He [Dr. Tjaden] was very direct with expectations. Felt that he cared about outcome. He was straight forward and did not give false hope. He gave advice during first cycle, but left the choice to continue to us...First cycle was typical, low initial levels during bloodwork so medications increased. Low follicle counts led to suggestions of cancelling cycle, but ultimate decision was left to us. Proceeded with retrieval that resulted 7 eggs, but only one embryo made it to transfer.

Cared about my success, did not respond well during injections but allowed me to make the choice to continue during the first cycle.
Describe the protocols Bruce Tjaden used in your cycles at Center for Reproductive Medicine in Kansas and their degree of success.
First cycle was typical, low initial levels during bloodwork so medications increased. Low follicle counts led to suggestions of cancelling cycle, but ultimate decision was left to us. Proceeded with retrieval that resulted 7 eggs, but only one embryo made it to transfer. First cycle ended with an initial negative pregnancy test. Second cycle was started with Lupron micro dose protocol followed by higher initial doses of injections. Produced even less follicles. Went to retrieval with 5 follicles, 4 eggs retrieved. Due to COVID-19, we were far enough along in treatment to continue to retrieval, but all embryos would be frozen. Currently have 2 frozen day 5 embryos awaiting transfer.

Ask all your questions. Question why he suggests a treatment. Don't forget that everyone is different and responds different. It can take lots of time and money and in the end it's up to you to know when enough is enough. That's for any patient with any doctor though honestly.

I felt like a human. Everyone was nice and listened and answered all my questions and never tried to rush.

He started me on a few rounds of just Clomid and then femira. Then added ovidrel. Those didn't work. I had a dye test done before i started doing iui. Did that 3 times and it was a bust. I had exploratory surgery to make sure everything looked ok and it did. I moved in to injectable meds after that for a few rounds which also resulted in neg tests. I have only 1 tube. In the end he said ivf was the only way i would get pregnant, but we can't afford that.

They were all very nice and understanding. If they didn't know the answer to a question you had they would get someone that would know. They know what they were doing.

It's usually a few months out before you get your first appointment. There's usually not many in at one single time. They try to be as accommodating as they can. You can call or email them and they get back as soon as they can.

It all varies on if and what insurance you have on what things are coveted and how much. If you're not seeing the doctor there's no co pay for the cost, it's just billed to insurance first then you.

We have done IUI with femara and a trigger shot each time. We have yet to become pregnant. Rational for IUI was that the sperm would be placed directly into the uterus and increase changes of pregnancy.

We have seen a few different nurses. One in particular, named Mary, has been very kind and explains things very well. The others have been so quick that I don’t even remember their names.

Weakness are that they don’t offer IVF and you have to travel to Kansas City if you want to pursue this option, they don’t have the best hours which makes it difficult to work around my schedule and this adds additional stress, and you are frequently thrown around to different nurses who don’t know your back story and then give conflicting information.

I had a nurse ask me if I had an internal ultrasound during the first couple days of my period as this is standard every other cycle - it was my 3rd cycle and I hadn’t had one any cycle. Also, had another nurse asked me if my femara had been increased to 3 pills instead of two this round as this would be standard after 2 failed cycles but again this was not done.

He explained everything and answered any questions we have... He is blunt and gets to the point...With each cycle I’ve done blood work and vaginal ultrasounds to check for follicle growth...He took the time to explain everything with me and my husband. While determining which medication to start on he said “if you were my daughter, I’d prescribe this.”
All the nurse [at Center for Reproductive Medicine in Kansas] (not sure I’ve met them all) have been nice and helpful. They answered any question I’ve had...So far I’ve enjoyed coming to this clinic. It is clean and calm. The only thing that gets annoying is when you call to speak with a nurse you always get a voicemail. And if you miss the call you end up playing phone tag. On the upside I feel like leaving a voicemail with your question/concern helps facilitate the appropriate answer and gets things (appointments, medication and treatment) moving along.

Started with a vaginal ultrasound to look at my uterus and ovaries. This is where he diagnose me with PCOS. He started me on Metformin and told to continue with ovulation prediction tests. He offered to perform an HSG procedure which we chose to wait and he was okay with it. After months of negative tests we decided to schedule the HSG procedure and a sperm Antalya is test which was both normal, good news! At the 2nd office visit we started on Letrozole 2.5mg and Ovidrel trigger shot. First round was negative, was increased to Letrozole 7.5mg and trigger shot. Second round also negative but since I produced 2 larger follicles this 3rd round we continued with Letrozole 7.5mg. Now on the 2 week wait before testing. Hoping this is works otherwise was told we’d try an extended cycle of Letrozole 10 days vs the 5 days I’m currently on. With each cycle I’ve done blood work and vaginal ultrasounds to check for follicle growth.

1 office visit was $350 which is what was left of my insurance deductible. 2nd office visit was $200. This clinic is not under my insurance 1tier. Ovidrel shots have been $84 thanks to the compassionate program. Look into this for help with medication costs. Letrozole is $10. The vaginal ultra sounds for follicle check have been $116 after insurance. HSG was $350, Sperm analysis done at lab located in same building was $125, insurance didn’t cover this unfortunately. With lab, procedure and medications, I’d say I’ve payed more or less than $1,500 so far

He went above and beyond to make sure we had what we needed...He laid out all of the options from Injections to IVF and didn't leave us guessing about anything. He let us know what approximately things would cost, what we were to expect physically, emotionally, etc., and he never once made us feel like we were the only ones going through this. She was amazing!

Everytime i walked into the clinic, they all knew me and my husband and always talked to us and took great care of us.

We were male factor so he told me he didn’t want me to get tons of eggs because I’m a healthy 24 year old who is dealing with a husband with no sperm. He had me on Gonal-F, Menopur, Cetrotide, HCG trigger and progesterone post egg retrieval. At my retrieval, I had 13 eggs retrieved, 11 mature, 11 fertilized and 6 day 5 healthy embryos. One 5AA embryo was transferred and I am currently 8 weeks pregnant. The other 5 embryos have been frozen.

Megan was absolutely amazing! She always called me back ASAP when I had a question, was always available on email even on a Friday night at 10pm and she was so kind and even gave us free meds because we needed one more dose of menopur! She was amazing!

I paid $4,000 to Dr. Tjaden for monitoring and everything. I paid $7,000 to KU Med in Kansas City and that included all of my egg retrieval and transfer needs! Including anesthesia and all of that.

I wanted to transfer two embryos but Dr. Tjaden was 100% against it because I am 24 and healthy and he told me he knew I would get pregnant and he didn’t want me to deal with multiples or miscarriage. He told me ‘just because we transfer 2 it doesn’t mean you’ll be pregnant with 2 and it honestly doesn’t increase your chances of pregnancy.’ So we did one and are so happy we did!

I have had many friends go to him and had an enjoyable experience. I think if you have a difficult situation that he may not know about, he will more than likely not put in the effort to understand.

In the beginning he simply put me on a dose of Femara and planned on doing no follow up. No sonograms, no labs, nothing. I wanted to ensure I was actually getting follicle growth before I wasted money on an IUI. I had to contact the nurses and ask for a more guided cycle. Otherwise we would not have known if the meds were even working.

I had been on clomid for way too many cycles from previous doctors. He [Dr. Tjaden] right away changed my oral drugs and things started to work a bit better. He tried his hardest to figure out what our fertility issues were, but could never find a definite answer. He monitored my IVF cycle closely and make the decision that a full freeze and frozen transfer would be the safest and result in a better outcome, which it did.

Dr. Tjaden is very ept at repro endocrinology. From the first meeting on he set a plan that we followed. He found a fibroid and the week after he removed it. He also found a small area of Endometriosis that I didn't know about. His bed side manner is exactly what I need which is direct but with compassion. I have recommended him to many of my friends. There are not a lot of African American women who dicuss the stigma of infertility and he has made me comfortable with my journey.

When I was trying to become a patient they made me feel “dumb” and always blamed their lack of organization on “the fax machine” (I’ve heard that same story from many of my friends who have gone there.). They break HIPPA in the waiting room, luckily no one in there that I cared if they knew my business. After being a patient their for several months (9ish) they became more friendly and by the second time we came back they were great. Soooo to a new patient, yes they come off cranky and unprofessional but the outcome of a baby is worth it!

Dr. Tjaden performed 2 separate frozen embryo transfers on me. He prescribed 5 mL progesterone IM injections daily during both transfers. The first transfer resulted in a negative pregnancy test, the second transfer a couple months later resulted in a positive test and pregnancy that lasted until I miscarried at 15 weeks.

Dr. Tjaden had a strong preference for transferring one 1 embryo, based on my age (30), reason for infertility (tubal blockage) and that it was only my second transfer (his first to do on me). On my third transfer (Dr. Tjaden's second to perform on me), he was more open to entertaining my thoughts on transferring two embryos, but ultimately convinced me that 1 was still best.

Rationale: I have anovulation, unexplained infertility. With the medications they could help my body mature follicles sooner & with the trigger shot 'promise' ovulation. This would also give us the best window for our IUI to hopefully work. Initially we chose to do IUI's with oral medication (femara) & a trigger shot (ovidril). 2 IUI's & the last one done in November 2015, ended up in a very early miscarriage. April we started our journey again, but this time with oral pills & injections of Gonal F to help mature the follicles & continuing with the trigger shot (ovidril) with IUI's. April they had as stop mid-cycle as follicles were not as big as they needed to be & they stated it would take too many injections of Gonal F to complete, so we stopped that cycle. May 2016, same protocol: oral medication, Gonal F, Ovidril, with IUI. Then vaginal progesterone 2X/day until your able to test. This was an unsussful cycle. June 2016, exact same protocol as last month. Currently waiting one more week before I can take a HPT.

Insurance doesn't pay for much for infertility. I know lab draws are $15, but insurance usually waves that fee. $170 I believe for all ultrasounds, Blood pregnancy tests $70, I believe. That's all I can think of off the top of my head.

Dr.Tjaden pretty much diagnosed me with endometriosis off a sonogram; however explained that it could only be a confirmed diagnosis with surgery. He explained the pros and cons of surgery as well as the option to use medication or go straight to IVF. Post surgery we decided to start at least aggressive and work our way towards more aggressive IF treatments. We started with several cycles of clomid, then Timed Intercourse, and with Dr.Tjaden's support we will do 2-3 IUI before moving towards IVF. Dr. Tjaden has been very supportive and has provided information to us that we feel has helped make decisions that are best for our situation.

He appeared to be down to earth & it wasn't his way or no way. He very much left our approach up to us & what we felt was best. I don't feel like they explained what will happen next. For example: they explained the meds which I was familiar with from research & experience of taking similar medications before. I don't think they explained what we do IF we were to get pregnant (like do I call my OBGYN & then they take care of me from here on out, or if I called them & they follow me for some many weeks, when do we increase the medication, etc.). This left some confusion. The only other issue I have had, is we did our last IUI on CD21 due to egg size/quality/uterine lining etc. I called to see if this was okay, or if should up the dose. The IVF nurse was a little snippy on the phone telling me "3 pills/day is our max dose." Okay thats fine, so do we need to stay on the 2 pills/day I am currently taking or should we go up? I didn't find out until my next appt that she had talked to the doctor & he agreed to up my dose. When I mentioned getting an IVF consult, she told me that it was expensive between X-X $ amount & that it didn't include the shots, which, "can get pretty expensive!" She also reminded me that payment needs to be paid upfront. I felt like she was short with me & slightly discouraging me from going towards IVF at this point. I don't feel like its a waste of anybody's time to discuss IVF since that is our next step!
What's one piece of advice would you give a prospective patient of Bruce Tjaden at Center for Reproductive Medicine in Kansas?
Ask lots of questions!! How many rounds of this medication do you usually do, if I ovulate late 20-30's or later, do we up my medication dose/change my medication all together, or what is the protocol for that. If/when we get pregnant, do I contact your office or my OBGYN. These are just some basic questions I would ask that were never explained to me.

I feel as though I was human because when he sat down to talk to us it was at an even level. I didn't feel as he was the doctor & we were going to have to do XYZ. Under his [Dr. Bruce Tjaden's] care we did several IUI with injections and Clomid. After several failed attempts we did IVF which resulted in a live birth after a failed fresh transfer. After retrieval we had 23 eggs. 18 fertilized with ICSI and 4 survived day 5. We transferred 2 and got a negative pregnancy test. We did a frozen transfer a few months later. We transferred the last 2 embryos and got pregnant with 1 baby.
I thought they [Center for Reproductive Med] could have had a better phlebotomist. Getting blood draws frequently I think requires someone who can get the needle in quick and easy and less painful. The rest of the staff and care was always exceptional. You rarely saw the Dr until retrieval and transfer days but if I was really concerned or nervous during a procedure or exam he would always come in and calm my nerves and take the time to see me if needed.

Dr tjaden is absolutely amazing. I have referred several friends to him. 2 of which have babies now because of him. He is very kind,patient,knowledgeable and genuinely cares about his patients and staff. If I could seek treatment from him again I would but I moved out of state and have to start all over again with a new Dr. I still tell everyone about him and it's been 6 years since I was in his care. I compare every Dr to him and no one has been as great. I will forever be grateful for him and his expertise and helping me finally become a mom after 5 years of trying.

To understand that a lot of the procedures and visits will be with his nurses. But he is always available to answer questions.

Fertility treatments are expensive and are not covered by insurance. Payments have to be made up front making it very challenging for people to have children when these treatments are needed. This is the same at every fertility clinic.

I don't recall him being strongly against more than 1 embryo transfer. I was more terrified of the risks. We both agreed on 2 embryos to transfer due to the length of time we had already been trying to conceive.

Birth control for almost a full cycle before retrieval. I used Gonal F and Menopur for stimulation hormones before the retrieval. I had a sonogram and blood draw every 48 hours-ish. I had 24 eggs removed so my body responded well. After the retrieval it took another two months before the transfer. More birth control, 2mg 3x daily of estrogen pills, 1ml progesterone oil injections a day (suppositories were offered as an option) leading up to the transfer, and continued injections after. We did not get our happy ending and our going through the second transfer, same protocol.

For the first two cycles I did self injections to trigger timed egg release and I was given Clomid - which ended up causing polyps in my uterus and I had to take a cycle off to allow them to clear. For the following cycle I was given a different drug (I forget the name) and the same injection - I got pregnant that time, my third attempt.

I left like Dr. Tjaden dismissed our infertility struggles. I remember him saying it would be so easy but it sure didn't feel easy nor was it easy.

We did 2 rounds of IUI. Both with Clomid/trigger/IUI. I did not ovulate on my own but with clomid was all to produce 1-2 high quality follicles on day 20. Our second IUI, my partner's count post wash was only 4 million. We were offered to cancel the IUI since it was less than 5 million. We chose to continue the cycle and got our first baby.

This [Ctr for Repro Med] is a busy clinic, but they will get to know you quickly. Be prepared to wait for an initial appointment. Everyone is kind and considerate. First, I had a vaginal ultrasound and an hsg. Then I started on clomid on cycle days 3-7. I had another ultrasound on day 12. I triggered with ovidrel 36 hours before IUI on cycle day 14.

I believe he [Dr. Tjaden] tells you everything he wants to do for you. Doesn't seem quite as open to discussing alternatives with why/why not questions. He doesn't spend much time with you after he is finished talking (somewhat understandable)...I believe he is very friendly..I had sonogram and labs on my very first visit. I liked that because I didn't feel like they were using my time or costing me a lot of money to take off work and drive and a copay. My next visit I had HSG. Things happened quickly. Which I appreciated.

DR. Tjaden took the time to answer all of our questions. For all 4 of my miscarriages he was very compassionate and always checked up on me to see how I was doing. He genuinely cares!

I was on a pretty high doses of injections due to having diminished ovarian reserve. He was upfront with me that we may not retrieve many eggs but it was the quality that mattered.
