Dr. Levine is a very caring funny guy. He shows compassion to his patients. He listens and answers. Clinically though I am not that happy with him. He diagnosed me with PCOS and placed me on metformin. He never told me to supplement with vitamin B12. He ordered an elaborate blood clotting panel after 2 failed embryo transfers and a miscarriage, but never spoke to me about the results. After I looked up my own results on my portal with labcorp it was found that there was an abnormality with the MTHFR.
Weakness [of Hudson Valley Fertility] is that the lack of communication adds to the aggravation and stress already associated with infertility. For the amount of money that you spend, you at least should get a call back from the clinical team. ... The biggest problem with this office is that you aren’t assigned to one nurse. No one in the clinical dept answers the phone, and when you do leave a message they don’t call back. They are nice when you come in for monitoring.

I brought it up and he said that he doesn’t really look at that (If he didn’t care I don’t know why it was ordered in the first place). After each transfer he wished us luck, but we had no luck with Dr. Levine which ultimately made us change fertility clinics.
What's one piece of advice would you give a prospective patient of Daniel Levine at Hudson Valley Fertility?
Make sure you are your own advocate. Look up your own lab work because he will not go over it with you.
During treatment, were you treated like a number or a human with Daniel Levine at Hudson Valley Fertility?
I felt like nobody was listening. I had called the office multiple times to clarify symptoms and medications and no one responded back. I called the day before my retrieval to tell the doctor that I could not keep anything down and couldn’t stop vomiting and I never received a call back. Every time I went in for monitoring they asked about my meds and always questioned why I wasn’t on certain ones. I was put on metformin and never told to supplement vitamin b12. After I requested bloodwork from my primary doctor, it was found that my b12 and vitamin D was low.
Describe the protocols Daniel Levine used in your cycles at Hudson Valley Fertility and their degree of success.
First Dr Levine gave me a script for clomid. We did 4 unmonitored cycles (diagnosis PCOS) all failed. Next we tried IUI. My first IUI worked with letrazole and an ovidrel trigger shot. We were discharged from the clinic when I miscarried at 11 weeks. After that we came back to Hudson valley Fertility and tried another IUI which my follicles did not get to the appropriate size so it was canceled. We did 2 more IUI’s with letrazole and ovidrel that both failed. Next IVF was recommended, and per Dr. Levine I would “definitely” get pregnant. I did my retrieval using follistim 175u, low dose hch 5u, and ganirelix. I stimmed for 10 days. My retrieval went well, 16 eggs retrieved, 10 mature, 10 icsi, 4 sent for Pgs. We did 2 frozen transfers using estrace vaginally, progesterone vaginally and progesterone in oil injections. Both transfers failed after being told that this would “definitely” work for us. That was when we transferred to CNY.

After I did all my pre lab work and tests I called the office to let them know that I was ready for my follow up and to start treatment. The clerk on the phone told me that I still needed my day 3 labs. I told her I did them weeks ago at their office, she said I did not. I am a nurse and ALWAYS ask what labs are being drawn and I know I had them done. They called me back and found them. I brought in a signed consent for the ivf transfer that they requested that we did before the appt. they lost that and the day of they were scrambling to get new papers in order. The week before my transfer I called the clinical dept to let them know that my pharmacy was out of Valium, and that I wanted the script sent somewhere else. No one returned my call. I called again and was told that the dr was not in and they would have him send the prescription to Walmart the morning of my transfer. So the morning of my transfer I had to sit and wait in Walmart for my prescription. After my last transfer I asked the nurse for another script for progesterone. She asked if I needed my estrogen patches refilled as well. I told her I am not on patches. The nurse said the Dr’s note in the chart says I was to be on patches... which I never was. So I never got the meds my dr intended me to get.

Success w/ Doc Too early to know
I felt like he [Dr. Levine] was so dead set on diagnosing me with PCOS that he ignored my past patient history of Endometriosis, which I've been treated for for years by multiple doctors...He ignored or forgot what symptoms I described, or he'd fail to take a thorough history. I had to correct him multiple times on very key facts like the regularity of my periods or my frequency of ovulation...He rambles on and mumbles, jumping from topic to topic...
the whole office [Hudson Valley Fertility] is pretty disorganized. Very hard to get nursing staff on the phone or to call you back...They called my first trigger shot into a semi-local specialty pharmacy before we'd ever decided if we wanted to start IUIs and then seemed very impatient and annoyed when I called to find out why this pharmacy was calling me. Then when it came time to do that first IUI cycle a few months later, no one followed-up or asked if I'd received a call about the trigger shot or confirmed I'd received it...
How was your experience with Daniel Levine at Hudson Valley Fertility?
Dr. Levine is pretty spastic in his communication. He rambles on and mumbles, jumping from topic to topic. He seems to really want to highlight the positive, which I appreciate, but by the fourth time he unnecessarily congratulated my husband on his "good sperm" within one visit (we already knew it was not male factor infertility prior to seeing him) we began to wonder if he was just at a loss for other things to say. We would ask him questions and he'd either half answer or not at all. I have begun doing IUIs and prior to my first cycle he didn't give us any instructions, nothing about when to have sex or abstain, what to expect, etc. I felt like he was so dead set on diagnosing me with PCOS that he ignored my past patient history of Endometriosis, which I've been treated for for years by multiple doctors. Are my severe symptoms and past responses to treatment all in my head? He ignored or forgot what symptoms I described, or he'd fail to take a thorough history. I had to correct him multiple times on very key facts like the regularity of my periods or my frequency of ovulation. I only have one of three markers of PCOS and yet he insists I have a mild case. I have been seeing specialists for years and no one has ever diagnosed me with PCOS. I got a second opinion from another REI who disagreed entirely that I can be diagnosed on one marker, tested my androgen levels, and confirmed that I do not have PCOS. When it came time to do a polypectomy I had to specifically request that he also do a laparoscopy to look for signs of Endometriosis. It seemed he'd forgotten that part of my history or didn't think it could be part of my infertility.
What's one piece of advice would you give a prospective patient of Daniel Levine at Hudson Valley Fertility?
Overall, I strongly recommend finding another doctor. He seems like a nice guy, but he's disorganized and a poor communicator and we leave his office with more questions than answers. If he's your best option (there aren't a lot of REIs near him) then really advocate for yourself. Be super clear with him about your symptoms- I made the mistake of asking clarifying questions, like "what is excessive facial hair?" which he took to mean I had excessive facial hair and diagnosed me with PCOS without ever asking me to pull down my mask. Take very careful notes, ask for step-by-step details of each procedures, ask what you need to be doing or not doing. I feel like we've wasted 10 months working with him and found out very little and made very little progress.
During treatment, were you treated like a number or a human with Daniel Levine at Hudson Valley Fertility?
The staff gets to know you and will remember your name. However, Dr. Levine seemed almost insistent on diagnosing me with PCOS (despite only testing for one of three markers for it), misremembered many symptoms, even after I corrected him, and seemed to lose track of where I was in my treatment. So either I'm just a number or he's terrible at taking notes.
Describe the protocols Daniel Levine used in your cycles at Hudson Valley Fertility and their degree of success.
First Dr. Levine did a very basic work-up on top of the testing I'd had done with my previous doctor. He then did a polypectomy and suggested we begin IUIs. We chose to try timed intercourse for a couple of months following the surgery and have done one round of IUIs. I did a baseline ultrasound on Day 2 and began oral Letrozole on Day 5 for 5 days. I did a second ultrasound on Day 9 and did an Ovidrel trigger shot on Day 9. Insemination was on Day 11 with his NP, Casey, who I like a lot better than Dr. Levine. Was told the first IUI doesn't typically work, wasn't told why. It did not. Did the baseline and follow-up ultrasound for a second cycle, but was told not to trigger on Day 11 as my follicle wasn't large enough. Went back on Day 13 for another look and was told I'd already ovulated and to save my trigger shot for next cycle but to have lots of sex over the next 3 days (which doesn't make a ton of sense given I'd already ovulated).
Describe your experience with the nursing staff at Hudson Valley Fertility.
Casey, the Nurse Practitioner, is great. She is a much better communicator than Dr. Levine. She's patient and answers all of my questions and seems to be very considerate of me. I much prefer to see her than Dr. Levine. The rest of the nursing staff is kind, perhaps a big disorganized, but no major mishaps. I don't find anyone on the nursing staff answers the phone or returns messages well at all.
Describe your experience with Hudson Valley Fertility.
Poor communication at every turn. It's easy to book an appointment, but not much else is easy. I feel like I do their jobs for them a lot. The nursing staff is all very sweet and personable, but I think the whole office is pretty disorganized. Very hard to get nursing staff on the phone or to call you back.
Describe the costs associated with your care under Daniel Levine at Hudson Valley Fertility.
IUI $600 per round plus cost of trigger shot $30 (with insurance)

Describe the specific things that went wrong at Hudson Valley Fertility.
Before we came in the first time, we filled out all the initial paperwork emailed by the patient coordinator, only to be told days later that we needed to answer the EXACT SAME questions in their online portal- which clearly wasn't well set-up, it would ask the same questions over and over. We scanned in our IDs like the system required and then the staff made us scan them in-person again as if we had never bothered to upload them. They switched systems a month later and seemed to lose our medical records from previous testing at a urologist and gynecologist, claiming we never sent it, even though I'd called and confirmed before our first visit that they had received it all. They called my first trigger shot into a semi-local specialty pharmacy before we'd ever decided if we wanted to start IUIs and then seemed very impatient and annoyed when I called to find out why this pharmacy was calling me. Then when it came time to do that first IUI cycle a few months later, no one followed-up or asked if I'd received a call about the trigger shot or confirmed I'd received it. The day I was supposed to take it I ended up having to drive an hour away to pick it up because it was too late to have it shipped. I don't know what I don't know- why should it fall to me to ask well in advance if I need to call the pharmacy about my trigger shot? Second cycle, they switched the prescription to a mail-order pharmacy without telling me. When I ask what I need to do to get my trigger shot they told me about the switch and said "oh, no one has called you??" They claimed they made the switch so my insurance would cover the shot... no one had asked if the first pharmacy took my insurance (luckily it did). Then I had to do the leg work of tracking down the second trigger shot with the second pharmacy which couldn't guarantee delivery. It's just been a mess, I feel like I think of 3 new questions every time I leave and it's very difficult to get someone on the phone to answer them.

How was your experience with Daniel Levine at Hudson Valley Fertility?
Dr Levine is friendly but he really seems unknowledgable and this is his primary practice. You have to do your own research and make suggestions about the type of care that you would receive. He has a way of giving you high hopes. And then the experience that you get is nothing like what he explained was going to happen. Overall I'm only satisfied that he did perform IUI on me and I appreciate the time that he put in but I won't let him do anything else.
What's one piece of advice would you give a prospective patient of Daniel Levine at Hudson Valley Fertility?
Go find another doctor at another practice do not go to Hudson Valley for fertility

Describe the protocols Daniel Levine used in your cycles at Hudson Valley Fertility and their degree of success.
My IUI Cycles went well although they did not result in any pregnancy I was still happy with the process however I am not satisfied with another procedure that he needed to perform which he did very sloppily and left scars. He paid no attention to the healing process he paid no attention to my complaints he should have referred me to another provider because he did not know what he was doing when he performed my procedure and it is very apparent that he was clueless in this aspect.
Describe your experience with the nursing staff at Hudson Valley Fertility.
I like the nursing staff that works in his office however I have no idea what their names are. They've never introduced themselves I don't know what the call them when I come in I simply just say hi and good morning. One of the problems I do have is that they always have me waiting either in the for significant amount of time, or in a room for significant amount of time. And I can honestly tell you that there really never appears to be any other patients in the office when I'm there. Very rarely do I see three or more patients in the office at any given time
Describe your experience with Hudson Valley Fertility.
I can't say that they have any strengths. Communication is best with them by email. They don't tell you what department they're in so you don't know what extension you're supposed to dial when you call. Casey the nurse practitioner is very very nice and knowledgeable and very compassionate she would be the better option if you need to see a provider at this office.
Describe the costs associated with your care under Daniel Levine at Hudson Valley Fertility.
I'm very grateful that my insurance has been covering the majority of my visits with the fertility doctor I simply just have to pay co-pays when I go in. And sometimes there is no copay at all.

I liked Dr. Levine when we first started, but his lack of empathy and bedside manner became cause for concern after our first IVF resulted in a missed miscarriage in which he just left the room during our ultrasound. After that he just pushed me into a d&c, and kept telling us PGT would be our answer. After 2 failed transfers later he still had no answers for us so we decided to move care. He maxed out my fertility benefits without ever individualizing my care or reviewing my records. I came in having thyroid issues but not once during the 17 months I was in his care did he question or run thyroid labs...
HVF [Hudson Valley Fertility] has major issues when it comes to billing, we were miss-billed over 5 separate times during the 17 months I was in Dr. Levine's care. Some were smaller charges I'd notice on my credit card for storage after we had used the sperm we had in storage (that took about 2 months to get them to stop charging). And some were because someone didn't put in that I'd already been charged for things so I would get more bills and had to pull receipts to get them to stop harassing me...

I came in having thyroid issues but not once during the 17 months I was in his care did he question or run thyroid labs. I feel the same now looking back at my IUIs with him and wonder if he ever looked at timing or if he just pushed more and more meds on me that my body didn't need. He pushes PGT testing - but after all my embryos were tested and still not implanting he refused to look at any more tests or getting any further answers. It was not a good experience and when I made an effort to leave the practice he was not gracious at all in our final visit which just confirmed my feelings about moving my care.
What's one piece of advice would you give a prospective patient of Daniel Levine at Hudson Valley Fertility?
Don't. Honestly, there are other doctors. But if you're set on it, be very diligent in your questions, get your records in writing. Always question charges that seem out of the norm (their billing practices are a mess!). Push for the care you want and need, do not expect him to review your records and offer you suggestions - you need to do the research.
During treatment, were you treated like a number or a human with Daniel Levine at Hudson Valley Fertility?
While I believe that Dr. Levine tries, it became increasingly apparent as treatment continued and we became more of a 'difficult' case - there was no individualized care. He would say things like 'well I have been trying this with my patients' but my questions were always 'how does that translate to me or my situation?' Being LGBT and having no infertility diagnosis or reasons for needing IVF outside of failed iuis, I felt lumped into his PCOS/Endo patients, or patients with infertility. He never treated us as if we were an individual case he was trying to solve for.
Describe the protocols Daniel Levine used in your cycles at Hudson Valley Fertility and their degree of success.
During my 6 IUIs I went from - Letrozole (#1), Clomid (#2), Clomid + Follistim (#3), Gonal F (#4 & 5) to natural (#6). All 6 were negative, but the rational was trying to find something that increased the number of follicles. Ultimately from Gonal I did end up getting one IUI with 3-4 follicles. Still did not end up pregnant on any IUI. For my first egg retrieval, I was on Menopur 375 IU daily, Micro-dose Lupron 20mcg twice daily. Used HCG 10,000 trigger. I got 10 eggs retrieved. For my second egg retrieval, I was on Menopur 300 IU daily, Gonal F 150 IU daily, Micro-dose Lupron 20mcg twice daily. Used HCG 10,000 trigger. I had 21 eggs retrieved. We added the Gonal F after my first retrieval didn't result in as many eggs/embryos.
Describe your experience with the nursing staff at Hudson Valley Fertility.
The nurses have their strengths and weaknesses I guess. On one hand they can be quite nice, and since it is a small practice you'll get to know them well since they are the only ones who email or call you back. On more than one occasion I had to continue calling them to phone in my prescriptions to the right pharmacy. I had them call in to the wrong resulting in charges I ended up having to cover because they called in the same prescription twice which was filled by both pharmacies and if you're like my insurance they require specific pharmacies for things which the nurses could never remember or seem to understand. Some are much better at responding and being compassionate than others - depends on who you get on any given day I guess.

Some were smaller charges I'd notice on my credit card for storage after we had used the sperm we had in storage (that took about 2 months to get them to stop charging). And some were because someone didn't put in that I'd already been charged for things so I would get more bills and had to pull receipts to get them to stop harassing me. On the visit to confirm our miscarriage I actually had the woman from billing come ask me about an invoice from my transfer (which we were now confirming I miscarried) so their coordination and bedside manner are horrible. It was not the time.
Describe the costs associated with your care under Daniel Levine at Hudson Valley Fertility.
I was using insurance but before my insurance kicked in IUIs were $700/just treatment. FET are $3500/without insurance just for the transfer.
Describe Daniel Levine's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Hudson Valley Fertility.
Dr. Levine does have a strong feeling toward transferring one embryo at a time. He pushes hard for PGT testing and hence wants to transfer one PGT at a time. I personally had 2 fresh transferred on my first transfer, so he will transfer two. I also know of someone who he transferred two PGT embryos for, but we never personally did this.

When I started IVF treatment with Levin I was extremely hopeful...My embryos transfer was not successful I was devastated...I felt cheated out of having an opportunity. Dr. Levin was so cold and no compassion what so ever...I was treated exactly like a number. Once my insurance was exhausted the kindness stopped.
The [Hudson Valley Fertility] clinic is clean and the staff needs to have more compassion, and be truthful. I would recommend any patients to get a second opinion before any decision making there are other Doctors that really want to help people become parents. At this clinic that’s not their goal is just to about money.
How was your experience with Daniel Levine at Hudson Valley Fertility?
When I started IVF treatment with Levin I was extremely hopeful. My insurance covered the treatment but I had a three time life coverage for the IVF treatment, two of my coverage was use in one embryo transfer, Dr. Levin had me stop all medication and waited after the 60 days to do the embryos transfer. My embryos transfer was not successful I was devastated I thought that I still had two more chances to conceive, I was wrong when I went for the second embryos transfer my insurance notified me that it was considered as a third and last transfer coverage. I felt cheated out of having an opportunity. Dr. Levin was so cold and no compassion what so ever. They used me

Describe your experience with Hudson Valley Fertility.
The clinic is clean and the staff needs to have more compassion, and be truthful. I would recommend any patients to get a second opinion before any decision making there are other Doctors that really want to help people become parents. At this clinic that’s not their goal is just to about money.

My insurance coverage has a 3 time life policy. The clinic waited to do the my first embryos transfer after the 60 days which I was extremely confused and I asked why they were waiting for so long, I was told because they wanted the medication to flush out my system, I was extremely confused confusing, I to start the same medication all over again.

Dr. Levine is the only doctor in the practice. He speaks very quickly and it is clear that he speaks about IVF all day long and is not interested in speaking in layman's terms and relating to a nervous new patient who has never gone through IVF before. He races around the clinic during monitoring and barely stays in the room post-ultrasound to answer questions. I always felt very rushed and though it seemed everything was fine, he was not able to make me feel important or explain things in a way I felt comfortable with...Our fert rate from IVF cycle was low and he could provide very little explanation which was frustrating and he just blamed the embryologist. Lastly, due to COVID most visits were telemedicine and I was appalled to receive bills following where we were charged for EVERY phone conversation. Now that I am in a different clinic I see how this is not the standard procedure and we feel super taken advantage of.

How was your experience with Daniel Levine at Hudson Valley Fertility?
Dr. Levine will perform IVF safely and with standard protocols that are comparable with other clinics. He is a good doctor and I did not feel that his care was inadequate. However, he is very abrupt and has a bit of a "can't be bothered" attitude during conversations. I constantly felt rushed by him. Prior to a hysteroscopy surgery pre-IVF, I read some pretty scary interviews about him on the Internet and called the IVF nurse to raise my concerns. Dr. Levine called me to "address" the issue however he never once acknowledged it and I felt he should have done so to ease my concerns and address negative reviews out on the web about him. Our fert rate from IVF cycle was low and he could provide very little explanation which was frustrating and he just blamed the embryologist. Lastly, due to COVID most visits were telemedicine and I was appalled to receive bills following where we were charged for EVERY phone conversation. What's one piece of advice would you give a prospective patient of Daniel Levine at Hudson Valley Fertility?
Dr. Levine is the only RE locally in the Hudson Valley. He is good but not great. If you want to feel like you are getting great, thorough care and that someone is looking out for you, go elsewhere.

Dr. Levine was friendly, warm, welcoming, funny, encouraging, and had a positive attitude. He put results of visits in a positive light (for example, when I had a couple of good follicles he made me feel better by mentioning that is exactly what they want to see). He also would wave his hands over my stomach after an IUI to transfer positive energy and lighten the mood.... Dr. Levine helped to monitor me closer to IUI time. On some cycles he helped to monitor my follicle size and determined when I needed to trigger. He also completed some IUI. In the beginning of one cycle a cyst/cysts were noticed.
I absolutely LOVE the nursing staff at Hudson Valley Fertility. They were so kind, caring, and compassionate. When I would bring my son with me to visits they were so good with him. They were engaging and enjoyed having him in the office. They were reassuring and always eased my worries.... At times, I had to wait a decent amount of time either in the waiting room or once in the patient room. But, this can happen at any doctor's office and I absolutely understand and appreciate everything that the staff does.

I believe that Dr. Levine is genuine but I do think it's difficult as a patient to understand him. It's the way he delivers information that is hard to understand. I think he needs to slow down and speak in more generic terms that people hearing not so great news would better be able to understand...We froze my eggs in January. Put me on prenatals and BC in August and started my fertility meds for a transfer in early September. They did rapid PGS testing the night before my retrieval and when I got to the office for my retrieval they told me all my embryos were abnormal and we couldn't proceed.
The staff [at Hudson Valley Fertility] is so friendly...The inability to get someone on the phone in the clinical department if you really needed to talk to someone. And billing can of course be a bit sporadic. They do give you a really thorough breakdown of your entire cycle and print it for you which is great...They did rapid PGS testing the night before my retrieval and when I got to the office for my retrieval they told me all my embryos were abnormal and we couldn't proceed.

To make sure you have him slow down and write everything down. He doesn't explain things the way someone without a medical degree would understand. He also speaks very quickly.

Hudson Valley Fertility is such a small clinic that the entire staff knows your name the second you walk in the door. They talk to you and hear you and it's appreciated.

The strengths of the clinic are the ease of getting an appointment, how quickly they take you in, I never waited more than 10 minutes for any appointment. The staff is so friendly. The weaknesses are the lack of understanding Dr. Levine's process of how he wants to handle the cycle in detail. The inability to get someone on the phone in the clinical department if you really needed to talk to someone. And billing can of course be a bit sporadic. They do give you a really thorough breakdown of your entire cycle and print it for you which is great.
Describe the costs associated with your care under Daniel Levine at Hudson Valley Fertility.
13,500 for one full IVF cycle. $50 monthly for sperm storage. PGS testing $350 an embryo.

Dr. Levine is very compassionate, explanatory, friendly, and has a great great bedside manner. He makes himself available for many of the appointments personally. He doesn't sugar coat the odds. He does ample testing to cover all bases. He, personally, has performed most of my IUI's (a few were done by his P.A.), all of my saline sonograms, hysteroscopies/D&C's, ERA, and my IVF retrieval/transfer. If you don't want to be passed between many different doctors, this is the place. He adjusts treatments frequently based on lab work and ultrasounds. His lab had phenomenal results for us using ICSI and growing out my poor aging eggs to blastocyst stage for PGS testing.
One huge strength is their [Hudson Valley Fertility] working with many insurance companies. In addition, they often were able to get many things covered that my insurance claimed were not covered. Also, if they happen to have any sample meds or donated meds around, they'll give you what they can. So I believe some of the weaknesses would be the phone system and return phone calls. I believe a human answering the phone initially for every call would be better than an answering machine routing calls. I also believe there should be an option to speak to a nurse or be put on hold for a nurse (instead of message and call backs eventually). I also think they could use improvement for medication instructions for injections, med storage, and instructions for time of day for meds. Being sent to a generic web site for instructions that aren't the exact meds or exact needle delivery system wasn't very helpful. A good printed instruction sheet and FAQ for meds frequently used would have been better. I googled things far too often to figure out what I was doing. Even with these few negatives, the positives of Dr Levine's office FAR outweigh the negatives.

If I had to pick a few downsides, I'd say it's that they barely have a donor egg/embryo program and the little that is available seems much more expensive than other places.
What's one piece of advice would you give a prospective patient of Daniel Levine at Hudson Valley Fertility?
Ask lots of questions. Take notes after each appointment. Speak up if you're uncomfortable or don't understand any procedure.

Most of the staff were extremely friendly and knew us by name. Very supportive during breaking bad news.
Describe the protocols Daniel Levine used in your cycles at Hudson Valley Fertility and their degree of success.
I have PCOS so I always had a decent amount of follicles/eggs during IUI cycles and IVF. I am also AMA. Dr Levine was always careful to balance meds and not overstimulate. Our very first IUI cycle (many years ago) was cancelled due to fast response to meds and far too many follicles to procede with IUI. All of my meds for IUI were either Follistim alone (150-225iu per day), or a combination of either Letrozole 5mg (or Clomid 100mg) and Follistim together. For IUI we occasionally used 250mcg Ganarellix for a few days to hold off ovulation and let follicles catch up with others. For every IUI we used 250mg Ovidrel as the trigger. Every cycle we used progesterone suppositories during the luteal phase. For our one IVF cycle, we used 225iu of monopur for 11 days and 225iu of Follistim/Gonal F concurrently for 11 days. For 5 of the 11 days, we also used Ganarellix 250iu. We used a half dose trigger of 5,000iu of Pregnyl to lessen the risk of OHSS since my estrogen was high (over 5,000) and I had approx 27 follicles. We used cabergoline after retrieval to avoid OHSS also. On day 14 we had 22 eggs retrieved. 15 were mature and 2 more were able to be matured in the lab for a total of 17. All 17 fertilized with ICSI. At day 3 we still had 17 embryos. (9 grade 1; 5 grade 2; and 3 grade 3). By day 5 we had 3 AA blastocysts that were biopsied for PGS and then frozen. At day 6 we had 3 more blastocysts (grade BB) that were biopsied for PGS and then frozen. The other 11 embryos were either low quality C blasts or deteriorated and stop growing. We had 1 PGS normal blast and 1 inconclusive blast from the testing. The other 4 had monosomies, trisomies, or multiple chromosomal errors. 7 months later we transferred both blastocysts. The 7 month delay was from me needing a few procedures on my uterus and also having 2 transfers cancelled due to lining issues and unexpected bleeding. For lining prep and during and after transfer we ultimately used BCP, Lupron, Estrace orally, Estrace vaginally, estrogen patches, delestrogen injections, progesterone vaginally, progesterone in oil injections, Lovenox, Medrol, and doxycycline. We also used Valium the day of transfer. 1 of our 2 embryos stuck but stopped growing near 5 to 6 weeks. A heavy bleed following shortly after, but I ultimately ended up needing a hysteroscopy/D&C near 10 weeks for a retained gestational sack.

The nursing staff were all extremely friendly and compassionate. Their phlebotomist was excellent and gets blood on the first poke 99% of the time.

Describe the costs associated with your care under Daniel Levine at Hudson Valley Fertility.
Costs were broken up into a million different costs also billed thru insurance. So it's difficult to say what total costs would have been for cash pay. Many options were ala cart and could have been chosen or not depending on your preferences and needs.
Describe Daniel Levine's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Hudson Valley Fertility.
Dr Levine runs a very tight ethical ship however he uses common sense and weighs the odds and uses his experience to make sounds decisions with number of embryos transferred. Before we knew how many embryos I would get (remember I'm advanced maternal age) he was leaning towards transferring a couple (2-4) day 3 embryos. When we had 17 day 3 embryos we decided to grow out to day 5 and go along with my husband and my desire to PGS test. When we had 1 PGS normal and 1 inconclusive blastocyst, Dr Levine was supportive of our choice to transfer 2 back in.

Dr.levine is extremely knowledgeable. He gave me hope when another high rating clinic turned me away because of DOR. He literally said to me “I will never deny a woman her right to at least try”. That statement made all the difference....Dr.levine and some staff [Hudson Valley] made me feel comfortable. Others did make me feel like a number. During blood monitoring I did feel like it was a bit like a factory conveyer belt with the same women in the waiting room as we all took turns getting our blood drawn and ultrasounds.
Communication with office staff [Hudson Valley] was less than stellar. It was hard to ever get through to someone or have a call back when leaving a message. Also they had the teeeible habit of waiting to the last minute to call with questions literally hours before a procedure. On one occasion. I almost missed my retrieval because Simone dropped the ball with insurance info and my retrieval was not approved.... The nurses were the ones who made me feel at ease through the whole process

Out of pocket expenses without insurance coverage I would say we spent about 4-5thousand dollars. Paying for a premium coverage to include fertility coverage for out insurance to cover process I would say it was about 20-25k

Again office management was less than stellar through the process. The communication was the biggest problem. They had the terrible habit of calling with questions minutes before their office closed and if we couldn’t answer they would become short with us- a few times my husband needed to remind them that they were the ones calling us at the last minute so their frustration with us was uncalled for.

He [Dr. Daniel Levine] suspected endometriosis and I had a hysterosalpingogram and a saline infused sonogram. However, I was never diagnosed with endo with him and he never performed or suggested a laparoscopy. His next step with me was going to be Clomid...He was kind, knowledgeable, to the point... The basic work up with Dr Levine was fine. Nothing stellar or exceptional - but I felt fine and taken care of to a comfortable level.
[Nurses at Hudson Valley Fertility were] Kind. Helpful. Compassionate...HVF worked with us only at the beginning stages of our infertility, but I saw Dr Levine more than I saw my doctor here in Seattle at the clinic that I had 4 IUIs. He was kind, knowledgeable, to the point...I was never diagnosed with endo with him and he never performed or suggested a laparoscopy. His next step with me was going to be Clomid
How was your experience with Daniel Levine at Hudson Valley Fertility?
I saw dr Levine during the very first part of our 5.5 year journey with infertility and then we moved out of state. I saw Dr. Levine over the course of about 2 years (maybe 1.5) and we were just in the beginning stages of figuring things out. My husband and I weren’t ready to try IUI (we were still in disbelieve that WE couldn’t get pregnant) and so Dr Levine ordered/ performed some of the basic diagnostic tests. Through him, we had bloodwork done, I had a hysterosalpingogram, and I had a saline infused sonogram. Everything came back normal and our next step was going to be to try Clomid on it’s own for a few months. However, we moved to Seattle and didn’t even get that far with Dr Levine. The basic work up with Dr Levine was fine. Nothing stellar or exceptional - but I felt fine and taken care of to a comfortable level.

Ask questions about everything? Why this drug and not that one? Why do I need this? How will it help with conceiving a baby or figuring out the problem.
Describe the protocols Daniel Levine used in your cycles at Hudson Valley Fertility and their degree of success.
I only did the beginning work with him. He suspected endometriosis and I had a hysterosalpingogram and a saline infused sonogram. However, I was never diagnosed with endo with him and he never performed or suggested a laparoscopy. His next step with me was going to be Clomid.
Describe your experience with the nursing staff at Hudson Valley Fertility.

Describe your experience with Hudson Valley Fertility.
HVF worked with us only at the beginning stages of our infertility, but I saw Dr Levine more than I saw my doctor here in Seattle at the clinic that I had 4 IUIs. He was kind, knowledgeable, to the point.
Describe the costs associated with your care under Daniel Levine at Hudson Valley Fertility.
The state of NY had good coverage for teachers and infertility. Most of my expenses were paid for as long as they were pre approved. This was a hassle, but now that I live in a state with zero coverage, I realize i should have been more thankful!!! As a teacher in NY, I was allotted $25,000.00 for fertility treatments. We just weren’t ready to do IUI or ivf then. Kicking myself now ...3.5 years later living in Seattle with $0 coverage!!!!

When my husband and I first met Dr. Levine we were very nervous but he was reassuring. He explained all of the paths we could go and explained them throughly. There were times that we saw his assistant and she was very helpful and attentive as well. When we had a very disappointing miscarriage, he was very sensitive to the situation and very informative.
The [Hudson Valley Fertility] would accomadate me. They, unfortunately, closed their Middletown office and it was difficult for me to arrive before 4pm to their Fishkill office and they would wait until I arrived. My husband had to get his supply of semen for the IUI and they were very discreet when collecting the samples. ... Everyone was very kind and cared for all of the patients. They knew everyone by name.

I wish he did the iui's, not the assistant. Dr Levine seemed like he understood my situation but, I had to ask for some of the testing to be done. I wish I had more of an explanation for my conditions also.... Ask all questions when you're there. It's hard to get answers otherwise. I wish I was told that I had to lay down after inserting the progesterone.... Most were covered under insurance but, I was charged incorrectly twice

Dr Levine was never present for the iui's. He was the "talker" that would try to sell IVF (in my opinion).

How was your experience with Daniel Levine at Hudson Valley Fertility?
Dr. Levine explained everything to me and answered any questions I had. He never rushed me out of his office. He had a good sense of humor which helped ease the anxiety and concerns I may have had at the time. He was very polite, professional and made sure I understood everything that was going on. He didn't give up on me like my previous doctor.
What's one piece of advice would you give a prospective patient of Daniel Levine at Hudson Valley Fertility?
If you have any questions, or concerns, don't be afraid to bring them all up because Dr. Levine will answer them and put you at ease. He doesn't rush you out of his office. He genuinely cares.
During treatment, were you treated like a number or a human with Daniel Levine at Hudson Valley Fertility?
Dr. Levine was kind, professional, upfront and also added some humor to our conversations. He answered all of my questions and remained respectful and informative during times I had challenged him on some issues that came up which helped put me at ease. As one of many who are going through this journey, it can be very difficult and very emotional and Dr. Levine was delicate in his treatment of me. Courtney was also very kind to me and if I e-mailed her a question or concern, she answered it in a timely manner.
Describe the protocols Daniel Levine used in your cycles at Hudson Valley Fertility and their degree of success.
I've had issues with estrogen pills in the past which caused my triglycerides to go up and that resulted in pancreatitis. One was with birth control and another was with preparing for a transfer. Dr. Levine assured me that he wouldn't let that happen as he would change the way I receive estrogen. The plan was to do the egg retrieval first and then remove some polyps so that my uterus would be clean of anything that could cause an issue for the embryo and then do the transfer.
Describe your experience with your nurse at Hudson Valley Fertility. (Assigned nurse: Courtney Stellwag)
Courtney was very kind to me and always answered my questions or concerns. She has great bedside manners and always had a smile on her face.
Describe your experience with Hudson Valley Fertility.
What I love about the clinic is that it's intimate and not a huge place. It's in a good location upstate and has plenty of parking. Every time I walked in, I was greeted with a smile and polite manners. What I think they could do better on is reminding patients of their upcoming appointments. My previous clinic would call me to remind me what medicine to take, when to come in for blood test, and also e-mail/call me if there was a change in medication, etc. Initially, I didn't know we had to make an appointment to come in for blood work and U/S as I never had to make an appointment at my last clinic since it was first come, first serve. Because of this and not getting a reminder to come in for blood and U/S before starting a cycle, I missed the first day and didn't start my meds until three days late. I also felt that it was sometimes difficult to get in touch with clinical when I had a question so my main contact was Courtney and I would always e-mail her for everything. At my last clinic, I could e-mail the doctor and he would answer me almost immediately. He allowed his patients to contact him with any questions or concerns which I liked. I just felt it was somewhat of a challenge when I needed to call and ask a question, especially when they're busy. All in all, I did get my questions answered.
Describe the costs associated with your care under Daniel Levine at Hudson Valley Fertility.
There was always a copay of $20 and we had to pay out of pocket for the transfer of $3500 since our insurance money ran out. We also paid for the PGD which wasn't covered.

No complaints. We will see him again in the future when it's time for baby #2. He always made us feel comfortable. IUI- sometimes clomid and trigger shot- sometimes gonal f, trigger shot, progesterone suppositories. Started with clomid as standard protocol to investigate my body's reaction; moved on to gonal after 2 tries. Never did timed intercourse because we knew we had male factor issues. After 3 failed IUIs conducted myomectomy to see if removal of fibroid would help. It did not, but was still important for IVF later anyway. IVF- menopur, cetrotide, gonal, trigger shot, progesterone suppositories, progesterone oil, estrogen. Frozen cycle because I am younger and didn't need to rush; let the body heal after retrieval.

Describe the costs associated with your care under Daniel Levine at Hudson Valley Fertility.
My insurance covered my surgeries, IUI, and medicine. It covered 75% of IVF. We paid about 2500 for IVF.
Describe Daniel Levine's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Hudson Valley Fertility.
We only had one viable embryo so it didn't matter. But we did discuss before retrieval, and we agreed on transferring 1 because of my age (younger, higher success rate) and multiples can be high risk.

I dealt with Jaime a lot and she was incredible. Every time I came in she greeted me with a smile and tried to help make the best of a hard situation.
