However overall I still give them a 9 out of 10 compared to my other experiences. I felt like they all cared about me and our outcome and gave us the time and patients we needed during the process.

Make sure to ask questions. If you don't know how to give a shot, make sure to ask the nurse. It might be because I had already gone through the experience before. They are much more clear on the process than others, but still didn't cover everything. Do your research no matter who you see and ask questions.

We only did an embroyo transfer with Aspire. They use a pretty agreesive approach compared to my other fertility clinic, which I much appreciated. They were great at making sure the uturus lining was the right size - over 7mm. I took Progesterone shots pre transfer and until week 10 of pregnancy. The question asked if anyone ever didn't call me with results, it happened one time and wasn't a big deal. I just called them.

I first met Dr. Prokai after a failed FET with another clinic. I had many questions about my situation so I wanted a second opinion. I knew I would only switch to him if I really really felt it was the right fit. My husband and I met with him (remotely due to covid). He spent almost an hour talking with us and was willing to explain anything I asked.
Many times Dr Prokai would call me personally if I had a question about anything. He is a great doctor and a down to earth person. Cassy (his nurse) and Marci (his patient point person) are wonderful and attentive.

After a failed FET with a pgt tested normal
Embryo I wanted to rule everything out. Dr Prokai suggested a cycle that started with about 2 weeks of birth control. Then we did a mock transfer (I had a extremely painful first transfer and this was a huge selling point for me that he said he always does a mock to make sure he knows how to place the catheter before it’s actually the time for the transfer). He also did a test for inflammation and that was normal. I did estrogen and PIO before the transfer. Had the transfer (it was soooo easy) and then continued the medication until 12 weeks pregnant.

Cassy (nurse) and Marci (coordinator) are great! They are super responsive and always made sure someone got back to me. When I first started seeing Dr Prokai Cassy was out of the office for a family emergency and so I had a couple emails to her that took a while to get back. But that is soooo out of character for her I learned. Also dr P and Marci would call me and answer questions for Cassy when she was out so I was taken care of!

Dr. Prokai reviewed my previous protocol (with a different clinic) and adjusted the plan according to my response. I will be on estorgen priming instead of birth control and then off to the races with a mixture of follistim, menopur, and omnitrope. I am so hopeful this time around.

We weren't financially ready to spent $1,400/month on Aspire IUI treatments and we were vocal about that. I wish he had been more creative with ways to help us given our financial limitations at the time. For example, adding progesterone, trigger shot, etc.

When communication breaks down, don't just accept it as is. You are your own strongest advocate and you HAVE to play hard ball at Aspire to get what you need.

Dr. Prokai is thoughtful and really did try to help us, but his staff was a complete disaster. The left hand was never talking to the right. They also never helped us create a plan. Just kept telling us to take Letrozole over and over again with the same negative results. By the end of our time with them, we were actually the ones suggesting treatment ideas. Their app interface is awful and I regularly would send questions and would never get a response. This total lack of communication was extremely frustrating and we "lost" several cycles as a result, despite starting planning conversations a week+ out from my expected period date. Every time I allowed myself to believe that we had worked out the communication issues or that maybe we had a solid plan, Aspire disappointed me again, and again, and again.
In addition to the horrible communication, they also sent me IUI contracts on multiple occasions without the procedure even being initially discussed. It felt like they were trying to sell us an IUI (at the outrageous price of $1,400) and if we weren't ready for that financial commitment, then we weren't worth the time. We desperately tried to find less expensive options to try before moving to IUI, but they were too busy to notice or care apparently.

After we completed the final diagnostic tests, we were told that I had some kind of ovulatory issue, but it wasn't super clear. I do ovulate on my own, but it is often delayed. We decided to do a monitoring cycle with Letrozole to "gather information." That cycle really didn't tell us much. From there, we were told to continue on Letrozole, which we did for many months with no positive test. Finally, we asked about incorporating a trigger shot or even progesterone just to try something different. We added progesterone, though my labs didn't suggest that I needed any additional, in this last month with no success. We were supposed to do a timed cycle this month again but their office never responded to my messages at the first of the month with concerns about timing of the cycle and upcoming travel. So, that was the final straw and I'm going back to my OBGYN for an IUI next month. WHICH BY THE WAY only cost $292!!! At Aspire, they charge over $400 for a monitored cycle and $1,400 for an IUI. Absolute highway robbery.

Dr. Prokai's staff was a disaster in terms of communication. I would sometimes have to call and email 3+ times to get a response. Kathy was particularly short and rude to us, which made an already painful situation even more so. There was a specific occasion when I had finally had enough of the miscommunications about treatment, appointments, etc. My husband called and spoke directly to Dr. Prokai and they agreed to have a meeting for the three of us to discuss next steps and clear up confusion. I got a call about an hour later from Kathy who was insistent that we were not supposed to have an appointment at all and that my husband was clearly wrong. It was, in fact, Kathy who was wrong and we did schedule the conversation with Dr. Prokai directly. This is just one example of the many, many instances of miscommunication and poor coordination from his team.

I later found out that an IUI with my OBGYN cost $292. Needless to say, we are not doing IUI with Aspire.

Communication was the #1 issue. Currently, their online portal is set up so that you can message the doctor and his/her care team all at the same time. They can respond to your message, but you cannot write back in the same chain. This month, I asked about delaying my period 2-3 days so that we would be able to schedule an IUI around a weekend that I was going to be out of town. I got one response saying that I could take a "short course" of birth control but then I would lose an entire month of opportunity. I wrote back asking if I could just continue with progesterone instead to bump it back a few extra days. Never got a response. So, my cycle came as anticipated and I'll now be out of town for ovulation-- thus losing another opportunity. Another cycle, I called three times and emailed several times trying to get a re-fill of Letrozole. Despite reaching out over a week before my cycle began, they didn't get back to me until CD5, so that cycle was a bust too.
From our very first consultation, Dr. Prokai handled our case with a personal touch an incredible amount of empathy. Each time we saw him, he took the time to check in on us personally (weekend plans, how we were feeling etc.) and not just from a medical perspective...First round IUI - Clomid with Ovidrel. We noticed this caused my lining to be a bit thin. Because of this we changed strategies for the second round
Aspire is extremely welcoming and filled with kind people. Appointments were always on time, scheduling was easy, and the staff was very knowledgeable and easy to work with. Everyone from the front desk to the nursing staff works to make your experience as mentally/emotionally painless as possible...They were always lightning fast and thorough. It's clear that they're passionate about this work and care deeply for their patients.
How was your experience with David Prokai at Aspire Fertility - Austin (Prelude Network clinic)?
We feel incredibly lucky to have seen Dr. Prokai through this process. Unexplained infertility is both a good and bad thing - good because nothing is wrong, bad because nothing is wrong (and still nothing is happening!). It can be very confusing, isolating and sad. Dr. Prokai balanced empathy and optimism with educating us and presenting the facts. He always took the time to answer our questions thoroughly and patiently. On my first visit I mentioned how I hated getting blood drawn - he stayed back while the nurses did this and just chatted with me to distract me. I knew then that he would be different. All of our fears going into this process were quickly alleviated- we trusted in Dr. Prokai and his plan - he didn't let us down. Now we are pregnant with our baby boy and couldn't be happier! While we hope we don't have to go through this again, we would in a heartbeat knowing Dr. Prokai is on our team.

Trust him, be honest and communicate! He will always give you the time if you need it- so don't shy away from it.

First round IUI - Clomid with Ovidrel. We noticed this caused my lining to be a bit thin. Because of this we changed strategies for the second round.
Second round IUI - Letrozole with Ovidrel and Progesterone supplements. Success!

We were lucky enough to have Progyny as a fertility benefit which covered most of the costs associated with Dr. Prokai's care.
