped me.
Describe your experience with your nurse at Utah Fertility Center. (Assigned nurse: Morgan)
I had good experience with all the nurses I worked with. I prefer to lay down while getting blood draws abs they were always willing to accommodate me.

What specific things went wrong at Utah Fertility Center?
I rarely ever felt rushed and he patiently answered all of my questions...He is extremely compassionate and understanding. He's willing to discuss alternative approaches. He's very willing to provided suggestions to help with issues that come up. I've had no major negative experiences with him. Sometimes it's difficult to follow his train of thought or he's over eager to share thoughts and I'm not totally sure we're on the same page.
Strength: it's a very nice atmosphere, very clean, the bathrooms are very well stocked. Weaknesses: partner was uncomfortable in the the semen collection room and hearing people walk past the door. Calling the [UFC] clinic often requires leaving a voicemail...Nursing staff has always been friendly and competent...The office was great about providing a list of fees to expect. We were only given a range to expect on medication cost which was stressful.

I've had no major negative experiences with him. Sometimes it's difficult to follow his train of thought or he's over eager to share thoughts and I'm not totally sure we're on the same page. I'm uncertain of his fine motor skills and have questioned his ability to handle delicate procedures with skill. My egg retrieval was surprisingly painful and another brief procedure also made me question his fine motor skills.
What's one piece of advice would you give a prospective patient of Jason Parker at Utah Fertility Center?
Educate yourself and come with as many questions as you can and he will answer them. This is a very overwhelming process but you can't expect your hand to be held through this entire process. The best advocate you can have is yourself.
During treatment, were you treated like a number or a human with Jason Parker at Utah Fertility Center?

Describe the protocols Jason Parker used in your cycles at Utah Fertility Center and their degree of success.
I did not do birth control prior to IUI or IVF at my request. My IUI used femara and a trigger shot. IVF was part of a double blind study. I had a lot of uncomfortable reactions to the medications but nothing overly concerning. Headaches and nausea affected my ability to work. Bloating was uncomfortable and a lot of aching and stabby pain from ovarian stimulation.

Describe Jason Parker's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Utah Fertility Center.
I was not swayed one way or the other. I could have been more educated on the potential risks. After experiencing a single pregnancy, I would never risk a multiple embryo transfer.
What specific things went wrong at Utah Fertility Center?
Failed to call with results
Failed to consider drug intolerance
Describe the specific things that went wrong at Utah Fertility Center.
I think less prepared patients may have issues knowing what steps to expect and to push for details they may not understand. A lot more could have been done to help alleviate the symptoms and discomfort I experienced during the first trimester. It seemed like the standard bare minimum was suggested when I was really struggling to function.
When doctor Foulk has met with us he is always is present. He explains complex science in an understandable way. He is never rushed. He has a calm demeanor that is calming for us...He acted with urgency and suggested 3 rounds of IUI before performing IVF. He remembers our life situation when he talks with us and asked how my husband's grad school was going
[the Utah Fertility Center nurses] are very efficient and we felt in great care. They always called back same day to answer questions. We felt like they had our back and did great at showing us next steps...They are organized and very on time. They know your name and where you're headed. They label specimens at bedside and always confirm patient name and date of birth. Very impressive. They have RN's perform the IUI. Nurse practitioners perform the ultrasounds. That was my experience. They had seamless scheduling between the Pleasant Grove and Murray locations.
How was your experience with Russell Foulk at Utah Fertility Center?
I'm 34 and no other doctor noticed my AMH levels or took concern until doctor Foulk. He understood the low AMH meant my egg reserve was as if I was in my late 30s. He acted with urgency and suggested 3 rounds of IUI before performing IVF. He remembers our life situation when he talks with us and asked how my husband's grad school was going. We feel very confident in him as our doctor.
What's one piece of advice would you give a prospective patient of Russell Foulk at Utah Fertility Center?
Ask all your questions and trust his process.

When doctor Foulk has met with us he is always is present. He explains complex science in an understandable way. He is never rushed. He has a calm demeanor that is calming for us. He is very sought-after as a doctor and his schedule is very full. Sometimes appointment times have been changed last minute (day-of) because of his schedule.
Describe the protocols Russell Foulk used in your cycles at Utah Fertility Center and their degree of success.
Doctor Foulk recommended only 3 rounds of IUI because the statistics of getting pregnant after 3-4 rounds doesn't go up. He performed a hysteroscopy to remove 5-6 polyps. Two were on my cervix and he said they may have acted as sentinels preventing sperm from entering the uterus. We had our first egg retrieval today. I took Menopour and Gonal-F for 10 days and had 3 ultrasounds to monitor follicle growth. I took a ovulation preventing drug for two days before the trigger shot. I took Pregnyl as the trigger shot because during my first IUI the trigger shot made me itchy at the injection site. We are awaiting results from the lab for fertilization of eggs and survival of embryos. It was recommended I eat a low carb diet and drink lots of electrolytes to get the ovarys back to the normal size.

She [Dr. Conway] took the time to explain everything thoroughly to us, she was kind and compassionate while still being honest about the likelihood of the success of IVF for us, and when it came time for our transfer, she made me feel safe and comfortable...She preferred to transfer only one embryo for us and after her explaining the pros and cons of each option...
My IVF coordinator [at Utah Fertility Center] was great. She was always quick to respond to me and never made me feel like I was bugging her even though I had a lot of questions. The entire nursing staff always made me feel safe and cared for.....my retrieval, transfer, and any weekend appointments were done at the Pleasant Grove location, which is their main office. It is definitely a much nicer office as far as the building goes, but the Murray location was nice as well. The staff at both offices were great....
How was your experience with Deirdre Conway at Utah Fertility Center?
I truly couldn't have asked for a better doctor. Our first visit had to be done as a video call since it was in the midst of covid, and even with that I still felt like she truly cared about our fertility journey. She took the time to explain everything thoroughly to us, she was kind and compassionate while still being honest about the likelihood of the success of IVF for us, and when it came time for our transfer, she made me feel safe and comfortable.
What's one piece of advice would you give a prospective patient of Deirdre Conway at Utah Fertility Center?
I wouldn't hesitate for a minute if you are considering working with her. Don't be afraid to ask questions. She won't make you feel like you are annoying her, she genuinely cares.

Describe the protocols Deirdre Conway used in your cycles at Utah Fertility Center and their degree of success.
For our retrieval cycle, the month before retrieval, she had me start birth control on cycle day 3 and take that for 22 days. We scheduled a saline US between cycle day 6-10. She had both my husband and I take a Z-pack with the last pill to be taken one week before my baseline US. 28 days after cycle day 1, I started taking baby aspirin daily, had my baseline US, and bloodwork done. The following morning, I started stims with Gonal-F150 IU and Menopur 1 powder and continued both of these throughout the rest of my retrieval cycle with bloodwork on day 3 of the meds, and ultrasounds and bloodwork on days 6, 8, 9, and 10. We added in Cetrotide daily on day 6, did one extra dose of Gonal-F on day 10 as well as my trigger shot, with egg retrieval on day 12.
For our FET, we did a modified natural cycle per my request. On cycle day 2, I had my baseline ultrasound, bloodwork, and began taking baby aspirin daily. Cycle day 3-7 I took Femara 5mg. I took OPKs on CD8-11 to make sure I didn't ovulate too early. CD 12, 13, and 14, I had follicle ultrasounds and bloodwork and did a trigger shot on CD 14. 3 days after my trigger shot, I began a Z-pack and I started taking Prometrium 200mg vaginally 3x a day which I continued through 10 weeks of pregnancy. My embryo transfer was 7 days after my trigger shot.
She also had me take prenatal vitamins throughout all of these cycles and throughout my pregnancy.

I went to both the Murray location as well as the Pleasant Grove location. Most of my bloodwork and ultrasounds were done at the Murray location because it was closest to me, but my retrieval, transfer, and any weekend appointments were done at the Pleasant Grove location, which is their main office. It is definitely a much nicer office as far as the building goes, but the Murray location was nice as well. The staff at both offices were great. I didn't meet anyone who left me feeling uncomfortable at all.

Describe Deirdre Conway's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Utah Fertility Center.
She preferred to transfer only one embryo for us and after her explaining the pros and cons of each option, we fully agreed with her.
Dr. G listened to our concerns as our previous clinic didn't. We felt so heard and involved in our treatment plan...Very compassionate and understanding of where we were in our infertility journey. We jumped right into IVF when we switched to UFC and it was an amazing experience! Even as we went through IVF we couldn't have hoped for a better experience or outcome...We transferred 1 during our first fresh transfer. Then the next 2 transfers, with warning, we transferred 2 each time. We had a singleton
Only thing I had to follow up on during my cycles [at Utah Fertility Center] were to push to get transfers quicker. They would put the calendar together and put it at least a month and a half out so I had to push for sooner transfers all 3 times...We transferred 1 during our first fresh transfer. Do the consultation! You will fall in love with Dr. G! She helps make the rough patch of infertility treatments a little easier and helps you understand as you go as well as involves you in a game plan moving forward (especially after our chemical pregnancies when we went back to the drawing board!)
During treatment, were you treated like a number or a human with Shawn Gurtcheff at Utah Fertility Center?

Describe the protocols Shawn Gurtcheff used in your cycles at Utah Fertility Center and their degree of success.
First 2 transfers, estrogen started day 3 after ultrasound. Lining checks throughout cycle. 5 days before transfer started PIO. Both ended in chemical pregnancies
Transfer 3 we added 2 "experimental procedures" (deemed here in the US as experimental) to try to help our embryos stick. Still did estrogen and PIO in same sequence and added triple progesterone, so oral progesterone 3 times a day and vaginally at night after transfer. The other procedure we added was lipid infusions. I got one a week before transfer, after confirmed pregnancy test and then once a month until 20 weeks. We truly believe that infusion is what helped our embryo continue to grow!!

Dr Conway spent time with us to come up with a plan. She reviewed my history and had some ideas of things we could try after experiencing 7 miscarriages...we tried using plaquenil which was something I hadn’t done during any of my miscarriages before. It’s hard to know if that’s what worked but with my autoimmune conditions it seemed to help! We also made sure my thyroid levels were exactly where we wanted them before transferring and I think that made a difference.
[the Utah Fertility Center nurse] was responsive and patiently answered my many questions! She was also accommodating when we wanted to do something a little different with our most recent transfer...I think the clinic is organized and well run. The staff is compassionate and caring. They have handled Covid precautions well and been communicative of the requirements in place before appointments.

Dr Conway was always very warm and welcoming when we met with her. Her staff really knew us and treated us like humans and not numbers.
Describe the protocols Deirdre Conway used in your cycles at Utah Fertility Center and their degree of success.
I don’t remember my protocol for the egg retrieval. But for my two transfers (both of which were successful - first one resulted in a live birth and second I’m 36 weeks pregnant with and things are going well) we tried using plaquenil which was something I hadn’t done during any of my miscarriages before. It’s hard to know if that’s what worked but with my autoimmune conditions it seemed to help! We also made sure my thyroid levels were exactly where we wanted them before transferring and I think that made a difference.
Describe your experience with your nurse at Utah Fertility Center. (Assigned nurse: Lacey)
Lacey was great to work with! She was responsive and patiently answered my many questions! She was also accommodating when we wanted to do something a little different with our most recent transfer. My husband wanted to take the call and potentially choose the gender or at least know and I wanted it to be a surprise. I don’t think they had that request very often and Lacey made it happen!

I don’t remember. We were lucky to have insurance cover a lot of it.
Describe Deirdre Conway's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Utah Fertility Center.
Dr Conway left it up to us but we decided to transfer one at a time. With so many miscarriages I worried my body wouldn’t be able to support a twin pregnancy. She was supportive of that decision.
[Dr. Parker] is extremely business oriented. His bedside manner isn't as compassionate as I expected, but he is extremely knowledgeable...I saw [him] for our initial consultation, the egg retrieval, and then the transfer. So of all of the appointments I had, I only saw my doctor 3 times...on a call to discuss results, he wasn't sure why we were calling and we had to remind him it was about the analysis of my husband's sperm.
The clinic [at Utah Fertility Center] was always clean, but the staff seemed to be unorganized. They have an online portal to view your medical documents, bills, and labs - but the site only seems to work half of the time. You have to specifically ask for itemized bills, otherwise you just get a lump sum due each billing cycle... only saw my doctor 3 times...I felt passed around between different clinic staff during all of my IVF process. I appreciated Dr. Parkers candidness and his medical/scientific explanations.
How was your experience with Jason Parker at Utah Fertility Center?
Dr. Parker is incredibly intelligent. He was able to talk with my partner and I, discuss why we were experiencing infertility from a scientific perspective and answer all of the questions we had.
We did have some hiccups with communication, since most of our comms were virtual. A few calls were either missed or late, or on a call to discuss results, he wasn't sure why we were calling and we had to remind him it was about the analysis of my husband's sperm.
What's one piece of advice would you give a prospective patient of Jason Parker at Utah Fertility Center?
He is extremely business oriented. His bedside manner isn't as compassionate as I expected, but he is extremely knowledgeable. He will answer your questions, you just may need to be persistent in asking them.
During treatment, were you treated like a number or a human with Jason Parker at Utah Fertility Center?
Some of my interactions weren't with Dr. Parker at all, and I was mostly working with the clinic nursing staff for appointments. I saw doctor parker for our initial consultation, the egg retrieval, and then the transfer. So of all of the appointments I had, I only saw my doctor 3 times. I also had a nursing staff change mid-IVF cycle, and no one was contacting me about scheduling, so I had to follow up. It was a frustrating experience that required getting the office manager involved. I felt passed around between different clinic staff during all of my IVF process.
I appreciated Dr. Parkers candidness and his medical/scientific explanations.
Describe the protocols Jason Parker used in your cycles at Utah Fertility Center and their degree of success.
My husband's sperm analysis came back with low morphology and motility. Based on these results, Dr. Parker decided to skip IUI or a procedure on my husband altogether, given our ages and goals for starting a family. His recommendation to go straight to IVF streamlined our treatment and set us up to be pregnant 4 months later.
Describe your experience with the nursing staff at Utah Fertility Center.
The nursing staff in office were always kind during visits. Most appointments didn't take very long either. The issue I had was mid-IVF cycle, my main nurse either quit or was laid off, and my treatment and care fell through the cracks. I had to call and complain and speak with the office manager to figure out the rest of my care, while I was in the middle of treatment. It was very frustrating. As a patient, you can tell the nurses handle MANY patients at a time, and miscommunications seemed to happen often.
Describe your experience with Utah Fertility Center.
The clinic was always clean, but the staff seemed to be unorganized. They have an online portal to view your medical documents, bills, and labs - but the site only seems to work half of the time. You have to specifically ask for itemized bills, otherwise you just get a lump sum due each billing cycle.
Describe Jason Parker's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Utah Fertility Center.
The office recommended eSET to us, and I agreed. With the few embryos we had, and risks during a multiples pregnancy, we appreciated this recommendation.

Describe the specific things that went wrong at Utah Fertility Center.
Mid-IVF cycle, my coordinator either resigned or was laid off. I was left with no communication from the clinic until I realized and called that they had missed a call to me and we were getting closer to my egg retrieval date. I had to speak with multiple nurses because no one could tell me who my coordinator new POC was, and it eventually was escalated to the office manager to resolve. It was frustrating, and made me feel most "like a number" because my treatment fell through the cracks.
Shawn [Dr. Shawn Gurtcheff] was incredibly informed. She was able to cite recent research and provide data on best practices for our situation. She was honest about the success rates of our situation and spent the time to explain costs and what to expect with assistance in the costs. She walked through the process but also explained that there would be deviations in the process and the deviations didn’t mean anything was wrong only that my care was specific to me and could change based on hormones and need for changed timing, etc. Going into the procedures I knew exactly what to expect and what the cost would be. There were no surprises and I was given the opportunity to make the decisions with the doctor at each visit. I was so scared during the process because I am an anxious person, I would call all the time and Shawn would call me back on her personal cell after hours to comfort and reassure me.
I loved this clinic [Utah Fertility Center]...They were kind, attentive, respectful, professional, and understanding of the hard and confusing situation we were in trying to have a child. They never made any of concerns feel unwarranted. They always were willing to work around our work schedules and change locations to have labs done so I wouldn’t need as much time off work. The office was always organized and clean. They were always ready for me and there was hardly ever a wait time. The staff was plentiful, efficient and knowledgeable. The only weakness was the location. I live about an hour and 15 min away from the clinic. Since my visits they have built an office 10 min away. So this is no longer a weakness.

Just go for it with her, if you have any doubts ask she is so kind and willing to answer anything or research it and follow up with you. You will safe and at ease in her care.
During treatment, were you treated like a number or a human with Shawn Gurtcheff at Utah Fertility Center?
Shawn took plenty of time to talk with me and my husband, especially after our first transfer that failed. I was devastated. She answered so many questions, explained why this could have happened, helped me to feel confident in another try. She also would call me herself to discuss labs and how my embryos were growing.
Describe the protocols Shawn Gurtcheff used in your cycles at Utah Fertility Center and their degree of success.
Shawn first used a rapid hormone cycle. I was able to produce many eggs but only 6 made it to 5 day embryos and then only 2 were grade B at implantation. We implanted one and it didn’t take. Then when I did my next round with her we had to do a re-retrieval since none of the embryos from the first round survived. She talk to me about doing a long cycle using hormone for a month before retrieval. She explained that with my PCOS many times women require the older methods. She went into great detail of why this was the case. I felt so much better the second round. I used less hormone injections because my body responded better to the hormones. The eggs retrieved were much better quality and thriving. We implanted day 5 embryos, 2, and I ended up with twins. Super healthy and completely normal twins.
Describe your experience with your nurse at Utah Fertility Center.
My nurse was so kind, even with my 100 phone calls, she was patient, understanding and respectful. She told me to call at anytime for any questions so there wasn’t ever any confusion during the process. She was always willing to provide options and make the process easy to navigate around my full time work schedule.

Describe the costs associated with your care under Shawn Gurtcheff at Utah Fertility Center.
The costs were comparable to all the other fertility centers in Utah. I did feel that Shawn and her nurses had amazing ideas on grants, medication assistance, using the cheapest pharmacies for meds and fertility insurance.
Describe Shawn Gurtcheff's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Utah Fertility Center.
Shawn said she always recommended single embryo transfer because of the risk to the mother and babies if there are multiples. She explained that length that she never implants more than two and she always recommends one. She also said that of course there are better odds of success with two embryos at once. She made sure to inform myself and my husband thoroughly before we chose to implant two. She was never forceful or upset either way, just kind and knowledgeable about our decision.
[Dr. Russell Foulk] was so nice...had a good bedside manner...treated me well and addressed any questions I had with patience. If I was concerned he always would get back to me...worked with my husband with his sperm retrieval...used a low dose birth control in the beginning...Constant labs to measure where I was.
[At Utah Fertility Center] They would shut off phones right at 5pm or during lunch hour. I always had to make sure to make time during my work schedule to get a hold of them...would tell me they would call after 5 and often didn’t get back to me until way after hours...had to switch a coordinator because I was having a bad experience with the first one with her lack of communication
How was your experience with Russell Foulk at Utah Fertility Center?
He was so nice. I was very awkward and he had a good bedside manner. He treated me well and addressed any questions I had with patience. If I was concerned he always would get back to me. He also worked with my husband with his sperm retrieval.
What's one piece of advice would you give a prospective patient of Russell Foulk at Utah Fertility Center?
Ask questions if you have any and you’ll know everything of what to expect. If you don’t have that, ask for a different coordinator.
During treatment, were you treated like a number or a human with Russell Foulk at Utah Fertility Center?
He always treated me with respect and addressed my concerns.
Describe the protocols Russell Foulk used in your cycles at Utah Fertility Center and their degree of success.
He used a low dose birth control in the beginning, and told me why we were doing that. As an infertility patient I felt that was ironic but it made sense when he explained the reasoning. Constant labs to measure where I was.
Describe your experience with the nursing staff at Utah Fertility Center.
They would tell me they would call after 5 and often didn’t get back to me until way after hours and I didn’t have a way to contact them which was very frustrating. I actually had to switch a coordinator because I was having a bad experience with the first one with her lack of communication
Describe your experience with Utah Fertility Center.
They would shut off phones right at 5pm or during lunch hour. I always had to make sure to make time during my work schedule to get a hold of them. They did well with texting me when I could come in-this was during the pandemic (summer of 2020).
Describe the costs associated with your care under Russell Foulk at Utah Fertility Center.
I think all together was $17,000 for IVf and 1,500-1800 for sperm retrieval
Describe Russell Foulk's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Utah Fertility Center.
I chose to transfer 2 embryos and he warned me of the risks but was willing to try it out with our situation.

Describe the specific things that went wrong at Utah Fertility Center.
I had to have a scheduling coordinator change in the middle of doing an ivf round to retrieve eggs and it was stressful because it was my first time and I wasn’t getting the care I expected but once I got a new coordinator everything was much smoother and I was more at ease than stressed.
[Dr. Dorais] showed great empathy for the rough road that led me to her...always remained patient and listened to all of my concerns...She was open to a lot of the requests I had, such as doing an endometrial biopsy, and even added steroids to my protocol to help with egg quality. If there was something I wanted to try, but she didn't feel was necessary, she would give me a detailed research/fact based response as to why she didn't feel it was necessary for me to do which I really appreciated...
I asked her [nurse at Utah Fertility Center] a lot of questions and she always answered promptly and took each concern to Dr Dorais so I always felt important. She sent me detailed information to help each step of the way and told me what I needed to focus on at that moment so I wasn't confused or overwhelmed. She even helped me find some medication that was donated to help with the cost of my cycle...Each office has friendly staff who always greeted me with kindness. They were always prepared and I rarely had to wait in the waiting room before my appointment....
How was your experience with Jessie Dorais at Utah Fertility Center?
Dr Dorais is the best of everything at what she does. I could not have asked for better results with my IVF cycle and I can not thank her enough for doing all she did to ensure I had the most positive experience with no regrets. Dr Dorais was very open and made sure that I understood the success rates for my age range so I knew exactly what I was getting myself into.
I am not going to lie, I was a needy patient, and needed a lot of reassurance on things throughout the process. Dr Dorais always remained patient and listened to all of my concerns. She acknowledged everything I said, even when I was being overly anxious about something. She was open to a lot of the requests I had, such as doing an endometrial biopsy, and even added steroids to my protocol to help with egg quality. If there was something I wanted to try, but she didn't feel was necessary, she would give me a detailed research/fact based response as to why she didn't feel it was necessary for me to do which I really appreciated, because she knew I wouldn't be content with just hearing no I don't need that.
During my cycle, she was able to retrieve 21 eggs, 19 of those fertilized, 8 made it to 5 day embryos which we had PGT tested. Of those we tested, 6 came back abnormal and 2 were normal, 1 boy and 1 girl. I was a little disappointed we didn't have more normal embryos, but Dr Dorais was very pleased with the results. I wanted to transfer both embryos, but since Dr Dorais wanted us to have the best possible chance at having a successful pregnancy we made a plan to transfer only one of them. We were on a time crunch to do the transfer because my insurance with IVF coverage was going to lapse soon. I started my FET with estrogen and when I went in for testing my levels were optimal sooner than expected so she pushed up the date of my transfer by a week. Dr Dorais took the time to call me after hours to explain the change herself because she knew I might stress out since the new date of my transfer would be during her vacation she already had planned. I was terrified of the change and even though I tried to hide it she could tell. Dr Dorais did something I never expected and even offered to postpone her trip by 1 day so she could do my transfer. After I had a few minutes to process things, I told her no that would not be necessary. I trusted who she suggested to do my transfer. Dr Conway in the same clinic did my transfer a week later. I still stressed for various reasons, but despite all of that I wouldn't change a single thing we did because it resulted in a healthy baby girl!
What's one piece of advice would you give a prospective patient of Jessie Dorais at Utah Fertility Center?
Like you should with any doctor, be prepared with a list of questions beforehand. Don't hesitate to ask anything, even if you think it's stupid. She was very impressed with the type of questions I had because she knew I did some homework and I feel like it made her respect me more and make it easier to communicate things with me.
During treatment, were you treated like a number or a human with Jessie Dorais at Utah Fertility Center?
From my first appointment with Dr Dorais I felt like she genuinely cared about me and showed great empathy for the rough road that led me to her. She was kind and genuine from the start. She comforted me when I became emotional about my miscarriages and assured me it was nothing I did wrong which no other doctor bothered to take the time to do. She was the first doctor to take the time to really listen to me which was very important to me.
Describe the protocols Jessie Dorais used in your cycles at Utah Fertility Center and their degree of success.
We did an estrace priming protocol and added dexomethasone to aid in egg quality.
Describe your experience with your nurse at Utah Fertility Center. (Assigned nurse: Amy Mitton (IVF Coordinator))
She was very friendly and knowledgeable. I asked her a lot of questions and she always answered promptly and took each concern to Dr Dorais so I always felt important. She sent me detailed information to help each step of the way and told me what I needed to focus on at that moment so I wasn't confused or overwhelmed. She even helped me find some medication that was donated to help with the cost of my cycle.
Describe your experience with Utah Fertility Center.
I had most of my testing at the Murray office and procedures at the Pleasant Grove office. Each office has friendly staff who always greeted me with kindness. They were always prepared and I rarely had to wait in the waiting room before my appointment.
Describe your experience with your monitoring appointments at Utah Fertility Center.
There was rarely a wait in the waiting room. Once I was in the room I don't remember waiting more than 5 minutes for someone to come in and perform the ultrasound.
Describe the costs associated with your care under Jessie Dorais at Utah Fertility Center.
Egg retrieval and frozen embryo transfer were billed separately through the clinic. My insurance covered 80% of it. I paid for medications separately and insurance covered $10,000 of it.
Describe Jessie Dorais's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Utah Fertility Center.
I assumed we would transfer 2 embryos because that's what I had and I assumed transferring both would increase the chances of a viable pregnancy. But since we did PGT testing we already removed the chance of the pregnancy failing due to chromosomal abnormalities which I know for a fact contributed to 1 of my miscarriages. Dr Dorais took into consideration my age, and desire for a girl since I already had 2 boys, and since we only had 1 normal girl embryo from my egg retrieval it made sense to transfer just 1.
What specific things went wrong at Utah Fertility Center?
Failed to call with results
Describe the specific things that went wrong at Utah Fertility Center.
There was only one time when I was waiting on my HCG results after my transfer that a nurse didn't call with my results before they closed. I already knew I was pregnant, but was eagerly waiting for the results to give me piece of mind that my levels were rising. I was able to email my IVF Coordinator who then called me not even 30 minutes later!
Dr. Gurtcheff is knowledgeable and compassionate. Although we were given a hard diagnosis she had a way of giving us hope. Describe the protocols Shawn Gurtcheff used in your cycles at Utah Fertility Center and their degree of success.
Trying to conceive our second child Dr. Gurtcheff recommended doing a mini stim cycle. With the hopes of reducing my chances of OHSS while still producing good quality embryos. She took into account the fact that we already had a son and wanted to only have 1-2 more children. She prescribed femara along with gonal-f. I haven’t completed the cycle yet so we will see if it will be successful.
Describe your experience with your nurse at Utah Fertility Center. (Assigned nurse: Morgan )
Morgan is really good about returning voicemails promptly. She has also been great at making sure my future cycle is organized and that I feel prepared. I did have a few instances where so left a voicemail to the other staff and was never called back. Other than that every phone call has been positive.

I had a surgery with Dr. Gurtcheff. On surgery day she explained that she wanted to see me during a post op appointment. I tried calling the staff to schedule a post op appointment and was never called back. When I did finally speak to my nurse she said I wouldn’t need a post op appointment. One of my incisions ended up getting infected. And although it probably isn’t correlated with having a post op appointment maybe it could have been caught earlier.
He [Dr. Foulk] is the most caring doctor. I have had some really hard times and he always delivered the hard news with so much empathy. I am proud to be one of his patients. He is very organized and made me feel like I was his only patient. I even remember one time he made sure he called me even though it was 8 pm. He wanted to make sure I was okay and that my questions were answered.
The nursing staff at Utah Fertility Center is amazing! They treat you with so much love and compassion. Lacey was our IVF coordinator and she was so helpful through everything. She made sure our meds were right and she spent countless hours helping us. Every time I see her she greets me with a smile and makes my husband and I feel at ease. The nurses are an extension of they doctors at UFC and they take such good care of their patients.

UFC and the doctors there are so caring! They want to help you reach your goals of having children. They do everything in their power to help you get there. They are there for the low of lows and I hope they will be there for the high of highs! They care so much about you and want to help you get pregnant. They have tremendous bedside manner.
Describe the protocols Russell Foulk used in your cycles at Utah Fertility Center and their degree of success.
I started off by doing shots and medicine for an egg retrieval. After a few weeks we realized that I was not producing follicles like I should so he cancelled my egg retrieval. The second time I did shots and I also did testosterone gel on my arm. I did produce follicles. We were hoping for 6-8 eggs and when I had my egg retrieval I had 3 eggs. 2 of them made it to day 4 and 1 made it. My embryo was tested and came back abnormal meaning it wouldn’t implant. I then decided to do some IUIs and I did 2 back to back with clomid and femara and a trigger shot and that has led me to right now. We are meeting with them tomorrow to figure out our next steps. To say this has been a long road would be an understatement but we will do anything to get our babies here safely.

Utah Fertility Center is amazing at communicating with their patients. They are organized and they get appointments scheduled quickly. They are always on schedule and you can count on them to not keep you waiting. The front desk is so inviting and they know you by name as you walk in. You feel loved there. I honestly cannot think of weaknesses. I have had nothing but amazing care at their clinic.
Describe the costs associated with your care under Russell Foulk at Utah Fertility Center.
IVF-$7500 in medicines and then paying for anesthesia for egg retrieval and money for testing of the embryos.
IUI-we have had 2 of them and paid about 1,000 for each one.
We have also paid for progesterone suppositories several times which cost a couple hundred each time.
Dorais is my number one! She is literally the nicest person I have ever met. She is brilliant and so reliable and has so much knowledge about this. Her patience is 100%, and she will answer every SINGLE question you have until you understand. She explains things in a manner that makes sense to people not in the industry. She is solid in her suggestions and support. If there is something that you are concerned with, she will let you know every outcome no matter the severity.
UFC I feel like is helpful but mostly is trying to hit a quota. Everyone is nice and helpful but I believe there is a protocol that they have that isn't necessarily my preference...I liked that every nurse was able to help instead of one specific nurse being assigned to a doctor. They did always move quickly with getting back to me...The clinic itself I think treats you like its their job vs. helping families create a life.

There are even times that I have told her we were uncertain about things, she has provided options but has kept us in the loop to explain why it would/wouldn't be the best. I absolutely would recommend her to anyone! She is what I think people look for in a doctor when they are looking to be cared for. She is the only doctor (in any field at all) that makes me appreciate a doctor and the help that they give.
What's one piece of advice would you give a prospective patient of Jessie Dorais at Utah Fertility Center?
She will have your back. And she really will put her heart into this with you.
During treatment, were you treated like a number or a human with Jessie Dorais at Utah Fertility Center?
We have seen Dorais at multiple centers. This review is for UFC in Utah. She is an amazing woman and so extremely knowledgeable and helpful. At this clinic, I feel like she has to follow guides differently than at the UofU. Because she has to go off of a protocol from UFC, I feel like when we got pregnant at UofU there are things she cannot suggest the same. The clinic itself I think treats you like its their job vs. helping families create a life.
Describe the protocols Jessie Dorais used in your cycles at Utah Fertility Center and their degree of success.
We have used Clomid at the maximum strength every treatment (100, or 150mg?) Trigger shot most of the times. There are times when I didnt agree with using it because we had other success with OPK. We only did a baseline ultrasound the last 3 of 12 because of choice. She wanted to be sure that I had 3 large eggs each time because of the difficulty of getting us pregnant. The last 4 to 5 treatments of 12 we used Gonyl F for 4 days (dont remember the dose). I responded well to the Gonyl F, Trigger and Clomid well together. However, we are to the point where we need to move onto IVF and we would complete this with ICSI if the sperm count was low, and we would do a frozen cycle.
Describe your experience with the nursing staff at Utah Fertility Center.
Nursing staff was always helpful. They always found out answers to my 100000 questions. I liked that every nurse was able to help instead of one specific nurse being assigned to a doctor. They did always move quickly with getting back to me.
Describe your experience with Utah Fertility Center.
UFC I feel like is helpful but mostly is trying to hit a quota. Everyone is nice and helpful but I believe there is a protocol that they have that isn't necessarily my preference - it could be my preference but I still haven't received clarification on why they use certain protocols. They say that they are open 24/7, that would make me think that they will work with you any day. But when you are actually getting treatment, they will make you trigger on a day so they don't have to come in on a Sunday or certain holidays. I want to trigger when I need to be triggered for success and not have it based on their preferred schedule. We use donor sperm and have to be even more specific with our arrival for the procedure, etc. The hardest and possible second most stressful experience was when we had an IUI procedure on a Tuesday morning. I receive a phone call on Wednesday afternoon that the andrology department thought my appointment was on Monday and they thawed one of our vials of donor sperm for my procedure. That was stressful because of the cost of the vials and because my donor no longer donates, so that was a vial (almost) completely wasted. They remedied it by refreezing and letting me know they "could" possibly use it for a procedure but the sperm count would be half of what is was originally. They on their own accord called my cryobank I use and ordered another vial at no cost. I see that they did their due diligence in fixing the issue but the issue was a very scary one. I think that since the 3rd IUI out of the 12, I have felt more negative towards UFC because of the examples I have given but I have faith in my doctor so we have continued the path with Dr Dorais at this time.
Describe the costs associated with your care under Jessie Dorais at Utah Fertility Center.
More expensive but very comparable to UofU
Describe Jessie Dorais's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Utah Fertility Center.
She strongly suggested to do only single embryo transfer. She WOULD do multiple eggs but would notate that we went against her suggestion.
What specific things went wrong at Utah Fertility Center?
Lost or damaged samples
Describe the specific things that went wrong at Utah Fertility Center.
The donor sperm vial that was thawed by mistake on their end was the biggest issue we have had yet.
Dr. Gurcheff is literally the best doctor I have ever seen and worked with in my life. This is such an emotional and stressful journey to be on, and from the 1st time I walked through the door, she completely calmed me down, explained everything in detail to me, was incredibly encouraging, and has a wonderful bedside manner. She is almost like a "reproductive therapist" as well as a highly skilled Doctor/embryologist. She is an amazing human being and absolutely incredible doctor.
[Utah Fertility Center has a] Great Atmosphere, great team, positive and encouraging environment. Everything is very clean and they are very organized. They always call when they say they will call...[the nursing staff] are compassionate and friendly. They are knowledgeable and warm. They are genuinely concerned about the outcome you experience and are on board with you every step of the way

Dr. Gurtcheff is an amazing doctor. She is incredibly intelligent, highly compassionate and competent, and very friendly and warm. She made me feel so comfortable in her presence and I always felt very hopeful and motivated after speaking with her. She is very detailed in her explanations and she is with you every step of the way. She calls you personally on the phone to explain something, instead of having a nurse call. She instilled a feeling of complete trust, hope for the future, and faith in me. I had been lacking this for a while, as infertility is a very stressful experience. I always knew I was being taken care of and Dr. Gurtcheff genuinely cared about getting us to our destination.

Describe the protocols Jessie Dorais used in your cycles at Utah Fertility Center and their degree of success.
We are a lesbian couple so obviously TI was not something we were capable of trying. When we first started with Dr. Dorais we had no reason to believe we would have any trouble getting pregnant so for the first two IUIs we didn't do any pharmaceutical interventions. Those didn't work so we ran some tests, and found I had a blocked fallopian tube which was able to be fixed in office. We then adjusted to doing IUIs with Clomid. Those didn't work either. We ran more tests and discovered I had polyps in my uterus. I had surgery to remove those and we moved on to IVF. We used a pretty standard protocol of Menopur, Follistim, Cetrotide and an HCG trigger shot we got 17 eggs but only two embryos. Only one embryo tested normal through pgs testing. We transferred it and I got pregnant. However it resulted in a missed miscarriage at around 11 weeks. Babe stopped growing somewhere around 9 weeks. We are trying IVF again when covid-19 is over. Our stimulation cycle has been increased slightly and we have added Novarel to the trigger. We had taken almost a year off to improve my health with supplements that are shown to improve egg quality, and to work on my nutrition.

It was definitely an efficient process. I am almost always able to get in for appointments that make it easy for me and my schedule (not having to get a sub for an hour or so is huge for a teacher).

We have insurance that covers fertility treatments, so I can't remember exactly. However I feel like what we're going into this time (where we haven't met our deductible yet) is pretty typical for an IVF cycle cost wise.

Describe the specific things that went wrong at Utah Fertility Center.
The first IUI they thawed two vials of sperm to use without discussing with us first. That was frustrating as it's precious and we only have so much to work with. It's never happened since though.
