[Dr. Joseph Sanfilippo] wanted me to start weight watchers and did a full blood work up. Nothing was concluded from the blood work....He just told me to lose weight. He looked at my chart and said it could be endometriosis but he didn’t want to test for it as I have a severe reaction to some anesthesia and he did not want to risk it...He then wanted me to begin with an aggressive letrozole dosage for an iui cycle. We also did vaginal sonograms and HSG...I only saw Dr. Sanfillipo once during 3 years of treatment. Everything was automated. I felt like they were running a machine and I was just another part going through the steps...He did not give me any insight as to why I was infertile. He wanted me to try iui and see what happened. The nursing staff [at UPMC Magee] was my main point of contact. They are the best thing about the clinic but again I just felt like a number and I was being rushed through the process like everyone else...The cost was higher than their clinics. An iui cycle out of pocket was around $1800 with all medication and testing...The clinic was very easy to find. I like the use of the app to communicate quickly. The clinic was clean. The semen donation room was more like a suite and was very comfortable for my husband. I was seen quickly after signing in and never had to wait more than a few minutes in the waiting room.

Do not get your hopes up. Be prepared to feel like you are being processed. He did not give me any insight as to why I was infertile. He wanted me to try iui and see what happened.

He [Dr. Joseph Sanfilippo] has always been very thorough. He’s always been willing to listen to my needs and wants and try to make that happen to the best of his abilities. He’s always been honest and kind and patient. Even after reoccurent failure at achieving pregnancy, he has never given up on us and has been an amazing doctor overall! So much so, that I cant imagine going anywhere else for treatment.
Their [UPMC Magee] strength is that they have a wide range of options for fertility and conceiving. They prioritize women's health above all else. They are willing to work with you in all aspects. The only weaknes I would say is its sometimes hard to get ahold of nursing staff or make an appointment over the phone...[nurses are] Kind, caring, good listeners, always answered questions and made sure I was comfortable.

He is kind, caring, patient and always willing to listen to our wants and needs. If we wanted to go a specific route, he made it happen. Even after reoccurrent failures he never gave up on us and always pushed forward to give us more answers and opportunities. I honestly dont think I could find someone better to be my fertility doctor.
What's one piece of advice would you give a prospective patient of Joseph Sanfilippo at UPMC Magee?
Be open with him about what you want. He will make it happen if its possible.

Describe the protocols Joseph Sanfilippo used in your cycles at UPMC Magee and their degree of success.
We started with medicated cycles using letrozole, as clomid was used in the past and did not produce results. He instructed me to test for ovulation and have sex the day of and the days following a positive test. When we fid our IUIs with him it was medicated as well, with letrozole being taken on cycle day 3-7 I believe and I was to come in for a scan around cycle day 12 to see if my follicles were within an acceptable range. Then I was instructed to test for ovulation and call them to inform them of such. They would the. Tell me to use the trigger shot ovidrel at a certain time and would schedule me and my husband to come in within a specific time within 24-48 hours to have the IUI performed. I was then told after it was complete to use a progesterone suppository pill and test for pregnancy close to my period date.

He is lovely, he cares, but he just didn't provide the communication that I needed after a few failed tries...I've always felt like he just sort of brushes concern off without fully explaining and helping me to understand. He is the "OG" of fertility and RE here in Pittsburgh. The best of the best, for sure. He has trained everyone in the practice, but his beside manner left me wanting more. I have been with him for 7 years...
I think sometimes there is lack of communication internally [at UPMC Magee], so either the docs or the nurses aren't talking to each other enough OR they disagree. There is also a lack of communication with the patient and you have to ask for extra time and sometimes demand it to get it in a timely manner...They [nurses] get the brunt of the work, they hear me cry, yell, they are the bearers of bad news, and good. They often don't have answers but are always so kind and generous with their time. I feel awful when I am upset and they have to deal with me. I am so grateful for them...

If you are not confident enough or outspoken enough to be able to advocate for your needs or ask for what you need, Dr S is not going to work for you.
During treatment, were you treated like a number or a human with Joseph Sanfilippo at UPMC Magee?
Dr S. seems to read from a script every time he talks to me, and that is when I actually get to talk to him. You never see the docs , except at the start of a protocol cycle and sometimes for procedures. Otherwise you see the phlebotomists, ultrasound techs pretty exclusively. You do talk to the nurse team, but never see them unless you are signing consents. Its really not a personal process so I know you have to advocate for yourself, but when you do have a doc. for a tele-med apt or a call, they should take the time and answer all your questions. I don't know if it is because he has been doing this for so long that he has forgotten how emotional this process is for patients, or he is frustrated because I ask so many questions (I really want to understand everything to make sense of the process and procedures, medicines, protocol, etc.) but he never fully answers questions, I've always felt like he just sort of brushes concern off without fully explaining and helping me to understand. He is the "OG" of fertility and RE here in Pittsburgh. The best of the best, for sure. He has trained everyone in the practice, but his beside manner left me wanting more. I have been with him for 7 years (two rounds, the first was egg freezing as an insurance policy) and then went back in 2021 to actually get pregnant. I felt much more "taken care of" in 2016/2017. It could be a combination of things, it wasn't as emotional, it was pre-covid, etc. but I have decided to switch docs at the practice in hopes for better communication.
Describe the protocols Joseph Sanfilippo used in your cycles at UPMC Magee and their degree of success.
My first egg retrieval, was 6 years ago so I don't recall the full protocol, but it was pretty straight forward - PCOS so lots of follicles and produced 19 eggs were frozen. We lost a ton when we thawed them this year and only got 3 blasts. 2 came back euploid. My second stims/egg retrieval started in April with Gonal F (150) and Menopur (75) and then added in Ganurelix in the am on day 5. I went to day 12 or so before triggering with Lupron (2) and HCG (10,000). My follicles grew more slowly this protocol (I had slight OHSS my first cycle so they wanted to be careful) and we ended up with 20 eggs 16 mature which is pretty good for my age (37 at the time). 11 blasts went to PGT testing 5 were euploid. I did a day 5 fresh transfer with one of the tested embryos and it did not implant. My second FET cycle got canceled due to my lining not getting past 7.4mm. We used 4mg of estrace to start and went up to 6mg. This third FET cycle we started with 9mg of estrace and added 4mg vaginally after day 8 scan and my lining was 7.5mm. Day 13 my lining had not increased, but we decided to transfer anyway. The embryo did not implant again.

I think they are great, they may take on too many patients and not have enough staff at times. But they know what they are doing, and I trust them. You do have to advocate for what you want or feel like you need. But I think that is par for the course with this process unfortunately. It is a full time job.

I paid out of pocket for egg freezing cycle. I used insurance for all of my current IVF care. It is expensive however you pay for it.

They always do a single FET i think unless you don't do PGT testing, are older, or have multiple miscarriages. the risks associated with multiples outweighs anything else.

Dr. Sanfilippo has always been straight forward with me about everything. He is understanding and really listens to my concerns. He gives me options but also gives me direction at the same time when I don't know which way to turn...He is always willing to change protocols to increase positive outcomes and he listens to me when I want to do things a certain way. How was your experience with Joseph Sanfilippo at UPMC Magee?
I was diagnosed with Asherman's Syndrome a few years ago and my uterus was about 80% scarred. Dr. Sanfilippo was able to clear my uterus of all scarring and does frequent checks to make sure it hasn't come back. When all of my gynecologists didn't listen to me when I said I had pain, Dr. Sanfilippo did and found that I had stage 3 endometrosis which he removed and I had to have one of my tubes clipped. I have been through 2 retrievals and I am getting ready for a third but Dr. Sanfilippo is always very realistic with us regarding our odds and has looked into research studies and consults other doctors to try to assist us the best that he can. He is always willing to change protocols to increase positive outcomes and he listens to me when I want to do things a certain way. I have had multiple surgeries (getting ready to have a 10th soon) due to miscarriages, removal of scarring and removal of endometriosis and Dr. Sanfilippo has never left me in the dark about anything. He always makes sure I understand everything that is going on. He is so compassionate and empathetic and I have never had a doctor listen to me as much as he does.

Dr. Sanfilippo is extremely thorough and spends time with you to really make sure that you understand everything going because infertility can be really scary. For those with Asherman's syndrome, he has a lot of experience with it an has done wonders for me. I cannot say enough great things about him. There is a wait to get in with him though but once you have your initial visit, he is always willing to give you a call if you have questions or if you want to discuss treatment options.

He shows compassion, empathy and it is easy to see that he truly just wants to help people achieve their dreams of being parents.
Describe the protocols Joseph Sanfilippo used in your cycles at UPMC Magee and their degree of success.
Due to having Asherman's, Dr. Sanfilippo has had to do multiple surgeries on me to remove scarring. He does a hysteroscopy each time to remove the scarring and inserts a intrauterine balloon for a week to prevent the walls of the uterus from sticking and causing more scarring. He has also put me on estrogen for 28 days with progesterone for the last 10 days of that in order to start a new cycle. The estrogen helps to thicken the lining to prevent scarring as well. I get sonohysterograms every couple of months now to double check for scarring. For my endo, he did a laparoscopy and removed endo from my bowels, ovary and cavity walls. I also had to have my tube clipped due to it being blocked and fused to my ovary. It was a 3hr surgery to remove everything. I have done 2 retrievals with Dr. Sanfilippo and the second retrieval he was able to switch up meds which resulted in better response. Unfortunately we think the first transfer we had didn't take due to harmful fluid leaking from my tube into my uterus which has now been taking care of and and my second transfer was negative for unknown reasons. I have trouble getting my lining to thicken due to the Asherman's and Dr. Sanfilippo tried different protocols and was able to get my lining to 8mm for my 2nd transfer which is the thickest it has been yet. I am getting ready to do my 3rd retrieval soon.
Describe your experience with the nursing staff at UPMC Magee.
The nursing staff are very nice but at times do not communicate well regarding what is going on during treatment cycles. There have been times that I have had questions and did not hear back for a few days and have had to follow up again to get an answer. The nurses do show compassion and empathy. They are very thorough with nurse talks that explain your treatment cycles and they are good about getting you in to be seen if there is an issue after a surgery or retrieval. The IVF coordinators are wonderful. They get back to me right away when I have questions.
Describe your experience with UPMC Magee.
Strengths: knowledgeable doctors, staff show empathy, IVF coordinators are very helpful if you are looking into IVF, wide range of treatment options, very thorough breakdown of what treatment will cost, they get you in as needed and required and they have hours as early at 6:30am so that you can go before work. There is a lab in the office so that you do not have to go elsewhere to have labs done. Weaknesses: There are different doctors on retrieval days so you more than likely will not have your doctor to do your retrieval. Staff sometimes do not get back to you right away with results and your questions and but they do always get back to you. If you use MyUPMC app they do get back to you quickly though. Clinic does "batching" so they only start treatment cycles at certain times so that everyone can start treatment cycles together.

Costs vary depending on what all you are getting done. My insurance does not cover any IVF treatment so it was around $10,000-$12,000 each retrieval and that was with a little discount that I get due to being employed through the same healthcare system. My insurance covered my surgeries. They do use a private anesthesia group for retrievals that do not take insurance. All retrievals are done in the office with that group. The billing department will try to bill insurance first before billing you. You pay a lump sum up front and they take what is not covered from that and then they reimburse you for anything that was covered by insurance such as ultrasounds and what not. The billing department does check with your insurance first to see what all should be covered or not covered.

Dr. Sanfilippo prefers to do FET with genetic testing if needed but is totally willing to do fresh transfers too. Due to us having 2 failed transfers, he was willing to do a fresh transfer with 2 embryos to increase our chances but we have decided to do another freeze all cycle and do PGS testing instead.

We had consent papers that were "lost" prior to one of our transfers. When our ERA results came back, I had to call multiple times to get the results due to them forgetting to call me. When my PGS results came back, no one ever called me with the results and it wasn't until a nurse started discussing transfer with us at an appt that we found out that we had a normal embryo and no one had called us to tell us in which they apologized for. They have forgot to put orders in for labs and the lab has to get the order when I was already there ready to get my labs done.

Dr. Sanfillipo and his staff have always been wonderful. Dr.Sanfillipo is always kind and takes the time to make sure he answers any questions you have... is always very thorough in all of my appointments. He is also very candid about any health risks that I might encounter during treatments, which I appreciate. He never makes me feel like any of my questions are stupid...Healthy diet and weight is very important to him...I am currently trying to lose weight before we try the clomid again with IUI. Dr. Sanfillipo did not recommend injectables without IVF for me due to the high number of immature follicles on each of my ovaries during my 5 cycles. I found this frustrating at times but I appreciate that safety is always his number one concern.
The nursing staff [at UPMC Magee] is fabulous. I am not easy to draw blood from and they would remember me. One nurse in particular was better at getting blood from me and she always made sure to grab me. Everyone is very kind...I have not had any bad experiences with this clinic. From billing to staff, everything has been great. It can be difficult to get to with traffic. It's in the city, so you just never know how long it takes to get there. They have always been understanding when I've been stuck in traffic. They also work around my work schedule.

We used femara with an ovidrel shot for 4 cycles without ovulation. We then tried 1 clomid/ovidrel cycle during which I did have one small follicle and ovulation, but did not get pregnant. I am currently trying to lose weight before we try the clomid again with IUI. Dr. Sanfillipo did not recommend injectables without IVF for me due to the high number of immature follicles on each of my ovaries during my 5 cycles. I found this frustrating at times but I appreciate that safety is always his number one concern.

The nursing staff is fabulous. I am not easy to draw blood from and they would remember me. One nurse in particular was better at getting blood from me and she always made sure to grab me. Everyone is very kind.

I have not had any bad experiences with this clinic. From billing to staff, everything has been great. It can be difficult to get to with traffic. It's in the city, so you just never know how long it takes to get there. They have always been understanding when I've been stuck in traffic. They also work around my work schedule.

He [Dr. Sanfilippo] did not take the time to review my extensive infertility records prior to our visit. Despite my going through IVF elsewhere 6 prior times (I had moved from another state), he went the basics of IVF with me. They [UPMC Magee] lacked compassion and knowledge. They may be ok for someone trying IVF for the first time. I would never do an IVF cycle with him or his staff. I had a special request, and they were unable to make any accommodations despite the circumstances. They definitely did not show any compassion.

Listen to him. He knows what he's doing. Do all the tests, they ARE necessary and he will find a treatment plan suited to you and your needs. He has a positive pregnancy test as his main goal when it is yours.

Dr. Sanfilippo is always very attentive to us. He sees hundreds of patients and had never treated us like a number. He realizes that each case and couple are unique and different in their needs/treatment and you can see he truly believes it.
Describe the protocols Joseph Sanfilippo used in your cycles at UPMC Magee and their degree of success.
First you will have a sit down consultation with him and potentially one of his partners. He will go over your previous history and what you wish to accomplish in his care. He orders a full panel of blood work depending on your case (you'll also find out your blood type!). He then ordered a sonohysterogram (SHG) to check on uterus health. From there, he ordered a Hysterosalpingogram (HSG) to check on Fallopian tubes and more detailed uterus health. After all results, he goes over them with you during a follow up and together you make a plan of treatment. He then prescribed timed intercourse with either clomid or letrozole. We would do a few treatments of that and if nothing worked, we would do some monitored cycles with ultrasounds and continue from there.

The ladies in the office are always pleasant. They are organized and get everything situated before you're called back so that you may see your doctor as soon as they're able.

When we started this process i was diganosed [by Dr. Sanfilippo] with endometrial hyperplasia with atypia. So I had to go see the oncologist before we could move on with my fertility treatment. Dr. Sanfilippo made sure to give details about your procedure and had a good bedside manner...He took into account all of our prior history and agreed that our best course of action was IVF. The only negative I have to say about Dr. Sanfilippo is his support staff is not good or organized which made our experience at the facility negative... listens to your concerns and will put you on a path that is specific to your needs. He did listen when we told him that we had been doing fertility treatments for years and just wanted to go straight into IVF without having to do anymore IUIs or medication cycles.
There were a ton of nurses on the nursing staff [at UPMC Magee] that made it difficult to ever see the same nurse twice...very unorganized and sometimes wouldn't even look at your chart before they spoke to you... I told the nurse I hadn't had a recent ultrasound and discovered they were using an old ultrasound to make decisions during the IVF cycle...The clinic is clean and capable of providing adequate care. Our experience there is not a good one. During our IVF, we had 31 eggs retrieved, 20 were mature and only 1 fertilized. The clinic didn't perform ICSI and at the time I had no idea what that was. I didn't fee like they were very aggressive in their approach.

Dr. Sanfilippo was a great doctor who was unfortunately surrounded a poor support staff. Dr. Sanfilippo made sure to give details about your procedure and had a good bedside manner. We unfortunately had a lot of bad experiences at UPMC, but Dr. Sanfilippo was the only reason we kept going back. If Dr. Sanfilippo was at any other clinic, we wouldn't hesitate to see him again.

There were so many nurses on staff nobody had any idea who you were even though I was in the clinic all the time. If you wanted to get in contact with a nurse, you had to call the "nurses line" and leave a message and wait for a call back from whichever nurse decided to call. The nurses never knew who you were, so if you don't keep detailed notes on your cycle, medication, ultrasounds, etc., the probability of the nurses knowing where you are in your cycle is slim.

Dr. Sanfilippo suggested IVF with menopur and follistem injections. I was monitored with ultrasounds and bloodwork. The support staff (nurses) didn't perform an ultrasound and kept using an old one to determine future protocols. Once I realized what was going on, I brought it to their attention and they immediately got me in for a "fresh" ultrasound to monitor.

It was more with the nurses not being on the same page as Dr. S.. When this would happen I would get conflicting information which then made me feel like my specific case was not being taken into consideration. From this, I felt like I was receiving generic treatment.

For the first IVF cycle we went with the lowest dose of medication which resulted in only 1 embryo that made it to freeze. For the second IVF, Dr. Sanfilippo increased the medication which resulted in 6 frozen embryos that made it to freeze. Unfortunately I do not remember the amounts of each medication.

Initially, the biggest problem we ran into was that we were not receiving the protocol for what we had discussed with the doctors. After that was addressed we did not run into any problems and each cycle was run extremely well without issue.

Dr. S [Sanfilippo] clearly has a very busy schedule. My partner and I felt very rushed through our consultation and did not get thorough answers to any of our questions. I felt like rather than explaining treatment options to me and tailoring treatment to me as an individual, we were given binders and pamphlets to review on our own-with no opportunity for follow up....Dr. S does not have a very warm bedside manner and doesn't do a great job of listening or providing individualized care.
The clinic [UPMC] is located in a womans hospital, which is a welcoming atmosphere. They made a few errors with my treatment; forgetting medicines, poor or misleading instructions, allergies, etc. They also struggle with timely and clear communication. Again, they're very busy and it causes some issues.... All of the nurses are kind and compassionate. It can be very hard to get ahold of someone and even if you leave a message, they often do not get back to you for several days.
How was your experience with Joseph Sanfilippo at UPMC Magee?
Dr. S does not have a very warm bedside manner and doesn't do a great job of listening or providing individualized care. He comes across as very skilled, but as a new patient, I felt lost and like I was bothering him to ask questions or take up his time.

Have a list of questions ready and do your research PRIOR to an appointment.

Great experience with Dr Sanfillipo. He shows compassion and takes his time explaining everything. He explains things in ways for someone not in the medical field can understand, which I've found some doctors have difficulty doing. He took his time and listened to all of my questions and concerns. I never felt like I was asking a dumb question...Dr S took his time to explain every detail of our diagnosis and our best options. He was not in a rush to get out of the door. He was there to answer all questions and was very compassionate towards us

It's better to leave detailed messages so they can have answers for you when they return the call...Medications were 4K IVF was 12k...Dr S took his time to explain every detail of our diagnosis and our best options. However, the entire proves was very much impersonal.... liked that he was honest, but did not like that my multiple miscarriages could not be explained as all my tests came back normal. Eventually, only after going through IVF treatment did we discover that the issue was Luteau Phase Defect and that the solution was simply to inject myself during the first trimester of pregnancy with progesterone.

I liked that he was honest, but did not like that my multiple miscarriages could not be explained as all my tests came back normal. Eventually, only after going through IVF treatment did we discover that the issue was Luteau Phase Defect and that the solution was simply to inject myself during the first trimester of pregnancy with progesterone. It was a lot of heartache to reach this conclusion, but ultimately I am now a mother of 1 and 9 months pregnant with my second. Both naturally conceived and maintained by injecting progesterone during the first 13 weeks of pregnancy.
What's one piece of advice would you give a prospective patient of Joseph Sanfilippo at UPMC Magee?
Leave no stone unturned, the simplest of solutions could be overlooked by the providers as they tend to focus so much on the more complicated factors.

There is a large nursing staff used by a number of doctors. They tend to get to know the pacients and as they do they warm up to us, but it takes a while.

The dr [Sanfilippo] was knowledgeable and was able to walk me through the process including the next steps with us being in the current situation with covid19 and the stay home order in my state....I have a clear understanding of the upcoming steps and the nursing staff responded quickly to questions I had later.
They [UPMC] were able to adapt to tele health in light of covid19. I have not been to the facilities as of yet, but I am looking forward to it once the situation with covid19 situation clears up enough to allow me to and to move on to the next steps...
