Dr. O'Hern took the time to understand and explain how my endometriosis would impact my IVF protocol and potential results. I felt like she understood my condition well and was very knowledgeable about it, and I got a treatment process that was tailored to me. She always takes the time to explain things thoroughly, screen-sharing reports on virtual visits and answering every single question I have...My only complaint is that I didn't see her very often. My egg retrieval and transfer were both performed by other doctors in the practice. But I understand this is common practice, and I did get the results I wanted, so I'm putting that into perspective.
The [Shady Grove Fertility Houston] clinic generally runs pretty efficiently. I almost never waited more than 5 minutes to be seen at my appointments, and the phone calls afterward to discuss results were always prompt. They use Quest for lab work, and there seems to be a lot of turnover of phlebotomists. They are very hit or miss...The clinic staff have also been very friendly and empathetic, and the nurses called me promptly with results, knowing I was anxiously waiting. I always got a call back within the hour if I left a voicemail.
How was your experience with Candice O'Hern Perfetto at Shady Grove Fertility Texas (US Fertility Network clinic)?
Dr. O'Hern is so knowledgeable and took the time to educate me. She never judged me or got annoyed by my many questions. She always addressed both me and my partner, never making him feel left out of the process. She was the first doctor I've worked with who seemed genuinely interested in finding answers and helping me get closure on the underlying reasons I wasn't getting pregnant on my own. That in itself is a HUGE gift, given the uncertainty we face in times of infertility. I will always be grateful for having the answers I needed.
I trusted her with my protocols because she immediately and continually adjusted my medication doses when needed. I could tell she was really paying attention, and I wasn't just put on the same protocol as every other patient.
My only complaint is that I didn't see her very often. My egg retrieval and transfer were both performed by other doctors in the practice. But I understand this is common practice, and I did get the results I wanted, so I'm putting that into perspective.
What's one piece of advice would you give a prospective patient of Candice O'Hern Perfetto at Shady Grove Fertility Texas (US Fertility Network clinic)?
Appointments will go quickly, so be ready with a list of questions, and keep a notebook or IVF planner to jot down notes as she's talking. She talks fast!

Describe the protocols Candice O'Hern Perfetto used in your cycles at Shady Grove Fertility Texas (US Fertility Network clinic) and their degree of success.
Egg retrieval cycle: Birth control for 2-3 weeks, then waited for period start. Day 1 of stims, started at 300IU Gonal-F and 10 IU micro-dose ovidrel (HCG) daily. This is considered a mild-moderate antagonist protocol, I was told. Went in for monitoring on day 5, day 7 and day 9 of stims. After each monitoring visit, she lowered my dose of Gonal-F (down to 225, then 175) because my estrogen was going off the charts. Started cetrotide kits at day 7 to prevent ovulation. Pregnyl 10K unit trigger injection on day 10, then egg retrieval. Throughout my monitoring visits, I had between 20-21 follicles on the ultrasound. At me egg retrieval, they retrieved 11 eggs, 9 were mature, 7 fertilized and 3 became embryos (2 on day 6, and one on day 7). All were frozen & PGT-tested, and 2 of the 3 (the day 6 embryos) came back normal. Our grades were 5BB and 5CB, which Dr. O'Hern said was due to my endometriosis causing issues with egg quality. She expected to see higher quality and quantity from someone my age.
Waited a cycle and got on birth control pills for a few weeks while waiting for the PGT results to come back, and Dr. O'Hern performed a hysteroscopy at this time to remove some uterine polyps and a very small/slight septom.
Embryo transfer: She went over the embryology lab report with us and we selected the best-looking, PGT-normal embryo to transfer from the freezer (grade 5BB). Dr. O'Hern recommended a natural FET cycle, which is what I wanted too, so I was relieved to hear that. Weeks of estrogen and extra hormones could have flared up my endometriosis, so I was grateful to her for being conservative and allowing me to go natural. Started vaginal progesterone and a low dose of oral estrace about 5 days before transfer. Had 4 monitoring visits during the cycle: a baseline appt on cycle day 4, then 2 follow-ups on cycle days 11 and 13. On day 13 my follicle wasn't as big as they like it to be, so I went in one more time on day 15 and I had an 18mm follicle. Took my trigger shot that evening (5K IU Novarel), and my transfer was scheduled for one week later. Dr. Vicki Schnell performed my transfer and told us it went really well. Felt implantation cramping and bleeding on day 4-5 afterward and got my first faint positive urine test at day 7. Lines kept getting darker each day and my first beta was 1317 on day 14, then second was 4381 on day 16. Just had my first ultrasound this morning & everything's looking great!
Describe your experience with your nurse at Shady Grove Fertility Texas (US Fertility Network clinic). (Assigned nurse: Rachel and Brittany)
I talked with several nurses over the course of my treatment, but they were all great. We did have some hiccups in the beginning where my stimm medication doses were inconsistent on my treatment calendar (one day it said 225, then the next day it said 300, then somewhere else it said 350.) I had to call and get that sorted out. During my treatment, they were also in the process of switching software/portal systems, which meant information showed up inconsistently and I had to check both places for a while, and sometimes call to confirm accuracy of things. But the nurses were wonderful and always did the best they could. Sometimes hiccups happen when there's tech changes involved.
Also my nurse who cared for me during my procedures, Pam, is so compassionate and sweet. She knew IVs make me queasy and always accommodated me & never made me feel judged.

They use Quest for lab work, and there seems to be a lot of turnover of phlebotomists. They are very hit or miss. Sometimes you get a confident, practiced and quick person taking blood, and sometimes you get a nervous person who uses giant needles and bruises your arm. When you're being stuck every other day, this can make a big impact on your experience. I also had an ultrasound tech walk in and try to start my scan without even introducing herself. And during a couple of my monitoring visits, there was a pregnant staff member talking pretty openly about her pregnancy and impending delivery with the front desk staff, loud enough for the whole waiting room to hear. It didn't bother me that much but I imagine if I were having a particularly bad day, it could have been triggering. Another thing to note is no matter which location you choose, all procedures (egg retrieval, transfers, etc.) will be in Clear Lake, as well as any monitoring appointments that happen on a weekend.
Describe the costs associated with your care under Candice O'Hern Perfetto at Shady Grove Fertility Texas (US Fertility Network clinic).
$25,500 flat fee for a Shared Risk program that included all treatment, monitoring, procedures, etc. for up to 6 cycles until we have a live baby or decide to quit. If we decided to quit at any time or weren't successful, we'd get a full refund.
$1650 flat fee for PGT testing on any/all embryos we created.
$2957 for retrieval meds.
$730 for transfer meds.
Total: $30,837

I explained above, but they were going through a change in portal software, so some of my treatment instructions and test results were difficult to find, and I had to keep checking 2 portals and calling if information was missing.
She’s [Candice O'Hern Perfetto] extra conservative. She understands our situation and didn’t just look at my age. She’s patient and lets me ask a million questions and answers patiently (and I ask a lot since I’m a chemist/engineer). I’m a planner so she helped come up with plan A, B, and C to calm my nerves...She’s kind and patient. She truly want to help. She understands the situation of every patient is different and that age is just a number. She plans way far out with lots of back up plan options. She is genuinely a caring person.

What's one piece of advice would you give a prospective patient of Candice O'Hern Perfetto at Shady Grove Fertility Texas (US Fertility Network clinic)?
Unless you are not a candidate or you want to bank embryos, IUI is still a great option. My IUI was a success and my embryo transfer (tested) ended in a miscarriage. We knew there was a small chance due to age.

Frozen embryo transfer: estradiol patches and pills, steroid, baby aspirin, progesterone, technically it was a success because I got pregnant, ended in a blight ovum.
IUI: clomid 100 for 5 days, trigger shot and progesterone. Failed. My husband got sick and it killed his dowel count (40 M down to 7 M).
IUI: just preformed a few days ago. Same procedure as before but added estradiol vaginally and a new form of progesterone. Too soon to tell, but my husband’s sperm count barely improved so she said it’s time to consider an egg retrieval since I’m 40. It could take him 6 months to get back to normal at the rate he’s improving.
She was very hopeful until my husbands sperm count and quality dropped so significantly.

Describe Candice O'Hern Perfetto's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Shady Grove Fertility Texas (US Fertility Network clinic).
I had to remind Dr. Perfetto many times about non-medical specifics (wanting to do an unmedicated FET, phobia of needles) of our situation before she remembered them. Once she was reminded though she did her best to accommodate my needs...For each retrieval she controlled my medications to keep the number of eggs retrieved around 20 (22 and 16) and I never had OHSS because she used a lupron trigger
I had minimal communication with the nurse who called each day to give me instructions on protocol adjustments, but the ultrasound tech and phlebotomist at the Medical Center location are awesome...CORM has great success rates and I’ve been pleased with my results so far, but there’s no way to communicate with them besides by phone and you can only leave a voicemail. Inevitably they would call me back while I was in a meeting and we would play phone tag back and forth several times
How was your experience with Candice O'Hern Perfetto at Shady Grove Fertility Texas (US Fertility Network clinic)?
Medically, she is very skilled. I had a high AMH and AFC so was at risk of developing lots of poor quality eggs and OHSS. For each retrieval she controlled my medications to keep the number of eggs retrieved around 20 (22 and 16) and I never had OHSS because she used a lupron trigger. My eggs must have been good quality because we had blast conversion rates of 60% and 80% for our two ER cycles.
When you meet with her she does seem rushed but will give you the time you need if you have questions to ask.
What's one piece of advice would you give a prospective patient of Candice O'Hern Perfetto at Shady Grove Fertility Texas (US Fertility Network clinic)?
Ask plenty of questions.

Describe the protocols Candice O'Hern Perfetto used in your cycles at Shady Grove Fertility Texas (US Fertility Network clinic) and their degree of success.
I have a high AMH and AFC, but no official PCOS. Dr. Perfetto kept my stim doses low and triggered me with lupron because of this and I responded almost too well to gonal. For this same reason both of my ER cycles were freeze-all.
ER Cycle 1 we started with 2 weeks of birth control pills and then 185 IU of Gonal-F and micro dose ovidrel. I had monitoring appointments starting at day 5 of stims and started daily cetrotide once I had a follicle over 12/13 mm. She also continually lowered my gonal dose as I was over-responding to gonal. After 11 days of stims I was triggered with lupron that night and had the ER about 36 hours later. They retrieved 23 eggs, 22 were mature, 21 fertilized (MFI and ICSI) and 12 made it to Day 5. 5 embryos from this cycle were euploid.
For ER Cycle 2 we did the same protocol but started with 150 IU of gonal which was reduced through the cycle as I was over-responding again. From this ER we got 18 eggs, 16 mature, 15 fertilized with ICSI, and we had 12 blasts. 4 of these were euploid, but we elected not to test the one Day 7 embryo.
For my first FET cycle we went with a modified unmedicated cycle. Midway through the cycle my lining wasn’t developing as hoped and I was first given vaginal estrogen. This made my estrogen levels skyrocket and I was then given cetrotide which ground my estrogen levels to almost 0. Then I did 2 days of 50 IU of gonal and finally had a 16 mm follicle with 7.5 mm lining and was triggered. We transferred a 5BB embryo that ended in a CP.
The most recent FET cycle went much smoother. We did a modified unmedicated cycle again but this time I had a 17 mm follicle and 8 mm lining at CD 12 and was triggered with no additional medications. My estrogen level stayed between 300 and 500 for this cycle, which was the goal. I’m 6 days post transfer now so the result is still pending.
Describe your experience with the nursing staff at Shady Grove Fertility Texas (US Fertility Network clinic).
I had minimal communication with the nurse who called each day to give me instructions on protocol adjustments, but the ultrasound tech and phlebotomist at the Medical Center location are awesome! They’re personable and my veins didn’t blow out once even with frequent blood draws for an extended period of time.

Describe the costs associated with your care under Candice O'Hern Perfetto at Shady Grove Fertility Texas (US Fertility Network clinic).
The first ER cycle contract was $17,500 plus PGT. This included everything to do with the ER cycle and the first FET. My second ER cycle (covered the same items but no FET) was $7,900. Subsequent FETs are $3,000
Describe Candice O'Hern Perfetto's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Shady Grove Fertility Texas (US Fertility Network clinic).
eSET was the only option presented to me, but I’m in my early 30s, have PGT tested embryos, and don’t have a history of pregnancy loss.
[Dr. Perfetto] is compassionate yet still has professional boundaries...Our cycle did not go as she or we expected but she quickly adapted, gave us alternatives...Dr.Perfetto called personally to “check on us”...It is a great feeling when you are under the care of a provider who knows when to call themselves and share in the disappointment yet maintain a professionalism to be able to carry-on and discuss next steps
Both nurses I had [at CORM] returned calls same day, stayed late when I needed bloodwork rechecked, provided samples to get us through until refills arrived (if needed), and always showed compassion and kindness. My monitoring Ultrasound tech and phlebotomists were equally as great...7 months out from our cycle and still trying to get our account settled. This is partially because I have dual insurance
How was your experience with Candice O'Hern Perfetto at Shady Grove Fertility Texas (US Fertility Network clinic)?
Dr.Perfetto listens, clearly explains her plans and hopeful outcomes. She is compassionate yet still has professional boundaries. Our cycle did not go as she or we expected but she quickly adapted, gave us alternatives. Based on my first blood work post Lupron trigger I did not respond as expected. Dr.Perfetto personally called with the news and within 2 hours my blood was redrawn, resulted, and a new medication was in hand to re-trigger. She then made sure her staff and colleague who was doing egg retrieval knew to ultrasound me before doing the retrieval as there was a chance of ovulating at this point. ET went well and 7 mature eggs retrieved. 6 fertilized, however they all appeared to have arrested on day 4, yet she let them go to day 5 as there was still a small amount of multiplying still happening. On days 4 and Day 5 we got calls from the embryologist and Dr.Perfetto called personally to “check on us”. Both my husband and I felt this was incredibly professional as it was not anything we had experienced with past providers. She then called a couple days to check on us again and offered a prompt follow-up, “if we were ready”. When we went in a few days later for that appointment I could see she was quite upset about the outcome as well. It is a great feeling when you are under the care of a provider who knows when to call themselves and share in the disappointment yet maintain a professionalism to be able to carry-on and discuss next steps.

Describe the protocols Candice O'Hern Perfetto used in your cycles at Shady Grove Fertility Texas (US Fertility Network clinic) and their degree of success.
Microdose ovidrel and Gonal F with prestim antagonist cycle—We did 19 days of OCP. The I started with antagonist (cetrotide 0.25mg) daily for 3 days.
A day with no meds,
Stim days 1-4: Gonal F 225 with micro hcg 0.1(10 units)
SD 5: Monitoring appointment. 13 <10mm follicles. Gonal F increased to 300 units with micro hcg 10units
SD 6: Gonal F 300 units with micro hcg 10 units
SD 7: Monitoring appt. 5 follicles over 10mm and 9 follicles <10mm.still Gonal F increased to 375 units with micro hcg 10 units and added antagonist Cetrotide 0.25mg
SD 8: Gonal F 375units, micro hcg 10units, Cetrotide 0.25mg
SD 9: Monitoring appointment. 11 follicles between 10.3-15.9mm and 3 still <10mm. Dr.Perfetto still did not like the response, and increased Gonal F again. Gonal F 450units, micro hcg 10units, and cetrotide 0.25mg
SD 10: Gonal F 225units in AM and Gonal F 225 units with Micro Hcg and cetrotide 0.25mg in PM
SD 11: Monitoring appointment. 11 follicles 11.6-18.4 and still a few <10mm. Gonal F 225units in AM, and Gonal F 225units, Micro HCG 10units, and cetrotide 0.25mg in PM
SD 12: monitoring appt. 11 follicles 16.0-22.5mm. Gonal F 225mg in AM with Lupron trigger 80units in PM.
SD 13: timed bloodwork in the morning. Had no response to Lupron. Got a call from Dr.Perfetto (not a nurse) and immediately went back to the office for a redraw. 30 mins later got new results. I forgot to mention the lab tech stayed specifically to run my blood in the house so it didn’t have to be sent out since it was so urgent. Same results as the morning, no response. Novarel 10,000 units ordered and picked up within another 30 mins. Given in PM.
SD 14: positive pregnancy test to show response to novarel.
SD 15: follicular ultrasound to ensure ovulation hadn’t occurred between Lupron and Novarel triggers. Multiple large follicles seen. Egg retrieval occurred. 9 retrieved, 7 mature, 6 fertilized.
Had 6 growing embryos days 1-4. On Day 4 the multiplying slowed and looked like all 6 were arresting. Day 5- embryologist called. All 6 arrested. Dr.Perfetto personally called a few hours later to check on me (us). Called again a couple days later to offer follow-up when we were ready. We did a follow-up within a week. Dr.Perfetto appeared just as shocked as we were, was very compassionate, and took her time allowing us to ask all our questions.

Strengths- staff listens, shows kindness and compassion, don’t be afraid to ask questions.
Weaknesses- billing department. 7 months out from our cycle and still trying to get our account settled. This is partially because I have dual insurance but only had a fraction of insurance coverage left. Yet I feel 7 months is too long to get it figured out.
Describe the costs associated with your care under Candice O'Hern Perfetto at Shady Grove Fertility Texas (US Fertility Network clinic).
We had about $5000 left in insurance coverage so we did not have to do a cash contract. We did have to put a total of around $3800 cash/fsa down to cover Surgical center fees for both egg retrieval and FET and PGT fees. We also paid cash for all meds approx $3500-4000. Total cash paid between meds and fees approx $8000.
Describe Candice O'Hern Perfetto's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Shady Grove Fertility Texas (US Fertility Network clinic).
Unfortunately we did not get to this point. We did discuss single embryo as the safest option for myself and to reduce risk of multiples and all risks associated with multiples.
she [Dr Perfetto] never made me feel like any of my questions were dumb and she never ever rushed me during our consultations...she really pushed PGS testing for me due to my age 41. She will only transfer one embryo at a time however if you don’t get genetic testing she said we could discuss transferring 2 at a time but she doesn’t like doing that...She is patient and she takes as much time as needed during your consultations to answer all your questions....
They are a very busy clinic but never too busy to take your call answer your question and even fit you into the schedule...very pleased with the nursing staff and their promptness with returning phone calls the same day...I was actually OK with the nurses doing the inseminations because I know that they do them quite frequently probably more than the doctor...An egg retrieval is about $10,000 and transfers start at $4500...
How was your experience with Candice O'Hern Perfetto at Shady Grove Fertility Texas (US Fertility Network clinic)?
I came to Dr Perfetto after I had recently had a consultation at another clinic that had given us bad news. The other clinic had reviewed my husband sperm analysis and told us we would not be successful and we would have to do IVF. Dr Perfetto noted that clinic is notorious for telling patients this let’s re-test and go from there and that’s exactly what we did. After my husband’s analysis came back B+ she was willing to try at least three IUI even though I was 41 years old at the time. With each are you why she changed the medication protocol she however does not perform her own ultrasounds she does review them after each visit and they call you back within a couple of hours of having them done. She did not perform My IUIs however I did request her at one point and was told she was booked doing consultations. I understood this and I was actually OK with the nurses doing the inseminations because I know that they do them quite frequently probably more than the doctor. I was especially happy with my last insemination she change the protocol and she also started me on injectables prior to my third and insemination. Unfortunately all three inseminations failed. I quickly started IVF and had one egg retrieval however she was not on call for my retrieval but another one of her partners did the retrieval. She did make me aware that this could be a possibility but she was the one who performed other diagnostic procedures on me in the office like the Saline ultrasound and my test transfer she also performed my hysteroscopy recently and she will perform my transfer which is soon. During my second insemination I had some pretty bad pain and bloating and was very nervous I called the on-call number and her nurse called me back immediately with her suggestions. I’ve actually even personal messaged her and got no response for some questions that could not be left on the nurse triage line which she answered gladly and never gave me grief about it . She also has listened to my suggestions for possible test and treatments and has given me honest answers along with her recommendations and her experience in the past with women in my age group with my history. All in all I’m very happy with Dr Perfetto .
What's one piece of advice would you give a prospective patient of Candice O'Hern Perfetto at Shady Grove Fertility Texas (US Fertility Network clinic)?
Dr Perfetto will ask you more than once if you have any questions which I really like because sometimes I was afraid to ask certain questions but because she continue to probe me I finally said yes I do have questions and she expects you to have them and she will answer them honestly and help you understand! And she never made me feel like any of my questions were dumb and she never ever rushed me during our consultations.
During treatment, were you treated like a number or a human with Candice O'Hern Perfetto at Shady Grove Fertility Texas (US Fertility Network clinic)?
Dr. Candice Perfetto has a wonderful bedside manner.
She is patient and she takes as much time as needed during your consultations to answer all your questions and I had a lot even being a nurse. She didn’t make me feel like I should know things being in the medical field she treated me just like a normal patient. She is down to Earth and she shows a lot of compassion and empathize with her patients and their struggles and she truly cares.
Describe the protocols Candice O'Hern Perfetto used in your cycles at Shady Grove Fertility Texas (US Fertility Network clinic) and their degree of success.
During my 3 IUIs she used a combination of femara, tamoxifen, Gonal F, and Ovidrel. During my IUI
She performed Lab draws and monitoring ultrasounds. Not successful moved on to IVF used a Eprime Antagonist Addback. She said she was surprised with how many eggs were retrieved because I was a slow responder to the stimulation cycle which I stimulated for 12 days with my trigger on day 13 and retrieval 2 days after. I had 16 eggs retrieved with only 3 mature which is lower than the average which is because of my slow response to the meds. I was on Gonal f 300 units daily Menopur 150 mg daily and cetrotide 0.25 mg x 6 doses with a Novarel trigger. No birth control was used in any cycles leading up to my retrieval however I am currently on birth control now to help get me to my embryo transfer quicker. I have 3 embryos (1 of which is PGS normal embryo) she really pushed PGS testing for me due to my age 41. She will only transfer one embryo at a time however if you don’t get genetic testing she said we could discuss transferring 2 at a time but she doesn’t like doing that. I am awaiting my transfer now.
Describe your experience with your nurse at Shady Grove Fertility Texas (US Fertility Network clinic). (Assigned nurse: Taylor; Julie; Katy)
Taylor was great she has a lot of experience and she answers all your questions and even gives you some of her personal advice of things she seen in her time as a nurse. She’s very quick and direct to the point but she’s compassionate. I also ran out of medicine at one point and she gave me a loaner. Julie is very sweet she remembers personal details about you at one point she did make a mistake no harm was done and she apologized completely and could understand why it made me anxious and ask more questions she and Taylor took the time to answer all my questions knowing I’m a nurse they still treated me with respect and never made me feel like I should know the answers to the questions I’m asking this is not my specialty and they realize that. Katie also is very good at the time I was doing my inseminations and monitoring ultrasounds she was learning to do the ultrasounds she was meticulous she took her time she’s thorough and she answered my questions that I had during my monitoring appointments. I was very pleased with the nursing staff and their promptness with returning phone calls the same day which I hear is not the case with other clinics. I’ve even had to call multiple times in one day and they were turn each and everyone of my phone calls email me or text me.

The front desk and office staff and billing department are very prompt they are compassionate they talk to you in a caring manner and want to help you I genuinely feel that when I go there. The staff really helped me optimize my insurance coverage to the best of their ability before having to start paying out of pocket they suggested things to me which I hadn’t thought of and they have many locations which is good however the main facility is in Clearlake. They are a very busy clinic but never too busy to take your call answer your question and even fit you into the schedule. When I was at one of my appointments awaiting a lady came in and signed in and the front desk lady told her ma’am you don’t have an appointment today but she was there already so Dr Perfetto did fit her in that day which I thought was very nice and a lot of other places or doctors would not have done that!
Describe the costs associated with your care under Candice O'Hern Perfetto at Shady Grove Fertility Texas (US Fertility Network clinic).
An egg retrieval is about $10,000 and transfers start at $4500 then they go down to $2000 and after that they are free until pregnant within one year. They do accept ARC fertility packages which is a third-party company that finances cycles for patients and then pays them directly to CORM. We purchased a two cycle package which reimburses you some of your money back if you’re able to get pregnant on the first cycle. They use an outside genetics company to do the PGS testing however at this time I am unsure as to whether I would consider doing PGS testing in the future or on my future cycles as I am not sure to the validity of it anymore there’s lots of?‘s concerning it I believe.
Describe Candice O'Hern Perfetto's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Shady Grove Fertility Texas (US Fertility Network clinic).
Dr Perfetto notes she only likes to transfer one embryo at a time however in cases where you don’t have genetic testing she is open to a discussion about transferring more than one but she does not prefer to do that.
Dr. Perfetto did a wonderful job informing me about the tests and processes but sometimes I would leave and go home then think of something else. ... After about 3 vists and initial testing, we figured out I was suffering from a miscarriage due to an ectopic pregnancy. Dr. Perfetto made me feel like I was a priority. She called me on the phone personally to explain what was happening and guided me through it.
I will admit, in the beginning, it was very difficult to get a hold of someone to confirm a new patient appointment for me. However, after I did my initial consultation, the communication was much better. ... The entire nursing staff at CORM has been nothing but helpful to me. From answering all the questions, to returning my frantic phone calls, and even helping me with my payment/bill options. Any time I needed them, I would call the triage line and leave a message and they would always get back to me that day no matter how busy they were. Everyone I have worked with has wonderful bedside manner and made me feel nothing but welcomed. I cannot comment on any of their weaknesses because I have yet to observe any.
How was your experience with Candice O'Hern Perfetto at Shady Grove Fertility Texas (US Fertility Network clinic)?
When I first went to visit Candice Perfetto for a consultation, I was nervous. After about 3 vists and initial testing, we figured out I was suffering from a miscarriage due to an ectopic pregnancy. Dr. Perfetto made me feel like I was a priority. She called me on the phone personally to explain what was happening and guided me through it. She even gave me her cell phone number and told me I could call her if I had any questions or concerns. She checked on me consistently and made me feel a little less horrible about the situation at hand. She has wonderful bedside manner and is very caring and compassionate. She even has an instagram dedicated to inform her followers about infertility and all the things that accompany it.
What's one piece of advice would you give a prospective patient of Candice O'Hern Perfetto at Shady Grove Fertility Texas (US Fertility Network clinic)?
Ask a ton of questions. There is no such thing as a silly question. Dr. Perfetto did a wonderful job informing me about the tests and processes but sometimes I would leave and go home then think of something else. Don't hesistate to call the office and leave a message to speak to the nurses. They will 100% call you back and help you with anything you need.
During treatment, were you treated like a number or a human with Candice O'Hern Perfetto at Shady Grove Fertility Texas (US Fertility Network clinic)?
When I first went to Dr. Perfetto (after my initial consultation) I was going through a miscarriage/ectopic pregnancy. Dr. Perfetto called me personally to check on me, tell me information, and remained consistently worried about me through the entire process. I know she had tons of patients, but she truly made me feel like I was a priority.

I will admit, in the beginning, it was very difficult to get a hold of someone to confirm a new patient appointment for me. However, after I did my initial consultation, the communication was much better. The clinic is so inviting and cozy. You don't feel so much as though you are at a hospital. The waiting room kind of reminds me of my own living room with a big comfy couch and television always on. They even have snacks at the counter for you when you leave! The bathroom and facilities have always been top-notch clean and no complaints from me.
Describe the costs associated with your care under Candice O'Hern Perfetto at Shady Grove Fertility Texas (US Fertility Network clinic).
Unfortunately, being a teacher, nothing is covered under my insurance. Not medicine, not testing, not clinic visits and definitely not consultations. It definitely is a costly thing to go through. Our office visits range from from 175-300. Our medications cost around 1,200. Our IUI's range from 2,000 to 3,000 depending on the management cycle suggested.
Describe Candice O'Hern Perfetto's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Shady Grove Fertility Texas (US Fertility Network clinic).
we are using a HCG trigger shot to assist in the quality of my eggs as well as a Lupron antagonist protocol [with Dr. Candice O'Hern Perfetto] because there may be some underlying endometriosis/inflammation that we aren't aware of...makes you feel like you are part of a family and your care is important to her to the extent she will do everything she can based on the knowledge she has to help you achieve your potential. She even remembered or Skype conversation from a month previous when we met her in the office. I can't say I've had to many Dr.s do that especially in the fertility community.
Overall this clinic [CORM] has way more strengths than weaknesses and I would recommend it to anyone looking for compassionate and knowledgable fertility information and treatment...It seems as though there is a separate on call nurse or nurse that returns messages left vs. the Dr.s primary nurse. I believe there is going to be some turnover with the primary nurse so I can't say yet how this will affect my interactions with the clinic...
How was your experience with Candice O'Hern Perfetto at Shady Grove Fertility Texas (US Fertility Network clinic)?
As a woman it is important to feel as though you are being treated the way a Dr. would treat their best friend or family member. Did you do your research? Did you explore all options and tests? Did you look at things that are new and maybe controversial at this time? I asked Dr. Perfetto if I was her best friend, sister, or daughter what would she recommend me do. Without hesitation I was given an answer and an explanation as to why. It's important that your Dr. doesn't always feel like they have every answer. It keeps them seeking new information and treatments in the area of their expertise. I explained my situation with my previous RE and how after failed IVF I was given the same protocol with no changes and no explanations. Dr. Perfetto immediately told me of research that had suggested that the protocol I had been on may not have been the best choice and explained why.
What's one piece of advice would you give a prospective patient of Candice O'Hern Perfetto at Shady Grove Fertility Texas (US Fertility Network clinic)?
Be open, honest, and thorough in your history. That would be my recommendation for anyone doing IVF. Somethings you may not want to discuss or feel embarrassed about but with infertility you quickly lose your modesty and if the Dr. can have a full picture from the beginning they will be more equipped to help you achieve your goals/dreams.
During treatment, were you treated like a number or a human with Candice O'Hern Perfetto at Shady Grove Fertility Texas (US Fertility Network clinic)?
Dr. Perfetto from the moment we first spoke with her made us feel like we were important and more than a number. It is important to us to be treated for our infertility and not just be treated for IVF. What I mean by that is whatever we can do to improve our fertility or find out why we are struggling it important to start there and not just do rounds of IVF with no answers and the same protocols. Dr. Perfetto makes you feel like you are part of a family and your care is important to her to the extent she will do everything she can based on the knowledge she has to help you achieve your potential. She even remembered or Skype conversation from a month previous when we met her in the office. I can't say I've had to many Dr.s do that especially in the fertility community.
Describe the protocols Candice O'Hern Perfetto used in your cycles at Shady Grove Fertility Texas (US Fertility Network clinic) and their degree of success.
I am starting my first treatment with Dr. Perfetto so I can't say quite yet how the outcome will be, however, I can say that we are using a HCG trigger shot to assist in the quality of my eggs as well as a Lupron antagonist protocol because there may be some underlying endometriosis/inflammation that we aren't aware of.

Describe your experience with Shady Grove Fertility Texas (US Fertility Network clinic).
The strengths of this clinic are that the Dr. always preforms their own transfers, The billing is as simplified as you can get it so there are no hidden fees or potential for cancelled cycles due to unknown balances due, they are very open to questions and suggestions about your cycle and why or why not something might work for you. The weaknesses are nursing turnover. I don't think this happens a lot at this clinic but when it does happen there is always a transition period that is a little chaotic. Also, though very nice and helpful, being out of state I had to fax and upload all of my previous medical information for both my spouse and I. When I showed up for my in office appointment the intake girl had not forwarded any of my files or information to Dr. Perfetto. This is especially frustrating because she wasn't able to look at any of my previous cycle information prior to my visit. Which I specifically said if there was anything additional she needed to please let me know so I could make sure I provided it. I will say though that during the time we spoke with billing and her personal nurse she requested that information and reviewed it and came back with details on what protocol she recommended, why we were going to do things a little differently, etc. Rather than looking at it after we left (since I won't start my cycle for a another month or so) she immediately was on a plan for us. Overall this clinic has way more strengths than weaknesses and I would recommend it to anyone looking for compassionate and knowledgable fertility information and treatment.
Describe the costs associated with your care under Candice O'Hern Perfetto at Shady Grove Fertility Texas (US Fertility Network clinic).
The billing is simplified into charges for retrieval, transfer, PGS/PGD, etc. So it is not small amounts for individual procedures such as ultrasound, lab, nursing hours, etc. It makes it simple to pay for and there are no hidden fees that could potentially cancel a cycle for mishaps in the billing department. Some of the fees are high such as sperm cryopreservation and storage compared to other clinics I've seen or heard of but overall I think it's comparable.
Describe Candice O'Hern Perfetto's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Shady Grove Fertility Texas (US Fertility Network clinic).
I have no comments on eSET as I always do what my Dr. recommends. I did have a previous Dr. suggest transferring 3 embryos but if they all took we would have to make a selective deduction choice and so we declined to have three transferred. I understand both rationales for single vs multiple so unless there would be selective reduction I typically am fine with my Dr.s recommendation.
When I first consulted with Dr. Perfetto, she was very educational and explained the process in depth. We decided to do a medicated cycle and she told us step by step what to expect. Unfortunately the cycle resulted in a negative pregnancy. Dr. Perfetto wanted to increase my chances and we decided for surgery to remove uterine polyps before proceeding another cycle...shedule is busy but don’t be impatient. Seeing her is worthwhile...I was prescribed provera, one pill for 10 days to induce a cycle as I have amenorrhea.
The financial counselor [at CORM] gave us a contract with options. The contract had the charges in writing and we were told upfront that it did not cover medication...The clinic is very private and the ambiance is very warm and inviting. They have on site lab, ultrasound machines, HSG testing and more. It helps when it’s a one stop clinic because you don’t have to travel to so many places. The one thing I would change is parking. It is kind of difficult to enter and exit and you also have to pay to park.

Unfortunately the cycle resulted in a negative pregnancy. Dr. Perfetto wanted to increase my chances and we decided for surgery to remove uterine polyps before proceeding another cycle. Unfortunately, I was diagnosed with endometrial hyperplasia with atypia and could not proceed. During that crucial and devastating diagnosis, she was very empathetic and I felt she was on my side. She gave me options and didn’t leave me blindsided. After hormone therapy , I returned to her facility for further treatment. I wouldn’t choose anyone else for my care.

Dr. Perfetto’s shedule is busy but don’t be impatient. Seeing her is worthwhile.
During treatment, were you treated like a number or a human with Candice O'Hern Perfetto at Shady Grove Fertility Texas (US Fertility Network clinic)?
I felt like a human because she understood my struggles with PCOS and didn’t automatically weight shame me. She explains everything in terms I understand. Most important, when I was at my lowest point and received an unexpected diagnosis, she was empathetic and even called me to check and see if I was okay. I will always remember that.
Describe the protocols Candice O'Hern Perfetto used in your cycles at Shady Grove Fertility Texas (US Fertility Network clinic) and their degree of success.
I was prescribed provera, one pill for 10 days to induce a cycle as I have amenorrhea. After my cycle, I went in for an ultrasound to check my uterine lining, I was then prescribed Letrozole to induce ovulation. During this process, we did have to up the dose and it worked. When I had an ultrasound with a follicle, I was instructed to trigger shot. I was given a calendar with instructions of every step of the way. After trigger, we timed intercourse based on instructions. We then waited two weeks for blood work which resulted in a negative pregnancy. During the two week wait, I was prescribed progesterone suppositories to help prevent miscarriage. After the negative test, I was instructed to discontinue progesterone suppositories.

The financial counselor gave us a contract with options. The contract had the charges in writing and we were told upfront that it did not cover medication. This current time, the counselor provided an updated contract with updated pricing and we were once again told up front what was not included in the contract. Since my insurance doesn’t cover fertility, all my charges must be paid upfront with the contract. This is probably the most difficult part of this process. The contracts cost thousands of dollars upfront and it is heart wrenching when the funds are not available or limited.
I saw 4-5 specialists in Houston before seeing her [Dr. O'Hern Perfetto] and we love her her!... we are ready to start letrazole with an IUI per her recommendation, as well as a low dose steroid for my case...She’s personable and super nice. Very knowledgeable...relatable...really loves what she does. She has an Instagram that really raises awareness about infertility and she’s always so honest and nice. You can see how much she cares about her patients and about what she does
I’ve had minimal contact with the nursing staff [at CORM] but from the contact I did have they were very nice and helpful...For every cycle of IUI it will cost us 2,700-3000$ approximately...The cost for IVF there was 14,500$ without the meds...This clinic is by far the best one I have been to in Houston. They are quick to answer, work with your schedule, and they are nice and compassionate. They respect your time as well. I don’t really have any other comments as of right now. My only thing would be to definitely ask about the pricing up front which they are really good at

I had tried clomid in the past under the care of another physician and it did not work. She explained why letrazole would be a better fit but since I was not having a period (2 years without a period when I saw her), she recommended we start wth progesterone every month for 10 days and I got a period for the first time in 2 years! I may not be ovulating but I have a period and we are ready to start letrazole with an IUI per her recommendation, as well as a low dose steroid for my case.

Candace [O'Hern Perfetto] has great bed side manner, she is nice, approachable, explained things very well to my husband and I and was very gentle. I am excited to continue our journey with her. ... Candice explained both my husband and I's questions in detail, we did the dye test and she explained everything about it, my husband did the sperm test and she explained the results in detail. Our next step is IUI and she explained that in detail and what to do next when we are ready.
The clinic is very nice, modern, has a television, snacks and magazines for the guest. The clinic looks great. Dr. Perfetto is honest & upfront, but kind and emphatetic. I always felt like I knew the plan and rationale. When things didn’t go as planned, she helped my understand why and how we should best change the course of treatment.... Not much time is scheduled directly with Dr. Perfetto during the monitoring phase of your cycle. If you feel like you need extra time with her, just ask.... She always acts like I am her only patient - no amount of questioning seems like too much.
This clinic [CORM Houston] is located on the outskirts of the Texas Medical Center. It is a quiet practice, never too busy. A major con is that you have to pay for parking - it adds up quickly with the frequent visits required for monitoring....Monitoring appointments were always very easy & convenient. I was able to go in at 7:30am before work. They would draw my blood & then complete ultrasound. In and out in less than 20 minutes usually.

Dr. Perfetto is very warm and compassionate. She reassures me that my feelings are real and valid. Dr. Perfetto has been compassionate, patient and understanding. I initially consulted with her for my unexplained infertility issues and later developed a endometrioma that required surgical removal. She diagnosed me with endometriosis related infertility. Since then, she has been honest about what she believes will work and what will not. She provides solid information to support those beliefs. Her kind, nurturing nature has been what I've needed while constantly getting bad news.

Describe the protocols Candice O'Hern Perfetto used in your cycles at Shady Grove Fertility Texas (US Fertility Network clinic) and their degree of success.
After laparoscopy to remove an endometrioma, I took one cycle of birth control pills.
IUI Round 1: I began my first IUI cycle by day 2 blood work and ultrasound. On day 3, I started with five days of letrozole and then three days of Gonal-F injections (100). The letrozole was not supposed to interfere with my endo as much as other meds. I returned to the clinic on day 11 and only had two follicles at ultrasound time so I took another dose of Gonal-F (150) and then ovadril (100) to ovulate. IUI was preformed on day 13. I began progesterone supposotories the day after IUI. On day 20, I retuned to the clinic for progesteone test and continued suppositories until I began my period naturally. That resulted in a negative pregnancy test.
IUI Cycle 2: I began my second IUI cycle by day 2 blood work and vaginal ultrasound. On day 3, I started with five days of letrozole and then three days of Gonal-F injections (100). The letrozole was not supposed to interfere with my endo as much as other meds. I returned to the clinic on day 13 and only had one follicles at ultrasound time. I had also started to ovulated on my own. IUI was preformed on day 14. I began profesterone supposotories the day after IUI. On day 21, I retuned to the clinic for progesteone test and continued suppositories until I began my period naturally. That resulted in a negative pregnancy test.
IUI Cycle 3: I began by day 3 ultrasound and blood work/pregnancy test. I began 5 days of clomid (one tab/50) followed by 3 days of Gonal-F (100). Dr Perfetto decided that clomid might produce more follicles than the previous results from letrozole. She commented that I might have more endo side effects but we felt it was worth it. I am scheduled to return for vaginal ultrasound and lab work on day 11.
Describe your experience with the nursing staff at Shady Grove Fertility Texas (US Fertility Network clinic).
The nursing staff has been nice, but unorganized. I'm not sure if Dr. Perfetto has one head nurse because I've seen and spoken with several. I'm not sure what role each nurse plays. One of her nurses has been very short and agitated with me on the phone. The other is incredibly sweet but seems uninformed about some of my issues and provides more administrative type duties.
Describe your experience with Shady Grove Fertility Texas (US Fertility Network clinic).
This is a tough one. There is an office near my work that I initially began at. I started here because of this convenience. However, it doesn't have the same lab abilities and my Dr. is only there once a week so I am constantly referred to the medical center location which is 45 min away.
Also, they do not have the ability to call and speak with a nurse or even email with a question. You can only call the nurse triage line and leave a voice message. The previous clinic I went to had both options and made me feel they were much more available.
They also aren't open on the weekends except for their Clear Lake location (2 hours from my house) and require (sometimes 7AM) Saturday appointments. The previous clinic I went to was open every single day.
Describe the costs associated with your care under Candice O'Hern Perfetto at Shady Grove Fertility Texas (US Fertility Network clinic).
For my endometrioma surgery, my insurance covered a good part and I had a co-insurance.
My IUI is covered by my insurance for 6 rounds. I only pay for the medicine out of pocket. The last three cycles have been about $600 for letrozole/clomid, Gonal-F, Ovadril and progesterone suppositories.
I fee that when I have questions about my treatment and I question why certain meds were used, she gets very defensive and does not give a good explanation why she chose it. I asked if we did IVF with here would she [Dr. Perfetto] implant multiple embryos and she said no she refuses to do it. No explanation of certain medical reasons of why she chooses to not do it. I just feel like it’s an empty clinic with no real answers.
Was prescribed the wrong dose of a medication and I ended up spending more than I had to and wasted the extra medication. ... When walking in the clinic [CORM], no one greets you. ... They tell you your amount when leaving and give you a small receipt but you never are told what you are paying for or an explanation. Don’t expect to see the doctor while going to all of your appointments because most of the time you will just see the nurse.

Most are really nice but I’ve seen a different nurse each time I’ve went in and they were trying a new girl and she was learning the IUI procedure for my first one a she performed the second one. I would have liked to feel more comfortable and confident with the ones performing the procedure.

She [Dr. O'Hern Perfetto] Takes the time to get to know you and treats you like a friend... always had a smile on her face and treated my husband and I so well. Always making you feel like she had all the time in the world to spend on your case. She took the time to explain everything and made sure we both understood. While under her care, I had to have surgery, she told me exactly what would happen and what to expect and made me feel very comfortable.
The nursing staff [at CORM] was always wonderful. They answered every question, were comforting, made you feel at ease, and got back to me in a timely mannner if I called...Every person I encountered at CORM treated me so well. I never had a bad experience. They treat you like a person not another patient. Everything was done fast, which I loved. Some places take forever for results. Also at CORM I always had my info and knew what was going on. They have the patient portal which I thought was a lot of help!

I have PCOS so our first try at fertility help was me taking a pill to induce ovulation. I can’t remember the name of the pill. If that didn’t work we were planning to do an IUI. I started taking the pill and got pregnant very quick, unfortunately I miscarried at about 7 weeks. I had no clue I was pregnant and when Dr. Perfetto told me I was shocked. She was wonderful through the whole process. Talked me through everything that was happening.

Describe the costs associated with your care under Candice O'Hern Perfetto at Shady Grove Fertility Texas (US Fertility Network clinic).
I think there was an initial appointment and work up fee, maybe $550, but I could be off. After that if I saw Dr. Perfetto I had to pay the specialist doctor co-pay from my insurance which was $75, but if I just had an ultrasound or saw a nurse it was a regular co-pay of $40.
Dr. Perfetto is amazing. I would highly recommend her to anyone in the Houston area looking for a fertility specialist. She listens with empathy and addresses any concerns as they arise. She took the time for necessary testing and explaining of all results. I appreciate her honesty and never giving us false hope...She prefers single embryo to give you the safest pregnancy and reduce the risk of multiples

The transfers took place in the Clear Lake office and my appointments were all in Houston...Every staff person in this office is so helpful and organized. I received enough communication to know what was going on at all times.

dr perfetto is really good at making you feel comfortable. She is totally invested in the care of her patients. Check out her Instagram and you will see for yourself...very pleasant and relatable. Always smiling, very listening, even joking and laughing when appropriate... Also since it’s a smaller clinic than other juggernauts in Houston, you really feel like your doctor is following you and invested in your care. They are very reactive (they turned my second iui cycle into an IVF) to make sure you make the most of your situation. Definitely, definitely recommend if you are looking for a fertility specialist in the greater Houston Area.

Dr.Prefetto was very nice and compassionate dr and always answered our questions with honesty.... De Prefetto was very seeet and always felt important although we weren’t successful she was very compassionate about everything. She tried to be there every step of the way and was very emotional the day the last embryo transfer was negative.... For each egg transfer I would have antibiotic and steroid for three days after transfer. I was also given estrogen and progesterone. First cycle I had estrogen pills and the second and third cycles I did estrogen patches with lovenox shots.
I also liked they [CORM Houston] were able to provide payment plans for paying that helped us tremendously. No other clinic provides that. She [Dr. O'Hern Perfetto] seems to be very knowledgeable, however she passes on a grunt of the work to her nurses, which makes you so unsure about what you are getting. You pay all this money- you want access to the specialist you're paying for...Just don't expect 100% access. As long as you make peace with that, you'll be fine...We used Gonal F for my last IUI cycle. Halfway through, she added another medication Menipur. The cycle ultimately failed. We are looking to start iVF next.
The clinic [CORM] has multiple locations which helps with access, especially on the weekends...The weakness is that they seem to have a high turnover rate with their nurses. Not sure why...Their office is hard to get a hold of. You have to call and leave a message, then wait for a call back. It is disheartening especially when you need to talk to someone asap...the one [nurse] who took the ultrasounds was really cold and disassociated...

Dr Perfetto was very nice, courteous and always addressed is by our names...was very professional. We had a good experience with her. However, she did not address the root cause of our infertility. She diagnosed a low sperm count, but neglected to treat it. We went ahead and did an IUI. Even the nurse that performed the insemination was surprised we were trying with such a low count.
There really wasn't much of a treatment [at CORM]. We just performed an IUI. She had us give my wife a shot in the stomach for her eggs to drop and then inseminated the sperm which was almost nonexistent. Needless to say, it did not take...the IUI was $400, office visits were $100+...They [nurses] were very professional...This clinic didn't seem to have a lot of patients so we were able to get in and out in no time. They pretty much take you right away. They have plenty of reading material in waiting area. Clinic was very clean and the staff was very discrete with the whole process.

Describe Candice O'Hern Perfetto's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Shady Grove Fertility Texas (US Fertility Network clinic).
She [Dr.Perfetto] explained everything very good. There was no rush in the appointments. She mentioned the pro's & con's of every treatment as well as realistic chances. Also was very open about the question to continue treatment or not....Unfortunately, all treatments did not result in any pregnancy. She adjusted the treatment and drug dose every cycle....Unfortunately, the staff [CORM] was not always aware of the facts that have been discussed between me and the doctor.
Unfortunately, the staff [CORM] was not always aware of the facts that have been discussed between me and the doctor. Therefore, we had several disputes on the phone because they wanted to make an appointment for treatments that were not necessary. Overall, I felt the staff was not up to date with my file....I did not feel well handled on the phone when appointments were asked for treatments that were not necessary.... eSET was preferred
