I wanted to do another retrieval before I transferred my last embryo and he felt like that was not worth while knowing I only wanted one more living child. Additionally, he was confident that my less well graded embryos were both worth transferring, which many people on the internet disagree with. I chose to trust him and not let instagram infographics get me down and he ended up being right. I really appreciated how smart and data driven Dr. Kaser was, but still straight forward and kind. He never made me feel overly confident but did let me know when he truly felt like giving X strategy a shot. I also appreciated how quick his response time was on Spruce. Frankly, RMA has gotten really popular (as it should be), but I didn't see him that frequently in person for monitoring and other appointments, but he always responded so quickly to my questions and requests. He also was very open when I asked if we could add meds or change protocols, and never acted like his toes were stepped on and was very collaborative and receptive. My first cycle was a moderate to high dose antagonist protocol of 300 FSH, 2 vials of Menopur, Cetrotide antagonist, and Pregnyl trigger. Cycle 2 was a letrozole flare with a lower dose of FSH. These two protocols did not work well for me, so the next cycle I did a clomid flare with a ovidrel and Lupron trigger. Some people respond differently to the medication than others. He also recommended an ERA before transfer since I have only a few embryos.

I don't think I could have wished for a more competent and communicative physician. While I only got to see Dr. Kaser for the consultation (just the way the schedule worked out with him covering the SF clinic when I was down in Palo Alto, and vice versa), he was the one who usually answered my questions on Spruce (the messaging app), and the other providers I saw for the monitoring ultrasounds were lovely and had a good sense of what the plan was, so I would know when leaving the clinic what to expect for next steps. At my consultation, given the number of follicles on US, he explained clearly and kindly that there might be a reason to pursue workup for PCOS, which I had never thought/worried about before. The calm, accurate way he described why he thought I might have PCOS and what logical next steps would be to work it up with my PCP meant a lot, especially considering how surprised I was to get that news. Furthermore, the egg freezing cycle worked exceptionally well, with credit to him for planning and communicating such a successful strategy.

We used a standard antagonist protocol (Menopur + Gonal F for 6 days, then add Cetrotide on day 7), with a total of 12 days of stimulation. Menopur and Gonal F were both 150 to start, but Gonal F was upped to 225 when the Centrotide was added. Lupron trigger was used because I was high OHSS risk (lots of follicles, high AMH, very high estrodiol (~7000) by the end). Trigger (0.4 mL) was given 36 and 24 hours before retrieval, retrieval was on cycle day 15. Dr. Morin did the retrieval and got a whopping # of eggs (57!!), 49 of which were mature and frozen down.

All of the nurses and clinic staff were awesome. Friendly, kind, caring, organized, prompt. I'm sad I'm done in that I won't get to keep seeing them! Katie, who did my blood draws, was a ray of sunshine. And Ashley, the PACU nurse who gave me a call the day after, was funny, kind, and made retrieval day a breeze. Everyone was a superstar!

I did a TON of research before choosing RMA. I read every single review on here, Google, and Yelp of all the highest-rated clinics in the Bay Area before making my choice, which was informed by all the positive things that folks had to say about everyone who works there, from the physicians to the nurses to the staff, and I couldn't be happier with my choice, honestly. For a fast-moving process that can change quickly, having a clinic that EXCELS in communication (using Spruce, the messaging app) was a gift. Every time I reached out over Spruce, someone got back to me within minutes with the answer. Every person I interacted with was organized, competent, and kind. I never waited more than 10 minutes in the waiting room (usually less than 2 min) before being seen, and the facilities were top-of-the-line nice.
While the reviewing system here is I think designed to somewhat penalize physicians who don't manage to see their patients every time they come for a visit, I think RMA's model (where you're assigned a primary physician who will see you if they are at the same office as you are that day; but there's a chance you could also see another physician or an NP if they are at the other office) works extremely well. With Dr. Kaser available over Spruce seemingly 24/7, I didn't mind seeing different providers for the monitoring appointments, especially because everyone was clued in to the plan. In addition, the team approach made me feel like I had access to a bunch of talented and kind people, not just one! And RMA is small enough (and their providers good enough) that everyone knew me/called me by name from the first visit to the last, so I very much felt known by and valued by their team.
In addition, while I was willing to pay extra (none of this was covered by insurance, since it was elective and I have grad student insurance) for the best care, RMA ended up being *less* expensive than other top Bay Area clinics, so I feel like at the end of the day I got the best care AND at the best cost.

I paid $11k out of pocket - $6.4 to RMA (with 20% discount because I work in healthcare), $650 to anesthesia, and $4k in meds (using Alto pharmacy). My insurance didn't cover any of it, so this was expected. I had planned $12k with a cushion up to $15, so coming in at $11 actually was slightly better than expected!

Dan Kaser at RMA was recommended to me by a good friend, and my partner and I found him to be the exact right match for us. He was considerate, thorough, responsive, and made us feel like he not only knew what he was doing, he was invested in our outcome.
Our prognosis was good for our first round of IVF, however we experienced a set-back when I broke through the ovulation-delaying medication and all my eggs released before the retrieval. It was heartbreaking to have such an unexpected outcome at that early stage. It felt to us like Dan was on our team though, and he gave us hope. His support made it possible for us to try again with a different treatment regimen.
That second round was successful for us, and we ended up with three viable embryos-- one of which was successfully transferred. Our baby was born one week ago today!

How was your experience with Daniel Kaser at RMA of Northern California?
Dr. Kaser seemed so genuinely sweet when my husband and I met him over video visit for our initial consult. He was very caring and seemed so invested in us having the family that we wanted. After my initial ultrasound he was thrilled with my antral follicle count and thought we would be very successful with IVF. I definitely wish he spent more time telling us the truth about the potential steps in the IVF process (he oversimplified the process and never mentioned certain procedures like saline ultrasound, hysteroscopy, birth control, etc.) in our consult, as well as the potential outcomes after an embryo transfer in more realistic terms (he gave us a 70% success rate with our first embryo transfer which was not successful). I am a physician myself (not an Ob) so I expected him to be very thorough in his explanation of all the procedures. He was so generous with his time and sometimes didn't charge us for certain procedures, but once we were in the throws of IVF, I felt completely bewildered by our experience - it was not at all how he had presented IVF to us. During our first frozen embryo transfer, we did get a positive pregnancy test, but there was no yolk sac visible during the initial ultrasound. My HCG level had started really low, but it doubled quickly as expected. Instead of explaining why there was nothing on the initial scan, he ran out of the room looking panicked and said "you need blood work" over his shoulder before heading out the door. My husband and I totally freaked out and started sobbing when he left. We didn't sleep well until our follow up ultrasound 5 days later where thankfully we saw a yolk sac, but unfortunately this transfer ended in a blighted ovum (I had no idea what a blighted ovum was or that it was a potential outcome of a transfer). I had lots of issues with my insurance so I decided to do a D&C outside of RMA. My colleague also went to RMA where she was charged almost $5K for a D&C (she was not told this cost upfront) that was not covered with our insurance so I decided to get the procedure done in my hospital system. I had told Dr. Kaser that I needed a break because I was so devastated by this outcome. I did not feel properly prepared for the potential of a loss after a successful implantation. He never called me afterwards to check in on me after this devastating loss. I was so distressed that I might have put myself at risk for breast cancer (I have a strong family history of breast cancer and was rightly concerned about taking high potency hormone injections for IVF). When my husband called Dr. Kaser crying that I was inconsolable after the loss because of my fear of having increased my chances of breast cancer by doing IVF Dr. Kaser immediately became defensive and said "our drugs don't cause cancer!". I felt completely abandoned, invalidated and betrayed by Dr. Kaser. I could not see him as my provider afterwards even though we still had 3 remaining embryos at that time. I lost my respect for him and no longer felt safe in his care.
I think Dr. Kaser is a great doctor if everything goes smoothly in your case and there are no setbacks. As soon as we hit rough waters, he completely abandoned us and left us completely lost as to what to do next. I think he lacks maturity and will hopefully learn to be a more steadfast guide on this very scary, tumultuous and personal journey for couples.
What's one piece of advice would you give a prospective patient of Daniel Kaser at RMA of Northern California?
Make sure you ask detailed questions about the process. This process costs tens of thousands of dollars, has many risks, and is a LONG journey - you deserve his time. Do not feel rushed to make any decisions while you are in his presence (we did). Think things over and make sure you have all the information you need to make an informed decision. Hold Dr. Kaser accountable when mistakes are made.

Dr. Kaser has wonderful bedside manner. He always made you feel welcome in his presence. He also shared his personal IVF journey to make us feel comfortable as IVF is very emotional and overwhelming. The reason I have rated him neutrally is although he was very kind, so many mistakes were made to my care at RMA under Dr. Kaser's care. My hysteroscopy was cancelled an hour and a half before it was scheduled even though I was NPO for 10 hours because the anesthesiologist needed a negative covid test. No one had told me to get a covid test so of course my case needed to be cancelled. Both my husband and I had taken time off work to get this procedure done and we were very upset that this happened when it easily could have been avoided if we were told to get covid tested beforehand. I also had to go back to the clinic 3 times on the same day to get supplies for our IVF injections because of communication error. Every time we messaged on the Spruce app, some random person would respond, not just Dr. Kaser. We also had to wait over a week to get our PGT-A testing results back. Dr. Kaser is usually so prompt in his messaging we were surprised that he took almost 2 weeks to get our results back. My husband even called the lab directly and they said they had already given the results to RMA. When my husband called the front desk about our test results, the secretary told him that they didn't know where our results were (my husband thought they had lost our precious embryos). The whole experience was very traumatizing and could have been smoother with more effective communication. I definitely felt like the clinic dropped the ball several times in our care.

I overreacted to the medications so I underwent less medications than I was supposed to do. I believe I did 3 days of gonal F and menopur, then they added Ganirelix until I was ready to do the trigger shot (0.25 of 10,000 units HCG). I was told I was ready for retrieval on my first ultrasound which made me so overwhelmed because I was expecting to do the medications for 2 weeks.

Kristen is a really sweet, but she was so disorganized and became very defensive if a mistake was made because she failed to communicate something important to me. The day before my retrieval she called me panicked because I did not have an HIV test results (and something else sorry I forgot). We had gone over the lab results with our doctor before the retrieval and he said everything was in order. Instead of admitting that a mistake was made she said "Dr Kaser told you to get this done". He did not. We were given a list of lab tests to be done. I showed them the tests that I had done. They said they had reviewed them but clearly they did not. I had to wake up 2 hours early to go to the lab to get these last minute tests done before working as a physician myself in order to get the results to them prior to the retrieval. The retrieval was SO stressful and having this last minute test done added unnecessary stress. I also had so many nurses interact with me who often didn't know my protocol (or anything about me). When Kristen did call with HCG results she was always professional and sweet, but it was not consistently her who was calling me.

Pros:
- RMA can get you in super quickly and they will churn and burn you.
- They are open 7 days a week so a modified natural cycle is a possibility at this site.
- They use an app called Spruce to communicate with you. You can expect a response from someone on the team within 2 hours (sometimes as quick as 5 minutes!).
- The staff are very welcoming and kind.
Cons:
- Billing department is a NIGHTMARE. I was billed for things that were not done and overcharged for certain procedures even though we had signed a contract for a bundled package. We needed to get the central billing department involved and we still do not have our issues resolved. The billing is not transparent and the person who we are interacting with is not consistent in emailing you back and gives you contradictory information. SO FRUSTRATING.
- You feel very pressured to keep doing procedures. The price of these procedures is not given to you beforehand.
- The IVF process was oversimplified and the expected outcomes over promised. For something as intimate as having a family, I would expect these doctors to deliver more realistic odds of success and to have more compassion when things go wrong. Most other clinics have a follow up check in 6 weeks after a loss so you don't feel lost and abandoned by your clinic. My friends who have gone elsewhere thought the fact that our doctor never reached out to us was inhumane and just mean.
- You are definitely treated like a lab rat as a woman at RMA - poked and prodded SO many times with no impact on your outcome. Your protocol is not tailored to you at all. It is there way or the highway.
- I also think they promote procedures to get more money without always adding to your clinical outcome. I think it is wrong that they added certain blood tests without my permission (of course not covered by my insurance) just for monetary gain.
- Spruce is accessible by the entire clinical staff, not just the nurses and doctors. I remember receiving a message from one of the billing staff about my vaginal bleeding and I was horrified. I asked that my account remain private to just MD/RNs but that is not possible through the Spruce app. I felt like this was an invasion of my privacy and I wish someone had told me this before I started using it.
Describe your experience with your monitoring appointments at RMA of Northern California.
Monitoring appointments were so overwhelming. We would get an appointment slot but the doctor would be doing an egg retrieval or something else in an entirely different area so you were often asked if you wouldn't mind seeing the NP instead. I am not sure why they offered appointments when the doctor was not available. I found this very frustrating.

IUI was around $650; egg retrieval cost around $14K, drugs for IVF cost around $4.5K; transfer was supposed to cost $4200, but ended up being $4500-7000+ for each transfer not including medications.

We were not given our PGT-A test results during the time frame promised (7 days). When we called the lab who did our testing on a Wednesday, they said they had already sent it to RMA on the previous Monday. When my husband called RMA about this, the front desk staff said they didn't know where our test results were. My husband thought she meant they lost our embryos and we were both so upset. Thankfully Dr. Kaser called us the following evening to go over the results, but this was so upsetting and completely avoidable.
I was told I needed an emergent hysteroscopy after Dr. Kaser saw something on my saline ultrasound on a Friday. We were told to do the procedure on Tuesday when we both had work. We cancelled our work schedules, I was NPO since the night before and then 1.5 hours prior to my procedure, Dr. Kaser called to ask me if by chance I had a covid test done. I was not advised to do a covid test and didn't need one for my egg retrieval a couple weeks prior so I was surprised that he didn't tell me about this requirement. The anesthesiologist refused to see me and my scope was cancelled. My husband is a lawyer and had to cancel he had with a team in Japan and I am a physician so I had to cancel my clinic. I was so upset about this cancellation and had to wait until the following Wednesday to get the scope done.
I had specifically asked how painful the hysteroscopy was but Dr. Kaser said I shouldn't have any issues since I had no pain after the egg retrieval. I woke up with 8/10 pain while still foggy from the anesthesia and needed two boluses of fentanyl to stop sobbing from the pain. When I informed him that I experienced a lot more pain than I had expected he dismissed me and said "cervices are irritable". I was LLIVID with this response. We are taught in medical school to show compassion and to say you are sorry if things do not go well. He has never said he was sorry for causing me so much pain.
Before even starting IVF, Dr. Kaser ordered several routine examinations since I hadn't been menstruating including a SIS and discovered that I have scarring in my uterus and fallopian tubes. He performed a laparoscopy so successfully that I only needed that one surgery and then we continued with IVF and egg retrieval, etc. When we did my egg retrieval I had 57 eggs retrieved due to PCOS, and he had me use a lupron trigger which prevented me from getting OHSS. During my preparation for embryo transfer I did progesterone injections, took estrogen pills orally, and also a thyroid med.
I loved all the nurses, admin, and everybody working at both the Silicon Valley and SF clinic [RMA of Northern California]. They knew me as soon as I walked in and were very warm and sweet. We were given step-by-step instructions throughout the process by our primary nurse through Spruce, their messaging app. We were given exact dosages and dates to start and end each medication. During the monitoring stage of IVF, we were sent daily updates on bloodwork and any medication changes. We were sent videos of how to administer the meds and received an in-person demonstration. Whenever I had questions, they were immediately answered on Spruce. The egg retrieval was very smooth and I felt well cared-for by the nursing staff. They explained everything to me before the procedure and gave me aftercare instructions after I woke up. When I woke up, I had a heating pad on my stomach, a warm blanket, and a nurse offered me snacks and water. It was my best experience after a surgical procedure.

RMA has been my best experience with a medical care provider in my life. I was in constant communication with my doctor and primary nurse over their messaging app Spruce. All my questions were answered and my concerns addressed. Both the clinics in SF and Silicon Valley are great. Major procedures were done in SF, while monitoring appointments were done in Silicon Valley. Everyone I dealt with was competent, caring, and compassionate.

We had our first consult in early February, and at the end of July I'm 8 weeks pregnant. And this is considering a 4-week delay when SIP first started and RMA paused new treatment cycles. Also, I elected to immediately go through a 2nd egg retrieval cycle before results from Cycle 1 had been PGS tested, to be on the safe side that I'd have enough embryos frozen for transfer now and in the future. Fairly quick turn of events all in all! Very happy with RMA and Dr. Kaser.

I would do your homework before going in for your first consultation. He can talk fairly quickly so it's helpful if you already know the IVF lingo and acronyms. Also, since RMA does give you any written documents, take notes in your talks with him and take photos if he whiteboards the "math" of egg retrieval.

Luckily I have Progyny as a benefit through work, so almost all of it was paid for between them and regular insurance. Insurance paid for some things, like some of the bloodwork, genetic screening, Endometrin, etc. Progyny pays for the rest. I think I paid one or two small co-pays too.
With Progyny I was eligible for 8 "smart cycle segments" or the equivalent of 2 rounds of IVF, plus $15000 toward fertility meds. I ended up using 7 smart cycle segments. The smart cycles only refer to the medical procedures (i.e. egg retrieval, frozen transfer). I did have to pay out of pocket for some of the meds. But...thankfully with Carrot, my husband's fertility benefit, we can expense up to $10000 per year, and this includes out of pocket expenses. So we are VERY VERY lucky that we didn't have to pay for any of it.

There was one instance where a new prescription didn't get called in along with the others. I had missed that Dr. Kaser didn't call this prescription in, so it was my mistake as well. Luckily, it was a prescription I could get at Walgreens so I picked it up late night. All went fine in the end.

In terms of patient experience and bedside manner, Dr Kaser was great - I would highly recommend him on those points. This might not have been his fault but it was extremely disappointing for me to retrieve only half of my follicles. In my scan, I had around 20 but only half of them were mature - I'm not sure if medication would have changed the numbers. But now I've only frozen 6...I was given the protocol that's given to 85% of patients my age - follistim and Menopur at night, and 3 days into the treatment, ganirelix in the morning. I had 3 days of birth control, then 2-3 days off (b/c the pharmacy didn't deliver my fertility drugs on time). I injected Lupron as my trigger shot. I had 8 days of injections and retrieved on 10th day. In the scans, I can see about 19 follicles - but in the end, only 9 were mature. I know 2-3 were too big and the rest were too small. I wish I had retrieved about 12.

I would recommend them. All of them had good personalities - made you feel at ease, made me laugh.

Then I had the egg transfer and I got pregnant right away. He monitored me for the next 8 weeks to make sure that embryo was growing correctly through the beginning of my pregnancy.

I was on a tough road that led me to Dr. Kaser. He was extremely compassionate, patient, non-judgmental, driven, positive when I couldn’t be, and had such determined reassurance for me. I was a mess when I came to him and he took the time to really listen and learn about me and together we worked for me to get my baby, and I am typing this as my almost 5 month old is sleeping across from me in his crib. Dr. Kaser knew I wanted to be informed and involved every step of the way and he made certain I understood each step. Always responsive and caring.

We began with comprehensive bloodwork and screening for both my husband and me. It was already decided I’d do a freeze all retrieval using my husband’s sperm. I took CoQ10 600mg daily, baby aspirin, and other vitamins to begin with. My husband also had to take meds. I had follistim objectively twice a day (I think 225iu twice a day), as well as lo dose hcg, microdose lupron, and a trigger shot. This was my fifth Retrieval (the precious ones were at another clinic). Due to my precious retrieval’s, Dr Kaser adjusted the timing to do it a little earlier since it seemed like a previous one was a little late. We did CCS testing and we had 4 genetically normal embryos - the most at one time ever. Next he texted my lining and discovered I’d need to do a transfer one day earlier than standard, and that served as a scratch to get the embryo to stick. For the frozen embryo transfer cycle, I used endometrin 3 times a day, methotrexate (I believe) for about a week, Valium day of transfer (to relax uterus) and laser acupuncture, and was successful the first transfer! I preferred not to know the sex until I got a positive result. It was amazing to get the call that it was positive! When I next met with Dr. Kaser for my scan we were both beyond thrilled. I then asked for the sex of the baby and he told me.

Hillary was on top of my care. I probably spoke more with her than anyone else. My communication with her was primarily email which I preferred since I feel it’s faster and I could always make sure my questions were detailed to my liking. I would ask her anything and everything and she would always respond with kindness and logic - which I always appreciated. She was also detrimental to my success.

RMANJ is big. And they know how to run sufficiently. They opened early for monitoring and had a streamlined process to make sure everyone is seen in a timely manner. I never felt rushed with my doctors and always could ask all my questions. Due to my previous experiences I know to speak up for myself and I did, and they had no problem with that. The facilities are beautiful and clean, modern. (I’ve been to Princeton, Marlton, and Basking Ridge). Top of the line technology. Also, the transfer room was the best experience ever, having my husband with me, having the embryo brought in inside an incubator, seeing it all on a big screen - it was beautiful. They are huge, they do not treat you like a number when they are treating you. So many people go to them for a reason, they know what they are doing. When they first see you the have to start with some kind of care, their golden standard, and adjust and be flexible accordingly to find what works best with you. It works. For billing and meds - stay on top of it. Know your insurance, know your pharmacy, know your scripts. Follow up daily and twice daily if need be. Some people don’t have to work to get pregnant, some, like me, do. You need to be your number one champion and let RMA help you along the way.

I at times felt rushed by seeing other women in the waiting room or elevator, but once signed in would always be taken within 10 minutes for blood work, and then within 10 minutes to the room (usually faster). The phlebotomists were friendly and did a great job finding a vein (mine are always so hard to find), the nursing staff was great, and even if I didn’t see my doctor, the one on duty always took time to go over and explain my ultrasound, the protocol, and answer all my questions.

I had to pay a $25 copay a few times before I met my out of pocket limit with insurance. Also the insurance I had covered unlimited infertility treatments and it covered my medicine 100% after meeting my out of pocket. I did have to pay $2500 for embryo testing, $600 for the ERA, and $200 for laser acupuncture day of transfer. Insurance also covered freezing and storage of semen and embryos.

For the first I took Clomid and overreacted and had to skip the cycle - I was very upset but Dr. Kaser convinced me to stay positive and continue and was prepared with a plan for the next cycle. After that we tried several "natural" cycles, which also failed. Then Dr. Kaser recommended trying injectable medications...convinced me that I would have much better chances with IVF. I completely trusted him and went forward with the egg retrieval cycle which was successful and frozen embryo transfer - also successful!

I started without knowing I had any infertility issues with high prospects that I would become pregnant quickly. I ended up having 9 unsuccessful IUIs with and without medication. For the first I took Clomid and overreacted and had to skip the cycle - I was very upset but Dr. Kaser convinced me to stay positive and continue and was prepared with a plan for the next cycle. Once I did have a positive pregnancy test, but it turned out to be a chemical pregnancy. It was very difficult to get through, but again Dr. Kaser encouraged me not to give up. After a few more unsuccessful IUIs (my insurance did not cover IVF which is why I tried to avoid it), Dr Kaser convinced me that I would have much better chances with IVF. I completely trusted him and went forward with the egg retrieval cycle which was successful and frozen embryo transfer - also successful! I am now 38 weeks pregnant. I feel even though it took well over a year for me to get pregnant, Dr. Kaser did whatever he could to make it happen while taking my concerns into account. I know if I went with IVF sooner it wouldn't have taken so long but it was a process I had to go through before understanding that was the best (maybe only) option. Dr. Kaser was patient and supportive.

Overall the clinic is professional but caring and a comfortable place for patients. The location was not convenient considering how often I had to visit, but it was worth the extra travel. They could not perform certain procedures at the clinic and I had to travel quite a distance to another facility on those occasions. There will be a new office opening soon so I don't think this will be an issue anymore.
