Dr. Smikle was by my side through it ALL...always looking out for me via patient advocacy....egg retrieval surgery, endometriosis surgery, 3 PRP injections and two transfers...appreciated that Dr. Smikle didn't allow it me to go to transfer because it would have failed...told me that due to my size, and T-shaped uterus I was not a candidate to transfer more than one embryo at a time.

I know this is unknown and unwanted territory so it is hard to have trust, especially when treatments fail. However, rest assured that Dr. Smikle and his team know exactly what they are doing to help you become a parent. Dr. Smikle is in the business of good outcomes - he wants this for you too, so just trust in him.

Describe the protocols Collin Smikle used in your cycles at Laurel Fertility Care and their degree of success.
I had two cycles that didn't go to transfer due to the lining not reaching an ideal measurement. While this was hard, I appreciated that Dr. Smikle didn't allow it me to go to transfer because it would have failed and I would've subsequently lost 2 embryos (children). During the third cycle he performed endometriosis surgery and 2 PRP injections. The lining increased, but still wasn't perfect. The third cycle went to transfer and failed. The fourth cycle we did another round of PRP injections and then we did trigger shots - that were used during the egg retrieval process. The lining measured the same as the 3rd cycle, BUT it worked and I am now pregnant!
Describe your experience with your nurse at Laurel Fertility Care. (Assigned nurse: Leslie Barahona)
Leslie was my biggest fan throughout my IVF journey and she displayed so much tenderness along the way. I can’t even count the number of scans she performed for me throughout this past year, and until recently, the scans were always less than ideal. But, she had a gentle way of breaking the news to me that made me feel cared for in such times of need.
Describe your experience with Laurel Fertility Care.
The clinic and staff was always rooting for me. They are like a cheer squad and it is lovely. They all work very well together and are a solid team. I attended the Modesto clinic AND I live an hour away. The one thing that was challenging for me was that they only did appointments in the morning (8:30 -12). Thank goodness my work schedule was flexible, but this is something you need to keep in mind. Lastly, make sure that you USE your "patient portal." They don't always answer their phones which can be a little frustrating but I learned to use my portal and found it easier to convey what I needed better in writing anyways. Additionally, they're super responsive with the portal messages too - more so than the phone. That's the only thing I think you should know. And again - remember that they want this for you too. Just trust them and don't be afraid to ask as many questions as you need as it can be overwhelming at times. They won't make you feel stupid.

Dr Colin Smikle barely talked to me during my consultation. I did appreciate that he gave room for a new very nice and caring Nurse Practitioner to practice nextside him to lead, but I didn't feel like Dr Smikle was very alert, nor did he speak or provide follicle count in the room which makes a patient nervous. I believe he's experiences, but his care does not feel personal or warm.
[Laurel Fertility Center] Strengths: The nurses. They were sweet and attentive, and knowledgeable. Weaknesses: Very very expensive storage costs, somewhat disorganize / requires active patient coordination, old fashioned offices that are quite cluttered. I consulted four clinics and my gut and other reviews just told me to not go with Laurel. I do hope they improve.
How was your experience with Collin Smikle at Laurel Fertility Care?
Dr Collin Smikle is excperiences, but ultimately I didn't particularly like his communication style or bedside manner. He is the quiet type. He also falsely told me that some of my homone levels were elevated, which made me worry. Ultimately, the lab was not done on the right day, and the E2 number confirmed the lab result wasn't accurate. I only found this out after going to another clinic with a doctor to reassure me, explain, and take repeat labs (which were normal). I felt like Dr Smikle wanted to sell me on a big promise on number of eggs and create urgency based on the off labs.
What's one piece of advice would you give a prospective patient of Collin Smikle at Laurel Fertility Care?
Be prepared to lean on nurses and other staff for warmth if that's what you need. Ask Dr. Smikle detailed questions as well as follow-up questions if you feel you aren't getting enough information.

I talked to a new NP, whom I can't remember, whom did most of the prep, intake and baseline ultrasound. She was lovely, compassionate, competent, and reassuring. Other clinical staff were responsive on the portal for labs and next steps. However, some staff missed phone appointments and were avoidant in follow-up times.

Dr Smikle has good bedside manner and seems to genuinely cares about his patients...there were many times information about my health status, were missed. He seems rushed during appointments. Overall, he is very knowledgeable...He also does not perform investigative procedures like ERA and doesn't seem to believe in it. Overall, Dr. Smikle is competent, kind and wants his patients to succeed. But if you are the kind of patients that wants to know every step, you might get frustrated because sometimes he rushes through appointments and not as thorough in explanations.
The clinic [Laural Fertility Care] runs very efficiently, so be on time for appointments. They are located in a busy part of SF, but there are ample street parking. You have to pay before any procedure. All the staff are very nice and friendly...The nursing staff is very orgainized and efficient. They were very efficient with prompt communication. I have no complaints about the nursing staff...18K for eggs retrieval, FET, ICSI,PGT-A to the clinic with a 5% discount for paying cash. 1-2K for the company that performs the PGT-A. 4K for meds. to first cycle costs around 23K. Each FET costs arouns 3.5-4K not including meds.

I had 1 procedure with progesterone suppositories and estrace and FET transfer which resulted in a chemical pregnancy due to a variety of unforeseen problems ( a degenerative fibroid and fluid in the uterus). Then after we stablized the fibroid and I had surgery to remove my fallopian tubes, I had a stimulated transfer with Gonal-F, PIO and Estrace. The FET failed and we are trying to figureout the reason.

The nursing staff is very orgainized and efficient. They were very efficient with prompt communication. I have no complaints about the nursing staff.

I would advise a prospective patient to stay with Dr. Smikle, he's thorough, and willing to change treatment plans as necessary. Very important this doctor will LISTEN to all your concerns! He will try everything possible to help attain your goal of bringing a baby into your life.

All the phone conversations we had with Dr. Smikle from the beginning gave us hope we can fulfill our dream. His voice was soft spoken and he made us feel very comfortable. Dr. Smikle was very knowledgeable in all aspects of fertility, answering all questions, and spent all the time with us we needed. We knew we were in the right clinic from the very beginning.

I can't say specifically the protocols used, but I will say plans and treatments were altered as necessary. Dr. Smikle did not give up... we are expecting our miracle baby in Nov. 2018!!!!!

Yes we were fully informed when we needed to make a decision on the number of embryos to transfer, we all were in agreement to transfer one embryo. We did PGS on our embryos before any transfer was done.

During treatment, were you treated like a number or a human with Collin Smikle at Laurel Fertility Care?
I was referred to Dr. Smikle by my OBGYN after an ultrasound revealed that I had endometriosis that was potentially affecting my fertility. My husband and I had been trying to conceive naturally for about a year with no luck and, while not quite desperate, we were definitely looking for a different and hopefully more successful approach. I actually remember how comforted I felt the first time Dr. Smikle shook my hand...I just knew I was in the right place. At my initial consultation, after the exam, he worked with me to outline a plan on how to proceed. I wasn't quite ready to try IVF, so we started with Chlomid and IUI, but with the knowledge that I would likely have to try IVF if we wanted to have more than one child. After 3 unsuccessful attempts with IUI, we decided to switch to IVF, which is an intimate, intricate and sometimes very difficult process. The whole time, Dr. Smikle, Donna my nurse, and the rest of the staff were always available to answer any questions, fill my prescriptions with the lowest cost possible options, and even give me a shot or 2 when I was initially scared to do it myself. In short, I felt like they treated my like they would a good friend...not just a patient. When the first round of IVF didn't work, I felt like everyone involved cared just as much as we did...and the thing I loved most was, after they comforted me, we all went straight back into planning mode. Dr. Smikle wanted to try something a little different, just so we didn't keep doing the same thing with the same result, and, when my second round of worked, I felt like the whole office was cheering for us. They even asked to see the ultrasound pictures...which I'm sure they have seen a million tiny gray "kidney beans" on a piece of paper...but when it's your "gray kidney bean" it's pretty special. In short, my experience at Laurel Fertility was exceptional..The service was incredibly personalized and professional and Dr. Smikle and the entire office staff were always available (on the first ring of the phone) to answer any questions I had...I look froward to going back for baby number 2!

3 IUI - after 2 negative results he recommended we switch to IVF because he didn't think another IUI would be successful but I asked for one more since I already had enough medicine to move forward. 2 IVF... one with progesterone and estrogen injections and one without. After the first round didn't work, he suggested trying something different. We essentially "tricked" my body into thinking it got pregnant naturally. and it worked

Donna was amazing. She was always available to answer questions and help with the medicine. I couldn't imagine trying this process without her.

Weaknesses: Having a doctor who travels between clinics can be hard, especially when you re scheduling a transfer or extraction. But I felt comfortable in everyone's hands and luckily Dr. Smikle was always able to be there for my procedures.

Laurel SF has all the strengths of a larger clinic while getting much more intimate care. ... The finance team works with your insurance ... Egg retrievals and transfers are done in the clinic and the laboratory is on-site. They are much more progressive with pre-cycle preparation than other clinics. They do recommend supplements and lifestyle changes in preparation for cycles which is different than other conventional IVF clinics. They are also open to working with acupuncturists especially before and after transfer.

Dr. Smikle is compassionate and approachable while being no non-sense. I never felt like a number in his practice. If you are looking for a doctor using the conventional IVF approach with big clinic certification but in a small, intimate clinic, then I would highly recommend Dr. Smikle.

Come to each visit prepared with all of your questions. Dr. Smikle will take the time to answer all of your questions and concerns. During each visit, he will give you all of the information necessary for the next step but may not necessarily paint the full picture with all the details. If you ask, however, he will answer.

Based on egg yield success at my previous IVF clinic, Dr. Smikle tweaked and added to the estrogen priming protocol. Dr. Smikle added human growth hormone in an effort to enhance egg quality in addition to the potential higher yield. Both egg retrieval rounds did not yield as many eggs as the past, however, more embryos made it further than before.

My primary nurse and coordinator were always available by phone or through the patient portal. They were always very patient and careful to explain the process step by step. Blood draw, vitals and weight were always performed in a private room rather than in a common area and all attention was focused on the patient.
Describe your experience with Laurel Fertility Care.
Laurel SF has all the strengths of a larger clinic while getting much more intimate care. Egg retrievals and transfers are done in the clinic and the laboratory is on-site. They are much more progressive with pre-cycle preparation than other clinics. They do recommend supplements and lifestyle changes in preparation for cycles which is different than other conventional IVF clinics. They are also open to working with acupuncturists especially before and after transfer. I think that all clinics can use improvement in the area of working with integrative medicine practitioners to improve the general health of the patients and with success rates, but it was a comfort to know that Laurel was open to this course of side-by-side treatment. This clinic works with health insurance which is a plus for decreasing stress on the finance side of things. Although, we were not successful with Laurel, our overall experience was very good. There is no regret that we might have had better success at a different facility.

When I began to have major side affects to the growth hormone he [Dr. Collin Smikle] didn't seem concerned. When I hyperstimulated he also wasn't too concerned even after I ended up in the emergency room. He was unapologetic when we miscarried. My biggest complaint is the lack of medical information protection we experienced at the clinic. At each appointment the ultrasound screen would show the last patient full name, date of birth and last image taken. They didn't sign out of the machine after each appointment. The front desk area had no privacy, everyone waiting for an appointment can here us verifying our names, date of birth, payment details, at some point I wrote down my social security number so that no one could here it. The positive was I got pregnant, the negative was the OHSS and miscarriage. The nurses were friendly but left the clinic shortly after our cycle ended. Overall it was an awkward cycle and experience.

Felt like a human because of one nurse who was always happy to see us, responded to my inquires in a timely manner.

Estrogen primed Antagonist with growth hormone. I do terribly on birth control, Dr. Smikle agreed to work with my natural cycle which other clinics at the time wouldn't. Due to previous miscarriage he had had me on baby aspirin and a steroid as an anti-inflammatory. The growth hormone wasn't explained. The overall result was Pregnancy with hyperstimulation followed by a miscarriage. I was on medical leave for 3 months and it took 1 year to gain my health back. For a year we couldn't do IVF treatment which was super upsetting.

If they were unsure they'd quickly get an answer. They always greeted us with a smile, cared about our cycle and showed compassion with my OHSS symptoms and miscarriage.

I was given a treatment plan that seems very "standard"...not tailored specifically to me or my partner's unique situation.

I did a standard IVF cycle. Lupron, gonal F, etc. halfway through the stimulation phase they measured and saw I had 33 follicles growing. Though I was then at risk of developing OHsS, the protocol was not changed. I went on to harvest 44 eggs - and had extreme OHSS which cause us to freeze-all for that cycle. It took 6 weeks to recover and do a frozen transfer. I think, with more careful monitoring and and adjustment in my mess (reducing the amount) I could have avoided a lot of the suffering the OHSS caused.

I felt like some nurses were good (Meredith was great!) and some were not. Donna seemed inexperienced and often kind of casual about everything. it was all kind of hit or miss. Some were great at drawing blood, others were horrible. Some were kind and warm, others were not. I never really developed a relationship with any of them, which is kind of incredible given the emotional experience that IVF is.

I had fluid in my uterus that would occur around each ovulation; Dr. Smikle believed this was from my past C-Section, and used extra caution doing a transfer, despite how impatient we were to get it done. He had me go through a natural cycle with no meds, followed by one with meds to see if we could control the fluid. When we was comfortable all the factors were favorable, then he made the transfer, and I trusted his judgement throughout the process. If he would have rushed and just done the transfer, despite the fluid, we would have wasted an embryo.

Him and his staff were very helpful in guiding and explaining decisions... Visits were often not as long as i liked but i never felt rushed... We did letrozole and trigger first then 3 IUIs following with same meds. He used letrozole specific due to my history of breast cancer. He provided me information on how to overcome a clotting disorder to prevent micarriage if i got pregnant

Unfortunately the results were not what we expected nor what he predicted but he didn’t have a lot to explain for that. I understand that not all medical issues can be explained, however. I am hopeful for our next steps.... They didn’t inform me about my OHSS appropriately even though prior ultrasounds were evident until I asked specifically.

He [Dr. Smikle] was very kind, heard my concerns and was open and honest.... hes not a man of many words but hes smart, and honest and competent at what he does.i always felt like i was in good hands and that he wanted me to have good results. He very good at responding to calls and emails also and he was good at explaining things i could understand it was a good experience.... he only wanted to transfer one emryo due to my history if premature births he felt twins would increase it.

Dr. Smikle is a fantastic Dr who truly cares about his patients. He is always positive. Every time I saw him he had something positive to say. I felt very welcome. I left every appointment feeling positive. Always took the time to listen to all my concerns and answer to every question I had...with one failed transfer Dr Smikle didn’t waist no time he had me do another retrieval to bank more embryos. I had a total of 6 tested embryos and my third transfer was successful
The [Laurel Fertility Care] clinic was always on time. If I had a 9am appointment I was in by 9am and a few times I was running late and the clinic managed to see me. The medical assistants are so sweet and nice. I graduated from the clinic today and I’m sad i really wish they would have keeped me longer...I had the best nurses and team every time I would message Melissa she would respond fast or call me to make sure I received the message in case it was towards the end of the day! Leslie is just the best always positive always made me feel comfortable and positive
