Words cannot fully express my gratitude for Dr. Schnorr and the staff at Coastal Fertility Specialists in Charleston, SC. In 2017, after a year of trying to conceive and an early miscarriage, my husband and I decided to consult with an RE. Initial testing revealed our problem was twofold: a diminished ovarian reserve for me and a decreased sperm count for my husband. We were devastated to learn our chance of natural conception was approximately 5%. Dr. Schnorr spent a great deal of time explaining the fundamentals of fertility and the IVF process as well as answering our many questions. As anyone who has been through fertility treatment knows, the process is an emotional roller coaster. But Dr. Schnorr and our nurse Andrea gave us every assurance that with persistence we could have the family we’ve always dreamed of. One cycle of IVF later, I became pregnant with our son William! We felt incredibly fortunate and grateful that our first cycle was a success.
Fast forward nearly two years. We now find ourselves trying to conceive our second child but, sadly, the three remaining embryos from our first cycle did not take. Dr. Schnorr and my nurse (now Haley) called me after each transfer to deliver the disappointing news and discuss the next steps. One thing that truly stands out about Dr. Schnorr is his dedication to his patients — he always takes time to personally call with treatment updates, and his consult visits are never rushed. That we are back at square one initially seemed daunting, but I know we are in the most capable hands. We have since moved out of state, but I cannot imagine working with another clinic. The staff is incredibly warm, compassionate, and supportive; each visit feels more like a reunion with friends and family than a medical appointment! Unless it is medically indicated, the physicians at Coastal will not transfer multiple embryos due to risk of maternal/fetal complications. Many clinics will transfer multiple embryos to improve their success rate or simply to please the patients, but I think this is unethical. I respect and support Coastal's commitment to providing the most safe and ethical fertility practices. I would advise prospective patients who wanted twins/multiples to keep and open mind and research the risks involved.

I do not remember many specifics, as my cycle took place in 2017. It began with birth control, and I remember using Gonal-F as well as a "trigger shot" as we neared the retrieval. Before my transfers, I used birth control pills followed by estrace and progesterone (injections, suppositories).

Describe your experience with Coastal Fertility Specialists.
CFS is a well oiled machine. I have experienced short to non-existent wait times, clean facilities, friendly staff, and cutting edge treatment strategies. Coastal has also been very reassuring and accommodating with regard to COVID-19.
If I was forced to list one con, it is that my assigned billing coordinator is not always quick to respond to my calls. However, this is very minor and has not detracted from my overall experience.
Describe your experience with your monitoring appointments at Coastal Fertility Specialists.
If I recall it was preferred that I come in the morning, as the physicians and nurses meet everyday during lunch to discuss treatment plans, but I know they would have been flexible if my schedule didn't allow. Coastal opens at 7 am, so I was able to complete my blood work and ultrasound before going to work. There was little to no wait.
Describe the costs associated with your care under John Schnorr at Coastal Fertility Specialists.
An IVF cycle is approximately $14,000 including monitoring and a transfer. Anesthesia for the retrieval was about $500. Embryo preservation is $400 every 6 months. Medication through Freedom Fertility Pharmacy was between $3000-4000. Coastal works with ATTAIN, a company that provides insurance plans for 2-3 cycles of IVF with money back guarantees.

As previously stated, Coastal physicians will not transfer multiple embryos unless it is medically indicated. They base this decision on ample research detailing potential risks of maternal/fetal complications in pregnancies with multiples.
It was suggested to transfer a single embyro and we did not discuss in any depth a multiple embryo transfer option. Early on Dr. Schnorr provided education on the risks of a multiple pregnancy...At times I felt like his approaches were aggressive and looking back I understand his approach but I lacked the education on infertility, treatment protocols and endometriosis to understand all of his decision making...I was extremely emotional during the consultation to begin treatment and he suggested I see a therapist implying that I was overly emotional.

Dr. Schnorr explained things thoroughly in terms of my diagnosis and treatment. I was extremely emotional during the consultation to begin treatment and he suggested I see a therapist implying that I was overly emotional. Although I value therapy, I did not want to pursue it and it made me feel as if it was not OK for me to share the emotional impact infertility has had on me with him. At times I felt he became defensive with me when I questioned why I was doing ultrasounds and definitely when we went against his recommendation for our IVF protocol. At times my experiences with him were positive but I preferred interacting with the nursing staff.
What's one piece of advice would you give a prospective patient of John Schnorr at Coastal Fertility Specialists?
Do your own research so that you can be informed and clarify your own expectations. Fertility IQ has been a great resource to fill in the blanks for education the clinic leaves out or to make our own decisions on treatment protocols.
During treatment, were you treated like a number or a human with John Schnorr at Coastal Fertility Specialists?
I have never really felt like my personality meshed well with Dr. Schnorr. It could be in part because I have some discomfort working with a male physician. I think that the rest of the staff at Coastal is excellent. At times I felt like his approaches were aggressive and looking back I understand his approach but I lacked the education on infertility, treatment protocols and endometriosis to understand all of his decision making. I often had my guard up due to perceptions that fertility clinics operate as a business and may make recommendations for patients based on revenue. On a few occasions I was called out of the waiting room due to other patients being present with the same first name and being mixed up. At one point a staff member (who I had seen 3 days prior) tried do an intake on me after I had been a patient for about a year.
Describe the protocols John Schnorr used in your cycles at Coastal Fertility Specialists and their degree of success.
We did three IUI cycles using letrazole, gonal F injections, ovidrel and prometrium. I was happy with the IUI protocols because adjustments were made each protocol that felt individualized, and we were more aggressive with gonal F each round to increase my chance of success. Although our IUI's were not successful it helped me become more comfortable with the clinic and build trust as I got to know staff and they tailored my protocol. I had friends seeing other doctor's tell me that they were disappointed their doctor had not been open to using a similar medication protocol or that they were using clomid with significantly worse side effects which gave me confidence in my treatment.
We started IVF with a sonohysterogram and Lupron in the luteal phase.

I have had positive experiences with all nursing staff and especially Andrea. She has been warm, compassionate and prompt in answering questions and returning phone calls. She shared that she was a fertility patient in the past which is comforting to know. I enjoyed having Sharon as an ultrasound tech because she was gentle and thorough and would explain in detail what the ultrasound readings were.
Describe your experience with Coastal Fertility Specialists.
Coastal has better success rates than other clinics/RE's in the Charleston area. I would definitely recommend fertility treatment there vs. with another doctor or clinic. At times they weren't great at communicating next steps, the reason for procedures or providing thorough patient education.

I had some insurance benefits. We paid about $2200 out of pocket for 3 IUI's which was comparable to what we were quoted. We paid a $6200 cycle fee for IVF and chose not to use insurance benefits for medication and were gives a quote of $3900 out of pocket for medication. My husband and I had high hopes when we began seeing Dr Schnorr. Our first retrieval revealed great numbers, which we never achieved at our previous clinic. Slowly our hope dwindled after each unsuccessful transfer. Failure after failure and test after test revealed no explanation to our fertility struggles. Dr Schnorr was compassionate and caring but we did get a little frustrated with his techniques. He was very apprehensive about making changes in protocols even though ours continued to fail. This is very similar to our previous physician as well, which is one of the main reasons we left that clinic. When we presented questions to Dr Schnorr about making changes he became stern and resistant to the point of suggesting we change physicians. This whole process is very frustrating and to have a physician seemly uninterested in a patients concerns and disregard for advocating for oneself has been truly devastating. We have been through so much and still have no children of our own, yet at times feel like Dr Schnorr does not care about us as people anymore. Dr Schnorr has agreed to do one last transfer with us, but other obstacles within their clinic continue to prevent this. We feel quite lost and unheard.

Always advocate for yourself regardless of his response. You know your body better than anyone so be prepared to fight for what is right for you!

We did an antagonist protocol both times. Our protocol consisted of low dose hcg, gonal, cetritide, and a lupron trigger. We had great results with both retrievals resulting in a total of 9 normal embryos between the two retrievals.

Whitney was great about returning phone calls and providing feedback in a timely manner. However, Whitney was very condescending and belittled her patients with every interaction. We never felt as though she was OUR coordinator because she did not do anything to help us personally through this journey. Would simply give information the doctor provided her and nothing more.

I had minimal fertility coverage through my work. However, I still accrued an extensive bill. The financial advisor gave very little advice. Once my coverage was exhausted I had to cancel all future appointments because we could not afford to move forward. NOBODY in the office offered any advice or help. After a year of saving we were ready to move forward. However, the financial department had mistakenly allowed us to do 2 transfers that were not covered by insurance and expected us to pay for them before moving forward! After calling and leaving many messages we still have not heard back from the financial department and now have no way of moving forward. Even with insurance coverage we still ended up paying well over $10000. For the 2 transfers that were not covered they are charging us $6500. The process is very expensive, but this does not seem to concern the clinic.

He corrected himself later but didn’t apologize. He did not listen to what we wanted or and was disrespectful about our wishes regarding speaking about donor sperm or that we wanted to grow out our embryo on the 2nd round instead of transferring a 3 day that wouldn’t have made it again.
This place [Coastal Fertility Specialists] churns out patients, it’s a money making machine. The financial department is a complete mess that I had to call so many times to get things straightened out even though we paid everything up front. Not transparent pricing or add on procedures at all...The positives I can say are the technicians who took blood in the mornings and the ultrasound tech were amazing, compassionate, good people. They deserve a raise.
How was your experience with John Schnorr at Coastal Fertility Specialists?
Dr. Schnorr at the initial consult came across as optimistic about my age and overall fertility despite our major male factor concerns so I was excited at first. I wanted to use a urologist to begin with and he basically told us no and then we ended having to go that route the 2nd round and cost us a lot more money. I had more eggs the first round when he used sperm fragments and wasted all my good eggs, it might have been completely different the 2nd time around if I had that number of eggs again but I had less the 2nd time. He told me on my 32nd birthday after our first failed round of IVF that, “I would remain childless if we did not use donor sperm”. I actually have 3 step children as I informed him again and he said, “if that were enough you wouldn’t be here.”
On the our 2nd attempt through IVF, we did not want to transfer 3 day embryos like we had been encouraged too before, we wanted to see if it would grow out to a cytoblast which only one did and only on day 6 which they graded a 3BB embryo. There’s no way to confirm whether it really was or not, but it didn’t look particularly good to me or my husband the day of transfer. I was disappointed and felt that transfer was encouraged again despite extremely low odds. You pay for everything at Coastal up front so it is financially beneficial for them to transfer and not have to refund any portion of your fees.
Dr. Schnorr made me cry in the parking lot of my workplace at the time when I said that I wanted to give the embryo a chance to grow out before deciding to transfer. He said that no patient had ever told him that before and that it was always better to have a shot than to have no embryo to transfer at all. I calmly explained to him that although both options were not great, the two week wait after transfer the first time and then the resulting negative was absolutely gut wrenching for me and I didn’t want to unnecessarily go through that again. He thought my opinion was completely invalid and got a little huffy with me on the phone and said he would “make note of that” in my file right away and that I could do what I want basically.
He was not supportive or nice in an extremely difficult time, both my husband and I felt like we were being called it the principle’s office like we had done something wrong every time we had to go to the clinic. I have PTSD about anything regarding Dr. Schnorr or Coastal as the financial department also messed up on both of our rounds and sent bills for services that has already been paid months and months after both of our failed transfers. Opening an envelope from them felt like being punched in the gut all over again.
What's one piece of advice would you give a prospective patient of John Schnorr at Coastal Fertility Specialists?
He really likes to cherry pick easy patients that will likely work and I think that’s why he did not like us because our situation did not end up being easy. I would tell a patient of his to ask to change to Dr. Cook instead, she was amazing and caring. Stick to your guns when you know something doesn’t quite seem right as you know your body better than anyone else.

For someone with less intensive treatment plans....well, I'm switching treatment plans and leaving this one.

Dr. Schnorr is kind of like a car salesman; he speaks quickly and doesn't spend too much time covering information that he deems irrelevant to the case he's presenting at that time. I brought up my concern about possibly having PCOS or something else which would be the cause of my irregular cycles and he essentially said nothing points to that at all and I am prime fertility health. My cycles vary from 30-50 days in length and I'm not sure I am even ovulating some cycles as the ovulation predictor kits are coming up positive multiple times a cycle or sometimes not at all. These concerns were brushed off and I was told theres no reason to discuss fertility/ovulation induction medications because there doesn't seem to be a need for them right now because my lab work is fine and my Antral Follicle Count was within normal limits.
What's one piece of advice would you give a prospective patient of John Schnorr at Coastal Fertility Specialists?
Be very direct with your concerns. I have moderate-to-severe social anxiety so when I am in new places I stumble on my words and have a hard time getting them out, because of that I don't think I was able to accurately express my intense worry about not ovulating which put a disconnect in our communication. I think Dr. Schnorr would be a great doctor for someone who has known infertility or is seeking IVF, for us being a supposed non-medicated IUI he seemed uninterested.
During treatment, were you treated like a number or a human with John Schnorr at Coastal Fertility Specialists?
I really believe that because I am young, have given birth once before, and already had everything lined up for my treatment (my wife and I already have banked donor sperm, we had already tried an at home insemination with no positive result, etc), the clinic seemed uninterested in our case. I don't need a physician coddling me but my concerns were sort of scoffed at, at least I felt like they were, and I was just told there was no reason to suggest I would have any issues getting pregnant.

Dr. Schnorr refused to discuss medications for my treatment and stated they were not warranted at this time. I would have been content with that answer if he could have offered suggestions at to why my OPKs were unreliable and why my cycles are so wildly unpredictable.
How competent was John Schnorr at LGBTQ+ care?
Yes, they've obviously worked with LGBTQ clients before and understood the process of banking sperm in addition to offering suggestions on having IUI performed at the lexington branch which would require me to ship the sperm and bring it to the office on my own or I could ship it to the Mt. Pleasant office where they are able to bank the sperm there until it's needed.
Describe your experience with your nurse at Coastal Fertility Specialists. (Assigned nurse: Cherub Hutto)
Cherub is very friendly and responds to emails quick. That's about the extent of any praise I would give her. My wife and I are a lesbian couple and while I understand a lot of people are likely going through the motions of many emotions during their visits in an infertility clinic (due to news of infertility, failed cycles, not knowing what to expect because this wasn't in the plan, etc.) that wasn't the case for us. It's never been a shock to us that getting pregnant would require the help of donor sperm and potentially a fertility doctor so it's been in the plan for us throughout our entire relationship. We have researched IUI and at home inseminations extensively so really we just needed someone to 1. do the procedure and 2. give some insight on my irregular cycles. Cherub pressed us multiple times in our first visit regarding whether or not we had questions then made way too many attempts at small talk after we both stated "no, we're good." Despite the multiple checks to see if we had any further questions, Cherub has rarely directly answered a question I've sent her by email. She is prompt to respond but incredibly short in her emails which is a total contrast to how she is in person. She never lets you know that she has received paperwork/labs that you've had faxed over from different facilities and I asked multiple times about the results of a lab that she said she would touch base with dr. schnorr about to which she never responded even to say "that result is fine"
The biggest source of my emails is the fact that OPKs are *not* reliable for us and the results are not accurate which is discouraging and scary given the fact that we have been told that's what we need to rely on for our IUI (which isn't cheap or covered by our insurance at all). Cherub responds each time to try a different brand or just change the timing of when I check even though in the beginning she told me that I need to stop testing multiple times a day and just stick to the initial time (before 10am) and keep to that. She's now recanted that and stated I need to start checking before 7 am. Pretty much useless emails and a waste of my time.

Because we are using donor sperm with my own egg, we were required to get basic prenatal labs (cycle day 3 blood work) in addition to a lot of labs to check for STDs, chicken pox immunity, hepatitis panels, etc. as well as told we would have to do an HSG prior to IUI. Because of my irregular cycles/ovulation, we were given the option to bank our sperm at the Mt. Pleasant office.

After diagnosed hydrosalpinx Dr. Schnorr suggested IVF as our best possibility for getting pregnant. We did one egg retrieval with a freeze-all. I believe this is Dr Schnorr's typical protocol in order to give your body time to recover after stinking and retrieval and get prepared for transfer. Genetic testing was not recommended for us. We transferred day 5 single embryos twice stating with the highest grades embryo (one early miscarriage, one failed). For our third transfer Dr. Schnorr recommended an endometrial scratch to help with implantation and gave us the option to transfer 2 embryos, which we did. This cycle also failed. Next, an ERA has been recommended to test receptivity and make and protocol changes as needed. This will be our next step.

Initially we tried letrozole 2.5mg x days cd 2-5, then with a trigger shot after monitoring-We did an IUi with a 17mm follicle. IUI #1 failed. Then we tried Gonal f 75 stim cd 3-14 and then with monitoring and gradual increases and my body didn’t respond at all, then we took the next month off. Then...he added Gonal f again at 125 daily 3-9 with micro hcg at 30 daily and I triggered a large follicle with 10,000 hcg and had iui that was successful!

Once these tests were complete he not only went over all of the results and what they meant, he also gave us a packet of the information for when we went home. That included what we went over, videos, and numbers and emails we could contact him and our charge nurse. He is very informative yet has a very warm bedside manner. All of the tests my husband and I underwent he made sure himself that we were comfortable and understood what the test was for. Our second opinion consultation lasted three hours. I appreciated that he went through the results and wanted to redo some. He also wanted to complete more results than our prior Dr. Once they were completed he went over our best options and consulted with us to make sure this was what we were wanting. Our previous doctor just wanted to “jump right into IVF”. I appreciate that Dr. Schnorr treats his patients as humans and cares about the outcome of their journey.

My protocol was testosterone cream for 20ish days that was applied to the inner arm once a day, 10 units of Lupron began 2 weeks before my period. Testosterone was stopped right after my period and 10 units of HCG began and Lupron was dropped to 5 units. I also began Gonal-F at 350 units and stimmed for 11 days. The last two days I was dropped down to 300 units of Gonal-F and then a HCG trigger on day 12. He used testosterone as prime to assist in producing more eggs. He expected 7-8 and I had 11 with 10 that were mature and then 5 made it to blast.

Dr. Schnorr is so kind and compassionate. I saw him for a second opinion, and my husband and I were so impressed that we started a cycle with him less than a week later. Dr. Schnorr was very compassionate and confident that he could help me. He does what is best for his patients instead of batching then like other doctors. He answered all of my questions candidly. He was going to be on vacation the day of my egg retrieval, and he called me personally a few days ahead of time to let me know that he wouldn't be there. He talked about my how my cycle was going and told me about the doctor who would be doing the retrieval. Dr. Schnorr always called me personally with my PGD results.

I had previously done a microdose lupron flare protocol with another doctor. I ended up with OHSS and all PGD abnormal embryos. Dr. Schnorr changed my protocol and had me use an LH trigger. I didn't get sick, and all of my embryos tested normal. He also liked to wait an extra day for the eggs to have more time to mature.

I did 6 unmedicated iuis to reduce risk of multiples. Then I moved to ivf. I did retrieval one year ago transfer the next year. When I was on my retrieval protocol I was started on a very low dose and had to quickly change as I was not responding. I used low dose hcg in the morning and then added gonal f a few days in in the evening and then ganirelix to prevent early ovulation. I did this for about 17 days before getting my ovidrel shot to get my eggs retrieved. Because I’m petite I think he was worried about ohhs but I needed larger doses as I was not responding.

I had my regular nurse but she was on leave when I went through the process and felt very tossed around wit the nurses. There was a ton of miscommunication that I finally had to ask to speak only to dr schnoor about my protocols. There is a nurse who would do my u/s and she was amazing. Very encouraging and thoughtful. I felt like a team with her. When my other nurse returned she was there for retrieval but she made me feel like a burden.

I was prescribed Gonal F injections, as well as an oral mediation to help promote ovulation (I can't remember the name). My right ovary needs more shots to respond than my left, so dosages are different, depending on which side I'm ovulating from. I would take a trigger shot when it was time to ovulate, and then we'd perform the IUI, with progesterone vaginal inserts to promote implantation. My first IUI resulted in my son in 2016. I naturally conceived in 2017, but it was ectopic and miscarried naturally. I went back to Dr. Schnorr, and we have started IUI treatments (the same course as before) with no success. We are about to begin our 3rd cycle.

Dr Schnor explained the whole process himself, taking time to cover our options and the chances they gave us in his opinion. He then let us choose to do an unmedicated IUI before moving on to meds because I was so nervous about fertility meds, which was in retrospect very supportive of him because I know his success rates for unmedicated IUIs can't be great. When we returned to try to conceive a second child, he was visibly thrilled to meet our son that he helped us conceive and he tailored our plan to maximize insurance coverage while also using the plan that worked the first time (going straight to injectables).

We started with unmedicated IUI because I was afraid of fertility drugs as an unexplined infertility patient. He had recommended Femara IUI because he said in his experience people our age with unexplained infertility have a 5% chance per cycle on their own, 5% more with meds, and 5% more with IUI, so he wanted to combine it all for best chance of success. After one failed IUI, we moved on to Femara + Follistim TI cycle to maximize insurance coverage (at the clinic's recommendation). Then we did a Femara + Follistim IUI, and finally we did a Follistim IUI that resulted in a healthy singleton pregnancy. When we returned to conceive again, we did one Follistim TI cycle at the doses that worked the first time, and that resulted in a healthy singleton pregnancy.

Jan was empathetic and warm, prompt to respond and helpful in sorting out numerous issues with the lab where I had blood work done. She treated me and my myriad questions with respect and intelligent consideration. I did not like one of the nurses I met with when Jan was out of the office, however, and despite asking not to be assigned to her again, I continued to see her whenever Jan was absent. I cannot remember her name, but she had a way of making my every worry three times bigger (when I mentioned I was nervous about potentially conceiving twins, she started telling me that I had a 30% chance of twins, a 5% chance of triplets and "we've only had one set of quads" - whoa wasn't even thinking about triplets and quads, thanks lady) and making me feel like a cycle had failed before we even started ("Wow, the doctor said you have a 15% chance? That's way higher than we usually see with IUIs! So after this cycle, we will move on to IVF.") She was not my favorite person.

They scheduled me very promptly for a consultation, within a month of calling. My first go round, I went to the office in Charleston, which was brutal because it was a 2 hour drive there, but the office runs like a well oiled machine and my appointments were always quick and easy. While the drive was rough, the office support system was just what I needed when I was most emotionally vulnerable. They handled my insurance with expert adeptness, minimizing our costs and even going above and beyond to help me with a billing issue that arose when I chose to do bloodwork closer to home for one of my betas. The doctors do rotate procedure days, so Dr Slowey did two of my IUIs, but they're both excellent doctors. For my second attempt to conceive, i did my monitoring at a satellite office in Bluffton, which was not nearly as smooth of an experience. I had to get my blood work done at LabCorps, which often had a 1+ hour wait and usually lost my blood work order so I had to print copies. I seem to remember IUI cycles cost us $900 - $250 for a monitored cycle administrative fee and $650 fee for the IUI (I may not have those numbers perfect, and it was 2014). They did a great job working with our insurance and finding loopholes to get medication and monitoring covered.

Dr. Schnorr spoke to me by my name and presented information in a down to Earth manner...is a very knowledgeable physician. He answers any questions you may have. Exhibits a positive attitude about every situation. He always addresses you with a warming smile. He is a trustworthy physician that I can rely on to give me the most up to date clinical information. He gives you all the information you need to know to make an informed decision.
My primary nurse Jessica [at Coastal Fertility Specialists] was amazing! She was always fast at getting back to me via email. She would sit down with me and answer any questions I had (which were many). She showed compassion and care towards her patients...$1,200 for IUI including medications. $12,000 for IVF including medications but not including PGD...Strengths were that they are very organized and prepared for any situation. For IUI I started with letrozade for 3 days then started Gonal F. I increased my dose of Gonal F every 3 days based on my labs and ultrasounds. When the follicles where the correct size a HCG trigger shot was given and the following day IUI was preformed.

The only weakness i could mention is the financial staff [at Coastal Fertility Specialists] dropped the ball on my insurance quote, however im not sure that was their fault. When we came into the office we felt like we were the only one in that office. The Doctor would call us personally to see how things were going. Every time we met with the doctor or his nurse they asked questions about our life and remembered from visit to visit. I really feel like i made a friend with this doctor.

Mo was awesome. She was funny and caring. She was very very quick to respond to emails and phone calls. She related well to me. Mo was willing to take all the time she needed with me. She sat in the room with me for the entire time i was in recovery from my retrieval. This blew me away. So awesome!!!!!

Dr. Schnorr has helped us understand why it’s difficult and yet what we can try to better our chances. We are currently trying our first IVF cycle this month. We are excited to try this and we are hopeful. We appreciate how thorough and wonderful he has been.

He has been very thorough, and our nurse has been very, very compassionate and helpful during our treatments.
