[Dr. Nemiro] was kind, honest, and committed to success and safety!...frank even when it was hard and helped me make decisions that aligned with my values and not just my heart...always spent the necessary time with me...often was running behind, due to the time he spent with other patients. That was difficult at times, but I also appreciated and benefitted from his extra time...He let me lead and do what I felt would be best for me. We tried multiple protocols.

Give yourself extra time to spend when you see him. He is thorough and will take time with you, but he often runs late.

Dr. Nemiro and his staff were always kind and amazing to me. They all knew me by name, asked about my life, worked with me when I needed help, and provided support when I experienced pregnancy loss. I always felt cared for and like I mattered to them.
Describe the protocols Jay Nemiro (retired) used in your cycles at CCRM Fertility of Arizona and their degree of success.
He let me lead and do what I felt would be best for me. We tried multiple protocols. Including use of FSH and HMG. My successful protocol was Letrozole days 3-7, 75iu HMG from day 8-17, 10000iu HCG trigger on day 18 (when follicles were 18-21mm), IUI 42 hours after trigger, 5000iu HCG booster 5days post IUI.

Jennifer was amazing, she was so sweet and kind, and knowledgeable about treatment. She always did everything she could to help me and to make things easier for me!

The clinic was beautiful. It was always clean, but didn't feel sterile. The rooms are comfortable and homey. I like that they use cloth sheets to cover up, most rooms have a private bathroom. Medication costs were between $150-400 per medicated cycle. IUI was $300, $150 for sperm thaw and wash, $400 for all ultrasounds in a cycle.

As an IUI patient, I never saw Dr. Nemiro. I saw him twice and once was for the consultation. He was not involved in my care aside from approving medication.... He was not open to changing my dosage throughout a cycle (so if you're on 150 Follistim, you will stay on 150 Follistim, it won't increase or decrease according to your response). I felt like he was not flexible and treated me according to a pre-set formula of how a woman my age should respond, rather than recognizing I wasn't responding (6 times) and my treatment was never aggressive enough due to my age and high AFC.
There is a difference in cost for IUI cycles with injectibles and IUIs with oral medication [at AZ Center for Fertility]. The IUI itself is only $248 regardless of injectibles or oral meds. Sperm thaw is $100. Sperm wash is $256. Monitoring for oral med cycles is $327. Monitoring for injectible cycles is $540. So IUI costs can range from $931-$1144, which doesn't include the cost of medications.

Ask questions and demand to see him if you're doing IUIs. You can get a lot of valuable information from him and explanations, but you have to call him yourself or ask to see him specifically, otherwise you will likely never see him. He is a nice man, but the lack of flexibility in medications and lack of communication was not for me.

I felt like a number since I was hardly ever spoken to by my doctor. However, his RN, Jennifer, was lovely and very caring. She was understanding and positive about our situation.

I started with 50 mg Clomid with IUI, then second cycle 5 mg Femara with IUI, then 5 mg Femara with home insemination, then 5 mg Femara with IUI, then 75IU Follistim and finally 150IU Follistim.

Jennifer has been through infertility herself and therefore was very compassionate and caring. She is a highly detailed person, which I very much appreciated about her. I wouldn't have wanted to go through treatment with anyone else. While I was disappointment in Dr. Nemiro's lack of involvement, I very much trusted and appreciated Jennifer.

This clinic can be a bit disorganized at times and details can slip through the cracks. Scheduling is very easy and quick, I never had a problem getting an appointment when I needed it. Everyone seems to care and want you to be successful, as a whole, I liked the clinic, but I'm moving onto another doctor that I think might be a better fit for me.

There is a difference in cost for IUI cycles with injectibles and IUIs with oral medication. The IUI itself is only $248 regardless of injectibles or oral meds. Sperm thaw is $100. Our IUIs were often running late (and this clinic does IUIs later than most clinics, at 42-43 hours. Often my IUIs were scheduled for 42 hours, but didn't occur until 43-44 hours. I question the quality of some of the equipment at this clinic, one room has updated equipment and the rest of the rooms are a bit lacking in this area and seem to be stuck in the 1980s. Due to the continuous low counts using donor sperm from multiple donors and from multiple clinics, I question equipment quality and/or staff that are responsible for sperm washing. We were initially told that IUIs were much cheaper than they actually ended up being since they failed to include the costs for monitoring, along with sperm thaw and sperm wash (thaw and wash are something that many clinics do not charge for in this area).

I like how analytical Dr Nemiro is. He really give me the realistic numbers when we were trying to decide what was next for us. His expert advise is what we need to make an educated decision and not waste our time, emotions or our money.... for the egg retrieval we did an ovarian stimulation protocol with viagra suppositories 25 mp 4x day for 2 wks, growth hormone 1/2 cc once day for 2 wks, luprion 20 units for 10 days and then 10 units for 2 wks, estrace 2mg tab once a day for 2 wks, cetrotide 50 units for 2 wks & FSH 10 vials once a day for 2 wks. I also had to take cabergoline .5 mg after the retrieval & more luprion 20 units once a day until my next mense. This was because my estrogen levels were at 6400 after the retrieval.
All of the nursing staff [at AZ Center for Fertility Studies-Scottsdale] showed me compassion, from my primary nurse, financial coordinator to the front desk staff. They are all wonderful.... The office layout can be confusing but the result is a private space and keeps it from feeling clinical or cold. The office does not feel like it is sterile and cold as other clinics do. Makes it feel almost homey.

Do your own research and ask questions. As educated and wonderful a doctor may be, asking smart questions will always be in your best interest. It also allows the Doctor or staff to help you better when they know what you are thinking.

The staff is amazing and always available by phone or email with question. Dr. Nemiro spent several evenings on phone calls with me to answer all of my questions and give me hope.

IUI's were around $1500 to $2000. The egg retrieval was about $10,000 or so which includes a 20% discount on provider services because our infertility was due to my husband's cancer treatments. medications were an additional $5,00 to $6,000.

I don't know his approach just yet but I do know that he has advised that we transfer no more than two embryos.

I would look twice and consider practices because some things are very expensive there and consider an office w more nurses to handle cases and one w more fair transfer pricing.

For transfer, I had to have prednisone perhaps due to all my prior losses and was supposed to have intralipids also but couldn’t get them wher I lived. I think many things he does (treating for immune) is excellent.

Regular cycles for retrieval and egg banking/freezing we’re 11k a cycle. When everything was fertilized on the last cycle, it was probably around 15K. Donor cycle was very expensive. PGS is reasonable there at $2000,00 for up to 8 embryos, but transfers of any embryos are $4,400 which I found to be very very high.

My doctor is progressive on this - he believes over say 43 & up the incidence of twins is low so why not put two in. I put two in both times and by the 2nd transfer, one did stick so it supported his theory.

They mixed me up w someone else and changed my protocol to match that other person’s history I believe and denied me a drug I needed for my transfer based on false info. I had a CP. That may have contributed to the CP. They also missed a small procedure I needed to have done for the 1st transfer and that probably didn’t help either. They just flat-out forgot to tell me to get I done. It went better for the 2ns transfer...

He runs very late and the waiting room is packed...If you are ready to jump straight to IVF then he may be the Dr for you...He put me on oral meds for 2 cycles and then to daily HMG injections. The thinking being more eggs giving higher chance of pregnancy. I produced 4 follicles each time and did IUI’s.
The nurses [at Arizone Center for Fertility Studies] ran the show. Gina was very nice and caring. Unfortunately the clinic was missing my ovulation each time so the IUI’s were a waste of time and money. The clinic makes you come in on cycle day 1-3 for a baseline US and then every other day and still this mistake was made. The amount of time put in to getting so many ultrasounds was very difficult on me. Michelle the financial coordinator was very brash and came across as uncaring. They will not work with your insurance at all for any testing.

He [Dr. Jay Nemiro] met with me personally to go over game plan before each IVF, listened to my concerns, and was candid about why he recommended what he did recommend. He even met with me, free of charge, in between children, for a question I had. He stayed in the office to call le with test results as late as 7 pm one Friday night, so I wouldn't have to worry all weekend. I always felt like I was a person (rather than a number or a statistic) and that we were a team with a stake in the fight to get me pregnant.

He met withwith me personally to go over game plan before each IVF, listened to my concerns, and was candid about why he recommended what he did reccommend. He even met with me, free of charge, in between children, for a question I had. He stayed in the office to call le with test results as late as 7 pm one Friday night, so I wouldn't have to worry all weekend. I always felt like I was a person (rather than a number or a statistic) and that we were a team with a stake in the fight to get me pregnant.

Most of this I have forgotten, but I do remember we switched IBF stim meds when the first didn't get us to retrieval.

Gina is amazing! I cried in her arms a couple times when we had setbacks. She is very knowledgeable and safe when the process becomes overwhelming. Jenn is very sweet and caring andand honestly rooting for you!

It can be a bit of a shock at first, but he will fight to get you pregnant...I felt listened to, respected, and like he was personally rooting for me. Prior to getting pregnant, I needed surgery on my uterus and he called results at 6:30 on a Friday night so I wouldn't have to be in limbo all weekend. He spoke to a friend of mine who liveswho lives across the country over the phone who had suffered RPL and was near giving up. (She's a Mom now, too)!
My phone calls [at Arizona Center for Fertility Studies] were returned in a timely manner (not the case at my first clinic). All my options were discussed beforehand. When I had concerns (which I think happen most of the time doing ART), I knew I could get thru to Dr. Jay and have satisfaction of an answer. Strengths: they fought with me to get me pregnant and most of the staff I came in contact with seemed to care about my getting a successful outcome. Weakness: sometimes a bit of a support staff turnover. Not really anything that effected my getting pregnant, tho.

He was compassionate and friendly...missed a large cyst during an ultrasound resulting in the okay moving forward with a cycle of injections. This created the cyst to grow substantially larger requiring us to cancel the cycle...His treatments however were to the book. He did what worked for most people...explained everything to us. He was very late to most appointments but made up for it as he would spend as much time with you during the appointment as needed. Never felt rushed with him.

The clinic is very outdated and they use paper and pen for documentation which sometimes caused a delay in certain stuff...HMG and HCG for timed intercourse, heparin 2x a day for MTHFR, prednisone (steroid), and intralipid infusion...The nurses and office staff were the best. The only weakness [at Arizona Center for Fertility Studies] I can think of if I have to pick one is maybe oldish machines but they do the job so no complaints really. A clinic does not have to have top notch brand new equipment to do a good job. Great clinic, great staff, GREAT Dr! :)...They [nurses] are awesome, funny, and that you like a friend. They answer all questions either via phone or email by the end of each day if not sooner.

He also helped with my anxiety and kept me from blanket-restricting all actions during the process (exercise, hot tubs, etc). He was confident but not arrogant...Be direct with what you want to know. Dr Nemiro uses very high dosing of stimulation drugs, but is also accompanied by drugs that will prevent OHSS and you are closely monitored. The overall goal is to get all the eggs you have and keeping you safe. Dr Nemiro uses PGS/PGD testing and performs frozen 5 day transfers as they have proven to be more successful, even though it is a longer process than the 3 day fresh transfer...always very personable and kind, remembering aspects about not only our medical history, but as human being as well.

We were grateful for Dr. Nemiro's 'get down to business' and aggressive treatment plan. We knew that this was a huge financial investment with no 100% guarantee and were thankful for Dr. Nemiro leaving no stone unturned, while also being very conservative. Throughout our research, we have found that not every provider practices the same way. He was always very open, honest and compassionate

We teamferred 2 embryos with the hope of both 'sticking.' Dr Nemiro was very honest with us that this was a 20% probability. One embryo 'stuck' and we were not disappointed nor surprised. We were very happy Dr Nemiro encouraged a 2 embryo transfer and we were paying completely out of pocket and wanted to avoid multiple transfers
