The first cycle we used 250 gonal F and 150 menopur, adding cetrotide when it was time. My follicles were over 40 in number and my estrogen over 5000 so we coasted until trigger day. We used luporon and Novaral totrigger. I got 14 eggs,13 were mature.
The second round we used 150 gonal f and 150 menopur to start since we end up having to coast last time. We adjusted the doses down into we saw my count was again over 40 and my estrogen shot up to over 10,000. Due to this we had to coast for three days and use Lupton only trigger. I had follicles as big as 31. We got 14 eggs today and all were mature!

Trish always knew what questions I would ask and have be the info before I had to ask. She was always very caring and even helped me out when a medicine delivery ran late ensuring me we wouldn't let anything happen to me or my precious eggs. She also went out of her way to schedule me at different locations based on my work schedule. I wasn't entirely sure i wanted ro for a second round so quickly but when I called thinking it was impossible to change my mind and start this cyle she made that happen too.

Clean welcoming environment. Staff is very easy to talk to and makes you feel like it's common place to talk of such intimate details making it easy to be open and honest. The men I saw in the waitingroom seemed to be at ease as well. They also made my partner feel feel comfortable. We visit another fertility clinic and it seemed like a baby mill with them actually handing me a piece of paper and saying this is your patient number please reference this when ever you come in or call! Talk about not seeing me as a real person, they couldn't even be bothered to use my name. You won't find that with this place. Besides how we felt at the office and with the staff the main reason we went with this clinic was because they do everything including long term egg storage in house. No transfer to another long term storage facility like the other place. That along with all of the proper credentials was very important to me. This clinic also had all of the latest technologies and options available and would be able to help me not only now to preserve eggs but for the second half of the process when I was ready.

Unfortunately you don't ever see her again unless it's retrieval or transfer day for IVF. Her nurses performed the IUIs I had in 2016-2017. A few years ago, I felt more comfortable as a patient but last year it was a completely different feel. The staff was awesome, but they aren't doctors. When I had a question regarding treatment, it was always the nurses who called me back. If it was something they couldn't answer, they would talk to Dr. Kowalczyk and then call me back but I was never able to speak with Dr. Kowalczyk. I had to demand to talk to her on one occasion because my questions were not answered and I was getting frustrated with speaking through the nurses to her.
I feel like my care could have been better. At one point after my second failed transfer, I looked into going to another clinic.

I am indifferent. When I started in 2016, she was like a breath of fresh air. Always attentive and in communication with me about results from testing, assuring me that our plan was the best, caring when I had a chemical pregnancy, ect. In 2021, I only ever spoke with her during egg retrievals, transfers, and once over the phone. I also don't like that "batch" their patients, where they change your cycle with birth control to make everyone on the same cycle for their convenience of scheduling retrievals and transfers.

I was billed incorrectly for something my insurance covers. I was also billed for a second IVF cycle that I already paid for (which both were resolved but the stress of getting the bill should not have happened. )
I had an awful time trying to get my prescriptions refilled because getting someone to call me back after letting them know I need more of something would take at least a day or two (IVF meds are very time sensitive and having to order through specialty pharmacies would take longer so a prompt call back is necessary.)
I had to call for the results of one of my tests after not hearing anything for 2 days.
They ordered new medication for me and I found out from getting a call from my specialty pharmacy. They never told me I was supposed to take it or any details about it.
I showed up for my appointment (and during Covid you had to wait in your call until they called you to come in) and after waiting 40 minutes with no call, I called them and they said they "missed" me being on the schedule. This happened twice.

One cycle of IVF using antagonist protocol(BCP,Gonal-f,low dose HCG,Gainirelix,ovidrel)
2nd cycle using Antagonist protocol (same meds with higher doses)but had to convert to IUI.
Waiting for 3rd IVF cycle to start and protocol.

The nursing staff we have interacted with were extremely nice and welcoming. They made me feel safe and joked around with me to help me feel more comfortable. One nurse in particular stated that she works really hard to ensure patients feel comfortable because she knows how hard this process is. This really meant a lot to us as at our last fertility doctor we felt like we were not treated as humans.

My only complaint is that I wish she would have done more testing prior to my FET. We only had 2 healthy embryos after PGS and PGD testing. FET failed and then she had me get an HSG, ERA and scratch. Scratch revealed endometrial polypoid fragments and will now need hysteroscopy or D&C.

In 2016 we did letrozole with ovidrel trigger and timed intercourse to conceive our son who was born in 2017.
Trying for our 2nd child we are doing IVF with PGD/PGS. I did injectable meds with ovidrel trigger resulting in OHSS due to PCOS. Waited 3 months and did FET which was a negative. For next transfer will follow same protocol and also add antihistamine protocol.

The Dr. [Kowalczyk] was the first health professional to check for things like my inability to process Folic Acid, which is a critical factor in pregnancy and a clotting factor. I felt no stone was left unturned. She gave recommendations based on our actual situation and needs rather than steering us to trying treatments that were not appropriate. She was also sensitive to our moral/spiritual leanings.
The overall professionalism and treatment options are strengths. The fact that there are multiple locations [of MI Ctr for Fert - Women's Health] where you can take care of routine tests makes the process even more manageable. I can go for routine ultrasounds and bloodwork close to home without making the drive. ... The entire nursing staff is highly knowledgeable and patient. They rather you ask 100 questions than remain confused.

Of course, you are going to see nurses and other staff for bloodwork, baseline ultrasounds, etc, but I saw Dr. Kowalcyzk for consultations, trial transfer and will see her for the actual transfer as well. I do feel comfortable with the knowledge of the whole staff and have never once felt like I was an "assembly line" patient. We left another clinic because of that feeling.

The Dr. was the first health professional to check for things like my inability to process Folic Acid, which is a critical factor in pregnancy and a clotting factor. I felt no stone was left unturned. She gave recommendations based on our actual situation and needs rather than steering us to trying treatments that were not appropriate. She was also sensitive to our moral/spiritual leanings.

Dr. Kowalczyk was careful to investigate my individual health. We decided to use donor eggs. We did not use any egg retrieval protocol (obviously) but followed protocol with birth control, the lupron 2 week kit, baby aspirin patches and progesterone. Since my inability to process folic acid was caught, I am on vitamins that contain methylfolate. The purpose was to make sure my ovaries were quiet and to prepare my body to receive the embryo

Another weakness was I prescribed Ovidrel and later found out that if I used A pharmacy in Florida that would send it directly to my house I would save over $90-$100 I wish they had told me that from the beginning so three of the months I could’ve been saving that money if I had known about the pharmacy that they had a discount through and that was the one nurse just happening to tell me over the phone one day . Ferility treatment is expensive every discount helps

On the medical side, everything is great [at Michigan Center for Fertility and Women's Health]! Expect to wait a little bit for appointments as people can be late before you and that can hold things up. They have so many offices though for your convenience. I loved going to laser acupuncture too and using that side of the clinic as well. Be sure to ask billing questions, some of the women know more on the billing team then others and are more familiar with insurance coverage. If something doesn’t seem right, ask. They are always there to answer and fix it if it’s fixable.

I met Dr. Kowalczyk right before my first ER. I met with her in office after each failed treatment and we set a plan, this was my favorite part of her office. Being able to set a plan for the future made me feel more comfortable. We had plan A fail, so before we started plan B she also told me what plan C could be. When plan B failed. She told me plan C and plan D. I asked if we could move up plan D which was a Lapro surgery to check for endo and she was fully supportive. She explained the extra work involved with that but was really supportive of it being up to my husband and I. When we finally had success the third time, she met with me for an exit appointment and it was amazing to see. She was always open and made my husband also feel comfortable. She’s very blunt and direct which made it all easier since I wasn’t asking questions to get to the real details. 3: frozen cycle with shots for about 20 days to prepare and patches, transferred 2 embryos from cycle 2. Success. Expecting twins.

During my last IUI cycle, I was not called with the results of my HCG test until a The day after the bloodwork was done (usually all results are same day). They was likely due to the fact that I did not have my blood drawn at the office (went to an offsite location on the other side of the state that was affiliated with a different hospital/network)
So we actually saw her [Dr. Kowalczyk's] PA, Alex Karbowski, for our consultation and monitoring. We got into the clinic faster this way. Alexa specializes in PCOS, my diagnosis, so her and Dr. K reviewed our testing together and created a plan. It was nice knowing both of them were overseeing our care. I never met Dr. K in person, but she did call me to confirm my lab results and protocol. She also did my graduation consultation over the phone because we lived an hour away from the clinic. It felt more like a chat with a good friend than a talk with a doctor, which I really liked.

Do expect her to give you a phone call to follow up and be ready to have some questions for her (she wouldn't let me off the phone without asking at least one question). She really wanted me to be fully informed with each phone call we had, despite me being under Alexa (her PAs) care.

We had a missed miscarriage 11 months before we saw Alexa & Dr.K, so after some diagnostic testing to be sure my tubes were clear and my problem was PCOS, they were confident that we didn't need to jump into anything crazy. We did letrozole, Ovidrel injection, and timed intercourse. Alexa was confident that since our sperm and egg could meet, IVF was unnecessary at the time. She said we could do an IUI to increase our chances, but we declined (our insurance covered 50% of infertility treatment and monitoring. No IVF). Their policy was to try a protocol for 3 months, and then move onto more aggressive protocols. They even mentioned possibly switching letrozole to clomid if I didn't respond properly. Our first cycle resulted in a negative pregnancy test, but our second cycle worked.

This practice is definitely getting big. I think they've added a new doctor since we've been there. Their Lake Orion location was closest to me (I live in the Flint area), but they have limited services and hours. I think they were able to do SAs and IUIs there, but any IVF services had to be done at the Warren location. Once you get a positive beta, they prefer you to keep going to the same lab for as long as they want you to so be aware (I had a week and a half of blood draws). Also, once you are pregnant, they graduate you FAST! I was shocked when I got a 5w2d ultrasound and then Dr.K called me later for my exit consultation. It was bittersweet, and I wish they would let people stick around for viability checks like some other clinics do.

Our insurance covered 50% of infertility-related testing and treatments, excluding IVF (not covered). Beta hcg blood draws were super cheap ($5 a pop), and our last ultrasound, OB/gynecological, was something like $100.

The only issue I had here was with the front desk staff. I forget the woman's name, but she is super cranky when you call. She would not let me set up an appointment without a referral from an OB. My PCP had to refer me because my insurance requires her to do my referrals. Her office required me to set up an appointment first, then get the referral set up so my insurance would cover infertility services. They don't do this at this office because most people pay out of pocket. She swore up and down she'd mail us the new patient paperwork, but decided to brush me off because I wasn't doing things the way she wanted. I ended up calling a day later and got Dawn, who was super awesome and understanding when I explained my problem. She set up my appointment and mailed out my new patient paperwork the same day.

As best as I can remember, my protocol started with birth control, then Lupron, Gonal F, Menopure ( with Dr. K adjusting the amounts of Gonal F and Menopure based upon size and rate of growth of my eggs as measured during ultrasounds and based upon numbers from bloodwork results. Lastly the trigger shot take 36 hours before my scheduled retrieval.

I had numerous things that I tried. At first, we did basic testing and as there was nothing glaring, we did fertility medication with ovidril to induce ovulation. A couple of months after that didn't work, we added IUI to this. The rationale was that she wash the sperm, to make sure the best get a chance to try and impregnate. After IUI didn't work, I was referred back to my GYN, who performed surgery to remove a polyp, and consequently diagnosed me with endometriosis. Upon going back to Dr. K., she recommended IVF, as I would have trouble with egg retrevial due to the Endo. We went forward with the simpler IVF. I had trouble producing eggs timely, and had a vein blocking one side during the retrieval so only one egg was retrieved. It was successfully an embryo and implanted on day 3. It resulted in a negative pregnancy test.

They were always available for any questions that I had and always called me back promptly. I felt as thought I was fully informed on my treatment options and what expectations I should have.

After going through numerous testing, I was told that I had a thyroid condition and that the shape of my husband's sperm was off, which could make it hard to fertilize my eggs. I was put on medication and told to eat a low carb diet. We were instructed to try IUI's first, it was possible that his sperm just needed a couple more targets in order for us to become pregnant.
1.) IUI's-I was first put on clomid, which ended up making the lining of my uterus thin even though I ovulated 5 eggs. Pregnancy test negative.
2.) The next two IUI's- I was put on letrozole, which unfortunately did not trigger the release of more then one egg in each cycle. Negative pregnancy test.
The doctor only wanted to perform 3 IUI's before moving onto another treatment. We were given the option of IVF or other stimulation drugs to be used with IUI. The only problem with not doing IVF was that the other treatments were unpredictable and could result in high order multiples. She was concerned because I am very small (short and thin), which could lead to many health complications. We decided to move forward with IVF.
3.) IVF- The day before the egg retrieval, I was told that my numbers that day were extremely high and I was at risk for over stimulation. We went through with the egg retrieval in which 7 eggs were removed. They were fertilized and we ended up with 5 eggs. We were told that we should proceed with a frozen embryo transfer to avoid further health complications.
4.) Frozen embryo transfer- We transferred 2 eggs and I became pregnant with one embryo. We still have 3 left for future transfers.

The nursing staff was amazing, if it wasn't for them, none of this would be possible. The nurse that I ended up talking with the most was Barbara. She was so helpful through this entire process, always calling me back and answering all of my questions. She became like a friend to me, who I could depend on and who knew exactly what I was going through. When I finally became pregnant all the nurses that I had contact with were so happy for me. The last time I talked to Barbara it was such a bittersweet moment that I cried. We had been through so much together and she was so happy for me.

With one of my IUI cycles, I went in for an ultrasound and my eggs were not ready to ovulate, I was told to come back a few days later for another ultrasound. During the next ultrasound it looked like one of the eggs had started to ovulate on their own and I was told that I should have sex and not do an IUI that cycle because it was not timed out perfectly.
. Takes a little bit of time to get the first appointment [with Dr. Carole Kowalczyk]. The nurses do the IUIs but the doctor is available to talk to by phone with questions after hours...I only met with her a few times but she really understood our pain and gave us all of our options with realistic expectations. Each medicated cycle started with an ultrasound to make sure that everything was all right. Clomid was given cycle day 5-9, then a follow up ultrasound was done to count the number of follicles and to make sure everything was all right. We were instructed went to trigger, went to have sex, and then came back for a progesterone blood draw, then the following week the pregnancy blood draw. She was very thorough and I felt very safe.

I've always felt cared for and listened to by Dr. Kowalczyk. Given my age and diagnosis, I decided to only met once with a physician assistant and always opted for appointments with Dr. Kowalczyk. During our appointments, she always took the time to listen to our questions/concerns and I never once felt rushed...has a great bedside manner. She is warm and is a great listener. She's always explained our next steps in great detail and has set realistic expectations. My only complaint is that I wish she was more available during the IVF process.
The nurses I have worked with at Dr. Kowalczyk's office [Michigan Center for Fertility and Women's Health] have been great. They show a lot of compassion and always listen to my questions/concerns. They have reassured me more times than I can count...The staff has been very helpful in finding available appointments when I need to get squeezed in to make a quick decision. The weakness of the clinic is definitely the size and lack of time with Dr. Kowalczyk through the beginning steps.

Dr. Kowalczyk has a great bedside manner. She is warm and is a great listener. She's always explained our next steps in great detail and has set realistic expectations. My only complaint is that I wish she was more available during the IVF process.

While it is a big clinic, I've always felt cared for and listened to by Dr. Kowalczyk. The nurses and ultrasound techs are fantastic. They are very knowledgeable and take the time to explain what is going on and answer my questions. The staff has been very helpful in finding available appointments when I need to get squeezed in to make a quick decision. She [Dr. Kowalczyk] is great at giving compassion during this rough journey. When I first met her I was ready to jump into IVF but she was cautious due to our issue being male infertility and had me proceeded with lab work, a baseline ultrasound, and saline sonogram. Results were discussed in great detail and I walked out of the office fully confident in her and her staff... You may not always see the dr but her PA’s Alexa and Emily are just as amazing as Dr. Kowalczyk!
It is easy to make appointments, phone wait times can be longer than other places, but they [Michigan Ctr for Fertility] are busy and it is worth the wait. The front desk staff is great with confirming appointments and discussing protocols to follow prior to procedures. They are in great hands in this clinic.... She discussed everything with me in great detail and did not jump to IVF and wanted us to follow up with urology to see if there was a cheaper option. My follow up with her discussing IVF put me at ease.

She [Dr. Carole Kowalczyk] is a very passionate doctor. She is extremely versed in her field and is a very personable doctor. She takes her time and explains current paths and possibly alternatives if necessary. Will personally follow up with patients as needed.... Very personable and always asks how you are or what is going on in life or over the weekend. Takes time to answer questions and has a heart.
[MI Center for Fertility and Women's Health is] Very organized. Warm environment. Nice decoration everywhere so you don’t feel like you are at a normal doctors office. It’s not just a white sterile feeling office. Waiting area is calming. They are going for a spa like feel.... Nurses and support team are all very passionate and direct. Calls and questions are answered quickly and fully.
