We went to Fora after 3 failed IUI attempts (wit a blocked tube) at another clinic.
Given that one of my tubes was blocked, they informed us that it is more likely that there are issues with the other one and recommended IVF. We were already planning on switching to IVF so we were all on the same page.
I underwent typical testing; bloodwork, ultrasound. All was good.
Went through the typical IVF protocol with injections and a retrieval.
They retrieved 9 eggs, and ultimately 4 made it passed all the stages and through genetic testing. This was great news! Statistically, we only should have had 1 or 2.
I was on medication to prepare for embryo transfer and I am currently pregnant from that first transfer attempt.
How competent was Amanda Skillern at LGBTQ+ care?
This was the difference maker at this clinic. They were the only ones (we had been to other clinics) that asked about our whole family plan since my partner and I both intend to carry. After hearing about us wanting to each carry two babies, they not only focused on me since I was going first, but they got into detail about my partner and wanted to do all the same testing on her to make sure that there were no issues that could impact our family plan. The other clinics just brushed her aside and said "Oh, she's young. We'll worry about her in a few years".

They are masters of efficiency. You are usually in and out in about 20 minutes or so. You chat with the nurses, see the doctor for your ultrasound, ask any questions and then you're all set!
If you are someone that is very nervous and needs a long conversation about everything that is going on at every appointment...you will have a small window to chat with the doctors, but I would recommend chatting with the staff or contacting them through the portal. They respond super fast and you can always call them, too.
They explain everything in a lot of detail upfront so we didn't have too many questions throughout the process.

You see the actual doctors for about 2 minutes during the monitoring appointments and then you're left to communicate with the nurses via portal messages only. My nurse specifically asked that I do not call with questions which I found to be odd. If I am taking powerful meds and spending a large amount of money, I want to be informed about the process and protocol chosen

I was showing little response to the meds so they cancelled a monitoring appointment but they did not adjust the dosages or add additional meds. I did not feel like the success of my cycle was a priority.

Protocols and general timelines weren't provided, even when asked. I was left wondering what protocol I would be on and what medicine I would be putting into my body. I only found out what I'd be taking when the medicine was actually delivered. I did not receive any rational for why certain meds were prescribed and others, that I anticipated taking, were not prescribed.
For 12 days, I took 300 IU Follistim and I showing little response (my e2 bloodwork was only 200 after stimming for 12 days). I asked why my dosage wasn't being adjusted or why Menopur wasn't added and I simply told that wasn't the protocol I was on. A few days later, they added in menopur for two days and my e2 went up to 500. I had a AFC of 20 so I expected my e2 to be much higher since it is thought that 1 egg = approximately 200-400 e2. I was told to trigger when my e2 was 500.
I was told by Dr. Crawford that nobody wants to be a unicorn but maybe my E2 blood tests weren't pulling correctly. When you are spending 20k on a procedure, I would prefer to follow the facts instead of hoping the tests were wrong.
I was never provided the option to cancel my IVF round due to my poor response on a low dosage. About 2/3 were not mature as my bloodwork indicated would be the case.

There is a clear conflict of interest with the "Fora Pharmacy" program and providing care that is in your best interest. If you do not use all of your meds, you have to give your unused meds to them. If you use more meds than they planned for, then they will supply meds. I used more meds than what they anticipated but when I showed very little response to the meds, they did not increase my dosage accordingly (as it would be a cost to their clinic).
Fora does not provide a discount for additional rounds as well. I did not have any embryos from the IVF cycle and was not given a discounted rate for another round.
I did not see any strengths in their clinic.

I was put on a typical egg retrieval protocol for PCOS and had great results (29 eggs retrieved, 23 mature, 18 fertilized, 9 developed to 5-day blast stage, 8 were found to be euploid after PGS testing. When my first two transfers ended with negative pregnancy tests, Dr. Skillern suggested a “tickle” procedure to help my lining and also prescribed vaginal viagra. My 3rd transfer was successful. For my second child, Dr. Skillern suggested completing an ERA analysis before transferring an embryo to check progesterone levels and receptivity. My ERA came back pre-receptive, and my protocol was updated to account for additional progesterone. My 4th transfer was successful.

Despite being diagnosed with fibroid (which are a KNOWN cause of infertility), she [Dr. Amanda Skillern] wanted us to move forward with IUI or IVF. I asked about surgery to remove them to which she replied, "No, we'll leave them in. That would be a bloody mess." She only wanted money from us and didn't care about the outcome.... She was candid about me having fibroids but completely focused on getting us to move forward with IUI or IVF. She never provided any other alternatives and was just matter of fact in saying that I had "unexplained infertility" -- which was most definitely not the case after a second opinion
They [Aspire/RMA of TX] don't provide you much information, even if you ask for it regarding IUI or IVF pricing and procedures. That's a huge life decision that you don't just jump into blindly. After two weeks of calling every other day, the info they provided was only high level pricing and zero details. They need to get their stuff together!... When seeking a second opinion, a records request was sent 3 weeks in advance. My new doctor never received any records. When seeking a second opinion, a records request was sent 3 weeks in advance. My new doctor never received any records. While sitting in the doctors office, we called them about this and they blatantly lied to us telling us it had been sent, but they'd send it over now. Still to this day, it was never received by the new doctor.
They also fraudulently coded my consultation as an "infertility" visit. Took months of working with my insurance company and back and forth with the practice manager, but was never able to get it fully resolved. Ended up having to pay out of pocket for what would have been fully covered by insurance for diagnostics because they used the word "infertility".

We found this out after 2 retrievals and couldn't believe this wasn't done before hand. We wasted two potential cycles and hard earned money as insurance didn't cover IVF. Extremely disappointed.
A year after we were called by RMA that we owe money for the storage of our abnormal embryos, never once contacted about this until they asked for money. It was resolved but couldn't believe it when we got the invoice. They offered us a discount for a third cycle and never discussed time restraints when when asked. We had some unfortunate medical issues arise and we unable to commit until a year after. Now the discount is no longer available! A few months ago it would of cost $3,000 with insurance covering upto $10,000 and we're being told $17,000+ out of pocket.

I was fortunate enough to have insurance that covered IVF [at RMA of Taxas in Austin]. I only had to pay 20% after my deductible was met. I did pay for CCS services and that was $2,500 I think cash payment of IVF ranged from 10k-25k...All the staff were fantastic! I had a fertility coach who helped guide me with our plan and medications. She was great at helping and always was available to answer questions.

She will be very frank with you and not sugar coat. She does not talk to much about the future and talks about today and the next step. I'm a huge planner, so it took me awhile to adjust to this, but with infertility it was the best for me. You can't plan to far ahead with this. You need to be in the moment and think about today and the next appointment and what needs to be done between then.

The only hiccup we had was the insurance coordinator forgot to send over the results from my husbands SA to the urologist for his appointment. It ended up not mattering as it was unexplained MFI.
Dr. Skillern is not the most touchy-feely, but she is very honest and pretty easy to talk to. She was completely up-front with us from the get-go, and seemed to communicate with myself and my husband as equals. Also, did every appointment except for blood draws, and was always willing to answer a call or email if I had questions...I always felt like a human, the practice seemed relatively small, never a long wait or crowded waiting room, and the doctors, nurses, and staff were always warm and friendly and answered any questions I had quickly and thoroughly...
The [RMA - Austin] clinic is small and cozy. It is well decorated and relaxing and there was never more than one other person in the waiting room in all my visits. Also I never waited more than 5 minutes for an appointment and the doctor was never double-booked for an appointment time (unlike other clinics in town we tried). The clinic staff is also well organized and easy to deal with in case of insurance issues or other admin questions...

I used the antagonist protocol, with a couple weeks of birth control pills before starting stims. Because of my large follicle count, and risk of OHSS, we planned a freeze-all cycle from the beginning. I started off with relatively conservative dosage amounts of Menopur and Follistim and did reduce as we moved forward. Also, since we did a freeze-all, I triggered with Luperon instead of HCG. I started with 24 eggs retrieved, 20 were mature, and 17 fertilized with ICSI. After six days, I had six remaining embryos for transfer. I was able to transfer my next cycle, after a hysteroscopy to double-check my uterus. Because of the grades of my embryos, we transferred one and it was successful.

I had one primary nurse who took care of my case and scheduling, she was very organized and good at explaining all the steps. She was also very thorough at our IVF class where we discussed my calendar and medications. Also, the rest of the staff including the other nurses and those who took blood and helped out with my retrieval and transfer were great. They make you feel welcome and I remember during my transfer having nearly all the nurses who worked with me while I was being treated in the room keeping my company and wishing me well.

The clinic is small and cozy. It is well decorated and relaxing and there was never more than one other person in the waiting room in all my visits. Also I never waited more than 5 minutes for an appointment and the doctor was never double-booked for an appointment time (unlike other clinics in town we tried). The clinic staff is also well organized and easy to deal with in case of insurance issues or other admin questions.

Unfortunately I did not have insurance coverage for IVF. The clinic offers a service that provided a "bundle price" for the procedure plus medications that was lower than the "list price." For all meds, plus IVF, plus a frozen transfer I paid roughly $16,000. The clinic does a good job billing what they can to insurance and they helped me out writing care summaries and letters to insurance to get me a couple claims covered that were originally denied.
She [Dr. Amanda Skillern] is very blunt. I do appreciate this but prefer more compassion than I got from her. She did not do both of my egg retrieval’s which I didn’t appreciate. Come prepared...The baseline examinations are very quick. If you have questions during a procedure, be sure to let them know you have questions and they need to be answered.

Dr. Skillern and Dr. Crawford have created a fantastic environment at Fora Fertility. The staff has worked for them for years and it felt like they really cared about my progress and our personal journey. Everyone knew who we were and were invested in our success.

Given my consecutive losses, Dr. Skillern immediately ruled out IUI. I had an "ok" number of eggs but bad quality, so quickly she recommended IVF and explained why. She knew we didn't want to chance having additional miscarriages but rather "have a baby" in the most direct way possible.
We started with a typical protocol for egg retrieval - birth control to normalize followed by the hormone injections. Unfortunately, we got only 3 embryos and none were normal. We decided to proceed with a second round, adding HGH. We ended up with 6 embryos, 4 normal. Since I had no uterine issues we forewent a mock transfer in favor for the frozen embryo transfer after genetic testing.

Dr. Skillern was incredibly thorough with me and I felt as though my case was specifically tailored for me and my needs. All my medications/dosages/etc were continuously monitored throughout my treatment and was altered as needed. Everyone was so kind and really listened to me and my husband throughout this process.

For my treatment plan, I used Gonal for about 12/13 days. I responded perfectly, with lots of follicles and was closely monitored throughout the process. Towards the end, my dose was altered to achieve the results we wanted based on bloodwork results. I eventually added in Ganirelix to prevent ovulation. Since I was a high responder, I triggered with Lupron and had an incredibly successful egg retrieval. 32 eggs retrieved, 27 were mature, 20 made it 5 day blastocysts (and subsequently PGT tested), and 13 embryos were chromosomally normal.

I had insurance which was helpful, but overall, it was still really expensive. For one cycle of IVF (egg retrieval, PGT testing, anesthesia, monitoring all included) it was 10K. With insurance all my meds only cost me $500. Embryo transfer was around 2K with subsequent OB ultrasounds costing around $300. In total, the costs were around $12,900.

Dr. Skillern really only transfers a single embryo unless some other medical reason to transfer multiple. This was perfectly ok with us.
I was extremely nervous going into IVF and immediately felt like I was on the right path when I met Dr. Skillern. She was direct and took the time to educate me, and answer our questions. She also brought a wonderful sense of humor, which is so needed in this process to keep your spirits up...She is not a Dr. who just tells you what you want to hear, which can be hard for people looking for certain answers. She will not sugar coat the state of your fertility or your options – so if you're ready for the truth, go see her.
Loved the boutique feel of this clinic, think that helps you feel like a human and not a number. Unlike a lot of offices, Fora feels cool and modern vs. old, cold, sterile and ugly. While it's not necessarily a weakness, it would be nice for them to be able to do treatments under the same roof...This was my first experience going into a clinic and not caring who I saw because everyone was so amazing. I felt equally at home with Rikki and Mel and actually ended up seeing one for most of my clinic visits and one for my transfers so there was some consistency there. I was extremely nervous going into IVF and immediately felt like I was on the right path when I met Dr. Skillern. She was direct and took the time to educate me, and answer our questions. She also brought a wonderful sense of humor, which is so needed in this process to keep your spirits up.
What's one piece of advice would you give a prospective patient of Amanda Skillern at Fora Fertility?
She is not a Dr. who just tells you what you want to hear, which can be hard for people looking for certain answers. She will not sugar coat the state of your fertility or your options – so if you're ready for the truth, go see her.

They genuinely acted like they were excited to see me during my visits. The communication was clear and kind. The care was so good I didn't want to go back to my OB!

This was my first experience going into a clinic and not caring who I saw because everyone was so amazing. I felt equally at home with Rikki and Mel and actually ended up seeing one for most of my clinic visits and one for my transfers so there was some consistency there. Excellent communicators, I never wondered what was going to happen next and they answered all my questions.
Describe your experience with Fora Fertility.
Loved the boutique feel of this clinic, think that helps you feel like a human and not a number. Unlike a lot of offices, Fora feels cool and modern vs. old, cold, sterile and ugly. While it's not necessarily a weakness, it would be nice for them to be able to do treatments under the same roof but going to Aspire was also a great experience and close by so just something to be aware of.

Obviously at my age (38) I would've liked the option to transfer more than one embryo but I understood why she only recommended one.
Dr. Skillern provided all possible options for my wife and I and helped us make the best choice for our LGBTQ family...completely patient-centered care specific to our particular situation and needs, guided by our goal for our family...We chose to do reciprocal IVF...Our first transfer from our first retrieval resulted in our perfect daughter...We are moving back to California next month, but will be back in Austin without question for Baby #2. We couldn't imagine trusting our future with anyone else.

You're also treated as a whole person, going through a difficult process, who deserves to be informed and cared for throughout it. How was your experience with Amanda Skillern at Fora Fertility?
I could not recommend Fora Fertility more. Dr. Skillern and Rikki Richardson were a team made for us; they were consistently honest, careful, thorough, and patient-centered throughout our entire process. My wife and I met Dr. Skillern April of 2020. She did thorough testing and guided us through the options for us, laying out statistical likelihoods of each path and leaving the decision up to us. We chose to do reciprocal IVF and began the first cycle in the summer. Dr. Skillern proscribed our meds and monitored everything to make sure all was aligned for the retrievals and transfers. Our first transfer from our first retrieval resulted in our perfect daughter, born in summer 2021. Rikki is amazing. She was so organized, coordinated everything, cared for us deeply, and made sure everything went as smoothly as possible. Everyday for months, we came in for Rikki's injection clinic, building not only confidence in our path but a beautiful friendship along the way. Our first retrieval resulted in one embryo and our second resulted in three because Dr. Skillern and Rikki assessed and adjusted based on the results from the first retrieval, always working as hard as possible for the best outcome for each patient. Every day with our daughter is a gift and we will forever be grateful to Fora Fertility and our dream team Dr. Skillern and Rikki for making our most beautiful dreams materialize.
