I doubt Dr. Fischer remembers my name or anything about my personal life, which doesn't matter because she provided the top-notch medical care I was hoping for. She answered my questions with deep knowledge, empathy, and a refreshingly dry wit. Dr. Fischer gave accurate estimates and, as far as I could tell, did a near-perfect job with the medication dosages and timing. She ended up retrieving 15 mature eggs while inducing minimal negative side effects from the medications or the procedure itself.
The only hitch in my treatment was when Dr. Fischer prescribed Aygestin to try to time my cycle when I didn't actually need it, but after I contacted my care navigator Dr. Fischer fixed the prescriptions quickly. By this point we'd talked about scheduling my cycle for months, each time with some Aygestin thrown in to avoid some inconvenient travel I had scheduled, so I can understand why the mix-up happened.

She's great! As a <30 year old woman with no known history of hormonal or fertility issues, I think I had a relatively uncomplicated egg retrieval. That said, even with my cookie-cutter cycle Dr. Fischer and the team did a great job accommodating my needs and schedule.

I had a 5 year old Mirena IUD throughout my treatment. We attempted a cycle in September 2023 but couldn't start when predicted due to an ovarian cyst. Dr. Fischer had me blast it out with Lupron, which didn't work because I'd forgotten to put the medicine in the fridge when I got it (oops) and we followed up with Pregnyl. Afterwards, I probably could have started my cycle as normal but I chose not to because it conflicted with upcoming travel plans.
In 2024 I think I had a relatively cookie-cutter egg freezing cycle. I never got a positive ovulation test when waiting to start and Dr. Fischer said it was possible I never ovulated, but the timing and medications seemed to work all the same:
- 150 units Menopur/225 units Follistim nightly for the first ~week, to simulate the growth of multiple follicles. After the first week, the Follistim was reduced to 150 units.
- 250 units Ganirelix every morning so the follicles wouldn't ovulate early after the first week
- 75 units Menopur/100 units Follistim ~40 hours before retrieval
- Leuprolide acetate trigger shot 36 hours before retrieval

My care coordinator, Aaliyah, responded quickly to messages in my patient portal and consulted with her teammates or followed up on my calls exactly when she said she would. She was patient with my questions and inconvenient travel plans and she helped assuage a lot of my early anxiety when I didn't see positive ovulation test results.
The nurses at Spring were kind, helpful, and mostly in the loop with my treatment plan. And when I had a question they couldn't answer, they could reach the doctor pretty easily to get it resolved. I never felt ignored or misled and was basically never fed conflicting information.
All the staff at Spring really do seem to care about their patients and will go the extra mile to make sure what they're telling you is correct.

The conflicting information I received is minor enough as to almost not be worth mentioning, but I'll mention it because I think it demonstrates how this clinic goes above and beyond for its patients even when things are confusing.
Dr. Fischer prescribed me a Lupron shot to force my body to get rid of an ovarian cyst that was interfering with my treatment, but I realized at injection time I'd forgotten to refrigerate the medication. In a panic I phoned the doctor on call, Dr. Klatsky, who guided me through the shot and scheduled followup blood work. Over the course of a single weekend Dr. Klatsky reviewed my results, prescribed a second shot of Pregnyl, scheduled/reviewed my bloodwork again, then made a point of following up with my care navigator to let her know that my cyst was cleared and I should be good for a slightly delayed egg retrieval cycle. This confused my care navigator on Monday, who (correctly) understood that the delay wouldn't work with my schedule that month.
I'm not Dr. Klatsky's patient, so it makes sense that he didn't have the full context behind my cycle scheduling. Still, he took the time to ensure that Dr. Fischer's directive (popping my ovarian cyst) was carried out well and to think about my cycle more holistically as though I were his patient.
Dr. Fischer is a brilliant doctor and scientist. At the same time, she is an excellent and caring clinician with an exceptional bedside manner. Throughout my treatment, starting with our initial meeting and consultation, I felt that she strove to meet me where I was, and I am sure she does so with every one of her patients. She is willing to cater her care to each individual. I am someone who wants a lot of information to understand what is going on (especially when it came to understanding as much as possible about why I had had repeated losses), and she was willing to provide that information to me whenever I had questions and to the extent she was able to find answers for us. She created a trusting relationship right off the bat, and this also allowed me to relax and sometimes let go of needing to understand each step (especially once we were in the thick of egg retrieval, and I just went with whatever she recommended I do).
The [Spring Fertilty] clinic space itself is really nice, and the surgical and lab floor where retrievals happen and eggs are fertilized is state of the art...everything flowed like a well-oiled machine...I rarely had to wait longer than 5 minutes for my appointments to start. It was easy to schedule the next appointment...My only minor critique is related to the finance department. I did occasionally have trouble getting timely responses to emails, though when I did eventually get responses, I was able to get resolution to my questions. I did have to follow up a few times, which was sometimes frustrating.

Dr. Fischer went above and beyond to provide me with excellent care. I am fairly certain she was supposed to be off work on the day of my second egg retrieval, and she came to do the procedure anyway. (I would note that I don't agree with this, and I think she should take her well earned days off, but I did appreciate it very much!) Our relationship started after I read reviews of Spring and Dr. Fischer specifically on FertilityIQ. My husband and I had a first meeting with her where we discussed our history of miscarriage, and where we began talking about IVF. We came to the table ready to start IVF right away - we did not want to begin with other interventions - and she was understanding of that decision. She immediately began strategizing our next steps for and assured us that she would do all she could to help us find success. Fertility is extremely complicated and there are so many nuances; I always felt that she would provide as much information as we wanted to have about what was going on, and that we could always ask questions. Dr. Fischer is clearly a busy doctor, and very beloved by her patients, but I always felt that she was extremely present every time I saw her for an ultrasound or an appointment during this process. I am grateful to her for her dedication and excellent care, and I would refer my friends to her in a heartbeat.

I did two rounds of egg retrieval back to back. The first was shorter, requirng about 8 days of medication, and the second required about 14 days of medication. I think I was on a fairly routine dosage schedule, but she pushed harder the second round to get more mature eggs. I did typical Gonal-F and Menopur injections the first 5-6 nights and then the antagonist (Cetrotide) was added to suppress ovulation. During the second round, we went longer and my night time injections were reduced slightly towards the end, but we kept going for longer. In the first round, I produced 15 eggs, 9 of which were mature and fertilized via ICSI, and 5 of those became day 6 embryos. We PGT-A tested, and all 5 were euploid. For the second round, I produced 16 eggs, 11 were mature and fertilized via ICSI, and all 11 made it to day 6; 7 were euploid after PGT-A testing. We were very impressed with these results, and were of course grateful that Dr. Fischer achieved such a good outcome for us.

Dana was my assigned nurse during the entire process (genetic work up, two egg retrievals, and prep and completion of transfer), and she is wonderful. I felt so lucky to get to work with her. Not only is she extremely smart and on top of the protocol I was on (and I imagine she has a lot of patients!), she had a wonderful and calming presence. Even when she was not available, I was always able to reach someone on Dr. Fischer's care team, and they were all very professional and great to work with. I really appreciated that if someone did not know the answer to a question, they would just say so and then check and call me back. In general, the office experience is very smooth and appointments moved quickly and on time.
During the egg retrievals and IVF transfer, I was assisted by a wonderful team of nursing staff on the surgical floor. They were also very kind and had wonderful beside manners.

Dr. Fischer (and I believe Spring in general) only transfers one embryo at a time as a matter of policy. My understanding is that they do not make exceptions to this. I believe they are making this recommendation because the science supports it, and I did not have a problem with this approach.

There is currently a national shortage of Menopur, so I took a higher dose of Follistim combined with the low dose Pregnyl instead. Around day 7, I started Ganirellix as the antagonist, and then used Pregnyl and Lupron for the trigger shots. After each visit, which consisted of an ultrasound to measure the size of the follicles and bloodwork, I received updated medication instructions tailored to me based on how my follicles were responding to the treatment. The results were excellent, as 94% (all except 1 egg) reach maturity and were able to be frozen. I could see that there is almost an art (and a lot of medical expertise) when it comes to how Dr. Fischer chose to modify the protocol over the course of the 2 weeks, and then chose the right day for the trigger shot.

She follows up and explains everything to you with compassion, and offers to be there for you even when I couldn’t financially go forward with the procedure.

I did the initial assessment but due to cost, I was unable to move forward yet with the egg freezing I medically need due to low AMH and FSH and follicle counts. She said I will likely need 2-3 cycles performed, but cannot without insurance. She has helped write letters and provide information for me to bring to my employer in my fight for coverage.

Since my first consultation with Dr. Fischer, which was virtual due to the COVID pandemic, I immediately liked her and felt I could trust her 100%. Her knowledge and compassion were extremely evident. She was very reassuring and she was the first doctor who diagnosed me with PCOS at the age of 34. For most of my adult life, I tried to tell my doctors that I felt that something was "off," but I felt that my doctors never really listened to me and brushed aside my symptoms. Within the first 5 minutes of my first conversation with Dr. Fischer, she was able to diagnose me with PCOS, which was one of the factors of our infertility. It was both reassuring and upsetting for me, and Dr. Fischer displayed so much compassion and gave my husband and me hope that we could conceive with fertility treatments. Dr. Fischer fully explained the treatments that were available to us, guided us on the probability of conception through IUI vs IVF, and never pushed us towards a specific treatment. (we decided on IVF due to the higher probability that we would conceive.) I was also so happy that Dr. Fischer was able to perform both my egg retrieval and embryo transfer. My husband and I decided to pursue IVF rather than try IUI first. The success rate of IVF was much higher for us, and we didn't want to wait any longer! I have PCOS and my husband also has an infertility factor having to do with his sperm (low morphology and motility,) and Dr. Fischer felt that we would be very successful with IVF. I did one cycle of ovarian stimulation injections for IVF. The embryos were biopsied and tested for genetic abnormalities (PGS testing.) Turns out my IVF cycle was very successful- we created 25 embryos and we decided to test 12. Amazingly, 11 of the 12 embryos were viable! The embryos were then frozen, and I had my embryo transfer a little over a month after my egg retrieval. I am currently 12 weeks pregnant!

The nurse assigned to me was always prompt with returning my phone calls and calling me to give medication instructions during my cycles. I really can't criticize too much, however, I felt that she didn't always communicate certain things to me. I felt that I had to ask a lot of questions in order to get all of the information I needed. For example, there are certain restrictions you have before and after egg retrieval as well as after the embryo transfer (i.e. what to avoid doing, such as exercising.) I felt that my nurse wasn't fully forthcoming with some of that information, so I researched a lot online, then asked her questions to confirm. At times I felt a little rushed during our calls as well. Also, when I spoke with other nurses that covered for my nurse when she was off, I felt that they spent more time with me on the phone, I didn't feel rushed, and they were more detailed about my treatments, what to expect, side effects, what to avoid, etc. So I think it really depends on which nurse is assigned to you and whether you will have a more positive experience.

(Dr. Margolis and Dr. Maguire were great!) I only had 2 bad experiences during ultrasounds- one doctor was kind of aggressive during an US during egg stimulation which was very painful, and another nurse practitioner was very insensitive to me when she found a sub-chorionic bump on my first US after I was pregnant, which scared me. There are times when you feel like "just a number," but overall, I had a good experience at RMA.

The hours for monitoring are very early in the morning, which was fine with me because it worked well with my work schedule. I barely ever had to wait for blood work or ultrasound. I think there were maybe 3-4 times when I had to wait longer than usual.

I was fortunate that my insurance covered all of the IVF procedures and most of my medications (or I had a small co-pay.) However, we still had out of pocket costs. You also have to pay out of pocket for cryopreservation, and you have to continue to pay an annual fee to keep them stored. I had a couple of other bills for one of my test procedures and for anesthesia during egg retrieval. In all, we spent a little over $6,000 for IVF.

We met Dr. Fischer on our first consultation visit. My husband and I had seen a series of OBGYN's that just didn't know what was wrong with us. I had recurrent miscarriages and no one knew why. When we met Dr. Fischer, she provided us with more information than we had received over the course of 2 years. I won't lie, we left the clinic a bit overwhelmed with the amount of information that was laid on us. She called us back for testing, but I was working full time and felt like it was just too much. The clinic was so understanding of this when I canceled my appointment. I took a month-long break and then decided I was ready for testing. I went into the clinic and had everything drawn up. Dr. Fischer's response came fairly quickly after all the results were in. I was diagnosed with Balanced Translocation (BT). She spent about an hour going over the diagnosis. She waited patiently as I took notes and tried to understand what was going on and what my options were. She offered to call my husband and re-explain everything to him as well since he couldn't be on the call at the same time as me. She offered a couple of different solutions to our diagnosis but it was obvious that we needed to move forward with IVF with PGD testing in order to reduce the risk of having more miscarriages. Dr. Fischer was readily available for us, to prescribe medication as my bloodwork developed, to answer questions as they arose, and put us at realistic ease when things were really looking south. She performed both the retrieval and the transfer. During both procedures, we felt like we were in capable hands. My retrieval resulted in 33 eggs, 27 fertilized, 15 Day 5 blasts, and 5 were balanced/normal PGD. Her medical protocol had worked outstandingly for us. She really has a way of making you feel like you're her most important patient. The best part, I'm currently 20 weeks pregnant with our miracle baby.

When I came to her I had experienced two losses; before even getting to the testing portion I felt extremely overwhelmed coming into a fertility clinic. Dr. Fischer took her time with us, she was extremely patient as we came to terms with our diagnosis. She called and explained everything thoroughly and ensured I understood every aspect of it. She has an extremely calming demeanor that just puts you at ease. I'm not sure how it's possible, but out of her all her patients she truly makes you feel like she knows you well. Monitoring appointments were always every morning from 5:45 AM to 7:30 AM. If you needed to go in over the weekend, the office would then transfer you to Basking Ridge. They were usually very quick, I would be in an out within 30 minutes. I think on two occasions they were quite overwhelmed and it took longer -- closer to an hour. I suspect because those were days coming off of holidays / long weekends. Either way, the entire process itself was very efficient.

Insurance did not cover anything, everything was out of pocket. It cost us about 30k for one full IVF cycle (including testing, bloodwork, retrieval, PGD testing, and retrieval). It will continue to cost us $1200 every year to keep our embryos on ice, and furthermore when we transfer future embryos.
Dr. Fischer was extremely kind, answered all my questions, scheduled extra phone calls when I wanted to, and was also extremely positive even when we received what I thought was discouraging news. She pointed out reasons for hope and optimism and was encouraging the whole way.... There were two mornings that I showed up and my orders were not in their system, which meant a lot of phone calls and waiting around to figure it out. One of those times I believe it was the other clinic's fault, but one time the nurse had forgotten to send them over.
Dr. Fischer put me on Gonal F and other injection medications for my egg retrieval...after my egg retrieval I did a genetic screen test on all my embryos, she then froze the rest of the good ones and during my transfer she transferred only one healthy embryo and I am now pregnant!...is a sweetheart, always so kind and positive. I never felt a negative vibe from her...was always speaking to me as if my IVF was going to work, she would always re-assure me that everything will be ok, she said in the end, I will have a beautiful family and she was right!
I also chose to do genetic embryo testing [at RMA of New Jersey] which was not covered by insurance and I paid $5,000.00 out of pocket for that...Everyone at the clinic is amazing. The receptionist girls are great, very friendly, always remember you by your first name. Early monitoring employees are also so nice. You begin to establish a friendly relationship with everyone which is nice. I can't comment on any weaknesses of the clinic. Sometimes the monitoring are quick sometimes it takes longer. Dr.Fischer is very caring and friendly. She understands our needs and respect our choices. I got Covid during the process on day3 after I started my cycle, i think dr.fischer did adjust the treatment a little bit but in the end we are very satisfied with our result...is very nice and friendly, we love her energy and she is very patient...the result is perfect, we retrieved 21 in total, and in the end 19 of them are excellent and good.

This advice is moreso for anyone seeking fertility treatment in general. Before walking into my initial consult, I honestly didn't know much about fertility treatments and tests (Chlomid, IUI, IVF, HSG, Saline Sonogram, etc). While Catha took the time to explain our options, pros and cons, and timelines associated with each option, it's a lot to take in at the first meeting if you're not fully aware of them at all. I would advice prospective patients do a little research on their own to understand the options at a high level so that you don't walk into the consult blindly!

Dr. Catha and everyone on her medical team never treated me just like any other patient. They thoroughly reviewed my history, results, and next steps before communicating them to me in a clear and optimistic way. Everyone was so kind during the process. Going through infertility is tough as is, but having an empathetic patient-oriented team helped me feel at ease through out the whole cycle.

After our initial consult, we knew we wanted to pursue IVF because it statistically has the highest chance of success, even though it is the most invasive. Following our consult, we informed Spring that we'd like to proceed right away.
Even though we struggled to get pregnant on our own, we didn't have any serious infertility diagnosis. It ultimately came down to ovulation timing a mild case of PCOS. Dr. Fisher wasn't worried and assured us that due to my age, health, and follicle count that we should have a high chance of success with one cycle.
We started with an HSG to confirm my tubes were clear. From there, I started the IVF injections after my menstrual cycle arrived. I did Gonal-F, Menopur, and Cetrotide (mid-way through) injections for about 12 days, and triggered with Lupron. Following any bloodwork at the clinic, Spring immediately updated me with dosage amount prior to my next injection. We had a successful egg retrieval. Following PGT-A testing, we ended up with 10 blasts. More than I was expecting!
My FET was about a month and a half following egg retrieval. The waiting period between retrieval and FET is long, but necessary for your body to recover and for you to prepare your uterine lining. Before transfer, I did a saline sonogram to ensure my uterine lining was normal. Dr. Fisher prescribed a natural FET protocol for me, which involved progesterone pills and one trigger shot. After the retrieval, I didn't have to take any supplemental medication.
After a few weeks of nerve-wracking waiting, two blood draws and ultrasounds later, we heard a heartbeat :)

I first met with Alyssa to schedule my appointments and discuss timeline. Following that, I primarily communicated with Maggie (my IVF nurse coordinator) and on occasion, Samantha, who covered for Maggie. Maggie always followed up with me on the same day following my appointments with next steps.
Communication from Spring would come as a phone call or as a message in the portal. If Spring wanted to communicate results, I usually got those in the form of a call, which I appreciate. For example, following egg retrieval and PGT-A test results, they called me to discuss results. It's always helpful to hear a human on the other end of the line!
With regards to in person staff... The front desk staff at the NYC location are super friendly! After just a few appointments, they knew me my name and always greeted me with a smile. The nurses that help out at your appointments are great. I usually dread getting my blood drawn, but the nurses at Spring are gentle!

I am not sure how other clinics work, but the one thing I think Spring can do better with is helping patients order refills for prescriptions. Spring set up my initial prescriptions for my injections, but refills are up to the patient to order through the partner pharmcy. You could always ask the nurse to help you calculate, but they do not place orders for you. You have to make sure that you order the right amount based on your updated protocol. This means you have to do some math and guesses as to how long your cycle might be and how many refills you may need. I think this is important to consider, especially if you're paying fully out of pocket for medication. Every dollar counts!

Monitoring appointments are usually in the morning. The clinic can be quite crowded during this time. Spring is usually pretty efficient at getting you in and out of there within 15 minutes.

In between the waiting period between the egg retrieval and FET, you may be prescribed birth control, which I was. After 10 or so days of birth control, you stop and wait until you get your period. Spring mentioned to me that I should message them "after my first full flow". However, after I stopped BC, I only ever spotted and never experienced a full flow period like I typically do in a normal cycle. I messaged Spring to ask if this was normal, but was told to keep waiting until my first full flow. When I saw Dr. Fisher for my saline sonogram, she said BC makes your period weird so light spotting is totally normal. This was probably just a minor misunderstanding/miscommunication between the nurses and the doctor, so I am glad Dr. Fisher was able to assure me that I was ready to start the FET protocol.
For my second egg stimulation, Dr. Fischer changed the dose of Follistim which resulted in a significant higher number of eggs and consequently more fertilized eggs. For my second egg transfer, Dr. Fischer decided to do a synthetic cycle, opposed to a natural cycle, where she could control my ovulation period and resulted in my current pregnancy...I felt like I was the only patient she was treating, even though I new I was not. From our first visit, Dr. Fischer was very supportive and caring. She listened to all our issues and concerns and answered every single question we had. She would answer the same questions a thousand times if I needed without hesitation.

RMA in Basking Ridge is amazing, it felt like home during my two years of treatment there. The front desk were always very helpful and kind, the nurses where very skillful and caring, the exam rooms were always clean, very equipped and the ultrasound machines were all new. RMA Basking ridge has an incredible infrastructure and that gave us peace of mind giving us confidence during the procedures. RMA Basking Ridge has a great system when you go for the morning monitoring, which can get very busy. There's always a nurse in the clinic if you need as well. My only complain is that you can't call the front desk directly. RMA has a call system that transfers you to whom you need to speak with. During the weekend, if you need to speak with a nurse, you have to call the call center, try to explain your concerns to a non medical person, leave a message and the nurse would call you back. It is all very efficient, and I understand that in order to control volume they have to have a centralized system. But I wish I could also had the option to talk to the front desk especially if I had dealt with someone there with a specific issue. I also had some issues with the financial department. In the beginning we were never completely sure how much certain procedures would cost us, it wasn't as clear to us. I removed myself completely from the financial issues because I couldn't deal with my financial coordinator, I passed that ball to my husband who has a better understanding of insurance since he works with insurance.

Since I didn't have any hormone issues, Dr. Fischer did a moderate medicine protocol for the egg retrieval and found the perfect balance of quality and quantity. We had 13 eggs and were left with 5 embryos frozen. For the transfer, she decided not to put my body through more than necessary and went with a natural transfer. This followed my body's natural cycle. It can be frustrating when your body takes longer than expected to ovulate following a retrieval but I'm glad we did that because all I took was a trigger shot followed by progesterone suppositories. That transfer took and currently 14 weeks pregnant!

She gave honest opinions on treatments and never gave false promises but definitely gave me the ability to want to try one more time. She was at every visit, she explained everything to me...Dr Fischer gave my husband and I a son and my daughter a sibling.
Spring is full of great communicators. I never had to worry about next steps or what was coming. They took care of everything and made sure to tell me all of the details. When I had questions they were always answered within 24hrs but usually by close of business. I always had total faith that things were being taken care of...I recommend Dr Fischer to everyone who needs hope. Don’t waste your time and money going other places. Just go to Spring.

I came to Dr Fischer somewhat broken. I had had multiple failures over the course of 2 years at the “best and “most prominent “ fertility clinics across the country . I didn’t have much hope. Dr fischer did an hour consult w me. Went over what had happened in the past, and came up w a new plan. She gave honest opinions on treatments and never gave false promises but definitely gave me the ability to want to try one more time. She was at every visit, she explained everything to me, and I seriously felt like her team were my best friends while I was in treatment. I miss them! I’m looking at my baby boy right now. Dr Fischer gave my husband and I a son and my daughter a sibling. We have two more boys on ice. There are no words to describe the gift that she has given us. I recommend Dr Fischer to everyone who needs hope. Don’t waste your time and money going other places. Just go to Spring.
