How was your experience with David Cohen at Institute for Human Reproduction?
Dr. Cohen is kind and warm and disarming from the first meeting. He makes me feel special—like I’m his favorite patient. Even though he probably makes all of his patients feel that way, it never feels insincere or contrived.
What's one piece of advice would you give a prospective patient of David Cohen at Institute for Human Reproduction?
Don’t be afraid to show your heart because Dr. Cohen will be gentle with it.
During treatment, were you treated like a number or a human with David Cohen at Institute for Human Reproduction?
Dr. Cohen is very busy but you’d never know that unless you asked. He makes you feel like you’re his only patient. He responds to emails at 5 am; rescheduled his morning so he could do my D&C himself when I had a miscarriage (and sat with me and held my hand while I sobbed). He has shared in every disappointment and celebrated our great joy when we welcomed our baby girl. Dr. Cohen and his team aren’t just medical professionals; they make miracles happen and are angels here on Earth.
Describe the protocols David Cohen used in your cycles at Institute for Human Reproduction and their degree of success.
Dr. Cohen was aggressive in my protocol and I appreciated that he didn’t make me mess with IUI or clomid given my age. My retrievals produced good results so we didn’t modify from one to the next, but he was open to suggestions like a dual trigger to mitigate the chances of OHS and maximize chances of retrieving mature eggs. Similarly, when I asked to add progesterone in oil during a transfer cycle, he added it. I appreciate that he was willing to collaborate when I had questions about my treatment and if he disagreed, he would explain his rationale.
Describe your experience with your nurse at Institute for Human Reproduction. (Assigned nurse: Linda Alvidrez is everyone’s coordinator and is amazing!)
Linda Alvidrez is the first person you meet at IHR and they couldn’t have chosen a better, more loving, warm person to greet each patient. Morning monitoring never feels like a cattle call because Linda makes each patient feel like a person. She remembers everyone, wants to stay in touch, shares little things about her own kids when chatting, and feels like a friend you’re seeing multiple mornings a week!
Alejandra gave the gentlest blood draws ever and is so personable and fun, yet knows how to read the room and to be quiet and sit with you on a the mornings when you just CAN’T. It’s no wonder she was promoted to be a patient concierge—though I do miss her blood draws!
Every nurse I worked with at IHR was wonderful, though when they called, they always said “it’s one of the nurses at IHR” and I wished they’d tell me who so I could build more rapport and feel more personal but that was likely part of their procedures and not an issue with any of the nurses. They very obviously do care about their patients and I always felt like I was in good hands. There were never issues with my meds or calendars and I could completely trust the nurses to be on top of things even when I didn’t have the brainpower to be myself
Describe your experience with Institute for Human Reproduction.
IHR is a friendly, boutique feeling clinic. You feel like a person, not even just a patient, and certainly not like a number. It’s a clinic that doesn’t feel overly clinical. Even though they are a very busy and growing practice, they still manage to make each individual feel seen and heard.
Describe the costs associated with your care under David Cohen at Institute for Human Reproduction.
I had Progyny coverage during my retrievals and three transfer cycles. I’m about to do a transfer out of pocket and the costs seem in line with standard market rates. They don’t appear to increase prices very much on a yearly basis.
Dr. Cohen has the right blend, in my opinion, of frank and honest guidance that is still warm and compassionate. He also kept us well-informed in multiple consultations of the procedures that lay ahead and what options were available. The process felt orderly and methodical but Dr. Cohen factored in our opinions and what was important for us. He adjusted our protocol after our first egg retrieval yielded no viable embryos and our 2nd round yielded better results.
The monitoring appointments [at Inst. for Human Repro] are conveniently set in the early morning so it's easy to make before work. They don't do weekend monitoring but I didn't find that to be an issue. I had no problems with billing with IHR working with Progyny, our coverage provider. ... It'd be great to have an assigned nurse and a very close relationship to that nurse but IHR's setup is set up more for efficiency and that you'll round-robin between different nurses at monitoring appointments. The surgical procedures were done at the surgical center at 900 N. Michigan and the facilities were top-notch there.
How was your experience with David Cohen at Institute for Human Reproduction?
I feel very confident in Dr. Cohen and the staff at IHR -- not that they'll guarantee us a baby but that they're trying their darnedest to help us achieve that goal and care about my husband and I as patients.
He let us know of the pros/cons of adding HGH to our protocol, balancing the possible added value with the out-of-pocket costs. We went through a hysteroscopy to get my uterus into the best possible shape for transfer. We could have done a costly ERA test at the start of our transfers but as Dr. Cohen explained the various scenarios with ERA tests (conclusive, inconclusive), we felt we could make a calculated risk to hold off the ERA until our 2nd transfer, knowing that we'd be happy with one child at the end of all of this.
None of the IVF process is easy but we feel we have a valued partner in all of this with Dr. Cohen & IHR.
What's one piece of advice would you give a prospective patient of David Cohen at Institute for Human Reproduction?
Don't be afraid to ask questions. Dr. Cohen does a great job of explaining things (diagrams and all!) and doesn't hold back on information but being your own advocate and putting forth the questions you need answers to, just benefits you, the patient.
During treatment, were you treated like a number or a human with David Cohen at Institute for Human Reproduction?
While we're still in the midst of our fertility journey, I feel like Dr. Cohen has always treated us with a warm, personal manner and we couldn't have asked for a better doctor to guide us through all of this. We've experienced devastating loss before starting IVF and explaining my history to Dr. Cohen, I felt he really sympathized. We've had a number of consultations with him every step of the way -- after our first retrieval; after our first failed FET; after our ERA test. He's always taken the time to explain the process and what our options are, always in the context of "If you were my sister, here's what I'd recommend". He doesn't always tell us what we hear but that's OK -- I'd rather he be realistic with me than to sugar-coat the problem. It's a fine line to be both straight-forward but warm and empathetic and I think Dr. Cohen does a great job of this.
Describe your experience with the nursing staff at Institute for Human Reproduction.
I'm not entirely sure if the nursing staff that does the morning monitoring is the same staff that gives me my afternoon results and answers my questions over email. But all in all, they are total professionals and are friendly.
It'd be great to have an assigned nurse and a very close relationship to that nurse but IHR's setup is set up more for efficiency and that you'll round-robin between different nurses at monitoring appointments. I did have to remind them of each stage of my IVF process but that's to be expected when you don't have a 1:1 nurse/patient assignment.
The front desk staff are warm and cheerful.

How was your experience with David Cohen at Institute for Human Reproduction?
Dr. Cohen initially came off as knowledgeable and trustworthy, he made it seem like getting pregnant would be easy for us. As we moved through treatment though I grew very concerned about the care I was receiving.
He stuck with the same treatments over and over even when they didn't work. He would only change course after I did my own research and bought him ideas of changes that we could make. He seemed happy to just continue with the same treatments (through timed intercourse, IUI and IVF cycles) with no sense of urgency to get us pregnant.
After multiple TI cycles and 3 IUIs we moved onto IVF. Dr. Cohen had diagnosed me with PCOS so he expected a strong response. I had a very slow response to IVF drugs (similar to the struggles I had for TI/IUI, so you would think he would have learned from that) and was tracking to retrieve very few eggs, after over a week on (very expensive) IVF drugs we cancelled the cycle. We nearly tripled the drugs dose for our second attempt at retrieval and I did respond better. IHR is closed on the weekends so we missed some crucial monitoring. At retrieval we retrieved less than half the eggs they told us to expect. It was very disappointing and I was upset after retrieval. Dr. Cohen checked in after surgery, saw I was upset, and immediately left. Absolutely horrible bedside manner.
After a fresh and frozen transfer, he consented to checking my uterus before we transferred any additional embryos. We found polyps (likely caused by high estrogen exposure according to Dr. Cohen), we scheduled a hysteroscopy a month out for removal. Within a week Dr. Cohen had me back on very high dose estrogen to prepare to another transfer. When I didn't respond we scanned the uterus again only to find even more polyps than before. So either the surgery was not done properly or I was rushed back on estrogen too soon. (Also Dr. Cohen does not check in with you after a hysteroscopy, the nurses basically tell you it went ok and you can leave now.) We did another hysteroscopy and again within a week he had me back on very high dose estrogen! I again did not respond and decided to seek a second opinion, 6 months after our last transfer I felt we were moving further away from getting pregnant. We consulted with FCI and the doctor confirmed my fears. He said that the course of treatment Dr. Cohen was pursuing was "insane" and I had likely faced a 6 month delay and 2 surgeries for no reason (later had immediate success with this doctor).
What's one piece of advice would you give a prospective patient of David Cohen at Institute for Human Reproduction?
Go to a different doctor.
During treatment, were you treated like a number or a human with David Cohen at Institute for Human Reproduction?
I had to fight every time to make any changes to my treatment. Dr. Cohen seemed happy to continue with the same treatments that were not working, and in some cases actively moving us further from the goal of getting pregnant.
I had to check and double check all of the instructions given by Dr. Cohen and the nurses because there were constant errors.
He seemed uncaring with every set back we faced. We would get on the phone and all the sympathy he would convey was a quick "I bet you're frustrated."
Describe the protocols David Cohen used in your cycles at Institute for Human Reproduction and their degree of success.
Best I could tell there was no strategy. Dr. Cohen put me on a very standard treatment protocol and would not change treatment unless I advocated for myself. My husband described some of the treatment as "torture" (I know treatment is not fun, but doing the same thing over and over again, physically and emotionally was too much).
Describe your experience with the nursing staff at Institute for Human Reproduction.
There was no assigned nurse, which most of the time was fine because most of the communication came through the portal. Some of the nurses were better than others but no one stuck out to me either way.
There was an ultrasound tech was was extremely rude. She would basically tell me to suck it up every monitoring. She also one commented that I looked 6 months pregnant while doing IVF, so that was great.
Describe your experience with Institute for Human Reproduction.
I thought from reading other reviews that it was standard process to have the "cattle call" every monitoring. Only after I switched clinics did I realize this was not the case. I would sometimes spend 30-45 minutes waiting in the morning with a dozen other patients. It really started to feel like they were taking as many (and probably more) patients as possible, regardless of whether they could responsibly manage their care.
Right as I was switching clinics they were merging with another clinic, this seemed to create a lot of chaos. I can't imagine things have improved.
Also the blood techs were so hit or miss and would sometimes have to try 2-3 times to draw blood. And then blame it on me. Never had a single issue any where else.
Linda at the front desk was very nice.
Describe David Cohen's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Institute for Human Reproduction.
He didn't seem to care. He said it was up to us even though transferring multiple was not indicted based on age or history.

Describe the specific things that went wrong at Institute for Human Reproduction.
Unfortunately I have nearly exclusively negative feedback for IHR. I thought their treatment was standard with fertility clinics and only realized how wrong I was when I switched clinics. I feel like I wasted a year and half there.
Dr. Cohen NEVER changed the regimen of meds and dosages I was on - except to add expensive (and ineffective) experimental HGH to my meds...Do not see this doctor. I've never given this kind of negative review before, but his handling of our case was so clearly irresponsible and just profit oriented, that I cannot in good conscience say anything else.
This [Institute for Human Reproduction] clinic will force you take very expensive option possible and see its clients as just sources of profit. Almost all the communication is through a web portal, they try to avoid any voice to voice or face to face contact with you if they can help it. And except for our first appt and surgery, we never got a chance to speak to any doctor or nurse unless we insisted.
How was your experience with David Cohen at Institute for Human Reproduction?
It was a terrible experience. We were treated like a number from the first time we visited the clinic. We only learned what a BIG difference there could be after we changed clinics. Dr. Cohen and his staff were almost comically careless. Every cycle we had major financial and insurance errors done, which would make us pay thousands of dollars out of pocket. HIs clinic never helped us with insurance reimbursements - I had to do them all on my own - and our new clinic (out of Indiana) handles everything for us! Dr. Cohen scheduled me for 3 BACK-TO-BACK cycles that were crammed into 3 months. We only learned how astoundingly irresponsible this is to do for any infertility doctor after we left that clinic. And Dr. Cohen NEVER changed the regimen of meds and dosages I was on - except to add expensive (and ineffective) experimental HGH to my meds, which we paid and additional $2-3K out of pocket for. Out best cycle, and the only one I got pregnant on, was my 4th cycle, which we did with a doctor AFTER we left Dr. Cohen's clinic. Stay as far away from his clinic as you can. He sadly really only sees you as a source of money.

During treatment, were you treated like a number or a human with David Cohen at Institute for Human Reproduction?
This was the first doctor we ever used for infertility, and we now wish we could have recognized all the red flags from the beginning that signaled how careless he was with his patients. We saw this doctor for 3 cycles, and he crammed the cycles in back to back - 1 ever 4 weeks, so that in about 3 months, we had already completed 3 cycles. He and his staff essentially steered us into taking almost every expensive option there was -- PGTA testing (which added an extra $3-5K per cycle) - even though I was only generating 2 blastocysts on my best cycle with him, and he could have simply transferred to see if they took, and Human Growth Hormone, which had no significant effect on me. We only learned after speaking to other medical friends that doing back to back cycles is an ABSOLUTE no-no, and is guaranteed to produce bad results. We also learned that for someone my age, who doesn't produce many blasts per cycle, you might as well transfer and save the PGTA testing fees. His staff never informed us of any of this, and in fact, made MULTIPLE mistakes in our finances (which forced to essentially be on the phone with my insurance every week). When I finally changed doctors for my 4th cycle, we saw an INCREDIBLE change. He changed my meds and dosages immediately, and not only did my numbers improve drastically, but we reached the transfer stage, and we DID get pregnant. It didn't stay (we learned it wasn't a good pregnancy) and I eventually miscarried, but we will absolutely stick with the final doctor I had for at least 1 more cycle.
Describe the protocols David Cohen used in your cycles at Institute for Human Reproduction and their degree of success.
Our treatment was very unusual, in that he scheduled us for 3 back to back cycles, without ever letting us know that this is unusual, and it's necessary to give your ovaries a rest between cycles. He also recommended PGTA testing - which added an extra 3-5K per cycle. He was even so insistent that when I said I wasn't sure I was a fan of PGTA testing, and was considering not doing it for a cycle, he said he would cover 50% of the costs. But we didn't need to - and should NEVER have opted for ANY PGTA testing, at whatever cost. Since I only had 2 blasts on my best cycle with him, he could have transferred them both and simply seen if any of them took. He never even let us TRY a transfer, even tho my insurance covered this 100% and did NOT cover any PGTA testing.
Describe your experience with the nursing staff at Institute for Human Reproduction.
I don't blame the nurses. I think they are just managed by a very callous and profit-driven doctor team that also encourages/allows them to see patients as only sources of income, and not as specific individuals under their care.
We only could see how bad their "treatment" was when we changed doctors and were handled by another doctor and nurse team. It was literally night and day. The other doctor and nurses CALLED us (rather than uploaded info into a web portal) to deliver any critical news, and the doctor even let us call him on multiple occasions when we had questions about our cycle by phone. They gave us photos of our transfer, and were ALWAYS available and supportive (without getting our hopes up too much). We absolutely love our new doctor and nurse team. I wish I could say I even "liked" Dr Cohen and his team, but they were just awful.

And when his staff made obvious financial errors in our case, he would take no responsibility and claim not to know, and would tell us to iron it out with them.
Describe the costs associated with your care under David Cohen at Institute for Human Reproduction.
This clinic will force you take very expensive option possible and see its clients as just sources of profit. He essentially forced us into PGTA testing, even when I said I'd prefer not to based on the expenses alone. (he offered then to pay for 50% of the cost, but we could/should simply have transferred 2 blasts we had, and simply seen it they would take. A transfer is something our insurance would have covered, PGTA testing was NOT. Also, his clinic never helped us with any of the insurance reimbursements associated with the surgery for the egg retrieval itself. We had to pay out of pocket for this, and then had to work out how to do the paperwork for reimbursement ourselves. With our new clinic, the clinic's surgical facility site is also not covered by our insurance (so we had to pay for the surgery out of pocket), but they filed all the paper work for us, and all we had to do was wait for the reimbursement check to arrive.
Describe David Cohen's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Institute for Human Reproduction.
We never even got to a transfer with this doctor. He will throw every other expensive option (PGTA testing esp). at you to keep you from getting to this point.

I went through 2 IVF retrievals and 1 frozen embryo transfer with Dr. Cohen and all were very successful...was very careful and thoughtful about tailoring it for me, and tweaking it when need be. I truly think his careful monitoring and attentive adjustments in medications when need be are the reason my cycles were so successful and I avoided complications (such as OHSS, etc.)
IHR is super well organized and on top of things. It is a well-oiled machine. They always gave me accurate information. Things always ran on time and felt very dependable...respects patients' time and schedules...amazing at dealing with insurance and always following protocol to get things approved and making sure to prescribe things that were covered.

Describe your experience with the nursing staff at Institute for Human Reproduction.
Wonderful nursing staff that was always on top of things and answered questions quickly. I did not truly appreciate how amazing and attentive Dr. Cohen's staff was until I had to deal with another IVF clinic (I moved and unfortunately had to see a different doctor for my second FET). Dr. Cohen's office is heads and tails more knowledgeable and attentive than the current clinic I am using.

IHR is super well organized and on top of things. It is a well-oiled machine. They always gave me accurate information. Things always ran on time and felt very dependable. I had three successful procedures here (two IVF retrievals and one FET) and I honestly could not recommend this clinic enough. Dealing with a different clinic now, I realize even more how amazing my experience at IHR was.

Dr. Cohen was absolutely patient and willing to answer questions repeatedly. When I would do research and ask about a specific course of treatment, he would consider it. My biggest piece of advice would be to make sure that you have copies of everything, particularly financial agreements...

Just like anywhere, there can be some hiccups between departments. We had one small incident with the billing department where they tried to bill us for a procedure after all of the papers had been signed and that procedure had been left off. My biggest piece of advice would be to make sure that you have copies of everything, particularly financial agreements. It’s been better ever since they went to a digital system. Also worth noting: Dr. Cohen went to bat for us when the billing department made the mistake. He made sure that they gave us the discount they were supposed to.

Around $14,000 per full IVF cycle + $2500 for frozen transfer.

If you are reading this, it’s because your path to starting a family hasn’t been easy, and you are now assessing where to turn to next.
I write this having recently “graduated” from the clinic and on to my regular OBGYN to manage my PREGNANCY!
After 2.5+ years of a PCOS diagnosis, infertility, and multiple pregnancy losses, I was referred to IHR by a friend who works as an outpatient nurse with Dr. Cohen's patients. I never thought our situation would end up needing the IVF route to start our family. I tried every possible lifestyle change that could optimize both my and my husband's fertility. But when it came down to it, IVF was the best option for us to start our family.
At our consultation, Dr. Cohen very clearly explained a few options and the success odds based on different treatment types. I appreciated his straightforward approach in telling us the reality of our situation. He graciously answered my 50+ questions during this appointment as I grilled him on everything my IVF Google research told me to. Very quickly I realized, I can trust Dr. Cohen to advise us on treatment options and manage my fertility journey.
Here are a few things that IHR provided that significantly reduced the mental workload of going through IVF
- Morning monitoring appointments that didn’t interfere with work schedule. The staff, nurses, and ultrasound technicians treated me with such kindness, thoroughness, and efficient appointments.
- Same-day test results through a portal + a phone call from clinical team and updated medication schedules by early afternoon
- Access to Dr. Cohen through messages and easily set up appointments
- Access to instructional videos for medications & a 24/7 phone number to call with questions
- State of the art embryology lab, fertility technology, and testing
After 1 round of egg stimulation with ICSI and 1 transfer, I am now pregnant. While many women need multiple rounds of IVF, I feel that Dr. Cohen’s experience, real-time testing results, and day-by-day adjustments of medications led to this incredible outcome.
I highly recommend Dr. Cohen and the IHR Practice.

Describe David Cohen's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Institute for Human Reproduction.
Initially, I had an interest in putting in 2 embryos. However, after Dr. Cohen educated me on the data, I agreed that eSet was the ideal way to go to achieve a healthy pregnancy.
Dr. Cohen is excellent at what he does. During every meeting he was able to clearly describe procedures, risks, success %, etc. I don't think we left a meeting without a diagram to help further his explanations. Dr. Cohen's bed side manners are 100%. He's caring and gentle. He always made me feel comfortable and knowledgeable of what was happening while he was preforming a procedure.
I live in the western suburbs so I was also able to blood work and ultrasounds out of the Oakbrook office [of Institute for Human Reproduction] which was very convenient (on the days they were open). It was great to be able to do blood work before my work hours and have same day results...The clinic's strengths are making you feel like you are a person and not a number; from the front desk staff to Dr. Cohen. There is easy street parking near the Chicago office.

What's one piece of advice would you give a prospective patient of David Cohen at Institute for Human Reproduction?
I would tell any prospective patient of Dr. Cohen to feel comfortable and confident to ask as many questions as you need. Don't hesitate to tell him you don't understand or you need further explanation. Ultimately this is your body and your decision so you need to empower yourself.
During treatment, were you treated like a number or a human with David Cohen at Institute for Human Reproduction?
Dr. Cohen always treated my husband and I with full professionalism while making us feel cared for and a priority. We never felt like a number with Dr. Cohen. For each Dr. appointment and/or meeting he was knowledgably of our case and always took time to ask how we were feeling, our thoughts/concerns, etc. After our (1) IUI and (2) IVFs he was always cheering for us and sending us home with well wishes.
Describe the protocols David Cohen used in your cycles at Institute for Human Reproduction and their degree of success.
I have unexplained infertility so Dr. Cohen and my husband and I discussed trying IUI before IVF. Then during IVF not PGT testing or ERA. Then when that didn't work we opted to pay out of pocket for both those tests. He didn't feel it was necessary to spend money where it might not be needed and we appreciated that. He tweak our treatments and medicine based on what was learned from previous attempts. We are now currently pregnant.
Describe your experience with the nursing staff at Institute for Human Reproduction.
At IHR they use the nAble Health app. It was super useful to test the patient concierge team or clinical team. By doing so you'd receive a quick response during hours. After hours there is always an all call nurse available to call with questions.

Describe your experience with your monitoring appointments at Institute for Human Reproduction.
In a typical non Covid year there were monitoring hours, I believe 6am-9:30am. First come first serve. They had the process down pat and I never had to wait very long to get through blood and then the ultrasound. With Covid they changed to set times and I was in and out in a flash.
Describe the costs associated with your care under David Cohen at Institute for Human Reproduction.
We were fortunate that once we reached out max out of pocket insurance covered almost everything. It did not cover the ERA test, PGT testing, freezing and I believe one shot around $1k but I don't recall which one.
He [Dr. Cohen] is very clear about what his goals are. He met with me and explained the entire process, but when I expressed concerns and my wishes/decisions I felt like I was getting blown off. During testing, he explained the results of the ultrasound live, but I was so shaken up by the procedure that I wasn't processing anything. I expected some kind of follow up and never got any.
It's [IHR Chicago] kind of weird to find, limited parking and you need a code to get through the front door. Nurses were very very friendly. Hours are also limited. Double and triple check that they have your address correct. You will probably have to call them for test results. Take notes and be sure to write down all the numbers they give you as they relate to your health, bc you will not receive a copy.

He called me from his personal cell phone to explain testing, however during visits he kept forgetting my wishes. I had to call repeatedly and ask for test results and explanations.

I only went through the consultations and initial testing. I understood the need for the initial ultrasound, but when they said "okay everything looks good now let's schedule the more invasive ultrasound" I had questions as to why I need another test if everything "looked good". I felt like Dr Cohen kept pushing IUI and IVF but never explained why.
Describe your experience with your nurse at Institute for Human Reproduction.
They were supposed to call me with explanations of results and never did. I had to call repeatedly to obtain test results. They also incorrectly recorded my address and never attempted to contact me other than via mail for billing. I finally called and asked why I hadn't received a bill, only to find that the staff had the wrong address.

After two failed attempts, he [Dr. David Cohen] counseled my husband and I to try a new test that might tell us why the transfers had failed. The test worked and on the fourth try we had success. We’re trying again for another baby and there isn’t anyone else I would trust to help us conceive than Dr. Cohen. He is truly the best at what he does!!... felt very strongly that having one baby was better than transferring two and ending up with multiples. Since we chose to screen the embryos he recommended only transferring one. We did decide to transfer two on the fourth attempt, but there was a miscommunication with the lab and only one was thawed. It turned out wonderful though because I got my beautiful little girl from that cycle.
I have had trouble with scripts not being ready when the nurse [at Institute for Human Reproduction] said they had been called in and the office was already closed...There are times I will show up and be called right back, and other times I will have to wait more than an hour to be called. It just depends on how busy the clinic is...offer us the option to do back to back cycles and test all the embryos up to a certain amount. It was much cheaper than doing two separate cycles with testing.
How was your experience with David Cohen at Institute for Human Reproduction?
My experience with Dr. Cohen was and continues to be top notch. I was a tricky case when trying to get pregnant with my daughter and had three failed transfers before finally getting pregnant. Each time, he would call me, give me encouragement, and help me get back into the game. After two failed attempts, he counseled my husband and I to try a new test that might tell us why the transfers had failed. The test worked and on the fourth try we had success. We’re trying again for another baby and there isn’t anyone else I would trust to help us conceive than Dr. Cohen. He is truly the best at what he does!!

While I have nothing but the best to say about Dr. Cohen and my time at IHR, it’s always best to be your own advocate and know what’s going on with your care.
During treatment, were you treated like a number or a human with David Cohen at Institute for Human Reproduction?
Everyone from the front desk, to lab and ultrasound, to nursing staff, all take the time to make you feel as if you’re the only person in the practice. They know me by name when I call and always have something good to say. Dr. Cohen does an excellent job of explaining the process in layman’s terms to you. Throughout my time at IHR, I have never felt like it was too big or that I was just a number. It truly seems as if they’re your friends. When my nurse found out I was pregnant with my daughter, she was so excited to call and give me the news. I have always felt special here and wouldn’t consider going anywhere else.
Describe the protocols David Cohen used in your cycles at Institute for Human Reproduction and their degree of success.
I’m not sure of the particular protocol I did for my retrievals, nor the dosages, but I do recall being on Menopur, Follostim, and Ganirelix. I triggered with Lupron for all of the cycles. For my frozen transfers, it was estrogen pills and Crinone for the first two transfers, then patches and Crinone for the second two. My body did stop responding to the pills so we had to switch to the patches. For my positive cycle I was also supplemented with progesterone in oil due to spotting.
Describe your experience with the nursing staff at Institute for Human Reproduction.
I had a wonderful experience with my previous coordinator, Joseline, who left the practice shortly after I got pregnant. This time around I have had some trouble with communication, but it is improving. Still, everyone I have dealt with is as nice and friendly as can be. For the most part, I think they prefer to communicate via email.
Describe your experience with Institute for Human Reproduction.
I really only have good things to say about the clinic and Dr. Cohen. The only thing I will say is that you should keep on top of your cycles here. Know what is going on and don’t be afraid to ask questions. Sometimes the communication isn’t very good but I do think they’re really working on this. If you’re looking for a compassionate, caring practice, this is the place for you!
Describe your experience with your monitoring appointments at Institute for Human Reproduction.
For the most past the monitoring hours don’t bother me. I am a stay at home mom now and don’t have anywhere to be. There are times I will show up and be called right back, and other times I will have to wait more than an hour to be called. It just depends on how busy the clinic is.
Describe the costs associated with your care under David Cohen at Institute for Human Reproduction.
Most of our costs have been covered by our insurance. The genetic testing was not, nor the embryo storage fees. They did, however, offer us the option to do back to back cycles and test all the embryos up to a certain amount. It was much cheaper than doing two separate cycles with testing.

Describe the specific things that went wrong at Institute for Human Reproduction.
On the day of my fourth transfer, we were supposed to transfer two embryos and there was only one thawed. There had been a miscommunication between the office and the lab. As it happened though, it all worked out because that was my positive cycle.
This cycle I have had trouble with scripts not being ready when the nurse said they had been called in and the office was already closed. Thankfully, the nurse had emailed me with the dosage, and my husband (who is a doctor) wrote the script for me. Twice I was told a medication would be called in for me and when I went to pick it up, it was a different medication than what I was instructed to take. Both times, I couldn’t reach anyone at the office or have them return my calls. I ended up just taking the medications. The lab caught an error in my orders when I went in for day 3 testing and needed to draw all new labs. This time around has not been as smooth as my experience with the staff from 2016.
Dr Cohen was super cordial. He went through things, made himself available. He always made me feel like things would be ok....The care of Dr Cohen far outweighs the communication struggles with the staff.... He began doing an HSN on me which I never had. My OB said I had a T shaped uterus but only did a HSG....He found a septum in my uterus and we did surgery the next week. He placed a balloon catheter in there for a week. Removal and then we proceeded to get a final cycle started to begin monitoring.
They [Inst for Human Reproduction - Chi] offer great monitoring hours that allowed me to get there early and make it to work on time. It saved me taking off work. I didn’t like that they don’t have weekend hours...They are inconsistent with email vs phone calls.. so that’s disappointing. I had an email go unanswered form a nurse. I let it go as it wasn’t urgent. But I didn’t like being passed around to different nurses. They did have an after hours answering service which was helpful for emergency questions.

I felt like even if I was the 1st one in the office that day for monitoring, I could be one of the last to be called with my results. And sometimes the news delivered by staff on a recording I would never imagine getting that way instead of a personal call. When I first started off it seemed like my questions were annoying staff and I wasn’t really sure who to ask.. once I got a few months in, I got the hang of it!

He began doing an HSN on me which I never had. My OB said I had a T shaped uterus but only did a HSG. I lost my daughter at 23 weeks and Dr Cohen said that we needed to look further into why that may have happened. He found a septum in my uterus and we did surgery the next week. He placed a balloon catheter in there for a week. Removal and then we proceeded to get a final cycle started to begin monitoring. He gave me 100mg of clomid and I didn’t respond as needed. I was canceled my 1st planned IUI cycle. I was then given more provera to induce another cycle. I requested doing Letrozol instead of the 150mg of clomid as he had planned to do. I then continued with monitoring, triggered with ovidrel and got my 1st IUI. Started crinone 2 days after. I’m still waiting to see if it worked.

Melissa is the main coordinator there. There was a new trainee who is building her confidence in this field. Overall they answer questions but if you don’t ask.. they won’t tell you.

They offer great monitoring hours that allowed me to get there early and make it to work on time. It saved me taking off work. I didn’t like that they don’t have weekend hours.. what if you lined up on a weekend and needed to proceed?? Also, they could use a patient portal. That would do wonders! Describe the specific things that went wrong at Institute for Human Reproduction.
They failed to give me results a few times. Just said to either come back in to be checked again or that I was just canceled. I had ultrasound techs say things that later were disagreed with based on the Drs review of results. They also didn’t correctly update my chart a few times and tried to re-draw labs they already took.
When he [Dr. David Cohen] explains procedures, it comes with charts and graphs and numbers, like a college lecture. My wife and I loved that level of detail, and he explained every step and decision as he went along. How many more IUIs to do, whether or not to do testing, how many to transfer, all came with percentages and evidence-based decisions that he was still open to change if we were to insist.
They [IHR-Chicago] are wonderfully thorough with their procedures and information, but its hard to wait all day for results sometimes. They are a busy clinic, and its understandable. They do a very good job conveying detailed lab results. An issue arising at 3pm on Friday is liable not to be resolved until Monday morning, and the office lacks any real after-hours care. The coordinator is very willing to work around personal schedules. No one has every negatively influenced our decision making, they have always been supportive of our personal choices and thoughts. There hasn't been any judgment or guilt over mistakes we made.
How was your experience with David Cohen at Institute for Human Reproduction?
He did inter-change with the other doctor at the center, but they both were sure to make us feel we were having comprehensive care. Both the egg retrieval and a pollup removal during an IUI cycle were done at another medical facility, but by our doctors. We did not see him often, but 95% of visits were lab work and it wouldn't have been necessary. IHR came highly recommended to us, and has fulfilled that reputation.

Dr. Cohen gave us an insiders look to our insurance, and explained in detail how to make the most out of it. Out of pocket costs ended up being only sperm and shipments, and a delivery fee to the lab, and the freezer cost for a year for the embryos.

None, we agree in the 'one healthy baby at a time' idea
I felt more like a friend than a patient with Dr Cohen. He was always available when I needed him (which was a lot) and he seemed like he genuinely cared about me as a person. I know he treats many many patients at a time, but he always made me feel like I was his only patient...empathetic, kind, and funny. He was always in tune with exactly how I was feeling without me having to tell him. I can't say enough good things about him. Without him I would have been completely lost and confused during the IVF process.
If not for Dr Cohen I don't think I would have stayed with IHR. I enjoyed seeing most of the staff on my many visits. They were friendly and welcoming. Otherwise the office just seemed like a mess to me and information was conflicting or non existent. I would expect to be lost and confused here if you are new to fertility treatments... During my time with IHR they seemed to have a difficult time keeping nurse coordinators and she was extremely over worked during my time with her.

Be open and honest. He will be with you. He will answer all of your questions if you ask them. IVF (or other fertility treatments) is especially confusing if you've never been through it before and it's almost impossible to retain all the information that's provided during an initial visit with Dr Cohen. You can always ask him to clarify or go over anything you need to again outside of appointments with him.

Describe your experience with your nurse at Institute for Human Reproduction. (Assigned nurse: Melissa)
Melissa was a great nurse coordinator. Due to circumstances beyond her control there were a number of times that results were given to me much later in the day than I was told to expect them. She was always apologetic and still took the time to answer any questions I had. I also had to reach out to her to get information about my transfer time and information because I never received it from her. I don't blame any of this on her. During my time with IHR they seemed to have a difficult time keeping nurse coordinators and she was extremely over worked during my time with her.

Describe David Cohen's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Institute for Human Reproduction.
Dr Cohen told me right away that he would only transfer one embryo and I was just fine with this. I believe what he said was his goal was for IVF to be successful for me and it wouldn't be a success if I had two (or more) babies in the NICU or worse. I got the impression you have to have a pretty good reason for him to transfer more than one embryo on your first try.
What specific things went wrong at Institute for Human Reproduction?
He [Dr. Cohen] was very personable and talked to you in terms you understand. Really took the time to get to know you and make you feel comfortable...We did ivf with our embryos genetically tested...Dr. Cohen personally called us and told us how concerned he was that we weren’t doing ivf since we could get our embryos genetics tested to make sure they didn’t carry my brca gene. And how we didn’t just want a baby but a healthy one.
I really enjoyed this clinic. It was always first come first serve when we were going through your cycle. Which made it really nice since we were three hours away... It was always very clean and everyone was very helpful...[nurses were] Very personable and also kept me informed. Very easy to get ahold of...It [monitoring] was always in the morning so it was nice that it didn’t take up your whole day. During our consultation we had decided to try IUI since the cost of IVF is so high. Since we live three hours away from his office we were driving home and Dr. Cohen personally called us and told us how concerned he was that we weren’t doing ivf since we could get our embryos genetics tested to make sure they didn’t carry my brca gene. And how we didn’t just want a baby but a healthy one. It really meant a lot and we ended up doing ivf with testing. So thankful we did and now have our beautiful baby boy. He kept saying how he couldn’t stop thinking about us and our case. Just really meant a lot and we knew he truly cared about us.

I believe we spent around $28,000 for everything including one retrieval, genetic testing, two transfers and all medications.
Describe David Cohen's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Institute for Human Reproduction.
We choose to do one embryo during our first transfer since we only had two good embryos.
Dr. Cohen had made himself available if we have had any questions in the process. He did not trust my hsg results from my local hospital and completed an hsn in office. He felt that my previous diagnosis of blocked tubes was wrong based on his testing. At his recommendation we are trying IUI when I had previously been told my tubes needed to be taken out and ivf was my only option...Dr. Cohen presented the pros and cons of each treatment option. He often said, "if you were my sister I wouldn't recommend.." for x treatment. I felt like he had both the best interest of my personal health and family at heart. He would often ask how I was doing if he could tell I was uncomfortable. I felt he genuinely cared about me as a patient.
The nursing staff [at Institute for Human Reproduction] isn't always easy to get In touch with. I believe they start lasts morning/afternoon. I have found that email is the best mode of communication. My coordination has made sure to answer all of my questions. She always makes sure to speak with me directly regarding protocol instructions so I can insure we are on the same page...The billing department is not great. I have had trouble contacting them both via phone and email. I have been seen at the office for many months before ever receiving an EOB for my fort visit. Payment for treatment is required prior to cycle start. Yet there was no one available to taken my payment when I went for my first monitoring appointment. I was told to call back in the afternoon so it could be taken over the phone. This may be in part because my initial appointment was at my OB/GYN's office where Dr. Cohen travels once a month to see patients. The staff isn't particularly warm, but has always been really helpful if any issues arise.

If possible I would recommend you have an initial consult at his office in Chicago. I live in another state and Dr. Cohen comes to my OB/GYN office once a month to see new referrals from that office. He did not have any office staff with him and even called us back from the waiting room. While there is nothing wrong with that, I felt I may have gotten more handouts or material to take home had we been seen in Chicago. It also may have allowed us time to meet with their financial coordinator. Dr. Cohen didn't leave a lot of time for questions in the first appointment. So if you have them, make sure to be upfront and ask at the beginning of the appointment. While he met with us for an hour and a half a majority of the time was reviewing my history, previous testing results, and discussing types/outcomes of ART.

After my hsn Dr. Cohen recommended IUI as he felt my tubes were clear and uterus fine. My first IUI was 50 mg if clomid says 5-9 of my cycle. I had baseline monitoring cycle days 5, 12, and 14. I had an ovidrel trigger shot on cycle day 15.My IUI was completed on cycle day 16. I was given progesterone and estrogen supplements to take untill my blood test.

My coordination has made sure to answer all of my questions. She always makes sure to speak with me directly regarding protocol instructions so I can insure we are on the same page.
Describe your experience with Institute for Human Reproduction.
The billing department is not great.
Describe the costs associated with your care under David Cohen at Institute for Human Reproduction.
There is a self-pay discount for treatment services as my insurance has no fertility coverage.
Dr. Cohen is very caring, straightforward, but also optimistic about treatment options. I never felt like he gave me false hope. Has has always been honest about potential outcomes and realistic success rates. To be honest, I wish I was able to work with him more frequently than other members of office staff. My only qualm is about a diagnosis. I was initally referred for bilaterally blocked tubes. He completed an hsn and stated all was clear and that my tubes were fine.
There is no weekend monitoring [at IHR] which can cause some difficulties. The office staff had been very helpful when I have called with questions. I was just thrown into treatment and would have liked a better understanding of exactly what to expect each month. It makes planning for monitoring a little easier as I live in another state. The billing department does not seem to be on top of things at all. It was several months into treatment before I even received an EOB for my initial visit.

What's one piece of advice would you give a prospective patient of David Cohen at Institute for Human Reproduction?
If possible try to complete your initial
consult visit at his main office. He came to my OB/GYN's office for my initial visit. He had no staff with him and was doing everything by himself. While this isn't necessarily a problem, I feel like we would have had more material like handouts to take home and review if the visit had been at his main office. He doesn't leave a lot of time in the first appointment for questions. So make sure you are upfront with those early in the appointment to get your answers.
During treatment, were you treated like a number or a human with David Cohen at Institute for Human Reproduction?
Dr. Cohen gave us pros and cons for each type of treatment option. He always asked if I was ok, and genuinely seemed to care. He would often say, "if you were my sister I wouldn't have you do.." for x treatment.
Describe your experience with the nursing staff at Institute for Human Reproduction.
Nursing staff has been prompt. If you call you are unlikely to get a response immediately. I believe they are not in untill late morning/afternoon. I have found the best mode of communication with my nurse is email. They also have a voicemail patient portal to leave patient's messages about results. My coordinator has always make sure to talk to me directly if she is calling about protocol instructions. That way I can ask questions and ensure I am on the same page.

What specific things went wrong at Institute for Human Reproduction?
He [Dr. Cohen] was compassionate and easy to talk to, very understanding, reassuring and experienced. I felt very comfortable and positive with him...Dr. Cohen was very informative and understanding.... I went through 3 retrievals - 4, 2 and then 7 5 day embryos resulted and were frozen to go through PGS, with 9 recommended for transfer. In retrospect I appreciate that he was conservative and I have enough embryos to potentially have additional children, but hind sight being 20 20 I could've stopped after 2 retrieval cycles and likely had plenty.
Everyone I talked to at IHR, from Nona the financial consultant to the nurses, were great, and except for one or two instances they were great with communicating the daily information that was needed....Sue was fantastic - compassionate and understanding, and worked with our schedules. I know she worked late hours responding to voicemails and emails at all hours. ..The surgery center where the procedures took place was great! Clean and organized, and a convenient location for me.
How was your experience with David Cohen at Institute for Human Reproduction?
I was referred to Dr. Cohen by a friend who had just delivered a perfectly healthy baby and shared her experiences with me along the way. He was compassionate and easy to talk to, very understanding, reassuring and experienced. I felt very comfortable and positive with him.
What's one piece of advice would you give a prospective patient of David Cohen at Institute for Human Reproduction?
Be patient with the frequent appointments, multiple times a week. They do have a lot of patients, but I appreciated that it was "first come first served" - on days that I needed to get to the airport for a 9am flight, I was able to come in at 6:30 and get my ultrasound and blood work done efficiently.

Describe the protocols David Cohen used in your cycles at Institute for Human Reproduction and their degree of success.
I started with Dr. Cohen in January after switching insurance so it would cover the procedures- I had been 6 months with another doctor on drugs and timed intercourse with no results. He gave me the option of doing IUI or going straight to IVF, which is what we chose due to my age (39, now 40) and everything was straightforward.
Describe your experience with your nurse at Institute for Human Reproduction. (Assigned nurse: Sue Padilla)
Sue was fantastic - compassionate and understanding, and worked with our schedules. I know she worked late hours responding to voicemails and emails at all hours. unfortunately she left the clinic towards the end of my treatment and there was a bit of confusion during the transition.
Describe your experience with Institute for Human Reproduction.
The surgery center where the procedures took place was great! Clean and organized, and a convenient location for me. I also liked the loft-style office and that they played the Today Show in the mornings while I was waiting for my appointments :) They were usually quick to get back to me.
Describe your experience with your monitoring appointments at Institute for Human Reproduction.
They had monitoring hours in the morning, so as I stated previously, it was very convenient for me to be able to go in early and still get to work on time (or occasionally to the airport for a flight!).
Describe the costs associated with your care under David Cohen at Institute for Human Reproduction.
Ugh! I had to go on an Exchange Blue Cross Blue Shield plan because neither mine nor my husband's employers companies covered the treatments because they were "self insured". That out of pocket max was around $6,000 which I hit with my first order of drugs, plus a $400/month premium. However, everything else was covered except for the genetic testing, and each retrieval cycle was a few thousand dollars, and then because I had more than the average (7) embryos go through PGS, there was an additional charge for that as well (but I suppose that's a good problem to have).
Describe David Cohen's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Institute for Human Reproduction.
Because we went through PGS, Dr. Cohen only transferred a single embryo, which made sense to me.
What specific things went wrong at Institute for Human Reproduction?
Failed to call with results

Describe the specific things that went wrong at Institute for Human Reproduction.
I did get conflicting information when Sue left and there was a new coordinator assigned to me (regarding the med dosage instructions), but overall, things were smooth.
He [Dr. Cohen] is experienced and looks at you as individual, opposed to just another patient. During his initial consult with him, we probably spent about 45 minutes together, and that was the first of many one on one consults. He is willing to try new things, but will always refer to the real statistics and scientifically proven data. He is down to earth, kind, trustworthy, good humored, and very available. I found his level to of care to be exceptional. Another thing I appreciated about him was that he didn't take advantage of us in terms of requiring procedures that were not necessary. I was desperate to have a child, and would have paid for every test under the sun, but he was confident that he had the information we needed, given our circumstances, and that with time we would have success...
[IHR - Chicago] Strengths: Dr. Cohen. He is the reason I stayed there. As a new patient, I would advise that you be assertive, but patient and set realistic expectations in terms of turn around time for yourself and your RN. They will always get you the information you need, but as a patient, I do believe it is your responsibility to be on top of your care and question anything that seems off...

I am forever grateful for his care, and respect him immensely.
What's one piece of advice would you give a prospective patient of David Cohen at Institute for Human Reproduction?
Be direct and honest with Dr. Cohen. Ask him for his thoughts and reasoning behind doing things a certain way. He is a great blend of warm, but honest.
During treatment, were you treated like a number or a human with David Cohen at Institute for Human Reproduction?
Dr. Cohen is AMAZING. He is caring and compassionate while setting realistic expectations. He is available to his patients and you are in constant interaction with him. He is not the kind of doctor who you do not see. He is down to earth, level headed, and treats you like an individual. He will move as quickly or as slowly as you like, but always keep your ending goal in mind. I feel so fortunate to have found him and know he is the best dr. around.
Describe the protocols David Cohen used in your cycles at Institute for Human Reproduction and their degree of success.
Given I have PCOS - we used Menopur and Follistm. I believe the dosage was between 75 - 150, so very minimal. He also recommended ICSI as my husband has sperm issues. His protocols resulted in tons of day 5 embryos. We initially did transfers without testing the embryos, but after no success, he recommended testing the embryos, which we did. My next transfer worked. We just had to find the right one, which he was saying all along.

The clinic is busy, and they typically run IVF cycles every other week. I would advise to get there early. They open at 6:30am, but you can typically arrive as early as 6:15am. That's how you will get in asap.
Describe the costs associated with your care under David Cohen at Institute for Human Reproduction.
I do not know the cost. Thankfully we had good infertility coverage.
Dr. Cohen and our assigned Nurse Coordinator were kind and patient. Dr. Cohen explained what his recommended treatments were, listening to what questions and concerns we had. Our NC responded quickly to my emails with questions and was patient in explaining things. The only thing that was sometimes a drag was the early morning monitoring appointments. They are first come first serve so you never knew whather you'd be out in an hour or in 5 minutes. But never too bad and always quick once you were called. We're really pleased with the thoughtful care from Dr. Cohen and the Nurses we worked with during the IUI's, as well as our NC...He took time to meet with us, answered all questions and appropriately used humor to put us at ease. What's one piece of advice would you give a prospective patient of David Cohen at Institute for Human Reproduction?
May be silly, but parking is very easy for the early morning monitoring appointments and is very easy to get to. There also seem to be some days that have less wait times for the monitoring visits-I would ask what those are and go those days, if possible.
During treatment, were you treated like a number or a human with David Cohen at Institute for Human Reproduction?
He took time to meet with us, answered all questions and appropriately used humor to put us at ease. Very kind.
Describe the protocols David Cohen used in your cycles at Institute for Human Reproduction and their degree of success.
I was on Clomid 50mg (increased after first failed cycle I think), Ovidrel, Estrace (3x/day vaginal) and Crinone 8% 1x/day, combined with IUI. The second cycle was when the estrace was added (due to a thin lining, found during the first cycle), which is when we became pregnant!

Covered by BCBS HMO, except for HSG co-pay and cost of estrace co pay.
Dr Cohen has a great bedside manner. We also saw his partner Dr Turkaspa who is aloof and hard to understand at times. Dr Cohen spent a lot of time with us answering questions and providing options when it was clear we needed to try something else...
[IHR is...] Small, intimate. Front desk staff - not friendly or warm. The phlebotomist is the nicest one! I felt like I had to be my own secretary- they often forgot paperwork and left me doing a lot of leg work to make sure each appointment happened. We waited over 45 minutes for an appt and Dr Turkaspa (Cohens partner) was not even made aware we were there waiting. It's walk in each day from 6:30-9am for blood work. I liked this aspect. I knew if I got there early I would be in and out fast!...
How was your experience with David Cohen at Institute for Human Reproduction?
Strength: compassion, care , concern
Weakness : not easy to reach via phone or email
What's one piece of advice would you give a prospective patient of David Cohen at Institute for Human Reproduction?
Be choosy about the coordinator they assign you - they are not all equal. Ask for email or how to contact after hours. I often had to scramble when things came up on weekends or evenings

I have low egg quality and diminished reserve. He used the "max dose " of gonal F and certrotide. They also suggested prenatal vitamins

Vernita had no "bedside" manner. She left confusing voicemails, was difficult to contact and once sent me someone else's test results ! We requested a new coordinator after that.

Describe the specific things that went wrong at Institute for Human Reproduction.
Hippa violation- sent me results with another patient name at top
Consistently not being clear about what tests I had done or needed to do in the beginning of treatment. Mixed messages on doses. No way to contact after hours. The billing office took days /over a week to respond to questions. The Surgery Center at 900 Michigan had RNs with little bedside manner. My husband said the room where he had to sit to produce the sperm sample was dirty! He had to sit on his coat because the chair was not clean.
Dr Cohen himself was nice, appeared competent and experienced. He is personable...he didn't even take my history into consideration. It felt that he made his recommendations based on routine practice instead of an individual assessment... The actual clinic is a mess and then the surgical center he uses for all of his procedures is also not the best (difficult parking, EXPENSIVE even with insurance and flighty staff).
The financial lady [at Institute for Human Reproduction] was terrible. Actually a large part of why we left th clinic. Took her over two weeks to get paperwork to us to even start treatment also was not friendly when trying to discuss insurance coverage, how tests/labs were going to be billed or already billed. We don't have infertility coverage and she genuinely seemed annoyed by that.

You correspond with your nurse more than the MD so if the nurse/ancillary staff are not strong it will be difficult to get through. Even with he best MD recommendations and prescriptions you need a good nurse to guide you.

Describe your experience with Institute for Human Reproduction.
All mornings are blocked off for walk ins for testing (labs, ultrasounds, etc) which is pretty nice. However, never received results like they promise you do at your initial consultation. The financial department is a nightmare to work with and that unfortunately is very important when receiving medical care that can add up to tens of thousands of dollars. Also very hard to reach staff by phone.
Describe the costs associated with your care under David Cohen at Institute for Human Reproduction.
Not too sure- most billed to insurance and then couldn't get additional costs from the financial lady.
What specific things went wrong at Institute for Human Reproduction?
Since we didn't have any other issues other than unexplained infertility, we followed fairly regular protocols with the addition of estrace to increase lining. Dr. Cohen didn't require birth control between each cycle which I preferred because of the added delay and birth control sometimes hurt more than helped my cycle. When we did have to use birth control in preparation for a test or getting to the next cycle, I often didn't have to go through the whole cycle before we could move ahead. Since the waiting can feel like forever, these small adjustments made a big difference in feeling like we were moving forward.
It is a smaller clinic [IHR] so you get more personalized care. The doctors are great, the nurses and staff, particularly the coordinators, can be hit or miss. The morning baseline and test routine is taxing and difficult, especially if you work and don't know how long things will take each time you go in. You don't always get results by the time you are supposed to which is also stressful. However, when there are scheduled procedures or appointments things go very smoothly and people are very nice and helpful.
How was your experience with David Cohen at Institute for Human Reproduction?
Dr. Cohen was very proactive with our care. When things didn't work he would suggest what next steps and tests we could do to find out answers. He wasn't pushy with these options (PGS, ERA, etc) since some cost extra money, but giving us the options right away was helpful rather than going through more failed cycles before trying. From the beginning he was wonderful and informative to meet with, however I don't think he set up our expectations on the time and emotional and physical toll everything can take, as well as about potential delays that can happen. Once we started to understand all of these items over time, on our own we were able to deal with them better. I just wish I had understood more about how long and taxing the whole journey could be earlier on.
What's one piece of advice would you give a prospective patient of David Cohen at Institute for Human Reproduction?
Don't be afraid to reach out to him with questions or concerns. He is caring and willing to explain things, but also busy so you also need to be your own advocate if you have questions or concerns.

Describe your experience with your nurse at Institute for Human Reproduction.
They have a lot of patience, and can be slow to respond or not very compassionate. Sometimes they don't explain things well and several times there were mistakes, mix-ups or medicines not ordered. I had to definitely be my own advocate and push to get the right information or things corrected. Usually they assumed things were my mistake when in fact they almost never were. Also, the protocol for morning baselines and test is long and trying. You have to be there early and never know how long it make take. Once you are meeting with someone, however, the staff are in general very nice, especially with it being so early in the morning.

Monitoring was only in the morning between 6:30 am - 9 am, first come, first serve, where you would have to go in and wait. At a minimum it would take 30 min if it was empty, other times I was there for close to 2 hours. You could not schedule time and you never knew how long it would take, so adjusting for work was difficult.
Describe the costs associated with your care under David Cohen at Institute for Human Reproduction.
IUI's were $1,500 (before insurance), 2 cycles of IVF + PGS (not including medicine) $10,000 (after insurance), ERA test $900

Two times a prescription was not called in, so I had to follow up to get things right so I would have it in time. In addition, other time I was given the wrong results and had to remind them of where I was in my cycle and what the likely results should actually be. When I talked to the doctor about it, he gave a conflicting, new plan of next steps. One or two times, I didn't get my results and would have to call in to make sure I knew what I was supposed to do next.
He [Dr. Cohen] quickly responded to emails, never rushed me, got to know me and my husband, and was always professional. I adore Dr. Cohen and he is the only person I ever refer friends to. His staff was fantastic. Fertility treatment sucks, and I never dreaded going to his office knowing I would be met by such warmth by Dr. Cohen and his staff...He was honest, rational, empathetic, supportive, and always accessible...
Gary, Maria, the others [staff at IHR - Chicago] were fantastic. I always looked forward to seeing Gary! Since the staff was small, people got to know you and you got to know them. All the staff were kind and professional...Luckily, our insurance covered most of the service and medications (though we still had considerable expenses). Billing was mostly clear and handled through the University. I never felt pressure to pay before services...

Dr. Cohen said I had a 15% chance of getting pregnant, yet I did. Twice. Dr. Cohen always explained everything extremely well, he was considerate, kind, and even funny. I truly felt like I was treated like an individual.

Come prepared and ask as many questions as you have. Dr. Cohen will be honest, and he will not sugarcoat the answers, but he will try his hardest for you.

To clarify... I saw Dr. Cohen when he was the head of the University of Chicago's office. He is now at a different practice. This review is of the UofC's clinic. Their small size is their strength. They can treat patients with respect and kindness.
Describe your experience with your monitoring appointments at Institute for Human Reproduction.
Monitoring appointments were first-come, first-serve, first thing in the morning, but that worked fine with my work schedule. There was one ultrasound technician, Deb!, who was kind and funny. There were two changing rooms to get changed in and the procedures were quick and easy.

dr cohen always had time for me to explain everything. ... When we had a bump in the road dr. Cohen always talked it over with the other doctor [at IHR] to find a solution and to have a what is next plan. He was very knowledgeable and honest with everything we have gone through this far. ... the first ivf was your typical protocol when that wasn't successful dr cohen changed my protocol. He did my medicine different in order to get good eggs.
Don't get discouraged by the staff [at Inst. for Human Repro]. They open up to you and make you feel welcome. If i ever needed anything the front desk was so helpful, loved the blood girl....no bruising ... the ultrasound techs are great to and encouraging ... they were great in getting people in and out [monitoring appointments] and on their way. ... the communication at first wasn't the best but it seemed to get better. There was a little disconnect from them and the doctor but nothing crazy.

Dr. Cohen is extremely personable, compassionate, and knowledgable in the medical field. He always offers an explanation and does so in a manner that does not feel as if he is belittling you. He also has a great sense of humor, which definitely lightens the mood during times which can be stressful. Dr. Cohen is extremely compassionate and treats you like an individual. I genuinely feel as though he wants success for me, because it would complete my family, not to increase his stats. He is always available to answer questions, and is open to hear my input and questions regarding treatment options. Overall, he has been a wonderful addition to my treatment team!
Dr. Tur-Kaspa [at IHR] did the ERA test on me in November and took too much of a sample, which appears to be unheard of. I had to redo the test, which was stressful. The only positives that came from this is Dr. Cohen did the second test and it came back that I was not receptive during the time that I was transferring. My protocol was adjusted. I also did not have to pay for the second ERA test.... Robin is phenomenal! She is punctual with her communication and always goes above and beyond to inform me of upcoming procedures and will answer any questions that I have.
