We went to Dr Ryan as our 1st doctor not knowing anything...She was very frank, didn't beat around the bush on basically saying I'm old and have old eggs. Could be a bit more warm and friendly. Visits were quick and she was onto the next...I thought she was fine. Nothing special, for sure no love or warmth came from her. She was very by the book, cut and dry. If you need a bit more information, time and care - I would find another doctor. She did provide some good information to my husband and I and set us on a path towards frozen egg donors which is a journey we are still on.
You can tell that PFC is big business with the operation...They are quick to take your money, even though everything we did with them was covered by insurance they are so slow to submit to insurance...Lots of back and forth in their online tool. Not much help, there is zero sense of ownership or urgency in helping to me to solve this...Can't get a copy of my account to see what insurance covered and why I haven't received a refund yet. Lots of unanswered emails in their online tool.

Be ready to hear the a lot of information much of which we had no idea meant as she was the first doctor we had seen about our Fertility problems. She doesn't handhold or provide any warmth or love. It was overwhelming. She would probably be a better doctor if we had seen someone else first and knew a bit more about the process. Coming in cold to her was tough.

We did the initial consultation, my husband did his donation and I did the water ultrasound. We got stuck looking for donor eggs with PFC. There donor pool is very small and we spent this past year looking with not much help from them so we ended up stuck with PFC and just switched clinics.

I don't remember much. They all seemed fine. They communicate through their online tool. Sometimes they are great and sometimes they are slow on the tool. I think they rely too much of the tool. Let's just have a call and knock out the information but that was always a challenge.

They are a big business, don't forget that. They clearly are successful but at times you feel like a number. You have to keep reminding them you are a patient because they forget about you. I was a patient trying to find donor eggs through their bank which I was searching for this past year. Since I wasn't active constantly with the clinic they weren't super friendly or helpful. The egg bank - I went through a few different contacts, helpful at times and then other times would not reply to emails. I would lose access to their site randomly and have to request it. It was frustrating because time is a huge thing with locking in an egg donor before someone else takes them so at times it felt like I was going in circles with them. The finance team is also a challenge - I had to ask several times to please submit everything to insurance after they had us pay everything up front. They only submitted a few items not everything, no idea why. Lots of back and forth for months and can't get a straight answer. Can't get a copy of my account to see what insurance covered and why I haven't received a refund yet. Lots of unanswered emails in their online tool. I finally got someone on the phone to discuss our account and billing and they admitted they hadn't followed up on it since July (this was October)! Not impressed with that lack of attention. They are quick to take your money and then they fall off. I'm still waiting for a refund. sigh.

Well, I mentioned in the clinic review section so I'll briefly explain here. They are quick to take your money, even though everything we did with them was covered by insurance they are so slow to submit to insurance. I've had to follow up several times to ask them to submit everything, not just one or two visits but everything. We've yet to receive any refund from PFC even though insurance has covered our appointments so I'm currently trying to get a refund since they have received payment from our insurance company as well. Lots of back and forth in their online tool. Not much help, there is zero sense of ownership or urgency in helping to me to solve this. Again they take your money very quick and then they couldn't be any slower to work with insurance and solve problems. It was very disappointing and really left a bad taste in our mouth with this clinic. Huge area where you feel like a number.

She took the time to answer all of my questions and always kept me updated on where I stood in the process of my egg retrieval, embryo transfer, and post care. Pacific Fertility Center is a great clinic that is very modern and offers a serene office space, excellent patient care, high level of service, and has an online portal that made it easy to keep track of appointments, test results, and calendars to track your individual cycles. The only thing I would say is that since it is located deep in the heart of San Francisco, traffic may be hard to navigate through.

This was a big help to me emotionally because it helped me stay grounded for whatever the results would be after all of our processes that include egg retrieval, genetic testing of the embryos, our own genetic testing, and chances for success with the embryo transfers and preparation of the lining. She even went as far to recommend an ERA test prior to our actual embryo transfer due to my medical history and we were allowed to do our own research and make the decision to proceed or not on our own. She did not pressure us either way. That proved that she was really paying attention to my health history and was doing all that she can to increase our chances of a successful transfer. Overall, I really enjoyed working with Dr. Ryan and her approach to care really meshed with my expectations and made me feel confident with every decision along the way.
What's one piece of advice would you give a prospective patient of Isabelle Ryan at Pacific Fertility Center (Prelude Network clinic)?
One piece of advice to those prospective patients of Dr. Isabelle Ryan is to do your research prior to your first consultation. Come prepared with questions you may have and really use that time to dig in and have a deep conversation about your possible treatment, areas of concern, and get her opinion on your overall chances for success given your medical history and reasons for infertility. When you come prepared, that will instantly make your appointments more meaningful and keep you more informed to make decisions regarding your own care.

I felt that I was treated like a human with respect because Dr. Ryan took her time to go over all of the scenarios and statistics regarding my specific situation and laid out realistic expectations for this journey through IVF. Her team constantly checked in on me and their response time to my calls and emails was fantastic. They really do focus on caring for the patient.
Describe the protocols Isabelle Ryan used in your cycles at Pacific Fertility Center (Prelude Network clinic) and their degree of success.
Egg Retrieval Treatment included daily dosages of follistim and menopur, and later included ganirelix. This was successful and resulted in 43 eggs retrieved. ICSI was used to combine my eggs with my spouses semen to produce embryos. We then proceeded with PGS testing for the resulting 9 embryos that made it to day 5. Of those 9 embryos, we had 3 healthy embryos with no chromosomal abnormalities. We also did ERA testing to make sure my lining would be receptive prior to the actual embryo transfer. Once the ERA testing came back, we proceeded with the Frozen Embryo Transfer based on the timing listed in the ERA report. Due to my age, we did ESET and implanted one embryo which has resulted in a pregnancy.

Alvi was the absolute sweetest and most caring nurse. She was an excellent communicator of all of my care and was diligent to follow up in a reasonable time on all inquiries I had regarding my care and appointments. She had excellent bed manner and always took the time to make sure all of my questions were answered.

I appreciate Dr. Ryan suggesting we go with eSET instead of multiple embryo transfers because of my age and high risk pregnancy if we were to have multiples. I agree with her approach and it showed that she was not only focused on doing everything she could to increase our chances of success, but also was looking out for my own health. She took the time to explain her reasoning and I was confident with this approach given all of the facts.

Unfortunately, it resulted in a negative pregnancy result... Dr. Ryan was very caring, however when I pressed her about next steps and diagnostics, she stated that she did not think anything was "wrong" with me, and that she would want to do 4-6 more transfers before doing any diagnostic procedures.
PFC is a boutique clinic, and you will be well treated and attended to. They are responsive and clear about logistics and protocols.... All of the nursing staff and care coordinators at PFC that I dealt with were great.... Given that the numbers are supposed to be so much better with donor eggs, and the fact that no one has given me any reason that we would not be successful going this route, I don't have the most positive feeling about PFC. Of course, there are probably many people who have had successful outcomes there, so this is just my experience.

We did one IVF transfer, with a tested, euploid embyro created with my partner's sperm and a donor egg. I took estrogen and progesterone leading up to, and for a couple weeks after, the transfer. There was no pregnancy.

I feel like Dr. Ryan nailed my medication dosages perfectly. I once saw another doctor at the practice for an ultrasound and he changed my medication dosage. After reviewing the doctor's recommendations Dr. Ryan's team called me shortly after my appointment to negate the other doctors recommendations and to stick to Dr. Ryan's protocol. I got excellent numbers for my retrieval and I think Dr. Ryan's medication dosage is one of the contributing factors to that.

I was diagnosed with annovulation a and possible PCOS. I had tried letrozole for several months prior to seeing Dr. Ryan (with my ob/gyn). We tried IUI with letrozole twice without any success. I then had a very successful egg retrieval (34) and had the eggs tested and then frozen (20). We then did the first transfer with only a brief chemical pregnancy, but the second time it worked and I'm now 10 Weeks pregnant.

Isabelle Ryan has a very direct, succinct, blunt manner. That didn’t bother me at first, but at a later stage in my care I realized that many crisply communicated statements really glossed over important nuances, and areas of uncertainty. Our meetings were always very quick, and I left thinking I had gotten all the information I needed, but it’s hard to know what you don’t know. As a small example, I was a first-time patient, and it was never communicated to me that at PFC my doctor likely wouldn’t be performing my retrieval surgery. I now know this is very common, but this shouldn’t be communicated to a patient the first time in the middle of a stimulation cycle. Isabelle Ryan has a very direct, succinct, blunt manner. That didn’t bother me at first, but at a later stage in my care I realized that many crisply communicated statements really glossed over important nuances, and areas of uncertainty. Our meetings were always very quick, and I left thinking I had gotten all the information I needed, but it’s hard to know what you don’t know. As a small example, I was a first-time patient, and it was never communicated to me that at PFC my doctor likely wouldn’t be performing my retrieval surgery. I now know this is very common, but this shouldn’t be communicated to a patient the first time in the middle of a stimulation cycle. Most importantly, I feel she handled my case poorly from a medical standpoint, and made critical errors which, subsequent doctors pointed out, introduced unnecessary risk. She and her care team overlooked disturbing symptoms that I had and, in retrospect, gave me a treatment which was dangerous rather than helpful. This aspect of my case would not apply to most patients, but I think it speaks to a lax attitude about complications, and an unwillingness to take the time to think beyond the obvious norms. Finally, she was coordinating care with an outside specialist, and the communication about the status of this coordination was bad -- I realized months later that the tests from the outside specialist had actually given me the opposite results that Dr. Ryan had used in her assumptions and care.

I probably would see a different doctor unless you are ready to go home and independently fact check what you hear, and almost act as your own secondary doctor.

It was somewhere in the middle. The clinic is not an over-crowded factory type atmosphere, but I sometimes felt like a number because important details of my care often slipped through the cracks. I felt that Dr. Ryan’s care team, in particular nurse coordinator Doris Dalida at the time, didn’t do a good communicating clearly about my specific case, but would speak more generically meaning important details were left out.

Before cycling she had my husband and I do prophylactic antibiotics (zithromax and cipro). First cycle (which ended in a cycle cancellation with no retrieval): 3 weeks birth control pills, started 200 IU Follistim & 75 IU Menopur, this was increased but not sure to what levels. Never triggered because cycle was canceled for poor response. Second cycle: Removed IUD; no birth control pills. Estrace priming before cycle start (4 mg daily). At baseline ultrasound start micro-dose lupron (10 units, twice per day); then starting dose 300 IU Follistim and 150 IU Menopur. Trigger 10,000 IU hCG. Resulted in 19 eggs retrieved.

I found Doris, Dr. Ryan’s nurse coordinator to be very underwhelming. She wasn’t very helpful in providing the information I needed and was not helpful in organizing my overall care. There were other nurses who was nice, but I never felt that I had someone really competent, who cared, who was on top of things for me.

Not sure how much we paid for the first canceled cycle. Second cycle where we did a retrieval was about $12,000, though they gave us a $1,500 discount because of issues in our first cycle, and because we were self-pay patients. This did NOT include all of the lab tests and drugs, which added several thousand dollars, or the $1500 to Natera which is the outside reference lab used for PGS testing.

Also, if I remember correctly, think the scheduling sheet (for my IVF cycles) was much easier to follow and understand than the one I had at Cornell.

Although I only had one pregnancy at PFC and it was an ectopic pregnancy, i can't help but think that the fact that I was able to get pregnant at all, led me to be able to get pregnant again.

On my third IVF, I got pregnant, but the embryo implanted in my Fallopian tube (ectopic pregnancy), I don't think the team looked closely enough during my first ultrasound to see if there was something in the tube, when they realized there was nothing in the uterus. Especially since I later learned that women who undergo IVF have a higher chance of ectopic pregnancies. It is possible it would not have been visible, but if it could have been visible in the uterus, why not in the tube? so a week later when the ectopic pregnancy became a large blood clot and threatened to rupture the tube, they gave me a drug to abort the non-viable pregnancy and told me i would be experiencing severe cramping over the next two days and that i had to stay in bed. On the way out the door, I asked, "is there anything i should look out for?" and she said, "yes, if you start having really severe pain and it lasts for 5 minutes, go to the emergency room immediately." That advice that was only given once prompted by question, saved my life later that night when the pregnancy did rupture causing massive internal bleeding that would have been fatal if I had not gone immediately to the hospital and had emergency surgery. Perhaps I would have gone anyway when the pain continued, but perhaps not. Regardless, it was something she should have told me without me asking the question.

I wish she [Dr. Isabelle Ryan] would've explained a bit more about the number of eggs actually needed in order to conceive a child. I would've done another round or two of retrievals to be safe. Other than that she provided me with a lot of knowledge of how everything worked, the process, what was to be expected with the injections, side effects, etc. There were no surprises...Ask questions until you fully 100% understand every detail of the process...Sometimes I waited longer than expected...
