Once my husband got his male infertility factor diagnosis I was put onto birth control called Alyacen for a couple of weeks and then when directed I was told to start my injections to begin maturing my eggs. The medications used were Gonal F 150 IU which increased over the next week of injections to 250 IU and 2 vials of menopur as well as 250 MCG of centrotide. Those injections continued for 12 days and when my eggs were grown to a mature size ( measured via ultrasound and bloodwork) it was time for the trigger medications. The trigger shots were Novarel and Lupron taken at a specific time which prepared my body for the retrieval 3 days later.

I can't rave enough about how much I appreciated the support and understanding of the nursing team. I felt comfortable with them and most times looked forward to talking to them.

I felt like a human because Dr. Pena listened to both me and my partner, always allowing for questions, and giving us detailed answers. He also was honest and always polite. I even like his sense of humor. Watching how he interacted with his staff also made me see his compassionate human side.
Describe the protocols Joseph Pena used in your cycles at RMA of New York - Long Island IVF and their degree of success.
Dr. Pena's approach was first having a consultation with myself and my partner to get to know our goals and history with trying to conceive. We were trying to conceive for 6 months on our own before coming in. After review, my partner had a sperm analysis and I did FemVue and a Water Sonogram to check our fertility. From there, we moved forward with IVF egg retrieval because of my age, being 38, I had a higher success rate of IVF working over insemination, and also so I could test the embryos for PGT-a, abnormal chromosomes to try and have the healthiest child possible. For the retreival, I was put on a few different stimulation meds such as Menopur and cetrotide. We also used a trigger injection to time everything precisely for retrieval. I reacted well to the meds and came in for monitoring very frequently. After the retrieval, after receiving the results - I ended up having 35 eggs retrieved where 25 fertilized... and then 5 embryos made it to blastophast - 3 came back eupoloid. After this, we prepared for the transfer where I was put on and still currently am on Estrace and Progesterone. We transferred 1 of the eupoloids and had a positive result. I am currently 7 weeks pregnant and heard the heartbeat today. I have one more appointment next week with Dr. Pena before graduating and seeing my regular obgyn. The whole process for me has been approximately 6 months to get here to reaching my goal of potentially being a mom.

I had an accidental natural pregnancy when I took breaks between cycles and he guided me through that as well when blood work indicated it was ectopic. For the transfers - oral estrace, progesterone in oil, Endometrin suppositories. 2/3 retrievals were successful. All 3 transfers were successful, I became pregnant, but the first two ended up being chemical. I am currently 6 weeks and 3 days pregnant with my third transfer.

Considering my age at the time was less than 35, transferring a single embryo would be a better option.

My husband and I met Dr. Pena when we decided to pursue IVF after being diagnosed with a chromosome translocation. We met with him during our first consultation to discuss the options related to chromosome translocations. I had several procedures done with him including an egg retrieval, three transfers, and an ERA test. Through each procedure, Dr. Pena always explained the steps of each procedure and what I could expect. He was always honest with us in the results that we could expect. Fertility treatment and the number of medications and dosages that you need to take can be complicated. However, the nurses that I worked with, Jessica and Alexis, always took the time to explain the exact steps that I needed to take and the amount of dosage each day. This really made the process less overwhelming for me. If I had a question and called the office, I always received a call or an email back timely. After receiving bloodwork during morning monitoring hours, I always received a call with my results that afternoon on the same day.

RMA Long Island IVF has multiple offices on Long Island that you can go to. For example Dr. Pena's main office was East Patchogue. However, I often went to the West Islip or Melville offices for monitoring as they were closer to where I lived.
Morning monitoring was always quick and efficient and I never waited long to be seen.
The receptionists and admin team that I worked with were always responsive and helpful with any scheduling or billing questions that I had.
At times there were long wait times, mostly when I set up my first consultation and initial scans prior to my egg retrieval but they always went in order of when the patient arrived.
Facilities were clean and safe even during COVID 19. I never felt at risk of contracting COVID 19 while attending their facilities.

On my IUI cycles I was on letrazole and then I was given a trigger shot. On my second IUI I was slower to respond and Dr Pena increased my letrazole dose.
On my first egg retrieval cycle I was on gonal-f and one vial of menopur and as the cycle progressed my menopur was increased to two vials and my gonal-f was decreased. As the cycle progressed cetrotide was added in. Then I did trigger shots of pregnyl and Luprolide.
On my next two retrievals I started taking metformin a few weeks prior to my injections. This time I was on 3 vials of menopur and no gonal-f. Then the rest of the protocol of cetrotide and triggers occurred.
The only difference between my last 2 retrievals was that a few days into it my bloodwork showed high estrogen levels and my menopur was decreased and I started cetrotide a few days earlier.
For transfers I started by taking 2 pills orally of estrace, after a few days that increased to 1 pill taken 3x a day. Then progesterone in oil was added. After my transfers those continued and endometrin 3x a day was added.

Jessica and Alexis were so easy to talk to and always took the time to answer any questions. They were kind, compassionate, and very knowledgeable. They were always thorough with instructions as well. When my local pharmacy didn’t have my rhogam shot, they both called around to multiple pharmacies to help me get it sooner without me asking just to make sure I got it that day (even though I had 72 hours)

It was very easy to get in touch with the office and call backs always occurred the same day. Sometimes wait times were long for monitoring. I would tell a new patient that sonograms were done by sonogram techs, nurses, or a doctor so sometimes you needed to wait until your call later that day to get all your questions answered but you always heard back the same day.

Monitoring was run pretty smoothly. On most days I was out in under 45 minutes when I had bloodwork and a sonogram. On some days it was very crowded. It was organized and not overwhelming. Everyone was friendly and I never felt rushed.

We paid for PGT testing and once pregnant I had $25 copays, the rest was covered by my insurance.

Dr Pena mentioned transferring one because it is the safest and poses the least risk to myself and the baby being transferred. He did mention if there were a few failed transfers we could discuss transferring 2.
Dr. Pena is very easy to talk to and presents information very clearly...Because I have PCOS Dr. Pena was very careful to develop a protocol that would develop quality eggs while minimizing the risk of OHSS. He started me on a low dose of medications and adjusted my trigger shot (lupron instead of hcg) and timing of my retrieval in order to maximize our outcome while minimizing potential risk. He explained this entire rationale and we ended up with the results he planned for.
The entire staff [at RMA Long Island IVF] was wonderful. Jess and Alexis are both extremely knowledgable and so supportive during the difficult parts of IVF...The office manager, Peggy was always incredibly helpful and was always a friendly face and another source of support throughout this journey. I have never experienced medical care where I truly felt so strongly about how wonderful the entire staff was as I do here.

Because I have PCOS Dr. Pena was very careful to develop a protocol that would develop quality eggs while minimizing the risk of OHSS. He started me on a low dose of medications and adjusted my trigger shot (lupron instead of hcg) and timing of my retrieval in order to maximize our outcome while minimizing potential risk. He explained this entire rationale and we ended up with the results he planned for.

I always felt like I was in good hands with both nurses and I am very thankful for not only their medical care, but also for their support.

The entire clinic is a well oiled machine. The med techs are all great and become part of your journey. I need to lie down for blood work and I was never made to feel like I was inconveniencing anyone despite a busy practice. I have never experienced medical care where I truly felt so strongly about how wonderful the entire staff was as I do here.

Morning monitoring hours were perfect. Especially the team they have their morning monitoring hours. I can always run to my appointment before I have to go into work. I love that they start morning monitoring at 6:45am. It is very convenient for people that have a 9-5 jobs and do not have to show up to their jobs late. The monitoring rooms are cleaned and stacked with liners and tissues if needed.

I did one round of IUI - I don't remember the specifics of those medications. For IVF, I take estrogen every day (3 times a day) and I do a shot progesterone in oil every evening. The estrogen is to maintain the thickness in the uterine walls, and the progesterone in oil is to keep the embryo safe inside of the uterus. The progesterone in oil shots are not painful - sometimes it helps to use a heating pad after the shot to help the oil dissolve.

I do not recall the exact doses for egg retrievals. I remember that the doses were lower for my first egg retrieval and we increased the doses, added coq10 and acupuncture for my subsequent retrievals. These changes made a significant increase in the retrieval numbers.
Currently, for my FET, I have been taking 2mg estradiol 3x per day and 50mg of progesterone in oil 1x per day. My recent embryo transfer was successful and my hormone levels are progressing appropriately.

I naively initially wanted to transfer 2 embryos with my first transfer, with the idea that I ideally wanted at least 2 children and thought it would be easier to have one pregnancy. Dr. Pena spent a considerable amount of time explaining the concerns and increased risks for both pregnancy & delivery of multiples. He felt strongly that transferring one embryo was the safest approach for an healthy pregnancy and a healthy baby. Ultimately, I trusted his judgement and we transferred one embryo. I am glad that I trusted his judgement because I ended up developing preeclampsia, which could have been much worse if I had multiples. Thankfully, My son was born safe & healthy. For my recent embryo transfer, we again transferred one embryo.

I underwent 2 water sonograms. Both were done before my transfer to make sure my uterus was a good place for an embryo. I had 2 egg retrievals. I did 2 egg retrievals back to back because we wanted to get as many eggs as possible when I was the youngest. Because we had male infertility and were using donor sperm, the eggs were frozen. During those retrievals I was on different meds like birth control (to keep track of my cycle) and injections to help produce more follicles. After we got the donor sperm, the embryos were injected and we sent out the 11 embryos for genetic testing. I underwent 2 transfers at first. The first resulted in a pregnancy, but unfortunately, my hcg started to decrease and there was an early miscarriage. The second transfer resulted in my daughter that was born in June 2022. I just had a recent transfer and am currently 6 weeks pregnant. During the first pregnancy I was on estrace, progesterone in oil and endometrin vaginal inserts. This pregnancy I am on the same minus the endometrin vaginal inserts because there is a shortage. I will continue meds until I graduate from RMA and am fully under my OB care.

Dr. Pena tried many thing with me. We used estrogen patches when trying to increase my follicle count for egg retrievals. He also recommended Co-Q-20 for egg retrieval as well, He recommended acupuncture to relieve my stress. We tried baby aspirin to help with transfers. He did a lining biopsy when my transfers weren't working. Finally, he thought I might have had Endometritis which is inflammation of the uterine lining and prescribed me two heavy antibiotics (one being Cipro and the other I don't remember) before attempting my last successful embryo transfer.

This clinic is very clean and welcoming. It is organized and friendly. Sometimes there is a long wait but they open early and are very accommodating to people who work jobs with early hours. They normally start at 6:30 but are sometimes open a little earlier so I recommend getting there as early as you can. They used to have coffee and treats in the office which was a very nice touch but since Covid have discontinued this ,it would be nice if they bring this back because it was a nice comfort when being there for such a sensitive reason.

It can be a bit of weight but on busy days they usually have at least 2 sonogram technicians on.

It's hard for me to recall the specifics of treatment. First round, we tried with my own eggs. I started with birth control followed by stimulation meds and ovul suppressants. The success rate was 5 % and based on the stimulation meds, I only generated 2 eggs. Unfortunately I ovulated even on meds that were there to help prevent it. Dr Pena and I had a good conversation about donor eggs and he explained to me that I had a much higher chance of getting pregnant and having a child with donor eggs. It was a no brainer to me. I didn't want to go through heartbreak again and I wanted a real chance at becoming a mother. So second round started with birth control followed by estrace and progesterone injections. We used donor eggs and the result was three good quality embryos. I am starting to wean off my meds now and I am currently 9 wks, 6 days pregnant after a five day embryo transfer on July 26th, 2023.

Due to my diminished ovarian reserve and advanced age, I was on the higher dose of Gonal F (300 units) and Menopur 150 units (which increased to 225-300 units variably throughout the retrieval process based on my clinical presentation) for my first egg retrieval. This resulted in 16 eggs retrieved. For my second egg retrieval, I used Follistim AQ (300 units) due to diminished ovarian reserve and Menopur 150 units (increased to 225 units throughout the cycle based on my clinical presentation). This resulted in 11 eggs retrieved.

Jess was one of the nurses who monitored my bloodwork, progress during my cycle, and reported my instructions after each appointment. She was kind, compassionate, and knowledgeable. She patiently answered all of my questions, went above and beyond and offered to give me a tutorial on how to properly use my medications. She gave me comfort and humor when I needed it and I trusted that she was attentively taking care of me. Alexis was my nurse practitioner and was was amazing. My first cycle, I paid out of pocket as my insurance at the time did not cover any IVF services. I would say that my total costs including medications and monitoring costs was around $25,000. I had IVF insurance coverage during my second cycle and the cost was much lower. Since I had a deductible, my costs were probably around $6,000 in total.

In my first treatment in 2020, I had one successful IUI. The nurses were very candid that another IUI was probably not the best course of treatment for me, which saved me time in moving forward with IVF. Unfortunately the embryos from my first egg retrieval did not continue past day 5, so I did another cycle of stimulation and egg retrieval. I was able to get 3 healthy embryos, 2 of which have resulted in transfers and pregnancies.

I think the clinic is nice and clean, it is very busy but they do a good job of moving patients along and I have never had an experience where I have waiting a long time, unlike other doctor offices. Morning monitoring runs smoothly and I'm grateful that they open so early because I need to be at work before 8:00AM.

Monitoring goes smoothly, even though there are so many patients. The phlebotomy staff is great also. I like that the clinic is open at 6:30 AM and I would even show up early if they let me.

I only paid my copay and about $60 a month for medications.
Dr. Peña is very professional and attentive to details. He is very serious and blunt. While he tries to interact with patients in a friendly manner, this is not his strong suit. He is clearly more comfortable sticking with the facts and science rather than small talk...advised when he recommended one versus two embryo transfers and honored my decision...Don’t expect an overly friendly bedside manner - expect straight talk.

They are extremely disorganized in this regards and somehow always “never received” anything any of my other doctors sent over. Finally I had to demand that they deal with obtaining my records or I had records sent to me directly and I would forward them myself. Try to deal Alexis over Jess whenever possible.

Dr. Pena and his staff were very attentive. Dr. Peña does have a bit of a rough bedside manner.

I believe I went through typical treatment and protocol for both egg retrievals and transfers.

Monitoring appointments were usually very timely. If another clinic was closed and this one was covering both patients that day, it would result in lengthy delays, but this was a rare occurrence.

When I was scheduled to begin my first cycle, they failed to tell me that the donor sperm needed to be in their possession until the 11th hour. Also, after my first transfer, they failed to tell me to start a necessary medication.
He [Dr. Joseph Pena] has been honest and direct with us since our very first meeting. We are an older couple and have fewer options than younger couples might have. Dr. Pena has been very straight forward about our chances throughout this entire process. No sugar-coating, no BS. And when we’ve had questions (and there have been many) he responds immediately.

Choosing a fertility specialist is a big decision. We spent countless hours looking at reviews, gathering input from friends, etc …and are blessed that we chose Dr. Pena. Our drive to his office is extensive (there are many other fertility specialists much closer) and every single minute of every single trip has been worth it. He has been honest and direct with us since our very first meeting. We are an older couple and have fewer options than younger couples might have. Dr. Pena has been very straight forward about our chances throughout this entire process. No sugar-coating, no BS. And when we’ve had questions (and there have been many) he responds immediately. In one case, he was on vacation in Mexico and returned our e-mail question, in only several minutes, with a phone call. His attention to our concerns, and promptness with responses, has provided us with a peace of mind that no other doctor (or any kind) has ever provided. We started our fertility journey with a different provider and are so unbelievably happy that we later chose Dr. Pena.

Describe the protocols Joseph Pena used in your cycles at RMA of New York - Long Island IVF and their degree of success.
my first ER was an antagonist protocol. i was on high dosage of Gonal F and Menopur. Cetrotide was introduced a few days after followed by 2 trigger shots. They retrieved 3 eggs, 2 fetilized but did not produce any blastocyst. second ER was a Microdose Lupron Flare Protocol. i was on 5 units of Lupron shots twice a day partnered with Gonal F and Menopur, triggered with one HCG shot. They got 4 eggs, 3 fertilized and 2 blastocysts. Dr. Pena advised us to get it PGT-A tested but I was worried it won't survived and we won't have anything to transfer (i regret this decision to this day). for our 3rd ER, there was a shortage of menopur so Dr. Pena had me take Gonal F, 5 units of lupron twice a day and 4 units of novarel everyday. i triggered with novarel as well. This cycle gave us 7 eggs retrieved and 4 fertilized. 2 made it to blast and 1 euploid.

Everyone who i had in contact with was kind, compassionate and accommodating yet till very professional. No matter what time of the day you show up at the clinic, they all seem happy, ready and willing to help.

the strength would be their knowledge of fertility in all. the organization level they have is beyond any place i have ever been and I've seen a lot of doctors and clinics. they're so reliable on following up with results, next appointment or just by checking on me after every procedure.
Describe your experience with your monitoring appointments at RMA of New York - Long Island IVF.
The monitoring would be from 6.30am-7.45am, i believe. I had to drive an hour each way to get to the Patchogue clinic so it was helpful that i could just go to a closer location for just bloodwork and sonograms and Dr. Pena or Alexis/Jessica would call in the afternoon with the results. wherever location i go, the wait is very minimal.

Our insurance did not cover IVF so my husband and i paid for all the cycles, FET and PGT-A testing. The total cost of 3 ER and 1 frozen transfer was $40,310 (this include the CooperGenomics testing cost of $2700). Medications was $13, 450. third party Anesthesia was $1800. and additional $310 for 2 embryos sent for testing.

I couldn’t be happier with the treatment I received from Dr. Peña helping us conceive our two blessings.

The staff was always friendly and made you comfortable. The phlebotomist were great and never had an issue drawing my blood...[nurses] Jess and Alexis were always very friendly and took their time to explain everything and what next steps would be. When I received a positive pregnancy test, both Jess and Alexis truly showed happiness.

Be patient and trust the process. Doctor Peña is a fabulous doctor who will succeed in helping you conceive.

The nurses would call you every day after morning monitoring and would explain next steps in detail. They were always very patient and would answer any questions you may have. Everything was closely monitored by Dr. Peña and I always felt like I was in good hands

Before beginning IUI doctor Peña had my husband complete a seman analysis, I had to complete a HSG to confirm the was no blockages in my tubes. From there, we were able to proceed onto IUI. On cycle day 3 I began 50mg of Clomid and was closely monitored throughout my cycle with blood work and sonograms. Prior to ovulation I would use the trigger shot and then come in 1-2 days later for IUI.

I worked closely with Jessica and Alexis. Both Jessica and Alexis were very caring, compassionate and truly cared about you. Jess and Alexis were always very friendly and took their time to explain everything and what next steps would be. When I received a positive pregnancy test, both Jess and Alexis truly showed happiness.
