He suspected that I had a ismothecele from my previous c-section, and he was correct.

He performed a saline ultrasound to diagnosis an isthmocele. He then referred me to a surgeon to determine if an isthmocele repair was necessary or not. He performed an ERA at his office to assess the best day for an embryo transfer. For my FET and ERA, I was put on a protocol of estrogen pills 3x a day and a daily progesterone shot. After it was discovered that there was fluid in my uterus from the estrogen, he changed the protocol so that one of the estrogen pills was taken internally.

The nursing staff at FCI is compassionate, organized, and communicative. My nurse went out of town for a week which led to some difficultly with getting blood test results for a day, but other than that everything went very smoothly.

The clinic is accessible and comfortable. Appointments are easy to schedule through the patient portal. Occasionally you'll be put on hold on the phone to schedule an appointment. We had to transfer a frozen embryo here from another clinic. The embryologist was very cooperative and communicated well with us. One thing which we did not anticipate was seeing other doctors at the clinic for one of the ERA's and for our FET. I was very nervous about this but I ended up really liking both the doctors we saw, Dr. Marut and Dr. Sipe. I was impressed about how much Dr. Marut knew about my case. Dr. Sipe had a great bedside manner and performed the FET effortlessly. The one criticism I have for the clinic was that there was some miscommunication about the grade of my embryo, which had been fertilized at my previous clinic.

There was some miscommunication about the grade of my embryo, which was fertilized and frozen at another clinic. My previous clinic used a previous grading system. This miscommunication caused a lot of anxiety since they gave me the impression that my embryo was poor quality. After speaking to my nurse, this miscommunication was cleared up.
Another miscommunication happened when my nurse was out of town. I had blood drawn several times at a hospital outside of Chicago. The hospital faxed the results but the results were lost twice. I also did not receive my results or a phone call until the next day.
I enjoyed meeting with Dr. Rapisarda. He was thorough and answered all of my questions. I don't know if this is typical, but unless you have a consult, it's as if you have no access to the Doctor. That I do not like. Unlike other healthcare providers, there's no option to message the Doctor via the portal. I don't like that I have to schedule a separate consultation if I have a question for the doctor...after my second consult I never spoke to Dr. Rapisarda, everything was through the nurses and clinic staff.
Working with FCI has only elevated my stress levels. It is a very poorly run clinic when it comes to customer service. The patient portal is a nightmare to use. My frustration with Fertility Centers of Illinois is not specifically with Dr. Rapisarda. I've met him on a couple of occasions and enjoyed working with him. My frustrations about with how the practice is managed (or not). I am not sure if Dr. Rapisarda has oversight or knowledge into how the practice is run, so I don't want to place blame on him. That said, it very much operates like a mill. Grab a number and stand in line.

My first round of egg freezing didn't produce a lot of eggs. They wanted to do a more aggressive approach for round two, but after my second consult I never spoke to Dr. Rapisarda, everything was through the nurses and clinic staff.

From my experience, a nurse is assigned to work with one doctor. This means that you have one nurse that's assigned to you. I did not like working with the nurse I was assigned during my first round. I thought she was rude and lacked compassion. I understand she does this daily, but I had never gone through it and found it overwhelming and challenging. She expressed no sympathy. It was so frustrating that I asked to speak with someone to be assigned a different nurse for my second round. I should have asked to speak with them another time, but they asked that I speak with the office manager the day of my retrieval. So, immediately after my procedure, I met with the office manager to share my concerns. It felt like she didn't care. For my second round I was assigned another nurse, who I did enjoy working with. She was kind, listened to and answered my questions and was relatively accessible. A few days prior to my retrieval she called me to tell me it was her last day. So then I was back with the nurse I didn't particularly like. For round three, I figured I knew what to expect more or less, so it wasn't worth asking for a different nurse again. I'm sure the clinic is extra busy this time of year since patients are trying to get in before the end of the year, but that has made scheduling a disaster. In my previous two rounds, I've gotten an email with a notification on med update as well as a phone call in case I have questions. This time around I've only received an email, so it's virtually impossible to ask questions.

I went with FCI and Dr. Rapisarda because they have good results. That said, I'd urge a new patient to explore other options. This is a stressful time period and it's important for your success to try to minimize any stressors. Working with FCI has only elevated my stress levels. It is a very poorly run clinic when it comes to customer service. The patient portal is a nightmare to use. I thought having multiple locations would be helpful, but it has not helped me at all. It's very disjointed, so even if it all functions under one umbrella organization, you have to deal with each location separately. The Glenview location does not pick up their phone. Of my last 6-8 attempts, they've picked up once. The River North location pretty much always picks up, but they say they don't normally schedule for other locations. If that's the case, I'm not sure how you're supposed to schedule anything at Glenview. You now have the ability to reach out via the portal for scheduling (I think this is new, but it's not a useful function. You can't actually pick a time, you just message someone and wait for a response.) It does not seem like they have the ability to (or perhaps just don't know how) to re-direct a message. For instance, I messaged "scheduling" about trying to set up an appointment in Glenview. They said they didn't have availability and to try River North. So I asked what time River North was available. The response was that I was messaging the Glenview office, so they didn't know and I needed to call River North. So I did. They told me they didn't have any appointments before 10:30am. Unfortunately that didn't work for me. I asked what they recommended doing and they said I needed to speak with my nurse. They transferred me to the nurse, who has today off and never works the day I called. Her voicemail explicitly says no one else can check her VM, so to send a message on the portal. I called back again and asked to speak with a nurse who's covering for her. They transferred me to someone who works in finance. I called back a third time and was transferred to another nurse's voicemail. I left a message and then had to send a message via the portal. I still haven't received a helpful response.
Every interaction (this is not an exaggeration) is similar to this. I went in for a monitoring appointment on a Saturday. I got a call (a call for once!) from my nurse with next steps. She told me the office was closed, but that she would tell the front desk to call me to set up an appointment for Monday. They called me Sunday morning at 5:49am. This wasn't an automated call. A real person thought that 5:49am on a Sunday was a good time to call a patient. Fortunately, I slept through the call. Unfortunately, they don't answer their phones on Sunday so I was unable to schedule something. I went back to the portal. I requested a time and location. They replied asking if 15 minutes later was ok. I confirmed. No response. So when I woke up on Monday I had to keep calling until someone picked up. My appointment had never been booked. Fortunately, the slot was still open.
Now let's talk about financials. This is not unique to FCI and it's common in healthcare in the US, but I walked in thinking I'd be able to get some clarification on how much this would cost me. Nope. I ended up signing the paper work each time, hoping for the best. Each time they have been unable to tell me how much the procedure and medicine will cost. They give huge ranges, so it's been a gamble each time. You're not able to start a cycle until you have sign off from the financial department. Good luck contacting them! I tried email, phone and portal. I never got a response. My nurse then asked me if I had heard from my finance officer since I didn't have financial approval yet. (It seems a bit odd that this isn't something she could have handled on the backend.) And to answer her question, no, I had not heard from them. I'd filled out the required paperwork as soon as it was available and it appears that the finance office did not process it. Why the nurse was asking me about it is beyond me.
Please note that the clinic is closed for the Christmas holidays and closes most of their offices on other holidays, with only River North open (same for weekends.) So, if you live in a suburban location and go to an office there, be prepared to go downtown on weekends and holidays. Additionally, since they are closed for the Christmas holiday, it means they will not start a new cycle after roughly December 10. As a patient that means you could miss out on a cycle in the same calendar year because they are closed. Considering you have no control over when your body is ready for this, I would investigate if other clinics close for the holidays if you ever plan on doing more than one round or if you have to make sure your cycle occurs in a certain calendar year.
For the positives, the vast majority of ultrasound techs and phlebotomists are absolutely LOVELY! They are kind, helpful individuals that I genuinely enjoyed seeing (and you get to see them often!)

Trying to get my medication this time around was a disaster. I was ready to start and contacted the nurse on Monday. She did not send in the medication orders until Friday and to the wrong pharmacy. Thank goodness the right pharmacy was able to turn it around quickly. It is confusing and frustrating that they tell you you need the medication to start, but then they don't order it, effectively prohibiting you from beginning.
When I did speak with Dr. Rapisarda he was very kind, but could never answer questions or give me the guidance I was looking for...We tried ~5 separate times for an FET and the doctor barely made any changes to my protocal that clearly was not working...was very kind and compassionate. We had to do 3 IUIs with chlomid before moving to ivf. For our first round of IVF we did an antagonist protocol and retrieved 16 eggs. Dr. Rapisarda discovered my blocked tube during this cycle, which my old OB missed, and he helped me get it taken care of through surgery right away so we could get back on track. I had my left tube removed and they cleaned up my low grade endometriosis, as well. We ended up with 3 day 5 embryos-no PGS, all grades A/B. We transferred one embryo that was not successful and decided to do another egg retrieval before moving on. We did a similar protocol this time with higher doses to try to get better eggs. We retrieved 14 eggs with better development, but we still ended up with 3 day 5 embryos. We decided to do PGS with these and discovered only one normal embryo was created. I was diagnosed with poor egg quality and we are preparing for our next transfer after doing an ERA.

The larger clinic in River North in Chicago is not always as organized as it should be. A nurse, not my assigned nurse, told me I was at risk for a cancelled cycle when I was not. This caused great distress in the middle of an IVF cycle, but my primary nurse cleared it up right away and apologized for the mistake.
Dr. Rapisarda seems very knowledgeable in the field. From day 1 he was very open to answering all of my questions and providing information. I never felt like I was being pushed into any specific treatments or tests. He maintains a professional manner without being too personal (which I personally don't like in doctors) and without also seeming cold.
I like that they [FCI-Glenview] open very early in the morning. I'm able to schedule majority of my ultrasound appointments and blood collection before I go to work. It works great with my work schedule. Only the doctor consultation and IUI are done during working hours. All of the staff are efficient and I'm able to get in and out of the office in great time... There are different nurses. The ultrasound techs only perform the ultrasounds for monitoring. They've been very helpful, pointing out anything that was relevant. The nurses that take blood are always caring. I have bad veins and have had issues being poked, but they've done a much better job than other places I've been to.

Describe the protocols John Rapisarda used in your cycles at Fertility Centers of Illinois (US Fertility Network clinic) and their degree of success.
We started the process with testing my husband's semen for analysis and getting bloodwork done on both of us. Since we had been trying to get pregnant for a couple years of trying, and with normal cycles and ovulation, the first couple months we monitored my cycles and bloodwork and ovulation. I also had ultrasounds done to look at any physical issues and saline ultrasound to verify that my tubes were clear. Genetic testing was also done on me to see if I was a carrier of any of the most common bad genes. Everything came back good.
Once everything came back clear, the doctor recommended starting with IUI and Clomid. The cycle used OTC ovulation kit to predict ovulation, a trigger shot and a scheduled IUI. Monitoring using bloodwork was done to rest estrogen levels and progesterone levels. I was given Prometrium due to low progesterone levels post IUI. Two weeks later I had the pregnancy test bloodwork done and it came back positive but with low HCG levels. Follow up tests over the course of a couple weeks weren't looking good. It turned out to be an ectopic pregnancy. Since I was being monitored so closely, we caught it early. I was prescribed Methatrexate to end the pregnancy and was told that I wouldn't be able to try to get pregnant for 3 months due to possible birth defects from the medication. I was given that information in advance of taking the medication. My options were to take Methatrexate or remove my fallopian tube where the pregnancy was.
We ended up waiting about 5 months before the next IUI. In the meantime I had bloodwork monitoring until the HCG levels came back down to 0 and another saline ultrasound to make sure my tubes were cleared following the ectopic pregnancy.
Round #2 of IUI, the doctor switched it up to try Femara. It works similar to Clomid but in a different way. Starting this cycle, I was monitored more closely with more ultrasounds to track the size of the follicles. Also did a triggers shot with Ovidrel, IUI and Prometrium on the latter part of the cycle. Pregnancy test came back negative.
Round #3 of IUI, we went with Letrozole. Ultrasound monitoring of follicle sizes, Ovidrel triggershot, IUI and progesterone again on the latter half. Once again, negative pregnancy test.
After two rounds of no results with IUI, the doctor recommended going with FSH injectibles. I was injecting 75IU of Follistim daily for 3 days, had a checkup and was told to go 2 more days at 75IU. After the next appointment, I was instructed on 1 day of 75IU and 1 day 66IU. Following those days, I was told to trigger shot with Ovidrel that night and IUI the following 1.5 day. A few days following the IUI, I was put on Progesterone in Oil shots daily. Pregnancy test came back positive. The first appointment to measure heartbeat (6wk), I was told the heartbeat wasn't as strong as they wanted to to continue monitoring. At the 7wk appointment, there wasn't too much growth from 6wks and the heartbeat was still lower than they wanted. They also said the gestational sac was irregular in shape. A few days later I had some light spotting. I was able to get in the next day and still there was low heartbeat and not any measured growth. The doctor indicated that it didn't look like a healthy fetus. I as told I could continue progesterone shots and see what happens. I was still spotting this whole time. A few days later I decided to stop the progesterone shots. When I went in for an ultrasound a few days after that, there was no more heartbeat. I was given several options following this. 1) I could wait and let my body miscarry the fetus on it's own which could take several weeks. 2) I could have a D&C to remove the tissue but there would be risk of scarring that could cause problems getting pregnant down the road. Or 3) Take Misoprostal to induce 'labor' essentially. It causes the uterus to contract and release the fetal tissues. The doctor explained the side effects of the medication and what the process would be like. He also explained that there is a chance that my body wouldn't expel all of the tissue and I might still require a D&C. I decided to take the Misoprostal that following weekend. It took about 5-10 hours for major cramping to kick in. About 12 hours later I had bad cramping and felt a rush of liquid and tissue pass. I collected the tissue in a specimen cup since we were going to send it off for genetic testing. Cramping continued for another day or so but got a lot lighter. 1.5 days later, I passed some more tissue, saved it and dropped it off with the other sample at the doctor's office. Bleeding continued, similar to period bleeding, for another couple weeks. It was very light. I went in weekly for HCG level monitoring until it dropped to zero. The tissue specimens were sent for genetic testing. All of the tests they ran came back normal. Since miscarriages happen typically due to a genetic abnormality, the doctor wanted to run a few tests to check for possible other issues. I had bloodwork done for immune system sensitivity that could have caused the body to reject the fetus. I also had a uterine biopsy done to check for bacterial issues that could have caused it. And lastly a bacterial swab of the vaginal cavity to check for bacterial issues. All tests came back normal. Essentially the miscarriage is unexplainable. The doctor said it still could have been genetic but something rare that isn't tested for typically. They only test the most common issues.
After my cycle came back, we waited 2 months of normal cycles before trying two more rounds of IUI with Follistim injections and Progesterone in Oil. Both resulted in no pregnancy. We are currently in a self-imposed wait before jumping into IVF. This was a decision we have made as a couple. The doctor didn't even mention anything to us about switching to IVF. We just feel it's time to move on from IUI. In the next month or so we'll go in for our IVF consultation and see where that all goes.

There are different nurses. The ultrasound techs only perform the ultrasounds for monitoring. They've been very helpful, pointing out anything that was relevant. The nurses that take blood are always caring. I have bad veins and have had issues being poked, but they've done a much better job than other places I've been to. The doctor's nurse is the one who calls to follow up on blood results and ultrasounds. She was also the one to perform the IUI and met with me to go over how to administer my shots at the beginning of this all.

I like that they open very early in the morning. I'm able to schedule majority of my ultrasound appointments and blood collection before I go to work. It works great with my work schedule. Only the doctor consultation and IUI are done during working hours. All of the staff are efficient and I'm able to get in and out of the office in great time.

An office visit consultation with the doctor was about $300.

When going to see Dr. R [John Rapisarda] we were nervous, anxious, and hoping for the best. He exceeded my expectations. He spent as much time with us as we needed him to, in our first initial consultation appointment. He went over everything about my condition with me, he let me know how each procedure works, and truly made us feel he was willing to do what it takes to get us our baby.

They are your biggest supporters!

Dr. Rapisarda never made me feel rushed. He explained my diagnosis and treatment options as many times as I needed to fully understand. During my first visits, I was an emotional wreck. It was extremely difficult to grasp I could not get pregnant on my own (or that my chances were very low). I came in with a very negative mindset and slowly I began to trust Dr. Rapisarda and the process. I began to understand the science behind the treatment. He was very neutral, never giving me false hope and never telling me it was going to happen. I really appreciated the level of hope he was giving me because it felt realistic.

I had a day 3 transfer. It was advised by a different doctor who was on call that we transfer 2 embryos. Given that is was a day 3 transfer and the embryos were less developed. It was a difficult decision to make on the on the spot. I wish I had done a little more research and had information about this possible decision beforehand.
We did 6 rounds if IUI [with Dr. John Rapisarda]. First two with injectables (gonal F). The first IUI resulted in a pregnancy. I misscarried at 7 weeks. IUI cycles 4 through 6 were just using chlomid. We were trying to get through 6 rounds of IUI for insurance purposes so we could move on to IVF/FET. Those attempts were unsuccessful. We proceeded with an egg retrieval and did a FET and not IVF so that we could do PGS. First FET attempt resulted in pregnancy but again I miscarried at 7-8 weeks.

It was challenging to get in touch with him and he never actually spoke to me during my cycle. I know the process is unpredictable and there's no way you can get the same doctor every time, but I truly NEVER spoke with him besides at the beginning and at the very end, once we had a successful pregnancy. When we met for our ending session, my wife and I both felt like he had no idea who we were. Even though we achieved a pregnancy, it was a very stressful process and I was looking for more care and interaction from my provider.

Downsides of this clinic are that it is so huge, you are frequently being shuffled around with hoards of other people. It's hard not to feel absolutely like a number or cattle at times, which is so challenging when you're pumped up on hormones and dealing with infertility.

I had one day during my stim cycle where the nurses did not call me to inform me of my results and tell me what the plan was for the day's medication dosage. I had to call a number of times to get the information and finally received a call back (and we were going to do the retrieval once I received the call). I also had them lose all the paperwork that we filled out on freezing embryos and we had to resign everything on the day of our transfer. They misinformed me about the start of my cycle and I had to rush to get medications last minute from a local pharmacy because we started earlier than they originally told me. I also had an instance where they informed me to take a medication on the day of the transfer, but then when I arrived told me I had to wait to take it until I signed the consent forms.
He [Dr. Rapisarda] was open and honest with us about our situation. He spoke bluntly while remaining positive about his ability to help us with our specific situation. The game plan was very clear from the beginning. He made sure we were comfortable with the procedures we were going to undertake and did not push us to do anything we were not comfortable with. I also needed to be prepared with my records and know which tests I had already completed with my obgyn, etc. as the nurses weren't the greatest about checking my records before telling me i needed to get specific tests/work-ups done.
Dr. Rapisarda spoke honestly and completely. He explained everything slowly and repeated when necessary -as I was taking notes. I felt like he knew was knowledgable and that I was in good hands. After my first negative pregnancy he explained our next course of action.... One day I went to the clinic for the mandatory psych eval and saw Dr. Rapisarda and he spoke to me by name. I'm not a very vocal, questioning patient so that meant a lot.
The first cycle I worked with Nikki [a nurse at FCI]. She was very compassionate and available. She answered all of my questions, looked me in the eye and kept great track of my records. During this second FET, I'm working with Ali and she has been just as compassionate and responsive. I want to buy them gifts!... 'm amazed at how well FCI runs. If I ever needed an office manager, I would try to poach theirs.

(13 retrieved, 8 ICSI'd, 5 fertilized, 2 embryos on day 5, transferred one (miscarriage at 9wks), froze one).

Describe John Rapisarda's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Fertility Centers of Illinois (US Fertility Network clinic).
Our doctor [Dr. Rapisarda] recommended transferring one due to age and no known fertility issues...The time we were with Dr Rapisarda it was very personable and treated as if we were the most important patient. He was thorough and answered all my wife’s questions...He was kind, caring, and very thorough. He had no insight into why our iuis were not working other then we were using frozen sperm. When we hit the number of iuis needed for insurance to pay for ivf he fully supported us on that journey.
Strengths are that they [Fertility Centers of Illinois] are efficient. Weakness is mornings are a mass assembly line of a ton of women going through bloodwork and ultrasounds. Not most personable at times especially during busy mornings. The billing office was super helpful. They worked with us when our authorization for treatment was randomly lost. They helped us figure it out with insurance. They gave a 20% discount if we paid our balance in full. Another downside was not knowing the anesthesiologist was out of network so it was not covered at all. Never thought to ask as it was an in network provider and clinic.

The clinic itself is an assembly line of bloodwork and ultrasounds. The time we were with Dr Rapisarda it was very personable and treated as if we were the most important patient. He was thorough and answered all my wife’s questions.
Describe the protocols John Rapisarda used in your cycles at Fertility Centers of Illinois (US Fertility Network clinic) and their degree of success.
We started with 3 unmedicated iuis and ovidrel trigger. Two of those we did back to back inseminations. All negatives so we added clomid and ovidrel trigger for two cycles. Also switched sperm donor. Still negative. Iui six was done with gonal f and ovidrel. Again negative. Moved to ivf. Pretty typical protocol of gonal f, menopur, and centricide. Retrieved 17 eggs. 10 mature and fertilized. Transferred one AA embryo on Day 5. Froze two on day 5. Froze anonyher on Day 6. Pregnant with a singleton. Gave birth 9 months later. Now did our consult for a FET for baby 2. Started birth control today. Will use lupron, estradiol, and PIO.

All the nurses were kind. They delivered the really crappy news of not being pregnant very kindly. They managed my wife’s anxiety about things very patiently.
