Because my employer has Carrot Fertility and Cigna insurance, my costs were very minimal compared to the top line numbers. Ex/ the day before my egg freezing, Weill Cornell called and said I could either a) Bill my retrieval to the insurance on file or pay $1500 out of pocket (which sounded nuts at the time). I told them to go through my insurance and the insurance covered it, as I had hit my full $3,000 out of pocket deductible by that point. After all the charges/ insurance coverage was done, I paid ~$2,000 from my HSA then used my Carrot fertility benefit for the $4,000 medication charges.

Dr. Melnick and her team are warm and friendly. She has been honest with us during treatment and it feels like she's on our side to provide the best care that she can without taking unnecessary steps. She tends to be direct and fast paced, which I found to be helpful during before work monitoring appointments. She tailored her approach to provide a softer level of care when I was going through a miscarriage.

Our first appointment with Dr. Melnick was after one year of TTC on our own with no success. Various tests (started by my OBGYN) led us to unexplained infertility. Dr. Melnick's recommended protocol started with natural IUIs. The first natural IUI was negative. The second natural IUI resulted in a pregnancy, but ended in miscarriage after about six weeks. We tried a third natural IUI that was also negative. For the fourth IUI, we added 50mg Clomid. This IUI resulted still in only one follicle and a negative. Our current status is that we are going to do one more Clomid IUI with an increased dose of 100mg. We wanted to start to plan ahead and have already had a conversation with Dr. Melnick about IVF. I've appreciated that Dr. Melnick is taking into consideration my age, test results, and desired family size when working through the protocol and treatment rounds.

The entire staff at Dr. Melnick's office on 68th Street has been very helpful and caring - Holly, Jersey, Connie, Jenny. Jersey and Connie are the coordinators and they're always quick to answer messages and schedule appointments. Jenny typically works with Dr. Melnick during morning monitoring appointments. She makes me feel really at ease with the blood work and her blood draws are never painful. I've only interacted with Holly over the phone, but I've found her to also be informative and kind. The office on York Ave is larger and is not Dr. Melnick's primary office (patients only go there on Wednesdays). I've found the nurses there to be less personal, which I can understand because they're seeing a lot more patients.

Dr. Melnick understood how stressful fertility treatments can be and acknowledged the added stress of going through them during a pandemic. She made time for video visits with my husband and I to answer questions between appointments about next steps in our treatment. During a Saturday monitoring visit near the beginning of my IVF cycle, I saw one of the Fellows who had told me he wasn't seeing many follicles and that more may not grow or I could lose some. I was incredibly upset leaving this appointment thinking I would end up with a cancelled cycle. I wrote to Dr. Melnick to ask for her thoughts on my progress and she let me know that it was quantity not quality that mattered most. Dr. Melnick told me that for the remainder of the cycle (outside of weekend appointments and procedures which are performed by whichever doctor is in the OR that day), she wanted to make sure that my appointments were with her so that she and I could discuss my progress. If I hadn't already felt confident with Dr. Melnick, that experience made me trust her completely.

Dr. Melnick spoke to me candidly but warmly. She always remembered where we were and what was going on with my current cycle. I also had her direct email address and could ask additional questions in between appointments. I felt like a human being and knew I was in good hands.

I started on day 2 of my cycle with 150 units of Gonal F and 150 units of Menopur. The following week I started Ganerelix (which made me incredibly itchy). I had a double trigger before the retrieval (Lupron 80 units and Pregnyl 1500 units). Day of retrieval I started azithromycin (5 days) and Medrol (16mg once daily for 4 days). The day after the retrieval I started progesterone injections (1mg a day) and estrogen patches changed every other day--I was on progesterone and estrogen patches until the 7th week of pregnancy.

Dr. Melnick's entire nursing team is spectacular though I spoke to Holly Nadal most frequently. During the IVF cycle, I received a call from the nursing team every day I had gone in for monitoring with next step instructions before 2pm so I had a good understanding of what to do that evening. Prior to beginning the IVF cycle, Holly called me one afternoon to walk me through the entire process including specifics on the injections. She sat on the phone with me for an hour answering all of my questions and told me to email with any follow up questions or concerns. My emails were always answered on the same day and if I had a complicated question, a nurse would call with my answer. It would have been very difficult to get through fertility treatments (especially the IVF cycle) without this nursing team!

Dr. Melnick is in a satellite office two blocks from the main Weill Cornell RE office. I had appointments in both locations. Dr. Melnick's office is smaller and quiet--the staff are really responsive and friendly. The main office is a little more of what you may expect from a hospital doctor's office however, I felt like everything moved quickly and the staff were responsive there as well. My appointments were all during the COVID pandemic so, there were many fewer patients in the waiting room at a time than there would have been previously. I was able to book specific times for all of my monitoring appointments.

My health insurance covered $10,000 lifetime maximum in fertility treatments. Other than co-pays associated with visits that were note part of the IVF cycle, and the drug co-pays, I payed about $7000 out of pocket after insurance (this was for 3 IUI cycles and one IVF cycle).

Dr. Melnick, my husband and I had all been on the same page about a single embryo transfer. My husband and I did make it clear that if the changes of a singleton were higher with two embryos transferred, we were open to it. On day 3, Dr. Melnick called after talking to the embryologist and the recommendation was to go with two embryos (both embryos with a chance around 50%) to increase the likelihood of the transfer working. We were fine with this recommendation and ended up with a singleton pregnancy.
Dr. Melnick was always calm, answered my questions using data...gave me realistic expectations for how many egg retrievals we would need to bank enough embryos to test for my BRCA mutation and have enough for 2-3 kids...3 cycles, I got 36 fertilized eggs (using ICSI) and 19 embryos. 13 didn’t have BRCA and 9 of those were chromosomally normal.
[Weill Cornell Medical College is] really big. Find a good office for monitoring that isn’t the UES because you’ll be waiting there for a while. When my first retrieval only produced 3 embryos (before any genetic testing) she adjusted my protocol and the next cycle I got 9 embryos to send for testing. I’m so grateful.

Antagonist short protocol with Follistim 300/Menopur 150 + Ganirelix and Leuprolide Acetate + HCG for trigger. Cycle 1 = 7 eggs, 1immature, 5 fertilized resulting in 1 5-day blastocyst. Cycle 2 = 7 eggs, 3 immature, 4 fertilized resulting in 3 5-day blastocysts. Cycle 3 = 9 eggs, 9 mature, 7 fertilized resulting in 2 5-day blastocysts. All blastocysts were rated 3BB. One fresh transfer failed, one frozen transfer failed, and another frozen transfer succeeded. One transfer cycle was cancelled due to thin uterine lining that did not respond well to medication. The successful transfer was done naturally with no meds to “thicken” the lining.

Dr. Melnick is a great reproductive endocrinologist. She is friendly and trusting and is also straight forward and efficient. She made a pretty scary and daunting process approachable and manageable.
We were dealing with male factor infertility and on the whole, I'd say my protocol was pretty straight forward so I cannot speak to her ability to diagnose and create protocols based on different cases.

Every time I saw Dr. Melnick, I felt like I was speaking with a close friend. She is warm and caring and will spend time explaining everything and answering your questions while also being straight forward and efficient. It's worth noting at Weill Cornell that once you start treatment, you do not see your doctor for every step of the process. I saw Dr. Melnick once during monitoring and other doctors on rotation did my retrieval and transfer. Dr. Melnick and her nursing team were always available for questions and called me everyday during monitoring to update me on protocols.

Due to dealing with male factor infertility, I assume my treatment was pretty standard and did not require a ton of customization.
I started on day 2 of my cycle with 100 units Gonal F and 75 units of Menopur. After one day of shots, they reduced the Gonnal F to 75 and after two days, increased it to 87.5 where it stayed for the duration of stimulation. I started one Cetrotide kit per day on day 3 or 4 of shots.
I did 8 days of shots and then did my trigger shot on day 10 of my cycle and the retrieval was 2 days after. I did a dual trigger - HCG and Lupron.
The day after the egg retrieval, I started 1ml progesterone oil shots and estradiol patches.

Jessica, Holly, and Dr. Melnick's whole nursing team is hands down the best nursing team around. There is a nursing email address that you can reach out to any questions and I always received an answer that same day, sometimes within a few hours. At times, it could feel factory-like when you're sitting in a waiting room with 30+ women waiting for daily monitoring but every time you interact with a person, you feel like you're getting the 1:1 attention you need. At times, it can be difficult to get in touch with their pre-authorization team which was frustrating at the start of treatment.
I'm not sure what it's like at other clinics but it's worth noting that you will not see your doctor for everything. Various attendings, fellows, and residents rotate for monitoring and attendings rotate for egg retrievals and transfers. While my doctor was overseeing my entire treatment, I only met with her at the beginning, once when she was on rotation for monitoring, and for my first check up after a positive pregnancy test.

When I saw Dr Melnick I was 1 year into TTC with 1 miscarriage (first month trying) because my labs / test showed no reasons why I couldn't get pregnant again we started with natural cycle IUI. The first cycle was negative and then we tried clomid + IUI for 3 rounds which resulted in 2 chemical pregnancies and 1 full miscarriage. At that point I was certain nothing would work however Dr. Melnick suggested IVF with PGS testing (which was confirmed recommendation by 2 other second opinions). We started IVF a few months later and had a very successful round with 23 eggs resulting in 12 embryos, 6 of which tested PGS normal. Because of my history, Dr Melnick had me take baby aspirin and increased doses of Progesterone during my transfer and early pregnancy. I had a healthy baby boy and am now starting to think about round #2!

(Maybe this is standard practice because nothing was alarming in any of my test results) This has worked out well for us as it has taken the stress away at times. Dr. Melnick and her team are very warm and very efficient. Most of my appointments have been in the morning before work (as it seems with most of her patients). She teaches and has surgeries during the regular hours I would have possibly been able to go in. My experience was always one of an efficient morning with caring people getting things done (reminded me of a family environment getting ready to go school / work in the morning).

I have had 7 rounds of Clomid and 4 IUI’s. My body responded well the first month to Clomid (I believe it was 50mg), but the second month only 1 or 2 follicles so the third cycle the prescription was increased to two tablets. This worked well for the last 5 cycles. Everything she prescribed made a lot of sense. Our insurance does not cover IVF - and, if it did we probably would have tried IVF after 2 IUI’s. She told us that the percentage of IUI working goes down each time, but she worked with us with what made sense financially. Because our insurance deductible was paid, we continued with IUI’s and Clomid through the rest of the calendar year. Now we are gearing up for IVF in this new year.

I now am out-of-pocket that I’m starting the process for IVF and I just paid $600 for my sounding. The IUI procedures along with all the bloodwork and ultrasounds were paid by insurance and I saw some very bizarre negotiations between the clinic and the insurance so I don’t have straight numbers for them. All of my medications needed pre-authorization and I would pay out of pocket mainly due to timing for Clomid, which is quite inexpensive.

Only 3 mature, didn't do ICSI cause she didn't explain it to us, only one fertilized and transferred day 3 and didn't take. The second round, we only got 3 eggs out of what looked like 10 follicles growing. I think I ovulated the eggs early, but of course, she wouldn't tell me that. And she allowed me to believe it was a problem with my eggs. Those two rounds of insurance covered IVF went up in smoke due to her lack of experience and attention to detail. I always sensed I had a carrying issue, cause I've had chemicals and not real miscarriages. I asked for the receptivity and ERA tests on my lining - Melnick told me those tests weren't necessary. My second private doctor (with YEARS more experience than Melnick) has been able to retrieve 11 eggs, with at least four making it to day 5 blasts. I got these numbers two retrievals in a row. Melnick only retrieved a total of 5 eggs over two retrievals. My current doctor also did the biopsies on my lining and discovered I have endometriosis - something Melnick didn't think to look into - even after requesting testing. For 3-4 days after, I experienced nausea and dizziness that I've never experienced from being under anesthesia before.
