Since we are an LGBT couple with no known fertility issues, Dr. Lange recommended starting with IUI using a sperm donor. I did 2 medicated cycles using Letrozole and Ovidrel as a trigger shot. The first round was unsuccessful. The second round resulted in an ectopic pregnancy. We will be moving towards IVF due to the concerns of a higher chance of another ectopic pregnancy...In our case due to age and egg reserve, Dr. Lange recommended when we proceed with IVF to do a single embryo transfer.
IUI cycles [at RMA of Philadelphia] were about 1,200 with monitoring but not including medications...IVF cycles range from 10,750-26,000 depending on need not including medication...The clinic starts you off with a visit with the doctor of your choice to discuss a potential treatment plan. They discuss any procedures or bloodwork you may need to get done to start the process. The clinic is made up of multiple offices so some doctors do travel between clinics. Therefore, you may not always get to meet with your specific doctor if you go in for bloodwork, ultrasounds, etc. If visits are not time sensitive, such as baseline, you can try to schedule with your specific doctor. You also have the option of scheduling appointments at different offices if you they are closer to you. The King of Prussia office is open 7 days a week. I live a distance away from my clinic so they work with me to get bloodwork done local to my house to try to help with travel. While you may not see your specific doctor, the other doctors are equally qualified and willing to help. My IUI procedures were done by a different doctor and I still felt
How was your experience with Allison Lange at RMA of Pennsylvania?
Dr. Allison Lange was very knowledgeable and thorough when she created our treatment plan. She was able to walk us through the plan and answer any questions regarding our specific case. When I had blood work or ultrasounds done, if she was not the doctor on call, she still reviewed all of the results and made the final decisions. When we went through our unfortunate ectopic pregnancy, she was there to tell me the next steps. Dr. Lange went out of her way to personally make arrangements for me and made sure that I understood everything fully. She was very compassionate and made me feel like I was important. After, she personally called to check in on how I was doing physically and emotionally. She set up another appointment to review our treatment plan and answer any questions we may have had.

One piece of advice would be to go in with an open mind. She will take the time to review your case and create a plan for you based on your specific needs or goals.

During our visits Dr. Lange was very welcoming and talked to us about our specific case. Our second IUI ended in an ectopic pregnancy. Dr. Lange sat with us and explained everything that was happening and comforted us. She also called after to check in on us following the procedure and creating a new plan.

The nurses were very compassionate, professional and willing to help. They called the same day with test results when applicable. If I sent an email via the secure portal, I always received an answer promptly.

Changed my protocol after I failed a medicated round of ivf to a natural medicated cycle...natural cycle where I took letrozole, vitamin E, aspirin, a trigger shot and the PIO...really helpful during this process as it was confusing and scary
The front desk [at RMA of Pennsylvania] is so welcoming...doctor or nurse practitioner does the ultrasound....try to get you in with your primary doctor as much as possible. But depending on your protocol you may see different doctors or the NP...nurses are very compassionate and caring

During treatment, were you treated like a number or a human with Allison Lange at RMA of Pennsylvania?
Dr. Lange cared for me through two egg retrievals, my first pregnancy and my current pregnancy. She has given me great news and bad news. But through all of it I knew she cared and was on my side. She did have to tell me I had miscarried and her compassion made me feel like I was not alone in that moment.
Describe the protocols Allison Lange used in your cycles at RMA of Pennsylvania and their degree of success.
My initial round did not work, I was taking estrogen along with PIO. I had an initial rise in HCG but then decline. She then switched me to a natural cycle where I took letrozole, vitamin E, aspirin, a trigger shot and the PIO. This significantly helped my lining thicken. I was able to get pregnant with this. My second pregnancy initially took using the same protocol. But then tried to split into twins where neither then actually took. I did use misoprostol to start my period. I then did another round where my lining did not get thick enough, so no transfer. We then did the same protocol but added follistim which worked very well. The transfer did work, but the HCG was initially borderline, I did eventually have a miscarriage at 8 weeks. I had retained product. Dr. Lange was really helpful during this process as it was confusing and scary. I went to an OB-GYN (who was wonderful) at her recommendation and had the product removed in the OR. We then did another round, same protocol, and I had a successful transfer and am now 12 weeks pregnant.

The nurses are very compassionate and caring. Arieanne was my primary nurse and I could tell she really cared about how I felt. During my miscarriage, Earline at the surgery center took such amazing care of me. The nurses are on your side through it all

The front desk is so welcoming. The lab does an awesome job, even on tough sticks. The doctor or nurse practitioner does the ultrasound. They try to get you in with your primary doctor as much as possible. But depending on your protocol you may see different doctors or the NP.

My first pregnancy was covered by my insurance. My second was out of pocket. It was $3900 for the Frozen egg transfer. $800/year for embryo storage

Dr Lange provided great treatment for my 5 IUIs; although they did not result in pregnancies my medication dose, timing, hormone levels, etc. were good...very thoughtful, whether at their facilities for treatment or over zoom for consultation. No complaints on their "bedside manner"...5 medicated IUIs with HCG trigger (ovidrel or pregnyl). First medication was Clomid then switched to Letrozole. Clinic also put me on Synthroid.
RMA has highly skilled and intelligent doctors, they are well resourced ($$$) which means great systems/technology and quality nurses (they are not short staffed). If I stuck with them, they would have likely gotten me pregnant, at some point, with IVF. Unfortunately, when I went for a second opinion once I knew IVF was my next step, my next second clinic was alarmed at my endometriosis and, honestly, a little confused why I knew so little about it and I hadn't had imaging or specialized conversations about it.
How was your experience with Allison Lange at RMA of Pennsylvania?
Dr Lange provided great treatment for my 5 IUIs; although they did not result in pregnancies my medication dose, timing, hormone levels, etc. were good. Unfortunately, RMA did not provide quality service when it came to suspecting endometriosis. At the time of my treatment there I had a large suspected chocolate cyst. They never went through the symptom list to help me further diagnose, never dived into my medical history (maternal side has endo), never requested I go for "proper" imaging at ultrasound/MRI center, and didn't recommend I speak to a specialist. We breezed through it instead of taking the time to investigate - I guess this was outside of fertilty care for RMA. Turn out, I had stage 4 and needed surgery.
What's one piece of advice would you give a prospective patient of Allison Lange at RMA of Pennsylvania?
Get a diagnosis - don't automatically agree with an initial unexplained. It's easy for clinics to call it that.

Doctors and nurses were very thoughtful, whether at their facilities for treatment or over zoom for consultation. No complaints on their "bedside manner"! They want to help their patients.

I had to have a hysteroscopy, they removed a 3.5in endometrioma mass and was diagnosed stage 4. Like I said, they probably could get me pregnant with IVF and probably treat endo patients all the time, but I didn't trust my doctor after this and a new clinic was a better fit for me on a couple of factors.

I only did IUIs which was covered by insurance (IBX Keystone East). Each visit I had a co-pay. The only add-on would have been genetic testing for my spouse and myself, which thinking back was a little odd and pushy since nothing ran in our families and I was way under the typical female 35 age threshold at the time. I don't recall their IVF pricing.

I feel like she [Dr. Allison Lange] didn’t jump into doing anything until we had all our ducks in a row. She truly is one of the nicest doctors I have dealt with over the past 3 years and an overall genuine and kind person...We made adjustments like synthroid for my borderline tsh, also started me on letrozole for a month to suppress my system for my endometriosis, along with testing my pituitary by ct scan due to elevated prolactin levels and started me on a medication for hyperprolactinemia, and I will get intralipid infusions for elevated NO cells . And will adjust my progesterone injections from the results of my era. To say the least I think that is very thorough and shows she is trying to everything she can to make this cycle work.

When i first met with dr Lange she thoroughly reviewed my records from previous doctors and cycles and explained how we would approach treatment with her and what to expect. Once I went through an egg retrieval and then frozen transfer which resulted in a chemical, she recommended further testing as far as a miscarriage panel and ERA biopsy. I also recommended some further tests like natural killer cells and she agreed and wrote for it. Once we reviewed all results she took a different approach this time around which i am currently in the process of getting ready for a transfer. We made adjustments like synthroid for my borderline tsh, also started me on letrozole for a month to suppress my system for my endometriosis, along with testing my pituitary by ct scan due to elevated prolactin levels and started me on a medication for hyperprolactinemia, and I will get intralipid infusions for elevated NO cells . You are in good hands with a genuine and kind hearted physician that truly cares about her patients and developes a plan that works for you and if you have any questions or recommendations in your care she will take it into consideration and help as much as possible.

I felt like a human because every time I had questions or concerns Dr Lange would personally reach out to me via phone or in person and explain in detail what was going on. She did not push the work off to a nurse or anyone else. I would ask her what she would do in certain circumstances and she would be honest and tell me what she felt she would do and why. She is very compassionate and recommended different tests that she thought we help me during my treatment. I feel like she didn’t jump into doing anything until we had all our ducks in a row. She truly is one of the nicest doctors I have dealt with over the past 3 years and an overall genuine and kind person.

Unfortunately I don’t have results yet because I am getting ready for another transfer which I discussed earlier the different protocols she is currently using in my care.

She [Dr. Lange] was so thorough in the consultation and asked us in-depth, probing questions to get to know us and our history so well. There was no perfunctory presentation. The consultation was engaging, personal, and customized. She was also positive...Her instructions and care communication are all done through her nurse, who I loved, but when I needed to talk to her directly to get her thoughts on what path we should take moving forward as we debated another cycle, she was there for me.
They were welcoming, friendly, personable, and efficient! I didn't know before this experience how a fertility clinic worked and how different doctors rotate through the daily monitoring schedule, the testing and retrieval surgery center, etc. It's a machine...She [nurse] was 100% thorough, instructive, clear, compassionate, and reliable. She responded so quickly when I had a question or needed anything. I was in touch with her more than anyone else in the clinic, and I never had any doubt about her being invested in us, and I think she was a major part of our success and our overall happiness with RMA.
How was your experience with Allison Lange at RMA of Pennsylvania?
Dr. Lange was methodical, consistent, extremely knowledgeable, and made me feel like she was truly invested in us and our success. I never thought that she missed anything or wasn't paying attention or was hesitant to make adjustments as needed. She studied my chart, my case, my results, and I felt like I was in very good hands throughout all 4 cycles. Her personality is somewhat reserved and matter-of-fact and not overly chatty or social, so I didn't feel the same rapport or camaraderie with her that I had with some of the other doctors at the clinic, but because I felt so comfortable and confident with her oversight and medical care, I didn't especially need that from her.
What's one piece of advice would you give a prospective patient of Allison Lange at RMA of Pennsylvania?
Expect her to take excellent care of you "behind the scenes" but probably be more reserved in the in-person appointments and not spend her valuable time making a lot of small talk while doing the exam. Her instructions and care communication are all done through her nurse, who I loved, but when I needed to talk to her directly to get her thoughts on what path we should take moving forward as we debated another cycle, she was there for me.
During treatment, were you treated like a number or a human with Allison Lange at RMA of Pennsylvania?
From the initial consultation, I knew Dr. Lange would be "the one". She was so thorough in the consultation and asked us in-depth, probing questions to get to know us and our history so well. There was no perfunctory presentation. The consultation was engaging, personal, and customized. She was also positive. At my age (almost 41 at the time of consultation), I knew the odds, but talked about possibilities and how this could work, not all the reasons why it might not.
Describe the protocols Allison Lange used in your cycles at RMA of Pennsylvania and their degree of success.
My first two cycles were antagonist protocols - 150 Menopur & 300 Gonal-F for a few days, then I started Cetrotide, and finally we did an Hcg trigger shot 36 hours before egg retrieval. Dr. Lange noticed that despite my advanced age, I was a hyper-responder to the stimulants/gonadotropins, and some of my follicles were growing very very rapidly right away. It created asynchronous follicular growth, meaning we would have to trigger when at least half of them were ready, but it left many of them small and immature. So I had a large number of eggs retrieved but only about 50% of them were mature. Dr. Lange then moved me to an agonist protocol in which I would start Lupron 10 or more days before the stimulants to fully suppress my hormones, then go into 150 Menopur and 300 Gonal-F while continuing a very low dose of Lupron until the final Hcg trigger shot. This protocol dramatically increased the total number of eggs retrieved, and the percentage of mature eggs retrieved did slightly improve in the third and fourth cycles as well.

Sammi and I got to know one another very well during our year of 4 IVF cycles! She was 100% thorough, instructive, clear, compassionate, and reliable. She responded so quickly when I had a question or needed anything. I was in touch with her more than anyone else in the clinic, and I never had any doubt about her being invested in us, and I think she was a major part of our success and our overall happiness with RMA.
Describe your experience with RMA of Pennsylvania.
From the front-desk staff to the nurses that had to draw blood to the entire team of doctors that rotate each day for monitoring, I had a wonderful experience. They were welcoming, friendly, personable, and efficient! I didn't know before this experience how a fertility clinic worked and how different doctors rotate through the daily monitoring schedule, the testing and retrieval surgery center, etc. It's a machine. They have to time things so carefully that the system has to work perfectly. As I continued to watch how they organize appointments and calls to maximize effectiveness and efficiency and stay on top of every patient's care, I really was in awe. I have nothing negative to say about the clinic at all. Even though Dr. Lange was my primary doctor, I always felt that there was a huge team of people working for us and rooting for us the entire time.
Describe your experience with your monitoring appointments at RMA of Pennsylvania.
Monitoring was an efficient, well-oiled machine for sure. You were given about a 2.5-hour window early each morning for these, and regardless of whether there was 1 person in the waiting room or 10, I felt like I never waited for more than 15 minutes to be seen. It was a very quick in-and-out for bloodwork and ultrasound monitoring. Dr. Lange was more often in another clinic than in the one I went to, so I didn't see her as much for monitoring, but that didn't bother me at all. I got to know the other doctors who did monitoring in my clinic location quite well and really loved them, and I always knew that at the end of each day, Dr. Lange was the one reviewing my results and communicating them to my nurse Sammi to communicate to me. I was very impressed with the entire system.
Describe the costs associated with your care under Allison Lange at RMA of Pennsylvania.
My insurance covered all of my fertility testing and my medication, but it did not cover treatment. An IFV cycle with ICSI was about $14,000 and included embryo transfer. The only extra costs each cycle were the egg retrieval (about $400) and the genetic testing for any embryos, which was done through Foundation for Embryonic Competence.
Describe Allison Lange's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at RMA of Pennsylvania.
My doctor and the clinic strongly recommended that only one embryo be transferred at a time, and they explained why via the increased risks to mother and baby(ies), and I did not hesitate to follow their advice. Even though we ended up with 2 euploid embryos after ICSI and genetic testing, we only transferred one embryo per their recommendation, and we're glad we followed that recommendation.

For example, I did not want to do the PIO shots and so instead I was able to do progesteron suppositories. We also added another medication to my IVF retrieval cycle at my request. Even though I didn't see her at every visit, she always reviewed my results and determined the next step in the treatment process. I was consistently given enough information to make informed decisions.
What's one piece of advice would you give a prospective patient of Allison Lange at RMA of Pennsylvania?
She's great, but like with any doctor I would recommend you do your own homework and be prepared with questions. Any doctor can only do so much and you have to be your own advocate.

Dr. Lange listens to her patients and is willing to change or modify treatment in response to their individual needs.
Describe the protocols Allison Lange used in your cycles at RMA of Pennsylvania and their degree of success.
I did 4 IUIs, each more progressive. The first was self-monitored, the second was monitored in the clinic, the third I used Clomid, and the fourth I used Letrozole and a trigger shot. For my first IVF retrieval I used a microdoes of lupron, Follistim, Menopur, and omnitrope (which we added at my request; I've see research that it can help with egg quality) plus the trigger shot. For my second IVF retrieval we used Ganirelix, Follistim, Menopur, and omnitrope plus the trigger shot. After getting a positive result on the ReceptivaDX test, I then had 2 shots of lupron depot (each lasts 30 days) and took Letrozle during those 60 days. After the suppression I used Menopur to grow a dominant follicle and then triggered and started my progesterone for my transfer. The transfer was successful and my son just had his one month birthday!

The nurse who works with Dr. Lange's patients was always kind, helpful, and quick to reply. I could ask her anything and I knew that I would get an honest and timely response.

The nurses and staff are the strength of this clinic. The doctors are great of course, but the nurses and staff are the people who keep it all going. The nurses who do the bloodwork, go into the room with the doctor, and give callbacks are particularly important; they are the people patients see most consistently since the doctors rotate between offices. They are compassionate and understanding and provide much-needed emotional support to the patients. The staff, such as the women at the reception desk, are the first faces patients see or hear when they call. They are always kind and helpful, and I appreciated knowing that when I called I would get to speak to a person right away and didn't have to go through a long automated menu of options.

IVF is expensive, and you're kidding yourself if you think it's not. I was able to apply for a needs-based discount, which helped greatly. I will also say their finance team is quick to reply so I was never waiting around for days trying to get information on expenses. I was also due a refund after the end of one cycle, and it was processed quickly and without any issues.

After my HSG, a polyp was discovered, which was later surgically removed. Then we attempted a few natural cycles with Clomid, one of which resulted in a pregnancy and then another miscarriage. After my miscarriage, Dr. Lange ordered a recurring pregnancy lab panel, which confirmed an underactive thyroid. Dr. Lange prescribed thyroid medication, which she periodically adjusted based on my bloodwork. I then had 4 IUI cycles with the plan to move on to IVF after the 4th cycle. I had an IVF consultation, where the process and all the medication were explained to me, along with a financial consult. The 4th IUI ended up in a successful pregnancy, and I was discharged from RMA after the 9th week of pregnancy. I am now 19 weeks pregnant and progressing!
